Amaç: Çalışmamızın amacı, AMATEM biriminde tedavi gören hastaların madde kullanım profilini onların idrar örneklerindeki madde kullanım yüzde dağılımlarına ve çoklu madde kullanımlarına göre belirlemektir. Bunun için, İstanbul Nöropsikiyatri Hastanesi AMATEM biriminde 1 Ocak 2015 - 12 Aralık 2015 tarihleri arasında tedavi gören 13 ve 65 yaş aralığında erkek ve kadın 600 hastanın idrar örneklerinin analiz raporları retrospektif olarak değerlendirildi.Gereç ve Yöntemler: İdrar örnekleri Üsküdar Üniversitesi İleri Toksikoloji Analiz Laboratuvarı'na gönderildi ve orada UPLC kütle spektrometresi ile (UPLC-MS/MS) analiz edildi. AMATEM birimine başvuruda bulunan hastaların madde kullanım profillerini tanımlamak için hastaların analiz raporları üzerinden istatistiksel değerlendirme yapılmıştır.Bulgular: Altı yüz idrar örneğinin analiz raporu incelendiğinde, hastaların 293'ünde bağımlılık yapıcı madde kullanımı belirlenmiştir. İdrar örneklerinde çoğunlukla tespit edilen maddeler sırasıyla; esrar, alkol, morfin, kokain, sentetik kannabinoitler, 3,4-metilendioksimetamfetamin ve amfetamindir.Sonuç: Çalışmamızın sonuçları bütün dünyada genç nüfus tarafından esrar kullanım oranı ile benzerlik göstermektedir. Ayrıca, sentetik kannabinoitlerin çeşitliliği her yıl dünyada hızla değiştiği için çalışmamızın sonuçları ve literatüre sonuçları sentetik kannabinoit tüketimi konusunda farklılık göstermektedir. To determine the substance abuse profiles of patients treated a Drug Addiction Research, Treatment, and Education Center (AMATEM) in association with the percentage of substance use distribution and multiple substance use in their urine samples. For this, we retrospectively evaluated the urine sample analysis reports of 600 male and female patients aged 13 to 65 years who were treated at the AMATEM unit of İstanbul Neuropsychiatry Hospital between January 1st, 2015, and December 12th, 2015. Materials and Methods: The urine samples were sent to Üsküdar University Advanced Toxicology Analysis Laboratory and were analyzed using a UPLC tandem mass spectrometer (UPLC-MS/MS). To determine the substance use profiles of the patients applying to AMATEM, statistical assessment was performed on the analysis reports of the patients. Results: When the analysis reports of the 600 urine samples were examined, 293 patients were identified to have used addictive substances. The substances most frequently detected in the urine samples were respectively: cannabis, alcohol, morphine, cocaine, synthetic cannabinoids, 3,4-Methylenedioxymethamphetamine, and amphetamine. Conclusion: The findings in our study resemble the rates of cannabis use by the young population throughout the world. Our results show differences to the literature regarding the consumption of synthetic cannabinoids because the variety of synthetic cannabinoids change rapidly around the world each year.

Terapötik ilaç dozunun ayarlanması için kullanılan terapötik ilaç monitörizasyonu (TDM), plazmadaki ilacın konsantrasyonunu belirlemede kullanılır. TDM'nin başarılı bir şekilde uygulanması için ilaç ve metabolit seviyelerini belirlemede seçici ve duyarlı analitik yöntemler kullanılır. Çalışmanın amacı, insan plazma ve idrar örneklerinde essitalopram ( S-CT) ve metabolitlerinin kantitatif analizini yapmak için sıvı kromatografi kütle spektrometresi (LC-MS / MS) kullanarak yeni bir yöntem geliştirmek ve doğrulamaktır. Ayrıca, bireysel farmakokinetik özelliklerin belirlenmesi amacıyla, essitalopram ve metabolitlerinin metabolik oranı (MR) ile hastaların sitokrom P450 enzim aktiviteleri (CYP2C19 ve CYP2D6) arasındaki ilişki bu doğrulanmış yöntem kullanılarak değerlendirildi. Hastaların farmakokinetik özelliklerini bilmenin yanı sıra, bireylerin farmakodinamik özelliklerini bilmenin de tedavinin başarısını artırmada önemli bir rolü olduğu düşünülmektedir. Bu nedenle, aynı hastaların seratonin taşıyıcı protein (5-HTT) polimorfizim dağılımı da incelenmiştir. Bu çalışma ile hekimin hastalara uygun ve güvenli bir tedavi dozu sağlamasına yardımcı olması amaçlanmıştır.Bu çalışmada, NPİstanbul Beyin Hastanesi'nde essitalopram kullanan hastalardan alınan plazma ve idrar örnekleri Üsküdar Üniversitesi Klinik Farmakogenetik Laboratuvarı'nda sıvı kromatografi kütle spektrometresi (LC-MS / MS) cihazı kullanılarak analiz edildi ve S-CT, dimetil essitalopram (S-DCT) ve didemetil essitalopram (S-DDCT) konsantrasyonları ölçüldü. Ek olarak, aynı hastaların CYP2C19, CYP2D6 ve farmakodinamik (5-HTT) polimorfizmlerini tanımlamak için alınan tüm kan örneklerinden bir genotipleme çalışması yapıldı. İstatistiksel değerlendirme, genotipleme sonuçları ile hastaların kan seviyesi sonuçları ve metabolik oranları arasındaki ilişkiye bakılarak yapıldı. Ortalama plazma S-CT konsantrasyonu 27.59 ng / mL olan 30 hastanın ortalama S-DCT ve S-DDCT konsantrasyonları sırasıyla 85.52 ng / mL ve 44.30 ng / mL idi. Major yol olarak CYP2C19 enzimi tarafından S-DCT'ye metabolize olan essitalopramın ortalama S-CT / S-DCT değeri 0.40 olarak hesaplandı. Bu çalışmada, CYP2C19 EM ve CYP2C19 IM gruplarındaki hastaların ortalama metabolik oranı, UM grubuna kıyasla farklı olduğu bulunmuştur. Bu nedenle, genotipleme analizi yapılmayan bir çok hasta için S-CT / S-DCT değeri dikkate alınarak, CYP2C19 enzim aktivitesi ile ilgili bir ön değerlendirme yapılabileceği düşünülmektedir. Ayrıca, CYP2D6 EM ve CYP2D6 Het EM hastalarında ortalama metabolik oranın da CYP2D6 IM grubuna göre farklı olduğu bulunmuştur. Hastaların 5-HTT polimorfizim dağılımı ile ilaç tedavisine verdikleri yanıt incelendiğinde, gruplar arasındaki farkın istatistiksel olarak anlamlı olduğu gözlendi (p 0.05). Çalışmamızdan elde edilen verilerin hekimler tarafından değerlendirilmesinin, bireysel ilaç tedavisinin uygulanmasında hastaların tedavisine katkıda bulunabileceği düşünülmektedir. Therapeutic Drug Monitorization (TDM) is used to determine the concentration of drug in plasma to adjust the dose of therapeutic drug. Selective and sensitive analytical methods are used to determine drug and metabolite levels for the successful application of TDM. The aim of the study was to develop and validate a new method using liquid chromatography mass spectrometry (LC-MS / MS) to analyze quantitative assay of escitalopram ( S-CT) and its metabolites in human plasma and urine samples. Also, association between metabolic ratio (MR) of S-CT and its metabolites and cytochrome P450 enzyme activities (CYP2C19 and CYP2D6) of patients were assessed using this validated method in order to determine individual pharmacokinetic characteristics. It is thought that besides knowing the pharmacokinetic characteristics of patients, pharmacodynamics characteristics of individuals have an essential role in increasing the success of the treatment. Therefore, serotonin transporter protein (5-HTT) polymorphism distribution of the same patients was examined as well. With the study, it is aimed that it could help the physician provide a convenient and safe treatment dose for the patients.In this study, plasma and urine samples collected from patients using escitalopram as part of their treatment in NPİstanbul Brain Hospital were analyzed by LC-MS / MS at Üsküdar University Clinical Pharmacogenetic Laboratory and the concentration of S-CT, dimethyl escitalopram (S-DCT) and didemethyl escitalopram (S-DDCT) were measured. In addition, a genotyping study was performed from whole blood samples taken to identify CYP2C19, CYP2D6 and pharmacodynamic (5-HTT) polymorphisms of the same patients and statistical evaluation was performed by looking at the relationship between blood level results and MR of patients with their genotyping results. The mean S-DCT and S-DDCT concentrations of the 30 patients with mean plasma escitalopram concentration of 27.59 ng/mL were 85.52 ng/mL and 44.30 ng/mL, respectively. The mean S-CT / S-DCT value of the S-CT, metabolized to the S-DCT by the CYP2C19 enzyme as the major pathway, was calculated to be 0.40. In this study, it is found that the mean MR of patients in the CYP2C19 EM and IM groups is different compared to the UM group. Therefore, it is thought that by considering the S-CT / S-DCT value for many patients without genotyping analysis, a pre-assessment can be made regarding the CYP2C19 enzyme activity. Also, it is found that the mean MR of patients in the CYP2D6 EM and CYP2D6 Het. EM group is different compared to the CYP2D6 IM group. When the response of 30 patients to drug treatment with 5-HTT polymorphism distribution of patients was examined, it was observed that the difference between the groups was statistically significant (p 0.05). Evaluation of the data obtained from our study by physicians is thought to be able to contribute treatment of patients in the application of individual drug treatment. 190

Bu çalışmada Haydarpaşa Numune Hastanesi Üroloji Servisi'ne başvuran hastaların tutumlarını ve ilaç kullanım alışkanlıklarının tanımlanması amaçlanmıştır. Araştırma, kesitsel analitik nieliktedir. Hastalara ait reçeteleri maliyet araştırması için kullanılmıştır. Katılımcıların % 50.7'si bayan, % 78'i 44? yaş grubunda, % 34.7'si lise, % 30.7'si üniversite mezunudur. Hastaların % 9'unun herhangi bir sağlık güvencesi yoktur. Çalışma sonunda hastaların % 4.3'ü çevresinden ilaç istediğini, % 52.7'si doktora başvurduğunu, % 25.3'ü kullandıkları ilacı başkasına da önerdiklerini belirtmişlerdir. Hastaların % 32'si ilaçların en çok yan etkisini, % 9.7'si tedavi süresini öğrenmek istediğini belirtmiştir. Prospektüs okuma alışkanlığı cinsiyet, yaş ve öğrenim durumuna göre farklılık göstermektedir. Son kullanma tarihine dikkat etme tutumu ise öğrenim durumuna göre farklılık göstermektedir. Katılımcıların % 40.3'ü evlerinde kullanılmayan ilaç bulunduğunu, % 67'si reçetesiz antibiyotik almadığını belirtmiştir. Sağlık güvencesine sahip olma niteliği; öğrenim durumu, yaş ve reçetesiz antibiyotik alma oranını değiştirmektedir. Hasta reçetelerinde toplam 469 ilaç yazılmıştır. Reçete başına düşen ortalama ilaç sayısı 1.56' dır. İlaçların % 30'u antibiyotiktir. Reçetelerin % 4'üne 3 ya da daha fazla ilaç yazılmıştır. Toplam reçete maliyeti 10166.82 YTL'dir. İlaç firmalarının Devlete uyguladıkları iskonto ile toplam ilaç fiyatı 9256.55 YTL olarak değişmiştir. Devletin ödediği fiyat kişilerin bağlı olduğu kuruma göre 7405.24 YTL ile 8330.895 YTL arasında değişmektedir.Akılcı ilaç kullanımı konusunda ülkemizde var olan sorunları çözmek için Sağlık Bakanlığı, Türk Tabipler Birliği, Üniversiteler ve Pratisyen Hekimlik Derneği'nin ortak çabalarına gereksinim vardır., The aim of this study is to determine the attitude of patients attending to Urology department of Haydarpaşa State Hospital during illness and drug using habits. This is a cross analytic type study. A copy of the patients? prescriptions was taken for the research of medical cost. The percentage of female participants was 50.7. % 78 of them was in ? 44 ages. In terms of education, % 34.7 of them were graduated from high school, % 30.7 of them were graduated from a university. % 9 of the participants didn?t have any social insurance. The percent 52.7 of the patients expressed that they applied to doctor and % 4.3 asked from others to prescribe their medicines. % 25.3 advised their medicines to others for use. Thirty two percent of participants wanted their doctors to inform them about side effects of medicines. % 9.7 of the patients asked to learn about the length of treatment prescribed. The habits of reading the drugs prospectus were significantly different depend on the age, sex and education level. The attitude of the checking the expiry date of drugs was significantly higher in the educated patients It was expressed by % 40.3 of participants that they have some unused medicines in their home. % 67 of the patients explained that they do not use antibiotics without prescription. The ratio of using antibiotics without prescription was affected by the education level, age and social insurance of the patient. The evaluation of the prescriptions demonstrated that 469 different type of drug was prescribed as total. The average of the drug number per one prescription was 1.56. % 30 of the drugs was an antibiotic. The percent 4 of the prescriptions included ? 3 drugs. Total prescriptions cost was 10166.82 YTL. The total cost of the drugs was changed to 9256.55 YTL, since the drug companies apply some discount for the Ministry of Health. The contribution of the government to the cost of drugs differ from 7405.24 to 8330.895 YTL according to type of the social insurance of the patients. The combined efforts of Ministry of Health, Turkish Medical Association, Universities and Association of General Practitioners are needed to solve the problems in the rational drug usage in Turkey.
