Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) The psychoeducational programs that are offered to caregivers who assist a family member with a major neurocognitive disorder (NCD, or dementia) typically have only modest effects on the mental health of the participants. The programs with the strongest effects are based on the cognitive-behavior therapy model of behavior change, and address context-specific demands. Two programs with these same characteristics are iSupport, a program prepared for the World Health Organization (WHO), and ComTato, a Brazilian program. Given their potential to contribute to caregivers’ mental health, it is important to obtain further evidence about the effects of these programs. With respect to iSupport, an on-line, self-help program, the objective was to evaluate an adapted version of the program, for use in Brazil (Studies 1 and 2). With respect to the ComTato program, offered via synchronous and individualized sessions, the objective was to evaluate the effects of this program, when offered on-line (Studies 3 and 4). In Study 1, the iSupport material, written in English, was translated into Brazilian Portuguese and the text was revised by experts in Gerontology. Next, nine (ex)caregivers assisting a family member with a major NCD and six healthcare professionals working with the elderly examined the material individually and then discussed their suggestions for the program in focus groups. These suggestions were analyzed by the research team for this project, and then approved by the WHO, resulting in the first Brazilian version of this program. In Study 2, the acceptability and usability of the iSupport-BR program were evaluated by five caregivers of people with a major NCD, after they used the program via a digital platform. Their evaluations were positive, and further changes to the program were made in response to suggestions they made that were approved by the research team. In Study 3, to examine the effects of the ComTato program, an experimental, multiple-baseline research design was used. Participants were 22 family caregivers who assisted an elderly person with Alzheimer’s disease (AD). Seven caregivers started in the control group (CG), then entered the intervention group (IG); thus there were 15 caregivers in the IG, and 14 in the CG. Evaluations were made of the caregivers’ knowledge about AD and other topics related to the socioemotional demands of caregiving, burden, coping strategies, quality of the caregiver-older person relationship, and perceptions of the effects and acceptability of the intervention program. Based on comparisons of the pre and posttest scores for the IG caregivers, differences were found for most of the measures, with effect sizes that were moderate to large, indicating important benefits for the participants of the ComTato program. These benefits were confirmed based on a comparison of the changes observed for the caregivers in the IG and the CG. In Study 4, effects of the ComTato program using the new, on-line format were compared with results from previous studies, using an in-person format. The caregivers who received in-person visits had larger gains in knowledge for some concepts, compared to the caregivers who participated on-line. However, no differences were found for the socioemotional variables, suggesting that both formats are effective. The positive effects of these two programs, iSupport BR and ComTato, increase the evidence that supports the possibility of offering these programs to caregivers assisting a family member with a major NCD. In addition, these results add examples and increase our knowledge about the adaptation of intervention programs in prevention and promotion of mental health, in Brazil. Programas psicoeducativos para cuidadores que assistem pessoas com um transtorno neurocognitivo maior (TNC maior, ou demência) geralmente apresentam efeitos modestos sobre a saúde mental dos participantes. As intervenções com efeitos maiores são fundamentadas no modelo de mudança de comportamento da terapia cognitivo-comportamental e abordam demandas específicas desse contexto. Dois programas com essas características são o iSupport, da Organização Mundial da Saúde (OMS), e o ComTato, um programa brasileiro. Diante do potencial em contribuírem para a saúde mental de cuidadores, é importante obter maiores evidências a respeito dos efeitos desses programas. Em relação ao iSupport, que é um programa online e autodidático, o objetivo geral foi adaptar a versão original para seu uso no Brasil (Estudos 1 e 2). Quanto ao ComTato, que envolve atendimentos síncronos e individuais, o objetivo principal foi avaliar seus efeitos, quando oferecido online (Estudos 3 e 4). No Estudo 1, o material do iSupport, escrito em inglês, foi traduzido para o português brasileiro e a verificação do texto foi realizada por experts em gerontologia. Em seguida, nove (ex)cuidadores de familiares com um TNC maior e seis profissionais que atuam no contexto da saúde de pessoas idosas examinaram o material individualmente e depois discutiram suas sugestões para adequações do programa, em grupos focais. As sugestões foram analisadas pela equipe de pesquisadores e aprovadas pela OMS, gerando uma primeira versão brasileira do programa. No Estudo 2, a usabilidade e aceitabilidade do iSupport-BR foram avaliadas por cinco cuidadores de pessoas com um TNC maior, que acessaram o programa via uma plataforma digital. As avaliações foram positivas e alterações adicionais do programa foram feitas, para contemplar sugestões, aprovadas pela equipe de pesquisadores. No Estudo 3, sobre os efeitos do programa ComTato, participaram 22 cuidadores familiares de pessoas com doença de Alzheimer (DA), a partir de um delineamento experimental e de linha de base múltipla. Sete cuidadores começaram no grupo controle (GC) e depois entraram no grupo de intervenção (GI); portanto, o GI foi formado por 15 cuidadores e o GC por 14 cuidadores. Foram avaliados: seus conhecimentos sobre DA e temas relacionados às demandas socioemocionais do cuidar, sobrecarga, uso de estratégias de coping, qualidade da relação cuidador-idoso e suas percepções dos efeitos e da aceitabilidade do programa. Ao comparar os resultados do pré-teste e do pós-teste para os cuidadores do GI, foram observadas diferenças na maior parte das medidas, com tamanho de efeito moderado ou grande, indicando benefícios importantes para os participantes do programa ComTato. Esses benefícios foram confirmados na comparação entre as mudanças observadas por parte dos cuidadores do GI e do GC. No Estudo 4, foram comparados os efeitos do programa ComTato, no novo formato online, com resultados de pesquisas anteriores, quando foi usado em formato presencial. Os cuidadores atendidos presencialmente apresentaram ganhos maiores na aprendizagem de alguns conceitos, em comparação com os cuidadores atendidos online. No entanto, não foram observadas diferenças nas variáveis socioemocionais, sugerindo que ambos os formatos são efetivos. Os resultados positivos para os dois programas, iSupport-BR e ComTato, acrescentam evidências que fundamentam a oferta desses programas para cuidadores familiares de pessoas com um TNC maior. Além disso, aumentam os exemplos e conhecimentos sobre a adaptação de programas de intervenção na área de prevenção e promoção em saúde mental, no Brasil. Objectives: Internet-based interventions can help carers of people living with dementia to cope with care-related challenges and can help improve their wellbeing. This systematic review aimed at investigating the criteria of usability and acceptability of self-guided internet-based interventions for family carers of people living with dementia. Method: Searches were conducted on PubMed, Virtual Health Library Regional Portal (Americas), MEDLINE, PsycINFO, Scopus, and Cochrane. Studies published up to December 2019, in English, Portuguese, or Spanish, were eligible. We followed the definition/criteria from ISO ISO-9241-11 for usability (efficiency, effectiveness, and satisfaction) and acceptability (barriers for using and utility). Methodological quality was evaluated using specific tools according to each study design. Results: Ten studies were included, all of which had high methodological quality. Carers of people living with dementia indicated that internet-based interventions were mostly effective, efficient, and satisfactory. They considered these to be informative, relevant, and functional, highlighting the utility and intention of using the resource in the future. The high heterogeneity in the terms and methods used to evaluate usability and acceptability hindered cross-study comparisons, however internet-based interventions were considered useful and acceptable by most carers. Conclusion: Future research should consider expanding the criteria of usability and acceptability to better reflect the needs of this population. [ABSTRACT FROM AUTHOR]

The World Health Organization developed the "iSupport for Dementia" program for family caregivers of people with dementia., Objective: To explore studies on adaptation, randomized clinical trial protocols, and preliminary results of iSupport by unpaid caregivers of people living with dementia in different countries., Methods: Systematic review., Results: Ten cultural adaptation studies, eight randomized clinical trial protocols, and two preliminary results were included. Adaptation studies showed adjustments in terminology, design, and additional resources. Clinical trial protocols included burden as the primary outcome, and baseline, three months of intervention, and follow-up after six months. Studies with preliminary results found positive effects on the mental health and well-being of caregivers after using the program., Conclusion: iSupport is an online program of the World Health Organization in response to dementia in implementation in different countries., Competing Interests: Disclosure: The authors report no conflicts of interest.
