6. USO PROLONGADO DE CÁNULA NASAL DE ALTO FLUJO EN NIÑOS CON PROBLEMAS RESPIRATORIOS

La cánula nasal de alto flujo (CNAF) es una modalidad ventilatoria no invasiva segura y efectiva, usada ampliamente en patología respiratoria aguda en adultos y niños. Objetivo: presentar casos clínicos pediátricos que utilizaron CNAF por tiempo prolongado por problemas respiratorios crónicos. Descripción de casos clínicos, revisión de fichas clínicas de 5 pacientes que utilizaron CNAF por más de 1 mes, entre los años 2017-2020 en el Complejo Asistencial Dr. Sótero del Río. Aprobado por Comité de Ética. Resultados: 5 pacientes varones de mediana 61 (44 a 212) días de edad al inicio del uso de CNAF. Diagnóstico de base: displasia broncopulmonar (2/5), síndrome de Treacher Collins (1/5), síndrome de cimitarra con hipoplasia pulmonar derecha (1/5) y traqueobroncomalacia severa (1/5). Todos requirieron previamente uso de ventilación invasiva o no invasiva con mediana de 59 (4 a 78) días. A todos se les realizó broncoscopia, saturometría contínua o poligrafía para diagnóstico y titulación de CNAF y oxígeno. Todos mejoraron clínicamente, la SpO2 y el número de apneas. Dos pacientes se enviaron a domicilio con uso de Airvo2 nocturno. La mediana de uso de CNAF fue 165 (34 a 445) días. Conclusiones: el uso prolongado de CNAF es útil en pacientes pediátricos seleccionados, bien tolerado y factible de utilizar en domicilio.

Se acompaña de una revisión de la literatura del tema.

The FORG (Fibre Optic Resonant Gyroscope) opeeration principle uses a recirculating ring resonant cavity to get a rotation-induced Sagnac effect enhancement (cited by Ezekiel and Balsamo). It grants to a FORG a comparable sensitivity in relation an I-FOG (Interferometric Fibre Optic Gyroscope)that has the fiber lenght equal a half of finesse factor times longer. Other advantages is despite of thermal drift because the FORG uses less quantity of fibre than the I-FOG, giving to the first less thermal drift sensitivity than the last. The proposal of this work is to show a applied to a FORG technique that simplifies the signal treatment, employing all digital setup, resulting in a maximally flat scale factor. In this investigation are presented over the simulations results, employing the modified digital FM spectrometry techniques by decimation and interpolation techniques over a ring resonator that pursuit a 10 meters SM-PM lenght fiber coil and 10 centimeters of diameter, with a 1550 nanometers laser source. The advantages of these techniques are to simplify the electronic circuitry, offering an upgrade facility, using only one DSP (Digital Signal Processor), realizing all needed functions. The investigation of this method is based in a optical field switching scheme and digital frequency domain spectrometry., Comment: Article submitted to Symposium of Gyro Technology 2003, Stuttgart, Germany. This dataset presents the outcomes of implementing the exploratory modelling and analysis (EMA) protocol for identifying robust policy mixes for clean energy transitions. The protocol, detailed in Protocol Exchange (https://protocolexchange.researchsquare.com/), is designed to explore the consequences of diverse policy alternatives and multiple uncertainties within energy transitions through computational experiments. EMA, a computational experimentation technique, plays a key role in systematically exploring the potential impacts of various policy alternatives and uncertainties within complex systems, particularly in the energy domain. This publication outlines the application of the EMA protocol in the specific case of Mexico, offering a detailed approach for researchers, policymakers, and energy analysts to explore the complex interactions between policy alternatives and uncertainties in the clean energy transition. The dataset provides insights into how different policy alternatives perform under various conditions, shedding light on their robustness and potential trade-offs. The dataset encompasses the outcomes of an open exploration and directed search processes, along with analytical sub-processes integrated to provide a comprehensive analysis. The results from implementing the EMA protocol offer a valuable resource for decision-makers and researchers seeking to navigate the complex interactions between policy alternatives and uncertainties in energy transitions.

Significance\ud \ud Communicating in ways that motivate engagement in social distancing remains a critical global public health priority during the COVID-19 pandemic. This study tested motivational qualities of messages about social distancing (those that promoted choice and agency vs. those that were forceful and shaming) in 25,718 people in 89 countries. The autonomy-supportive message decreased feelings of defying social distancing recommendations relative to the controlling message, and the controlling message increased controlled motivation, a less effective form of motivation, relative to no message. Message type did not impact intentions to socially distance, but people’s existing motivations were related to intentions. Findings were generalizable across a geographically diverse sample and may inform public health communication strategies in this and future global health emergencies.\ud \ud \ud \ud Abstract\ud \ud Finding communication strategies that effectively motivate social distancing continues to be a global public health priority during the COVID-19 pandemic. This cross-country, preregistered experiment (n = 25,718 from 89 countries) tested hypotheses concerning generalizable positive and negative outcomes of social distancing messages that promoted personal agency and reflective choices (i.e., an autonomy-supportive message) or were restrictive and shaming (i.e., a controlling message) compared with no message at all. Results partially supported experimental hypotheses in that the controlling message increased controlled motivation (a poorly internalized form of motivation relying on shame, guilt, and fear of social consequences) relative to no message. On the other hand, the autonomy-supportive message lowered feelings of defiance compared with the controlling message, but the controlling message did not differ from receiving no message at all. Unexpectedly, messages did not influence autonomous motivation (a highly internalized form of motivation relying on one’s core values) or behavioral intentions. Results supported hypothesized associations between people’s existing autonomous and controlled motivations and self-reported behavioral intentions to engage in social distancing. Controlled motivation was associated with more defiance and less long-term behavioral intention to engage in social distancing, whereas autonomous motivation was associated with less defiance and more short- and long-term intentions to social distance. Overall, this work highlights the potential harm of using shaming and pressuring language in public health communication, with implications for the current and future global health challenges.

This dataset includes input data to estimate learning-by-deploying (LbD) and learning-by-researching (LbR) rates for onshore wind and solar PV in the United States of America (USA). Using different learning curve approaches the simulated technological-based cost developments are also presented. Coefficient of determination (R squared) and Root Mean Square Error (RMSE) were applied for quantification of the agreement between simulated and observed technological-based costs.

- Abstract
Technological innovation planning for developing and deploying clean energy technologies plays a key role in reducing greenhouse gas emissions and transition to a low-carbon future. Learning curve theory has been adopted as a common framework for exploring the relationship between endogenous technological learning and technology cost developments. The aim of this article is to analyse the effects of selecting different learning curve approaches (i.e. model formulations) to describe energy technology cost changes over time. Experience and knowledge stock are chosen as the sources of learning to be considered. A new definition of experience was developed to account for the interaction between global and local experience. The new definition of experience also accounts for learning sub-processes (i.e. learning-by-doing, learning-by-using, and experience spillovers) to estimate total experience gained through technology deployment. An integrative model is developed for estimating the effects of learning-by-deploying and learning-by-researching on cost developments for onshore wind and solar PV in the USA. Publicly available data from government departments and organisations were utilised. It was found that technology cost developments are better explained when: (1) experience is defined as a function of global and local experience; (2) knowledge stock is also considered in the model formulation; and (3) technological processes affect only a fraction of the total capital cost. The findings suggested that the application of learning rates for model-based energy planning is context-dependent and how technological factors are explicitly defined may have significantly different policy implications (i.e. different technology costs predictions based on alternative model formulations).

- Abstract
The COVID-19 pandemic (and its aftermath) highlights a critical need to communicate health information effectively to the global public. Given that subtle differences in information framing can have meaningful effects on behavior, behavioral science research highlights a pressing question: Is it more effective to frame COVID-19 health messages in terms of potential losses (e.g., “If you do not practice these steps, you can endanger yourself and others”) or potential gains (e.g., “If you practice these steps, you can protect yourself and others”)? Collecting data in 48 languages from 15,929 participants in 84 countries, we experimentally tested the effects of message framing on COVID-19-related judgments, intentions, and feelings. Loss- (vs. gain-) framed messages increased self-reported anxiety among participants cross-nationally with little-to-no impact on policy attitudes, behavioral intentions, or information seeking relevant to pandemic risks. These results were consistent across 84 countries, three variations of the message framing wording, and 560 data processing and analytic choices. Thus, results provide an empirical answer to a global communication question and highlight the emotional toll of loss-framed messages. Critically, this work demonstrates the importance of considering unintended affective consequences when evaluating nudge-style interventions.

INTRODUCTION: Germline CNVs are important contributors to hereditary cancer. In genetic diagnostics, multiplex ligation-dependent probe amplification (MLPA) is commonly used to identify them. However, MLPA is time-consuming and expensive if applied to many genes, hence many routine laboratories test only a subset of genes of interest. METHODS AND RESULTS: We evaluated a next-generation sequencing (NGS)-based CNV detection tool (DECoN) as first-tier screening to decrease costs and turnaround time and expand CNV analysis to all genes of clinical interest in our diagnostics routine. We used DECoN in a retrospective cohort of 1860 patients where a limited number of genes were previously analysed by MLPA, and in a prospective cohort of 2041 patients, without MLPA analysis. In the retrospective cohort, 6 new CNVs were identified and confirmed by MLPA. In the prospective cohort, 19 CNVs were identified and confirmed by MLPA, 8 of these would have been lost in our previous MLPA-restricted detection strategy. Also, the number of genes tested by MLPA across all samples decreased by 93.0% in the prospective cohort. CONCLUSION: Including an in silico germline NGS CNV detection tool improved our genetic diagnostics strategy in hereditary cancer, both increasing the number of CNVs detected and reducing turnaround time and costs.

Given that subtle differences in information framing can have meaningful effects on behavior, behavioral science research highlights a pressing question: Is it more effective to frame COVID-19 health messages in terms of potential losses (e.g., "If you do not practice these steps, you can endanger yourself and others") or potential gains (e.g., "If you practice these steps, you can protect yourself and others")? Collecting data in 48 languages from 15,929 participants in 84 countries, we experimentally tested the effects of message framing on COVID-19-related judgments, intentions, and feelings. Loss- (vs. gain-) framed messages increased self-reported anxiety among participants cross-nationally with little-to-no impact on policy attitudes, behavioral intentions, or information seeking relevant to pandemic risks. These results were consistent across 84 countries, three variations of the message framing wording, and 560 data processing and analytic choices. Thus, results provide an empirical answer to a global communication question and highlight the emotional toll of loss-framed messages. Critically, this work demonstrates the importance of considering unintended affective consequences when evaluating nudge-style interventions. 26 p.

Although echocardiography has been used in rats, few studies have determined its efficacy for estimating myocardial infarct size. Our objective was to estimate the myocardial infarct size, and to evaluate anatomic and functional variables of the left ventricle. Myocardial infarction was produced in 43 female Wistar rats by ligature of the left coronary artery. Echocardiography was performed 5 weeks later to measure left ventricular diameter and transverse area (mean of 3 transverse planes), infarct size (percentage of the arc with infarct on 3 transverse planes), systolic function by the change in fractional area, and diastolic function by mitral inflow parameters. The histologic measurement of myocardial infarction size was similar to the echocardiographic method. Myocardial infarct size ranged from 4.8 to 66.6% when determined by histology and from 5 to 69.8% when determined by echocardiography, with good correlation (r = 0.88; P < 0.05; Pearson correlation coefficient). Left ventricular diameter and mean diastolic transverse area correlated with myocardial infarct size by histology (r = 0.57 and r = 0.78; P < 0.0005). The fractional area change ranged from 28.5 ± 5.6 (large-size myocardial infarction) to 53.1 ± 1.5% (control) and correlated with myocardial infarct size by echocardiography (r = -0.87; P < 0.00001) and histology (r = -0.78; P < 00001). The E/A wave ratio of mitral inflow velocity for animals with large-size myocardial infarction (5.6 ± 2.7) was significantly higher than for all others (control: 1.9 ± 0.1; small-size myocardial infarction: 1.9 ± 0.4; moderate-size myocardial infarction: 2.8 ± 2.3). There was good agreement between echocardiographic and histologic estimates of myocardial infarct size in rats.

- Abstract
The COVID-19 pandemic has increased negative emotions and decreased positive emotions globally. Left unchecked, these emotional changes might have a wide array of adverse impacts. To reduce negative emotions and increase positive emotions, we tested the effectiveness of reappraisal, an emotion-regulation strategy that modifies how one thinks about a situation. Participants from 87 countries and regions (n = 21,644) were randomly assigned to one of two brief reappraisal interventions (reconstrual or repurposing) or one of two control conditions (active or passive). Results revealed that both reappraisal interventions (vesus both control conditions) consistently reduced negative emotions and increased positive emotions across different measures. Reconstrual and repurposing interventions had similar effects. Importantly, planned exploratory analyses indicated that reappraisal interventions did not reduce intentions to practice preventive health behaviours. The findings demonstrate the viability of creating scalable, low-cost interventions for use around the world. - Abstract
The COVID-19 pandemic has increased negative emotions and decreased positive emotions globally. Left unchecked, these emotional changes might have a wide array of adverse impacts. To reduce negative emotions and increase positive emotions, we tested the effectiveness of reappraisal, an emotion-regulation strategy that modifies how one thinks about a situation. Participants from 87 countries and regions (n = 21,644) were randomly assigned to one of two brief reappraisal interventions (reconstrual or repurposing) or one of two control conditions (active or passive). Results revealed that both reappraisal interventions (vesus both control conditions) consistently reduced negative emotions and increased positive emotions across different measures. Reconstrual and repurposing interventions had similar effects. Importantly, planned exploratory analyses indicated that reappraisal interventions did not reduce intentions to practice preventive health behaviours. The findings demonstrate the viability of creating scalable, low-cost interventions for use around the world.

Three experiments were performed to study the nutritional characteristics of sugar beet pulp (SBP) according to the concentration of vinasse (condensed molasses solubles) added. Eighteen Merino ewes were used to study preference in Exp. 1, and 18 Merino ewes were used in Exp. 2 to study voluntary intake of SBP with different amounts of vinasse inclusion (0, 7, and 13% on a DM basis; SBP0, SBP7, and SPB13, respectively). Sheep showed a marked preference for SBP with vinasse, regardless of the amount of addition, whereas voluntary feed intake was not affected (P > 0.34). Eight ruminally cannulated Merino ewes were used in Exp. 3 to study rumen fermentation of SBP0 and SBP13. Regardless of the diet fed to the animals (SBP0 or SBP13), disappearance rates of DM, NDF, and CP were greater when SBP13, compared with SBP0, was incubated in sacco (P < 0.05), and NDF and CP disappearance rates were increased when animals were fed the SBP13 diet, No significant differences were observed for pH, ammonia concentration, or total or individual VFA in the ruminal liquor of sheep at different sampling times after feeding (P > 0.23) in response to vinasse addition. Experiment 4 was designed to study digestibility, solid passage rate, and excretion of purine derivatives from 12 Merino ewes fed SBP0 and SBP13. Digestibility of NDF tended to be greater (P < 0.10) for the SBP13 group compared with the SBP0 group; digestibilities of DM, OM, CP, and ADF were not affected (P > 0.11). Digesta flow kinetics and urinary excretion of purine derivatives were not significantly affected by the presence (SBP13) or absence (SBP0) of vinasse in the diet (P > 0.21). In conclusion, sheep showed a clear preference for SBP with vinasse. However, the lack of significant differences in most of the in vivo variables measured indicates that ewes can be satisfactorily fed with any of the amounts of vinasse inclusion studied. Objetivo Describir la seguridad de la administración de altas dosis de RAI en pacientes con CDT. Materiales y Método Estudio retrospectivo descriptivo. Se incluyó a todos los pacientes con diagnóstico de CDT que requirieron hospitalización para administración de RAI ≥ 30 mCi en el Hospital Regional de Talca (HRT) entre agosto-diciembre de 2018. Resultados Durante el período descrito 10 pacientes recibieron RAI bajo régimen hospitalario. La mediana de dosis de RAI administrada fue de 100 mCi (rango: 50-150 mCi). Todos los pacientes fueron manejados con asilamiento estricto. El promedio de hospitalización fue 28 horas, siendo dados de alta al reportar una tasa de dosis absorbida < 70 µSv/h a 1 metro. Se entregaron instrucciones al alta para minimizar el riesgo de irradiación o contaminación a terceras personas. Conclusiones Nuestro protocolo de administración de RAI permite tratar de manera segura a pacientes con CDT disminuyendo la exposición a radiación de terceros. Las salas de asilamiento de radioyodoterapia, podrían dar cobertura al 100% de la demanda de terapia con RAI en CDT a nivel local. Introduction Differentiated thyroid cancer (CDT) presents an increase in global levels. The selective use of radioiodine therapy (RAI) is a pillar of its treatment. Its therapeutic effect is due to beta radiation, while gamma makes hospitalization necessary to limit exposure. Aim To describe the safety treated with RAI inpatients and the functioning of the radioactive isolation rooms of our center. Materials and Method Retrospective descriptive study. All patients diagnosed with CDT who required RAI therapy under a hospital regimen at the Regional Hospital of Talca (HRT) between August-December 2018 were included. Results During the period described, 10 patients were treated. The median dose of RAI administered was 100 mCi (range: 50-150 mCi). The average of hospitalization was 28 hours, being discharged when reporting an absorbed dose rate < 70 μSv/h at 1 meter, giving the patient instructions, so that they follow to minimize the risk of irradiation or contamination of people in their environment. Conclusions Our RAI administration protocol allows patients with CDT to be treated safely. The radioactive isolation rooms could cover 100% of the demand for RAI therapy in CDT at the local level.
