The regional distribution of white matter (WM) abnormalities in schizophrenia remains poorly understood, and reported disease effects on the brain vary widely between studies. In an effort to identify commonalities across studies, we perform what we believe is the first ever large-scale coordinated study of WM microstructural differences in schizophrenia. Our analysis consisted of 2359 healthy controls and 1963 schizophrenia patients from 29 independent international studies; we harmonized the processing and statistical analyses of diffusion tensor imaging (DTI) data across sites and meta-analyzed effects across studies. Significant reductions in fractional anisotropy (FA) in schizophrenia patients were widespread, and detected in 20 of 25 regions of interest within a WM skeleton representing all major WM fasciculi. Effect sizes varied by region, peaking at (d=0.42) for the entire WM skeleton, driven more by peripheral areas as opposed to the core WM where regions of interest were defined. The anterior corona radiata (d=0.40) and corpus callosum (d=0.39), specifically its body (d=0.39) and genu (d=0.37), showed greatest effects. Significant decreases, to lesser degrees, were observed in almost all regions analyzed. Larger effect sizes were observed for FA than diffusivity measures; significantly higher mean and radial diffusivity was observed for schizophrenia patients compared with controls. No significant effects of age at onset of schizophrenia or medication dosage were detected. As the largest coordinated analysis of WM differences in a psychiatric disorder to date, the present study provides a robust profile of widespread WM abnormalities in schizophrenia patients worldwide. 3. Investigation of amyloid aggregate formation in various protein systems

This thesis investigates the self assembly of rod-like aggregates termed amyloid fibrils and higher ordered spherical aggregates known as spherulites in various protein systems. The overall theme is to develop an understanding of the mechanism of formation of these structures using in vitro proteins for models. Four proteins in total were studied for comparison: insulin, β-lactoglobulin, hen egg white lysozyme and amyloid-β. Studies on β-lactoglobulin and hen egg white lysozyme showed that the formation of amyloid aggregates occurred on the same time scale as the degradation of the protein molecules. Defined fragment masses were observed and increased in number with increasing incubation time of the proteins. This has lead to the observation that protein degradation may play a key role in amyloid formation. Stirring insulin, β-lactoglobulin, and hen egg white lysozyme samples during incubation altered the phase state of the in vitro fibrils via modification of the fibril aspect ratio. Higher stirring rates led to shorter fibrils and lower amounts of liquid crystallinity. This technique can prevent spherulites from forming and instead form aligned amyloid fibrils producing liquid-crystal like regions. Amyloid-β(1-40) and (1-42) has the ability to form spherulites in vitro under very specific conditions. This is the first observation of spherulites in this peptide which resemble those that readily form in insulin, β-lactoglobulin and hen egg white lysozyme which show the characteristic Maltese cross pattern when viewed between crossed polarisers. Finally, the secondary structure of spherulites formed in insulin, β-lactoglobulin and amyloid-β(1-40) was studied using Raman spectroscopy. This has shown direct similarities between insulin and amyloid-β(1-40). Both show a core which contains a higher proportion of amorphous material, with fibrils that grow radially from the core. BLG spherulites have a less well defined core, the fibrils which twist as they radiate from the centre.

4. The entropy and energy of intergalactic gas in galaxy clusters

Studies of the X-ray surface brightness profiles of clusters, coupled with theoretical considerations, suggest that the breaking of self-similarity in the hot gas results from an `entropy floor', established by some heating process, which affects the structure of the intracluster gas strongly in lower mass systems. Fitting analytical models for the radial variation in gas density and temperature to X-ray spectral images from the ROSAT PSPC and ASCA GIS, we derive gas entropy profiles for 20 galaxy clusters and groups. Scaling these profiles to coincide in the self-similar case, the lowest mass systems are found to have higher scaled entropy profiles than more massive systems. This appears to be due to a baseline entropy of 70-140 h50^-1/3 keV cm^2, depending on the extent to which shocks have been suppressed in low mass systems. The extra entropy may be present in all systems, but is detectable only in poor clusters, compared to the entropy generated by gravitational collapse. This excess entropy appears to be distributed uniformly with radius outside the central cooling regions. We determine the energy associated with this entropy floor, by studying the net reduction in binding energy of the gas in low mass systems, and find that it corresponds to a preheating temperature of ~0.3 keV. Since the relationship between entropy and energy injection depends upon gas density, we can combine the excesses of 70-140 keV cm^2 and 0.3 keV to derive the typical electron density of the gas into which the energy was injected. The resulting value of 1-3x10^-4 h50^1/2 cm-3, implies that the heating must have happened prior to cluster collapse but after a redshift z~7-10. 5. The Imprint of Galaxy Formation on X-ray Clusters

It is widely believed that structure in the Universe evolves hierarchically, as primordial density fluctuations, amplified by gravity, collapse and merge to form progressively larger systems. The structure and evolution of X-ray clusters, however, seems at odds with this hierarchical scenario for structure formation. Poor clusters and groups, as well as most distant clusters detected to date, are substantially fainter than expected from the tight relations between luminosity, temperature and redshift predicted by these models. Here we show that these discrepancies arise because, near the centre, the entropy of the hot, diffuse intracluster medium (ICM) is higher tha$ achievable through gravitational collapse, indicating substantial non-gravitational heating of the ICM. We estimate this excess entropy for the first time, and argue that it represents a relic of the energetic winds through which forming galaxies polluted the ICM with metals. Energetically, this is onl$ possible if the ICM is heated at modest redshift ($z \ltsim 2$) but prior to cluster collapse, indicating that the formation of galaxies precedes that of clusters and that most clusters have been assembled very recently., Comment: 5 pages, plus 2 postscript figures (one in colour), accepted for publication in Nature

Abell 2218 is one of a handful of clusters in which X-ray and lensing analyses of the cluster mass are in strong disagreement. It is also a system for which X-ray data and radio measurements of the Sunyaev-Zel'dovich decrement have been combined in an attempt to constrain the Hubble constant. However, in the absence of reliable information on the temperature structure of the intracluster gas, most analyses have been carried out under the assumption of isothermality. We combine X-ray data from the ROSAT PSPC and the ASCA GIS instruments, enabling us to fit non-isothermal models, and investigate the impact that this has on the X-ray derived mass and the predicted Sunyaev-Zel'dovich effect. We find that a strongly non-isothermal model for the intracluster gas, which implies a central cusp in the cluster mass distribution, is consistent with the available X-ray data and compatible with the lensing results. At r<1 arcmin, there is strong evidence to suggest that the cluster departs from a simple relaxed model. We analyse the dynamics of the galaxies and find that the central galaxy velocity dispersion is too high to allow a physical solution for the galaxy orbits. The quality of the radio and X-ray data do not at present allow very restrictive constraints to be placed on H_0. It is apparent that earlier analyses have under-estimated the uncertainties involved. Context: While 55 million Americans incorporate running into their exercise routines, up to 65% of runners sustain an overuse injury annually. It has been consistently shown that regular physical activity positively impacts quality of life (QOL), an essential public health indicator; however, the impact of running-related injuries on QOL is unknown. This study seeks to determine whether overuse injury severity impacts QOL in recreational runners, and if self-efficacy mediates this relationship. Design: Community-based prospective cohort study of 300 runners who had been running injury free for at least 5 miles/wk in the past 6 months. Methods: Self-efficacy for running and QOL measures (Short Form-12 Physical Component and Mental Component, Satisfaction with Life, Positive Affect and Negative Affect) were assessed at baseline, time of injury, and follow-up visits. Over 2 years of observation, overuse injuries were diagnosed by an orthopedic surgeon and injured runners were referred to a physical therapist. Results: Injury severity was significantly (P <.01) related with 2 indices of QOL, such that the effect of injury severity was −2.28 units on the Short Form-12 physical component and −0.73 units on positive affect. Self-efficacy accounted for 19% and 48% of the indirect effects on Short Form-12 physical component and positive affect, respectively. Conclusions: Since self-efficacy is a modifiable factor related to decreased QOL, these findings have important clinical implications for rehabilitation interventions. [ABSTRACT FROM AUTHOR]

Item does not contain fulltext, Major depressive disorder (MDD) is associated with an increased risk of brain atrophy, aging-related diseases, and mortality. We examined potential advanced brain aging in adult MDD patients, and whether this process is associated with clinical characteristics in a large multicenter international dataset. We performed a mega-analysis by pooling brain measures derived from T1-weighted MRI scans from 19 samples worldwide. Healthy brain aging was estimated by predicting chronological age (18-75 years) from 7 subcortical volumes, 34 cortical thickness and 34 surface area, lateral ventricles and total intracranial volume measures separately in 952 male and 1236 female controls from the ENIGMA MDD working group. The learned model coefficients were applied to 927 male controls and 986 depressed males, and 1199 female controls and 1689 depressed females to obtain independent unbiased brain-based age predictions. The difference between predicted "brain age" and chronological age was calculated to indicate brain-predicted age difference (brain-PAD). On average, MDD patients showed a higher brain-PAD of +1.08 (SE 0.22) years (Cohen's d = 0.14, 95% CI: 0.08-0.20) compared with controls. However, this difference did not seem to be driven by specific clinical characteristics (recurrent status, remission status, antidepressant medication use, age of onset, or symptom severity). This highly powered collaborative effort showed subtle patterns of age-related structural brain abnormalities in MDD. Substantial within-group variance and overlap between groups were observed. Longitudinal studies of MDD and somatic health outcomes are needed to further assess the clinical value of these brain-PAD estimates.

Background Schizophrenia and bipolar disorder share genetic liability, and some structural brain abnormalities are common to both conditions. First-degree relatives of patients with schizophrenia (FDRs-SZ) show similar brain abnormalities to patients, albeit with smaller effect sizes. Imaging findings in first-degree relatives of patients with bipolar disorder (FDRs-BD) have been inconsistent in the past, but recent studies report regionally greater volumes compared with control subjects. Methods We performed a meta-analysis of global and subcortical brain measures of 6008 individuals (1228 FDRs-SZ, 852 FDRs-BD, 2246 control subjects, 1016 patients with schizophrenia, 666 patients with bipolar disorder) from 34 schizophrenia and/or bipolar disorder family cohorts with standardized methods. Analyses were repeated with a correction for intracranial volume (ICV) and for the presence of any psychopathology in the relatives and control subjects. Results FDRs-BD had significantly larger ICV (d = +0.16, q < .05 corrected), whereas FDRs-SZ showed smaller thalamic volumes than control subjects (d = −0.12, q < .05 corrected). ICV explained the enlargements in the brain measures in FDRs-BD. In FDRs-SZ, after correction for ICV, total brain, cortical gray matter, cerebral white matter, cerebellar gray and white matter, and thalamus volumes were significantly smaller; the cortex was thinner (d < −0.09, q < .05 corrected); and third ventricle was larger (d = +0.15, q < .05 corrected). The findings were not explained by psychopathology in the relatives or control subjects. Conclusions Despite shared genetic liability, FDRs-SZ and FDRs-BD show a differential pattern of structural brain abnormalities, specifically a divergent effect in ICV. This may imply that the neurodevelopmental trajectories leading to brain anomalies in schizophrenia or bipolar disorder are distinct.

BACKGROUND: In a randomized, controlled trial that compared liraglutide, a glucagon-like peptide 1 analogue, with placebo in patients with type 2 diabetes and high cardiovascular risk who were receiving usual care, we found that liraglutide resulted in lower risks of the primary end point (nonfatal myocardial infarction, nonfatal stroke, or death from cardiovascular causes) and death. However, the long-term effects of liraglutide on renal outcomes in patients with type 2 diabetes are unknown. METHODS: We report the prespecified secondary renal outcomes of that randomized, controlled trial in which patients were assigned to receive liraglutide or placebo. The secondary renal outcome was a composite of new-onset persistent macroalbuminuria, persistent doubling of the serum creatinine level, end-stage renal disease, or death due to renal disease. The risk of renal outcomes was determined with the use of time-to-event analyses with an intention-to-treat approach. Changes in the estimated glomerular filtration rate and albuminuria were also analyzed. RESULTS: A total of 9340 patients underwent randomization, and the median follow-up of the patients was 3.84 years. The renal outcome occurred in fewer participants in the liraglutide group than in the placebo group (268 of 4668 patients vs. 337 of 4672; hazard ratio, 0.78; 95% confidence interval [CI], 0.67 to 0.92; P=0.003). This result was driven primarily by the new onset of persistent macroalbuminuria, which occurred in fewer participants in the liraglutide group than in the placebo group (161 vs. 215 patients; hazard ratio, 0.74; 95% CI, 0.60 to 0.91; P=0.004). The rates of renal adverse events were similar in the liraglutide group and the placebo group (15.1 events and 16.5 events per 1000 patient-years), including the rate of acute kidney injury (7.1 and 6.2 events per 1000 patient-years, respectively). CONCLUSIONS: This prespecified secondary analysis shows that, when added to usual care, liraglutide resulted in lower rates of the development and progression of diabetic kidney disease than placebo.
