4. Multidisciplinary Management and Autologous Skin Grafting in a Patient with Severe Burns: A Case Study

Abstract

Background: Heat burns are a prevalent type of trauma. Rapid and effective treatment is crucial for deep burns to minimize complications. Autologous skin grafting is a highly effective treatment for full-thickness burns. A multidisciplinary team plays a vital role in managing burn patients undergoing skin grafting, from initial contact to outpatient follow-up. Case Summary: This case study involves a 56-year-old patient who suffered burns on 60% of his body following an alcohol explosion on an open flame. The patient underwent autologous skin grafting at a Major Burn Center. Initial symptoms included severe pain and immobility, but the patient remained alert and breathed spontaneously. The diagnosis was a loss of epidermis and dermis with burns covering 60–69% of the total body surface area (TBSA) and third-degree burns covering 10% TBSA. Post-discharge, the patient showed significant improvement, with complete healing of the grafts and partial resolution of other lesions. Six months after the intervention, the patient significantly improved his autonomy and mobility. Conclusions: This case highlights the importance of burn prevention and the critical role of multidisciplinary teams in the entire care pathway of burn patients. Appropriate diagnosis, complete treatment, and continuous multidisciplinary support are essential to prevent complications and ensure recovery.

Abstract

Recent advances in the use of portable/handheld laser-induced breakdown spectroscopy (LIBS) instrumentation have allowed its use directly in the field. In this study, a portable/handheld LIBS demo kit was tested to detect the titanium (Ti) content in some quartz veinlets hosted by hornfels collected from the contact aureole of a Pliocene granite from Italy. Results of the present study demonstrate the promising potential of LIBS in the rapid detection of low and very variable Ti contents in quartz, which can be used as a preliminary test for the estimation of the temperature of quartz crystallization in the laboratory. However, to date, the limited availability of matrix-matched calibration samples, the refinement of sampling protocols, as well as the development of suitable algorithms for data processing and spectral analysis still require further investigation. Abstract

Aims Injectable medicines such as PCSK9 inhibitors are increasingly used to manage risk factors for cardiovascular events with little information around the perceptions of healthcare professionals (HCPs) on the administrative and clinical practicalities. The aim was to identify the facilitators and barriers on the use of injectable therapies with cardiovascular benefits through interviews with HCPs. Methods and results Qualitative interviews were conducted in the UK (London and Leeds) and Italy (Rome and Milan) in 2021. Coding was undertaken using NVivo and thematic analysis performed. A total of 38 HCPs were interviewed, 19 in each country composing of physicians (n = 18), pharmacists (n = 10), nurses (n = 9), and pharmacy technician (n = 1). Four themes emerged: (i) clinicians' previous experiences with injectable therapies, (ii) challenges with patients' behaviours and beliefs, (iii) clinicians' knowledge of injectable therapies and therapeutic inertia, and (iv) organizational and governance issues. The behaviour and beliefs from HCPs focused on facilitating behaviour change as well as the poor interdisciplinary working and collaboration. Therapeutic inertia was raised where physicians either lacked awareness of injectable therapies or were unwilling to prescribe them. The importance of facilitating patient education on injection techniques was highlighted, while organizational and governance issues identified the lack of guidance to inform practice. Clear pathways are required to identify those who were eligible for injectable therapies as well as on how injectables should be prescribed. Conclusion If medicine optimization is to be achieved, there need to be structured processes in place to identify eligible patients and the development of educational material. Abstract Aims Intense health‐care service use and high mortality are common in heart failure (HF) patients. This secondary analysis of the MOTIVATE‐HF trial investigates the effectiveness of motivational interviewing (MI) in reducing health‐care service use (e.g. emergency service use and hospitalizations) and all‐cause mortality. Methods and results This study used a randomized controlled trial. Patients and caregivers were randomized to Arm 1 (MI for patients), Arm 2 (MI for patients and caregivers), or Arm 3 (control group). Data were collected at baseline and at 3, 6, 9, and 12 months. Face‐to‐face MI plus three telephone calls were performed in Arms 1 and 2. The sample consisted of 510 patient (median age 74 years, 58% male patients) and caregiver dyads (median age 55 years, 75% female patients). At 12 months, 16.1%, 17%, and 11.2% of patients used health‐care services at least once in Arms 1, 2, and 3, respectively, without significant difference. At 3 months, 1.9%, 0.6%, and 5.1% of patients died in Arms 1, 2, and 3, respectively. Mortality was lower in Arm 2 vs. Arm 3 at 3 months [hazard ratio (HR) = 0.112, 95% CI: 0.014–0.882, P = 0.04]; no difference was found at subsequent follow‐ups. Mortality was lower in Arm 1 vs. Arm 3 at 3 months but did not reach statistical significance (HR = 0.38, 95% CI: 0.104–1.414, P = 0.15). Conclusion This study suggests that MI reduces mortality in patients with HF if caregivers are included in the intervention. Further studies with a stronger intervention and longer follow‐up are needed to clarify the benefits of MI on health‐care service use and mortality.

Abstract

Background: Heat burns are a prevalent type of trauma. Rapid and effective treatment is crucial for deep burns to minimize complications. Autologous skin grafting is a highly effective treatment for full-thickness burns. A multidisciplinary team plays a vital role in managing burn patients undergoing skin grafting, from initial contact to outpatient follow-up. Case Summary: This case study involves a 56-year-old patient who suffered burns on 60% of his body following an alcohol explosion on an open flame. The patient underwent autologous skin grafting at a Major Burn Center. Initial symptoms included severe pain and immobility, but the patient remained alert and breathed spontaneously. The diagnosis was a loss of epidermis and dermis with burns covering 60–69% of the total body surface area (TBSA) and third-degree burns covering 10% TBSA. Post-discharge, the patient showed significant improvement, with complete healing of the grafts and partial resolution of other lesions. Six months after the intervention, the patient significantly improved his autonomy and mobility. Conclusions: This case highlights the importance of burn prevention and the critical role of multidisciplinary teams in the entire care pathway of burn patients. Appropriate diagnosis, complete treatment, and continuous multidisciplinary support are essential to prevent complications and ensure recovery. [ABSTRACT FROM AUTHOR]

Abstract

Abstract A revision of late Palaeozoic tectonics recorded in Tuscany, Calabria and Corsica is here presented. We propose that, in Tuscany, upper Carboniferous-Permian shallow-marine to continental sedimentary basins, characterized by unconformities and abrupt changes in sedimentary facies, coal-measures, red fanglomerate deposits and felsic magmatism, may be related with a transtensional setting where upper-crustal splay faults are linked with a mid-crustal shear zone. The remnants of the latter can be found in the deep-well logs of Pontremoli and Larderello-Travale in northern and southern Tuscany respectively. In Calabria (Sila, Serre and Aspromonte), a continuous pre-Mesozoic crustal section is exposed, where the lower-crustal portion mainly includes granulites and migmatitic paragneisses, together with subordinate marbles and metabasites. The mid-crustal section, up to 13 km-thick, includes granitoids, tonalitic to granitic in composition, emplaced between 306 and 295 Ma. They were progressively deformed during retrograde extensional shearing, with a final magmatic activity, between 295 ± 1 and 277 ± 1 Ma, when shallower dykes were emplaced in a transtensional regime. The section is completed by an upper crustal portion, mainly formed by a Palaeozoic sedimentary succession deformed as a low-grade fold and thrust belt, and locally overlaying medium-grade paragneiss units. As a whole, these features are reminiscent of the nappe zone domains of the Sardinia Variscan Orogen. In Corsica, besides the well-known effusive and intrusive Permian magmatism of the “Autochthonous” domain, the Alpine Santa Lucia Nappe exposes a kilometer-scale portion of the Permian lower to mid-crust, exhibiting many similarities to the Ivrea Zone. The distinct Mafic and Granitic complexes characterizing this crustal domain are juxtaposed through an oblique-slip shear zone named Santa Lucia Shear Zone. Structural and petrological data witness the interaction between magmatism, metamorphism and retrograde shearing during Permian, in a temperature range of c. 800–400 °C. We frame the outlined paleotectonic domains within a regional-scale, strain–partitioned, tectonic setting controlled by a first-order transcurrent/transtensional fault network that includes a westernmost fault (Santa Lucia Fault) and an easternmost one (East Tuscan Fault), with intervening crustal domains affected by extensional to transtensional deformation. As a whole, our revision allows new suggestions for a better understanding of the tectonic framework and evolution of the Central Mediterranean during the late Palaeozoic.

Abstract

The Las Minas area corresponds to an exhumed geothermal system considered a proxy for the deep part of the nearby Los Humeros active geothermal system. The stratigraphic succession is made up of: Palaeozoic-Miocene granitoids, a thick Jurassic- Cretaceous carbonate succession, Neogene lava flows and volcano-sedimentary deposits. Linked to a Miocene magma intrusion, marble and skarn rock-volumes developed by contact metamorphism and geothermal fluid flow. Faults are arranged in SW- and NNW-striking systems. These controlled the morphological evolution and favored Neogene-Quaternary dyke emplacement. Faulting gave rise to a tectonic depression where lacustrine sediments and pyroclastics deposited. Skarn rocks are mainly located at fault intersections and along pre-existing discontinuities, suggesting the role of bedding and/or fractures in channeling deep fluids. Results give inputs for exploration at depth of Los Humeros geothermal system.

19. Motivational interviewing to improve self‐care in heart failure patients (MOTIVATE‐HF): a randomized controlled trial

Abstract Aims Self‐care, an essential component of heart failure (HF) treatment, is inadequate in most patients. We evaluated if motivational interviewing (MI) (i) improves patient self‐care maintenance (primary endpoint; e.g. taking medications), self‐care management (e.g. responding to symptoms) and self‐care confidence (or self‐efficacy) 3 months after enrolment; (ii) changes self‐care over 1 year, and (iii) augments patient self‐care if informal caregivers are involved. Methods and results Parallel randomized controlled trial (1:1:1). A sample of 510 patients (median 74 years, 58% male) and caregivers (median 55 years, 75% female) was randomized to Arm 1 (MI only for patients), Arm 2 (MI for patients and caregivers), or Arm 3 (usual care). The intervention in Arms 1 and 2 consisted of one face‐to‐face MI session with three telephone contacts. Self‐care was evaluated with the Self‐Care of HF Index measuring self‐care maintenance, management, and confidence. Scores on each scale range from 0 to 100 with higher scores indicating better self‐care; ≥70 is considered adequate. At 3 months, self‐care maintenance improved 6.99, 7.42 and 2.58 points in Arms 1, 2, and 3, respectively (P = 0.028). Self‐care maintenance was adequate in 18.4%, 19.4%, and 9.2% of patients in Arms 1, 2 and 3, respectively (P = 0.016). Over 1 year, self‐care maintenance, management, and confidence scores in Arms 1 and 2 were significantly higher than in Arm 3 in several follow‐ups. Over 1 year, Arm 2 had the best scores in self‐care management. Conclusions MI significantly improved self‐care in HF patients. The objective of this review is to evaluate the effectiveness of transitional care interventions for seriously ill patients and their caregivers.Seriously ill patients and their caregivers may have complex health and social care needs that require services from numerous providers across multiple sectors. Transitional care interventions have been designed to enhance a collaborative approach among providers to facilitate the care transition process. However, the effectiveness of transitional care interventions for seriously ill patients, their caregivers, and the effects of such interventions on their outcomes remain unclear.Randomized controlled trials with adult patients (≥18 y old) with serious illness and their caregivers involved in transitional care programs will be considered for inclusion. The patients' outcomes will include mortality and/or survival, symptoms (eg, pain, nausea), and health-related quality of life. The caregivers' outcomes will include caregiver burden, preparedness, and well-being.The JBI methodology for systematic reviews of effectiveness evidence will be followed. The search strategy aims to locate published and unpublished studies. Electronic databases, including PubMed, Embase, CINAHL, and the Cochrane Central Register of Controlled Trials, will be systematically searched from 2003 to the present. Studies in English, Italian, Spanish, French, and German will be included. Critical appraisal and data extraction will be conducted using standardized tools. Quantitative data will be pooled in statistical meta-analysis or, if statistical pooling is not possible, the findings will be reported narratively. Abstract

During the Permian-Triassic transition, dramatic environmental changes occurred, including the segmentation of Pangaea and the beginning of the Alpine orogenic cycle with widespread magmatism and tectonic activity. The Permian-Triassic succession deposited in the northern (palaeo-) Gondwana is now exposed in the inner Northern Apennines where it crops out discontinuously in the Middle Tuscan Ridge, a NS-oriented morphotectonic feature mainly formed by blue-schist and green-schist facies metamorphic rocks. In this paper, we describe a previously unreported late Palaeozoic succession exposed in the central part of the Middle Tuscan Ridge: the Montagnola Senese Ridge. This succession provides information concerning the enigmatic late Palaeozoic units exposed in southern Tuscany, and permits the age constraint of continentalization at the Permian-Triassic boundary with associated magmatism. Specifically, by integrating stratigraphic, structural, and petrographic data, we describe a siliciclastic, volcanoclastic-to-carbonate succession which recorded the transition from marine sedimentation to the typical continental, red-beds of the Verrucano tectofacies. Additionally, deformational features, metamorphism and age of the succession are discussed in terms of its tectono-sedimentary evolution and implications for the Palaeozoic-Triassic setting of the central Pangaea sector currently exposed in southern Tuscany. The results allow us to recognize a middle-late Permian pre-Verrucano succession, and to date the transition to continental facies as latest Permian. The microflora permits correlation of the pre-Verrucano succession to the late Palaeozoic units of the inner Northern Apennines, and establishes the diachronous nature of the Verrucano tectofacies exposed in the inner Northern Apennines. Abstract

Aims Caregiver characteristics and behaviours are associated with patients’ and caregivers’ outcomes. However, there is scarce evidence on interventions aimed at improving caregiver contribution (CC) to patient self-care, caregiver self-efficacy, and caregiver preparedness in heart failure (HF). This study aims to evaluate the effect of Motivational Interviewing (MI) on CC to patient self-care, caregiver self-efficacy, and caregiver preparedness in HF. Methods and results This is a secondary outcome analysis of MOTIVATE-HF randomized controlled trial. Caregivers of patients with HF were randomized into three arms: Arm 1 (MI for patients), Arm 2 (MI for patients and caregivers), and Arm 3 (standard care). The intervention consisted of one MI session plus three telephone contacts. Data were collected at baseline and after 3, 6, 9, and 12 months. A total of 510 patient and caregiver (median age 54 years, 76% females) dyads were enrolled, 235 caregivers and 238 patients completed the study until 12 months. Caregiver contribution to self-care maintenance and management and caregiver preparedness significantly improved over time, but without significant differences among the three arms. At 9 months, caregiver self-efficacy improved more in Arm 2 than Arm 3 [difference: 8.36, 95% confidence interval (CI) (3.13; 13.59), P = 0.001]. This improvement remained significant also at linear mixed model [β^ = 1.39, 95% CI (0.02; 2.75), P = 0.046]. Conclusion Motivational Interviewing did not improve CC to patient self-care and caregiver preparedness. However, it increased caregiver self-efficacy, which is known to be a powerful mediator of CC to self-care. Registration Clinicaltrials.gov: NCT02894502.

Abstract

We modelled thermo-rheological perturbations, related to the emplacement of a magmatic body in the upper crust. This approach was considered relevant for the areas characterized by elevated surface heat flow and chiefly for the geothermal fields. The numerical conductive thermal model applied to the Larderello geothermal area in Tuscany, allowed to constrain size, depth and timing of emplacement of the pluton. We inferred that the emplacement of a magmatic body, at a minimum depth of 3 km, having a horizontal extension of 14 km and a maximum thickness of 8 km, can reasonably reproduce the observed regional surface heat flow anomaly of the Larderello area, when 300 (± 100) kyr are elapsed from the magma emplacement. Even assuming an incremental growth, the first magma injection should not be older than 1 ± 0.3 Ma. Results of the thermal model were used to set up a rheological model and to simulate the drifting of the brittle-ductile transition during the cooling of the pluton. A comparison with the K-horizon profile, a prominent seismic reflector in the Larderello area, was then performed. It was found that the K-horizon approximately corresponds with the pluton roof and with the current location of the brittle-ductile transition.

Abstract

This article reports the analysis of six urban contexts in which a practical tool measuring individual skills concerning household waste recycling was tested. The tool is a structured questionnaire including a simulation task that assesses respondents' abilities to sort household waste adequately in a given context/municipality. Results indicate that the questionnaire can reach high levels of internal coherence and discrimination capacity and it can be administered together with a set of open-ended and closed questions to assess relationships with other relevant factors. Both the theoretical and applicative potentials of the instrument for educational and communicational domains are discussed.

Abstract

Geothermal systems in terrains affected by polyphase deformation have reservoirs with a 3D geometry that is always difficult to predict. In this paper we describe a fossil exhumed geothermal system exposed in eastern Elba Island that developed in a polyphase folded and faulted setting, which can help us to understand how geothermal fluids circulate in geological bodies with inherited structures. Geothermal circulation at Elba allowed the deposition of Fe-ore deposits (haematite/magnetite and pyrite) and altered rock volumes, which represent tracers of the palaeo-fluid flow. Normal and oblique-slip faults dissected a polyphase folded metasiliciclastic succession and produced a secondary permeability in the range of 5 × 10−13 to 5 × 10−16 m2. From the permeable fault zones acting as feeder conduits, geothermal fluids permeated the hydraulically connected metasiliciclastic rock bodies previously deformed by two generations of folds. Geothermal fluids followed the already defined geometry, thus giving rise to apparent folded mineralized levels. Fluid migration into the metasiliciclastic rocks was possible due to their chemical aggression, which favoured the dissolution and reprecipitation of quartz, and Fe-oxide and sulphide deposition. Renewed fluids maintained their chemical properties (pH value and temperature, mostly). This conclusion provides inputs for reconstructing geothermal conceptual models and evaluating the geothermal potentiality of exploitable areas developing in similar geological settings.

Mortality was lower in Arm 2 vs. Arm 3 at 3 months [hazard ratio (HR) = 0.112, 95% CI: 0.014–0.882, P = 0.04]; no difference was found at subsequent follow‐ups. Mortality was lower in Arm 1 vs. Arm 3 at 3 months but did not reach statistical significance (HR = 0.38, 95% CI: 0.104–1.414, P = 0.15). Conclusion This study suggests that MI reduces mortality in patients with HF if caregivers are included in the intervention. Abstract

We review and refine the geological setting of an area located nearby the Tyrrhenian seacoast, in the inner zone of the Northern Apennines (southern Tuscany), where a Neogene monzogranite body (estimated in about 3 km long, 1.5 km wide, and 0.7 km thick) emplaced during early Pliocene. This magmatic intrusion, known as the Gavorrano pluton, is partially exposed in a ridge bounded by regional faults delimiting broad structural depressions. A widespread circulation of geothermal fluids accompanied the cooling of the magmatic body and gave rise to an extensive Fe-ore deposit (mainly pyrite) exploited during the past century. The tectonic setting which favoured the emplacement and exhumation of the Gavorrano pluton is strongly debated with fallouts on the comprehension of the Neogene evolution of this sector of the inner Northern Apennines. Data from a new fieldwork dataset, integrated with information from the mining activity, have been integrated to refine the geological setting of the whole crustal sector where the Gavorrano monzogranite was emplaced and exhumed. Our review, implemented by new palynological, petrological and structural data pointed out that: (i) the age of the Palaeozoic phyllite (hosting rocks) is middle-late Permian, thus resulting younger than previously described (i.e., pre-Carboniferous); (ii) the conditions at which the metamorphic aureole developed are estimated at a temperature of c. 660 °C and at a depth lower than c. 6 km; (iii) the tectonic evolution which determined the emplacement and exhumation of the monzogranite is constrained in a transfer zone, in the frame of the extensional tectonics affecting the area continuously since Miocene.

Abstract

The aim of this presentation is to present the results of three qualitative research on the exercise of rights and duties on Parenting Practices (PP), held in Buenos Aires, Argentina. They included interviews with mothers of children with Unsatisfied Basic Needs concretized between 2009 and 2013. Their analysis revealed that in this set of households were carried out three types of PP: imposition, guide and free will, the latter was the most used. As part of this practice, children managed their hours of sleep, wakefulness and leisure, without the intervention of their parents. It was noted, for example, that children over 10 years decided on matters concerning their schooling, absenting progressively to school, to abandonment. These practices were respected by their parents. By default, the postponement of pleasure (tolerance to frustration) will not be exercised: they let children do at will. A trend of teenage pregnancy and the formation of pairs of children between 14-16 years with parental consent was also noted. In this sense, even when children lived in a house in contact with their parents, with a supply of food and available school, the indiscriminate exercise of free will put children's health at risk and full development, curtailing their rights. We recommend further studies such timely interventions to promote programs and projects designed to guide parents on issues related to the development of children as subjects of Rights and Duties.

Background Caregiver self-efficacy, which is caregiver confidence in her own abilities to help the patient in performing self-care, directly influences caregiver contribution (CC) to self-care, while acting as a mediator between predictors of CC to self-care and CC to self-care itself. Although caregiver self-efficacy is associated with patient and caregiver outcomes, evidence on interventions aiming to improve this variable, as well as CC to self-care, is scarce. Purpose To evaluate the effect of Motivational Interviewing (MI) on caregiver self-efficacy and CC to self-care in heart failure. Methods Secondary outcome analysis of the MOTIVATE-HF RCT. A total of 235 caregivers and 238 patients completed the study. Participants were recruited from three centres in Italy and randomized into Arm 1 (MI for patients), Arm 2 (MI for patients and caregivers), Arm 3 (standard care). The intervention consisted in one face-to-face MI session followed by three telephone contacts delivered to patients (Arm 1) or to patients and caregivers (Arm 2). Caregiver self-efficacy and CC to self-care were assessed with the Caregiver Contribution to Self-Care of HF Index. Data were collected at baseline, before the intervention, and after 3, 6, 9, 12 months from enrolment. Results Among the three arms, caregivers median age was 55 years, they were mainly female (76%), married (72%), spouses (37%), employed (73.5%), with a high school level or higher education (55%), and living with the patients (60%). Patients median age was 74 years, they were mainly male (58%), retired (76%), in NYHA Class II (61.9%). Over the year of the study, caregiver self-efficacy significantly improved in all arms, and was significantly higher in Arm 2 compared to Arm 3 at 9-month follow-up (difference: 8.36, 95% CI (3.13; 13.59), p = 0.002). At 12-month follow-up, caregiver self-efficacy was still higher in Arm 2 compared to Arm 3, but this difference only approached statistical significance (difference: 6.59, 95% CI (-0.41; 13.60), p = 0.064). The longitudinal mixed linear model on self-efficacy, accounting for time, living with the patient, randomization arm and their interaction, confirmed a significantly higher improvement of caregiver self-efficacy in Arm 2 compared to Arm 3 (β ̂ = 1.39, 95% CI (0.02; 2.75), p = 0.046). CC to self-care significantly improved over time in all Arms, but without significant differences among the three Arms. Conclusion Our results show that MI was effective in improving caregiver self-efficacy, but not CC to self-care. This may indicate that MI was able to improve how well caregivers were supporting patients, as showed by improvements in self-efficacy, but not how much they were doing it, as showed by the lack of improvement in CC to self-care. Further studies need to better understand how caregiver self-efficacy affects CC to self-care, how caregiver self-efficacy can be further improved, and the necessary intensity of MI to improve CC to self-care.

Abstract

BACKGROUND: High intraoperative PEEP with recruitment manoeuvres may improve perioperative outcomes. We re-examined this question by conducting a patient-level meta-analysis of three clinical trials in adult patients at increased risk for postoperative pulmonary complications who underwent non-cardiothoracic and non-neurological surgery. METHODS: The three trials enrolled patients at 128 hospitals in 24 countries from February 2011 to February 2018. All patients received volume-controlled ventilation with low tidal volume. Analyses were performed using one-stage, two-level, mixed modelling (site as a random effect; trial as a fixed effect). The primary outcome was a composite of postoperative pulmonary complications within the first week, analysed using mixed-effect logistic regression. Pre-specified subgroup analyses of nine patient characteristics and seven procedure and care-delivery characteristics were also performed. RESULTS: Complete datasets were available for 1913 participants ventilated with high PEEP and recruitment manoeuvres, compared with 1924 participants who received low PEEP. The primary outcome occurred in 562/1913 (29.4%) participants randomised to high PEEP, compared with 620/1924 (32.2%) participants randomised to low PEEP (unadjusted odds ratio [OR]=0.87; 95% confidence interval [95% CI], 0.75-1.01; P=0.06). Higher PEEP resulted in 87/1913 (4.5%) participants requiring interventions for desaturation, compared with 216/1924 (11.2%) participants randomised to low PEEP (OR=0.34; 95% CI, 0.26-0.45). Intraoperative hypotension was associated more frequently (784/1913 [41.0%]) with high PEEP, compared with low PEEP (579/1924 [30.1%]; OR=1.87; 95% CI, 1.60-2.17). CONCLUSIONS: High PEEP combined with recruitment manoeuvres during low tidal volume ventilation in patients undergoing major surgery did not reduce postoperative pulmonary complications.
