The present study determined whether modafinil (100-mgs after 17, 22, and 27 hours without sleep) attenuated the effects of fatigue on fighter-pilot alertness and performance. A quasi-experimental, single-blind, counterbalanced design was used in which 5 pilots from a previous F-117 fatigue study (in which no fatigue remedy was employed) were retested with modafinil. Their data were combined with the data from 5 newly-recruited F-117 pilots who were evaluated under modafinil and then placebo. Modafinil improved vigilance and tracking performance in a divided-attention task, CNS activation, oculomotor performance, and aspects of subjective mood. Flight performance decrements were mitigated on six of eight maneuvers. Benefits were most noticeable after 24 to 32 hours of continuous wakefulness. Although modafinil did not sustain performance at pre-deprivation levels, its numerous positive effects make it a useful adjunct to the currently-approved fatigue countermeasure dextroamphetamine. However, modafinil should not be considered a replacement for this older compound. A follow-on in-flight study is recommended.

The efficacy of Dexedrine for sustaining the performance of six female UH-60 pilots under conditions of moderate sleep deprivation was determined. Each subject was exposed to one week of testing which included 2, 40-hour sleep deprivation periods. During both deprivation periods, there were 5 equally-spaced test sessions, each of which included a UH-60 simulator flight, an electroencephalographic (EEG) evaluation, a test on the Microsoft Flight Simulator, a Profile of Mood States questionnaire, and a cognitive evaluation on the Synthetic Work Battery. These testing sessions began at 0100, 0500, 0900, 1300, and 1700. During one of the deprivation periods, subjects were administered a 10-mg dose of Dexedrine one hour prior to each of the first three sessions (for a total of 30 mgs). During the other deprivation period, subjects were administered placebos. A double-blind administration scheme was used. Vital signs were monitored at regular intervals throughout the study.

Nonadherence to therapeutic regimens which necessitate major lifestyle change is recognized as one of the most serious problems in medical practice today. Attempts to prevent and control chronic disease, including the development and progression of cardiovascular disease, are only effective when recommendations to the patient are carried out. Few studies have systematically studied the relationship of psychosocial and behavioral factors to adherence outside the theoretical framework of a specific behavioral model; no study has done so with a carefully measured diet as the outcome variable in a controlled prospective clinical trial with patients suffering from cardiovascular disease.
