- Abstract
RIVM rapport:De Nationale Voedselconsumptiepeiling 2019-2021 (VCP) geeft inzicht in wat, waar en wanneer Nederlanders eten en drinken. Dit rapport gaat in op de hoeveelheid energie en voedingsstoffen die mensen binnenkrijgen. De resultaten van ruim 3500 kinderen en volwassenen zijn vergeleken met de eerdere peilingen in 2007-2010 en 2012-2016. Hierbij is onderscheid gemaakt naar verschillende leeftijdsgroepen en geslacht. Het blijkt dat inwoners van Nederland de laatste jaren gemiddeld minder suiker en zout en meer vezels binnenkrijgen dan bij de vorige peiling in 2012-2016. Wel krijgen mensen nog te veel zout en te weinig vezel binnen. Ook drinken mensen, vooral mannen, minder alcohol. De gevonden ontwikkelingen zijn gunstig voor de gezondheid. Aangezien minder suiker en meer vezel in eten en drinken kan helpen om overgewicht en chronische ziekten te voorkomen. Minder zout is belangrijk voor een goede bloeddruk. Daarnaast krijgen alle leeftijdsgroepen meer vitamine D binnen. Ouderen in de leeftijd van 70-79 jaar krijgen nog steeds vaak te weinig van deze vitamine binnen. Voor hen is het belangrijk om het advies om vitamine D-supplementen te slikken, beter op te volgen. Genoeg vitamine D in combinatie met voldoende calcium maakt de kans op botbreuken kleiner. Er zijn nog andere voedingsstoffen die mensen nog veel of weinig binnenkrijgen in vergelijking met de voedingsnormen. Zo krijgen sommige bevolkingsgroepen weinig vitamines A, B2, B6, B11, C, calcium en ijzer binnen. Dit verschilt per leeftijdsgroep en geslacht. Als mensen weinig van zo'n voedingstof binnenkrijgen, hoeft dat niet meteen zorgelijk te zijn. Verder onderzoek, bijvoorbeeld naar hoeveel voedingsstoffen in het lichaam zitten en gezondheidsklachten, is wenselijk om hier meer inzicht in te krijgen en zo nodig maatregelen te kunnen nemen. Dit geldt ook voor vitamines en mineralen waar mensen veel van binnenkrijgen, zoals magnesium. Met gegevens van de VCP kunnen beleidsmakers en professionals werken aan een gezond en, The Dutch National Food Consumption Survey (DNFCS) 2019-2021 sheds light on what Dutch people eat and drink, where and how much. This report delves into the intake of energy and nutrients. Comparing data from over 3500 children and adults, with previous surveys conducted in 2007-2010 and 2012-2016, we examine trends across different age-gender groups. Recent years have shown a positive shift in Dutch diets: on average, there's been a reduction in sugar and salt intake and an increase in fibre consumption compared to the 2012-2016 survey. However, salt intake remains high and fibre intake is still insufficient. Notably, alcohol consumption, particularly among men, has decreased. These trends bode well for health, as less sugar and more fibre intake can aid in preventing overweight and chronic diseases, while low salt intake is important for a healthy blood pressure. Moreover, there's been an overall increase in vitamin D intake across all age groups. Nevertheless, older adults aged 70-79 often fall short on vitamin D intake. For this age group, it is important to take better note of the recommendation to take vitamin D supplements. Sufficient intake of vitamin D in combination with sufficient calcium reduces the risk of bone fractures. There are still other nutrients that people consume either too much or too little compared to dietary reference values. Some population groups have low intake of vitamins A, B2, B6, B11, C, calcium and iron. This varies by age group and gender. However, a deficiency in a nutrient may not immediately warrant concern. Further investigation, including assessing nutrient levels in the body and prevalence of clinical signs, is necessary to understand these patterns better and take measures if needed. This also applies to vitamins and minerals like magnesium, of which some individuals have high intakes. - Abstract
BACKGROUND: Central and bridge nodes can drive significant overall improvements within their respective networks. We aimed to identify them in 16 prevalent chronic diseases during the coronavirus disease 2019 (COVID-19) pandemic to guide effective intervention strategies and appropriate resource allocation for most significant holistic lifestyle and health improvements. METHODS: We surveyed 16 512 adults from July 2020 to August 2021 in 30 territories. Participants self-reported their medical histories and the perceived impact of COVID-19 on 18 lifestyle factors and 13 health outcomes. For each disease subgroup, we generated lifestyle, health outcome, and bridge networks. Variables with the highest centrality indices in each were identified central or bridge. We validated these networks using nonparametric and case-dropping subset bootstrapping and confirmed central and bridge variables' significantly higher indices through a centrality difference test. FINDINGS: Among the 48 networks, 44 were validated (all correlation-stability coefficients >0.25). Six central lifestyle factors were identified: less consumption of snacks (for the chronic disease: anxiety), less sugary drinks (cancer, gastric ulcer, hypertension, insomnia, and pre-diabetes), less smoking tobacco (chronic obstructive pulmonary disease), frequency of exercise (depression and fatty liver disease), duration of exercise (irritable bowel syndrome), and overall amount of exercise (autoimmune disease, diabetes, eczema, heart attack, and high cholesterol). Two central health outcomes emerged: less emotional distress (chronic obstructive pulmonary disease, eczema, fatty liver disease, gastric ulcer, heart attack, high cholesterol, hypertension, insomnia, and pre-diabetes) and quality of life (anxiety, autoimmune disease, cancer, depression, diabetes, and irritable bowel syndrome). 26. A compact single radiator CRLH-inspired circularly polarized leaky-wave antenna based on substrate-integrated waveguide

A circularly polarized (CP) CRLH-inspired leaky-wave antenna (LWA) based on substrate-integrated waveguide (SIW) and microstrip delay line is presented. The unit-cell of the proposed periodic antenna consists of two conventional CRLH SIW cells and two quarter-wavelength microstrip lines. The interdigital slots on the CRLH SIW are rotated at +45° and -45° with respect to the wave propagation direction and separated by 90° to create circular polarization. Utilizing the microstrip line instead of the SIW delay section, the total size of the five unit-cell antenna is reduced by 24.4% in comparison to the previous single radiator CP LWA. Furthermore, microstrip lines provide improved matching for broadside radiation as well as better CP purity. The unitcell characteristics are investigated in detail through dispersion and Bloch impedance analysis. In addition, space harmonic analysis is carried out to explain the radiation mechanism of the proposed antenna. The proposed CRLH-inspired SIW CP antenna operates from 4.2 to 4.85 GHz and frequency scans from -25° to 26° along the elevation angle. Axial ratio is maintained below 3 dB throughout the operating frequency. The measured results show good agreement with the simulated results.

In this letter, a novel dipole antenna based on substrate integrated waveguides (SIWs) is presented. The dipole antenna radiates through the equivalent magnetic current loops on the top and bottom annular ring slot that is excited by the fundamental mode of the SIW cavity. Therefore, the proposed dipole possesses a vertical polarization perpendicular to the metallic plane. Moreover, thanks to normal electric field, the proposed dipole is less affected by the ground plane size and surrounding metal. The proposed antenna topology provides a very easy way to realize vertical dipole antenna by just using planar circuits and is promising to be used as a fundamental element to realize antenna arrays. © 2002-2011 IEEE.

A comprehensive method to synthesize negative group delay (NGD) in the microwave regime with a maximally flat response is proposed in this paper. This method is based on transversal-filter topologies; it will be shown that by choosing proper coupling coefficients of each tap of a transversal filter, we can realize NGDs with maximally flat characteristics at the output of the transversal filter. The desired coefficients to realize maximally flat NGDs with various amount of group delay are analytically derived and tabulated in this paper. Furthermore, the results are verified experimentally through microwave transversal-filter approaches in both passive and active ways using multi-section asymmetric directional couplers and distributed amplifiers. © 1963-2012 IEEE.

Several types of multiplexers based on isolation circuits have been introduced and investigated. Combining method of two filtering circuits (CMTC) is one way to make multiplexers based on isolation circuits. This method fits well for designing contiguous channel triplexers. A triplexer based on CMTC consists of a conventional transmission line (TL) connected to a diplexer and a composite right/left-handed (CRLH) TL connected to a filter. The triplexer based on CMTC has two significant advantages. One is that it is not necessary to modify the design of a stand-alone filter and diplexer and there is freedom in the choice of filtering circuits. In addition, a designer does not need to perform three-dimensional full-wave optimization, because of a simple and straightforward design concept. In this paper, a triplexer prototype having 1, 1.125, and 1.25 GHz center frequencies, is designed and fabricated. In this letter, a novel reconfigurable negative group delay distributed amplifier (NGD-DA) circuit is proposed. This NGD-DA offers a new method to realize negative group delay. By properly choosing the gain coefficient of each tap for the DA, we can synthesize the negative group delay at the reverse port of the DA in the frequency band of interest, while maintaining a flat gain at the forward port; this results in a dual-purpose DA. Moreover, the proposed NGD-DA is reconfigurable; the phase response at the reverse port can be simply controlled by adjusting the tap gain coefficients, and therefore the bandwidth as well as the amount of negative group delay can be manipulated. The proposed active NGD circuit is validated through theoretical explanation and experimental results. © 2013 IEEE.

De voedselomgeving bepaalt voor een groot deel wat mensen kopen, en dus of mensen gezond of ongezond eten. De voedselomgeving is alles wat kan bepalen wat we eten: van aanbod, prijzen, aanbiedingen, informatie en verkooppunten, tot sociale normen, culturele eetgewoonten en wettelijke afspraken. Ongezonde voeding vergroot de kans op overgewicht en chronische ziekten als hart- en vaatziekten en diabetes type 2. De Nederlandse overheid werkt daarom aan een voedselomgeving die mensen helpt om gezondere keuzes te maken. Om beleid te kunnen maken, is inzicht nodig in de verschillende onderdelen van de voedselomgeving. Het ministerie van VWS(Ministerie van Volksgezondheid, Welzijn en Sport ) wil daarom onder andere inventariseren hoe de verkooppunten van voedsel in kaart kunnen worden gebracht. Hieronder vallen alle verkooppunten van alle soorten voedsel, dus van groenteboer en supermarkt tot restaurant en snackbar. Het RIVM geeft voorbeelden hoe de verkooppunten in kaart kunnen worden gebracht (indicatoren) en welke informatie (databronnen) hierover in Nederland beschikbaar is. De twee belangrijkste indicatoren zijn: de ‘dichtheid’ en ‘nabijheid’ van verkooppunten. Met dichtheid bedoelen we hoeveel verkooppunten er in een bepaald gebied zijn, bijvoorbeeld hoeveel verkooppunten in een wijk. Bij nabijheid gaat het erom hoe ver een verkooppunt ligt van bijvoorbeeld een school of woonwijk. Om te kunnen bepalen welke indicatoren en databronnen te gebruiken zijn, moet eerst het doel van de monitor van de hele voedselomgeving duidelijk worden. Zo moet worden bepaald of de monitor nodig is om beleid over de voedselomgeving op landelijk niveau te ontwikkelen. Of juist om beleid over specifieke vraagstukken voor een gemeente te evalueren. Ook kan de mate waarin het voedselaanbod gezond en duurzaam is onderdeel van de monitor zijn. Het monitoren van de verkooppunten kan onderdeel zijn van een brede monitor van de voedselomgeving.

OBJECTIVE: Chronic suppurative otitis media is a major global disease disproportionately affecting low- and middle-income countries, but few studies have explored access to care for those with ear and hearing disorders. METHOD: In a tertiary hospital in Cambodia providing specialist ear services, a mixed method study was undertaken. This study had three arms: (1) quantitative analysis of patients undergoing ear surgery, (2) a questionnaire survey and (3) semi-structured in-depth interviews. RESULTS: Patients presented with advanced middle-ear disease and associated hearing loss at rates that are amongst the highest per capita levels globally. Patients reported several structural, financial and socio-cultural barriers to treatment. This study showed a significant burden of ear disease in Cambodia, which reflects a delay in receiving timely and effective treatment. CONCLUSION: This study highlights the opportunity to integrate effective ear and hearing care into primary care service provision, strengthening the package of activities delivered at government facilities.

- Abstract
RIVM rapport:The Dutch National Food Consumption Survey (DNFCS) 2019-2021 shows what Dutch people eat and drink, and how much. The DNFCS also shows the level of adherence to the Dutch dietary guidelines and changes to this level since the previous surveys in 2012-2016 and 2007- 2010. In recent years, the Dutch have been eating more healthily. One of the reasons for this is that they have been eating more vegetable products, such as vegetables, fruit, unsalted nuts and legumes. At the same time, they have been consuming less red and/or processed meat and sugarcontaining beverages. Most improvements are apparent in both adults and children. People have also been using more dietary supplements, including supplements with vitamin D. In spite of this, the diets of most Dutch people do not comply with the Dutch dietary guidelines. Generally speaking, the diets of women, those aged over 65 and those educated to a higher level follow the Dutch dietary guidelines more closely than the diets of men, young adults and those with a lower level of education. These guidelines form the basis for the Netherlands Nutrition Centre’s Wheel of Five. A healthier diet can help prevent overweight and chronic diseases, such as cardiovascular disease. For this edition of the survey, RIVM researched the diets of over 3,500 children and adults. The data make it possible for policymakers and professionals to continue their work on the development of healthy and sustainable diets, product innovation, education and research. This report describes and discusses the survey method and results in detail. It is intended for a target audience of national and international scientists and professionals. The results are available on www.wateetnederland.nl. Results on the quantities of nutrients that the Dutch ingest will be published later in 2023., De Nationale Voedselconsumptiepeiling 2019-2021 (VCP(Voedselconsumptiepeiling)) laat zien hoeveel en wat mensen in Nederland eten en drinken. De VCP maakt ook duidelijk of zij de richtlijnen Goede voeding volgen en wat daarin is veranderd sinds de vorige peilingen in 2012-2016 en 2007-2010. De afgelopen jaren zijn Nederlanders gezonder gaan eten. Dat komt onder andere omdat ze meer plantaardige producten zoals groente, fruit, ongezouten noten en peulvruchten eten. Ook eten ze minder rood en bewerkt vlees en drinken ze minder suikerhoudende dranken. De meeste verbeteringen zijn te zien bij zowel de volwassenen als de kinderen. Verder zijn mensen meer voedingssupplementen gaan gebruiken, zoals vitamine D. Toch eten de meeste Nederlanders niet volgens de Richtlijnen Goede voeding. Over het algemeen volgen vrouwen, 65-plussers en hoger opgeleiden beter de Richtlijnen voor gezonde voeding dan mannen, jongvolwassenen en lager opgeleiden. Deze richtlijnen zijn de basis voor de Schijf van Vijf van het Voedingscentrum. Een gezondere voeding kan helpen om overgewicht en chronische ziekten als hart- en vaatziekten te voorkomen. Het RIVM bracht dit keer het voedingspatroon met ruim 3500 kinderen en volwassenen in kaart. Met deze gegevens kunnen beleidsmakers en professionals werken aan een gezond en duurzaam voedingspatroon, productinnovatie, voorlichting en onderzoek. Dit rapport beschrijft en bediscussieert de onderzoeksmethode en resultaten in detail. Het is bedoeld voor (inter)nationale wetenschappers en professionals. - Abstract
BACKGROUND: The health area being greatest impacted by coronavirus disease 2019 (COVID-19) and residents' perspective to better prepare for future pandemic remain unknown. We aimed to assess and make cross-country and cross-region comparisons of the global impacts of COVID-19 and preparation preferences of pandemic. METHODS: We recruited adults in 30 countries covering all World Health Organization (WHO) regions from July 2020 to August 2021. 5 Likert-point scales were used to measure their perceived change in 32 aspects due to COVID-19 (-2 = substantially reduced to 2 = substantially increased) and perceived importance of 13 preparations (1 = not important to 5 = extremely important). Samples were stratified by age and gender in the corresponding countries. Multidimensional preference analysis displays disparities between 30 countries, WHO regions, economic development levels, and COVID-19 severity levels. RESULTS: 16 512 adults participated, with 10 351 females. Among 32 aspects of impact, the most affected were having a meal at home (mean (m) = 0.84, standard error (SE) = 0.01), cooking at home (m = 0.78, SE = 0.01), social activities (m = -0.68, SE = 0.01), duration of screen time (m = 0.67, SE = 0.01), and duration of sitting (m = 0.59, SE = 0.01). Alcohol (m = -0.36, SE = 0.01) and tobacco (m = -0.38, SE = 0.01) consumption declined moderately. Among 13 preparations, respondents rated medicine delivery (m = 3.50, SE = 0.01), getting prescribed medicine in a hospital visit / follow-up in a community pharmacy (m = 3.37, SE = 0.01), and online shopping (m = 3.33, SE = 0.02) as the most important. The multidimensional preference analysis showed the European Region, Region of the Americas, Western Pacific Region and countries with a high-income level or medium to high COVID-19 severity were more adversely impacted on sitting and screen time duration and social activities, whereas other regions and countries experienced more cooking and eating at home. - Abstract
BACKGROUND: The interconnected nature of lifestyles and interim health outcomes implies the presence of the central lifestyle, central interim health outcome and bridge lifestyle, which are yet to be determined. Modifying these factors holds immense potential for substantial positive changes across all aspects of health and lifestyles. We aimed to identify these factors from a pool of 18 lifestyle factors and 13 interim health outcomes while investigating potential gender and occupation differences. METHODS: An international cross-sectional study was conducted in 30 countries across six World Health Organization regions from July 2020 to August 2021, with 16 512 adults self-reporting changes in 18 lifestyle factors and 13 interim health outcomes since the pandemic. RESULTS: Three networks were computed and tested. The central variables decided by the expected influence centrality were consumption of fruits and vegetables (centrality = 0.98) jointly with less sugary drinks (centrality = 0.93) in the lifestyles network; and quality of life (centrality = 1.00) co-dominant (centrality = 1.00) with less emotional distress in the interim health outcomes network. The overall amount of exercise had the highest bridge expected influence centrality in the bridge network (centrality = 0.51). No significant differences were found in the network global strength or the centrality of the aforementioned key variables within each network between males and females or health workers and non-health workers (all P-values >0.05 after Holm-Bonferroni correction). CONCLUSIONS: Consumption of fruits and vegetables, sugary drinks, quality of life, emotional distress, and the overall amount of exercise are key intervention components for improving overall lifestyle, overall health and overall health via lifestyle in the general population, respectively. - Abstract
BACKGROUND: Retinoblastoma is the most common intraocular cancer worldwide. There is some evidence to suggest that major differences exist in treatment outcomes for children with retinoblastoma from different regions, but these differences have not been assessed on a global scale. We aimed to report 3-year outcomes for children with retinoblastoma globally and to investigate factors associated with survival. METHODS: We did a prospective cluster-based analysis of treatment-naive patients with retinoblastoma who were diagnosed between Jan 1, 2017, and Dec 31, 2017, then treated and followed up for 3 years. Patients were recruited from 260 specialised treatment centres worldwide. Data were obtained from participating centres on primary and additional treatments, duration of follow-up, metastasis, eye globe salvage, and survival outcome. We analysed time to death and time to enucleation with Cox regression models. FINDINGS: The cohort included 4064 children from 149 countries. The median age at diagnosis was 23·2 months (IQR 11·0-36·5). Extraocular tumour spread (cT4 of the cTNMH classification) at diagnosis was reported in five (0·8%) of 636 children from high-income countries, 55 (5·4%) of 1027 children from upper-middle-income countries, 342 (19·7%) of 1738 children from lower-middle-income countries, and 196 (42·9%) of 457 children from low-income countries. Enucleation surgery was available for all children and intravenous chemotherapy was available for 4014 (98·8%) of 4064 children. The 3-year survival rate was 99·5% (95% CI 98·8-100·0) for children from high-income countries, 91·2% (89·5-93·0) for children from upper-middle-income countries, 80·3% (78·3-82·3) for children from lower-middle-income countries, and 57·3% (52·1-63·0) for children from low-income countries.
