Background: Broad-spectrum empirical antimicrobials are frequently prescribed for patients with coronavirus disease 2019 (COVID-19) despite the lack of evidence for bacterial coinfection. Aims: We aimed to cross-sectionally determine the frequency of antibiotics use, type of antibiotics prescribed, and the factors influencing antibiotics use in hospitalized patients with COVID-19 confirmed by polymerase chain reaction. Study Design: The study was a national, multicenter, retrospective, and single-day point prevalence study. Methods: This was a national, multicenter, retrospective, and single-day point-prevalence study, conducted in the 24-h period between 00:00 and 24:00 on November 18, 2020, during the start of the second COVID-19 peak in Turkey. Results: A total of 1500 patients hospitalized with a diagnosis of COVID-19 were included in the study. The mean age ± standard deviation of the patients was 65.0 ± 15.5, and 56.2% (n = 843) of these patients were men. Of these hospitalized patients, 11.9% (n = 178) were undergoing invasive mechanical ventilation or ECMO. It was observed that 1118 (74.5%) patients were receiving antibiotics, of which 416 (37.2%) were prescribed a combination of antibiotics. In total, 71.2% of the patients had neither a clinical diagnosis nor microbiological evidence for prescribing antibiotics. In the multivariate logistic regression analysis, hospitalization in a state hospital (p < 0.001), requiring any supplemental oxygen (p = 0.005), presence of moderate/diffuse lung involvement (p < 0.001), C-reactive protein > 10 ULT coefficient (p < 0.001), lymphocyte count < 800 (p = 0.007), and clinical diagnosis and/or confirmation by culture (p < 0.001) were found to be independent factors associated with increased antibiotic use. Conclusion: The necessity of empirical antibiotics use in patients with COVID-19 should be reconsidered according to their clinical, imaging, and laboratory findings.

Aim: Despite effective HBV vaccine and vaccination programs, Hepatitis B still causes nearly one million deaths annually, according to the World Health Organization. The immunization rates are high by hepatitis B vaccination especially in young healthy population. However, advanced age, comorbidities and immune deficiency affect the rates of vaccine response negatively. We aimed to evaluate the factors affecting the vaccine response in the patients who received hepatitis B vaccine in our hospital retrospectively.Materials and Methods: Patients who received hepatitis B vaccine and followed-up for immunization status were evaluated retrospectively in our vaccination unit. The patients were examined in terms of age, comorbidities, vaccine scheme, vaccine dose, utilization of immunosuppressive-biological agents, immunization success, and common characteristics of patients who did not develop immunity.Results: Anti-HBs titers were significantly lower in patients over 50 years old. In addition, lower Anti-Hbs titers were detected in patients with comorbidity and who received immunosuppressive therapy. Statistically higher Anti-HBs titers were obtained with double dose vaccination than single dose vaccination in patients receiving immunosuppressive treatment. Moreover, it was documented that a single dose vaccination protocol was applied to all seven patients who failed vaccination. Six of these patients were over 50 years old, two were diabetic, five were diagnosed with rheumatic disease and all of them were using biological agents.Conclusion: Vaccination before treatment in patients who will receive immunosuppressive therapy, administration of high dose vaccines in patients under immunosuppressive therapy or delaying immunosuppressive therapy during vaccination may be considered as appropriate alternatives. Considering the safety and cost-effectiveness of high-dose hepatitis B vaccine, it should be preferred in the immunosuppressive patient group.

Introduction: The human population is aging at an astonishing rate. The aim of this study is to capture a situation snapshot revealing the proportion of individuals aged 65 years and over among inpatients in healthcare institutions in Turkey and the prevalence and type of infections in this patient group in order to draw a road map. Materials and Methods: Hospitalized patients over 65 years at any of the 62 hospitals in 29 cities across Turkey on February 9, 2017 were included in the study. Web-based SurveyMonkey was used for data recording and evaluation system. Results: Of 17,351 patients 5871 (33.8%) were ≥65 years old. The mean age was 75.1 ± 7.2 years; 3075 (52.4%) patients were male. Infection was reason for admission for 1556 (26.5%) patients. Pneumonia was the most common infection. The median length of hospital stay was 5 days (IQR: 2–11 days). The Antibiotic therapy was initiated for 2917 (49.7%) patients at the time of admission, and 23% of the antibiotics prescribed were inappropriate. Healthcare-associated infections developed in 1059 (18%) patients. Urinary catheters were placed in 2388 (40.7%) patients with 7.5% invalid indication. Conclusion: This study used real data to reveal the proportion of elderly patients in hospital admissions. The interventions done, infections developed during hospitalization, length of hospital stay, and excessive drug load emphasize the significant impact on health costs and illustrate the importance of preventive medicine in this group of patients.

Introduction: In this study, it was aimed to obtain information about the perspectives of infectious diseases and clinical microbiology (IDCM) specialists on online education applications and web-based seminars, and their usage and utilization rates. Materials and Methods: In this descriptive, cross-sectional study, online survey responses of the IDCM specialists in who were members of Infectious Diseases Clinical Microbiology Specialty Society of Turkey (EKMUD) were evaluated. Results: A total of 184 IDCM specialists answered the survey questions. Of the participants, 77.2% thought that the pandemic disrupted post-graduate medical education and 84.8% thought that the pandemic disrupted pre-graduate medical education. Medical educators involved in education during the Coronavirus disease-2019 (COVID-19) pandemic reported that they mostly continued online education. Of the participants, 84.9% reported that they were able to use the internet in an effective way to gain access to information during the COVID-19 pandemic. Of the responders, 58.2% reported that they followed online training programs, such as web-based seminars and lectures, and 51.6% reported that these online programs contributed to their learning. Participants found online training programs such as web-based seminars and lectures useful in terms of learning competence, that online training programs provided people with gain in space and time, and that they found them more instructive because they gave them the chance to receive training in subjects of their interest. Participants reported that these programs provided learning competency in that these programs were more beneficial to the participants in view of place, the subject that they need/they are interested in and saving time for the trainer and the participants. Due to the COVID-19 pandemic, the participants mostly wanted the 2020 EKMUD congress to be held online. A total 92.9% of the participants reported that online congresses and seminars would decrease travel, participation and sponsorship costs. Only 24.5% of the participants considered that online congresses and seminars would be more effective in terms of social interaction and gaining information than face-to-face congresses. Conclusion: The IDCM specialists reported that online educational practices such as webinars were benefical. The rate of participation in webinars and online education practices were high during the COVID-19 pandemic.

Our retrospective study evaluates the effectiveness of steroid treatment and the factors affecting treatment in patients with severe Coronavirus disease-2019 (COVID-19) pneumonia that received favipiravir treatment. Introduction: In this study, it was aimed to determine the problems encountered by Infectious Diseases and Clinical Microbiology residents during residency training in order to identify solutions to these difficulties. Materials and Methods: A survey was conducted between December 19 and 31, 2018 using a 46-item questionnaire. Participants were able to participate in the survey anonymously via the website of the Infectious Diseases and Clinical Microbiology Specialty Society of Turkey. Residents were informed via text message and e-mail to encourage participation. The survey included questions about current problems with the physical conditions, access to scientific information resources, the quality of residency education, examinations, night shifts, mobbing, rotations, and the possibility of following up special patient subgroups. Results: A total of 134 residents completed the questionnaire, 73 of whom were in university educational hospitals and 61 were in training and research hospitals operating under the Ministry of Health. Around 68% of the participants stated that they were working in unsuitable physical conditions and 33% had no access to medical information resources outside of the hospital. Ninety-four participants (61%) declared that their clinic had a regular training program which was renewed annually, but only 36% of these asserted that it was adequate for their training. When asked about the frequency of coworking with faculty members for scientific data/study production, 46% of the respondents answered once a year or less. Fifty-nine participants (44%) had seven or more night shifts per month. Thirty-six percent of the participants reported that they were subject to workplace bullying, which they said was perpetrated most frequently by faculty members (36%). Only 22 participants (16%) considered the microbiology training was adequate and 15% rated the training received during rotations as adequate. Thirty-three of the residents had performed a liver biopsy or been trained in this procedure. Fifty-five participants (46%) had never worked at a vaccine outpatient clinic. According to the trainees, the most important problem was inadequate education (28%) and 55% declared that they would not choose the same specialty if they entered the national medical specialty exam again. Conclusion: Residents describe many problems during their specialty training, such as inadequate education, excessive night shifts, increased workload, and workplace bullying. These problems should be presented more frequently with valid scientific data within the professional societies, and in light of this information, physician working groups should be formed to develop solutions.

Introduction: In this study, we aimed to determine the risk factors associated with inadequate empirical antibiotherapy (IEAT) and hospital-related mortality in elderly patients being treated for upper urinary tract infections (UTI). Materials and Methods: This study included individuals aged 65 years and over who were hospitalized after being diagnosed of communityacquired UTI or community-onset healthcare-associated UTI and followed-up in clinics and/or intensive care units (ICU) of 33 hospitals between March and September 2017. Results: A total of 525 patients (48% males; mean age: 76.46±7.93 years) were included in the study. Overall, 68.2% of the patients were hospitalized through the emergency department and 73.9% of patients were followed-up for pyelonephritis. Gram-negative, Gram-positive, and Gram-negative and positive mix growths were determined in 88%, 9.3%, and 2.7% of urine cultures, respectively. Fifty-six (10.7%) of the patients died. In multivariate analysis, the presence of chronic obstructive pulmonary disease [Odds ratio (OR): 2.278], age 85 years and over (OR: 2.816), admission to the ICU (OR: 14.831), and IEAT (OR: 2.364) were independent factors that significantly affected mortality. Guidelines include recommendations of experts from various specialties within a topic in consideration of data specific to each country. However, to date there has not been a guideline standardizing the nomenclature and offering recommendations for intra-abdominal infections (IAIs) in Turkey. This is mainly due to the paucity of laboratory studies regarding the clinical diagnosis and treatment of IAIs or the sensitivity of microorganisms isolated from patients with IAIs. However, due to the diversification of host characteristics and advancements in technological treatment methods, it has become imperative to “speak a common language”. For this purpose, meetings were formed under the leadership of the Infectious Diseases and Clinical Microbiology Specialty Society of Turkey (EKMUD) consisting of 15 experts in IAIs from the Turkish Surgical Association, Turkish Society of Colon, Turkish Hernia Society, Turkish Society of Hepato-Pancreato-Biliary Surgery, and the Turkish Society of Hospital Infections and Control; and relevant studies were analyzed.. Ultimately, the suggestions for adults found in this consensus report were prepared using available data from Turkey, and referring predominantly to the 2010 guidelines for diagnosing and managing complicated IAIs in adults and children by the Infectious Diseases Society of America and the Surgical Infection Society. Recommendations are conducted in two sections from the initial evaluation of patients for diagnosis to treatment approach for IAI. This Consensus Report was presented in Turkey EKMUD 2016 Congress/Antalya and subsequently opened for any suggestions in Turkey EKMUD and Turkish Surgical Association official web sites for a month. The manuscript was updated according to the suggestions.

Introduction: The aim of this study was to determine the causative agents of community-acquired lower urinary tract infections (CALUTIs) in primary care. We also aimed to evaluate the antimicrobial susceptibility rates of urinary Escherichia coli isolates to various oral antibiotics and to assist primary care physicians with antibiotic selection. Materials and Methods: The study was performed in 55 primary care centers in 13 cities between May and July 2009. Adult patients with at least two of dysuria, pollakiuria, nocturia, suprapubic tenderness, or blurred urine symptoms, but not fever, were included in the study. Urinary samples were obtained and patient data were recorded at the primary care centers. Results: Totally, 400 patients were enrolled. In 175 (43.8%) patients, urine cultures yielded a urinary pathogen. The most frequently encountered pathogen was E. coli (62.8%). Among E. coli isolates, the lowest resistance rates were detected for nitrofurantoin (0.9%) and fosfomycin (3.6%) and the highest for trimethoprim/sulfamethoxazole (43.6%) and amoxicillin/clavulanate (41%). Resistance rates to quinolones were 23.6% for ciprofloxacin and 21% for levofloxacin. Minimum inhibitory concentration (MIC)50 and MIC90 values for ciprofloxacin and levofloxacin were 0.015 and 32 μg/mL and 0.06 and 16 μg/mL, respectively. Quinolone resistance was significantly higher in patients who received an antimicrobial treatment within the last three months (p< 0.001). Extended spectrum beta-lactamase (ESBL) positivity was detected in 15 of 110 (13.6%) E. coli isolates. Quinolone resistance was significantly higher among ESBL positive than ESBL negative strains (53.3% vs. 15.8%, p= 0.002). Conclusion: In conclusion, the most frequent causative agent in CALUTIs was E. coli. The lowest resistance rates among E. coli isolates were detected for nitrofurantoin and fosfomycin. Resistance rates to quinolones were over 20% in our study. Our identifikasyostudy provides important data about the causative agents and their antibiotic susceptibilities and also contains valuable data for rational antibiotic usage in the treatment of CALUTI in Turkey.

Background: Blood culture-negative endocarditis (BCNE) is a diagnostic challenge, therefore our objective was to pinpoint high-risk cohorts for BCNE. Methods: The study included adult patients with definite endocarditis. Data were collected via the Infectious Diseases International Research Initiative (ID-IRI). The study analysing one of the largest case series ever reported was conducted across 41 centers in 13 countries. We analysed the database to determine the predictors of BCNE using univariate and logistic regression analyses. Results: Blood cultures were negative in 101 (11.65 %) of 867 patients. Abstract Background Tuberculous meningitis (TBM) represents a diagnostic and management challenge to clinicians. The “Thwaites’ system” and “Lancet consensus scoring system” are utilized to differentiate TBM from bacterial meningitis but their utility in subacute and chronic meningitis where TBM is an important consideration is unknown. Methods A multicenter retrospective study of adults with subacute and chronic meningitis, defined by symptoms greater than 5 days and less than 30 days for subacute meningitis (SAM) and greater than 30 days for chronic meningitis (CM). The “Thwaites’ system” and “Lancet consensus scoring system” scores and the diagnostic accuracy by sensitivity, specificity, and area under the curve of receiver operating curve (AUC-ROC) were calculated. The “Thwaites’ system” and “Lancet consensus scoring system” suggest a high probability of TBM with scores ≤4, and with scores of ≥12, respectively. Abstract Background An incisional hernia is a common complication following abdominal surgery. Polypropylene mesh is frequently used in the repair of such defects and has nearly become the standard surgical treatment modality. Though they are very effective in reducing recurrence, mesh materials exhibit a strong stimulating effect for intraabdominal adhesion. The thymoquinone (TQ) extracted from Nigella sativa seeds has potential medical properties. TQ has anti-inflammatory, antioxidant and antibacterial properties. The aim of this study is to coat polypropylene mesh with TQ in order to investigate the effect of surface modification on intraabdominal adhesions. Methods TQ-coated polypropylene mesh material was tested for cytotoxicity, contact angle, surface spectroscopy, TQ content, sterility, and electron microscopic surface properties. An experimental incisional hernia model was created in study groups, each consisting of 12 female Wistar rats. The defect was closed with uncoated mesh in control group, with polylactic acid (PLA) coated mesh and PLA-TQ coated mesh in study groups. Adhesion scores and histopathologic properties were evaluated after sacrifice on postoperative 21th day.
