Methane (CH4) emissions from natural landscapes constitute roughly half of global CH4 contributions to the atmosphere, yet large uncertainties remain in the absolute magnitude and the seasonality of emission quantities and drivers. Eddy covariance (EC) measurements of CH4 flux are ideal for constraining ecosystem-scale CH4 emissions due to quasi-continuous and high-temporal-resolution CH4 flux measurements, coincident carbon dioxide, water, and energy flux measurements, lack of ecosystem disturbance, and increased availability of datasets over the last decade. Here, we (1) describe the newly published dataset, FLUXNET-CH4 Version 1.0, the first open-source global dataset of CH4 EC measurements (available at https://fluxnet.org/data/fluxnet-ch4-community-product/, last access: 7 April 2021). FLUXNET-CH4 includes half-hourly and daily gap-filled and non-gap-filled aggregated CH4 fluxes and meteorological data from 79 sites globally: 42 freshwater wetlands, 6 brackish and saline wetlands, 7 formerly drained ecosystems, 7 rice paddy sites, 2 lakes, and 15 uplands. Then, we (2) evaluate FLUXNET-CH4 representativeness for freshwater wetland coverage globally because the majority of sites in FLUXNET-CH4 Version 1.0 are freshwater wetlands which are a substantial source of total atmospheric CH4 emissions; and (3) we provide the first global estimates of the seasonal variability and seasonality predictors of freshwater wetland CH4 fluxes. Our representativeness analysis suggests that the freshwater wetland sites in the dataset cover global wetland bioclimatic attributes (encompassing energy, moisture, and vegetation-related parameters) in arctic, boreal, and temperate regions but only sparsely cover humid tropical regions. Seasonality metrics of wetland CH4 emissions vary considerably across latitudinal bands. In freshwater wetlands (except those between 20g g€¯S to 20g g€¯N) the spring onset of elevated CH4 emissions starts 3g€¯d earlier, and the CH4 emission season lasts 4g€¯d longer, for each degree Celsius increase in mean annual air temperature. On average, the spring onset of increasing CH4 emissions lags behind soil warming by 1 month, with very few sites experiencing increased CH4 emissions prior to the onset of soil warming. In contrast, roughly half of these sites experience the spring onset of rising CH4 emissions prior to the spring increase in gross primary productivity (GPP). The timing of peak summer CH4 emissions does not correlate with the timing for either peak summer temperature or peak GPP. Our results provide seasonality parameters for CH4 modeling and highlight seasonality metrics that cannot be predicted by temperature or GPP (i.e., seasonality of CH4 peak). FLUXNET-CH4 is a powerful new resource for diagnosing and understanding the role of terrestrial ecosystems and climate drivers in the global CH4 cycle, and future additions of sites in tropical ecosystems and site years of data collection will provide added value to this database. We describe a new coordination activity and initial results for a global synthesis of eddy covariance CH4 flux measurements.

Understanding central auditory processing critically depends on defining underlying auditory cortical networks and their relationship to the rest of the brain. We addressed these questions using resting state functional connectivity derived from human intracranial electroencephalography. Mapping recording sites into a low-dimensional space where proximity represents functional similarity revealed a hierarchical organization. At a fine scale, a group of auditory cortical regions excluded several higher-order auditory areas and segregated maximally from the prefrontal cortex. On mesoscale, the proximity of limbic structures to the auditory cortex suggested a limbic stream that parallels the classically described ventral and dorsal auditory processing streams. Identities of global hubs in anterior temporal and cingulate cortex depended on frequency band, consistent with diverse roles in semantic and cognitive processing. On a macroscale, observed hemispheric asymmetries were not specific for speech and language networks. Theories of consciousness suggest that brain mechanisms underlying transitions into and out of unconsciousness are conserved no matter the context or precipitating conditions. We compared signatures of these mechanisms using intracranial electroencephalography in neurosurgical patients during propofol anesthesia and overnight sleep and found strikingly similar reorganization of human cortical networks. We computed the "effective dimensionality" of the normalized resting state functional connectivity matrix to quantify network complexity. Effective dimensionality decreased during stages of reduced consciousness (anesthesia unresponsiveness, N2 and N3 sleep). These changes were not region-specific, suggesting global network reorganization. When connectivity data were embedded into a low-dimensional space in which proximity represents functional similarity, we observed greater distances between brain regions during stages of reduced consciousness, and individual recording sites became closer to their nearest neighbors. These changes corresponded to decreased differentiation and functional integration and correlated with decreases in effective dimensionality. This network reorganization constitutes a neural signature of states of reduced consciousness that is common to anesthesia and sleep. In 2020, the Australian and New Zealand flux research and monitoring network, OzFlux, celebrated its 20 th anniversary by reflecting on the lessons learned through two decades of ecosystem studies on global change biology. OzFlux is a network not only for ecosystem researchers, but also for those 'next users' of the knowledge, information and data that such networks provide. Here, we focus on eight lessons across topics of climate change and variability, disturbance and resilience, drought and heat stress and synergies with remote sensing and modelling. In distilling the key lessons learned, we also identify where further research is needed to fill knowledge gaps and improve the utility and relevance of the outputs from OzFlux. Extreme climate variability across Australia and New Zealand (droughts and flooding rains) provides a natural laboratory for a global understanding of ecosystems in this time of accelerating climate change. As evidence of worsening global fire risk emerges, the natural ability of these ecosystems to recover from disturbances, such as fire and cyclones, provides lessons on adaptation and resilience to disturbance. Drought and heatwaves are common occurrences across large parts of the region and can tip an ecosystem's carbon budget from a net CO 2 sink to a net CO 2 source. Despite such responses to stress, ecosystems at OzFlux sites show their resilience to climate variability by rapidly pivoting back to a strong carbon sink upon the return of favourable conditions. Located in under-represented areas, OzFlux data have the potential for reducing uncertainties in global remote sensing products, and these data provide several opportunities to develop new theories and improve our ecosystem models. The accumulated impacts of these lessons over the last 20 years highlights the value of long-term flux observations for natural and managed systems. This observational study describes the procedure technique, safety outcomes, and patient responses to celiac plexus blockade (CPB) in children with severe neurodisability with refractory feed intolerance, feed induced pain or feed induced dystonia (FID)., Method: A review of the pathophysiological response to feeding in children with significant neurodisability and the effect on the neuroenteric system. A 2-stage CT-guided temporary celiac plexus blockade followed by neurolysis technique is described. We compile a case series of 5 patients with life limiting conditions and significant disability undergoing CPB in a single tertiary pediatric hospital., Results: A total of 10 separate procedures in 5 children were completed. A positive outcome was observed in 3 out of 4 cases of pediatric FID. Two of the three patients on parenteral nutrition had improved feed tolerance postprocedure. While wetlands are the largest natural source of methane (CH 4 ) to the atmosphere, they represent a large source of uncertainty in the global CH 4 budget due to the complex biogeochemical controls on CH 4 dynamics. Here we present, to our knowledge, the first multi-site synthesis of how predictors of CH 4 fluxes (FCH4) in freshwater wetlands vary across wetland types at diel, multiday (synoptic), and seasonal time scales. We used several statistical approaches (correlation analysis, generalized additive modeling, mutual information, and random forests) in a wavelet-based multi-resolution framework to assess the importance of environmental predictors, nonlinearities and lags on FCH4 across 23 eddy covariance sites. Seasonally, soil and air temperature were dominant predictors of FCH4 at sites with smaller seasonal variation in water table depth (WTD). In contrast, WTD was the dominant predictor for wetlands with smaller variations in temperature (e.g., seasonal tropical/subtropical wetlands). Changes in seasonal FCH4 lagged fluctuations in WTD by ~17 ± 11 days, and lagged air and soil temperature by median values of 8 ± 16 and 5 ± 15 days, respectively. Temperature and WTD were also dominant predictors at the multiday scale. Atmospheric pressure (PA) was another important multiday scale predictor for peat-dominated sites, with drops in PA coinciding with synchronous releases of CH 4 . At the diel scale, synchronous relationships with latent heat flux and vapor pressure deficit suggest that physical processes controlling evaporation and boundary layer mixing exert similar controls on CH 4 volatilization, and suggest the influence of pressurized ventilation in aerenchymatous vegetation. In addition, 1- to 4-h lagged relationships with ecosystem photosynthesis indicate recent carbon substrates, such as root exudates, may also control FCH4. Wetland methane (CH 4 ) emissions ([Formula: see text]) are important in global carbon budgets and climate change assessments. Currently, [Formula: see text] projections rely on prescribed static temperature sensitivity that varies among biogeochemical models. Meta-analyses have proposed a consistent [Formula: see text] temperature dependence across spatial scales for use in models; however, site-level studies demonstrate that [Formula: see text] are often controlled by factors beyond temperature. Here, we evaluate the relationship between [Formula: see text] and temperature using observations from the FLUXNET-CH 4 database. Measurements collected across the globe show substantial seasonal hysteresis between [Formula: see text] and temperature, suggesting larger [Formula: see text] sensitivity to temperature later in the frost-free season (about 77% of site-years). Results derived from a machine-learning model and several regression models highlight the importance of representing the large spatial and temporal variability within site-years and ecosystem types. Mechanistic advancements in biogeochemical model parameterization and detailed measurements in factors modulating CH 4 production are thus needed to improve global CH 4 budget assessments.

Drained peatlands are major sources of CO 2 to the atmosphere, yet the effects of land management and hydrological extremes have been little-studied at spatial scales relevant to agricultural enterprises. We measured fluxes of CO 2 using the eddy covariance (EC) technique at two adjacent dairy farms on a drained peatland in Aotearoa New Zealand with remaining peat depths 5.5-8 m. One site (SD) had shallow surface drains and mean water table depth (WTD) -657 mm, while the other site (BD) had deep field border drains and mean WTD -838 mm. Net ecosystem CO 2 production (NEP) was similar at the two sites when the soils were moist but diverged during late-summer drying, with site BD having 4.56 t C ha -1 greater CO 2 emission than site SD over the four-month dry period. Soil drying reduced gross primary production (GPP) at both sites, while ecosystem respiration (ER) was reduced at site SD but not at site BD. The low dry season respiration rates at site SD contributed to near-zero annual NEP, while higher respiration rates at site BD led to annual CO 2 loss of -4.95 ± 0.59 t C ha -1  yr -1 . Accounting for other imports and exports of carbon, annual net ecosystem carbon balances were -2.23 and -8.47 t C ha -1  yr -1 at sites SD and BD, respectively. It is likely that the contrasting dry season respiration rates resulted from differences in soil physical properties affecting soil moisture vertical redistribution and availability to plants and microbes rather than from the relatively small differences in WTD. These differences could be caused by soil physical disturbances during pasture renewal or paddock recontouring, or time since initial drainage. Peatland biological, physical and chemical properties change over time in response to alterations in long-term water table position. Such changes complicate our ability to predict the response of peatland carbon stocks to sustained drying. In order to better understand the effect of sustained lowering of the water table on peatland carbon dynamics, we re-visited a drainage-affected bog, repeating eddy covariance measurements of CO 2 flux after a 16-year interval. We found the ecosystem CO 2 sink to have strengthened across the intervening period, despite a deep and fluctuating water table. This was mostly due to an increase in CO 2 uptake through photosynthesis associated with increased shrub growth. We also observed a decline in CO 2 loss through ecosystem respiration. These changes could not be attributed to environmental conditions. Air temperature was the only significant contemporaneous driver of monthly anomalies in CO 2 fluxes, with higher temperatures decreasing the net CO 2 sink via increased ecosystem respiration. However, the effect of air temperature was weak in comparison to the underlying differences between time periods. Therefore, we demonstrate that for drying peatlands, long-term changes within the ecosystem can be of primary importance as drivers of CO 2 exchange. Disruption of cortical connectivity likely contributes to loss of consciousness (LOC) during both sleep and general anesthesia, but the degree of overlap in the underlying mechanisms is unclear. Both sleep and anesthesia comprise states of varying levels of arousal and consciousness, including states of largely maintained conscious experience (sleep: N1, REM; anesthesia: sedated but responsive) as well as states of substantially reduced conscious experience (sleep: N2/N3; anesthesia: unresponsive). Here, we tested the hypotheses that (1) cortical connectivity will exhibit clear changes when transitioning into states of reduced consciousness, and (2) these changes will be similar for arousal states of comparable levels of consciousness during sleep and anesthesia. Using intracranial recordings from five adult neurosurgical patients, we compared resting state cortical functional connectivity (as measured by weighted phase lag index, wPLI) in the same subjects across arousal states during natural sleep [wake (WS), N1, N2, N3, REM] and propofol anesthesia [pre-drug wake (WA), sedated/responsive (S), and unresponsive (U)]. Analysis of alpha-band connectivity indicated a transition boundary distinguishing states of maintained and reduced conscious experience in both sleep and anesthesia. In wake states WS and WA, alpha-band wPLI within the temporal lobe was dominant. This pattern was largely unchanged in N1, REM, and S. Transitions into states of reduced consciousness N2, N3, and U were characterized by dramatic changes in connectivity, with dominant connections shifting to prefrontal cortex. Secondary analyses indicated similarities in reorganization of cortical connectivity in sleep and anesthesia. Shifts from temporal to frontal cortical connectivity may reflect impaired sensory processing in states of reduced consciousness. In New Zealand, pasture renewal is a routine management method for maintaining pasture productivity. However, knowledge of the renewal effects on soil organic carbon (SOC) stocks is still limited. Here we use a process-based model, CenW, to comprehensively assess the effects of pasture renewal on the carbon balance of a temperate pasture in the Waikato region of New Zealand. We investigated the effects of renewal frequency, length of fallow period, renewal timing, and the importance and quantification of age-related reductions in productivity. Our results suggest that SOC change depends on the combined effects of renewal on gross primary productivity (GPP), autotrophic and heterotrophic respiration, carbon removal by grazing and carbon allocation to roots. Pasture renewal reduces grazing removal proportionately more than GPP because newly established plants need to allocate more carbon to re-build their root system following renewal which limits foliage production. That lengthens the time before above-ground biomass has grown sufficiently to be grazed again. New plants have a lower ratio of autotrophic respiration to GPP, however, which partly compensates for the GPP loss during renewal. Our simulations suggested an average SOC loss of 0.16 tC ha -1  yr -1 if pastures were renewed every 25 years, but could gain an average of 0.3 tC ha -1  yr -1 if pastures were renewed every year. For maximizing pasture production, the optimal renewal frequency depends on the rate of pasture deterioration with more rapid deterioration rates favouring more frequent renewal. Additionally, the length of the fallow period, renewal timing, and associated environmental conditions are important factors that can affect SOC temporally, but the importance of those effects diminishes at the annual or longer time scales. Pastoral agriculture is important for supplying global demand for animal products but pasture productivity is often water limited. Increased plant diversity has been shown to increase water use efficiency (ω) and productivity under water limitation but the optimal mix of species varies spatially, dependent on climate, soil type, and plant water requirements. Consequently, a cost-effective method to screen for high ω plant species and mixes in situ at farm scale is needed. Using carbon isotope discrimination (∆ 13 C) to examine ω is attractive because the method integrates over useful time scales, does not modify the measurement environment, and is cost-effective. Field scale ω was measured using eddy covariance (EC) at two sites with contrasting plant diversity (2 species, 7 species) and compared to the seasonal progression of ω calculated from foliage ∆ 13 C (ω ∆ ). Soil water evaporation (E S ) was removed from EC measured total ecosystem evaporation using a modelling approach and canopy ω (ω C ) was calculated as gross primary production (GPP) divided by canopy evaporation. Mixed species foliage samples were harvested pre-grazing, dried, sub-sampled, ground, and the ratio of 13 C to 12 C was measured. A strong positive correlation was found between ω ∆ and ω C at both study sites (r 2  > 0.83, p < 0.01). In addition to bulk biomass samples, individual species were also harvested seasonally. Relative increases in both ω ∆ and production for some species showed that manipulation of pasture species mixtures may lead to increased ω. High water tables (WT) stabilise peatland carbon (C) through regulation of biogeochemical processes. The impact of peatland WT on ecosystem function, including C exchange, alters over time, and the factors that cause some peatlands to display resilience and others to undergo degradation are poorly understood. Here we use CO 2 flux measurements, measured by eddy covariance, to compare ecosystem function between two raised bogs; one drainage-affected, with a deep and fluctuating water table and the other near-natural, with a shallow and stable water table. The drainage-affected bog was found to be a moderate sink for CO 2 (69 g C m -2  yr -1 ), which was 134 g C m -2  yr -1 less than the near-natural bog (203 g C m -2  yr -1 ). Greater ecosystem productivity has allowed the drainage-impacted bog to act as a CO 2 sink despite higher ecosystem respiration; most likely due to an increase in photosynthetic capacity caused by expansion of ericaceous shrub cover. The tolerance of the vegetation community, particularly the main peat former Empodisma robustum (Restionaceae), to low and fluctuating WT appears to have been key in allowing the site to remain a sink. Despite the current resilience of the ecosystem CO 2 sink, we found gross primary production to be limited under both high and low water tables, even in a year with typical rainfall. This is best explained by the limited physiological ability of ericaceous shrubs to tolerate a fluctuating WT. As such we hypothesise that if the WT continues to drop and become even more unstable, then without further vegetation change, a reduction in gross primary production is likely which may in turn cause the site to become a source for CO 2 ., (Copyright © 2018 Elsevier B.V. Background: Cyclical vomiting syndrome is a disorder characterised by recurrent episodes of profuse vomiting. There are no cases in the literature on the management of children with presenting with cyclical vomiting syndrome and a Chiari malformation type I., Discussion: We report the case of a 12-year-old child diagnosed with cyclical vomiting syndrome and a Chiari malformation type I. The patient received symptomatic relief following a craniocervical decompression.

Dystonias can arise from any painful stimuli in neurologically disabled children. Classically, feed-induced dystonias from mediastinal pain due to severe gastroesophageal reflux disease are described as Sandifer spasm. We report a case series of 12 severely neurologically impaired children with enteral feed-induced dystonias. Intestinal dysmotility was demonstrated in several. Improvements are seen with jejunal feeds or gut rest with total parenteral nutrition. Use of parenteral nutrition in children with severe neurodisability requires thorough discussion with patient groups and commissioners to give clinicians guidelines to standardize care.

Aim: The Bravo wireless pH monitoring system enables oesophageal pH to be monitored in children intolerant to nasal catheters and records measurements over 48 hours. This study aimed to document the minimum child weight that enabled successful capsule placement and any associated complications. We also compared the diagnostic sensitivity of 48 hours versus 24 hours., Methods: We included 203 consecutive patients (122 male children) with clinically documented or suspected reflux symptoms. The age range was two years to 19 years, and the minimum weight was 9.29 kg. The pH capsule was deployed endoscopically under general anaesthetic. Recordings taken over 24 and 48 hours were compared to determine the frequency of differences and correlations in the reflux index and DeMeester scores., Results: Testing using the detached probe was successful in 190 children (93.6%) with weight as low as 9.29 kg, with no side effects, with failed deployment being the most frequent problem. Clinically different results in the reflux index were found in 16% of the children's day 1 or day 2 readings (p < 0.0001), suggesting the benefit of 48-hour measurements., Conclusion: Measuring oesophageal pH with the Bravo wireless pH monitoring system was successful in 93.6% of cases. A possible agricultural climate change mitigation option is to increase the amount of soil organic carbon (SOC). Conversely, some factors might lead to inadvertent losses of SOC. Here, we explore the effect of various management options and environmental changes on SOC storage and milk production of dairy pastures in New Zealand. We used CenW 4.1, a process-based ecophysiological model, to run a range of scenarios to assess the effects of changes in management options, plant properties and environmental factors on SOC and milk production. We tested the model by using 2years of observations of the exchanges of water and CO 2 measured with an eddy covariance system on a dairy farm in New Zealand's Waikato region. We obtained excellent agreement between the model and observations, especially for evapotranspiration and net photosynthesis. For the scenario analysis, we found that SOC could be increased through supplying supplemental feed, increasing fertiliser application, or increasing water availability through irrigation on very dry sites, but SOC decreased again for larger increases in water availability. Soil warming strongly reduced SOC. For other changes in key properties, such as changes in soil water-holding capacity and plant root:shoot ratios, SOC changes were often negatively correlated with changes in milk production. The work showed that changes in SOC were determined by the complex interplay between (1) changes in net primary production; (2) the carbon fraction taken off-site through grazing; (3) carbon allocation within the system between labile and stabilised SOC; and (4) changes in SOC decomposition rates. There is a particularly important trade-off between carbon either being removed by grazing or remaining on site and available for SOC formation. In Christchurch Hospital, rapid prototyping (RP) and intraoperative imaging are the standard of care in orbital trauma and has been used since February 2013. RP allows the fabrication of an anatomical model to visualize complex anatomical structures which is dimensionally accurate and cost effective. This assists diagnosis, planning, and preoperative implant adaptation for orbital reconstruction. Intraoperative imaging involves a computed tomography scan during surgery to evaluate surgical implants and restored anatomy and allows the clinician to correct errors in implant positioning that may occur during the same procedure. This article aims to demonstrate the potential clinical and cost saving benefits when both these technologies are used in orbital reconstruction which minimize the need for revision surgery.

We used two years of eddy covariance (EC) measurements collected over an intensively grazed dairy pasture to better understand the key drivers of changes in soil organic carbon stocks. Analysing grazing systems with EC measurements poses significant challenges as the respiration from grazing animals can result in large short-term CO2 fluxes. As paddocks are grazed only periodically, EC observations derive from a mosaic of paddocks with very different exchange rates. This violates the assumptions implicit in the use of EC methodology. To test whether these challenges could be overcome, and to develop a tool for wider scenario testing, we compared EC measurements with simulation runs with the detailed ecosystem model CenW 4.1. Simulations were run separately for 26 paddocks around the EC tower and coupled to a footprint analysis to estimate net fluxes at the EC tower. Overall, we obtained good agreement between modelled and measured fluxes, especially for the comparison of evapotranspiration rates, with model efficiency of 0.96 for weekly averaged values of the validation data. For net ecosystem productivity (NEP) comparisons, observations were omitted when cattle grazed the paddocks immediately around the tower. With those points omitted, model efficiencies for weekly averaged values of the validation data were 0.78, 0.67 and 0.54 for daytime, night-time and 24-hour NEP, respectively. While not included for model parameterisation, simulated gross primary production also agreed closely with values inferred from eddy covariance measurements (model efficiency of 0.84 for weekly averages). The study confirmed that CenW simulations could adequately model carbon and water exchange in grazed pastures. It highlighted the critical role of animal respiration for net CO2 fluxes, and showed that EC studies of grazed pastures need to consider the best approach of accounting for this important flux to avoid unbalanced accounting., (Copyright © 2015. The drainage and conversion of peatlands to productive agro-ecosystems leads to ongoing surface subsidence because of densification (shrinkage and consolidation) and oxidation of the peat substrate. Knowing the ra0te of this surface subsidence is important for future land-use planning, carbon accounting, and economic analysis of drainage and pumping costs. We measured subsidence rates over the past decade at 119 sites across three large, agriculturally managed peatlands in the Waikato region, New Zealand. The average contemporary (2000s-2012) subsidence rate for Waikato peatlands was 19 ± 2 mm yr (± SE) and was significantly less ( = 0.01) than the historic rate of 26 ± 1 mm yr between the 1920s and 2000s. A reduction in the rate of subsidence through time was attributed to the transition from rapid initial consolidation and shrinkage to slower, long-term, ongoing oxidation. These subsidence rates agree well with a literature synthesis of temperate zone subsidence rates reported for similar lengths of time since drainage. A strong nonlinear relationship was found between temperate zone subsidence rates and time since initial peatland drainage: Subsidence (mm yr) = 226 × (years since drained) ( = 0.88). This relationship suggests that time since drainage exerts strong control over the rate of peatland subsidence and that ongoing peatland subsidence rates can be predicted to gradually decline with time in the absence of major land disturbance., (Copyright © by the American Society of Agronomy, Crop Science Society of America, and Soil Science Society of America, Inc.)

Bioactive substances may be used to enhance the rate and quantity of bone healing during osseointegration of titanium dental implants. A pilot observational study was undertaken to assess a novel keratin hydrogel in six adult sheep utilising the femoral condyles as the surgical site to assess osseointegration. Implants and osteotomy sites were coated with the keratin gel prior to implant placement (test implants) whereas the opposite knee received unmodified control implants in each animal. Fifty 3.5 mm × 7 mm Neoss dental implants were surgically implanted with a range of 3-5 Neoss dental implants placed per surgical site in each knee and allowed to heal for 5 days or 2, 4, 8, 12 and 16 weeks prior to the sheep being sacrificed. Of the 50 placed implants, 24 were used for this study and analysed via resin-embedded, undemineralised sections from test and control implants to assess the range of healing around the unloaded dental implants. These dental implants were analysed using histomorphometric methods for the best 3 consecutive threads on each side and the percentage of bone to implant contact (%BIC) was used to determine the degree of osseointegration between test and control dental implants at each time point. All implants appeared osseointegrated at the time of sacrifice. One each of the pairs of control implants at 2, 4 and 12 weeks demonstrated minimal integration histologically, with %BIC <10 %. No test implants had %BIC <35 % at any time point. Mean %BIC for test implants was higher than controls at all time points except 5 days and 2 weeks. The range from 2 to 16 weeks healing was 39.7 % [SD 25.5 %] to 85.4 % [14.2 %] for test implants and 35.6 % [43.4 %] to 46.6 % [23.1 %] for controls. %BIC appeared to increase earlier in the test implants (from 4 weeks onwards) compared to controls. After 16 weeks, %BIC was almost twice as great in test implants as controls. This pilot observational study suggests that keratin hydrogel may promote earlier osseointegration around titanium dental implants. Further cross-sectional studies with larger sample sizes are warranted. The most marked difference between test and control implants was seen after 4 weeks. It is recommended that future studies in this model focus on healing after 4 weeks.

Background: Helicobacter pylori infection is acquired in early childhood and persists for life (or until eradication treatment is taken). Seropositivity of H. pylori at age 49-51 years was assessed in relation to socio-economic deprivation in early life and the timing of other childhood infections common at that time., Methods: Prospectively collected socio-economic and morbidity data from the Newcastle Thousand Families study, a birth cohort established in 1947. H. pylori IgG seropositivity was assessed at 49-51 years and examined in relation to both whether the individual had been diagnosed with one of measles, mumps or chicken pox, and, if so, the age at first infection. This was done in logistic regression models, allowing adjustment for socio-economic status and housing quality in childhood., Results: Adult H. pylori status was strongly linked to disadvantaged socio-economic status in early life (p ≤ 0.002), unlike measles, mumps and chicken pox which showed no associations. Early measles infection was independently associated with H. pylori seropositivity (p = 0.01)., Conclusions: Of the four infectious diseases that we have studied, it appears that H. pylori differs from the others by the strength of association with socio economic deprivation in early childhood. Our findings further highlight the complex interaction between measles, childhood infections and other non-microbiological factors that occur within a whole population. These data suggest a strong association between H. pylori and deprivation and raise the possibility of an interaction between early measles exposure and increased risk of exposure to H. pylori infection.

Rationale: The rapid analysis of volatile compounds, such as fragrances, is important in many commercial industries. The various ambient ionization methods have until now been largely applied to non-volatile or low-volatile compounds with success, and this study develops a semi-quantitative method for volatile compounds in commercial cleaning products., Methods: Low-temperature plasma (LTP) ionization was used to perform rapid analysis, determine limits of detection (LODs) and perform chemical imaging on eight fragrances. Several mass analyzers including an ion trap, a quadrupole and an orbitrap were used to rapidly screen volatile compounds from cloth, paper, and glass and determine compositions present in a commercial cleaning product. Peltier cooling was used in some cases to enhance the retention time of compounds on a surface., Results: This LTP method allowed the detection of fragrances in low picogram absolute amounts from glass, paper and cloth. Quantitation was demonstrated for compounds in a commercial cleaning product 1 min after the product was applied to a vinyl tile surface. High-throughput analysis and simultaneous detection of multiple compounds in a mixture were demonstrated with analysis times of less than 1 min. Modest spatial resolution (better than 1 cm) was achieved with LTP ionization., Conclusions: A semi-quantitative method has been demonstrated for the routine analysis of volatile and semi-volatile compounds. This method would be useful in quality control and production environments to determine product persistence, location of analytes and to complement olfactory studies for determining concentrations in the ambient environment., (Copyright © 2013 John Wiley & Sons, Ltd.)

The aza-Michael addition and the Mannich condensation occur in thin films deposited on ambient surfaces. The reagents for both C-N bond formation reactions were transferred onto the surface by drop-casting using a micropipette. The surface reactions were found to be much more efficient than the corresponding bulk solution-phase reactions performed on the same scale in the same acetonitrile solvent. The increase in rate of product formation in the thin film is attributed to solvent evaporation in the open air which results in reagent concentration and produces rate acceleration similar to that seen in evaporating droplets in desorption electrospray ionization. This thin film procedure has potential for the rapid synthesis of reaction products on a small scale, as well as allowing rapid derivatization of analytes to produce forms that are easily ionized by electrospray ionization. Analysis of the derivatized sample directly from the reaction surface through the use of desorption electrospray ionization is also demonstrated.

Desorption electrospray ionization imaging allows biomarker discovery and disease diagnosis through chemical characterization of biological samples in their native environment. Optimization of experimental parameters including emitter capillary size, solvent composition, solvent flow rate, mass spectrometry scan-rate and step-size is shown here to improve the resolution available in the study of biological tissue from 180 μm to about 35 μm using an unmodified commercial mass spectrometer. Mouse brain tissue was used to optimize and measure resolution based on known morphological features and their known relationships to major phospholipid components. Features of approximately 35 μm were resolved and correlations drawn between features in grey matter (principally PS (18:0/22:6), m/z 834) and in white matter (principally ST (24:1), m/z 888). The improved spatial resolution allowed characterization of the temporal changes in lipid profiles occurring within mouse ovaries during the ovulatory cycle. An increase in the production of phosphatidylinositol (PI 38:4) m/z 885 and associated fatty acids such as arachidonic acid (FA 20:4) m/z 303 and adrenic acid (FA 22:4) m/z 331was seen with the postovulatory formation of the corpus luteum.

The exposure of charged microdroplets containing organic ions to solid-phase reagents at ambient surfaces results in heterogeneous ion/surface reactions. The electrosprayed droplets were driven pneumatically in ambient air and then electrically directed onto a surface coated with reagent. Using this reactive soft landing approach, acid-catalyzed Girard condensation was achieved at an ambient surface by directing droplets containing Girard T ions onto a dry keto-steroid. The charged droplet/surface reaction was much more efficient than the corresponding bulk solution-phase reaction performed on the same scale. The increase in product yield is ascribed to solvent evaporation, which causes moderate pH values in the starting droplet to reach extreme values and increases reagent concentrations. Comparisons are made with an experiment in which the droplets were pneumatically accelerated onto the ambient surface (reactive desorption electrospray ionization, DESI). The same reaction products were observed but differences in spatial distribution were seen associated with the "splash" of the high velocity DESI droplets. In a third type of experiment, the reactions of charged droplets with vapor phase reagents were examined by allowing electrosprayed droplets containing a reagent to intercept the headspace vapor of an analyte. Deposition onto a collector surface and mass analysis showed that samples in the vapor phase were captured by the electrospray droplets, and that instantaneous derivatization of the captured sample is possible in the open air. The systems examined under this condition included the derivatization of cortisone vapor with Girard T and that of 4-phenylpyridine N-oxide and 2-phenylacetophenone vapors with ethanolamine.

In the developing world major public health issues such as malnutrition and compromised physical development are intimately linked to altered gut morphology and function with underlying chronic inflammatory responses. In these societies the downward spiral of malnutrition and infections does not seem to be remedied by well-informed nutritional interventions that supplement the identified nutrient deficiencies, suggesting that additional strategies are needed. The aim of this scientific opinion paper is to consider how a child from the developing world might benefit, separately and additively, from interventions targeted to impact hygiene, nutritional status, disease resistance and gut function, if successful interventions could be found. A failure to tackle environmental enteropathy (EE) may be a critical limiting factor that can explain the relative lack of success of interventions focussed on micronutrient supplementation so far. Therefore this paper starts with a summary of the aetiology and consequences of EE on child health and the current recommendations aimed at tackling this problem. Then a number of hypotheses will be considered in terms of research strategy to positively affect nutritional status, intestinal health and growth of children with EE, with the aim of inspiring future innovative strategies, for both the food industry and the public health sector, which could benefit millions of children.

Aims: The aim of this study was to assess the attitudes of hospital clinical staff to acute personal illness., Methods: A self-reported questionnaire was developed. Four hundred clinical staff employed by the district health board (DHB) who met the inclusion criteria who were randomly selected. Data were collected and analysed using SPSS software. Ethical approval was obtained from the Lower South Regional Ethics Committee and from the DHB Health Research Office., Results: Doctors were more likely to exhibit sickness presenteeism (SP)--i.e. working despite being sick--than any other occupational group at the DHB. Two main reasons were given for not taking sick leave: staff did not believe they were unwell enough to justify taking leave and they did not want to increase the workload of others. The majority of study participants would not contact anyone for advice about whether to take leave., Conclusion: This study provides evidence that SP, especially in doctors, is prevalent in the DHB and is similar to findings from elsewhere. Low rates of clinical staff contacting someone for advice on coming to work whilst ill could be targeted to improve infection control.

The use of non-aqueous solvents in desorption electrospray ionization mass spectrometry (DESI-MS) is explored by analyzing a set of 43 compounds using binary mixtures of chloroform, tetrahydrofuran, and acetonitrile as the spray solvent. Comparisons of data obtained from chloroform/tetrahydrofuran (1:1) and chloroform/acetonitrile (1:1) spray solvents with the standard aqueous-based spray solvent (methanol/water, 1:1) shows that the non-aqueous systems have practical value for DESI, especially in the analysis of hydrophobic compounds. Non-aqueous spray solvents were used to ionize thermometer molecules (benzyl pyridinium salts) and showed lower internal energies (softer DESI ionization compared with methanol/water, 1:1), a result that has parallels in known solvent effects in electrospray ionization and is explained by solvent effects on surface tension. Consideration of octanol/water partition coefficients (K(ow)) of the 43 analytes in the light of their DESI results reveals the importance of the solubility of analyte in the spray solvent in producing high quality mass spectra. This finding provides additional support for the droplet pick-up description of the DESI mechanism, which is based on analyte dissolution in the spray solvent, followed by splashing of subsequently arriving droplets in the liquid film to form microdroplets of dissolved analyte. DESI solvent optimization can be improved by the use of K(ow) of the analyte as an indication of the polarity of the most appropriate solvent system., (2010 American Society for Mass Spectrometry.
