Objective To describe demographic, clinical and laboratory features of systemic sclerosis sine scleroderma (ssSSc) in a large multicentre systemic sclerosis (SSc) cohort.Methods Data involving 1808 SSc patients from Italian Systemic sclerosis PRogression INvestiGation registry were collected. The ssSSc was defined by the absence of any cutaneous sclerosis and/or puffy fingers. Clinical and serological features of ssSSc were compared with limited cutaneous (lcSSc) and diffuse cutaneous (dcSSc) subsets.Results Among patients with SSc, only 61 (3.4%) were classified as having ssSSc (F/M=19/1). - Abstract
Objectives To investigate pregnancy outcomes in women with autoimmune rheumatic diseases (ARD) in the Italian prospective cohort study P-RHEUM.it. Methods Pregnant women with different ARD were enrolled for up to 20 gestational weeks in 29 Rheumatology Centres for 5 years (2018-2023). Maternal and infant information were collected in a web-based database. Results We analysed 866 pregnancies in 851 patients (systemic lupus erythematosus was the most represented disease, 19.6%). Maternal disease flares were observed in 135 (15.6%) pregnancies. 53 (6.1%) pregnancies were induced by assisted reproduction techniques, 61 (7%) ended in miscarriage and 11 (1.3%) underwent elective termination. Obstetrical complications occurred in 261 (30.1%) pregnancies, including 2.3% pre-eclampsia. Two cases of congenital heart block were observed out of 157 pregnancies (1.3%) with anti-Ro/SSA. Regarding treatments, 244 (28.2%) pregnancies were treated with glucocorticoids, 388 (44.8%) with hydroxychloroquine, 85 (9.8%) with conventional synthetic disease-modifying anti-rheumatic drugs and 122 (14.1%) with biological disease-modifying anti-rheumatic drugs. Live births were 794 (91.7%), mostly at term (84.9%); four perinatal deaths (0.5%) occurred. Among 790 newborns, 31 (3.9%) were small-for-gestational-age and 169 (21.4%) had perinatal complications. Exclusive maternal breast feeding was received by 404 (46.7%) neonates. The Edinburgh Postnatal Depression Scale was compiled by 414 women (52.4%); 89 (21.5%) scored positive for emotional distress. Conclusions Multiple factors including preconception counselling and treat-to-target with pregnancy-compatible medications may have contributed to mitigate disease-related risk factors, yielding limited disease flares, good pregnancy outcomes and frequency of complications which were similar to the Italian general obstetric population. Disease-specific issues need to be further addressed to plan preventative measures.

- Abstract
Background: Intravenous iloprost has been widely used for the treatment of systemic sclerosis peripheral vasculopathy. No agreement has been found on the regimen and the dosage of intravenous iloprost in different scleroderma subset conditions. This study aimed to evaluate the modalities of intravenous iloprost administration within a large cohort of systemic sclerosis patients from the SPRING Registry and to identify any associated clinical-demographic, instrumental or therapeutic data.Patients and Methods: Data of systemic sclerosis patients treated with intravenous iloprost for at least 1 year (case group) were retrospectively analyzed, including different timing and duration of intravenous iloprost session, and compared with those of untreated patients (control group).Results: Out of 1895 analyzed patients, 937 (49%) received intravenous iloprost treatment, while 958 (51%) were assigned to the control group. Among cases, about 70% were treated every 4 weeks, 24% with an interval of more than 4 weeks, and only 6% of less than 4 weeks. Most patients receiving the treatment every 4 weeks, or less, underwent infusion cycle for 1 day only, while if it was scheduled with an interval of more than 4 weeks, a total number of 5 consecutive days of infusions was the preferred regimen. The comparison between the two groups revealed that patients treated with intravenous iloprost had a higher frequency of DUs (p < 0.001), pitting scars (p < 0.001), diffuse cutaneous involvement (p < 0.001), interstitial lung disease (p < 0.002), as well as higher rates of anti-topoisomerase I, “late” scleroderma patternat nailfold videocapillaroscopy. These findings were confirmed by multivariate analysis.Conclusion: Our data provide a picture on the Italian use of intravenous iloprost among systemic sclerosis patients and showed that it was usually employed in patients with a more aggressive spectrum of the disease. The disparity of intravenous iloprost treatment strategies in the different centers suggests the need of a rational therapeutical approach based on the clinical characteristics of different patients’ subsets.

Abstract domains of knowledge may have social origins. However, whether abstract concepts (ACs) may also differentially affect communicative interaction and conversation has not been explored. Here, we studied ACs' communicative functions by collecting in an Italian and an English sample, ratings for concrete concept (CC) and ACs related to three main dimensions: communicative/pragmatic [i.e., Openness to Negotiation (ON), Easiness to Start a Conversation (ESC)], semantic/metacognitive [i.e., Social Metacognition (SM) – perceived need of others, Word Confidence (WC), Contextual Availability (CA)], and emotional–experiential (i.e., Pleasantness, Valence, Familiarity). Overall, Italian participants judged it was easier to start a conversation, the more pleasant, familiar, and positively valenced were rated the concepts. Crucially, at lower values of the emotional–experiential component (i.e., Familiarity in the Italian sample, also Pleasantness and Valence in an English sample), there was an advantage of ACs over CCs in the ESC. Moreover, in the Italian sample, participants rated ACs higher on SM, ON, and lower on WC and CA. Notably, in both the Italian and English sample, ACs with higher ratings on the ESC dimension belonged to the Self-Sociality subcluster. 26. Temporomandibular Disorders and Serological Tests in Patients with Rheumatoid Arthritis.

Background: Rheumatoid Arthritis (RA) is a systemic and chronic autoimmune disease with inflammation at the synovial joints. The purposes of this study were to evaluate the correlation between serological variables and temporomandibular disorders (TMDs) in patients diagnosed with RA, evaluated through diagnostic criteria for temporomandibular disorders (DC/TMD), and to check the influence of comorbidities on the different TMD signs and symptoms, or any serological variables. Methods: This observational cohort research study included seventeen patients affected by RA. The comorbidities and some variables from the initial serological analyses were collected from the clinical rheumatological charts. Then, the presence of any of the following TMD signs/symptoms, temporal myalgia, temporomandibular joint arthralgia, click or crepitus, was evaluated through the symptom questionnaire of the DC/TMD during clinical evaluation following the DC/TMD examination form. Results: Rheumatoid factor (RF), anticitrullinated protein antibodies (ACPA), and anti-nuclear antibody (ANA) positivity were present in 82.4%, 52.9%, and 41.2% of patients, respectively. Indicators of tissue inflammation were evaluated with median values of 21 mm/h for erythrocyte sedimentation rate (ESR) and 0.50 mg/dL for C-reactive protein (CRP). The RA patients presented comorbidities such as hypertension in 70.6% and diabetes in 23.5%. Negative ACPA had a significant association with myalgia (p = 0.03), and positive ANA was significantly associated with crepitus (p = 0.05). Conclusion: ANA and ACPA evaluation can be considered predictive serological tests associated with specific TMDs. Conversely, no influence of any comorbidities was found between different TMD signs and symptoms, or any serological variables. [ABSTRACT FROM AUTHOR]

Recently, the research on language has manifested the urgency to develop interactive methods to study conceptual categories (abstract, concrete) in ecological scenarios. We believe that studying concepts during online interactions might benefit on the one hand the research on abstract concepts and on the other hand the acquisition of knowledge about the effectiveness of online interactions in fostering interpersonal rapport. Since abstract concepts compared with concrete concepts are mainly acquired through verbal dialogue ( Borghi et al., 2017; 2018; Fini & Borghi, 2019; Fini et al., 2021), investigating them within a well-controlled social context represents the ideal experimental approach to capture how the semantics affect interpersonal relations. Along this perspective, in Zdrazilova et al., 2018, pairs of participants were required to perform the so-called ‘taboo task’, communicating to a partner the meaning of concrete and abstract words without using the words themselves. The authors then analyzed the speech and gestures associated with the different kinds of concepts, highlighting, for example, that when explaining the meaning of abstract concepts participants used more introspective expressions, referred more to people than to objects, and used more metaphorical and beat gestures, and fewer iconic gestures, than when they explained the meaning of concrete concepts. Fini et al. (2021), instead, used an interactive paradigm (Era et al., 2022; Boukarras et al., 2021) to investigate whether being helped when guessing abstract concepts from visual images led to improved motor coordination between the participant and the confederate who helped. Specifically, before and after a joint action task, participants took part in a concept guessing task, where two confederates provided participants with hints to guess the correct abstract or concrete concept associated with the image. The results indicate that participants asked for more hints to guess abstract concepts as compared with concrete concepts and were aware that the other’s contribution was more crucial for abstract compared with concrete concepts. Moreover, in the joint action task, participants were more synchronous in movement with the experimenter who offered them suggestions on abstract words. The same authors recently developed a new interactive paradigm requiring conversations on online platforms (Fini et al., in prep.). The hypothesis of this study was that, if interacting about abstract concepts improves motor coordination between the actors, it might be expected that also other constructs that are sensitive to interpersonal interactions, such as the physical/interpersonal distance, might be modulated after having interacted about an abstract topic. This was tested by asking participants to write some sentences associated to abstract and concrete concepts through an online platform. The experiment included a social condition, in which participants conversed in dyads, and an individual condition, in which they wrote about concrete or abstract concepts by knowing that on the other side of the screen there was a person doing the same and that later they would have read what the other had written. In the individual condition, thus, participants knew that they were writing at the same time of another person, and on the same content, but did not interact with each other. After each conversation/verbal production, the psychological distance and the physical/interpersonal distance between the actors were measured, respectively through the Inclusion of the Other in the Self (iOS) Scale (Aron et al., 1992) and a Visual Analogue Scale (VAS), where the distance between two avatars embodying the interlocutors could be regulated (Lisi et al., 2021). Together with these measures, participants were asked to evaluate on a Visual Analogue Scale (VAS) the Pleasantness, the Intimacy, the Perceived Difficulty experienced during the verbal production, and the Self contribution, the Other contribution and the Commitment in the experimental social condition. Results revealed that conversing increased the psychological and physical/interpersonal closeness between participants compared to the condition in which participants were not conversing, regardless of the abstract or concrete content of the verbal production. Crucially, the more actors perceive interlocutors' contribution to a conversation about abstract topics, the more psychologically close to the interlocutors they feel. The current study crucially expands the previous one, by exploring whether conversing about different kind of abstract concepts (i.e. philosophical-spiritual (PS), Emotional (EM), Self-sociality (SS), and physical-spatio-temporal-quantitative (PSTQ)) might differently modulate psychological and physical/interpersonal closeness. The experimental paradigm is identical to the one of the study described above, except for the fact that the present study focused only on the social experimental condition. In this regard, a recent study of Villani et al., 2022, has shown that during simulations of conversational exchanges, in which participants responded to sentences involving sub-kinds of concrete (tools, animals, food) and abstract concepts (philosophical-spiritual (PS), Emotional (EM), Self-sociality (SS), and physical-spatio-temporal-quantitative(PSTQ)), the content of the verbal production changed. Specifically, abstract concepts generated higher uncertainty and more interactive exchanges than concrete ones. Along with these results, our aim here was to better explore how concepts of different nature can affect the interpersonal representations, specifically the sense of closeness between the interlocutors, during real verbal exchanges in an on-line platform.

Background: Remdesivir is an antiviral used to treat COVID-19 which improves some clinical outcomes. Dexamethasone has been shown to be effective in reducing mortality. It has been hypothesized that combination of these two drugs can improve mortality. We evaluated the effect of combination on mortality of COVID-19 patients requiring O2 therapy. Methods: A prospective quasi-experimental study, including two independent, sequential controlled cohorts, one received remdesivir-dexamethasone and the other dexamethasone alone, was designed. All COVID-19 patients requiring supplemental O2 therapy were enrolled consecutively. The sample size to power mortality was a priori calculated. The primary endpoints were 30-day mortality and viral clearance differences. Secondary endpoints were differences in hospitalization times, improvement in respiratory failure (PO2/FiO2) and inflammatory indices (fibrinogen, CRP, neutrophil/lymphocyte ratio, D-Dimer). Kaplan-Meier curves and the log-rank test were used to evaluate significant differences in mortality between groups. Results: 151 COVID-19 patients were enrolled (remdesivir/dexamethasone group, 76 and dexamethasone alone,75). No differences in demographic, clinical and laboratory characteristics were observed between the two groups at baseline. concepts (i.e., “democracy”, “justice”, “peace”) are thought to be mainly acquired through linguistic interactions (Wauters et al., 2003), because they are more detached from a referent to be experienced through the five senses. Their meanings are grounded in social interactions, which represent the essential scaffolding allowing the emergence of “shared-cultural labels” conveying complex knowledge (Fini & Borghi, 2019; Fini et al., 2021). Evidence showing the activation of mouth motor areas during the processing of abstract concepts (Borghi et al., 2011; Granito et al., 2015; Borghi & Zarcone, 2016; Mazzuca et al., 2018) suggests the origin of these is tightly related to social aspects. In keeping with the the WAT proposal (Borghi et al., 2017; Borghi et al., 2017, 2018; Borghi et al., 2019), this activation may indicate that we: i) prepare ourselves to complement our knowledge by asking someone reliable to provide an explanation, or to validate a meaning (social metacognition, Shea et al., 2018; Borghi et al., 2017, 2018; Borghi et al., 2019; Borghi, 2022); ii) re-explain ourselves the meaning associated with the concept; iii) simulate the linguistic experience through which we have learned abstract concepts in the first place. The specific mechanisms -not mutually exclusive- which might undergo the activation of mouth motor areas share a common ground: abstract concepts evoke linguistic experiences. The interactive nature of abstract concepts is manifested not only in the modality through which they are acquired, but also in their use. The process of conflating different experiences in a single symbol characterizes all concepts and words (e.g., Dunbar, 1998; Deacon, 1998). However, the heterogeneity of experiences abstract concepts evoke makes us more uncertain of their meaning and renders the input and interaction with others particularly crucial. During their use, we might need more other people to understand better the meaning of words we are not certain of, or to negotiate and discuss with them this meaning (Borghi, 2022; Mazzuca & Santarelli, 2021). As proposed by Dunbar (1998), the development of abstract representations might evoke the need to be connected with conspecifics. Abstract concepts are not only the “glue” holding together scattered and heterogeneous information (see the notion of situational systematicity in Davies, Altman & Yee, 2020), but they also represent a “social glue” providing a common reference of knowledge within societies (Borghi, 2022; Borghi & Tummolini, 2020; Mazzuca & Santarelli, 2021; Mazzuca et al., 2021). Along these lines, dialogical exchanges might be a preferential grounding source for abstract concepts. Indeed, conversations are the spontaneous human practices of being in relation. So, given their intrinsic social and dialogical nature, we might expect that compared to concrete concepts abstract concepts might more easily “afford” conversations. Here, we intend to operationalise this intuition by investigating the facility to start a conversation, manipulating the abstractness and concreteness of prompting concepts. The Easiness to Start a Conversation with abstract concepts might be driven by different aspects: how much the meaning of a word can be negotiated/discussed (Openness to Negotiation - ON); how much we need others’ contribution to validate the meaning of a word (Social Metacognition - SC, Villani et al., 2019); how much we feel confident in mastering the meaning of the word (Word Confidence - WC), and the ease with which we can think of a context for each word (Context Availability - CA, Schwanenflugel, Harnishfeger & Stowe, 1988). So, investigating new dimensions like Easiness to Start a Conversation, Openness to Negotiation, Social Metacognition, and Word Confidence might be particularly relevant to gather knowledge on how different sub-kinds of concepts are used in linguistic exchanges. In the current study, we will ask participants to rate abstract and concrete words along different dimensions, spanning from more traditional dimensions to more innovative ones. These are: Easiness to Start a Conversation (ESC), Social Metacognition (SC), Openness to Negotiation (ON), Familiarity (FAM), Word Confidence (WC), Contextual availability (CA), Pleasantness and Valence.

- Abstract
Objective: Autoimmune systemic diseases (ASD) represent a predisposing condition to COVID-19. Our prospective, observational multicenter telephone survey study aimed to investigate the prevalence, prognostic factors, and outcomes of COVID-19 in Italian ASD patients. Methods: The study included 3,918 ASD pts (815 M, 3103 F; mean age 59 +/- 12SD years) consecutively recruited between March 2020 and May 2021 at the 36 referral centers of COVID-19 and ASD Italian Study Group. The possible development of COVID-19 was recorded by means of a telephone survey using a standardized symptom assessment questionnaire. Results: ASD patients showed a significantly higher prevalence of COVID-19 (8.37% vs. 6.49%; p<0.0001) but a death rate statistically comparable to the Italian general population (3.65% vs. 2.95%). Among the 328 ASD patients developing COVID-19, 17% needed hospitalization, while mild-moderate manifestations were observed in 83% of cases. Moreover, 12/57 hospitalized patients died due to severe interstitial pneumonia and/or cardiovascular events; systemic sclerosis (SSc) patients showed a significantly higher COVID-19-related death rate compared to the general population (6.29% vs. 2.95%; p=0.018). Major adverse prognostic factors to develop COVID-19 were: older age, male gender, SSc, pre-existing ASD-related interstitial lung involvement, and long-term steroid treatment. Of note, patients treated with conventional synthetic disease-modifying antirheumatic drugs (csDMARDs) showed a significantly lower prevalence of COVID-19 compared to those without (3.58% vs. 46.99%; p=0.000), as well as the SSc patients treated with low dose aspirin (with 5.57% vs. without 27.84%; p=0.000). Conclusion: During the first three pandemic waves, ASD patients showed a death rate comparable to the general population despite the significantly higher prevalence of COVID-19. Objective. There is still a great deal to learn about the influence of sex in systemic sclerosis (SSc). In this respect, national registries provide large and homogeneous patient cohorts for analytical studies. We therefore investigated a wide-ranging and well-characterized SSc series with the aim of identifying sex differences in disease expression, with a special focus on demographic, clinical, and serological characteristics.Methods. A multicenter SSc cohort of 2281 patients, including 247 men, was recruited in the Italian Systemic sclerosis PRogression INvestiGation (SPRING) registry. Demographic data, disease manifestations, serological profile, and internal organ involvement were compared.Results. The overall female/male ratio was 8.2:1. Female/male ratios for limited cutaneous SSc, diffuse cutaneous SSc, and SSc sine scleroderma subsets were 8.7:1, 4.9:1, and 10.7:1, respectively. A shorter time from onset of Raynaud phenomenon to SSc diagnosis, an increased prevalence of the diffuse cutaneous subset, renal crisis, and digital ulcers were found in males, whereas a significantly higher percentage of sicca syndrome, serum antinuclear antibodies, antiextractable nuclear antigens, anti-La/SSB, and anticentromere protein B was detected in the female group. Males exhibited lower left ventricular ejection fraction, as well as higher prevalence of conduction blocks, arrhythmias, ground glass, and honeycombing. Moreover, forced vital capacity and total lung capacity were medially lower in men than in women. Finally, males were more frequently treated with immunosuppressive drugs.Conclusion. Our study further supports the presence of several sex-related differences in patients with SSc. These differences were pronounced in the severity of cutaneous, peripheral vascular, and cardiopulmonary involvement for male patients, whereas an increased prevalence of sicca syndrome and a specific autoantibody profile characterized the female sex.

- Abstract
Background: Pain, related to Raynaud’s phenomenon or digital ulceration, has been identified as very prevalent and debilitating symptoms of systemic sclerosis (SSc), both significantly affecting patients’ quality of life (QoL). Pharmacological therapeutic strategies were found not to be sufficiently effective in the management of SSc-induced pain and fatigue, and evidence for exercise is scarce. As yet, the effects of a long-term, tailored exercise programme on pain and fatigue in patients with SSc have not been explored. In addition to pain and fatigue, this study aims to evaluate the effects of exercise on QoL, physical fitness, functional capacity, and vascular structure in people with SSc (PwSSc). Methods: This will be a multicentre (n = 6) randomised controlled clinical trial to assess the effect of a previously established, supervised 12-week combined exercise programme on pain and fatigue as compared to no exercise in PwSSc. The study will recruit 180 patients with SSc that will be allocated randomly to two groups. Group A will perform the exercise programme parallel to standard usual care and group B will receive usual care alone. Patients in the exercise group will undertake two, 45-min sessions each week consisting of 30-min high-intensity interval training (HIIT) (30-s 100% peak power output/30-s passive recovery) on an arm crank ergometer and 15 min of upper body circuit resistance training. Patients will be assessed before as well as at 3 and 6 months following randomisation. Primary outcomes of the study will be pain and fatigue assessed via questionnaires. Secondary outcomes include quality of life, structure of digital microvasculature, body composition, physical fitness, and functional capacity. Discussion: Data from this multi-centre research clinical trial will primarily be used to establish the effectiveness of a combined exercise protocol to improve pain and fatigue in SSc.
