Abstract The Brazilian Society of Angiology and Vascular Surgery has set up a committee to provide new evidence-based recommendations for patient care associated with chronic venous insufficiency. Topics were divided in five groups: 1. Classification, 2. Diagnosis, 3. Conservative or non-invasive treatment, 4. Invasive treatment and 5. Treatment of small vessels. This last series is closely related to the activities of Brazilian angiologists and vascular surgeons, who are heavily involved in the treatment of small superficial veins. These guidelines are intended to assist in clinical decision-making for attending physicians and health managers. The decision to follow a guideline recommendation should be made by the responsible physician on a case-by-case basis taking into account the patient's specific condition, as well as local resources, regulations, laws, and clinical practice recommendations.

However, little is known about how common genetic variation affects ROS-mediated antifungal host defense. OBJECTIVES: To evaluate how transtibial amputation (TT) affects bodyweight distribution, voluntary knee joint position sense (JPS), and quadriceps (QUA) and hamstrings (HAM) strength in prosthetized patients. METHODS: Only TT patients who had been prosthetized for more than one year were included, and an age-paired able-bodied group was used as control. The participants stood on force plates with their eyes open to measure bodyweight distribution between the limbs. Knee voluntary JPS was assessed by actively reproducing a set of given arbitrary joint angles using a video analysis approach, and QUA and HAM strength were assessed isometrically with a hand-held dynamometer. RESULTS: Sixteen TT subjects (age: 39.4±4.8 years) and sixteen age-paired control subjects (age: 38.4±4.3 years) participated in the study. Objective. The objective of this study was to determine the accuracy of 3 clinical tests (lateral step [LS], tandem gait [TG], and single-leg stance [SS]) in identifying older women with reduced hip abductor muscle strength and to determine the post-test probability of each test and of their combination in changing the certainty of diagnosis. Methods. In this cross-sectional study, a total of 123 older women received clinical testing to obtain the variables LS height, time for TG execution, percentage of errors in TG, and time of permanence on SS and were tested for isometric hip abductor peak torque using an isokinetic dynamometer. Only the dominant lower limb was evaluated. Multiple linear regression analysis with adjustment was performed to determine the association among variables, followed by the receiver operating characteristic curve to identify clinical variables that can discriminate older women with reduced abductor muscle strength. Post-test probability was then calculated based on the receiver operating characteristic curve data. Results. Although the 4 clinical variables showed correlation with abductor peak torque, only LS and TG time were able to discriminate reduced abductor muscle strength with low accuracy (area under the curve was between 0.5 and 0.7). However, the combination of LS and TG time increased post-test probability from 47% (prevalence of weakness in the population) to 76% when both tests were positive and reduced it from 47% to 18% when both tests were negative. Conclusion. The combination of the LS test and TG time is useful for the indirect assessment of hip abductor strength in community-dwelling older women. Impact. Hip abductor muscle strength is extremely important for function in older women. Background: Reactivation of latent human cytomegalovirus (CMV) in patients undergoing allogeneic stem-cell transplantation (HSCT) predisposes to several clinical complications and is therefore a major cause of morbidity and mortality. Although pentraxin-3 (PTX3) has been previously described to bind both human and murine CMV and mediate several host antiviral mechanisms, whether genetic variation in the PTX3 locus influences the risk of CMV infection is currently unknown.Methods: To dissect the contribution of genetic variation within PTX3 to the development of CMV infection, we analyzed described loss-of-function variants at the PTX3 locus in 394 recipients of HSCT and their corresponding donors and assessed the associated risk of CMV reactivation.Results: We report that the donor, but not recipient, h2/h2 haplotype in PTX3 increased the risk of CMV reactivation after 24 months following transplantation, with a significant effect on survival. Among recipients with h2/h2 donors, CMV seropositive patients as well as those receiving grafts from unrelated donors, regardless of the CMV serostatus, were more prone to develop viral reactivation after transplantation. Most importantly, the h2/h2 haplotype was demonstrated to display an influence toward risk of CMV reactivation comparable to that conferred by the unrelated status of the donor alone.Conclusions: Our findings demonstrate the important contribution of genetic variation in donor PTX3 to the risk of CMV reactivation in patients undergoing HSCT, highlighting a promising prognostic value of donor PTX3 to predict risk of CMV reactivation in this clinical setting.

Propõe-se, neste artigo, diferentes versões da metaheurística GRASP para solucionar o Problema de Roteamento de Veículos. Para a sua resolução diferentes algoritmos aplicados à fase de construção e de busca local do GRASP são propostos, com posterior realização de refinamento visando melhorar a qualidade da solução buscada. O produto final do algoritmo possibilitará minimizar custos de logística, tempos de entrega, melhores rotas e redução da quantidade total de veículos necessários para o oferecimento do serviço.

Solid lipid nanoparticles (SLNs) gained greater attention as a drug delivery system when in 1991 M€uller developed them [1]. This promising drug carrier system is at the interface in the preexisting lipid systems (emulsions and liposomes) and polymeric nanoparticle systems. Lipid nanoparticles, known as SLNs or nanostructured lipid carriers (NLCs), have specific features of structure and composition, showing benefits in comparison to conventional formulations [2]. Various controlled drug delivery systems like polymer-based controlledrelease systems, hydrogels, as well as nanoand microparticles have been introduced in recent years in order to improve solubility, stability and bioavailability of poorly water-soluble drugs. In this context, lipid nanoparticles offer attractive and ideal properties for drug or gene delivery.

Hypertensive disorders of pregnancy (HDP) are a leading cause of maternal and perinatal morbimortality. Dietetic, phenotypic, and genotypic factors influencing HDP were analyzed during a nutrigenetic trial in Rio de Janeiro, Brazil (2016–2020). Pregnant women with pregestational diabetes mellitus (n = 70) were randomly assigned to a traditional or DASH diet group. Systolic blood pressure (SBP) and diastolic blood pressure (DBP) were measured during prenatal visits and HDP were diagnosed using international criteria. Phenotypic data were obtained from medical records and personal interviews. Genotyping for FTO and ADRB2 polymorphisms used RT-PCR. Linear mixed-effect models and time-to-event analyses were performed. The variables with significant effect on the risk for progression to HDP were: black skin color (adjusted hazard ratio [aHR] 8.63, p = 0.01), preeclampsia in previous pregnancy (aHR 11.66, p < 0.01), SBP ≥ 114 mmHg in the third trimester (aHR 5.56, p 0.04), DBP ≥ 70 mmHg in the first trimester (aHR 70.15, p = 0.03), mean blood pressure > 100 mmHg (aHR 18.42, p = 0.03), and HbA1c ≥ 6.41% in the third trimester (aHR 4.76, p = 0.03). The purpose of this investigation was to compare ankle functional performance and ankle range of motion (ROM) between practitioners of resistance exercise (RE) with free-weights versus machines. Twenty-five men participated in this study. They were separated into two groups: (a) Free-weights; and (b) Machines. All subjects practiced regularly RE 5.3±0.7 d∙wk-1 and low aerobic training of 1.2±0.5 d∙wk-1 with a total time volume of 254.9±9.4 min∙wk-1. ROM measurements were taken in both ankles with a digital goniometer. Active ankle-dorsiflexion and plantar flexion range of motion were measured with subjects lying prone with an extended knee on a standard treatment table. The rising on the heel and the rising on toes were used to assess endurance of the ankle dorsiflexor and plantar flexor muscles, respectively. Since there are some concerns about the effectiveness of highly active antiretroviral therapy in developing countries, we compared the initial combination antiretroviral therapy with zidovudine and lamivudine plus either nelfinavir or efavirenz at a university-based outpatient service in Brazil. This was a retrospective comparative cohort study carried out in a tertiary level hospital. A total of 194 patients receiving either nelfinavir or efavirenz were identified through our electronic database search, but only 126 patients met the inclusion criteria. Patients were included if they were older than 18 years old, naive for antiretroviral therapy, and had at least 1 follow-up visit after starting the antiretroviral regimen. Fifty-one of the included patients were receiving a nelfinavir-based regimen and 75 an efavirenz-based regimen as outpatients. Antiretroviral therapy was prescribed to all patients according to current guidelines. By intention-to-treat (missing/switch = failure), after a 12-month period, 65% of the patients in the efavirenz group reached a viral load

OBJECTIVE: To compare the accuracy of positioning of the femoral tunnel in reconstructing the anterior cruciate ligament by means of 2 techniques: tibial tunnel and anteromedial portal. METHOD: Femoral tunnels were drilled in 20 knees from human cadavers by means of arthroscopy. Group I had the femoral tunnel drilled via a trans-tibial tunnel, and Group II via the anteromedial portal. Four variables were measured: A) posterior wall thickness; B) tunnel positioning at the notch; C) tunnel inclination in relation to the femoral axis; and D) distance between the wire guide exit and the lateral epicondyle. The aim of this experiment was to study the prevalence of sera agglutinins anti-leptospira in bulls in Goiânia Micro-Region. Sixty farms were chosen randomly in according to sorovars presented and the tax of prevalence of them. The correlation of the prevalence of the leptospirosis were establish with the type of herd (beef or dairy) and the occurrence of abortions in farms. One hundred forty samples bulls from “Goiânia Micro-Region” were analyzed (MG) regarding 60 properties. The sera reagents prevalence was 74.28% and . presented titles with variation of dilution from 1:100 to 1:800. The prevalence of the sera varieties, presents in this “Goiania Micro-Region”, was the following: wolffi (19.23%); hardjo (15.38%); djasiman and grippotyphosa (5.76%); shermani (4.80%); patoc (1.92%); andamana, castellonis, copenhageni, hebdomadis, sentot and tarassovi (0.96%). According to these results, it was possible to conclude that leptospirosis is an endemic agent in this part of Goiás State and there were not any relationship with production system or abortions and prevalence. Os objetivos deste experimento foram estudar a prevalência de soroaglutininas antileptospira em reprodutores bovinos na microrregião de Goiânia trabalhando com propriedades escolhidas de forma aleatória e quais os sorovares presentes e a taxa de prevalência dos mesmos. Estabelecer a correlação da prevalência com a aptidão do rebanho e a ocorrência de abortos nas propriedades. Foram analisadas 140 amostras da microregião de Goiânia (MG) referentes a 60 propriedades. A prevalência de sororeagentes foi 74,28% e apresentaram títulos com variação de diluição de 1:100 a 1:800. A prevalência dos sorovares, presentes na microrregião de Goiânia, foi a seguinte: wolffi (19,23%); hardjo (15,38%); djasiman e grippotyphosa (5,76%); shermani (4,80%); patoc (1,92%); andamana, castellonis, copenhageni, hebdomadis, sentot e tarassovi (0,96%).Com os resultados obtidos foi possível concluir que o agente está endemicamente distribuído na região e não houve relação entre a prevalência e a aptidão do rebanho ou a ocorrência de abortos. A doença de Behçet consiste em uma vasculite sistêmica que acomete veias, artérias e capilares, mas predominantemente pequenos vasos. Raramente há o envolvimento de veias e artérias de médio e grande calibre, sendo as artérias menos acometidas que as veias. No presente trabalho, os autores apresentam um relato de caso de aneurisma da artéria poplítea em um paciente com a doença de Behçet descompensada. Discutem-se os aspectos clínicos e radiológicos e o manejo cirúrgico dos pacientes com essa doença inflamatória sistêmica que apresentam peculiaridades importantes.Behçet's disease consists of a systemic vasculitis that affects veins, arteries and capillaries, but predominantly small vessels. Veins and medium- and large-caliber arteries are rarely involved; however, arteries are less involved than veins. In this paper the authors present a case of popliteal artery aneurysm in a patient with decompensated Behçet's disease. We discuss clinical, radiological and surgical aspects of patients with this inflammatory systemic disease who present important peculiarities.

Summary Background Among asymptomatic patients with severe carotid artery stenosis but no recent stroke or transient cerebral ischaemia, either carotid artery stenting (CAS) or carotid endarterectomy (CEA) can restore patency and reduce long-term stroke risks. However, from recent national registry data, each option causes about 1% procedural risk of disabling stroke or death. Comparison of their long-term protective effects requires large-scale randomised evidence. Methods ACST-2 is an international multicentre randomised trial of CAS versus CEA among asymptomatic patients with severe stenosis thought to require intervention, interpreted with all other relevant trials. Patients were eligible if they had severe unilateral or bilateral carotid artery stenosis and both doctor and patient agreed that a carotid procedure should be undertaken, but they were substantially uncertain which one to choose. Patients were randomly allocated to CAS or CEA and followed up at 1 month and then annually, for a mean 5 years. Procedural events were those within 30 days of the intervention. Intention-to-treat analyses are provided. Analyses including procedural hazards use tabular methods. Analyses and meta-analyses of non-procedural strokes use Kaplan-Meier and log-rank methods. The trial is registered with the ISRCTN registry, ISRCTN21144362. Findings Between Jan 15, 2008, and Dec 31, 2020, 3625 patients in 130 centres were randomly allocated, 1811 to CAS and 1814 to CEA, with good compliance, good medical therapy and a mean 5 years of follow-up. Overall, 1% had disabling stroke or death procedurally (15 allocated to CAS and 18 to CEA) and 2% had non-disabling procedural stroke (48 allocated to CAS and 29 to CEA). Kaplan-Meier estimates of 5-year non-procedural stroke were 2·5% in each group for fatal or disabling stroke, and 5·3% with CAS versus 4·5% with CEA for any stroke (rate ratio [RR] 1·16, 95% CI 0·86–1·57; p=0·33). Combining RRs for any non-procedural stroke in all CAS versus CEA trials, the RR was similar in symptomatic and asymptomatic patients (overall RR 1·11, 95% CI 0·91–1·32; p=0·21). Interpretation Serious complications are similarly uncommon after competent CAS and CEA, and the long-term effects of these two carotid artery procedures on fatal or disabling stroke are comparable. - Abstract
PURPOSE: Today, male and female adult and pediatric cancer patients, individuals transitioning between gender identities, and other individuals facing health extending but fertility limiting treatments can look forward to a fertile future. This is, in part, due to the work of members associated with the Oncofertility Consortium. METHODS: The Oncofertility Consortium is an international, interdisciplinary initiative originally designed to explore the urgent unmet need associated with the reproductive future of cancer survivors. As the strategies for fertility management were invented, developed or applied, the individuals for who the program offered hope, similarly expanded. As a community of practice, Consortium participants share information in an open and rapid manner to addresses the complex health care and quality-of-life issues of cancer, transgender and other patients. To ensure that the organization remains contemporary to the needs of the community, the field designed a fully inclusive mechanism for strategic planning and here present the findings of this process. RESULTS: This interprofessional network of medical specialists, scientists, and scholars in the law, medical ethics, religious studies and other disciplines associated with human interventions, explore the relationships between health, disease, survivorship, treatment, gender and reproductive longevity. CONCLUSION: The goals are to continually integrate the best science in the service of the needs of patients and build a community of care that is ready for the challenges of the field in the future.

This study presents an in–depth analysis and strategic planning techniques for approaching a situation or achieving progress related to information technology services of Nueva Ecija University of Science and Technology, particularly the San Isidro Campus. As an educational organization and one in the region excellence providers in education, the University has so much vital information to manage and services to offer to provide the student's needs through the Management Information System department, application of IT services as a competing instrument to combat tedious tasks such as online admission, grade release, enrollment information, and online learning, university announcement, and possible students’ internship. The objective of this study is to determine the influence of information technology in day–to–day activities of the organization and strategy applied to streamline processes more conveniently. The researchers develop a conceptual model that would analyze the IT services' impact by identifying its strengths, weaknesses, opportunities, and threats and cleverly act on it using the TOWS Matrix in generating strategic decisions. The overall results were beneficial to the University and MIS as a basis for formulating competitive strategies and integrating it in an extended development plan.
