However, in the emergency setting, accurate diagnosis is not always possible.Methods: Case report and review of the literature.Results: A 73-year-old woman was admitted to the emergency department with acute aphasia and right hemiparesis. The National Institute of Health Stroke Score was 21, compatible with severe stroke, so she received thrombolysis. Laboratory testing demonstrated severe hypomagnesemia. She had been taking proton pump inhibitors for years and neuroimaging did not demonstrate signs of acute ischemic disease. After correcting the metabolic alterations with intravenous and oral supplemental magnesium, the patient was discharged asymptomatic. No further episodes have been registered to date.Conclusion: Hypomagnesemia might cause acute neurological symptoms that could be confused with stroke. A careful history is essential for diagnosis but suspicion of stroke mimic should not prevent tPA administration. Keywords: hypomagnesemia, stroke mimic, aphasia, stroke

Introduction: Cerebral venous thrombosis (CVT) is an uncommon cause of stroke that mainly affects young adults. Early, accurate diagnosis can reduce the rate and severity of complications. Objective: The aim of this study was to analyse the clinical characteristics, management, and treatment of CVT in different centres in Spain. Methods: We conducted a multicentre, retrospective, descriptive study of patients hospitalised due to CVT between 2008 and 2017 at 11 Spanish centres. Results: We included 256 patients, with a mean age (SD) of 49.8 (18.7) years; 51% of patients were women. The most frequent symptoms were headache (73%), focal deficits (50%), epileptic seizures (33%), and encephalopathy (21%). The most frequent localisations were the superior sagittal sinus (12.5%), the transverse sinus (10.9%), and 2 or more sinuses or veins (66.4%). Thrombophilia was the most frequent known aetiology (24%), and was most commonly associated with the prothrombin G20210A mutation (19%). Forty-six percent of patients were treated with antithrombotics for 3-6 months, 21% for one year, and 22.6% required indefinite anticoagulation. Endovascular therapy was performed in 5% of cases, and 33% required neurosurgery. Regarding outcomes, 75% of patients were independent at 3 months (modified Rankin Scale [mRS] score = 2), with papilloedema (P =.03), focal deficits (P =.001), and encephalopathy (P 3). The in-hospital mortality rate was 4.3%, with a 3-month mortality rate of 6.3%. Conclusion: The diverse risk factors and variable presentation of CVT represent a challenge in the diagnosis and treatment of this condition. To improve prognosis and reduce mortality, it is essential to establish management protocols for this entity. © 2021 Sociedad Española de Neurología

34. Healthcare access for autistic adults A systematic review

Abstract

BACKGROUND: People with autism spectrum disorder (ASD) have an increased susceptibility for many chronic health conditions compared with their peers. An increasing number of adolescents are transitioning from pediatric to adult healthcare services. Thus, being able to access appropriate healthcare services that can not only address specific needs of the person but enable them to better manage healthcare conditions and decrease the development of preventable disease is necessary. A systematic review was conducted to identify barriers and enablers of healthcare access for autistic adults. METHODS: The studies included in the review were quantitative and qualitative and were published between 2003 and 2019. The participants for the review are considered to be adults (over 18 years of age) with a primary diagnosis of ASD. RESULTS: In total, 1290 studies were initially identified and 13 studies were included based on the inclusion and exclusion criteria outlined in a previous protocol paper. The analysis of these studies identified areas of concern to access appropriate healthcare, such as clinician knowledge, the environment, and life events. CONCLUSION: Identifying the barriers to healthcare, highlights ways healthcare services can regulate scope of practice, the physical environment, and the process of managing health conditions, thus, autistic adults can strive for optimal health. This review contributes to peer-reviewed evidence for future research and up-to-date information when developing and piloting health interventions for autistic adults. ETHICS AND DISSEMINATION: There are no human participants, data, or tissue being directly studied for the purposes of the review; therefore, ethics approval and consent to participate is not applicable. Many general practitioners feel inadequately trained and supported when managing the complex needs of this cohort, and individuals often present to hospital in crisis. The aim of this project was to better understand their healthcare needs and hospital service usage. Method(s): Study design included file reviews of patients attending a specialist clinic. Hospital usage data was extracted for demographic analysis. Result(s): Of 806 specialist clinic patients: 95% required support to communicate their health history. There were high percentages of comorbidities including obesity-overweight (38%), epilepsy (33%), Vitamin D deficiency (21%), and vision impairment (20%). Of 194 hospital patients, usage data highlighted: average age of admission 36 years, 69% presented via Emergency Department, average length of stay 5.4 days. Intellectual disability was coded in 2% of 998 Emergency Department presentations, and 14% of 1409 inpatient admissions. Implications: Results suggest patients with intellectual disabilities have multiple health needs and communication difficulty. Limited coding of intellectual disability in hospital patients limits understanding of service usage, health outcomes and patient experience. Consumer experience requires further investigation to help begin informing future service development.

Abstract

Introduction: Adults with intellectual disability often have multiple complex healthcare needs, poor health outcomes and may present to hospital in crisis. Health services struggle to understand and provide for their interrelated health and social needs. The aim of this project was to address the healthcare needs of adults with intellectual disability who are inpatients within a health network. Method(s): A model of care for a Complex Disability Liaison Service was designed and implemented in an Australian hospital. Data was collected by triage forms and clinical assessment. Consultation was provided and resources developed to support inpatient care. Evaluation data was obtained through patient, carer, and clinician surveys. Result(s): The pilot service has supported patients and clinicians with assessments, management advice and discharge planning. Results suggest that the interdisciplinary specialist service provided valued advice and support, effective resources and education, and coordinated care pathways to support healthcare. Implications: The Complex Disability Liaison Service was effective in identifying gaps in service provision, increased awareness of patient need, capacity building of clinicians and development of useful resources. Longer term evaluation of the service is required to determine the impact on patient health and wellbeing and service usage.

Abstract

INTRODUCTION: Adults living with autism spectrum disorder (ASD) can experience many factors that may impact their everyday lives. Striving for optimal health and enduring a healthy lifestyle comes with the ability to access appropriate healthcare services, yet adults with ASD have unmet healthcare needs. The barriers and enablers of healthcare access for adults with ASD remain unclear. We will conduct a systematic review to explore what is currently known about healthcare access for adults with ASD, this will determine the level and appropriateness of access to healthcare services to better support the lives of adults with ASD. METHOD AND ANALYSIS: The systematic review will report on all studies that include quantitative, qualitative, and mixed-methods designs that consider healthcare access for adults with ASD. We will search 5 databases: EBSCOhost, Scopus, PubMed, the Cochrane Library, and Web of Science. The Mixed Methods Appraisal Tool (MMAT) will be used to assess quality of articles and the Cochrane RoB 2.0 Tool will be used to assess for bias. Clarifying the evidence in this area will be important for future research directions when developing and piloting health interventions for researchers and healthcare clinicians in the field. ETHICS AND DISSEMINATION: There are no human participants, data, or tissue being directly studied for the purposes of the review; therefore, ethics approval and consent to participate is not applicable. Abstract

Background and Aims:There are few studies regarding utility of transcranial duplex sonography (TDS) in intracranial hemorrhage (IH). TDS is a non-invasive tool that can be easily used in patientu00b4s bed-side.Methods:Consecutive patients with spontaneous IH admitted to a stroke unit were recruited. TDS was performed within 12 hours of baseline CT and up to 3 control studies were scheduled every 24 hours. Included neurosonological markers were: hematoma volume (HV), midline shift (MLS), third ventricle diameter, pulsatility index and mean velocity (MV) in both MCAs and mean thickness of both optic nerve sheaths (MTONS). Correlation of HV and MLS between CT and TDS was analyzed if the time between both was less than 2 hours. As prognostic variables we evaluated early neurologic deterioration (END) and death during hospitalization (DDH).Results:33 patients were included (median age 74, 78% men). Median HV was 19 ml (11-52). A good correlation with CT was found (n=12) regarding both HV (ICC=0.93) and MLS (ICC=0.82).
