Se ha sugerido la relevancia del ácido siálico en los niveles y la función de las proteínas hemostáticas, pero hay pocos estudios disponibles. Nuestro objetivo fue analizar el contenido de ácido siálico y sus consecuencias funcionales en diferentes elementos hemostáticos en tres contextos. En el primer capítulo, estudiamos una paciente con trombosis recurrentes. Realizamos pruebas trombofílicas, análisis bioquímicos, funcionales, genéticos y glicómicos de antitrombina y otras proteínas plasmáticas en muestras seriadas. La paciente presentaba una nueva mutación en SERPINC1 (p.Ile218del), cuya asociación con una deficiencia de antitrombina tipo I se verificó en un modelo recombinante. Se observó una desialilación casi completa de proteínas plasmáticas mediante Western blot y experimentos con N-glicosidasa F y neuraminidasa, que se confirmó mediante espectrometría de masas para las glicoformas de transferrina. Sin embargo, se detectó desialilación parcial y patrones normales en muestras recogidas en otros momentos. Esta es la primera descripción de una desialilación global y transitoria de proteínas plasmáticas asociadas con trombosis. La disminución en la fuerte carga electronegativa asociada con la desialilación puede modular las interacciones proteína-proteína hemostáticas, lo que en combinación con una situación protrombótica grave, como la deficiencia de antitrombina, podría aumentar el riesgo de trombosis. En el segundo capítulo, realizamos un estudio multicéntrico de 35 pacientes con trombocitopenia inmune (PTI), 16 con PTI multirrefractaria. Datos recientes sugieren que anticuerpos antiplaquetarios específicos pueden causar la destrucción de las plaquetas al favorecer la pérdida de ácido siálico. Analizamos los anticuerpos antiplaquetarios, los niveles de trombopoyetina (TPO) y la desialilación de plaquetas. En 10 casos, se evaluó la respuesta a un nuevo tratamiento con oseltamivir (antiviral inhibidor de neuraminidasa). Descubrimos que los anticuerpos contra GPIbα estaban sobrerrepresentados en pacientes multirefractarios, quienes también presentaron un aumento significativo del estado de activación plaquetaria (secreción de gránulos) y desialilación (unión a RCA-1, restringida a plaquetas ya que las proteínas plasmáticas no estaban desialiladas), y una tendencia a niveles mayores de TPO. Cuando el oseltamivir se combinó con agonistas del receptor de TPO (TPO-RA) o con fármacos inmunosupresores, se observaron respuestas plaquetarias en el 66,7% de los pacientes. Todos los pacientes que respondieron presentaron anticuerpos reactivos solo contra GPIbα. Estos hallazgos sugieren que la desialilación puede desempeñar un papel patogénico clave en algunos pacientes con PTI multirrefractaria y proporcionar herramientas de diagnóstico para su identificación. Además, mostramos que el tratamiento con oseltamivir en combinación con terapias que ayudan a aumentar la producción de plaquetas puede inducir respuestas plaquetarias sostenidas en algunos pacientes con trombocitopenia mediada por anti-GPIbα sin respuesta a terapias previas. En el tercer capítulo desarrollamos un modelo in vitro de desialilación completa para estudiar su efecto sobre las proteínas clave de la coagulación y en procesos hemostáticos complejos como la coagulación, la fibrinólisis, la activación plaquetaria y la susceptibilidad a la hemólisis mediada por el complemento. La antitrombina y el factor XII desialilados mantuvieron su actividad. Las pruebas de coagulación global (tiempo de protrombina, tiempo de tromboplastina parcial activada, formación de coágulos y fibrinólisis) fueron similares en muestras con o sin tratamiento con neuraminidasa. Sin embargo, la desialilación completa revirtió el efecto de ciertas mutaciones en la antitrombina que afectan la afinidad por la heparina. Además, la desialilacion in vitro causa hiperreactividad plaquetaria. Estos resultados sugieren que en pacientes con otros factores de riesgo isquémico, la desialilación podría exacerbar su riesgo trombótico, y que los inhibidores de la neuraminidasa pueden ser útiles para disminuir la hiperreactividad plaquetaria sin aumentar el riesgo de sangrado. Finalmente, la desialilación completa no afectó significativamente la hemólisis, aunque se justifica un análisis más detallado en pacientes con variantes de factor H que aumentan el riesgo de síndrome urémico hemolítico. The relevance of sialic acid in the levels and function of hemostatic proteins have been suggested, but few studies are available. We aimed to analyze the sialic acid content and the functional consequences in different hemostatic elements from two in vivo situations and in an in vitro model. In the first section, we studied a patient suffering from early and recurrent venous and arterial thrombosis. We performed thrombophilic tests, biochemical, functional, genetic and glycomic analysis of antithrombin and other plasma proteins in serial samples. The patient carried a new SERPINC1 mutation (p.Ile218del), whose association with a type I deficiency of antithrombin was verified in a recombinant model. Additionally, a nearly full desialylation of plasma proteins was suggested by western blot and experiments with N-glycosidase F and neuraminidase, and was confirmed by mass spectrometry analysis of transferrin glycoforms. However, partial desialylation and normal patterns were detected in samples collected at other time-points. This is the first description of a global and transient desialylation of plasma proteins associated with thrombosis. The decrease in the strong electronegative charge of terminal glycans may modulate hemostatic protein-protein interactions, which in combination with a strong prothrombotic situation, such as antithrombin deficiency, could increase the risk of thrombosis. In the second section, we conducted a multicenter study including 35 patients with immune thrombocytopenia (ITP), including 16 with multirefractory disease. As recent data suggest that specific anti-platelet antibodies may cause destruction of platelets by favoring platelet loss of sialic acid, we analyzed antiplatelet-antibodies, thrombopoietin (TPO) levels, and platelet desialylation. In 10 cases, responses to a novel treatment strategy using oseltamivir (an antiviral neuraminidase inhibitor) were tested. We found that antibodies against GPIbα were overrepresented in multirefractory patients compared to responders, who also exhibited a significant increased platelet activation state (granule secretion) and desialylation (RCA-1 binding), and a trend toward higher plasma TPO concentrations. The decreased sialic acid content was restricted to platelet glycoproteins but did not affect other plasma proteins. When the antiviral drug was combined with TPO receptor agonists (TPO-RAs) or with immunosuppressant drugs, platelet responses were observed in 66.7% of patients. All responding patients presented with antibodies reactive only against GPIbα. These findings suggest that desialylation may play a key pathogenic role in some multirefractory ITP patients, and provide diagnostic tools for the identification of such patients. Furthermore, we show that sialidase inhibitor treatment in combination with therapies that help to increase platelet production can induce sustained platelet responses in some patients with anti-GPIbα -mediated thrombocytopenia that have failed previous therapies. In the third section, we developed an in vitro model of complete desialilation in order to study its effect on key coagulation proteins and on complex hemostatic processes such as coagulation, fibrinolysis, platelet activation and the susceptibility to complement-mediated hemolysis. Enzymatic removal of sialic acid from human antithrombin and factor XII had no effect on their activity. Global coagulation tests (prothrombin time, activated partial thromboplastin time, clot formation, and fibrinolysis) were similar in samples with or without neuraminidase treatment. However, complete desialylation reversed the effect of certain mutations in antithrombin that affect the heparin affinity. These findings suggest the possibility of testing antithrombin concentrates with lower sialic acid content in these specific patients. Furthermore, this treatment leads to platelet hyperreactivity. These results suggests that in patients with other ischemic risk factors, desialylation could exacerbate their thrombotic risk and that neuraminidase inhibitors may be useful in decreasing platelet hyperreactivity without increasing the risk of bleeding. Finally, complete desialylation did not significantly affect hemolysis, although a more detailed analysis is warrated in patients with factor H polymorphisms that increase the risk of hemolytic uremic syndrome.

[Resumen] Desarrollamos un marco conceptual que permite que los alumnos desarrollen las competencias específicas en el ámbito de la disciplina de Dirección Estratégica y Políticas de Empresa. Para ello, y utilizando como punto de partida las competencias establecidas en el plan de estudios de grado en Dirección y Administración de Empresas de la Universidad de A Coruña, se presenta un esquema de la dinámica del proceso de enseñanza-aprendizaje que, incorporando aspectos del contexto educativo y de la propia motivación del estudiante, permite una planificación de tareas orientada al desarrollo de competencias. El resultado de esta experiencia ha permitido documentar el análisis y formulación estratégica de 135 empresas-caso, implicando a los estudiantes en la solución de problemas de su entorno empresarial, objetivo clave de la disciplina de Dirección Estratégica y Política Empresarial I. [Abstract] We develop a conceptual framework that allows students to develop specific competencies in the field of Strategic Management and Company Policies. To do this, and using as a starting point the competences established in the degree program in Business Administration and Management of the University of A Coruña, we present a diagram of the dynamics of the teaching-learning process, including aspects of the educational context and the student's own motivation. The final goal of this framework is to allow a better planning of tasks focused on the development of students competences. This experience has allowed to understand the strategic analysis and business formulation of 135 companies-case, involving students in the solution of problems of their business environment, which is a key objective of the discipline of Strategic Management and Business Policy I.
