1. Osteoporosis and Medication-Related Osteonecrosis of the Jaws

Abstract

Osteoporosis is a metabolic bone disease characterized by decreased bone strength and an increased susceptibility to fractures. Anti-resorptive therapies are extremely effective in preventing skeletal adverse events in osteoporotic patients. However, these drugs have been also recognized as a principal risk factor for the development of medication-related osteonecrosis of the jaws (MRONJ). MRONJ consists of non-healing and necrosis of the bone associated with symptoms and infection. Although MRONJ management continues to be challenging for clinicians, data suggests a high rate of success for surgical treatments.

Malignancies of the oral mucosa comprise a wide variety of neoplasms. Beside the most common oral squamous cell carcinoma and salivary gland malignancies, there are other malignancies affecting the oral cavity, which are worth of consideration due to their local and systemic severity. In this section, three malignancies will be discussed: oral melanoma, oral lymphomas, and oral Kaposi sarcoma.

Abstract

Background: The wellbeing of oral lichen planus patients (OLPs) may be strongly influenced by a poor quality of sleep (QoS) and psychological impairment. The aims were to analyze the prevalence of sleep disturbance, anxiety, and depression in OLPs and to validate the Pittsburgh Sleep Quality Index (PSQI) in OLPs. Methods: Three hundred keratotic OLPs (K-OLPs), 300 with predominant non-keratotic OLP (nK-OLPs), and 300 controls were recruited in 15 Italian universities. The PSQI, Epworth Sleepiness Scale (ESS), Hamilton Rating Scales for Depression and Anxiety (HAM-D and HAM-A), Numeric Rating Scale (NRS), and Total Pain Rating Index (T-PRI) were administered. Results: Oral lichen planus patients had statistically higher scores than the controls in the majority of the PSQI sub-items (p-values < 0.001**). Moreover, OLPs had higher scores in the HAM-D, HAM-A, NRS, and T-PRI (p-values < 0.001**). No differences in the PSQI sub-items’ scores were found between the K-OLPs and nK-OLPs, although nK-OLPs suffered from higher levels of anxiety, depression, and pain (p-values: HAM-A, 0.007**, HAM-D, 0.009**, NRS, <0.001**, T-PRI, <0.001**). The female gender, anxiety, depression (p-value: 0.007**, 0.001**, 0.020*) and the intensity of pain, anxiety, and depression (p-value: 0.006**, <0.001**, 0.014*) were independent predictors of poor sleep (PSQI > 5) in K-OLPs and nK-OLPs, respectively. The PSQI’s validation demonstrated good internal consistency and reliability of both the total and subscale of the PSQI. Conclusions: The OLPs reported an overall impaired QoS, which seemed to be an independent parameter according to the regression analysis. Hence, clinicians should assess QoS in OLPs and treat sleep disturbances in order to improve OLPs management.

Abstract

Objectives: To analyze psychological profiles, pain, and oral symptoms in patients with oral lichen planus (OLP). Materials and methods: 300 patients with keratotic OLP (K-OLP; reticular, papular, plaque-like subtypes), 300 patients with predominant non-keratotic OLP (nK-OLP; erythematosus atrophic, erosive, ulcerative, bullous subtypes), and 300 controls were recruited in 15 universities. The number of oral sites involved and oral symptoms were recorded. The Numeric Rating Scale (NRS), Total Pain Rating Index (T-PRI), Hamilton Rating Scales for Depression and for Anxiety (HAM-D and HAM-A), Pittsburgh Sleep Quality Index (PSQI), and Epworth Sleepiness Scale (ESS) were administered. Results: The OLP patients, especially the nK-OLP, showed higher scores in the NRS, T-PRI, HAM-D, HAM-A and PSQI compared with the controls (p-value <.001**). A positive correlation between the NRS, T-PRI, HAM-A, HAM-D, and PSQI was found with the number of oral symptoms and number of oral sites involved. Pain was reported in 67.3% of nK-OLP and 49.7% of K-OLP cases with poor correspondence between the site of lesions and the site of the symptoms. Conclusions: Mood disorders are frequently associated with OLP with an unexpected symptomatology correlated with the number of oral symptoms and with the extension of disease suggesting a peripheral neuropathy.

11. Subjects of self-reliance: a critical history of refugees and development

Abstract

In four empirical chapters spanning three regions of the world in the 20th and 21st centuries, I examine continuities and changes in institutional assistance to foster refugee self-reliance. I employ archival and ethnographic methods to document assistance practices and forms of implementation. I draw upon programme and evaluation reports of the League of Nations and UNHCR as key primary texts due to these institutions’ historical influence and prominence in refugee assistance, as well as private collections by individual assistance actors. I find that efforts to foster refugee self-reliance have largely occurred through development projects targeting both refugees and locals, and even entire regions; in so doing, this assistance has largely treated refugees as workers in need of employment. This ‘refugee self-reliance assistance’, as I term it, has been a main feature of refugee assistance yet has been hitherto neglected in academic scholarship in Refugee Studies as well as International Development. Through a Marxian reading of history I link refugee self-reliance assistance to material interests, outcomes, and changes. Drawing on critical welfare studies, I develop a theoretical framework of ‘international welfare’ and employ the theoretical concepts of instrumentalisation and reserve army of labour to explain how refugee self-reliance has become an instrument that alternately serves and exemplifies changes in social, political, and economic structures. The rhetoric surrounding refugee self-reliance belies shifting interests and their underlying values – whether self-reliance is espoused as an economic imperative, a protection instrument, or a human right, for example. The identification of these linkages has implications for understanding the conditions under which refugee self-reliance is ‘fostered’, for analysing the means through which it is intended to be attained, and its explicit and implicit outcomes. Through this examination, my thesis reveals that refugee self-reliance is not an end in itself but instead a malleable instrument to achieve economic gain, political exploits, and social control.

12. High Intensity Interval Training Does Not Have Compensatory Effects on Physical Activity Levels in Older Adults

Abstract

Background: Exercise has beneficial effects on older adults, but controversy surrounds the purported “compensatory effects” that training may have on total daily physical activity and energy expenditure in the elderly. We wanted to determine whether 8 weeks of high-intensity interval training (HIIT) induced such effects on physical activity and energy expenditure in healthy, active older adult men. Methods: Twenty-four healthy elderly male volunteers were randomized to two groups. The experimental group performed HIIT (7 × 2 min cycling repetitions, 3 d/w); the control group performed continuous moderate-intensity training (20–30 min cycling, 3 d/w). Physical activity and energy expenditure were measured with a multisensor activity monitor SenseWear Armband Mini. Results: During HIIT, significant changes were observed in moderate and vigorous physical activity, average daily metabolic equivalents (METs), physical activity level, and activity energy expenditure (p < 0.05) but not in total energy expenditure. Sleep and sedentary time, and levels of light physical activity remained constant during the training period. Conclusions: The findings suggest that HIIT induced no compensatory effect: HIIT does not adversely affect lifestyle, as it does not reduce daily energy expenditure and/or increase sedentary time.

Abstract

Permeabilized muscle fibers (pfi) are widely used to assess mitochondrial (mt) respiratory function in skeletal muscle of various models in different physiological and pathological conditions. Facing the numerous data available for mt-respiration from the literature, it remains challenging to determine what the right values are for a specific respiratory protocol. Moreover, mt-respiration values are highly dependent on pfi preparation, which required good technical skills. In the frame of COST Action MITOEAGLE, one of the objectives of WG2 is the generation of reference values for mitochondrial respirometry in permeabilized skeletal muscle sample preparations. The idea is that new researchers in the field follow a reference protocol and check if their values are in an acceptable range. This approach could serve to test researchers’ technical skills and therefore determine if they are proficient enough to perform their own experiments with confidence.

Abstract

As the knowledge base and importance of mitochondrial physiology to human health expands, the necessity for harmonizing the terminologyconcerning mitochondrial respiratory states and rates has become increasingly apparent. Thechemiosmotic theoryestablishes the mechanism of energy transformationandcoupling in oxidative phosphorylation. Theunifying concept of the protonmotive force providestheframeworkfordeveloping a consistent theoretical foundation ofmitochondrial physiology and bioenergetics.We followguidelines of the International Union of Pure and Applied Chemistry(IUPAC)onterminology inphysical chemistry, extended by considerationsofopen systems and thermodynamicsof irreversible processes.Theconcept-driven constructive terminology incorporates the meaning of each quantity and alignsconcepts and symbols withthe nomenclature of classicalbioenergetics. We endeavour to provide a balanced view ofmitochondrial respiratory control and a critical discussion on reporting data of mitochondrial respiration in terms of metabolic flows and fluxes.Uniform standards for evaluation of respiratory states and rates will ultimatelycontribute to reproducibility between laboratories and thussupport the development of databases of mitochondrial respiratory function in species, tissues, and cells.Clarity of concept and consistency of nomenclature facilitate effective transdisciplinary communication, education, and ultimately further discovery.

Peter Mitchell’s chemiosmotic theory establishes the link between vectorial and scalar energy transformation and coupling in oxidative phosphorylation. The unifying concept of the protonmotive force provides the framework for developing a consistent theory and nomenclature for mitochondrial physiology and bioenergetics. Herein, we follow IUPAC guidelines on general terms of physical chemistry, extended by considerations on open systems and irreversible thermodynamics. We align the nomenclature and symbols of classical bioenergetics with a concept-driven constructive terminology to express the meaning of each quantity clearly and consistently. In this position statement, in the frame of COST Action MitoEAGLE, we endeavour to provide a balanced view on mitochondrial respiratory control and a critical discussion on reporting data of mitochondrial respiration in terms of metabolic flows and fluxes. Uniform standards for evaluation of respiratory states and rates will ultimately support the development of databases of mitochondrial respiratory function in species, tissues, and cells., We thank M. 28. Effect of aerobic and strength training on gas exchange kinetics during moderate- and heavy-intensity exercise in elderly

Abstract

Introduction Aerobic training accelerates Phase II of V’O2 kinetics at the onset of moderate intensity exercise (M) and decreases the amplitude of the slow component (A3) during heavy-intensity exercise (H). The physiological causes of the slow component are still matter of debate. The progressive recruitment of the less economical Type II muscle fibers during H (Jones et al., 2005) may be implied in its genesis. The increase of muscle strength caused by resistance training would decrease the number of Type II muscle fibers recruited during H at the same workload (WL), wherefrom a smaller A3 may derive. Methods 12 active older men (69.4±4.3 yy; 78.5±10.5 kg; 171.2±5.3 cm; V’O2peak: 29.5±4.1mL/kg/min) were exposed to 8 weeks of: i) high intensity interval training (HIT), 7 2-minute cycling repetitions @90% of V’O2peak (V’O2p), 3 times/week and, after 4 months, ii) isoinertial resistance training (IRT), 4 × 7 maximal concentric-eccentric knee extensions, 3 times/week. Before and after trainings we measured: i) V’O2p and gas exchange threshold (GET); ii) B-by-B V’O2 at the onset of: a) moderate intensity (M; @WL equal to 80 % of GET; n = 4); and b) heavy-intensity exercise (H, @WL equal to 50% between GET and V’O2p assessed before training; n = 3); iii) isokinetic concentric torque (Tc) and isometric torque (Ti) of the limb extensors. V’O2 kinetics were modeled, after synchronization and averaging, by means of double or triple exponential functions. Results Only HIT led to a significant increase of absolute and relative V’O2p (P < 0.05). IRT brought about a significant increase of Tc and Ti (P > 0.05). The time constant of Phase II of V’O2 kinetics ( 2) during M was significantly faster only after HIT (19.5 vs. 27.3; P < 0.05); the amplitude (A2) of V’O2 response during M was significantly lower at the end of IRT (-10%, P < 0.05) than after HIT. After HIT, A3 was smaller than before training. Conversely, A3 was not affected by IRT. Discussion 8 weeks of HIT

Abstract

In the expedition Gokyo Khumbu/Ama Dablam Trek 2012, we studied the effects of two 12-day training periods performed both at sea level and at high altitude. The main results on adult women have been published in six original articles. In women, high altitude trekking induced CD69 T cell activation and promoted anti-stress effects of the immune responses and the oxidative balance (1). Low-to-moderate exercise training at s.l. improves the regenerative capacity of skeletal muscle and depicted the epigenetic signature of satellite cells. The cell differentiation was favored by increased [Ca2+]i and fusion index (2). On the contrary, the training in hypobaric-hypoxia induced oxidative stress and impaired the regenerative capacity of satellite cells (6). Although training did not significantly modify muscle phenotype , it induced beneficial adaptations of the oxygen transport-utilization systems witnessed by faster VO2 kinetics at exercise onset (3). The two training periods did not influence the postural stability (4). In young adult women, micturition physiological parameters were affected during adaptation to hypoxia; the correlation with SpO2 strongly suggests a role of hypoxia in these changes (5)

Abstract

We investigated the effects of moderate-intensity training at low and high altitude on O-2 and kinetics and on myosin heavy-chain expression (MyHC) in seven women (36.3 yy +/- A 7.1; 65.8 kg +/- A 11.7; 165 cm +/- A 8) who participated in two 12- to 14-day trekking expeditions at low (598 m) and high altitude (4132 m) separated by 4 months of recovery.Breath-by-breath O-2 and beat-by-beat at the onset of moderate-intensity cycling exercise and energy cost of walking (C (w)) were assessed before and after trekking. MyHC expression of vastus lateralis was evaluated before and after low-altitude and after high-altitude trekking; muscle fiber high-resolution respirography was performed at the beginning of the study and after high-altitude trekking.Mean response time of O-2 kinetics was faster (P = 0.002 and P = 0.001) and oxygen deficit was smaller (P = 0.001 and P = 0.0004) after low- and high-altitude trekking, whereas kinetics and C (w) did not change. Percentages of slow and fast isoforms of MyHC and mitochondrial mass were not affected by low- and high-altitude training. After training altitude, muscle fiber ADP-stimulated mitochondrial respiration was decreased as compared with the control condition (P = 0.016), whereas leak respiration was increased (P = 0.031), leading to a significant increase in the respiratory control ratio (P = 0.016).Although training did not significantly modify muscle phenotype, it induced beneficial adaptations of the oxygen transport-utilization systems witnessed by faster O-2 kinetics at exercise onset.

31. Maximal aerobic power and anaerobic capacity in cycling across the age spectrum in male master athletes

Abstract

We analyzed the best performance times of master cycling athletes in the 200-3000 m track competitions to estimate the decay of maximal aerobic power (MAP) and anaerobic capacity (AnS) with aging.In various decades of age (30-80 years), MAP and AnS were estimated using an iterative procedure as the values that minimize the difference between: (1) the metabolic power () necessary to cover a given distance (d) in the time t and; (2) the maximal metabolic power () maintained at a constant level throughout the competition.MAP started decreasing at 45 years of age. Thereafter, it showed an average percent rate of decrease of about 16 % for decade, as previously shown in other classes of master athletes. In addition, AnS seemed to decay by about 11 % every 10 years from the second part of the fifth decade.The decay of MAP occurred in spite of the active lifestyle of the subjects and it may be attributed to the progressive impairment of maximal O-2 delivery and/or of peripheral O-2 utilization. The loss of AnS might derive from the progressive loss of muscle mass occurring after the fifth decade of life, to the progressive qualitative deterioration of the anaerobic energy yielding pathways or to the lower capacity of MN recruitment during maximal efforts. The proposed approach may be applied to other types of human locomotion of whom the relationship between performance t and is known.

Abstract

In November 2011 a 75-year-old man was admitted to our emergency department with a low back pain caused by a traumatic L1 vertebral collapse with backward projection of posterior wall superior third. The indication for neurosurgical instrumentation was placed, although he refused the treatment. Hence he was treated conservatively without a significant improvement up to January 2012 when, still refusing surgery, he accepted to undergo percutaneous vertebroplasty with a novel bioactive injectable strontium-hydroxyapatite cement. Vertebroplasty was performed without complications. A CT scan, performed the day after the procedure, ruled out extravertebral cement leakage. Pain improvement was significant (preprocedure VAS 10, one-week VAS 4) with a gradual decrease up to three months when it stabilized at 2. CT examination after 1 year showed a good cement osseointegration with osteophytic spurs bridging the superior endplate of the level involved to the inferior one of the level above. The new bone ingrowing property of the strontium-hydroxyapatite containing cement permits to extend the treatment indication also to unstable collapses in which the risk of pseudoarthrosis is very high. In this reported case we evaluated the potential role of percutaneous vertebroplasty in traumatic burst fracture with spinal channel involvement.

39. Effects of High-Intensity-Interval-Training (HIT) on cardiovascular fitness and cardiometabolic risk factors in older adults

Abstract

HIT has been shown improve cardiovascular fitness and seems to induce beneficial modifications of some cardiometabolic risk factors in healthy subjects or patients. Lees is known about the efficacy of HIT applied to healthy older adults PURPOSE. The aim of the study consists in testing if 8 weeks of HIT can induce significant improvements of cardiovascular fitness, exercise capacity and of selected cardiometabolic risk factors. METHODS. In 12 healthy male volunteers (68.7 ± 3.9 yy; 79.0 ± 10.8 kg; 171.4 ± 5.4 cm)) participated in the study we measured V’O2max, gas exchange threshold (GET ) and respiratory compensation point (RCP), mean, systolic and diastolic blood pressure (MBP, SDP, DBP) at rest, fasting blood glucose concentration (GLU), total cholesterol/HDL ratio (TC/HDL), % Body fat (BF) and waist circumference (WC) before and after before (PRE) and after (POST) 8 weeks of HIT performed 3 times a week cycling 7 times for 2 minutes, interspersed with 2 minutes of recovery, at about 85-90 % of V’O2max. RESULTS. Absolute and relative V’O2max significantly increased by 5.4 % (ES = 0.7) and 11.7 % (ES = 0.8), respectively. V’O2 at GET and RCP increased by 7.2% (ES = 0.7) and 15.4 % ( ES = 1.3), respectively. MBP, SDP and DBP decreased by 7 (ES = 0.8), 9 (ES = 0.9) and 4 % (ns), respectively. GLU was diminished by 7 % (ES = 0.5) and TC/CHO decreased by 5 % (ES = 0.2). FM and BMI decreased by 4 % (ES = 0.2) and x % (), respectively. CONCLUSION. 8 weeks of HIT seem to be sufficient to promote significant improvements of maximal aerobic power, and index highly predictive of cardiovascular mortality, exercise capacity and of selected cardiometablic risk factors in healthy, moderately active, male older.

Abstract

Since the large multicenter DIG trial has shown no effects of digitalis on the all-cause mortality of patients with chronic heart failure (HF), the broad prescription of this drug in patients with HF appears to be at the very least, questionable. The aims of this study were: to analyze prescription patterns of digitalis, from 1995 to 2000, in a large group of outpatients with HF; to analyze the independent predictors of digitalis prescription and to evaluate the impact of the results of the DIG trial on the prescription rate of this drug.From 1995 to 2000, 11 070 HF outpatients (mean age 64 +/- 12 years, ejection fraction 35 +/- 12%) were enrolled in a large Italian database.Out of 11 070 patients, 7198 (65%) were treated with digitalis. At multivariate analysis, the following variables were independently associated with digitalis prescription; atrial fibrillation (odds ratio [OR] 3.3, 95% confidence interval [CI] 2.9-3.8), ejection fraction30% (OR 1.7, 95% CI 1.5-1.9), NYHA class III-IV vs II-III (OR 1.3, 95% CI 1.2-1.5), admission for HF during the previous year (OR 1.4, 95% CI 1.2-1.5). After the publication of the DIG trial, there was a significant reduction in the rate of digitalis prescription: the percentage of patients taking digitalis fell from 68% in 1996-1997 to 61% in 1998-1999 (p0.001).Over 60% of Italian outpatients with HF were treated with digitalis; as expected, patients with a low ejection fraction, atrial fibrillation and in a more advanced stage of HF are more likely to receive this drug. Finally, after the publication of the DIG trial, the rate of digitalis prescription significantly decreased.

Abstract

Background: Nurse-led central venous catheter placement is an emerging clinical role internationally. Procedural characteristics and clinical outcomes is an important consideration in appraisal of such advanced nursing roles. Objectives: To review characteristics and outcomes of three nurse-led central venous catheter insertion services based in intensive care units in New South Wales, Australia. Design: Using data from the Central Line Associated Bacteraemia project in New South Wales intensive care units. Descriptive statistical techniques were used to ascertain comparison rates and proportions. Participants: De-identified outcome data of patients who had a central venous catheter inserted as part of their therapy by one of the four advanced practice nurses working in three separate hospitals in New South Wales. Results: Between March 2007 and June 2009, 760 vascular access devices were placed by the three nurse-led central venous catheter placement services. Hospital A inserted 520 catheters; Hospital C with 164; and Hospital B with 76. Over the study period, insertion outcomes were favourable with only 1 pneumothorax (1%), 1 arterial puncture (1%) and 1 CLAB (1%) being recorded across the three groups. The CLAB rate was lower in comparison to the aggregated CLAB data set [1.3 per 1000 catheters (95% CI. = 0.03-7.3) vs. 7.2 per 1000 catheters (95% CI = 5.9-8.7)]. Conclusion: This study has demonstrated safe patient outcomes with nurse led CVC insertion as compared with published data. Nurses who are formally trained and credentialed to insert CVCs can improve organisational efficiencies. This study adds to emerging data that developing clinical roles that focus on skills, procedural volume and competency can be a viable option in health care facilities. © 2011.
