Abstract

Simple Summary: Advanced ovarian cancer (Stages III-IV) continues to be one of the gynecological tumors with the highest mortality. Standard treatment consists of debulking surgery and subsequent adjuvant chemotherapy. Recently, some authors have postulated that the administration of hyperthermic chemotherapy during surgery could increase the survival of patients, especially in cases in which chemotherapy had already been administered before surgery to reduce tumor volume. Our study is important because it collects data from 11 tertiary hospitals in Spain, and the data are subjected to a statistical technique that reproduces the data that we would find in a prospective study but using retrospective data (propensity score matching). It also offers a current view of the status of ovarian cancer treatment in our country. Introduction: Epithelial ovarian cancer (EOC) is primarily confined to the peritoneal cavity. When primary complete surgery is not possible, neoadjuvant chemotherapy (NACT) is provided; however, the peritoneum-plasma barrier hinders the drug effect. The intraperitoneal administration of chemotherapy could eliminate residual microscopic peritoneal tumor cells and increase this effect by hyperthermia. Intraperitoneal hyperthermic chemotherapy (HIPEC) after interval cytoreductive surgery could improve outcomes in terms of disease-free survival (DFS) and overall survival (OS). Materials and Methods: A multicenter, retrospective observational study of advanced EOC patients who underwent interval cytoreductive surgery alone (CRSnoH) or interval cytoreductive surgery plus HIPEC (CRSH) was carried out in Spain between 07/2012 and 12/2021. A total of 515 patients were selected. Progression-free survival (PFS) and OS analyses were performed. The series of patients who underwent CRSH or CRSnoH was balanced regarding the risk factors using a statistical analysis technique called propensity score matching. Results: A total of 170 patients were included in each subgroup. The complete surgery rate was similar in both groups (79.4% vs. 84.7%). The median PFS times were 16 and 13 months in the CRSH and CRSnoH groups, respectively (Hazard ratio (HR) 0.74; 95% CI, 0.58–0.94; p = 0.031). The median OS times were 56 and 50 months in the CRSH and CRSnoH groups, respectively (HR, 0.88; 95% CI, 0.64–1.20; p = 0.44). There was no increase in complications in the CRSH group. Conclusion: The addition of HIPEC after interval cytoreductive surgery is safe and increases DFS in advanced EOC patients. [ABSTRACT FROM AUTHOR]

Abstract

The legal institution of substantial changes in working conditions is currently regulated in article 41 of the Law on the Workers' Statute. This has been object of several reforms in its regulation in recent years, especially in 2012, whose aimed to achieve greater internal flexibility, productivity and competitiveness in the field of companies. In this legal concept, we can find a number of affected subjects, such as the working day, the schedule and distribution of working time, the work shift regime, the system of remuneration and salary amount or system of work, among other possible. Therefore, the main objective of this Final Master's Project (TFM) is to carry out an exhaustive analysis of what the substantial modifications of working conditions entail, also from a jurisprudential point of view, taking into account the numerous judicial sentences existing in the matter. In addition, we will emphasize the situation of the worker in the face of a substantial modification of their working conditions, as the backbone of any company and productive system; as well as the legal procedure required to make such modifications, whether they are individual or collective. At this point, we will try to claim the need for jobs and working conditions that suit the needs of workers and not so much of companies. La institución jurídica de las modificaciones sustanciales de las condiciones de trabajo, regulada actualmente en el artículo 41 de la Ley del Estatuto de los Trabajadores, ha sido objeto de varias reformas en su regulación en los últimos años, especialmente en el 2012, cuyo objetivo fue conseguir una mayor flexibilidad interna, productividad y competitividad en el ámbito de las empresas. Dentro de este concepto jurídico, podemos encontrar afectadas una serie de materias, como pueden ser la jornada de trabajo, el horario y distribución del tiempo de trabajo, el régimen de trabajo a turnos, el sistema de remuneración y cuantía salarial o sistema de trabajo, entre otras posibles. Por ello, el objetivo principal de este Trabajo de Fin de Máster (TFM) es realizar un exhaustivo análisis de lo que suponen las modificaciones sustanciales de condiciones de trabajo, también desde un punto de vista jurisprudencial, teniendo en cuenta las numerosas sentencias judiciales existentes en la materia. Además, haremos hincapié en la situación del trabajador ante una modificación sustancial de sus condiciones de trabajo, como eje vertebrador de cualquier empresa y sistema productivo; así como el procedimiento legal exigible para practicar dichas modificaciones, bien sean éstas de carácter individual o colectivo. En este punto, trataremos de reivindicar la necesidad de puestos y condiciones de trabajo que se adecúen a las necesidades de los trabajadores y no tanto de las empresas

Abstract

Diagonal fue un periódico independiente que nació en 2004, en Madrid, con la idea de aglutinar toda la información sobre los movimientos sociales existentes en España. Diagonal apostó por un periodismo crítico y de interés social y por la imagen como una forma de explicar la realidad. El medio contaba con un área específica para la fotografía, por ello, hay que estudiar su archivo fotográfico. Esta área fue creada por el fotoperiodista Olmo Calvo, y estaba organizada por Álvaro Minguito y David Fernández. Juntos coordinaban una sección dentro de Diagonal destinada íntegramente al reportaje gráfico: Enfoques. Esta investigación se centrará en la importancia que tuvo la fotografía en Diagonal, se verá no solo a través de la sección Enfoques sino también revisando las portadas y el número total de imágenes incluídas en cada ejemplar. Se pretende mostrar cómo, en unos tiempos tan convulsos para el fotoperiodismo, Diagonal lo apoyó y publicó a fotoperiodistas freelance para que se pudieran visualizar sus trabajos en papel. Es interesante también clasificar las temáticas abordadas en los reportajes gráficos de Enfoques para ver qué tipo de periodismo llevó a cabo. Diagonal was an independent newspaper that was born in 2004, in Madrid. The objective of this newspaper was to gather all the information about social movements in Spain. Diagonal was a critical journalist. The contents of Diagonal had great social interest. Reality can be explained very well with images. Therefore, this newspaper decided to include many images on its pages. This means of communication created a photographic area, therefore, it had an important photographic archive. Currently, photographic archives have disappeared from most newsrooms in the world. The photojournalist Olmo Calvo created the photographic area of Diagonal, together with Álvaro Minguito and David Fernández. The three photographers made the section Enfoques in Diagonal. This research will focus on the importance that photography has had for Diagonal, for this, it is necessary to observe the characteristics of Enfoques, to check whether or not there are photographs on the covers of Diagonal, and to know the total number of images of each copy. This research aims to demonstrate how Diagonal opted for photojournalism in a complicated period for photojournalism.

Abstract

El turismo es un sistema complejo compuesto por diferentes elementos. Los recursos son claves para elaborar productos turísticos. El senderismo es una actividad deportiva no competitiva que explota el patrimonio natural y cultural por donde discurre. La Palma es pionera en este tipo de actividades y por ello se ha elegido para realizar este TFG. Se ha seleccionado el GR 130-GR 131 ya que cubre gran parte de la superficie insular. El objetivo del TFG es inventariar los atractivos en los diferentes tramos del GR en la isla. La metodología empleada es la clasificación de inventarios propuesto por la OEA. Los principales resultados muestran que hay una gran diversidad de atractivos, predominando los naturales y realizaciones contemporáneas en el 100% de los tramos frente al folclore que solo está presente en el 0,9% de los mismos. En general no se explotan bien los atractivos por falta de contenidos. Tourism is an intricate system formed by different factors. La Palma leads the way in this kind of activity, and this is the reason why it has been chosen for this final project. The project is focused on GR 130-GR 131, because these great routes almost embrace the whole island surface area. The aim of the survey is to establish an inventory of the main tourist attractions on each trail section in this island. The methodology used is the OAS tourism resources classification. The results show the great range of attractions, prevailing natural resources and contemporary manifestations in a 100% of the trail segments, in contrast with folklore, which is present only in a 0,9%. Basically the attractions are not being successfully exploited due to a lack of content.

Abstract

[EN] Structurally, the project addresses the analysis of working digitally on body three-dimensional space and its study focuses on the responses originate experimentally after the pattern making transfer process to a body by an anthropometric study mapped image. The investigation is focused on the study of the current context, to new types of consumption, connected in this cultural and socio-economic scenario. Creating a workflow methodology subject to analysis of experimentally synchronized with each other softwares under low cost systems, supported by an open source, contrasting attitudes to commercial use. Inclusively it is organized from recent technological developments, adapting to the changing needs of professionals in the fashion industry to achieve a personal production of experimental base, which will result in a workspace of digital development of interfaces and production of personal design, adapted to the end user. Concludes the guide line project research, DNA mutation fashion industry, where the market has been in constant demand own style. Decomposing the wide range of ideas and encapsulated in the current production model designs, the pattern making cad industry can not reinvents as speed as technological change contemporaries. Its presents everyday life intertwined with technology, new scenarios cause more accessible and more open economic characteristics., [ES] Estructuralmente el proyecto aborda el análisis de trabajar digitalmente sobre el espacio tridimensional corporal y su estudio versa, sobre las respuestas que se originan experimentalmente tras el proceso de traslado del patrón a un cuerpo mediante el estudio antropométrico de una imagen mapeada. La investigación se halla centrada en el estudio del contexto actual, ante las nuevas tipologías de consumo, conectadas en el presente escenario cultural y socio-económico. Creación de una metodología de proceso de trabajo sujeta al análisis de softwares experimentalmente sincronizados entre sí, bajo sistemas de bajo coste, apoyados en un código abierto, contraponiendo las actitudes comerciales a las de uso. Inclusivamente se organiza a partir de la evolución tecnológica reciente, adaptándose a las nuevas necesidades de los profesionales del sector moda para alcanzar una fabricación personal de base experimental, que dará lugar a un espacio de trabajo de elaboración digital de interfaces y producción de diseño personal, adaptada al usuario final. Concluye la línea guía de la investigación proyectual, el propio ADN de la mutación del sector moda, donde el mercado se encuentra en constante reclamo de estilo. Descomponiendo el amplio abanico de ideas y diseños encapsulados en el modelo productivo actual, el sector patronístico no se reinventa a la velocidad de los cambios tecnológicos contemporáneos. El negocio de la moda, está más vivo que nunca y ávido de nuevas acciones. 34. [Is there any indication for inserting a drain in elective laparoscopic cholecystectomy?].

Abstract

Introduction: Classically, a sub-hepatic drain was inserted routinely in a cholecystectomy to prevent intra-abdominal abscesses, possible post-surgical bleeding, and biliary fistulas. Over the years, it has been demonstrated that the systematic use of a drain does not have any benefits, and many studies conclude that, in special circumstances (bleeding, signs of gallbladder inflammation, incidental opening, or suspected bile leak), and depending on the experience of the individual surgeon, the insertion of a drain may be of use., Material and Methods: [corrected] A prospective study was conducted on 100 elective laparoscopic cholecystectomies performed due to symptomatic cholelithiasis or gallbladder polyps. A sub-hepatic drain was inserted in 15 of them. The indications for inserting it were: in 11 patients as a "control" due to a gallbladder bed bleed controlled during surgery, and in 4 due to a gallbladder opening with the excretion of turbid-purulent bile. The main outcomes investigated were the clinical benefit achieved by the insertion of the drain, the hospital stay, and the quantifying of the pain by the patients 24h after surgery, using a visual analogue scale., Results: The insertion of a drain was of no benefit to any patient. The median hospital stay increased by 1 day in patients with a drain (P=.002). The median pain score at 24h was higher in patients with a drain inserted (P=.018)., Conclusion: The insertion of a sub-hepatic drain after elective laparoscopic cholecystectomy increases post-surgical pain and prolongs hospital stay, and does not prevent the occurrence of intra-abdominal abscesses., (Copyright © 2011 AEC.
