2. Do Different TSH Suppression Levels Effect Heart Rate Variability and QT Dispersions in Patients with Differentiated Thyroid Cancer?

Objective: The aim of this study was to investigate changes in heart rate variability (HRV) and QT dispersion (QTd) in patients with differentiated thyroid cancer at different TSH suppression levels. Methods: The study included 125 DTC patients, who had been on TSH suppression treatment (TSHST) for at least 1 year. The patients were categorized into three groups: patients with TSH < 0.1 mIU/L (n:30), those with TSH 0.1 to 0.5 mIU/L (n:56), and those with TSH 0.5 to 2 mIU/L (n:39). The first two groups were classified as suppression groups, and the last as replacement (control) group. All patients underwent 12-lead electrocardiogram (ECG) recording and 24-hour rhythm holter echocardiography analysis. Results: The HRV results derived from a 24-hour rhythm holter did not exhibit any significant difference (p < 0.05). In dispersion evaluations, the QTd was significantly longer in the suppression groups (groups 1 and 2), than in the replacement group (group 3) (p < 0.001 and p:0.002, respectively). The same was found for corrected QT dispersion (QTcd) (p < 0.001 and p: 0.008, respectively). In multivariate linear regression analysis, TSH was found to affect QTd (β = -0.299; p = 0.002) and QTcd (β = -0.300; p = 0.002) values independently. Conclusion: In this study, it was shown that in patients with DTC receiving TSHST, QT dispersion prolonged as the TSH suppression level increased. Especially in high-risk DTC patients, evaluation of QTd may be useful in terms of evaluating cardiovascular risk and regulating TSHST level. [ABSTRACT FROM AUTHOR]

Background: In this study, we investigated whether left atrial functions evaluated by speckle tracking echocardiography, classic echocardiographic and clinic parameters predict appropriate Implantable Cardioverter Defibrillator (ICD) shock in patients who underwent ICD implantation for hypertrophic cardiomyopathy. Methods: Totally 87 patients who received ICD implantation for primary or secondary prevention were included in the study. Patients' clinical, elect rocar diogr aphic, 2 dimension classic, and speckle tracking echocardiographic data were collected. Left atrial functions were assessed by speckle tracking echocardiography. Left atrial strain just before mitral valve opening was taken as peak atrial longitudinal strain. Appropriate ICD therapy was defined as cardioversion or defibrillation due to ventricular tachycardia or fibrillation. Patients were divided into 2 groups as occurrence or absence of appropriate ICD therapy during follow-up (mean, 50.2 ± 9.3 months). Patients with an European Society of Cardiology (ESC) risk score >6% were considered high-risk patients. Results: A total of 24 (27.5 %) patients were observed to have an appropriate ICD therapy. In patients on whom appropriate ICD therapy was performed, a higher Sudden Cardiac Death risk Score and decreased peak atrial longitudinal strain and global longitudinal peak strain were observed. In patients with high ESC risk score (> 6%), in Cox regression analysis, peak atrial longitudinal strain (odds ratio: 0.806, P = .008), Sudden Cardiac Death risk score (odds ratio: 1.114, P = .03) and global longitudinal peak strain (odds ratio: 1.263, P = .02) were found to be independent predictors of occurrence of appropriate ICD therapy. Conclusion: Easily measurable peak atrial longitudinal strain may provide additional information in predicting ventricular arrhythmias or deciding on prophylactic medical treatment to prevent ventricular arrhythmias or reduce the frequency of appropriate shock in high-risk patients with ICD implanted. Introduction: Cardiacarrest, which may result in death without an effective cardiopulmonaryresuscitation (CPR), is the unexpected loss of cardiac functions. Suddencardiac arrest is classified as in-hospital and out-of-hospital depending on theplace where the event occurs.Patients and Methods: In thisstudy, 134 patients (age, >18 years) who were admitted or were brought withthe help of their relatives or in an ambulance and medical equipment to theEmergency Department of Kosuyolu Cardiac Hospital with the diagnosis ofin-hospital or out-of-hospital cardiac arrest between 2013 and 2016 wereenrolled. Demographic characteristics of the patients were obtained from thehospital database.Results: In total,134 patients were included in this study. Of these, 95 (71%) were males and 39(29%) were females. The mean patient age was 61.7 ± 14.6 years. In a total of134 cardiac arrests, 58 were in-hospital and 76 were out-of-hospital. Among thepatients who exhibited electrical activity with pulse after CPR, 35 (64.8%)experienced in-hospital cardiac arrest and 19 (35.2%) experiencedout-of-hospital cardiac arrest, whereas among the patients who exhibitedpulseless electrical activity after CPR, 23 (28.7%) experienced in-hospitalcardiac arrest and 57 (71.2%) experienced out-of-hospital cardiac arrest (p, Giriş: Kardiyak arrest etkili kardiyopulmoner resüsitasyon (KPR)yapılmazsa ölümle sonuçlanan kardiyak fonksiyonların beklenmedik şekildekaybolmasıdır. Ani kardiyak arrest olayın meydana geldiği yere bağlı olarakhastane içi ve dışı olarak sınıflandırılır. Hastalar ve Yöntem: Çalışmaya 2013-2016 yıllarıarasında kardiyak arrest tanısıyla hastane içi ve hastane dışı olmak üzereKoşuyolu Kalp Hastanesi Acil Servisine başvuran veya hasta yakını ya daambulans yardımı ile tıbbi ekipmanla getirilen 18 yaş üstü 134 hasta dahil edilmiştir.Hastaların demografik özellikleri hastane veri tabanından sağlanmıştır.Bulgular: Çalışmaya 95 (%71) erkek, 39 (%29) kadın hasta olmak üzere toplam 134 hasta dahiledilmiştir. Yaş ortalaması 61.7 ± 14.6 olarak saptandı. Total 134 kardiyakarrestin 58’i hastane içi, 76’sı hastane dışı olarak saptanmıştır. KPR sonrasınabızlı elektriksel aktivite saptanan hastaların 35 (%64.8)’i hastane içiarrest, 19 (%35.2)’u hastane dışı arrest; nabızlı elektriksel aktivitesağlanamayan hastaların 23 (%28.7)’ü hastane içi arrest, 57 (%71.2)’si hastane dışı arrest olarak saptanmıştır(p< 0.001).Sonuç: Çalışmamızda kardiyakarrest nedeni olarak en sık nedenler; ST yükselmeli miyokart infarktüsü,konjestif kalp yetersizliği ve nedeni belirlenemeyen grup olarak belirlenmiştir.Geliş ritmi olarak en fazla asistol ritmi saptanmıştır. KPR sonrası nabızlıelektriksel aktivite sağlanan grupta saptanan ventriküler fibrasyon oranınabızlı elektriksel aktivite sağlanamayan gruptakine göre daha yüksek orandatespit edilmiş ve istatistiksel olarak anlamlı saptanmıştır. Çalışmamızdaliteratüre benzer olarak şoklanabilir ritim olanlarda nabızlı elektrikselaktivite sağlanma oranı daha fazla saptanmıştır.

Hamidi, Mehmet (Balikesir Author), Objective: This study aimed to determine the differences in terms of demographic characteristics and preferred stroke prevention strategies for patients with non-valvular atrial fibrillation living in seven geographical regions of Turkey.Methods: In total, 6273 patients were enrolled to this prospective, observational RAMSES study. The patients were divided into seven groups based on the geographical region of residence.Results: In terms of the geographical distribution of the overall Turkish population, the highest number of patients were enrolled from Marmara (1677, 26.7%). All demographic characteristics were significantly different among regions. Preferred oral anticoagulants (OACs) also differed between geographical regions; non-vitamin K OACs were preceded by warfarin in East Anatolia, Aegean, Southeast Anatolia, and Black Sea. Nearly one-third of the patients (28%) did not receive any OAC therapy. However, the number of patients not receiving any OAC therapy was higher in Southeast Anatolia (51.1%) and East Anatolia (46.8%) compared with other geographical regions of Turkey. Inappropriate use of OACs was also more common in East and Southeast Anatolia.Conclusion: This study was the first to show that the demographic differences among the geographical regions may result in different preferences of stroke prevention strategies in Turkey. OACs are still under- or inappropriately utilized, particularly in the eastern provinces of Turkey.

WOS: 000402283900004, PubMed ID: 28407216, BackgroundThe data regarding stroke prevention strategies in nonvalvular atrial fibrillation (NVAF) are limited especially in patients with renal impairment (RI). We sought to evaluate management dilemmas in patients with concurrent NVAF and RI in RAMSES (ReAl-life Multicenter Survey Evaluating Stroke Prevention Strategies inTurkey) study. MethodsWe conducted a prospective, multicenter, nation-wide registry in NVAF patients in outpatient cardiology clinics. All consecutive patients with NVAF were enrolled in RAMSES study (ClinicalTrials.gov identifier NCT02344901). The baseline data were collected. Glomerular filtration rate (GFR) was estimated by Cockcroft-Gault equation. ResultsA total number of 6273 patients from 29 provinces of Turkey with the contribution of 83 investigators were enrolled to the study. Of the study population, 1964(33%) patients had RI which was defined as GFR < 60 mL/min. Patients with RI had significantly higher CHA(2)DS(2)VASc and HAS-BLED scores compared to those without RI (39 15 vs. 29 +/- 15, and 20 +/- 1 vs. 14 +/- 1; P < 0001). Prior history of major bleeding (69% vs. 41%, P < 0001) and stroke (162% vs. 118%, P < 0001) was significantly higher among individuals with concomitant RI and NVAF. Although RI patients had a higher risk for thromboembolism, number of the patients who did not receive any anticoagulant therapy was higher in patients with RI than without RI (301 vs. 264%, P = 0003). ConclusionRAMSES study showed that one-third of the patients with NVAF had RI in the real-world setting. Although it is mandatory in most of the patients with concomitant NVAF and RI, nearly one-third of these patients did not receive any anticoagulant therapy.

WOS: 000394997800003, PubMed ID: 27848065, The definition of non-valvular atrial fibrillation (NVAF) is controversial. We aimed to assess the impact of valvular heart disease on stroke prevention strategies in NVAF patients. The RAMSES study was a multicenter and cross-sectional study conducted on NVAF patients (ClinicalTrials.gov identifier NCT02344901). The study population was divided into patients with significant valvular disease (SVD) and non-significant valvular disease (NSVD), whether they had at least one moderate valvular disease or not. Patients with a mechanical prosthetic valve and mitral stenosis were excluded. Baseline characteristics and oral anticoagulant (OAC) therapies were compared. In 5987 patients with NVAF, there were 3929 (66%) NSVD and 2058 (34%) SVD patients. The predominant valvular disease was mitral regurgitation (58.1%), followed by aortic regurgitation (24.1%) and aortic stenosis (17.8%). Patients with SVD had higher CHA(2)DS(2)VASc [3.0 (2.0; 4.0) vs. 4.0 (2.0; 5.0), p < 0.001] and HAS-BLED [2.0 (1.0; 2.0) vs. 2.0 (1.0; 2.0), p = 0.004] scores compared to patients with NSVD. Overall, 2763 (71.2%) of NSVD and 1515 (73.8%) of SVD patients were on OAC therapy (p = 0.035). When the patients with SVD were analyzed separately, the mean CHA(2)DS(2)VASc and HAS-BLED scores were higher in patients with mitral regurgitation compared to patients with aortic regurgitation and aortic stenosis [4.0 (3.0; 5.0), 3.0 (2.0; 4.0), 3.0 (2.0; 4.0) p < 0.001 and 2.0 (1.0; 3.0), 1.0 (1.0; 2.0), 1.0 (0.0; 2.0) p < 0.001, respectively]. In patients with SVD, 65.7% of mitral regurgitation, 82.6% of aortic regurgitation and 88.0% of aortic stenosis patients were on OAC therapy. One out of three NVAF patients had at least one moderate valvular heart disease with the predominance of mitral regurgitation. Patients with SVD were at greater risk of stroke and bleeding compared to patients with NSVD. Although patients with mitral regurgitation should be given more aggressive anticoagulant therapy due to their higher risk of stroke, they are undertreated compared to patients with aortic valve diseases.

WOS: 000375095000005 PubMed ID: 27138310 Objective: Atrial fibrillation is the most common arrhythmia and is associated with a five-fold increased risk of thromboembolic events. Vitamin K antagonists (VKAs) have been the mainstay of oral anticoagulant prophylaxis and the data on stroke prevention strategies are limited to VKA era. The purpose of this study is to evaluate the use of VKA, non-Vitamin K antagonist oral anticoagulants (NOAC), and antiplatelet agents in patients with non-valvular atrial fibrillation (NVAF). Methods: The ReAl-life Multicenter Survey Evaluating Stroke prevention strategies in Turkey (RAMSES) is an observational, multicenter, prospective study of patients with NVAF. The study targeted enrollment of 7835 patients from 68 sites in Turkey. All the data will be collected at one point in time and current clinical practice will be evaluated. (ClinicalTrials.gov number NCT02344901). Results: Baseline characteristics of patients, antithrombotic therapies, transition to NOACs and rate/rhythm control strategies will be evaluated. Conclusion: The RAMSES registry will be the largest study in Turkish NVAF patients. The study will provide insights into real-world problems and anticoagulant treatment in patients with NVAF.

Objective: Although several hypotheses have been suggested, the underlying mechanism of coronary slow flow phenomenon (CSFP) has not been well established yet. The aim of this study was to determine the roles of plasma apolipoprotein-B100 (Apo-B) and apolipoprotein-A1 (Apo-A1) in CSFP which have an atherogenic effect and anti-atherogenic effects respectively. Methods: The study consisted of 31 patients with CSFP (group 1), 28 normal subjects as control group (group 2) and 30 patients with coronary artery disease (CAD) (group 3) detected by coronary angiography. CSFP was diagnosed by the TIMI frame count method. Blood Apo-B, Apo-A1, Apo-B/Apo-A1 ratio, and demographic parameters were compared between the groups. Results: The Apo-B values were 93±25 mg/dL, 90±26 mg/dL, and 106±27 mg/dL in groups 1, 2 and 3, respectively (p=0.048 between group 1 and 3, p=0.041 between group 2 and 3, p= NS between group 1 and 2). The Apo-A1values were 127±14 mg/dL, 125±21 mg/dL and 106±27 mg/dL in groups 1, 2 and 3 respectively (p=0.028 between group 1 and 3, p=0.021 between group 2 and 3, p= NS between group 1 and 2). The apo- B/apo-A1 ratio were 0.73±0.18, 0.69±0.23 and 0.98±0.20 in groups 1, 2 and 3 respectively (p=0.017 between group 1 and 3, p=0.010 between group 2 and 3, p= NS between group 1 and 2). Conclusion: Although lower levels of plasma Apo-A1 and higher levels of Apo-B and the ratio of Apo-B to Apo-A1 are related with CAD, there is no relationship between these apolipoproteins and CSFP., Amaç: Koroner yavafl akm fenomenin (KYAF) altta yatan mekanizmas henüz tam olarak tanmlanmamfl olmas na ra¤men çeflitli hipotezler ileri sürülmüfltür. Bu çalflmamz da srasyla aterojenik ve anti-aterojenik etki gösteren plazma apolipoprotein-B100 (Apo-B) ve apolipoprotein-A1'in (Apo-A1) KYAF'deki rolünü arafltrdk. Objective: Atrial fibrillation (AF) is the most common arrhythmia following coronary artery by-pass graft surgery (CABG). The value of SYNTAX score to predict postoperative atrial fibrillation (PoAF) has not been clearly addressed. We aimed to evaluate this relationship in patients undergoing isolated CABG. Methods: This study was designed as a single-center, non-randomized, observational, prospective study. Ninety-four patients undergoing isolated on-pump CABG, who had sinus rhythm and were older than 18 years, were enrolled. Demographic characteristics of the patients were recorded; SYNTAX score was calculated preoperatively for each patient. The univariate and multivariate logistic regression analysis were used to determine for predictors of PoAF. Results: The median SYNTAX score of the enrolled patients was 21, (56-5). PoAF was observed in 31 (33.3%) patients. Univariate logistic regression showed that age, chronic obstructive pulmonary disease (COPD), red blood cell distribution width (RDW), urea, initial troponin I, peak postoperative troponin I, interventricular septum, left atrial diameter, and SYNTAX score were significantly associated with the frequency of PoAF following CABG. An independent association was identified with age [β: 0.088, p:0.023, OR: 1.092, 95% CI (1.012-1.179)], COPD [(β: 2.222, p:0.003, OR: 9.228, 95% CI (2.150-39.602)], and SYNTAX score [(β: 0.130, p:0.002, OR: 1.139, 95% CI (1.050-1.235)]. Conclusion: This study showed that a higher SYNTAX score was related to more frequent PoAF in patients undergoing isolated on-pump CABG. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: Acute decompensated heart failure (ADHF) is a life-threatening medical condition and more than 30% of patients hospitalized for ADHF develop acute kidney injury (AKI), which increases the rate of both mortality and morbidity. Previous research has indicated that several biomar- kers may help to predict the development of AKI. The aim of this study was to investigate the relationship between lactate level at the time of admission and AKI in ADHF patients., Methods: A total of 154 consecutive ADHF patients were prospectively enrolled from June 2018 to December 2018, and after applying the exclusion criteria, a total of 91 patients were included in the study. The patients were divided into 2 groups: those with and without AKI., Results: There were 63 patients in the group without AKI and 28 patients in the group with AKI. The AKI group had a higher percentage of a history of chronic kidney disease (CKD), a higher creatinine level, lower glomerular filtration rate level, lower bicarbonate level, higher lactate level, and a lower left ventricular ejection fraction compared with the non-AKI group (p<0.05 for all parameters). Multiple logistic regression analysis determined that CKD history (odds ratio [OR]: 4.003, 95% confidence interval [CI]: 1.295-12.371; p=0.016) and lactate level (OR: 1.545, 95% CI: 1.222-1.954; p<0.001) were independent predictive parameters for developing AKI., Conclusion: An elevated lactate level may help to make an early diagnosis of AKI, an important concern in ADHF.

Objective: The aim of this study was to assess the validity of automated quantitative and semiquantitative visual analysis of total perfusion deficit (TPD) using the IQ SPECT gamma camera system compared to conventional coronary angiographically detected significant coronary artery disease (CAD)., Methods: The study included patients with suspected CAD who underwent myocardial perfusion single photon emission computed tomography and conventional coronary angiography. The summed stress score (SSS), summed rest score (SRS), and summed difference score (SDS) (semiquantitative visual analysis results) were assessed using a 5-point scale in a standard 17-segment model, and TPD (stress, rest, and ischemic TPD) was quantified using automated software., Results: In all, 84 patients (Group 1, those who underwent revascularization) had significant coronary artery lesions, and 81 (Group 2) had non-significant lesions. The median interquartile range values were: stress-TPD (sTPD): 16 (3.5- 33.5) vs 9.2 (2-17.9), rest-TPD: 9.4 (2.2-18.8) vs 4 (1-11), and 6.9 (1.9-14.1) vs 3.4 (1-6.1) for ischemic-TPD (iTPD) in Group 1 and Group 2, respectively. To detect ischemia, the optimal cut-off points were 9.5 (sensitivity: 75%, specificity; 60%) for sTPD, and 4.5 (sensitivity: 56%, specificity: 73%) for iTPD. There were significant correlations between quantitative and semi-quantitative methods in detection of significant coronary artery disease (sTPD-SSS: r=0.954, sTPD-SDS: r=0.746, iTPD-SSS: r=0.654, iTPD-SDS: r=0.759; p<0.05 for all)., Conclusion: The quantitative analysis and summed stress scores produced by the IQ SPECT system appear to be a useful and valid method to detect significant CAD.

Coronary artery bifurcation disease of a saphenous vein graft (SVG) is extremely rare. SVG disease remains a challenging lesion to treat because of increased morbidity and mortality with repeated coronary artery bypass graft (CABG) surgery, a high rate of periprocedural complications, and in-stent restenosis or occlusion requiring repeat revascularization with percutaneous coronary intervention. Presently described is use of the simultaneous kissing stent technique to treat inverted Y SVG bifurcation disease in a patient with a prior CABG and new-onset acute coronary syndrome.

- Abstract
Coronary fistulae may lead to coronary steal phenomenon or considerable volume overload on the cardiac chambers, causing significant hemodynamic problems. Coronary fistulae can be closed either surgically or percutaneously. Percutaneous closure is frequently performed with coil embolization or a vascular plug. Although percutaneous closure has significant advantages, such as a shorter duration of hospitalization and no sternal scarring, several complications, including coil embolization or failure to retrieve the device, may occasionally occur. In the current report, a patient with a left coronary to right atrium fistula who declined to have surgery underwent percutaneous coil embolization. However, after release of the coil, the catheter could not be retrieved using the standard anti-torque mechanism.

Endovascular interventions are now used more frequently in the treatment of femoropopliteal occlusions, and antegrade passage through the occlusion is preferred as the first option. However, when antegrade passage fails for any of several reasons, retrograde, or less frequently, collateral passage may be used. Although collateral passage may present serious complications, it can be successfully applied in selected cases in experienced centers. In the present case, a superficial femoral artery occlusion was successfully opened with a collateral approach.

Coronary fistulas are defined as the presence of an abnormal connection between the coronary arteries and the low-pressure vascular area or the cardiac cavity. The clinical significance depends on the amount of blood flow through the fistula segment, the volumetric load on the right and left heart chambers, and whether it leads to a coronary steal phenomenon. Although fistula flow can be better visualized by angiographic methods, it can also be seen by echocardiography. In this case, the fistula flow draining to the left ventricle was demonstratively visualized.

Presently described is the retrieval of an embolized device with a novel percutaneous technique.
