Introduzione: L’iperparatiroidismo è un’alterazione fisiopatologica della secrezione di paratormone dovuta o da un rilascio autonomo e anomalo (iperparatiroidismo primario o terziario) da parte delle paratiroidi o da un’alterazione dell’omeostasi del calcio che stimola l’eccessiva produzione di paratormone (iperparatiroidismo secondario). Obiettivi: Il trattamento clinico dell’iperparatiroidismo secondario o terziario non è univoco. Sorge pertanto la necessità di una revisione delle attuali metodiche diagnostico-terapeutiche. Discussione: Nell’iperparatiroidismo secondario (2HPT) si ha una progressiva iperplasia delle paratiroidi e un incremento della produzione di paratormone. Tra le cause sono annoverate l’insufficienza renale cronica, la carenza di vitamina D, la sindrome da malassorbimento. L’iperparatiroidismo terziario (3HPT) è attualmente considerato uno stato di eccessiva secrezione autonoma di PTH a causa di un’2HPT di lunga data ed è di solito il risultato di una mancata soppressione della produzione di PTH. Le implicazioni fisiopatologiche sono sia di tipo scheletrico che extrascheletrico a carico dell’apparato cardiovascolare, del sistema nervoso, immunitario, emopoietico ed endocrino. Negli ultimi anni grazie all’introduzione di nuovi farmaci che consentono di ottenere una efficace inibizione della sintesi del paratormone si è avuto un miglioramento della sopravvivenza di questi pazienti. Nei casi di 2HPT e 3HPT che non rispondono a terapia medica i pazienti devono essere avviati all’intervento chirurgico. Conclusioni. Non ci sono ampi studi prospettici che confrontino il trattamento medico e chirurgico, la scelta quindi non risulta univoca e bisogna quindi tener conto del singolo caso e delle condizioni cliniche del paziente. INTRODUCTION: Hyperparathyroidism is an alteration of the pathophysiological parathyroid hormone (PTH) secretion due or an independent and abnormal release (primary or tertiary hyperparathyroidism) by the parathyroid or an alteration of calcium homeostasis that stimulates the excessive production of parathyroid hormone (secondary hyperparathyroidism). AIMS: There is not a standard, clinical or surgical, treatment for hyperparathyroidism. We review current diagnostic and therapeutic methods. DISCUSSION: In secondary hyperparathyroidism (2HPT) there is a progressive hyperplasia of the parathyroid glands and an increased production of parathyroid hormone. Several causes are proposed: chronic renal insufficiency, vitamin D deficiency, malabsorption syndrome. The tertiary hyperparathyroidism (3HPT) is considered a state of excessive autonomous secretion of PTH due to long-standing 2HPT and it's usually the result of a lack of suppression in the production of PTH. The pathophysiological implications are both skeletal and extraskeletal: it damages the cardiovascular system, nervous system, immune, hematopoietic and endocrine system. The introduction of new drugs has improved the survival of these patients, allowing the inhibition of the synthesis of PTH. Indication for surgical treatment is unresponsive medical therapy. CONCLUSIONS: There are no large prospective studies that comparing the medical and surgical treatment. The choice is not unique and we have to consider the singolar case and the clinical condition of the patient.

Purpose: In many places, especially in emergency department, central venous catheter is still inserted using anatomical landmark guidance with a success rate up to 97.6% and complications up to 15%. This study was aimed to determine by the support of ultrasono-graphy (US) the anatomical variations of the internal jugular vein (IJV) in relation with other structures of the neck, such as the common carotid artery (CCA). Material and Methods: 830 patients requiring central vein catheterization (CVC) were in-cluded in the analysis. The position of the IJV in relation to the other structures of the neck was demonstrated by portable ultrasonography. Results: The mean diameter of IJV was 10.3 mm in right and 10.5 mm in left side of neck, in male (p > 0.05) and 9.1 mm in right and 10.5 mm in left side of neck, in female (p > 0.05). The mean distance between IJV and CCA was 1.9 mm in right and 1.7 mm in left side of neck in male, and 2.0 mm in right and 2.2 mm in left side of neck in female. The mean distance of IJV from the skin surface was 9.8 mm in right and 10.0 mm in left side of neck in male, and 12.1 mm in right and 12.5 mm in left side of neck in female. On 25.54% we observed variations of internal jugular vein site. On 3.97% we observed a small caliber of internal jugular vein that could complicate the catheterization of the vein. On 1.8% was diagnosed a thrombus of internal jugular vein, that is considered as an ab-solute contraindication for a CVC. Conclusion: Different patients had anatomical variations that are important and should be knowed, in order to reduce the possibility of severe complications.

Introduction. Acrylic cement pulmonary embolism is a potentially serious complication following vertebroplasty. Case Report. A 70-year-old male patient was treated with percutaneous vertebroplasty for osteoporotic nontraumatic vertebral collapse of L5-S1. Asymptomatic pulmonary cement embolism was detected on routine postoperative chest radiogram and the patient was treated with enoxaparin, amoxicillin, and dexamethasone. At the followup CT scan no further migration of any cement material was reported; and the course was uneventful. Discussion. The frequency of local leakage of bone cement is relatively high (about 80–90%), moreover, the rate of cement leakage into the perivertebral veins (seen in up to 24% of vertebral bodies treated) with consequent pulmonary cement embolism varies from 4.6 to 6.8% (up to 26% in radiologic studies); the risk of embolism is increased with the liquid consistency of the cement and with the treatment of some malignant lesions. Patients may remain asymptomatic and develop no known long-term sequelae. Conclusions. Our ancedotal case illustrates the need for close monitoring of patients undergoing percutaneous vertebroplasty and emphasizes the importance of prompt and correct diagnosis and treatment, even if actually there is no agreement regarding the therapeutic strategy.

Per definizione, il drenaggio è un presidio atto ad eliminare liquidi generalmente patologici da cavità naturali o neoformate. Il drenaggio di raccolte addominali non presenta particolari difficoltà poiché è sufficiente che il tubo di drenaggio sia ben posizionato, pervio, declive e di calibro proporzionato alla quantità e alla qualità del materiale da drenare. La sua assenza, anche se potrà ritardare o complicare notevolmente il processo di guarigione di una malattia, difficilmente comprometterà la vita del soggetto in maniera acuta e drammatica. Assai diverso è il drenaggio di raccolte intratoraciche, non soltanto per la assoluta necessità del suo impiego, ma anche per le modalità tecniche e i principi fisici di funzionamento, per molti versi unici e strettamente legati alla fisiologia del sistema toraco-polmonare. Per tale motivo, abbiamo deciso di riassumere quanto negli anni con il mio Maestro, Prof. Giuseppe Modica, abbiamo visto, studiato ed affrontato, mettendolo a disposizione di quanti lo vorranno. L’obiettivo di questo volume è quindi quello di fornire informazioni riguardo ai principi di funzionamento del drenaggio toracico, descrivere la tecnica di posizionamento, proporre delle linee guida di gestione del drenaggio stesso, nonché indicare gli errori più comuni connessi al suo impiego, basandoci più sull’esperienza di scuola che sul mero rigore scientifico.

(OBJECTIVE) Jacobeus and Bethune performed thoracoscopy under local anaesthesia, already on early 1900. A single access associated with local anaesthesia aims to improve procedure tolerance, shorten recovery and reduce costs. (METHODS) Premedication consists of atropine 0.01 mg/kg and midazolam (0,02-0.04 mg/kg). Pain control and sedation was obtained by remifentanile (0.05-0.1 mcg/kg/min). Local anaesthesia was made with lidocaine and ropivacaine. An incision of 20 mm is realised at 6° intercostal space for the “single-access-technique”, and two incisions for the “2-trocars-technique” (15 mm at 5°space - 10 mm at 6°/7°space). The 5 mm 0° camera is preferred for single-access technique. The effusion is not totally drained in order to maintain the lung collapsed, getting a better view of pleural cavity. Otherwise, it could be useful insufflating CO2. Liquid samples, multiple biopsies and eventual pleurodesis are performed. Thymidylate synthase (TS) catalyzes methylation of dUMP to dTMP and it is the target for the 5-Fluorouracil (5-FU) activity*. Barbour et al. showed that variant structural forms of TS in tumour cell lines confer resistance to fluoropyrimidines. We planned to perform the whole TS gene structure by means of sequencing techniques in human colorectal cancer (CRC) san-rples to try to identify the presence of any possible TS variant form that could be responsible of fluoropyrimidines drug resistance and of the worse prognosis. We performed the TS-DNA gene sequence in 68 CRC from patients of A, B, and C Dukes' stages and different histological grade, but we did not find any mutation in the TS-DNA structure. In the future we intend to widen the TS structure analysis to the metastatic CRCs, because due to their higher genomic instability, they could present a TS variant form responsilile of the fluoropyrimidines drug resistance and the worse prognosis.

Numerosi fattori sono stati coinvolti nell’insorgenza del carcinoma mammario e dell’ovaio tra cui l’età, fattori ormonali e riproduttivi come l’età e il numero di gravidanze e l’utilizzo di contraccettivi orali e, in minor misura, fattori ambientali e stili di vita. Le possibilità per la gestione clinica delle pazienti considerate ad alto rischio di sviluppare carcinoma della mammella e/o dell’ovaio includono chirurgia profilattica (es. mastectomia bilaterale profilattica o ovarosalpingectomia bilaterale), chemioprevenzione (principalmente tamoxifene e raloxifene ma anche, più recentemente, inibitori dell’aromatasi nelle donne ad alto rischio in postmenopausa), cambiamenti nelle abitudini di vita e sorveglianza intensiva attraverso l’esame clinico periodico, la Risonanza Magnetica Mammaria, la Mammografia talvolta in associazione all’Ecografia e, per la prevenzione del carcinoma ovarico, lo screening combinato utilizzando il dosaggio del CA125 e l’ecografia transvaginale.
