14. Reducing Substance Use by an Emergency Department Intervention

Abstract

Introduction: Substance use and misuse is prevalent in emergency department (ED) patients. We set out to determine substance use reduction rates after a brief ED intervention for patients with tobacco, alcohol, or drug use. Methods: In this pilot prospective study, we approached a convenience sample of subjects in 2 EDs in PA during scheduled provider nonclinical times. One site was a trauma center while the other was a smaller community hospital. Subjects had to be ≥18 yo, have capacity to answer survey questions and participate in the program interventions, could not be critically ill, and had to be willing to participate. Participating subjects admitted to definitions of unhealthy use of one or more of: tobacco products, alcohol, street drugs, or addictive prescription drugs. Subjects received a structured survey and intervention tool that was previously validated (Project ASSERT), a brief intervention based on motivational interviewing, and referral to treatment, which took on average 5-10 minutes1. The intervention was carried out by a medical student, Emergency Medicine (EM) Resident, or an Addiction Recovery Specialist (a licensed social worker and certified recovery specialist with lived substance use disorder experience). These providers had training in Project ASSERT prior to the study start. Phone follow-up was used to determine current substance use by the patient. Subjects received no financial incentives.Results: One-hundred ninety-one patients were recruited (105 for tobacco usage, 54 for alcohol, and 32 for drugs). At follow-up, 16/105 tobacco users (15.0%) reported stopping smoking, 51 (48.6%) a decrease in the number of cigarettes per day, and 32 (30.5%) attempting to quit. Of 54 patients in the high-risk alcohol utilization group, 40 (74.1%) reported either a decrease in the number of days per week of drinking, or a decrease in the number of drinks per day. Of the 32 patients who used drugs, 25 (78.1%) reported a decrease in usage.Conclusion: In this pilot study involving medical students, EM residents and drug counselors at 2 EDs, we found that a brief intervention to patients with unhealthy tobacco, alcohol, and drug use resulted in overall decreased use. A more robust study, with a larger patient sample size is indicated.

27. Implementing School-Based Management in Indonesia. RTI Research Report Series. Occasional Paper

Abstract

Indonesia, the world's fourth most populous nation, has been decentralizing its education sector for the past decade. In this context, school-based management is essential for improving the quality of education. A mixed-method, multisite assessment of a project that aimed to improve the management and governance of basic education in Indonesia documented positive impact on school-based management in both public and private schools, including "madrasah" (Islamic schools). The following factors were associated with this impact: (1) the program was explicitly based on government policy; (2) technical assistance was provided rather than funding, and the program was manageable and affordable for local partners; (3) the project strengthened local systems and institutions, building commitment at both provincial and district levels; (4) the program was school based and involved members of the entire school community: principals, teachers, staff, parents, and community members; and (5) training was provided on-site in school clusters, was ongoing, and included mentoring in schools to support implementation (one-off training events rarely result in successful reform). Abstract

Almost all research work in computational neuroscience involves software. As researchers try to understand ever more complex systems, there is a continual need for software with new capabilities. Because of the wide range of questions being investigated, new software is often developed rapidly by individuals or small groups. In these cases, it can be hard to demonstrate that the software gives the right results. Software developers are often open about the code they produce and willing to share it, but there is little appreciation among potential users of the great diversity of software development practices and end results, and how this affects the suitability of software tools for use in research projects. To help clarify these issues, we have reviewed a range of software tools and asked how the culture and practice of software development affects their validity and trustworthiness. We identified four key questions that can be used to categorize software projects and correlate them with the type of product that results. The first question addresses what is being produced. The other three concern why, how, and by whom the work is done. The answers to these questions show strong correlations with the nature of the software being produced, and its suitability for particular purposes. Based on our findings, we suggest ways in which current software development practice in computational neuroscience can be improved and propose checklists to help developers, reviewers and scientists to assess the quality whether particular pieces of software are ready for use in research.

40. Reaction Rates Uncertainties and the Production of F19 in AGB Stars

Abstract

We present nucleosynthesis calculations and the resulting 19F stellar yields for a large set of models with different masses and metallicity. We find that the production of fluorine depends on the temperature of the convective pulses, the amount of primary 12C mixed into the envelope by third dredge up and the extent of the partial mixing zone. Then we perform a detailed analysis of the reaction rates involved in the production of 19F and the effects of their uncertainties. We find that the major uncertainties are associated with the 14C(alpha,gamma)18O and the 19F(alpha,p)22Ne reaction rates. For these two reactions we present new estimates of the rates and their uncertainties. The importance of the partial mixing zone is reduced when using our estimate for the 14C(alpha,gamma)18O rate. Taking into account both the uncertainties related to the partial mixing zone and those related to nuclear reactions, the highest values of 19F enhancements observed in AGB stars are not matched by the models. This is a problem that will have to be revised by providing a better understanding of the formation and nucleosynthesis in the partial mixing zone, also in relation to reducing the uncertainties of the 14C(alpha,gamma)18O reaction rate. At the same time the possible effect of Cool Bottom Processing at the base of the convective envelope should be included in the computation of AGB nucleosynthesis. This process could in principle help matching the highest 19F abundances observed by decreasing the C/O ratio at the surface of the star, while leaving the 19F abundance unchanged., Comment: 40 pages, 8 figures, accepted for publication on the Astrophysical Journal

Abstract

ENUM marks either the convergence or collision of the public telephone network with the Internet. ENUM is an innovation in the domain name system (DNS). It starts with numerical domain names that are used to query DNS name servers. The servers respond with address information found in DNS records. This can be telephone numbers, email addresses, fax numbers, SIP addresses, or other information. The concept is to use a single number in order to obtain a plethora of contact information. By convention, the Internet Engineering Task Force (IETF) ENUM Working Group determined that an ENUM number would be the same numerical string as a telephone number. In addition, the assignee of an ENUM number would be the assignee of that telephone number. But ENUM could work with any numerical string or, in fact, any domain name.
