Abstract

A retrospective study conducted in Italy analyzed data from 116 patients with severe atopic dermatitis (AD) to determine if the drug upadacitinib could improve symptoms of concomitant allergic rhinitis (AR) or allergic asthma (AA). The study found that after 16 weeks of treatment with upadacitinib, 81.9% of patients achieved a reduction of at least 75% in their AD symptoms, and 56.03% achieved a reduction of at least 90%. Upadacitinib also showed a slight improvement in symptoms of AA and AR. The study suggests that upadacitinib may have a beneficial impact on AA and AR symptoms in patients with severe AD, but further research is needed. [Extracted from the article]

Abstract

Introduction:5-fluorouracil (5-FU) is one of the most effective topical treatments for actinic keratosis (AK). A new 4% formulation of 5-FU was recently approved in Europe. Objectives:This study aimed at evaluating 4% 5-FU cream safety and effectiveness in a real-world setting. Methods:Adult AK patients were retrospectively selected from the University of Campania Dermatology Unit database. Selection criteria included a diagnosis of non-hyperkeratotic, non-hypertrophic AK (Olsen grade I and II) of the face, ears, and/or scalp, treatment with 4% 5-FU once daily for 4 weeks, and at least 3 follow-up visits (4 and 8 weeks after treatment initiation, and 6 months after treatment end). The primary objectives were to evaluate AK lesions improvement at 8 weeks and relapse rate at 6 months. Patient-reported erythema and burning sensation intensity were also assessed at 4 weeks. Results: Ninety-eight patients were included in this analysis (male/female 80/18, mean age 74.7 years). AK lesions improvement at 8 weeks resulted complete or significant in 74.5% and 20.4% of the patients, respectively. At 6 months, 65.3% of the patients did not show AK relapses. Burning sensation at 4 weeks was reported as light, moderate, or absent by 44.9%, 22.4%, and 31.6% of the patients, respectively. Erythema was reported as light, moderate, or absent by 37.8%, 51%, and 10% of the patients, respectively. Burning sensation and erythema disappeared gradually during follow-up. No other side effects were reported. Conclusions:In this real-world study 4% 5-FU proved to be highly effective for AK lesions clearance with a favorable safety profile.

Abstract

Atopic dermatitis (AD) therapeutic approach calls for a long-term treatment. Treatment options for AD have recently undergone a revolutionary change by the introduction of the first biologic drug. Availability in daily practice of the last version of international AD guidelines, taking peculiarities of the country into account, can contribute to good clinical practice in Italy. To adapt European Dermatology Forum (EDF) guidelines for AD to the Italian medical–legal context, the EDF guidelines were assessed independently by two independent Italian renowned experts in the field and further integrated with articles published and systematically reviewed before May 2019. The first draft was collegially corrected and updated by the members of the SIDEMAST, ADOI, and SIDAPA. Recommendation levels (A; B; C; D) were graded based on the evidence levels (1–4). The adapted guidelines presented here focus on topical and systemic therapies in AD patients, both children and adults. As opposed to previous Italian guidelines, they include indications about biologics. New relevant evidence available from very recent literature and peculiarities of the Italian medical and legal context have been integrated in the revision process. If compared to general guidelines for AD not adapted to a specific national and cultural context, a revision for specific Italian needs is now available: It comprises the option of implementing the new biologic treatments and is likely to provide an important contribution to the improvement of clinical practice in Italy. Cooperation between patients, dermatologists, allergologists, and pediatricians remains mandatory in AD management. The authors of the present revision recommend an update of the Italian guidelines to be performed at least every second year.

Abstract

Patch testing is the standard procedure used to diagnose allergic contact dermatitis. It is an in-vivo test, which reproduces the reaction to a contact allergen. This in-vivo test aims to reproduce the elicitation phase of allergic contact dermatitis and is performed applying allergens under occlusion on the skin under standardized conditions. These guidelines for the best practice in performing patch test have been developed by an Italian group of experts taking in account the Italian legislation and local pharmacological governance. Guidelines are adapted from the original article under the guidance of the European Society of Contact Dermatitis (ESCD) and on the basis of the SIDAPA guidelines.

Abstract

Pemphigus is a life threatening autoimmune epidermal blistering disease involving skin and mucous membranes. Pemphigus usually affects middle age men and women involving oral mucosa first and then spreading on the skin. It is caused by the presence of autoantibodies (IgG and less frequently by IgA) directed against desmogleins, and/or other glycoproteins that plays a critical role in cell-cell attachment. Upon a predisposing genetic background, different agents have been shown to act as triggers for the pathogenesis of pemphigus. This guideline for the diagnosis and treatment of pemphigus has been developed by an Italian group of experts taking in account the Italian legislation and local pharmacological governance. Guidelines are adapted from the original article under the guidance of the European Dermatology Forum in collaboration with the European Academy of Dermatology and Venereology. It summarizes evidence-based and expert-based recommendations (S2 level).

Abstract

Bullous pemphigoid is the most common autoimmune subepidermal blistering disease of the skin and mucous membranes. This disease typically affects the elderly and presents with itch and localized or generalized bullous lesions. In up to 20% of affected patients blister may be completely absent, and only excoriations, prurigo-like lesions, eczematous lesions, urticated lesions, and/or infiltrated plaques are observed. The disease is significantly associated with neurological disorders. The morbidity of bullous pemphigoid and its impact on the quality of life are significant. So far, a limited number of national treatment guidelines have been proposed, but no common European consensus has emerged. This guideline for the treatment of bullous pemphigoid has been developed by an Italian group of experts taking in account the Italian legislation and local pharmacological governance. Guidelines are adapted from the original article under the guidance of the European Dermatology Forum (EDF) in collaboration with the European Academy of Dermatology and Venereology (EADV). It summarizes evidence-based and expert-based recommendations (S2 level).

Abstract

Summary: Alopecia areata (AA) is a complex immune‐mediated disorder, which is difficult to treat. The available treatment options seem to have limited benefit, help only some patients and have a high relapse rate. We evaluated a new therapeutic option for moderate to severe AA based on the combination of photodynamic therapy (PDT) with 5‐aminolaevulinic acid (ALA) and microneedling (MN). In total, 14 patients were enrolled, and these were randomly divided into 3 groups: Group A (MN alone; n = 9), Group B (ALA‐PDT alone; n = 15) and Group C (combination of MN and ALA‐PDT; n = 17). All patients were treated once every 3 weeks for a total of six treatments. The best clinical outcome was achieved in Group C, with complete hair regrowth observed in three patients, and an improvement of ≥ 50% and < 50% of the treated areas obtained in seven and six patients, respectively. Our report suggests that combination of ALA‐PDT with MN could be an additional therapeutic option in moderate to severe AA, as MN allows better skin penetration of ALA and subsequent indirect immunosuppression. 43. ERYTHEMA YRATUM REPENS AND RHEUMATOID ARTHRITIS:CONNECTED FOR THE FIRST TIMe

Abstract

Sir, A 62-year-old Caucasian man was referred to us with multiple erythematous arciform lesions with a wood-grain-like appearance of fifteen months' evolution. The dermatosis had initially affected the patient's legs, extending to his abdomen and arms. His medical history included rheumatoid arthritis, treated with systemic corticosteroids; DMARDs (methotrexate) with folic acid supplements; omeprazole; and cardioaspirin. Interestingly, the resolution of rheumatoid arthritis with various DMARDs was related to the resolution of the dermatosis. Physical examination disclosed an eruption on the legs characterized by irregular, multiple, gyrate bands of erythema bordered on one side by scale and on the other by normal-looking skin. The lesions migrated, leaving a thin scale at the edges and creating a bizarre pattern [Figure 1]. The dermatosis exhibited characteristic rapid movement over hours. Pruritus was moderate. The patient had previously taken oral corticosteroids which had been partially successful, but his condition had worsened rapidly when treatment was interrupted. Histopathological findings were non-specific, showing evidence of hyperkeratosis, orthokeratosis, and a superficial, perivascular, mononuclear infiltrate, which although non-diagnostic, were consistent with changes described for erythema gyratum repens (EGR) [Figure 2]. Several case reports have shown positive direct immunofluorescence (DIF) in the skin: DIF tests on the skin describe deposits of IgG and C3 along the basement membrane zone. [1] However, DIF of involved non-sun-exposed and uninvolved skin was negative in our patient. The patient had a family history of esophageal cancer, so the search for an associated neoplasm began. Laboratory tumor markers were all normal. Thirty-two months after the onset of EGR, investigations (chest X-ray, gastroscopy, colonoscopy, abdominal ultrasound, computerized tomography of the chest, abdomen and pelvis, and whole body PET-CT) still proved normal. The presentation of the skin disorder was so distinctive that a differential diagnosis was difficult to generate. In our patient, the differential diagnoses included erythema annulare centrifugum (EAC), urticarial vasculitis, and systemic lupus erythematosus. Distinction between EGR and EAC is very difficult, since clinical findings are similar; the histopathologic pictures of both diseases are nonspecific and both can be associated with malignancy (EGR more often). However, one of the differences between them is in the rapidity of movement. EGR exhibits characteristic rapid movement over minutes and hours; EAC moves slowly over days and months. The gyrate erythema presented in this case had clinical features similar to both EGR and EAC, but the rapidity of movement was more consistent with EGR. The differential diagnosis between EGR and urticarial vasculitis was made histopathologically. Urticarial vasculitis typically shows leukocytoclastic vasculitis. Classic symptoms of urticarial vasculitis, such as painful burning sensation, residual purpura, and pigmentation, were not present. Scaling trailing the leading edge of the eruption, which is one of the typical findings of EGR, was present in our case. The patient was studied for tuberculosis; chest X-ray, CT, Mantoux test, and Quantiferon test were all negative. EGR is a rare specific dermatosis, believed to be an obligate paraneoplastic cutaneous syndrome. In more than 80% of cases, EGR is associated with an underlying malignancy and in 80% of cases, this distinctive migratory eruption appears prior to a malignancy diagnosis. EGR can precede 1 to 72 months before malignancy, though the average is 4 to 9 months. Approximately, one-third of patients will manifest lung cancer, 8% esophageal cancer, and 6% an unknown primary. Tumors of the breast, bladder, prostate, cervix, and uterus have also been reported. [2] Here, however, we report a case of EGR which is non-paraneoplastic. Case reports demonstrating an association with non-neoplastic diseases have also been reported, such as pulmonary tuberculosis, drug use, hypereosinophilic syndrome, ichthyosis, autoimmune conditions such as CREST syndrome [3] or disorders with an EGR-like presentation such as tinea imbricata, lupus erythematosus, bullous diseases, resolving pityriasis rubra pilaris, [4] psoriasis, and Sjφgren syndrome. [5] This association may either be an example of an extremely unusual clinical presentation of the primary dermatosis in question, or may be a case of "true" EGR as observed during the course of other non-neoplastic conditions. Healthy patients with this distinctive eruption have been reported in literature, which might cast doubt on the reliability of the diagnosis. To our knowledge, no case of EGR associated with rheumatoid arthritis has, to date, been described in literature, making our case the first reported description of its kind. Moreover, a parallel evolution of the two affections (EGR and rheumatoid arthritis) was observed in our patient. In conclusion, our experience shows that EGR does not necessarily indicate underlying malignancy, but it can be associated with a non-neoplastic disease such as rheumatoid arthritis. References 1. Abstract

A demographic "revolution" has been taking place in the last decades: the world population, due to an increase in life expectancy, is ageing.1 Centenarians are no longer a rarity: they are 14 000 in Italy, and other countries, such as Japan, have more than 70 000 most of which are women.2 Data on skin surgery in the elderly have been reported only for patients up to 75 years old3; however, no data are available for centenarians. The lesions excised were as follows: 9 squamous cell carcinomas (SCC), 8 basal cell carcinomas (BCC) and 1 melanoma. Cutaneous surgery under local anesthesia in very elderly patients 90 years of age and older is as safe as in elderly patients ranging in age from 75 to 80 years old. [Extracted from the article]
