Best-corrected distance (CDVA) and uncorrected distance visual acuity (UDVA), uncorrected intermediate visual acuity (UIVA) at 66 cm, uncorrected near visual acuity (UNVA) at 40 cm, and defocus curve were assessed. Patient-reported visual function was inquired by Catquest-9SF, and the rate of spectacle independence in all daily-life activities was calculated. The presence of photic phenomena was evaluated. A p-value lower than 0.05 was considered statistically significant.Results: The mean IOL power was +21.50 ± 4D (16.5â 26D), and all were non-toric. Thirty-seven (92.5%) eyes were within ± 0.5D of predicted target. The postoperative MRSE was 0.06 ± 0.42D and â 0.45 ± 0.22D in dominant and non-dominant eyes. Every patient achieved distance binocular vision better than 0.1 logMAR. The non-dominant eyes showed lower CDVA (p< 0.001). The UIVA was higher in non-dominant eyes (p< 0.001). Binocular uncorrected near visual acuity was 0.12 ± 0.1, and uncorrected near visual acuity was higher in non-dominant eyes (p< 0.001). LuxSmart IOL provided a sustained visual acuity of 0.3 logMAR or better between +1.00D and â 2.50D. A total of 25% of patients reported frequent halos and glare. Upper eyelid blepharoplasty reduces upper tarsal strength and may have an impact on ocular surface. The aim of this study is to evaluate the effect of upper eyelid blepharoplasty on corneal epithelial thickness profile, astigmatism and aberrations.Methods: Patients with dermatochalasis underwent bilateral upper eyelid blepharoplasty. Anterior segment optical coherence tomography (AS-OCT) (Zeiss Cirrus 5000 HD-OCT) and Pentacam (Oculus, Wetzlar Germany) were performed before surgery and in the 8th postoperative week. Corneal epithelial thickness, keratometry, aberrations and asphericity were considered for statistical purposes. Only right eyes were considered. A p-value lower than 0.05 was considered significant.Results: Thirty eyes of 30 patients were included. The degree of astigmatism did not change after surgery (0.95D vs 0.83D, p=0.23). The difference between preoperative and postoperative steepest axis was 3.1° (p=0.04) with a tendency to change toward the vertical meridian. Mean epithelial thickness was higher in the inferior region both pre- and postoperatively and did not change. ET in the superior octant was lower (42 μm vs 45 μm, p< 0.01) and the difference between inferior and superior octants (I-S) was higher (7 μm vs 3 μm, p< 0.001) before surgery. Corneal epithelial thickness was evaluated using high-definition anterior segment optical coherence tomography (Cirrus 5000 HD-OCT) in the baseline first consultation, and four weeks after treatment with preservative free 1mg/1mL sodium hyaluronate. Schirmer test 1 (ST1), tear break-up time (TBUT), tear meniscus height (TMH), SICCA Ocular Surface Score (SICCA OSS) and Ocular Surface Disease Index (OSDI) were evaluated. Statistical significance was defined as p-value < 0.05.Results: The study included 40 eyes, 20 with SS-DED and 20 controls. At baseline, SS-DED patients had lower ST1 (11.1 ± 2.2mm vs 14.1 ± 3.1mm, p< 0.01), faster TBUT (9.1 ± 1.8s vs 13.2 ± 1.1s, p< 0.01) and lower TMH (211.2 ± 68.4 μm vs 217.2 ± 60.1μm; p< 0.01) than the control group, whereas SICCA OSS and OSDI were higher (p< 0.01). The superior epithelium was thinner in SS-DED group (41.1 ± 3.2 μm vs 43.1± 2.3 μm, p< 0.01). Hepatic artery pseudoaneurysms (HAP) after liver transplantation are rare and associated with high mortality. Different treatment options exist for HAP, which include surgical intervention, radiological coil embolization or stenting and retransplantation. We describe a case of a patient presenting with HAP after a liver transplantion, that was successfully treated with stent grafting, allowing preservation of the arterial flow and minimal risk of organ failure. Endovascular intervention of HAP should be regarded as the treatment of choice, either in symptomatic or in asymptomatic patients., Pseudoaneurismas da artéria hepática (PAH) após transplante hepático são raros e estão associados a elevada mortalidade. Existem diferentes opções terapêuticas para os PAH, incluindo a intervenção cirúrgica, tratamento endovascular através de embolização com coils ou aplicação de stents e ainda re-transplantação. Descrevemos um caso de um paciente que desenvolveu um PAH após transplantação hepática, tratado com sucesso através de aplicação de um stent vascular, permitindo a preservação do fluxo arterial hepático e assim um risco mínimo de falência do transplante. O tratamento endovascular de PAH deve ser encarado como opção terapêutica de primeira linha, quer em pacientes sintomáticos quer em doentes assintomáticos.

INTRODUCTION: Retinopathy is the most frequent ophthalmological manifestation of sickle cell disease (SCD). Advances in retinal multimodal imaging have revealed the presence of retinopathy in asymptomatic patients. The present study aims to characterize the typical retinal changes by multimodal imaging in a pediatric population with SCD, allowing the non-invasive observation of the vascular effects of the disease. MATERIAL AND METHODS: Observational study that included children diagnosed with SCD referred from the Pediatric Hematology consultation at Hospital Garcia de Orta. The evaluation consisted of a complete ophthalmologic evaluation and multimodal imaging: wide-field fundus photography (Zeiss CLARUS 700), optical coherence tomography (OCT) and OCT- angiography (OCT-A) (Zeiss Cirrus HD-OCT 5000), with particular interest in the assessment of the vascular zone area and superficial macular vascular flow and density. RESULTS: Our study included 28 children with SCD, mean age 12.33 ± 3.19 years, 16 (57%) female and 12 (43%) male. The most frequent genotype was SS (26 patients). Of the 56 eyes evalu- ated, 20 (36%) had non-proliferative retinopathy and 1 (1.8%) had proliferative retinopathy. The most frequently detected retinal change was the sunburst lesion (9 eyes, 16%). Also noteworthy is the presence of dark without pressure lesions in 4 eyes (2%). There was a statistically significant difference (p, INTRODUÇÃO : A retinopatia é a manifestação oftalmológica mais frequente da doença de células falciformes (DCF). Os avanços em imagem multimodal retiniana têm revelado a presença de retinopatia em doentes assintomáticos. O presente estudo tem como objetivo caracterizar as alterações retinianas típicas por imagem multimodal numa população pediátrica com DCF, permitindo observar os efeitos vasculares da doença de modo não invasivo. MATERIAL E MÉTODOS: Estudo observacional que incluiu crianças com o diagnóstico de DCF referenciadas da consulta de Hematologia Pediátrica do Hospital Garcia de Orta. INTRODUCTION: Our aim was to assess optic disc structural, vascular and functional parameters in patients with primary Raynaud’s phenomenon (PRP). METHODS: This study enrolled patients with PRP and healthy age-matched controls. Subjects underwent evaluation of intraocular pressure (IOP), anterior chamber angle, optic disc structural and vascular parameters (measured by optical coherence tomography) and visual field. RESULTS: Twenty-four eyes were included in the control group, with a mean age of 44.20 ± 4.67 years, and 20 eyes in the PRP group, with a mean age of 43.00 ± 6.08 years (p=0.20). Inferior and nasal retinal nerve fiber layer (RNFL) thickness was lower in PRP group (104.05 ± 17.64 vs 118.67 ± 12.60μm, p=0.01; 70.05 ± 12.21 vs 79.50 ± 12.70 μm, p=0.04, respectively). Vascular density was lower in PRP group, mostly in the inner ring (15.90 ± 2.10 vs 14.20 ± 0.43 mm-1, p=0.02). No difference was found in functional parameters (p>0.05). No correlation was found between optic disc vascular density and RNFL thickness on multivariate regression analysis (p>0.05). CONCLUSION: Our findings suggest that PRP may possibly negatively affect the optic nerve, with a reduced vascular density and RNFL thickness., INTRODUÇÃO: O nosso objetivo foi avaliar os parâmetros estruturais, vasculares e funcio- nais do disco ótico em doentes com fenómeno de Raynaud primário (FRP). MÉTODOS: Este estudo incluiu pacientes com FRP e controlos saudáveis de idade equi- valente. Avaliou-se a pressão intraocular (PIO), ângulo iridocorneano, parâmetros estruturais e vasculares do disco ótico (determinados por tomografia de coerência ótica) e os campos visuais. RESULTADOS: Foram incluídos 24 olhos no grupo de Controlo, com uma idade média de 44,20 ± 4,67 anos, e 20 olhos no grupo de FRP, com uma média de idade de 43,00 ± 6,08 anos (p=0,20). A espessura nasal e inferior da camada de fibras nervosas (CFN) foi inferior no grupo de FRP (104,05 ± 17,64 vs 118,67 ± 12,60 μm, p=0,01; 7,.05 ± 12,21 vs 79,50 ± 12,70 μm, p=0,04, respetivamente). A densidade vascular foi igualmente inferior no grupo de FRP group, particularmente no anel interno (15,90 ± 2,10 vs 14,20 ± 0,43 mm-1, p=0,02). Não foram encontradas diferenças nos parâmetros funcionais (p0,05). Não se verificou uma correlação entre a densidade vascular do disco óticos e a espessura da CFN na análise de regressão multivariada (p0,05). CONCLUSÃO: Os nossos achados sugerem que a FRP pode afetar negativamente o nervo ótico, com uma menor densidade vascular e espessura da CFN face a uma população normal.

Dissertação de Mestrado em Física apresentada à Faculdade de Ciências e Tecnologia Máquinas moleculares e as suas aplicações são uma área de investigação que se tem desenvolvido bastante tendo mesmo a Academia Real Sueca de Ciências laureado os cientistas Jean-Pierre Sauvage, Sir James Fraser Stoddart e Bernard L. Feringa com o prémio Nobel de Química em 2016 "pelo design e síntese das máquinas moleculares". Muitas classes de moléculas podem ser usadas para construir estas máquinas minúsculas, como por exemplo a classe das iminas. As iminas, que normalmente são relativamente fáceis de sintetizar, podem funcionar como motores moleculares de dois ou quatro passos. Esta tese foca-se no estudo conformacional de uma imina de canforaquinona e dos complexos que esta forma com moléculas de água, usando espectroscopia rotacional. Esta imina tem dois isómeros (E e Z) e pode transitar entre os dois quando excitada com luz com um comprimento de onda bem determinado, de forma reversível, tornando-se assim um interruptor molecular. Uma caracteristica interessante desta molécula é que um isómero (Z) é mais estável na fase gasosa, enquanto o outro (E) é mais estável em solução. A espectroscopia rotacional é uma técnica experimental em que radiação de microondas é usada para investigar transições entre os níveis de energia rotacionais de uma molécula. As frequências destas transições podem ser relacionadas com a estrutura molecular através dos momentos principais de inércia da molécula. Assim, medidas do espectro rotacional de isotopólogos (muitas vezes em abundância natural) permitem-nos calcular as posições dos átomos na molécula. A espectroscopia rotacional é pois uma ferramenta incrível para o estudo da estrutura de moléculas isoladas. Os espectros de alta resolução analisados nesta tese foram obtidos com o espectrómetro de microondas, COMPACT, em Hamburgo. Para analisar um espectro rotacional é necessário ter uma previsão inicial, aproximada, para várias propriedades da molécula. Esta estimativa foi obtida com cálculos de química quântica feitos usando o programa ORCA. Foi utilizada a teoria dos funcionais da densidade com o funcional B3LYP, uma correção de dispersão entre pares de átomos com o esquema de amortecimento de Becke-Johnson (D3BJ), e a base def2-TZVP para obter a energia e geometria das moléculas bem como o seu dipolo elétrico e as suas constantes de acoplamento do quadrupolo nuclear. A análise dos espectros rotacionais permitiu identificar inequivocamente as espécies predominantes contidas no jato molecular. Para a molécula isolada, foram observadas as duas conformações sendo o isómero Z muito mais prevalente. No complexo com uma molécula de água, os dois isómeros são visíveis. No complexo com duas moléculas de água, só conseguimos observar o complexo com o isómero E. Os valores experimentais finais obtidos por refinamento do modelo para melhor ajuste aos dados experimentais diferem da previsão inicial em cerca de 1 a 2% para as constantes rotacionais e 10 a 20% para as constantes de acoplamento do quadrupolo nuclear. O uso de cálculos de teoria de perturbação de simetria adaptada (SAPT) e de análises de interações não-covalentes permitiram-nos perceber e visualizar as interações não-covalentes intramoleculares e intermoleculares e como estas afetam a estabilidade relativa dos isómeros. Para a molécula isolada, a maior estabilidade do isómero Z deve-se à presença de duas ligações de hidrogénio intramoleculares que estão ausentes no outro isómero. Nos complexo com uma molécula de água, a interação primária, que liga os monómeros, em ambos os isómeros é a mesma, uma ligação de hidrogénio entre o átomo azoto da imina e um átomo de hidrogénio da água. Assim, a estabilidade relativa observada surge como consequência de interações secundárias. Com base nesta investigação ficou claro que a espectroscopia rotacional, em combinação com cálculos de química quântica, é uma ferramenta poderosa para o estudo e elucidação da estrutura e interações de moléculas e complexos fracamente ligados. Molecular machines and their applications are an area of research which has greatly developed, with the Royal Swedish Academy of Science awarding the 2016 Nobel prize in Chemistry to Jean-Pierre Sauvage, Sir James Fraser Stoddart and Bernand L. Feringa "for the design and synthesis of molecular machines". Many classes of molecules, like for example imines, can be used to create these small machines. Imines, which are generally relatively easy to synthesize, can function as two or four step molecular motors. This thesis focuses on a conformational study of a camphorquinone imine and the complexes it forms with water molecules using rotational spectroscopy. This imine has two isomers (E and Z) and can transition between the two using when excited using light with a well defined wavelength, in a reversible manner, and as such can work as a molecular switch. An interesting property of this molecule is that while one isomer (Z) is the more stable in the gas phase, the other (E) is the more stable in solution.Rotational spectroscopy is a experimental technique where microwave radiation is used to investigate transitions between the rotational energy levels of a molecule. The transition frequencies can be related to the molecular structure through the principal moments of inertia. As such, measurements of the rotational spectra of isotopologues (many times done at natural abundance) allow us to calculate the positions of atoms in the molecule. Rotational spectroscopy is thus an incredible tool for the study of the structure of isolated molecules. The high resolution spectra used in this dissertation were obtained using the broadband microwave spectrometer, COMPACT, in Hamburg. To analyse a rotational spectrum, it is necessary to have an approximate initial guess to multiple molecular properties. This estimate was obtained using quantum chemistry calculations performed using ORCA. We used density functional theory with the B3LYP functional, an atom-pairwise dispersion correction with the Becke-Johnson damping scheme (D3BJ), and the def2-TZVP basis set to obtain the energy and geometry of the molecules as well as their electric dipole moment and nuclear quadrupole coupling constants. The analysis of the spectra allowed us to unequivocally identify the species present in the molecular jet. For the isolated molecules, we observed both conformations although the Z isomer was much more prevalent. For the complexes with one water molecule, both isomers were also visible. For the complexes with two water molecules, only the E isomer form could be observed. The experimental values obtained by refining our models to best fit the experimental data differ from our initial predictions by about 1 to 2% for the rotational constants and 10 to 20% for the nuclear quadrupole coupling constants.The use of symmetry adapted perturbation theory (SAPT) calculations and non covalent interaction analysis (NCI) allowed us to understand and visualise the intramolecular and intermolecular non-covalent interactions, and how these affect the relative stability of the isomers. For the isolated molecule, the greater stability of the Z isomer is due to the presence two intramolecular hydrogen bonds which are absent in the other isomer. In the complexes with one water molecule, the primary interaction, which binds the monomers, is the same, a hydrogen bond between the nitrogen atom of the imine and one of the hydrogen atoms of the water. As such, the observed relative stability appears as a consequence of secondary interactions. From this investigation it became clear that rotational spectroscopy, in combination with quantum chemistry calculations, is a powerful tool for the study and elucidation of structure and interactions of molecules and weakly bound complexes.

Epithelial thickness (ET) and full corneal thickness (CT) parameters were compared between right and left eyes with a non-parametric MannâWhitney test. A p-value lower than 0.05 was considered for statistical significance.Results: No eyes had topometric nor tomographic criteria for keratoconus. The min-max ET was lower in right eyes (â 2.8 μm vs â 3.5; p = 0.02). The difference between inferior and superior (I-S) octants was lower in right eyes (1.1 μm vs 1.9 μm; p = 0.03) as a result of inferotemporal thinning. The highest ET difference was registered between nasal and temporal octants and was more pronounced in the right eyes (2 μm vs 3.1 μm; p < 0.001).Conclusion: AS-OCT analyses reveal different epithelial thickness patterns between the eyes in young atopic patients, likely eye rubbers. Introduction:To report acase of pressure induced stromal keratopathy (PISK) after phacoemulsification and intraocular lens (IOL) implantation in a patient with ectasia after laser in situ keratomileusis (LASIK) previously treated with femtosecond-assisted intracorneal ring segment (ICRS). Case Report:A 53-year-old man with post-LASIK ectasia in his right (OD), underwent phacoemulsification and intraocular lens implantation two months after ICRS implantation in OD. On the first operative day, the patient complained of decreased visual acuity. His best corrected visual acuity (BCVA) was 20/80, intraocular pressure was 55mmHg and biomicroscopy revealed diffuse interface haze and corneal edema. Anterior segment optical coherence tomography (AS-OCT) showed interface fluid accumulation. Steroid drops were discontinued and anti-hypertensive drops were added. One week after surgery, his BCVA was 20/40, the cornea was totally clear, and AS-OCT showed complete fluid reabsorption. Conclusion:Although PISK usually develops early after LASIK, it can also occur different setting associated with high IOP. This report raises concerns regarding postoperative intraocular hypertension prophylaxis in eyes with previous corneal interface creation, in particular the use of corticosteroids, which are known to trigger PISK development., Introdução:Descrição de um caso clínico respeitante a queratopatia estromal induzida pelo aumento da pressão intraocular (PISK) após facoemulsificação num doente previamente submetido a implante de anel intra-estromal por ectasia pós cirurgia refrativa. Descrição do caso: Doente do sexo masculino de 53 anos com história de ectasia pós LASIK no olho direito, foi submetido a facoemulsificação e implante de lente intraocular dois meses após implante de anel intraestromal. No primeiro dia pós-operatório, apresentava queixas de diminuição da acuidade visual.A melhor acuidade visual corrigida (MAVC) para longe era 20/80, a pressão intraocular (PIO) era 55mmHg e a biomicroscopia revelou edema de córnea e a presença de haze difuso na interface. A tomografia de coerência ótica do segmento anterior (OCT-SA) confirmou a presença de líquido na interface. Os corticoesteróides foram suspensos e foi introduzida medicação anti-hipertensora. Uma semana após a cirurgia, a MAVC melhorou para 20/40, a córnea apresentava-se transparente e o OCT-SA revelou uma completa reabsorção do fluido. Conclusão:Embora a PISK normalmente se apresente no pós-operatório imediato de LASIK, pode ocorrer noutras situações clínicas associadas a elevação da PIO. O presente caso clínico realça a importância da profilaxia dos picos hipertensivos pós-operatórios em olhos com história de cirurgia que envolva a criação de uma interface na córnea, particularmente no que respeita ao uso de corticosteróides que parecem potenciar o desenvolvimento de PISK.

Introduction: Our purpose was to evaluate the effect of eye rubbing discontinuation on the epithelial thickness profile by spectral-domain AS-OCT (anterior segment optical coherence tomography) in allergic children with no tomographic sign of corneal ectasia. Methods: Right-handed boys (average age 11.2 years, 8-12 years) with history of eye rubbing and relatively normal Pentacam (Oculus; Wetzlar, Germany) exams were recruited for the study. Patients were evaluated using AS-OCT (Zeiss Cirrus 5000 HD-OCT, Carl Zeiss Meditec, Dublin, CA, USA) at the baseline and after 8 weeks of treatment. Treatment included education to avoid eye rubbing and preservative free 0.25 mg/mL topical ketotifen. Epithelial thickness (ET) and full corneal thickness (CT) parameters were compared with paired non-parametric Wilcoxon (signed-ranked) test. A p-value lower than 0.05 was considered for statistical significance. Results: Twenty-five boys completed the protocol. No eyes had tomographic criteria for keratoconus (Belin-Ambrosio Deviation Value < 1.22). Minimum epithelial location moved from the inferotemporal to superotemporal octant after treatment. Introduction: With growing prevalence, reduced quality of life, significant socioeconomic burden and a definite impact in cataract and laser vision correction surgeries, dry eye disease (DED) is currently a hot topic in ophthalmology. In recent years, several guidelines have been carried out to standardize the diagnosis and improve treatment approach. We intend to characterize current practices in Portugal, identify opportunities for improvement and delineate strategies to address them. Material and Methods: Cross-sectional online survey designed to assess the diagnostic approach and treatment of DED and made available to ophthalmologists across the country. The survey included 5 questions on ophthalmological profile of participants, 4 questions on DED diagnosis and 3 questions on DED treatment. Statistical analysis was made using SPSS version 26. Results and Discussion: One hundred twenty two ophthalmologists answered the survey (about 10% of practitioners in Portugal). A percentage of 48% observe between 20-40 patients with DED per month. A total of 67% always examine ocular surface of laser vision correction candi- dates, whereas only 30% always do it for cataract surgery patients. The most frequently identified DED symptom is foreign body sensation. The most commonly used diagnostic methods are fluo- rescein staining and tear break up time. Regarding treatment modalities, almost 50% of participants never used lacrimal plugs and very few recommended contact lenses or autologous serum. Mild DED treatment is considered to be very effective by 80% of ophthalmologists, while in severe DED by only 0.01%. 36% believe available treatment options are ineffective in severe DED. Conclusion: DED represents a high volume of patients seen in ophthalmology clinics. Our results mimic others in Europe and the United States. Overall, diagnosis and treatment practice patterns are in agreement with the current recommendations. However, there is still room for improvement. All patient prior surgery should be examined for DED, symptoms should be considered more as part of diagnosis and follow-up, and some easily available treatment options should be considered more often. Our findings also highlight the concern with treatment in severe DED, in which more effective therapies are needed., Introdução: A doença do olho seco apresenta uma prevalência crescente e impacto socioeconómico significativo, resultando em diminuição de qualidade de vida e com repercussões demonstradas em cirurgia de catarata e refrativa a laser. Nos últimos anos, várias guidelines foram realizadas para padronizar a abordagem diagnóstica e terapêutica. Pretendemos com este estudo caracterizar as práticas atuais em Portugal e identificar oportunidades de melhoria. Material e Métodos: Estudo transversal com recurso a questionário online, elabora- do para avaliar a abordagem diagnóstica e tratamento do olho seco por oftalmologistas do setor público e privado em Portugal. O questionário incluía 5 perguntas sobre o perfil dos participantes, 4 perguntas sobre o diagnóstico e 3 sobre o tratamento da doença do olho seco. A análise estatísti- ca foi realizada com recurso a versão 26 do SPSS. Resultados e Discussão: Um total de 122 oftalmologistas responderam ao questionário. Dos inquiridos, 48% observa entre 20 e 40 doentes com olho seco por mês. Avaliam sempre a superfície ocular em candidatos a cirurgia refrativa a laser 67%, enquanto apenas 30% realizam essa avaliação na cirurgia de catarata. O sintoma de olho seco mais frequentemente identificado é a sensação de corpo estranho. Os métodos diagnósticos complementares mais utilizados são a coloração com fluoresceína e o TBUT. Quase 50% nunca utilizou plugs lacrimais e um menor número utilizou lentes de contato e soro autólogo. A maioria dos oftalmologistas (80%) considera a terapêutica para olho seco ligeiro muito eficaz. Por outro lado, apenas 0,01% considera a terapêu- tica muito eficaz no olho seco grave e 36% afirma que a mesma é ineficaz. Conclusão: O olho seco afeta um grande número de doentes observados em consulta de oftalmologia. Os nossos resultados são semelhantes a outros estudos na Europa e Estado Unidos e, em geral, estão de acordo com as recomendações atuais. No entanto, ainda há a necessidade de melhorar. Todos os pacientes antes da cirurgia devem ser avaliados para olho seco e algumas opções de tratamento devem ser consideradas mais frequentemente (por exemplo, plugs lacrimais).Destaca-se também a dificuldade no tratamento de doença grave, sugerindo que terapias mais eficazes são necessárias.

Apresentamos um caso clínico de hipertensão intracraniana (HIC) numa criança com síndrome de Turner sob tratamento com hormona do crescimento (HC). Criança de 14 anos com síndrome de Turner sob terapêutica hormonal com estrogénio e hormona de crescimento (1,6 mg/dia) apresentou-se no serviço de urgência por diminuição da acuidade visual. A fundoscopia revelou edema bilateral da papila, confirmada por tomografia de coerência ótica (OCT). A tomografia computorizada e a ressonância magnética cerebrais não revelaram quaisquer alterações. A criança foi referenciada ao departamento de Neuropediatria. Realizou-se punção lombar que revelou aumento da pressão intracraniana e, desta forma, fez-se o diagnóstico de HIC. Optou-se pela suspensão da hormona do crescimento. A hipertensão intracraniana é uma entidade rara na população pediátrica. Há uma relação causal entre o HIC, o tratamento com hormona do crescimento e a síndrome de Turner. Do nosso conhecimento, é o primeiro caso em Portugal que descreve esta associação. Dado que a HIC pode levar a perda irreversível da acuidade visual, todas as crianças sob terapêutica hormonal com hormona do crescimento devem ser avaliadas de forma rotineira por oftalmologista., Revista Sociedade Portuguesa de Oftalmologia, Vol. 45 N.º 2 (2021)

Best-corrected distance (CDVA) and uncorrected distance visual acuity (UDVA), uncorrected intermediate visual acuity (UIVA) at 66cm, uncorrected near visual acuity (UNVA) at 40cm and defocus curve were assessed. Patients-reported visual function was inquired by Catquest-9SF. The presence of photic phenomena was evaluated. A p-value lower than 0.05 was considered for statistical significance.Results: The mean IOL power was +21.90D in LuxSmart group and +22.30D in Akreos. Monocular UDVA (p = 0.32) and CDVA (p = 0.52) did not differ between groups. The average binocular UIVA (0.18 ± 0.12 logMAR vs. 0.30 ± 0.13 logMAR, p < 0.001) and UNVA (0.38 ± 0.14 logMAR vs. 0.44 ± 0.17 logMAR, p = 0.02) were higher in LuxSmart IOL group. No patients reported disabling photic phenomena in either group.Conclusion: This study shows that new LuxSmart EDOF IOL achieved higher performance for intermediate and near vision compared with a conventional monofocal IOL, without increasing the risk of dysphotopsias. LuxSmart may be an attractive and safe option for patients who desire spectacle independence for distance and intermediate vision after cataract surgery.Keywords: cataract, presbyopia, IOL, EDOF

The average ET and CT were assessed using Cirrus Review Software with predefined concentric corneal ring-shaped zones. Specific regions of ET (central, superior, inferior, temporal, nasal, superonasal, inferotemporal, superotemporal and inferonasal) were also assessed. The mean ET, the mean CT and the difference of ET in corresponding octants were compared by gender. Correlations between central epithelial thickness (CET), age and refractive error were evaluated.Results and Discussion: The average ET was lower in the peripheric zones, whereas the average CT was higher. ET was thinner in the superior area than in the inferior (p< 0.05). ET was thicker in boys than in girls (p< 0.05), but CT did not differ. CET was not correlated with older age or refractive error.Conclusion: Optical coherence tomography analysis of ET reveals that it is thinner in the periphery, where the CT is thicker. Unlike CT, ET seems to be influenced by gender. ET profile proved to be a useful tool in keratoconus diagnosis and subclinical keratoconus detection in adults. Purpose: Retinal folds (RF) are an underestimated complication of rhegmatogenous retinal detachment (RRD) surgery. Our aim was to report a case of spontaneous resolution of a full-thickness RF with foveal involvement following an uncomplicated pars plana vitrectomy (PPV) with intraocular gas tamponade for a superior on-macula RRD. Materials and Methods: Case report. Results: A 61-year old male underwent PPV with fluid-air exchange and intraocular gas tamponade (SF6) in the left eye for a superior on-macula RRD with vitreous hemorrhage. At the end of surgery, the retina was attached with very marginal subretinal fluid. He assumed face down positioning immediately after surgery, followed by right cheek-to-pillow positioning a few hours later. One week later, the retina remained attached but the patient had developed a mildly symptomatic full-thickness RF with foveal involvement confirmed by optic coherence tomography (OCT). A conservative approach was carried out, with spontaneous progressive anatomical and functional improvement. Two months after surgery, there was only a residual RF on fundus examination and OCT and autofluorescence were unremarkable. The patient was asymptomatic and best corrected visual acuity was 20/25. Conclusion: Full-thickness macular folds following RRD surgery may be present even after on macula RRD repair. Our case report illustrates that they may have a spontaneous regression with complete functional recovery, in accordance with a few other publications on this topic. As such, conservative management might be indicated even in full-thickness RF if the patient has few complaints and a reasonable visual function. O nosso objetivo foi descrever um caso de resolução espontânea de uma PR de espessura completa secundária a uma vitrectomia via pars plana (VVPP) não complicada com tamponamento intraocular com gás por DRR superior. Materiais e Métodos: Caso clínico. Resultados: Um homem de 61 anos foi submetido a uma VVPP com tamponamento intraocular com SF6 por um DRR superior com mácula-on com hemovítreo no olho esquerdo. No final da cirurgia, a retina estava aplicada com uma quantidade residual de líquido subretiniano. O paciente assumiu o posicionamento de decúbito ventral imediatamente após a cirurgia, seguido de decúbito lateral direito dentro de algumas horas. Uma semana depois, a retina mantinha-se aplicada, contudo tinha-se desenvolvido uma PR de espessura completa discretamente sintomática com envolvimento foveal confirmado por tomografia de coerência ótica (OCT). Adotou-se uma abordagem conservadora, com melhoria funcional e anatómica espontânea progressiva. Dois meses após a cirurgia, detetava-se apenas uma PR residual na fundoscopia, e o OCT e autofluorescência não apresentavam alterações foveais. O doente apresentava-se assintomático e com melhor acuidade visual corrigida de 20/25. Conclusões: PR de espessura completa podem ocorrer mesmo após cirurgia de DRR com mácula-on. O nosso caso clínico demonstrou que estas podem ter uma regressão espontânea com recuperação functional completa, em concordância com outras publicações neste âmbito. Assim, a abordagem conservadora pode estar indicada mesmo em PR de espessura completa em casos pouco sintomáticos e com uma função visual razoável. 49. Topical Cyclosporine in Severe Steroid-Dependent Phlyctenular Keratoconjunctivitis associated with Ocular Rosacea

Introduction: Phlyctenular keratoconjunctivitis is an uncommon and often misdiagnosed ocular immunological disorder, secondary to ocular or systemic illness, that usually responds well to topical steroids. In a few cases, it can be refractory or steroid dependent, hindering treatment. Case Report: A healthy 12-year-old dark skin female without prior ocular or systemic disease presented in our department with a corneal ulcer and centripetal corneal neovascularization in the left eye and phlyctenules in both eyes, without blepharitis. Testing for different possible inciting agents, such as chlamydia and tuberculosis, was negative. During the follow-up, the patient presented episodes of chalazia and facial pustules and the diagnosis of bilateral phlyctenular keratoconjunctivitis secondary to recurrent ocular rosacea was presumed. As soon as the corneal ulcer was completely healed, the patient started topical corticosteroid treatment with clinical improvement. After several failed attempts of steroids tapering, topical cyclosporine was added to therapy, enabling corticosteroid discontinuation. Discussion: Phlyctenular keratoconjunctivitis is frequently caused by a hypersensitivity reaction to microorganisms, such as staphylococcus species and mycobacterium tuberculosis, but it can be also associated with ocular rosacea without evidence of other inciting agents. The progression of phlyctenulosis to severe complications, as corneal scarring or even perforation, can compromise visual acuity at an early age and for that reason it is important to find and treat any inciting agent for the successful management of this disease. Even with adequate treatment for ocular rosacea with steroids, side effects of long-term therapy to control recurrent ocular inflammation grants topical cyclosporine an important role in disease management., Revista Sociedade Portuguesa de Oftalmologia, Vol. 44 N.º 4 (2020)
