Pulmonary rehabilitation (PR) is an evidenced-based, proven treatment as mentioned recent guidelines in patients with chronic obstructive pulmonary disease (COPD). Exercise training is a cornerstone of PR programs, Inspiratory muscle training, neuromuscular electrical stimulation (NMES) are effective in selected patients. Water-based rehabilitation and tai chi are well tolerated recent modalities. Although there is an absence of a specific PR protocol for special conditions, PR is recommended before and also after endobronchial volume reduction (EBVR), lung volume reduction surgery (LVRS), both before and after lung transplantation periods, before, after surgery, during the intensive care unit (ICU) period, the chemotherapy period and as a component of palliative care. After COPD exacerbation, it is recommended within 3 weeks of hospital discharge. Modifying PR programs while considering comorbidities might lead to greater improvement in outcomes. After PR, the important points are to follow prescribed home exercise programs, control programs in the PR center/unit, and being more active in daily living life for the purpose of preserving improvements. Tele-PR is an alternative to conventional modalities due to similar improvements. Although PR is effective, it is an under-utilized resource. The awareness of PR should be increased in patients and among health professionals.

Amaç: KOAH'ı olan olgularda daha çok polisitemi olduğu bilinmekle beraber son zamanlardaki yayınlarda anemi prevelansının da yüksek olduğu belirtilmektedir. Anemi ile dispne arasındaki ilişki biliniyor olmasına rağmen, diğer klinik durumlarla arasındaki ilişki net değildir. Bu çalışmada pulmoner rehabilitasyon merkezine başvuran KOAH'lı olgulardaki aneminin klinik ve fonksiyonel parametrelerle arasındaki ilişki değerlendirilmiştir. Yöntem ve Gereç: Atatürk Göğüs Hastalıkları ve Göğüs Cerrahisi Eğitim Araştırma Hastanesi Pulmoner Rehabilitasyon merkezinde Ocak 2008- Aralık 2011 tarihleri arasında, pulmoner rehabilitasyon programına yönlendirilen 507 KOAH'ı olan olgu retrospektif olarak değerlendirildi. Anemiye neden olacak hastalığı olan olgular dışlandı. Kalan 176 olgu DSÖ kriterlerine göre anemik, polisitemik, normal hemoglobin düzeyi olanlar olmak üzere 3 gruba ayrıldı. Olguların; dispne algısı Medical Research Council (MRC) dispne skalası ile, sağlıkla ilişkili yaşam kaliteleri St. George's solunum anketi ile, vücut kompozisyonları biyoelektriksel impedans yöntemi ile, egzersiz kapasiteleri artan hızda mekik yürüme testi ve endurans mekik yürüme testi ile, psikososyal durumları ise Hastane Anksiyete Depresyon Skalası ile değerlendirildi. Bulgular: 14 (%8) olguda anemi, 24 (% 14) olguda ise polisitemi saptandı. Anemik olguların normal hemoglobin düzeyi olan olgulara göre MRC dispne skalası skorları istatistiksel olarak daha yüksek (p=0.011), vücut kitle indeksi, yağsız vücut kitlesi, yağsız vücut kitle indeksi, değerleri ise polisitemik ve normal hemoglobin düzeyi olan olgulara göre daha düşük saptandı. Ayrıca polisitemik olguların normal hemoglobin düzeyi olan olgulara göre endurans zamanı istatistiksel olarak daha kısaydı (p=0.036). Sonuç: Anemi oranı stabil KOAH'ı olan olgularda %8 olarak saptandı. Anemi artmış dispne ve kötü nutrüsyonel durum ile ilişkiliydi. Pulmoner rehabilitasyon programına başvuran KOAH'ı olan olgular anemi yönünden değerlendirilmelidir, Çünkü anemi pulmoner rehabilitasyon program sonuçlarını etkileyebilir., Aim: Polycytemia is commonly associated with COPD but recent reports suggest that anemia is also prevalent. Though the associations between anemia and dyspnea, are generally well established the relation with the other clinical manifestations is unknown.The aim of this study was to determine the assosiation of anemia with clinical and functional parameters in patients with COPD who referred to pulmonary rehabilitation (PR) program Material and Methods: 507 COPD patients who referred to Atatürk Chest Disease and Chest Surgery Training Hospital PR Center between January 2008 December 2011 were evaluated retrospectively. Patients who had diseases likely to cause anemia were excluded and the remaining 176 patients were divided into three groups according to WHO criteria as anemia,normal,and polycythemia patients. Dyspnea was assessed with the Medical Research Council(MRC)scale;health related quality of life was assessed with St. George's respiratory questionnaire. Exercise capacity was measured using the incremental shuttle walk test,endurance shuttle walk test, body composition was assessed with bioelectirical impedance, anxiety and depression were assessed with the Hospital Anxiety Depression Scale. Results: Anemia was present in 14(8%)patients and polycythemia in 24 (14%).Anemia patients had significantly higher MRC scale scores than the normal patients(p=0.011),whereas had lower body mass index,fat-free mass index,and fat-free mass values as compared to polycythemia and normal patients.Endurance time was shorter in polycythemia patients than the normal patients (p=0.036). Conclusion: The rate of anemia was found approximately 8% in the patients with stable COPD and associated with increased dyspnea and poor nutritional status.COPD patients who referred to the PR programs should be assessed for anemia;because anemia could affect the PR program results.

Sarkoidoz, başlıca akciğer ve lenfatik sistemi etkileyen, nedeni bilinmeyen, sistemik granülomatöz bir hastalıktır. Pulmoner hipertansiyon önemli bir komplikasyonudur ve sarkoidozun prognozunu kötü yönden etkiler. Bu nedenle olgular bu açıdan dikkatle incelenmelidir. Çalışmamızda, olgularımızı pulmoner hipertansiyon olasılığı açısından, 2009 ERS ve ESC pulmoner hipertansiyon tanı ve tedavi rehberindeki EKO kriterlerine göre sınıflandırdık. Gruplarda, tanı ve takipte kullanılan çeşitli testleri değerlendirdik ve aralarındaki ilişkiyi inceledik.Çalışma, Aralık 2007- Haziran 2009 tarihleri arasında Ankara Üniversitesi Göğüs Hastalıkları Anabilim Dalı Sarkoidoz Ünitesinde izlenen olgular ile yürütüldü. Bu süre içinde yeni tanı alan veya takipte olup kontrole gelen hastalardan çalışmayı kabul edenlerle çalışıldı. Pulmoner arter basıncını etkileyecek ek hastalığı olan olgular (örneğin mitral darlığı, kalp yetmezliği, konjenital kalp hastalığı, hipertansiyon, sarkoidoza eşlik eden skleroderma gibi bağ doku hastalıkları vb) çalışma dışında bırakıldı. Toplam 50 olgu çalışmaya alındı.Sarkoidozda etnik gruplara göre farklı bulunan PH prevalansı bizim çalışmamızda %10 olarak bulundu. PH olgularında hastalık süresi daha uzun olduğu görülürken, evre ile ilişki bulunmadı. PH olasılıklı olgularda, SFT parametrelerinden beklenenin %FEV1, %FVC ve %MMF daha düşük olduğu görüldü. PH olasılıklı grupta DLCO %80 altında, diğer grupta %80 üzerinde olsa da, istatistiksel olarak iki grup arasında DLCO değerleri arasında anlamlı fark bulunmadı. Ayrıca, PAB ile %DLCO arasında korelasyon saptanmadı. Fakat %DLCO ile hastalık evresi ve süresi arasında korelasyon görüldü. İki grup arasında anlamlı fark olmaması, ileri evre PH olasılıklı olgu sayısının az olmasına veya PH nedeni olarak farklı mekanizmaların sorumlu olmasına bağlı olabilir. FVC/DLCO oranın; izole PH ayırt etmede, skleroderma olgularındaki gibi önemli olmadığı görüldü ve sarkoidozda PH mekanizmaların çok çeşitli olduğu göz önününe alınırsa, bu konuda daha fazla çalışmaya gerek olduğu düşünüldü.Çalışmamızda, 6 DYM ortalama 464 m olarak hesaplandı. Hipoksemi ve 6 DYM ile PAB arasında korelasyon görülmese de, PH olan olgularda eforla anlamlı desaturasyon izlendi. Hem PH olasılığına göre grupların karşılaştırılmasında, hem de ortalama pulmoner arter basıncı ile 6 DYM arasında korelasyon analizinde anlamlı fark görülmemesinin nedeni de; olgu sayımızın az olması olabilir. 6 DYT'in PH olan sarkoidozlu olgulardaki, önemi ve ilişkisini vurgulamak için daha fazla sayıda çalışmaya gerek olduğu düşünüldü. BT bulguları açısından anlamlı fark balpeteği alanları ve daha kuvvetli olarak da normalin üst sınırında veya genişlemiş pulmoner arter çapı olarak saptandı. Fakat, yine olgu sayımızın az olması kesin sonucuna varılmasında önemli bir engel olduğu düşünüldü.Sonuç olarak, sarkoidozda PH önemli bir komplikasyondur. Hastalarda radyolojik evreleri düşük de olsa PH gelişebilmektedir. Hastalık süresinde uzama, SFT parametrelerinde azalma, 6DYT sonrası oksijen saturasyonunda %10 veya daha fazla düşme olan ve BT çekildiğinde pulmoner arter çapında artma saptanan olgulara EKO yapılmalı ve PH varlığı araştırılmalıdırAnahtar kelimeler: sarkoidoz, PH, EKO, SFT, 6 DYT, BT Sarcoidosis, a disease of unknown origin, is a systemic granulomatous disease which principally affects lungs and lymphatic system. Pulmonary hypertension is a significant complication and is poor prognostic factor for sarcoidosis. In our study we grouped our patients according to echocardiographic criteria in 2009 ERS/ESC diagnosis and treatment guideline for pulmonary hypertension. We evaluated various tests of diagnosis and treatment as well as the associations between them.This study was conducted with fifty patients who were diagnosed or being followed up in Ankara University Sarcoidosis Unit between December 2007 - June 2009. Patients who had pathologies which could have affected pulmonary artery pressure (eg. Mitral stenosis, cardiac insufficiency, congenital cardiac disorders, hypertension, connective tissue diseases associated with sarcoidosis such as scleroderma) were excluded.PH prevalance in sarcoidosis, which was different in ethnic groups, was found to be 10 %in our study. Duration of disease in PH cases were observed to be longer without any association with the stage. In studies involving sarcoidosis patients with PH, correlation betweeen PFT and pulmonary arterial pressure were very different. In cases with likely PH, PFT parametres such as FEV1 %, FVC%, MMF %were found to be lower. Although DLCO was less than 80 %in the group of likely PH and over 80 %in the other, there were no statistical difference between two groups. There also no correlation between PAP and DLCO %. But, there were correlation between DLCO %and duration and stage of disease. Lack of difference between two groups could be due to relatively low number of advanced stage likely PH cases or other mechanisms responsible for development of PH. Ratio of FVC/ DLCO were observed to be not that much important in differentating isolated PH cases as it was in scleroderma, and considering the fact that there are various mechanisms responsible for development of PH in sarcoidosis, more further studies are needed on this issue.In our study 6 MWT was 464 m. Although there was no correlation between PAP and hypoxemia and 6 MWT in cases with PH significant desaturation with exercise was observed. Lack of significant difference in both the comparison of groups according to probability of PH and correlation analysis between 6 MWT and PAP could be due to small size of the study. It?s thought that more studies are still being needed to evaluate the importance and the association of 6 MWT with PH in sarcoidosis. Significant differences regarding CT findings were found to be honey-comb areas and more importantly upper normal or dilated pulmonary artery diameter. But still, small case number was an important obstacle preventing reaching exact conclusions.In conclusion, PH is an important complication of sarcoidosis. PH may develop even in cases with early radiological stages. Patients which has longer disease duration, decreased PFT parametres, 10% desaturation after 6 MWT, and patients who has increased pulmonary artery diameter in CT, should be evaluated by ECHO and should be searched for the presence of PH.Key words: sarcoidosis, PH, ECHO, PFT, 6 MWT, CT 73
