Abstract

Background Improving stroke services is critical for reducing the global stroke burden. The World Stroke Organization–World Health Organization– Lancet Neurology Commission on Stroke conducted a survey of the status of stroke services in low and middle-income countries (LMICs) compared to high-income countries. Methods Using a validated World Stroke Organization comprehensive questionnaire, we collected and compared data on stroke services along four pillars of the stroke quadrangle (surveillance, prevention, acute stroke, and rehabilitation) in 84 countries across World Health Organization regions and economic strata. The World Health Organization also conducted a survey of non-communicable diseases in 194 countries in 2019. Results Fewer surveillance activities (including presence of registries, presence of recent risk factors surveys, and participation in research) were reported in low-income countries than high-income countries. The overall global score for prevention was 40.2%. Stroke units were present in 91% of high-income countries in contrast to 18% of low-income countries (p Conclusions There is an urgent need to improve access to stroke units and services globally especially in LMICs. Countries with less stroke services can adapt strategies from those with better services. This could include establishment of a framework for regular monitoring of stroke burden and services, implementation of integrated prevention activities and essential acute stroke care services, and provision of interdisciplinary care for stroke rehabilitation.

Acute stroke treatments were offered in ∼ 60% of high-income countries compared to 26% of low-income countries (p = 0.009). Compared to high-income countries, LMICs provided less rehabilitation services including in-patient rehabilitation, home assessment, community rehabilitation, education, early hospital discharge program, and presence of rehabilitation protocol. Conclusions: There is an urgent need to improve access to stroke units and services globally especially in LMICs. Countries with less stroke services can adapt strategies from those with better services. This could include establishment of a framework for regular monitoring of stroke burden and services, implementation of integrated prevention activities and essential acute stroke care services, and provision of interdisciplinary care for stroke rehabilitation.

5. CONVERSION OF CLINICALLY ISOLATED SYNDROME (CIS) TO MULTIPLE SCLEROSIS: A FOUR YEAR MONITORING REPORT.

Abstract

Introduction: Demyelinating diseases, previously thought of as nonexistent in Brazil, are now experiencing a new scenario, numerous publications, new drugs and diagnostic criteria. The revision of diagnostic criteria for multiple sclerosis (MS) has enabled earlier treatment to be offered, including for clinically isolated syndrome (CIS), which is characterized as a first episode of neurological symptoms lasting for at least 24 hours and caused by one or more demyelinating lesions in the central nervous system. Research has shown an average period of 4 years for the conversion of CIS to MS to occur. In Joinville, Santa Catarina, the prevalence of MS is 17.1 cases per 100,000 inhabitants (95% Cl: 13.7-21), similar to other Brazilian populations. This present study aimed to determine the mean time for the conversion of CIS to MS in a population-based cohort in Joinville, Brazil. Materials and Methods: A prospective study was conducted of all cases of CIS and MS occurring between January 2010 and January 2014. The revised 2010 McDonald criteria were used for diagnosis. Results: A total of 90 patients with MS and 13 with CIS were recorded. Of those patients with CIS, 7 converted to MS in the four year study period, at a mean of 6 months. Additionally, 43% (3/7) converted in one year (2013). Discussion and Conclusion: The conversion time in this study was 6 months in the period of four years. The authors question whether there is under-diagnosis of CIS. A scarcity of published studies was noted in relation to conversion times for CIS to MS and of profile descriptions of these patients. Abstract

Introduction: The incidence of multiple sclerosis (MS) varies considerably around the world. To our knowledge, no population-based study related to MS has been carried out and published in Brazil. Therefore, the aim of this research was to measure the incidence of MS in the city of Joinville, in southern Brazil. Materials and Methods: All new patient cases diagnosed with MS between 2010 and 2014 in the city of Joinville were recorded. Individuals diagnosed with clinically isolated syndrome (CIS) and already in conversion before 2013 were recorded as MS cases. Tire revised 2010 McDonald criteria were applied for the clinical diagnosis of MS and CIS. The rates were adjusted to the Brazilian and world population. Results: The definite and probable age-adjusted MS incidence was 3.2 (95%CI, 1.9-5.1) per 100,000 inhabitants. Seven patients were diagnosed with CIS over the four year period. Of these, three patients (43%) converted to MS in 2013. The median conversion time from CIS to MS was six months. Discussion and Conclusion: The incidence of MS in Joinville, located in tropical Latin America, was similar to the 1.76 (95%CI, 1.1-2.8) per 100,000 inhabitants found in Argentina (southern Latin America). Taking into consideration that the time for evolution of CIS to MS is 3-4 years, this present study demonstrated a conversion of 43% of patients in 1 year. 7. Association between Albuminuria, Glomerular Filtration Rate and Mortality or Recurrence in Stroke Patients.

Abstract

Background/Aims: In an attempt to find new predictors of stroke prognosis, we evaluated the association of albuminuria (AUr) and the estimated glomerular filtration rate (eGFR) with the recurrence of stroke and mortality. Methods: We evaluated and followed for at least 7 months patients with first-ever stroke or transient ischemic attack admitted to a prospective cohort from March 2005 to December 2007. We analyzed traditional CV risk factors, albumin-to-creatinine ratio and eGFR (ml/min/1.73 m2) as predictors of mortality or recurrence. Results: From a total of 185 patients included [57% (104/185) men, 64 ± 13 years], 38 patients suffered from a recurrent stroke or died, with a mean follow-up of 25.1 ± 8.7 months. AUr (≥30 mg/g) was found in 50.2% (93/185), and 38.9% (72/185) presented an eGFR <60. In univariate analysis, age >65 years, eGFR ≤50, atrial fibrillation (AF), no alcohol intake and AUr >17 mg/g were associated with the composite endpoint. In a multivariate analysis, AF and AUr >17 mg/g were independent predictors of the composite endpoints, but eGFR ≤50 was not. Conclusion: The presence of AUr >17 mg/g is independently associated with death or recurrence after stroke. Abstract

Movement disorders are not common in acquired immunodeficiency syndrome. Hemichorea-hemiballism (HC-HB) is the most common of them all, and it is usually related to oportunistic toxoplasmosis of the basal ganglia. We present a 28-year-old man, HIV positive with HC-HB caused by a right subthalamic granuloma, which did not respond to treatment for toxoplasmosis. Cryptoccococic antigen was positive in the cerebrospinal fluid and antifungic therapy led to clinical and radiologic improvement, thus the diagnosis of a granulomatous lesion by Cryptococcus neoformans was established. Current literature on HC-HB and its relationship with AIDS is subsequently reviewed.

Abstract

Ossification of the posterior longitudinal ligament (OPLL) is an uncommon cause of compressive myelopathy in the Caucasian population. A case of spastic paraparesis in a Caucasian man whose radiological investigation showed OPLL is presented. The radiographs of the cervical spine showed a strip of bony density posterior to the vertebral bodies, extending from C2 to T1. Computerized tomography (CT) and CT myelography showed OPLL at the same level. Magnetic resonance showed an area of increased signal on T2-weighted sequences at C7-T1 level suggestive of myelomalacia. The patient underwent an open-door laminoplasty (C2 to C7) with improvement of the paraparesis. OPLL should be included in the differential diagnosis of cervical myelopathy. It can be easily detected by plain radiographs and CT of the cervical spine. A review of the clinical and radiological features and the treatment of OPLL is presented.

Abstract

The paucity of epidemiologic data, and the previous impression of high incidence of cerebrovascular disease in Brazil, made us elaborate a prospective institutional study in Joinville, Brazil, with objective of identifying first and recurrent episodes in stroke. This study occurred from March 1995 to March 1996. We evaluated during the first episode of stroke: incidence, mortality and fatality-case ráte (in 30 days lethality), frequency of risk factor, time in hospital and distribution of cerebral infarcts by pathological subtypes. In this period, 429 patients with stroke were registered, 320 with the first episode. 98% of all the patients were submited to at least one computed tomography scanning. The episodes of cerebral infarcts were divided in pathological sub-types. Results showed that the annual incidence adjusted to the age of first stroke episode was 156/100,000. The distribution by diagnosis was: 73.4% for cerebral infarct, 18.4% for cerebral hemorrhage and 7.5% for subarachnoid hemorrhage. The annual standard mortality rate was 25/100,000. The fatality case global rate in 30 days was 26%. Hypertension, previous stroke and diabetes were the most frequent risk factors. Only 25% of the patients were assisted within three hours of the onset of stroke. We concluded that the incidence rate of first episode of stroke is high in institutionalized patient in Joinville, Brazil. The mortality and fatality--case rates are similar to the ones of other populations.
