Abstract

OBJECTIVES:. Anemia has been associated with an increased risk of both cardiac arrest and stroke, frequent complications of COVID-19. The effect of hemoglobin level at ICU admission on a composite outcome of cardiac arrest or stroke in an international cohort of COVID-19 patients was investigated. DESIGN:. Retrospective analysis of prospectively collected database. SETTING:. A registry of COVID-19 patients admitted to ICUs at over 370 international sites was reviewed for patients diagnosed with cardiac arrest or stroke up to 30 days after ICU admission. Anemia was defined as: normal (hemoglobin ≥ 12.0 g/dL for women, ≥ 13.5 g/dL for men), mild (hemoglobin 10.0–11.9 g/dL for women, 10.0–13.4 g/dL for men), moderate (hemoglobin ≥ 8.0 and < 10.0 g/dL for women and men), and severe (hemoglobin < 8.0 g/dL for women and men). PATIENTS:. Patients older than 18 years with acute COVID-19 infection in the ICU. INTERVENTIONS:. None. MEASUREMENTS AND MAIN RESULTS:. Of 6926 patients (median age = 59 yr, male = 65%), 760 patients (11.0%) experienced stroke (2.0%) and/or cardiac arrest (9.4%). Cardiac arrest or stroke was more common in patients with low hemoglobin, occurring in 12.8% of patients with normal hemoglobin, 13.3% of patients with mild anemia, and 16.7% of patients with moderate/severe anemia. Time to stroke or cardiac arrest by anemia status was analyzed using Cox proportional hazards regression with death as a competing risk. Covariates selected through clinical knowledge were age, sex, comorbidities (diabetes, hypertension, obesity, and cardiac or neurologic conditions), pandemic era, country income, mechanical ventilation, and extracorporeal membrane oxygenation. Moderate/severe anemia was associated with a higher risk of cardiac arrest or stroke (hazard ratio, 1.32; 95% CI, 1.05–1.67). CONCLUSIONS:. In an international registry of ICU patients with COVID-19, moderate/severe anemia was associated with increased hazard of cardiac arrest or stroke.

Abstract

Brazilian protected areas are essential for the conservation of plants of the Atlantic Forest domain, one of 36 global biodiversity hotspots. A major challenge for improving conservation actions is to know the richness of plants protected by these areas. Online databases offer an accessible way to build plant species lists and to provide relevant information about biodiversity. A list of land plants of “Parque Nacional do Caparaó” (PNC) was previously built using online databases and published on the website "Catálogo de Plantas das Unidades de Conservação do Brasil." Here, we provide and discuss additional information about plant species richness, endemism, and conservation in the PNC that could not be included in the list. We documented 1,791 species of land plants as occurring in PNC, of which 63 are cited as threatened (CR, EN, or VU) by the Brazilian National Red List, seven as data deficient (DD), and five as priorities for conservation."Parque Nacional do Caparaó" houses 8% of the land plant species endemic to the Brazilian Atlantic Forest, including 6% of its angiosperms, 31% of its lycophytes and ferns, and 14% of its avascular plants. Twelve percent of the threatened species listed for the state of Espírito Santo and 7% listed for the state of Minas Gerais are also protected by PNC. Surprisingly, 79% of the collections analyzed here were carried out in Minas Gerais, which represents just 21% of the total extension of the park. The compiled data uncover a huge botanical collection gap in this federally protected area.

Abstract

IMPORTANCE:. Factors associated with mortality in coronavirus disease 2019 patients on invasive mechanical ventilation are still not fully elucidated. OBJECTIVES:. To identify patient-level parameters, readily available at the bedside, associated with the risk of in-hospital mortality within 28 days from commencement of invasive mechanical ventilation or coronavirus disease 2019. DESIGN, SETTING, AND PARTICIPANTS:. Prospective observational cohort study by the global Coronavirus Disease 2019 Critical Care Consortium. Patients with laboratory-confirmed coronavirus disease 2019 requiring invasive mechanical ventilation from February 2, 2020, to May 15, 2021. MAIN OUTCOMES AND MEASURES:. Patient characteristics and clinical data were assessed upon ICU admission, the commencement of invasive mechanical ventilation and for 28 days thereafter. We primarily aimed to identify time-independent and time-dependent risk factors for 28-day invasive mechanical ventilation mortality. RESULTS:. One-thousand five-hundred eighty-seven patients were included in the survival analysis; 588 patients died in hospital within 28 days of commencing invasive mechanical ventilation (37%). Cox-regression analysis identified associations between the hazard of 28-day invasive mechanical ventilation mortality with age (hazard ratio, 1.26 per 10-yr increase in age; 95% CI, 1.16–1.37; p < 0.001), positive end-expiratory pressure upon commencement of invasive mechanical ventilation (hazard ratio, 0.81 per 5 cm H2O increase; 95% CI, 0.67–0.97; p = 0.02). Time-dependent parameters associated with 28-day invasive mechanical ventilation mortality were serum creatinine (hazard ratio, 1.28 per doubling; 95% CI, 1.15–1.41; p < 0.001), lactate (hazard ratio, 1.22 per doubling; 95% CI, 1.11–1.34; p < 0.001), Paco2 (hazard ratio, 1.63 per doubling; 95% CI, 1.19–2.25; p < 0.001), pH (hazard ratio, 0.89 per 0.1 increase; 95% CI, 0.8–14; p = 0.041), Pao2/Fio2 (hazard ratio, 0.58 per doubling; 95% CI, 0.52–0.66; p < 0.001), and mean arterial pressure (hazard ratio, 0.92 per 10 mm Hg increase; 95% CI, 0.88–0.97; p = 0.003). CONCLUSIONS AND RELEVANCE:. This international study suggests that in patients with coronavirus disease 2019 on invasive mechanical ventilation, older age and clinically relevant variables monitored at baseline or sequentially during the course of invasive mechanical ventilation are associated with 28-day invasive mechanical ventilation mortality hazard. Further investigation is warranted to validate any causative roles these parameters might play in influencing clinical outcomes.

Abstract

La expansión del maxilar ya sea rápida o lenta, ha sido un procedimiento utilizado durante años para la corrección de maloclusiones transversales. Sus beneficios y efectos a nivel dentario, como de sutura palatina media son conocidos. Sin embargo, en los últimos años la repercusión y sus efectos a nivel de vía aérea han sido estudiados y se han establecido como un punto de interés sobre todo durante el tratamiento de pacientes con problemas respiratorios. Según la bibliografía actual, la expansión rápida del maxilar induce un aumento significativo del volumen de la vía aérea superior. Hoy en día, con el alto desarrollo de técnicas basadas en imágenes 3D y con el advenimiento de la tomografía computarizada Cone Beam, es posible realizar análisis cuantitativos y evaluar los efectos de la expansión maxilar a nivel de la vía aérea superior en los tres sentidos del espacio. Mediante el uso de imagenología es posible visualizar la ganancia en la vía aérea superior pero aún no es posible evidenciar si este aumento se correlaciona con una mejora en la función respiratoria.

Introducción: Las plantas están expuestas a ambientes con diferente disponibilidad de luz, la cuál es el factor más limitante en los bosques tropicales húmedos. En respuesta, las hojas modifican sus rasgos funcionales para aprovecharla y evitar el estrés hídrico. Sin embargo, aún falta información para comprender cómo la planta cambia los rasgos funcionales dependiendo de las condiciones lumínicas. Objetivos: Por ello, este estudio se concentró en la adaptación de las hojas de Piper reticulatum (Piperaceae) ante diferentes ambientes de luz. Además, se quiso conocer si estos cambios ocurren en etapas jóvenes o maduras de la hoja. Métodos: Se recolectaron sesenta hojas (una joven y una vieja) de P. reticulatum en 15 individuos que se encontraban en la sombra y 15 individuos expuestos al sol en la Estación Biológica La Selva, Costa Rica. Se midió el área foliar y la masa foliar específica de cada hoja para determinar diferencias en la asignación de biomasa. También, se contabilizaron los estomas y las venas terciarias para determinar si hay adaptaciones contra la desecación en ambientes de mayor cantidad de luz. Resultados: Las hojas de sol tuvieron mayor cantidad de estomas, menos venas terciarias y mayor masa foliar específica. También, las hojas jóvenes presentaron más estomas, pero menos masa foliar específica. Conclusión: Plantas bajo diferentes condiciones de luz, difieren en cantidad de estomas, venas y masa, siendo la edad de la hoja un factor de variación.

7. Use Pattern of Ophthalmic Antiglaucoma Agents with and without Preservatives: A Cross-Sectional Study

Abstract

Background: Different drugs have been approved to reduce the intraocular pressure. However, most of them contain preservatives to maintain sterility and these can be toxic to the ocular surface. The aim was to determine the patterns of use of antiglaucoma agents and ophthalmic preservatives in a group of patients from Colombia. Methods: A cross-sectional study that identified ophthalmic antiglaucoma agents from a population database of 9.2 million. Sociodemographic and pharmacological variables were considered. Descriptive and bivariate analyses were performed. Results: A total of 38,262 patients were identified, with a mean age of 69.2 ± 13.3 years, and 58.6% were women. A total of 98.8% were prescribed antiglaucoma drugs in multidose containers. The most widely used were prostaglandin analogs (59.9%), especially latanoprost (51.6%) and β-blockers (59.2%). A total of 54.7% of patients received combined management, especially with fixed-dose combination (FDC) drugs (41.3%). A total of 94.1% used antiglaucoma drugs with preservatives (benzalkonium chloride, 68.4%). Conclusions: The pharmacological treatment of glaucoma was very heterogeneous, but the most commonly used therapeutic groups were in accordance with the recommendations of clinical practice guidelines but with differences by sex and age. Most of the patients were exposed to preservatives, especially benzalkonium chloride, but the wide use of FDC drugs can minimize toxicity on the ocular surface.

Desde un enfoque inclusivo, a partir del estudio de un caso, se pretendió desarrollar una propuesta práctica para la inclusión de estudiantes con ceguera total en una carrera de ingeniería. Con una metodología cualitativa basada en las técnicas de juicio de expertos, la entrevista semi estructurada y el análisis documental se logró sistematizar y validar la información de diferentes fuentes, recolectada entre el 2012 y el 2018, durante las etapas de admisión, permanencia y graduación. Para la recolección de datos se utilizaron principalmente guías para orientar las entrevistas a jueces, docentes y equipos de apoyo. A partir de un análisis inductivo se generó una propuesta práctica derivada de las acciones ejecutadas durante este periodo, agrupadas en dos ejes conceptuales: gestión en accesibilidad y accesibilidad académica. La principal conclusión es la relevancia de la articulación de todas las acciones con las normativas vigentes para garantizar la accesibilidad y los apoyos educativos. Esta propuesta permitirá, a todas las personas involucradas en los distintos procesos universitarios, elaborar sus propias rutas de trabajo con estudiantes ciegos.

Abstract

Objective. Autoimmune idiopathic inflammatory myopathies (IIMs) are a group of pathologies that are generally characterized by muscle weakness. Their treatment involves glucocorticoids and immunosuppressants. The aim was to identify differences and similarities in the pharmacological management of a group of patients with autoimmune IIMs according to the type of disease, sex, age group, and city of residence in Colombia from 2020 to 2021. Methods. This cross-sectional study identified medication prescription patterns for outpatient use in patients with autoimmune IIMs between 2020 and 2021 based on a population database of 8.5 million Colombians affiliated with the Colombian health system. Sociodemographic and pharmacological variables were considered. Results. A total of 671 patients with autoimmune IIMs were identified, with a median age of 57 years, and 70.9% were women. Overlap myositis was the most frequent disease (31.4%). A total of 91.5% of the patients received pharmacological treatment, mainly systemic glucocorticoids (78.5%), conventional disease-modifying antirheumatic drugs (DMARDs) (74.1%), immunosuppressants (9.1%), and biological DMARDs (3.7%). Pharmacological management predominated among patients with overlap myositis, those who lived in cities, and those affiliated with the contributory regime of the Colombian health system. Conventional DMARDs were prescribed mainly to women and to those older than 65 years. Conclusions. Patients with autoimmune IIMs are not treated homogeneously. The pattern of drug use varies according to the type of IIM, sex, age group, city, and health system regime affiliation.

Abstract

Antibiotics are frequently prescribed to patients with COVID-19. The aim was to determine the pattern of use of systemic antibiotics in a group of patients diagnosed with COVID-19 in Colombia between 2020–2022. This was a descriptive cross-sectional study designed to identify antibiotics prescription patterns for patients diagnosed with COVID-19 treated in eight clinics in Colombia. The AWaRe tool of the World Health Organization (WHO) was used to classify the antibiotics. A total of 10,916 patients were included. The median age was 57 years, and 56.4% were male. A total of 57.5% received antibiotics, especially ampicillin/sulbactam (58.8%) and clarithromycin (47.9%). Most of the antibiotics were classified as Watch (65.1%), followed by Access (32.6%) and Reserve (2.4%). Men (OR: 1.29; 95%CI: 1.17–1.43), older adults (OR: 1.67; 95%CI: 1.48–1.88), patients with dyspnea (OR: 1.26; 95%CI: 1.13–1.41), rheumatoid arthritis (OR: 1.94; 95%CI: 1.17–3.20), and high blood pressure at admission (OR: 1.45; 95%CI: 1.29–1.63), patients treated in-hospital (OR: 5.15; 95%CI: 4.59–5.77), patients admitted to the ICU (OR: 10.48; 95%CI: 8.82–12.45), patients treated with systemic glucocorticoids (OR: 3.60; 95%CI: 3.21–4.03) and vasopressors (OR: 2.10; 95%CI: 1.60–2.75), and patients who received invasive mechanical ventilation (OR: 2.37; 95%CI: 1.82–3.09) were more likely to receive a systemic antibiotic. Most of the patients diagnosed with COVID-19 received antibiotics, despite evidence showing that bacterial coinfection is rare. Antibiotics from the Watch group predominated, a practice that goes against WHO recommendations.

Los tumores cardiacos primarios son raros, representan el 0.0017-0.03% en grandes series de autopsias, ocurren con mayor frecuencia entre mujeres en la quinta o sexta decada de la vida y generalmente se desarrollan en las auriculas. Se presenta el caso clinico de una paciente de sexo femenino de 74 anos que consulta a urgencias con sospecha de sindrome coronario agudo sin elevacion del segmento ST y troponinas positivas. El ecocardiograma transtoracico evidencio la presencia de un mixoma auricular gigante con area de 11 cm2, que protruye a traves de la valvula mitral, una presion sistolica de la arteria pulmonar de 110 mmHg y un resultado de dimero D positivo dando como diagnostico un mixoma auricular izquierdo gigante que causa obstruccion mitral e hipertension pulmonar, el cual fue remitido a otra institucion para manejo quirurgico.

Abstract

The most distal portion of the ventricular conduction system (VCS) contains cardiac Purkinje cells (PCs), which are essential for synchronous activation of the ventricular myocardium. Contactin-2 (CNTN2), a member of the immunoglobulin superfamily of cell adhesion molecules (IgSF-CAMs), was previously identified as a marker of the VCS. Through differential transcriptional profiling, we discovered two additional highly enriched IgSF-CAMs in the VCS: NCAM-1 and ALCAM. Immunofluorescence staining showed dynamic expression patterns for each IgSF-CAM during embryonic and early postnatal stages, but ultimately all three proteins became highly enriched in mature PCs. Mice deficient in NCAM-1, but not CNTN2 or ALCAM, exhibited defects in PC gene expression and VCS patterning, as well as cardiac conduction disease. Moreover, using ST8sia2 and ST8sia4 knockout mice, we show that inhibition of post-translational modification of NCAM-1 by polysialic acid leads to disrupted trafficking of sarcolemmal intercalated disc proteins to junctional membranes and abnormal expansion of the extracellular space between apposing PCs. Taken together, our data provide insights into the complex developmental biology of the ventricular conduction system., Summary: The cell adhesion molecule NCAM-1 and its post-translational modification by polysialylation are required for normal formation and function of the specialized ventricular conduction system.

Abstract

Background Effective therapies to combat coronavirus 2019 (COVID-19) are urgently needed. Hydroxychloroquine (HCQ) has in vitro antiviral activity against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), but the clinical benefit of HCQ in treating COVID-19 is unclear. Randomized controlled trials are needed to determine the safety and efficacy of HCQ for the treatment of hospitalized patients with COVID-19. Methods We conducted a multicenter, double-blind randomized clinical trial of HCQ among patients hospitalized with laboratory-confirmed COVID-19. Subjects were randomized in a 1:1 ratio to HCQ or placebo for 5 days and followed for 30 days. The primary efficacy outcome was a severe disease progression composite end point (death, intensive care unit admission, mechanical ventilation, extracorporeal membrane oxygenation, and/or vasopressor use) at day 14. Results A total of 128 patients were included in the intention-to-treat analysis. Baseline demographic, clinical, and laboratory characteristics were similar between the HCQ (n = 67) and placebo (n = 61) arms. At day 14, 11 (16.4%) subjects assigned to HCQ and 6 (9.8%) subjects assigned to placebo met the severe disease progression end point, but this did not achieve statistical significance (P = .350). There were no significant differences in COVID-19 clinical scores, number of oxygen-free days, SARS-CoV-2 clearance, or adverse events between HCQ and placebo. HCQ was associated with a slight increase in mean corrected QT interval, an increased D-dimer, and a trend toward an increased length of stay. Conclusions In hospitalized patients with COVID-19, our data suggest that HCQ does not prevent severe outcomes or improve clinical scores. However, our conclusions are limited by a relatively small sample size, and larger randomized controlled trials or pooled analyses are needed.

16. Outer-membrane protein LptD (PA0595) plays a role in the regulation of alginate synthesis in Pseudomonas aeruginosa

Abstract

PURPOSE: The presence of alginate-overproducing (Alg(+)) strains of Pseudomonas aeruginosa in cystic fibrosis patients is indicative of chronic infection. The Alg(+) phenotype is generally due to a mutation in the mucA gene, encoding an innermembrane protein that sequesters AlgT/U, the alginate-specific sigma factor. AlgT/U release from the anti-sigma factor MucA is orchestrated via a complex cascade called regulated intramembrane proteolysis. The goal of this study is to identify new players involved in the regulation of alginate production. METHODOLOGY: Previously, a mutant with a second-site suppressor of alginate production (sap), sap27, was isolated from the constitutively Alg(+) PDO300 that harbours the mucA22 allele. A cosmid from a P. aeruginosa minimum tiling path library was identified via en masse complementation of sap27. The cosmid was transposon mutagenized to map the contributing gene involved in the alginate production. The identified gene was sequenced in sap27 along with algT/U, mucA, algO and mucP. The role of the novel gene was explored using precise in-frame algO and algW deletion mutants of PAO1 and PDO300. RESULTS/KEY FINDINGS: The gene responsible for restoring the mucoid phenotype was mapped to lptD encoding an outer-membrane protein. However, the sequencing of sap27 revealed a mutation in algO, but not in lptD. In addition, we demonstrate that lipopolysaccharide transport protein D (LptD)-dependent alginate production requires AlgW in PAO1 and AlgO in PDO300. CONCLUSION: LptD plays a specific role in alginate production. Our findings suggest that there are two pathways for the production of alginate in P. aeruginosa, one involving AlgW in the wild-type, and one involving AlgO in the mucA22 mutant.

Abstract

According to the World Health Organization, Health Literacy (HL) corresponds to the cognitive and social abilities that determine the motivation and ability of individuals to gain access, to understand, and to use information in ways that provide and maintain good health. The development of HL in dentistry came late, and only in the last decade did it reach a level similar to that in the medical area. In dentistry, HL centered on the concept of Oral Health Literacy (OHL), defined as the degree to which individuals have the capacity to obtain, process, and understand basic information on oral health and services that is necessary to make appropriate health decisions. The evidence suggests that people with low HL have worse health status and greater use of medical resources, which results in an increase in costs in the general population. Determinants of the level of OHL include age, level of education, and socioeconomic level. These determinants are reflected in low oral health and in less access to information or less understanding regarding care, pathologies, or dental treatments. The instruments for measuring HL and OHL are mainly aimed at recognizing arithmetic and reading skills, which are not fully related to the ability of the people surveyed to find, understand and use information related to health. OHL is an important issue at the level of health programs, because knowledge of OHL helps in medical practice, in disease prevention and in health promotion. OHL instruments must have validated and demonstrate adequate psychometric properties.

Abstract

Von Recklinghausen disease or neurofibromatosis type 1 (NF1) is an autosomal dominant disease with a wide clinical expression, with manifestations ranging from multiple cafe-au-lait spots to severe functional and aesthetic complications affecting bone tissue and nerve. Oral manifestations of NF1 are rare. The relevance of this case lies in the fact that the disease rarely occurs in oral cavity. The patient was a 12-year-old boy with facial asymmetry caused by a swelling around the upper left jaw causing deformation. Intraorally, a tumor that completely occupies the vestibule was identified. Excisional biopsy of the lesion was performed, and results indicated neurofibroma plexiform and S-100 positivity. At the control, a favorable healing is observed, and the result is correlated with a clinical history: glaucoma causing blindness of left eye, palpebral fibroids, bilateral frontal neurofibromas, multiple periocular neurofibromas, and Von Recklinghausen disease diagnosis. The treatment of NF1 patients is difficult, which may be increased by the complications derived from deformations (and its resulting psychological problem), gliomas of the optic nerves (blindness), or the high risk of malignant transformation.

Abstract

The progression of cystic fibrosis (CF) from an acute to a chronic disease is often associated with the conversion of the opportunistic pathogen Pseudomonas aeruginosa from a nonmucoid form to a mucoid form in the lung. This conversion involves the constitutive synthesis of the exopolysaccharide alginate, whose production is under the control of the AlgT/U sigma factor. This factor is regulated posttranslationally by an extremely unstable process and has been commonly attributed to mutations in the algT (algU) gene. By exploiting this unstable phenotype, we isolated 34 spontaneous nonmucoid variants arising from the mucoid strain PDO300, a PAO1 derivative containing the mucA22 allele commonly found in mucoid CF isolates. Complementation analysis using a minimal tiling path cosmid library revealed that most of these mutants mapped to two protease-encoding genes, algO, also known as prc or PA3257, and mucP Interestingly, our algO mutations were complemented by both mucP and algO, leading us to delete, clone, and overexpress mucP, algO, mucE, and mucD in both wild-type PAO1 and PDO300 backgrounds to better understand the regulation of this complex regulatory mechanism. Our findings suggest that the regulatory proteases follow two pathways for regulated intramembrane proteolysis (RIP), where both the AlgO/MucP pathway and MucE/AlgW pathway are required in the wild-type strain but where the AlgO/MucP pathway can bypass the MucE/AlgW pathway in mucoid strains with membrane-associated forms of MucA with shortened C termini, such as the MucA22 variant. This work gives us a better understanding of how alginate production is regulated in the clinically important mucoid variants of Pseudomonas aeruginosaIMPORTANCE Infection by the opportunistic pathogen Pseudomonas aeruginosa is the leading cause of morbidity and mortality seen in CF patients. Poor patient prognosis correlates with the genotypic and phenotypic change of the bacteria from a typical nonmucoid to a mucoid form in the CF lung, characterized by the overproduction of alginate. The expression of this exopolysaccharide is under the control an alternate sigma factor, AlgT/U, that is regulated posttranslationally by a series of proteases. A better understanding of this regulatory phenomenon will help in the development of therapies targeting alginate production, ultimately leading to an increase in the length and quality of life for those suffering from CF.

Abstract

The cardiac Purkinje fiber network is composed of highly specialized cardiomyocytes responsible for the synchronous excitation and contraction of the ventricles. Computational modeling, experimental animal studies, and intracardiac electrical recordings from patients with heritable and acquired forms of heart disease suggest that Purkinje cells (PCs) may also serve as critical triggers of life-threatening arrhythmias. Nonetheless, owing to the difficulty in isolating and studying this rare population of cells, the precise role of PC in arrhythmogenesis and the underlying molecular mechanisms responsible for their proarrhythmic behavior are not fully characterized. Conceptually, a stem cell-based model system might facilitate studies of PC-dependent arrhythmia mechanisms and serve as a platform to test novel therapeutics. Here, we describe the generation of murine embryonic stem cells (ESC) harboring pan-cardiomyocyte and PC-specific reporter genes. We demonstrate that the dual reporter gene strategy may be used to identify and isolate the rare ESC-derived PC (ESC-PC) from a mixed population of cardiogenic cells. ESC-PC display transcriptional signatures and functional properties, including action potentials, intracellular calcium cycling, and chronotropic behavior comparable to endogenous PC. Our results suggest that stem-cell derived PC are a feasible new platform for studies of developmental biology, disease pathogenesis, and screening for novel antiarrhythmic therapies. Abstract

Noonan syndrome with multiple giant cell lesions (NS/MGCLS) is a term used to describe a subgroup of people with Noonan syndrome, a rare condition with phenotypic overlap with NS. Once thought to be a specific and separate entity, it is now suggested to be a variant of the NS spectrum. Here we report a 22-year-old patient misdiagnosed as having Rubinstein-Taybi syndrome (microcephaly, congenital heart disease, ventricular septal defect, cryptorchidism, pterygium coli, inguinal hernia, and short stature and neck). He had multiple dental retentions; therefore, a cone beam computed tomography was requested, which showed multiples radiolucent lesions of both sides of the jaw. An excisional biopsy of larger lesion was performed, and the results indicated giant cell granuloma. Patient was referred to the Genetics department because of suspicion of Noonan syndrome, which was confirmed after molecular study. Postoperative control shows bone formation in treated lesion on the right side and growth of the lesion on the left side.

Abstract

The presence of alginate-overproducing (AlgPreviously, a mutant with a second-site suppressor of alginate production (sap), sap27, was isolated from the constitutively AlgLptD plays a specific role in alginate production. Our findings suggest that there are two pathways for the production of alginate in P. aeruginosa, one involving AlgW in the wild-type, and one involving AlgO in the mucA22 mutant.

Abstract

We have previously demonstrated that contactin2 (Cntn2) protein is enriched in Purkinje cells of the adult cardiac conduction system (CCS). Here we present a mouse embryonic stem cell (mESC) reporter line to identify Purkinje-like cardiomyocytes in vitro. Methods and Results: Dual reporter Purkinje cell mESC were generated from Cntn2-eGFP blastocysts, transduced with αMHC-mCherry lentivirus. The mESC expressed markers of pluripotency (Oct3/4; Sox2, Nanog) and differentiated into all three germ layers (Nestin, a-fetoprotein, Brachyury). Cultures differentiated under serum-free, cardiogenic conditions were enriched in cardiomyocytes (n=5), consisting of single positive aMHC-mCherry [57.76±3.7%] and double positive αMHC-mCherry/Cntn2-eGFP cells [1.9±0.9%]. Compared with eGFP negative cardiomyocytes (n=15), double positive cells (n=11) revealed electrophysiological characteristics consistent with Purkinje-like derivatives, including increased peak sodium currents (180.3±20.5 pA/pF vs. =-118.0±21.2pA/pF), faster upstroke velocities (133.7±12.3 V/s vs.60.0±13.2V/s) and elongated action potential duration (APD90=170.2±17.5ms vs. APD90=120.6±17.3ms). Calcium imaging demonstrated chronotrop regulation and spontaneous Ca2+ oscillations in mCherry+eGFP+ cells. Immunofluorecence analysis demonstrated endogenous expression of Purkinje cell markers, including Cntn2, Cx40 and Troponin-T, in double positive cells. Abstract

The understanding of complex heritable psychiatric disorders such as schizophrenia could be clarified by examining endophenotypes within genetically isolated populations, such as the one found in the Central Valley of Costa Rica. The reduction of familial variability within a sample could allow the relationship between the cognitive and symptomatic manifestations of the illness and the genetic underpinnings to become more observable. This study investigates the neuropsychological test performances of 41 family members from four extended multiplex families within the Spanish origin population of the Central Valley of Costa Rica as potential endophenotypes for genetic studies.Individuals with a diagnosis of schizophrenia or schizoaffective disorder were compared with unaffected relatives and 15 unrelated controls with no family history of schizophrenia.Although the sample size is small, the results confirm previous reports in the literature of deficits in working memory, executive function, processing speed, and verbal fluency in individuals with schizophrenia compared with controls and intermediate performance in nonpsychotic family members compared with controls. We also found several suggestive quantitative cognitive trait loci with log of the odds greater than 1.75.These findings suggest that the cognitive deficits in schizophrenia are consistent aspects of the illness, although their usefulness as endophenotypes for genetic studies remains unclear.

Los gases arteriales son herramientas fundamentales para evaluar objetivamente a pacientes con patologías agudas y crónicas, dado que proporcionan información sobre los índices de oxigenación, ventilación, equilibrio ácido-base y el suministro de oxígeno a nivel celular. Interpretarlos adecuadamente tiene gran importancia clínica, dado que influye en la toma de decisiones permite evaluar la respuesta a intervenciones terapéuticas. Este artículo tiene como objetivo presentar al lector los conceptos básicos de la fisiología del intercambio gaseoso, los trastornos ácido-base más comunes y una interpretación práctica de los gases arteriales. Se utiliza una combinación de ilustraciones gráficas, tablas y algoritmos para facilitar la comprensión y el abordaje clínico de pacientes con alteraciones respiratorias y metabólicas.
