Abstract

There is a widely and deeply held belief on the part of the author's white colleagues that racism is something done by individuals through overt or covert actions against other individuals. Sometimes this belief is tied to one specific characteristic--skin color. Some would believe that they cannot commit racist acts against anyone who looks like them regardless of the identified race of that person. As a clever trick of psychological avoidance, this worldview allows many mainstream faculty to remain comfortably aloof to the infrastructures, the cultural, social, and academic "standards," that contribute to the unseen barriers American Indian colleagues must surmount. In this article, the author describes the cultural and religious barriers of American Indian faculty in the academy. (Contains 34 notes.)

Abstract

The Reconstruction Acts of 1867 brought significant social changes to the American South, including the enfranchisement of Black men and the placement of Southern states under the authority of the U.S. military. While Black Southerners celebrated their newfound rights, many White residents opposed federal occupation and Black suffrage. However, when feeling threatened by freedmen engaging in politics, White residents in St. Landry Parish, Louisiana, petitioned for U.S. military intervention. The presence of U.S. troops led to the disarming and demobilization of the Black militia, which ultimately contributed to the Opelousas Massacre and weeks of racial violence. Abstract

The management of musculoskeletal disorders is an increasing challenge to clinicians. Successful treatment relies on a wide range of multidisciplinary interventions. Adjunctive hyperbaric oxygen (HBO) therapy has been used as an orthopaedic treatment for several decades. Positive outcomes have been reported by many authors for orthopaedic infections, wound healing, delayed union and non-union of fractures, acute traumatic ischemia of the extremities, compromised grafts, and burn injuries. Severe side effects have also been reported with this therapy. To aid in the use of HBO therapy in orthopaedics, we reviewed 43 papers published in the past four decades and summarised the mechanisms, effectiveness, indications and contraindications, side effects, and cost impact of adjunctive hyperbaric oxygen therapy in the management of difficult musculoskeletal disorders. Adjunctive HBO therapy is an effective treatment modality for the management of some severe and refractory musculoskeletal problems. If appropriate candidates are carefully identified, hyperbaric oxygen is a limb- and sometimes life-saving therapy. HBO therapy significantly reduces the length of the patient′s hospital stay, amputation rate, and wound care expenses. Thus, it is a cost-effective modality. A clinician must understand the side effects and risks of HBO treatment. Close monitoring throughout the treatment is warranted to minimise the risk to the patients.

27. Incomplete reporting of enhanced recovery elements and its impact on achieving quality improvement

Abstract

BACKGROUND: Enhanced recovery (ER) protocols are used widely in surgical practice. As protocols are multidisciplinary with multiple components, it is difficult to compare and contrast reports. The present study examined compliance and transferability to clinical practice among ER publications related to colorectal surgery.METHODS: PubMed, Embase and Cochrane Central Register databases were searched for current colorectal ER manuscripts. Each publication was assessed for the number of ER elements, whether the element was explained sufficiently so that it could be transferred to clinical practice, and compliance with the ER element.RESULTS: Some 50 publications met the reporting criteria for inclusion. A total of 22 ER elements were described. The median number of elements included in each publication was 9, and the median number of included patients was 130. The elements most commonly included in ER pathways were early postoperative diet advancement (49, 98 per cent) and early mobilization (47, 94 per cent). Early diet advancement was sufficiently explained in 43 (86 per cent) of the 50 publications, but only 22 (45 per cent) of 49 listing the variable reported compliance. The explanation for early mobilization was satisfactory in 41 (82 per cent) of the 50 publications, although only 14 (30 per cent) of 47 listing the variable reported compliance. Other ER elements had similar rates of explanation and compliance. The most frequently analysed outcome measures were morbidity (49, 98 per cent), length of stay (47, 94 per cent) and mortality (45, 90 per cent).CONCLUSION: The current standard of reporting is frequently incomplete. To transfer knowledge and facilitate implementation of pathways that demonstrate improvements in perioperative care and recovery, a consistent structured reporting platform is needed.

Computer studies were conducted to analyze the potential gains associated with utilizing new airfoils for large wind turbine rotor blades. Attempts to include 3-dimensional stalling effects were inconclusive. It is recommended that blade pressure measurements be made to clarify the nature of blade stalling. It is also recommended that new laminar flow airfoils be used as rotor blade sections.

Abstract

Wind tunnel tests have been carried out to develop a spoiler lateral control system for use with the GA(W)-1 airfoil with a 30% Fowler flap. Tests show that unfavorable aerodynamic interactions can occur between spoiler and flap for large flap deflections. Providing venting of lower surface air through the spoiler opening substantially improves performance. Results of tests with a number of spoiler and cavity shapes are presented and discussed. Applications of two-dimensional wind tunnel results to the design of satisfactory manual lateral control systems are discussed.

43. Feasibility study of aileron and spoiler control systems for large horizontal axis wind turbines

Abstract

The feasibility of using aileron or spoiler controls as alternates to pitch control for large horizontal axis wind turbines was studied. The NASA Mod-0 100 kw machine was used as the basis for the study. Specific performance studies were conducted for 20% chord ailerons over the outboard 30% span, and for 10% chord spoilers over the same portion of the span. Both control systems utilized control deflections up to 60 deg. Results of the study show that either ailerons or spoilers can provide the control necessary to limit turbine power in high wind conditions. The aileron system, as designed, provides overspeed protection at hurricane wind speeds, low wind speed starting torque of 778 N-m (574 ft. lb) at 3.6 m/sec, and a 1.3 to 1.5% increase in annual energy compared to a fixed pitch rotor. The aileron control system preliminary design study includes aileron loads analysis and the design of a failsafe flyweight actuator for overspeed protection in the event of a hydraulic system failure.

Abstract

The International Space Station (ISS) has been operating in space for over 14 years and permanently crewed for over 12 years. Throughout this time, the ISS Program has implemented and continually improved its risk management process. This ISS risk management process identifies risks that may exist and alternatives for mitigation if the risks are significant. Risk management has provided input that has been essential to the program management decision making process. Risks can be reduced, mitigated or accepted and are prioritized to ensure program resources are used as efficiently as possible. The ISS risk management process has increased the probability of success of the ISS mission objectives and overall crew safety. This paper presents two case studies on how the ISS risk management process has been used to address specific areas of risk. The first case describes how the ISS Program has analyzed and mitigated the risk of Micrometeoroid Orbital Debris (MMOD) strikes to pressurized modules. MMOD has been a major safety concern since the beginning of the program. The second case study describes the risk assessment of Visiting Vehicle (VV) collisions. There has been a significant increase in the number of visiting vehicle flights to the ISS since the retirement of the Space Shuttle Program. This has caused an elevated cumulative risk of collision when considering the frequency of vehicle traffic to the ISS. [ABSTRACT FROM AUTHOR]
