Although the new coronavirus disease (COVID-19) has a wide variety of complications, one of the least common of these is pneumomediastinum. In pneumomediastinum, which is defined as the presence of free air in the mediastinum, clinically dyspnea, chest pain and subcutaneous emphysema are observed. Here, we present a case of spontaneous pneumomediastinum in a patient infected with COVID-19 without any risk factors., Yeni Koronavirüs Hastalığı’nın (COVID-19) çok çeşitli komplikasyonları olmakla birlikte, bunlardan en az görülenlerden biri de pnömomediastinumdur. Mediastinumda serbest hava bulunması olarak tanımlanan pnömomediastinumda, klinik olarak nefes darlığı, göğüs ağrısı ve cilt altı amfizemi izlenir. Biz burada hiçbir risk faktörü olmadığı halde COVID-19 ile enfekte bir hastada gelişen spontan pnömomediastinum vakasını sunuyoruz.

Aim: The purpose of this study was to explore the effects of exercise and smoking history of the COVID-19 patients on their recovery course and time. Methods: In this respect, as the data source, we observed a total of 310 patients, 176 males 134 females, who tested positive for COVID-19, had no chronic disease, and received inpatient or outpatient treatment. The patients also filled out a personal information form covering their demographic background, including smoking and exercise history. All participants received favipiravir as the standard medication, and their symptoms and the durations of these symptoms were evaluated using the focus group interview method. Objective: Famotidine is an H2 receptor antagonist (H2RA) and has been shown to have antiviral properties in in-vitro studies. Pantoprazole is one of the proton pump inhibitors (PPI). In this study, it was aimed to compare the efficacy of Famotidine with Pantoprazole in the treatment of COVID-19.Material-method: Patients who were hospitalized and given famotidine and pantoprazole treatment for at least 48 hours were included in the study. Demographic, clinical and laboratory findings of the patients were analyzed retroprospectively from the patient files. While the primary endpoints were the need for an intensive care unit (ICU) and death, the secondary endpoints were the absence of the need for oxygen support, fever to normal levels, and length of hospital stay.Results: A total of 179 Covid-19 patients (85 (47.5%) famotidine, 94 (52.5%) pantoprazole) were included in the study. Demographic findings and other symptoms except dyspnea were similar in both groups (p> 0.05). Dyspnea, chronic diseases, and the number of patients given steroids were higher in those who were given pantoprazole (p 0.05). Demografik bulgular ve dispne dışındaki diğer semptomlar her iki grupta benzerdi. Pantoprazol verilenlerde dispne, kronik hastalıklar ve steroid verilen hasta sayısı daha yüksekti. Mortalite ve YBÜ ihtiyacı her iki grupta benzerdi (sırasıyla; p=0.25, p=0.26). Famotidin verilenlerde ateşli gün sayısı, hastanede kalış süresi ve oksijen desteği gerektiren gün sayısı daha azdı (sırasıyla; p=0.04, p=0.003, p=0.014).Sonuç: Famotidin, hastanede tedavi gören COVID-19 hastalarında yoğun bakım ihtiyacını ve mortaliteyi azaltmadı. Hastalık şiddetini ve ölüm oranını azaltmak için yeni tedavilere ihtiyaç vardır.

Background/aim: Oxidative stress plays an important role in the pathogenesis of chronic obstructive pulmonary disease (COPD). Smoking is the leading source of oxidants in lungs. However, it is currently unknown why some individuals are more resistant to the detrimental effects of smoking and do not develop COPD. The aim in this study is to measure and compare the oxidant/antioxidant balance between in non-COPD individuals who smoke and COPD patients who smoke. Materials and methods: Included in the study were 137 patients with COPD and 102 healthy individuals. Participants were divided into groups as COPD patients (former and current smokers), non-COPD individuals who smoke and non-smokers healthy persons. In the following stage, the total antioxidant status (TAS), total oxidant status (TOS) and oxidative stress index (OSI) levels were measured in serum for all participants. Results: In the current-smoker COPD group, the level of oxidant status were significantly higher than the former-smoker COPD group (p < 0.001). Similarly, oxidant levels were significantly high in current-smoker healthy group than never smoker healthy group. According to these results TOS was associated with especially smoking status rather than COPD. Antioxidant status were similar between former-smoker COPD group and current-smoker COPD group. The antioxidant levels were found significantly low in current-smoker COPD patients, compared to the current-smoker non-COPD individuals (p = 0.007). Nevertheless, no significant difference was found in OSI levels between two groups. Briefly, high TOS and OSI values were correlated with only smoking, independently from COPD. Conclusion: It was concluded that there are complex pathogenetic mechanisms, including genetic and individual variations other than oxidant/antioxidant balance, involved in the development of smoking-related COPD. TOS and OSI values are not predictive parameters for the development of COPD, but high level of TAS in non-COPD smokers is promising for future studies. (C) 2019 Elsevier Inc. All rights reserved. Occupational asthma (OA) is characterized by reversible airway obstruction and/or bronchial hyperresponsiveness occurred after exposition to dust, vapor, gas or smoke which are present in the workplace. We aimed to determine the frequency of OA in auto and furniture dye workers in our city center. One-hundred-forty-two workers in Elazig Central Industrial Centre (86 auto, 56 furniture dyes) were included in the study. The workers were questioned with "Turkish Thorax Association Occupational and Environmental Pulmonary Diseases Evaluation Form", and physical examination and pulmonary function tests (PFT) were performed. Follow of peak expiratory flow rate (PEFR) and reversibility tests were performed to patients who had complaints or abnormality in PFT. The workers who had reversibility and positive daily PEFR variability were taken away from work and PEFR variables were followed. The workers had no symptoms when they were taken away from work and daily PEFR were below than 20%, accepted as OA. Twenty-one workers of 22 workers who have abnormal questionnaire, symptoms and abnormality in PFT, accepted daily PEFR measurements. Daily PEFR variability and reversibility test were positive in 5 (3.52%) workers who were accepted as OA. We detected the prevalence of OA, an important worker health problem, was 3.52% in auto and furniture dyes in industrial centre of our city. We think that the prevalence of OA can be determined with detailed history, serial PEFR follow and using PFT, in the absence of specific bronchial provocative tests. Thus, it is important to be become conscious about OA the groups who are under risk.
