Abstract

Ultrasound-based shear wave elastography is a promising technique to non-invasively assess the dynamic stiffness variations of the heart. The technique is based on tracking the propagation of acoustically induced shear waves in the myocardium of which the propagation speed is linked to tissue stiffness. This measurement is repeated multiple times across the cardiac cycle to assess the natural variations in wave propagation speed. The interpretation of these measurements remains however complex, as factors such as loading and contractility affect wave propagation. We therefore applied transthoracic shear wave elastography in 13 pigs to investigate the dependencies of wave speed on pressure–volume derived indices of loading, myocardial stiffness, and contractility, while altering loading and inducing myocardial ischemia/reperfusion injury. Our results show that diastolic wave speed correlates to a pressure–volume derived index of operational myocardial stiffness (R = 0.75, p < 0.001), suggesting that both loading and intrinsic properties can affect diastolic wave speed. Additionally, the wave speed ratio, i.e. the ratio of systolic and diastolic speed, correlates to a pressure–volume derived index of contractility, i.e. preload-recruitable stroke work (R = 0.67, p < 0.001). Abstract

Shear Wave Elastography (SWE) has been proposed to investigate cardiac health by non-invasively monitoring tissue stiffness. Previous work has shown that the plate-like geometry of the Interventricular Septum (IVS) may result in a dispersion similar to Lamb waves, complicating the link between shear wave speed and cardiac stiffness. However, the IVS is not a simple plate, e.g., its thickness tapers across its length. We have used 2-D Finite Element simulations to investigate the effects of tapering on Lamb waves. The model consists of an elastic slab immersed in water, with a thickness decreasing smoothly in space from 9 to 3 mm. Pulses with low (0-80 Hz) and high (0-700 Hz) frequency contents were used to excite natural and acoustic radiation force induced waves. The results show that natural waves can decelerate by up to 20% during propagation, leading to ambiguities in speed estimation. Moreover, neglecting tapering when fitting their dispersion curves can introduce errors in shear modulus estimation by up to 30%. In contrast, fits performed on waves with high frequency content yielded shear modulus estimations with < 5%. These results suggest that septal geometry can affect cardiac stiffness estimation performed by SWE, especially when natural waves are employed. [ABSTRACT FROM AUTHOR]

Abstract

(1) Background: We aimed to assess the validity of laser Doppler vibrometry (LDV) as an emerging method to measure the local pulse wave velocity (PWV) from skin displacement generated by the pressure pulse inside an underlying artery. (2) Methods: A finite element model representing a simplified common carotid artery embedded within a soft tissue mimicking material was used to reproduce how tissue motions due to a wave propagation along the artery radiates to the skin surface. A parametric study was set up, varying: (i) the pressure conditions inside the artery (shock and traveling pressure impulse), (ii) the arterial depth and (iii) the geometry in a patient-specific artery model. (3) Results: under all conditions, the arterial pulse induced primary and secondary waves at the skin surface; of which the propagation speed deviated from the imposed PWV (deviations between −5.0% to 47.0% for the primary wave front). (4) Conclusions: the propagation of a short pressure impulse induced complex skin displacement patterns revealing a complicated link between PWV and measured propagation speeds at the skin surface. Wave propagation at the skin level may convey information about arterial PWV, however, advanced signal analysis techniques will be necessary to extract local PWV values. [ABSTRACT FROM AUTHOR]
