Objective: Immune thrombocytopenia is an autoimmune disease characterised by decreased platelet count, which can cause increased bleeding risk. In our study, it was aimed to compare patients with immune thrombocytopenia according to their age groupsMaterial and Methods: Patients diagnosed with immune thrombocytopenia between September 2014 and January 2019 were divided into groups as 3-24 months, 2-10 years and 10-18 years. The groups were evaluated statistically according to demographic features, laboratory data, treatment modalities, platelet counts 12 months after treatment, resistance to treatment and chronicization states.Results: In this study, 95 of 104 patients diagnosed with ITP were included. There were 28 patients in the group between 3-24 months, 41 patients in the group between 2-10 years old and 26 patients in the group between 10-18 years old in the study. Patients were treated with IVIG, steroids and IVIG combined with steroids. It was observed that the frequency of steroid treatment alone increased with age (p=0.030). The number of chronic patients was 37. ÖZET ABSTRACT Amaç: Çocuk Acil Servisi’ne zehirlenme nedeniyle başvuran hastaların demografik özelliklerini inceleyip alınabilecek önlemleri belirlemektir. Gereç ve yöntem: Çalışmada Ocak 2013- Haziran 2017 tarihleri arasında Kahramanmaraş Sütçü İmam Üniversitesi Sağlık Uygulama ve Araştırma Hastanesi Çocuk Acil Servisi’ne zehirlenme nedeniyle başvuran 300 hastanın bilgileri dosyalarından incelenmiştir. Hastalar yaş, cinsiyet, zehirlenme etkeni, zehirlenme saati gibi parametrelere göre sınıflandırılmıştır. Bulgular: Çalışmaya önceden belirlenen kriterlere göre 300 hasta alınmıştır. Zehirlenme vakaları tüm acile başvuruların % 0.37’sini oluşturmaktadır. Olguların 146 (% 48,7)’si kız, 154 (% 51,3)’ü ise erkektir. Tüm zehirlenme olguları arasında cinsiyet açısından fark saptanmamıştır. Olguların 239 (%79,67)’si kaza sonucu, 61 (% 20,33)’ü ise intihar amaçlı zehirlenmelerdir. Tüm zehirlenme olguları arasında kaza-intihar açısından fark saptanmıştır. Vakalar zehirlenme etkenine göre farmakolojik ve non-farmakolojik olarak iki gruba ayrılmıştır. 90 hasta (%30) farmakolojik ajanla, 205 hasta (%68,3) non-farmakolojik bir ajanla zehirlenmiştir. 5 vakada (%1,7) zehirlenme etkeni saptanamamıştır. Zehirlenmeye neden olan farmakolojik ajanlardan ilk 3 sırada; Parasetamol (%4.0), çoklu ilaç alımı (%3.3), SSRI grubu antidepresan ilaçlar ve (%2.7) yer almaktadır. Non-farmokolojik ajanlardan en sık görülen 3’ü sırasıyla karbonmonoksit (%27.2), kostik-koroziv maddeler (20.3), Maraş otu (%4) şeklinde sıralanmaktadır. Sonuç: Çocuklarda farmakolojik ve non-farmakolojik ajanlar tek tek incelendiğinde zehirlenmelerin en sık nedeninin ilaçlar olduğu ortaya çıkmıştır. Ailelerin bu konudaki farkındalıklarının artırılması ve önleyici tedbirlerin alınmasının zehirlenmeleri azaltacağı düşünülmektedir. Objectives During the COVID-19 pandemic, healthcare professionals are recommended to use PPE to prevent the transmission of disease. Healthcare workers who use N95 FFR, which has an important place, experience complaints such as headache and dizziness. In this study, we plan to find the cause of these complaints and aim to clarify whether they are associated with the use of N95 mask. Method Healthcare workers first put on a surgical mask for at least 1 h and a maximum of 4 h, this process was then repeated on another day with the same workers wearing N95 masks. After removing the mask, capillary blood gases were taken and a questionnaire was given. Pulmonary embolism is rarely diagnosed in pediatric burn patients and is often only discovered at autopsy. Herein we would like to present the case of a pediatric patient with pulmonary thromboembolism who was burnt by hot water at home. A 4-year-old girl who was treated at an external center for 9 days due to second degree burn injuries in both legs. After the discharge she had suddenly become restless and than gone unconscious. Since pulmonary thromboembolism was suspected as the cause of cardiopulmonary arrest, a pulmonary computed tomography angiogram was taken, which showed a thrombus in the lobar and segmental branches of the right pulmonary artery supplying the right lower lobe. Enoxaparin sodium treatment (2×1mg/kg) was initiated. At day 16, a control computed tomography angiogram showed that the thrombus in the lobar and segmental branches of the pulmonary artery had disappeared. This case report illustrates the importance of chemoprophylaxis against pulmonary embolism in both pediatric and adult burn patients. In such cases, especially the severe ones, a sudden deterioration of patient’s clinical status in the form of respiratory distress or failure should raise suspicion about the possibility of pulmonary embolism.

Objective: Acute gastroenteritis (AGE) is the frequently seen infections of childhood. The aim of our study is observing the clinical and laboratory parameters of patients hospitalized with acute gastroenteritis. Material and methods: Totally, 130 pediatric patients hospitalized at our clinic between January 2016 and January 2019 with a diagnosis of AGE whose ages were between 0-17, diarrheas continued shorter than 14 days with were included in our study. The patients were separated into two groups according to the agents such as viral and bacterial AGE. Amaç: Port kateterleri çocuk hematoloi-onkoloji hastalarında ilaç uygulamaları için kolay bir vasküler erişim yöntemi sağlamaktadır. Bu retrospektif çalışma ile çocuk hastalarda uyguladığımız venöz port kateter uygulamalarının literatür eşliğinde irdelenmesi amaçlanmıştır. Gereç ve Yöntemler: Port kateter uygulaması için ilk tercih sağ internal juguler ven idi. Bu bölge kullanılamadığında kontrlateral internal juguler ven tercih edildi. İşlem ultrasonografi eşliğinde genel anestezi altında ameliyat odasında gerçekleştirildi. Tüm hastalar işlemden sonra pnömotoraks, kateter oryantasyonu, bükülmesi ve malpozisyon i göğüs grafisi ile açısından değerlendirildi. Bulgular: 11.02.2016 - 13.12.2019 tarihleri arasında toplam 26 pediatrik hastaya vasküler port kateteri yerleştirdik. Port kateter uygulaması için endikasyon uzun süreli kemoterapi uygulaması idi. Ortalama yaşı 6.96 (1 ile 16 arasında değişmektedir) olup 20 (% 76.9) erkek ve 6 (% 23.1) kız çocukdan oluşmaktaydı. Port kateter yeri olarak 23 çocukda sağ internal jugular ven, 3 çocukda sol internal jugular ven tercih edildi. 2 (%7.7) hastada port enfeksiyonu, 3 (%11.5) hastada port trombozu nedeniyle port çıkarıldı. Hiçbir hastada hemotoraks, pnömotoraks, ven trombozu gelişmedi. Sonuç: Port kateterini USG eşliğinde daha düşük komplikasyon oranları ile uygulamak mümkün olmaktadır. Ayrıca bu uygulama ile çocuk hastaların ve ailelerinin tedaviye uyumu ve günlük yaşam kalitesinin artacağı kanaatindeyiz.

Purpose: To evaluate the clinical features of pediatric patients with acute-onset, unilateral transient acquired blepharoptosis. Methods: In this retrospective study, the clinical records of patients between April 2015 and June 2020 were reviewed for evaluation of demographic features, accompanying neurological and ophthalmologic manifestations, symptom duration, etiological cause, and imaging findings. Patients with congenital and acquired blepharoptosis with chronic etiologies were excluded. Results: Sixteen pediatric patients (10 boys and 6 girls) with acquired acute-onset unilateral transient blepharoptosis were included in this study. The patients’ mean age was 6.93 ± 3.16 years. The most commonly identified etiological cause was trauma in 7 patients (43.75%) and infection (para-infection) in 5 patients (31.25%). In addition, Miller Fisher syndrome, Horner syndrome secondary to neuroblastoma, acquired Brown’s syndrome, and pseudotumor cerebri were identified as etiological causes in one patient each. Additional ocular findings accompanied blepharoptosis in 7 patients (58.33%). Blepharoptosis spontaneously resolved, without treatment, in all the patients, except those with Miller Fisher syndrome, neuroblastoma, and pseudotumor cerebri. None of the patients required surgical treatment and had ocular morbidities such as amblyopia. Conclusion: This study demonstrated that acute-onset unilateral transient blepharoptosis, which is rare in childhood, may regress without the need for surgical treatment in the pediatric population. However, serious pathologies that require treatment may present with blepharoptosis. RESUMO Objetivo: Avaliar as características clínicas de pacientes pediátricos com blefaroptose adquirida unilateral, transitória e de início agudo. Métodos: Neste estudo retrospectivo, foram revisados prontuários clínicos entre abril de 2015 e junho de 2020. Os pacientes foram avaliados em termos de características demográficas, manifestações neurológicas e oftalmológicas associadas, duração dos sintomas, etiologia e achados de imagem. Foram excluídos pacientes com blefaroptose congênita e com blefaroptose adquirida de etiologia crônica. Resultados: Foram incluídos neste estudo 16 pacientes pediátricos (10 masculinos e 6 femininos) com blefaroptose adquirida transitória unilateral de início agudo. A média de idade dos pacientes foi de 6,93 ± 3,16 anos. As causas etiológicas mais comumente identificadas foram trauma em 7 pacientes (43,75%) e infecção (casos parainfecciosos) em 5 pacientes (31,25%). Além disso, a síndrome de Miller-Fisher, a síndrome de Horner secundária a neuroblastoma, a síndrome de Brown adquirida e pseudotumor cerebral foram determinados como causas etiológicas em um paciente cada uma. Achados oculares adicionais estavam associados à blefaroptose em 7 pacientes (58,33%). Foi observada a resolução espontânea da blefaroptose, sem tratamento, em todos os pacientes, exceto nos pacientes com síndrome de Miller-Fisher, neuroblastoma e pseudotumor cerebral. Nenhum paciente precisou de tratamento cirúrgico. Morbidades oculares, como ambliopia, não foram encontradas em nenhum paciente. Conclusão: Este estudo demonstrou que a blefaroptose transitória unilateral de início agudo, rara na infância, pode regredir sem a necessidade de tratamento cirúrgico na população pediátrica. No entanto, também não deve ser esquecido que patologias graves que requerem tratamento podem se apresentar com blefaroptose.

Aim: A thrombosis and inflammation marker, Signal peptide-CUB (complement C1r/C1 s)-EGF (epidermal growth factor)-like domain-containing protein 1 (SCUBE-1) is expressed in endothelial cells and activated platelets. We aimed to investigate the levels of this marker in patients with Thalassemia major (TM), a disease which causes a tendency to thrombosis.Methods: Twenty patients with TM and age and gender-matched twenty healthy controls were included in this prospective case-control study. Other thrombosis markers such as D-dimer and homocysteine were studied in addition to SCUBE-1 in both groups.Results: The SCUBE1 and D-dimer levels were higher in the TM group compared to the control group (P=0.004, P=0.023, respectively). A significant positive correlation was detected between platelet count and SCUBE1 (r=0.342, P=0.031).Conclusion: We detected elevated SCUBE1 levels in children with TM compared to the healthy controls, which correlated with platelet count. SCUBE1 may be used as a thrombosis marker in TM in the early stages of the disease., Amaç: Sinyal peptidi-CUB (tamamlayıcı C1r / C1s) -EGF (epidermal büyüme faktörü) benzeri alan içeren protein 1 (SCUBE-1), endotel hücreleri ve aktive olmuş trombositlerde eksprese edilen tromboz ve inflamasyon belirteci olarak kullanılan bir proteindir. Tromboza eğilim yaratan bir hastalık olan talasemi majörde bu belirtecin düzeylerini incelemeyi amaçladık.Yöntemler: Bu kesitsel vaka-kontrol çalışmasına SCUBE-1 düzeylerini belirleyebilmek için 20 talasemi majorlü hasta ve kontrol grubu olarak da benzer yaş ve cinsiyette 20 sağlıklı çocuk dahil edildi. D-dimer, homosistein gibi diğer tromboz belirteçleri SCUBE-1 ek olarak her iki grupta çalışıldı.Bulgular: Gruplar arasında SCUBE-1 ve D-dimer düzeyleri açısından anlamlı fark vardı (P=0,004, P=0,023, sırasıyla). Trombosit sayısı ile SCUBE-1 arasında anlamlı pozitif korelasyon tespit edildi (r=0,342, P=0,031).Sonuç: SCUBE-1 seviyelerinin talasemi majorlü çocuk hastalarda sağlıklılara nazaran yüksek olduğunu ve trombosit sayısı ile korele olduğunu tespit ettik. SCUBE-1 belki erken evrelerde tromboz belirteci olarak kullanılabilir.

Amaç: Kanamabozuklukları arasında fibrinojen (Faktör I), Faktör II, Faktör V, kombine FaktörV ve Faktör VIII, Faktör VII, Faktör X, Faktör XI, Faktör XII ve Faktör XIIIeksiklikleri, dünya genelinde nadir görülür. Tüm faktör eksikliklerinin%3-5’ini oluşturur. Nadir faktör eksikliği olan hastalarda heterojen kliniktablo görülür. Hafif ya da orta dereceli kanamadan ciddi ve hayatı tehdit edenkanamalara kadar farklı semptomlar görülebilir. Bu çalışmada hastanemizde yenitanı alan nadir faktör eksikliği olan hastaların değerlendirilmesi amaçlandı.Gereç ve Yöntemler: Ocak 2014 ve Ocak 2019yılları arasında Kahramanmaraş Sütçü İmam Üniversitesi Tıp Fakültesi ÇocukHematoloji ve Onkoloji Bölümü’nde tanı alan nadir faktör eksikliği olan 18 yaşaltı hastalar incelendi. Bulgular: Nadir faktör eksikliği olan 17hastanın 9’u (%52,9) erkek, 8’i (%47,1) kızdı. Hastaların ortalama tanı yaşı 7,6±4,3 yıl (0-14 yıl) güncelyaşı 9,5±4,6 yıl (1-16 yıl) ve faktör düzeyi %9,1±11,1 olarak bulundu. Hastaların8’inde Faktör VII, 3’ünde Faktör XIII, 2’sinde Faktör V, 2’sinde Faktör XII,1’inde Faktör V ve VIII ve 1’inde Faktör I eksikliği tanısı konuldu. On yedihastanın 9’unda kanama vardı. İkisi diş eti, 2’si burun, 1’i göbek, 1’iiliopsoas, 1’i gastrointestinal sistem, 1’i sünnet sonrası kanama ve 1’i topukkanı alınırken kanamanın durmaması şeklindeydi. Ciddi kanama olan 5 hastanın3’ü Faktör XIII, 1’i Faktör VII ve 1’i Faktör I eksikliği tanılıydı. Faktör Ieksikliği olan kız hasta 10 günlükken topuk kanı alımı sırasında kanamanındurmaması nedeniyle tanı aldı. En küçük yaşta tanı konulan kız hasta 6günlükken ciddi göbek kanaması nedeniyle hastanemize başvurdu ve Faktör XIIIeksikliği tanısı konuldu. Faktör XIII eksikliği tanısı olan bir kız hastabisikletten düşme nedeniyle oluşan iliopsoas kanaması ve sonrasında gelişenintrakranial kanama nedeniyle 8 yaşında eksitus oldu. Sonuç: Kanama şikâyeti olan ya daolmayan hastalarda nadir faktör eksikliği tanısı ayırıcı tanıda olmalıdır. Buhastaların erken dönemde tanısı konularak, ciddi kanamaların önlenmesi vetedavisi önemlidir., Objective:Hemorrhagic disorders include fibrinogen (FactorI), Factor II, Factor V, combined Factor V, and Factor VIII, Factor VII, FactorX, Factor XI, Factor XII, and Factor XIII deficiencies. It constitutes 3-5% ofall factor deficiencies. A heterogeneous clinical picture is seen in patientswith rare factor deficiency. From mild to moderate bleeding to severe andlife-threatening bleeding, different symptoms may occur. The aim of this studywas to evaluate the patients with newly diagnosed rare factors in our hospital. Material and Methods: Patients under 18 years of age with a rare factor deficiency diagnosed in the Department of Pediatric Hematology and Oncology, Faculty of Medicine, Kahramanmaraş Sütçü İmam University between January 2014 and January 2019 were examined. Results: Of the 17 patients with rare factor deficiency, 9 (52.9%) were male and 8 (47.1%) were female. The mean age at diagnosis was 7.6 ± 4.3 years (0-14 years), the mean age was 9.5 ± 4.6 years (1-16 years) and factor level 9,1 ± 11,1%. Factor VII was diagnosed in 8 patients, Vactor XIII in 3 patients, Vactor V in 2 patients, Factor XII in 2 patients, Factor V and VIII in 1 patient and factor I deficiency in 1 patient. 9 out of 17 patients had bleeding. Two had gums, 2 had nose, 1 had umbilical cord, 1 had iliopsoas, 1 had gastrointestinal system, 1 had hemorrhage after circumcision and 1 had heel blood while bleeding did not stop. Of the 5 patients with severe bleeding, 3 had Factor XIII, 1 had Factor VII and 1 had Factor I deficiency. The girl patient with Factor I deficiency was diagnosed at the age of 10 days because he did not stop bleeding during heel blood intake. The girl patient, who was diagnosed at the young age, was admitted to our hospital with severe belly bleeding at the age of 6 days and was diagnosed as Factor XIII deficiency. A girl patient with Factor XIII deficiency died at the age of 8 due to iliopsoas hemorrhage due to falling off the bicycle and after occurring intracranial hemorrhage. Conclucion: The diagnosis of rare factor deficiency should be in the differential diagnosis in patients with or without bleeding complaints. Early diagnosis of these patients is important and prevention and treatment of serious bleeding are important.

Objective Myocarditis is an inflammatory disease of the cardiac muscle. Prognosis is most often good but, in some patients, the disease can be fulminant. Our aim with this study was to determine interleukin-9 (IL-9) and interleukin-17 (IL-17) levels in myocarditis cases with different etiologies. Materials and methods Thirty one patients with myocarditis and 30 healthy controls of similar age and gender without a history of chronic disease were included in the study. All 31 patients were clinically myocarditis. In some of these patients, the cause of myocarditis is acute rheumatic fever or Kawasaki disease. Serum samples of the patients were taken during diagnosis in order to analyze serum IL-9 and IL-17 levels and sedimentation rate, CRP, ASO, pro-BNP, CK-MB, and Troponin-I tests were performed. Results It was found that IL-17 levels were statistically significant in all acute rheumatic fever, Myocarditis and Kawasaki patients compared to the control group (p = 0.001) and that cut-off was 4.30 pg/mL. This value was determined to be 71% sensitive and 67% specific for IL-17 (AUC = 0.761). Conclusions Both of the mean and median levels of IL-17 were significantly higher in pediatric patients with myocarditis than in healthy children. Our study made us think that complications of myocarditis and associated morbidity can be prevented by IL17 inhibitors. The high levels of IL17 found in our study may be a reference for future study.

Leukocyte adhesion deficiency type II (LAD II) is a rare, autosomal, recessive inherited immunodeficiency disease that induces frequent and recurrent infections, persistent leukocytosis, severe mental and growth retardation, and impaired wound healing. The Bombay blood group is a rare blood group phenotype that is characterised by the deficiency of H, A, and B antigens on the surface of red cells. LAD II and the Bombay blood group are always seen together, because both of them are associated with a global defect in the common pathway of fucose metabolism. Here we report the case of an 11-year-old boy with LAD II, who presented with the Bombay blood group. Agglutination with strength of 4+ was detected in all cross-matching due to erythrocyte transfusions for our patient. Therefore, the Bombay blood group was incidentally determined due to deficient expression of the CD15 adhesion molecules on the surface of the leukocytes according to the results of flow cytometry. Upon detecting the Bombay blood type, LAD II was then diagnosed as a result of flow cytometry and the clinical findings of mental retardation and history of recurrent infections such as abscesses.

Amaç: Bu çalışma Suriyeli mülteci çocuklarda anemi prevalansını ve etiyolojisini araştırmak amacıyla planlanmıştır.Gereç ve Yöntemler: Bu retrospektif çalışmaya, Ocak 2015-Eylül 2018 tarihleri arasında Kahramanmaraş Sütçü İmam Üniversitesi (KSÜ) Pediatrik Hematoloji ve Onkoloji polikliniğine başvuran, herhangi bir kronik hastalığı olmayan 6 ay-18 yaş arası 266 Suriyeli mülteci çocuk dahil edildi. Hastalar yaşlarına göre6 ay-59 ay, 5-11 yaş, 12-14 yaş ve 15-18 yaş olmak üzere 4 gruba ayrıldı. Background and Aims Fatty liver increases oxidative stress and may trigger antioxidant mechanisms. We aimed to compare the levels of vitamin D, which has antioxidant properties, as well as total oxidant status (TOS), total antioxidant status (TAS), and catalase between patients with nonalcoholic fatty liver (NAFL) and the control group. Methods We compared vitamin D, TOS, TAS, catalase levels, and other biochemical parameters between pediatric patients with ultrasonographically detected NAFL and an age-matched healthy control group. Results NAFL patients had a significantly lower vitamin D level (p < 0.001). The patient group also had significantly greater height, weight, body mass index (BMI) Z score, parathyroid hormone, triglyceride, glucose, antioxidant (TAS and catalase), and TOS levels compared to the controls (p ≥ 0.001). There was no significant difference between the obese and non-obese NAFL patients with respect to TAS, TOS, catalase levels, and other biochemical parameters (p < 0.05). There was a positive correlation between height, weight, BMI Z score, and hepatosteatosis grade, and TAS, TOS, and catalase levels, and a negative correlation with vitamin D level. We also found a negative correlation between vitamin D level and TOS and catalase level. Conclusions Our study revealed lower levels of vitamin D and higher levels of oxidant-antioxidants including TOS, TAS, and catalase in patients with NAFL.

Bu çalışmaya 6-72 ay arasında ağır demir eksikliği anemisi olan 60 hasta ile benzer yaş ve cinsiyette 60 sağlıklı çocuk dahil edildi. A new type of coronavirus named as SARS-CoV-2 pandemic has begun to threaten human health. As with other types of coronaviruses, SARS-CoV-2 affects children less frequently, and it has been observed that the disease is mild. In the pathogenesis of a standard viral infection, the pathogen's contact with the mucosa is initially followed by an innate immunity response. T cells are the primary decisive element in adaptive immunity capability. For this reason, the adaptive immune response mediated by the thymus is a process that regulates the immune response responsible for preventing invasive damage from a virus. Regulatory T cells (T-reg) are active during the early periods of life and have precise roles in immunomodulation. The thymus is highly active in the intrauterine and neonatal period; it begins to shrink after birth and continues its activity until adolescence. The loss of T-reg function by age results in difficulty with the control of the immune response, increased inflammation as shown in coronavirus disease (COVID-19) as an inflammatory storm. Also, the thymus is typically able to replace the T cells destroyed by apoptosis caused by the virus. Thymus and T cells are the key factors of pathogenesis of SARS-CoV-2 in children.Conclusion: We speculated that thymus activity and T lymphocyte function in children protect them against the virus effects. Stimulating and preventing the inhibition of the thymus can be possible treatment components against COVID-19. What is Known: • The SARS-CoV-2 infection does not often progress with an invasive clinic in children. • Thymus activity and T lymphocyte functions are highly active in children. Ichthyosis which is an inherited keratinisation disorder of the skin, is heterogeneous as clinically. This disease’s clinic is considerably variable from widespread skin dryness to hyperkeratosis. While the prevention of complications such as dehydration, sepsis and electrolyte imbalance, is very important in respect to survival, in premature neonates, more severe complications may be seen. In this case, we reported that a premature infant who developed catastrophic candida sepsis with congenital ichthyosis.

INTRODUCTION: We aimed to compare the mean platelet volume (MPV) and plateletcrit (PCT) and vitamin-mineral levels in pediatric celiac disease patients with the healthy control group and to compare the results with the literature. METHODS: In this study, clinical and laboratory data of 80 pediatric patients diagnosed with celiac disease (CD) between July 2017 and December 2018 and 42 healthy children in the same age group were retrospectively analyzed. RESULTS: There was no significant difference between the groups in terms of age and gender (p=0.383, and p=0.462, respectively). Aim: Nutritional B12 deficiency is a treatable cause of neurodevelopmental delay in infants. We report 21 infants with developmental regression and brain atrophy as revealed using cranial magnetic resonance imaging (MRI), secondary to severe vitamin B12 deficiency. Methods: Twenty-one infants aged 4–24 months with B12 deficiencies who were admitted to our clinic between May 2013 and May 2018 were included in the study. MRI, bone marrow aspiration and the Denver-II Developmental Screening Test were performed in all infants. Results: The mean age of the infants was 12.3 months, and the mean B12 level was 70.15 ± 32.15 ng/L. Hypotonia and neurodevelopmental retardation, and anaemia were present in all patients. Their bone marrow examinations were compatible with megaloblastic anaemia. Twelve patients had microcephaly, seven had tremor and one patient died of severe sepsis. Almost all patients were fed with breast milk and their mothers were also malnourished. Nine (42.9%) of the patients were Turkish and 12 (57.1%) were Syrian. All patients had abnormal Denver-II Developmental Screening Test scores. Most patients had severe cortical atrophy, cerebral effusion, thinning of the corpus callosum and delayed myelinisation in cranial MRI. Treatment with B12 resulted in dramatic improvement in general activity and appetite within 72 h. Tremors resolved in all cases. Conclusion: Neurological findings and developmental delay related to nutritional B12 deficiency can be prevented without sequelae if diagnosed early. Ovarian tumors are the most common gynecological tumors seen in girls. Approximately 60-70% of them are germ cell tumors. Pseudo-Meigs syndrome is characterized by the presence of pelvic tumoral mass (benign or malign), pleural effusion, and massive acid. If the tumor is removed, acid and hydrothorax disappear. Endodermal sinus (yolk sac) tumor is a very rare cause in the diagnosis of Pseudo-Meigs syndrome, and only a few cases have been reported. This case is one of the rare cases presenting with Pseudo-Meigs syndrome and pathologically diagnosed as yolk sac tumor.

Dort (%19) hastada skolyoz gorulmustur. Olgularin 6’sinda (%28,5) ogrenme guclugu vardi. Sonuc : Norofibromatozis tip 1’in erken tanisi; ailelerin hastalik hakkinda bilgilendirilmesi ve bu cocuklarin duzenli klinik takibi ile tedavi edilebilir komplikasyonlari onlemek acisindan cok onemlidir.

Autoimmune hemolytic anemia is a disease characterized with destruction of erythrocytes as a result of antibody produce against patient’s own erythrocytes and anemia. Autoimmune hemolytic anemia can be roughly stratified into two groups according to serological features and secondary causes including drugs induced hemolytic anemia. Drugs induced autoimmune hemolytic anemia is very rare in pediatric patients. Even though hematological side effects such as leucopenia, agranulocytosis, eosinophilia, thrombocytopenic purpura and aplastic anemia might occur due to psychotropic drug use; to the best of our knowledge there is no autoimmune hemolytic anemia case due to quetiapine, an atypical antipsychotics, in literature. We hereby describe the first child case of autoimmune hemolytic anemia during quetiapine treatment.We also are pointing out that one should keep in mind serious hematological side effects with atypical antipsychotic drug use with this case report.

Disgerminom, dev boyutlara ulaşabilen overin en sık görülen malign tümörüdür. Genelde hormon salgılamamakla beraber beta-HCG salgılayabilirler. Bizde karında kitle ve karın ağrısı şikâyetleri olan radyolojik görüntüleme ile tespit edilen beta-HCG salgılayan dev boyutlara ulaşmış disgerminom tanılı 3 olguyu inceledik. Üç olguya da salpingo-ooferektomi ameliyatı yapıldı. Bir olguda komsu lenf nodlarından, peritondan ve karşı overden örnekleme yapıldı. Bir olgu da üç haftada bir dört kür BEP (Bleomisin, Etoposid, Sisplatin) kemoterapi protokolü ile fertilite de korunarak tedavi sağlanmış oldu. Bir olguda da 3 kür BEP kemoterpi protokülü verilmesi planlandı ve 2 kür BEP kemoterapi protokolü verildi.

Eosinophilic gastroenteritis (EGE) is a rare disorder and eosinophilic infiltration of the gastrointestinal tract is characteristic for it. Eosinophilic ascites may seen in lymphoma, eosinophilic gastroenteritis, peritoneal dialysis, and parasitic infections too. Depending on the dominant layer of infiltration EGE is classified into three types namely, mucosal, muscularis and serosal. Rarest type is the serosal type characterized by primary serosal disease, peripheral eosinophilia and eosinophilic ascites. Here we report a case of serosal EGE. We have discussed clinical cornerstones for diagnosis and response to treatment.A male patient age of 16 presented to the clinic with complaints of self-limiting diarrhea which stop within three weeks and abdominal distension. In physical examination tachypnea, orthopnea and marked abdominal distension were seen. Contrast-enhanced abdominal computed tomography showed the presence of ascites and diffuse thickening of small bowel wall, but did not detect a primary malignancy in the abdominal cavity; and no signs of portal hypertension or liver disease. In laboratory tests significant eosinophilia in blood and abdominal fluid were seen. Leukocyte count was 16680 /mm3 in the hemogram and 60% eosinophil was detected in the peripheral smear. In paracentesis fluid analysis; eosinophilia (70% eosinophil) were detected. Steroid treatment to be completed in six weeks in total was applied to the patient who was diagnosed with eosinophilic ascites. Therapy with corticosteroids resulted in resolution of both the gastrointestinal symptoms and the ascites without hesitation. He had not any complaint during four months follow-up; ascites regressed and examination was normal. EGE is a rare disorder and it should be flashed in patients who presented with unexplained ascites. Since diagnosis of EGE is a diagnosis of exclusion and, apart from malignancies, there are many other secondary causes of eosinophilic intestinal infiltration that must be excluded.

Iron refractory iron deficiency anemia is an autosomal recessive disorder arising from defects in iron metabolism that cause microcytic anemia to grow resistant to treatment. The patients usually do not respond to orally administered iron treatment and partially respond to intravenous iron administration. Mutations of TMPRSS6 gene which encodes matriptase-2 are the main cause of the disorder. Here, we describe the case of a 6-month-old Syrian boy who had hypochromic-microcytic anemia and normal ferritin levels at presentation. The patient did not respond to 1 month of iron therapy and his hemoglobin levels increased only after red blood cell transfusion. Mutation analysis demonstrated a novel 374 base pairs homozygote deletion spanning exon 15 of TMPRSS6 gene. Our results expand the mutation spectrum of TMPRSS6 gene in iron refractory iron deficiency anemia.

Objective Rhabdoid tumors, which are rare in childhood, are aggressive cancers. It can be particularly seen in 3 different anatomical regions, mostly in the central nervous system, kidneys, and soft tissue in early childhood. In this study, it was aimed to evaluate the clinical, radiological and pathological features of pediatric patients with rhabdoid tumors who were followed up and treated in 3 different pediatric oncology reference centers. Methodology Erciyes University Faculty of Medicine, Kahramanmaras Sutcu Imam University Faculty of Medicine, Health Practice and Research Hospital and Adana City Training and Research Hospital, 17 patients diagnosed with rhabdoid tumor between 2002-2021 were retrospectively analyzed. Results Of the patients, 6 (35%) were female and 11 (65%) were male. Chemotherapy (Doxorubicin, Ifosfamide, Carboplatinum, Etoposide, Vincristine, Actinomycin-D, Cyclophosphamide) was administered to the patients at different times. Radiotherapy was applied to 8 (47%) of the patients. The tumor was in the brain in 8 (47%) of the patients, in the kidney in 4 (23%), in the skin in 4 (23%), and the liver in 1 (6%). Conclusion In this study, the incidence of rhabdoid tumors was higher in males. This may be due to the small number of cases. The 2 years overall survival rates were 50% in brain tumors, 6% in kidney tumors, and 12% in others, according to tumor localization. The localization and stage of the tumor were determinants of the survival of the patients. More clinical studies are needed to improve survival and identify more effective treatment strategies in these tumors.

Background Familial hemophagocytic lymphohistiocytosis (HLH) is a fatal disease affecting infants and very young children. Central nervous system involvement of HLH can cause catastrophic results. Method We present a case with cranial involvement of familial HLH type 4 who showed diffuse infiltration of white matter complicated with intracranial thrombosis. A 5-year-old girl from a consanguineous couple presented with fever and pancytopenia, and was referred to our hematology unit. Examination revealed fever, lymphadenopathy, and hepatosplenomegaly. Ultrasound examination revealed hepatosplenomegaly and free intra-abdominal fluid. HLH was revealed on bone marrow aspiration biopsy. Defective natural killer and T lymphocyte cytotoxicity using degranulation tests was determined. In the genetic analysis, syntaxin gene mutation was found. On T2-weighted and T2-fluid-attenuated inversion recovery magnetic resonance imaging (MRI), diffuse hyperintense signal changes of cerebral white matter, indicating white matter demyelination, were observed. A second brain MRI showed an acute infarct involving the left temporooccipital region. Immunosuppressive therapy according to the HLH 2004 protocol was started. The infarct resolved but white matter lesions were stable on the brain MRI that was performed 1 month later. Brain MRI taken 4 months after the first examination showed stable cerebral white matter lesions, but hyperintense signal changes appeared in the cerebellar white matter and were regarded as progression. The patient died because of infection despite immunosuppressive therapy. Conclusions Physicians managing patients with HLH must be vigilant about the possibility of central nervous system involvement including stroke.

AMAÇ: Orak hücre anemisinde (OHA) cerrahi işlemler sıkça gereklidir. OHA’li hastaların postoperatif kompli-kasyonlar için nispeten daha yüksek riske sahip olduğu kabul edilmektedir. Bu çalışmanın amacı, ameliyat olan OHA’lı hastaların ameliyat öncesi ve sonrası yönetimini değerlendirmektir. GEREÇ VE YÖNTEM: Hastanemizde ameliyat olan tüm OHA’li çocukların yaş, cinsiyet, preoperatif incelemeler ve postoperatif komplikasyon bilgilerine geriye dönük olarak tıbbi kayıtları incelenerek ulaşıldı. BULGULAR: 2012-2014 yılları arasında, OHA’li 12 çocuk hasta hastanemizde ameliyat oldu. Hastaların ortalama yaşı 12.1 yıl (aralık, 6–17) idi. Beşi kız çocuktu. Cerrahi işlemler kolesistektomi (n=6), tonsillektomi/adenoidek-tomi (n=3), splenektomi (n=2) ve over kist eksizyonu (n=1) idi. Splenektomi olan bir hastaya eş zamanlı olarak kolesistektomi uygulandı. Hastalar ameliyattan önce ba-sit eritrosit transfüzyonu (n=1) veya total kan değişimi (n=10) tedavisi aldı. Bir hasta kronik transfüzyon progra-mına kayıtlı olduğu için transfüzyon almadı. Tüm hastalar ameliyat öncesi geceden başlayarak ve ameliyat sonra-sıda devam eden intravenöz hidrasyon aldı. Ameliyat sonrası ortalama hastanede kalış süresi 5,3 gün (dağılım, 3–10) idi. Hiç vazooklüziv kriz ya da ölüm saptanmadı. SONUÇ: OHA’lı hastalara elektif cerrahi öncesi, postope-ratif komplikasyonları azaltmak amacı ile Hemoglobin S oranını %40’ın altına düşürmek için transfüzyon yapılma-lıdır. İyi preoperatif ve postoperatif yönetim ile cerrahi OHA’lı çocuklarda etkili ve güvenlidir. OBJECTIVE: Surgical procedures are often required in si-ckle cell anemia (SCA). Patients with SCA are recognized as having a relatively higher risk for postoperative comp-lications. The aim of the present study was to evaluate the preoperative and postoperative management of pa-tients with SCA undergoing surgery. MATERIALS AND METHODS: The medical records of all children with SCA who had surgery at our hospital were retrospectively reviewed for the following: age, gender, preoperative investigations, and postoperative compli-cations. RESULTS: From 2012 to 2014, 12 children with SCA had surgery at our hospital. The patients’ mean age was 12.1 years (range, 6–17). Five patients were female. The surgical procedures were cholecystectomy (n=6), tonsillectomy/ adenoidectomy (n=3), splenectomy (n=2) and excision of ovarian cyst (n=1). In a patient who had undergone sple-nectomy, concomitant cholecystectomy were also per-formed. Patients received simple packed red blood cell transfusion (n=1) or total exchange transfusion (n=10) before the surgery. One patient was not transfused which was already enrolled in a chronic transfusion program. All patients received intravenous hydration the night before the surgery and postoperatively. The mean postoperati-ve hospital stay was 5.3 days (range, 3-10). There were no vaso-occlusive crises or deaths. CONCLUSIONS: Children with sickle cell disease presen-ting for elective surgery should be given a transfusion for a hemoglobin S ratio less than 40% in an attempt to redu-ce postoperative complications. With good preoperative and postoperative management, surgery is both safe and effective for children with SCA

Neuroblastoma is the most common type of extracranial solid tumor during childhood. Clinical presentation includes ipsilateral ptosis, myosis, anhydrosis and enophthalmos. The case of a 2.5-year-old boy who had a complaint of constriction of the left pupil for 3 days is presented. In the physical examination, the pupil of the OD was moderately dilated; there was myosis on the OS and ptosis on the left eyelid. Horner syndrome was considered due to these findings. History of the patient revealed that a central venous catheter insertion procedure was tried from the left side.

Objectives: This study, detection of beta globin gene mutations in thalassemia major patients who migrated from Syria to Kahramanmaraş region were planned. Materials and methods: The study included 35 Syrian national beta thalassemia major patients. Beta globin gene mutations were detected by ARMS (Amplification Refractory Mutation System) method, RFLP (Restriction Fragment Length Polymorphism) method and DNA sequence analysis. Codon 15, codon 9/10, codon 5 and codon 8 mutations, which we could not detect with other methods in our study, were detected by sequence analysis. Results: In beta thalassemia major patients, 16 types of mutations were detected, the most common being IVS-I-110 (n = 8). Other mutations are according to frequency order IVS-II-745 (n = 3), codon 44 (n = 3), codon 15 (n = 3), IVS-I-110/IVS-I-1 (n = 3), codon 5 (n = 2), IVS-I-1 (n = 2), codon 8/IVS-II-1 (n = 2), codon 44/codon 15 (n = 2), IVS-II-1 (n = 1), codon 39 (n = 1), IVS-I-6/codon 5 (n = 1), codon 9/10 (n = 1), IVS-I-110/codon 39 (n = 1), IVS-I-5/IVS-II-1 (n = 1), codon 39/IVS-II-745 (n = 1). Conclusions: According to the results of our study beta-thalassemia mutations in Syrian immigrant groups show heterogeneity and mutation types of mutation map is similar to Turkey. The conclusion is to prevent families to have a second patient child by genetic counseling. Gereç ve Yöntem: Çalışmaya 35 Suriye uyruklu beta talasemi majorlu hasta dahil edildi. Hastalardan tam kandan hemogram ve mutasyon çalışması için EDTA’lı tüplere kan alındı. Alınan kan örneklerinden ARMS (Amplification Refractory Mutation System) yöntemi, RFLP (Restriction Fragment Length Polimorfizm) yöntemi ve DNA dizi analizi ile beta globin gen mutasyonları saptandı. Çalışmamızda diğer yöntemlerle saptayamadığımız Kodon 15, Kodon 9/10, Kodon 5 ve Kodon 8 mutasyonları sekans yöntemi ile saptandı. Bulgular: Çalışmaya dahil edilen 35 hastanın 19’u (%54,3) erkek, 16’sı (%45,7) kız idi. Tüm hastaların yaşları ortalaması 9,48 ± 4,51 yıl (2 yaş-17 yaş) idi. Beta talasemi majörlü hastalarda en yaygın IVS-I-110 (n = 8) olmak üzere 16 çeşit mutasyon tespit edilmiştir. Objective: Infant feeding pattern in the first 1000 days period has significant effect on the growth and development of child. We aimed to investigate the effects of breastfeeding duration and feeding habits on malnutrition.Material and Methods: Patients who were consecutively admitted to the pediatric gastroenterology and pediatric outpatient clinics were included in the study. 250 patients with primary malnutrition and 250 patients without malnutrition were asked to complete questionnaire questioning breast milk intake times and feeding habits.Results: The mean age of the patients with malnutrition was 19.77 ± 9.90 months and 20.584 ± 10.971 months for patients without malnutrition. There was no statistically significant difference between the two groups in terms of age and sex (P = 0.385, P = 0.140, respectively). The program involved 6 months of training, supported by monthly exams. We, as a group of mentees from the Hemophilia Masterclass Academy, aimed to evaluate the bleeding phenotype of patients with von Willebrand Disease (VWD) using the International Society of Thrombosis and HaemostasisBleeding Assessment Tool (ISTH-BAT) and the Pediatric Bleeding Questionnaire (PBQ) scores and investigate the correlation of von Willebrand factor antigen (VWF:Ag) levels and bleedingm scores of the patients, as well as present the initial output of our Masterclass Academy.

Amaç: Anemi, insan ve toplum sağlığını ilgilendiren önemli bir halk sağlığı sorunudur. Bu çalışmada farklı yaş gruplarında anemi etiyolojisini belirlemeyi ve sosyal farkındalık yaratmayı amaçladık.Yöntemler: Çalışmaya (retrospektif kohort çalışması), Ocak 2015-Ocak 2018 tarihleri arasında, KSU Tıp Fakültesi Çocuk Hematoloji ve Onkoloji Polikliniğine başvuran 6 ay - 18 yaş arasındaki anemi hastaları dahil edilmiştir.Bulgular: Çalışmaya 1120 hasta dahil edildi. 873 hastada Demir eksikliği anemisi, 184 hastada talasemi minör, 25 hastada vitamin B12 eksikliği anemisi, 13 hastada otoimmün hemolitik anemi, 10 hastada ise kalıtsal sferotositoz ve diğer anemi etiyolojileri vardı. Tüm hastaların% 16,4'ünde, 6-12 yaş arası hastaların % 33,8'inde talasemi minor vardı. Tüm hastaların %77,9’unda demir eksikliği anemisi vardı. 6 ay ile 2 yaş arası hastaların % 81,3'ünde, 2-6 yaş arası hastaların % 74,3'ünde, 6-12 yaş arası hastaların % 60,4'ünde ve 12-18 yaş arası hastaların % 85,0'inde demir eksikliği anemisi vardı. Demir eksikliği anemisi 2-6 yaş grubunda erkeklerde (%60,2) sık görülürken (P=0,002), 12-18 yaş grubunda kadınlarda (%80,6) yaygındı (P=0,000). In this study, we aimed to determine the etiology of anemia in different age groups and create social awareness.Methods: The study (retrospective cohort study) included anemia patients between 6 months and 18 years old who applied to the Pediatric Hematology and Oncology Outpatient Clinic of KSU Medical Faculty between January 2015 and January 2018.Results:1120 patients were included in the study. There were 873 with iron deficiency anemia, 184 with thalassemia minor, 25 with vitamin B12 deficiency anemia, 13 with autoimmune hemolytic anemia, 10 with hereditary spherocytosis and the other etiology of anemia. While 16.4% of all patients and 33.8% of patients aged 6-12 years had thalassemia minor. On the other side, 81.3% of patients aged 6 months-2 years, 74.3% of patients aged 2-6 years, 60.4% of patients aged 6-12 years and 85.0% of patients aged 12-18 years had iron-deficiency anemia. The Gastrointestinal system is one of the most common places for extranodal Non-Hodgkin lymphomas. On the other hand, Non-Hodgkin lymphomas have been seen less in the small intestine. We present here a rare condition in the literature which is a case of bilateral adrenal mass and Burkitt lymphoma with small intestine origin. Eleven years old male patient applied to pediatric emergency service with acute abdomen. In the abdominal USG examination intussusception and bilateral adrenal mass had seen. In a surgical operation, five lesions were seen in the small intestine. Partial small intestine resection was performed. In the pathological examination of the resection material Burkitt Lymphoma with the origin of the small intestine diagnosed to the patient. Prognosis of NHL with the origin of the small intestine, adrenal mass and come with an acute abdomen depends on the histological type and grade. He was started on NHL - BFM95 chemotherapy and had a good clinical response and he was remained in complete remission for two months after completion of chemotherapy. Bilateral adrenal mass remains a rare form of Burkitt lymphoma and care must be taken in ruling out other causes of bilateral adrenal masses.
