Abstract

uyanikoglu, hacer/0000-0003-0316-4900 WOS: 000396411700020 A twenty-nine year old, multigravida woman was admitted to our clinic due to a gunshot injury. She had 24 weeks of gestation. It was observed by ultrasonography that there were 6 cm hypoechogenic image retroplacentally located and 1 cm hyperechogenic focus near to this image, and there was also a 2 cm actively bleeding incision area in subumbilical region by inspection of abdomen. We decided emergent operation because of the diagnosis of abruptio placenta. During the operation, we observed a piece of bullet between the placenta and myometrium and 50% separation of the placenta. We removed the piece of bullet, repaired the uterine perforation and then performed a caesarean section. We considered that this case is important because a gunshot injury had only caused an abruptio placenta without affecting mother and her fetus seriously.

Abstract

Plasenta perkratada konservatif tedavi uygulanan iki olgu sunuldu. Kliniğimizde plasenta perkrata tanısıkonan iki olguya konservatif tedavi uygulandı. İlk olguda kanamadan dolayı tedavide istenilen başarı eldeedilemedi. İkinci olguda; abdominal orta hat ve uterin fundusda klasik insizyon, internal iliak arterligasyonu, profilaktik geniş spektrumlu antibiyotik kullanımı, menstruel siklus supresyonu, human koryonikgonadotropin seviyesinde hızlı düşüş trendi elde edene kadar metotreksat uygulaması ve plasental atılımgerçekleşene kadar hastanın haftalık izlemi ile başarılı bir sonuç elde edildi. Hemodinamik olarak stabil vefertilite isteği olan plasenta perkrata olgularında konservatif yaklaşım daha uygun bir seçenek olabilir., To present two cases of placenta percreta which were treated with conservative management in our clinic.Two cases of retained placenta percreta are presented. Treatment was failed in the first case because ofhaemorrhage. The second one was treated successfully with the procedures which consisted of caesareansection with midline abdominal and classic incision in the uterine fundus, internal iliac artery ligation,prophylactic broad-spectrum antibiotic treatment, suppression of menstrual cycle, use of methotrexate untilthe achievement of rapid downtrend of human chorionic gonadotropin, and monitoring the patient untilspontan expulsion or resorbtion of placenta. It may be better to treat the cases of placenta percreta withconservative management, if the patients are hemodynamically stable and desirable for future fertility.

27. Comparison of conservative and radical surgery in the management of placenta previa percreta

Abstract

Objective We aimed to compare the uterine sparing (US) surgery and hysterectomy for placenta previa percreta (PPP) management. Methods Data from PPP patients with anterior invasion who underwent US surgery and caesarean hysterectomy were retrospectively analyzed. The clinical and surgical outcomes of patients with PPP were compared according to the type of surgery. Results The mean intraoperative blood loss was lower in US surgery group than in caesarean hysterectomy group (1227.78±204.80 ml vs 1442.22±125.68 ml; p=0.017). The hemoglobin drop was also significantly lower in the patients with US surgery (1.87±0.68 g/dl vs 2.88±1.04 g/dl; p=0.026). Moreover, the mean total transfusion rate was also significantly lower in the patients with US surgery (1.33±0.87 U vs 2.33±0.71 U; p=0.016). Conclusion Uterine sparing surgery reduces intraoperative blood loss and transfusion rate in PPP patients with anterior placental invasion compared to hysterectomy. The temporary blockage of bilateral uterine and uteroovarian arteries with Satinsky clamps may potentially contribute to the success of US surgery.

41. Assessment of Apoptotic Activity Dysregulation and Oxidative Stress in the Development of Epithelial Ovarian Cancer: A Case-Controlled Descriptive Analysis.

Abstract

Aim: In the present study, we aimed to assess whether oxidative stress and apoptotic activity play a role in the development of epithelial ovarian cancer (EOC).Methods: The study group included patients with EOC (n = 26) and benign ovarian tumour (BOT) (n = 25), while 30 healthy women were employed as a control group. Venous blood samples were drawn to evaluate oxidative stress parameters and serum M30/M65 antigen levels before surgery. In addition, blood samples were taken for the second time on postoperative day 8 to analyse whether the postoperative tumour load was decreased.Results: When the groups were assessed regarding oxidative stress, the highest values were detected in patients with EOC. Serum M30/M65 levels were found to be higher in patients with EOC when compared to the other groups (p < 0.001). A significant decrease was determined in the M30/M65 levels of serum samples taken on postoperative day 8 from the patients in the EOC and BOT groups (p < 0.001).Conclusion: Our results suggest that dysregulation of apoptotic activity could be effective in the development of ovarian tumoural tissue, whereas oxidative stress could be effective in malignant transformation. Abstract

Objective: This study aimed to evaluate whether trophoblastic transforming growth factor beta (TGF-β) and E-cadherin expression levels have a role in placenta percreta (PP) aetiopathogenesis.Methods: This study was carried out in the Obstetrics & Gynecology and Pathology Departments of Harran University Medicine School. Forty-four women who underwent caesarean section for PP and other obstetric reasons were included in this study. PP was defined as the detection of placental invasion during the histopathological examination of the hysterectomy specimen, which passes the uterine wall as a whole layer and involves the uterine serosa. Placental tissue samples were collected from all pregnant patients to evaluate TGF-β and E-cadherin expression levels.Results: No significant difference was found in demographic features, including age, gestational week, number of pregnancies and body mass index, among the groups. Immunohistochemical staining against E-cadherin, a cell adhesion molecule, showed significantly reduced staining in PP patients (p = 0.048). TGF-β staining was also low in PP patients, but this difference was not significant (p = 0.107).Conclusions: The findings of this study suggest that a decrease in trophoblastic E-cadherin expression may have an important role in PP aetiopathogenesis.
