When their treatment is not optimal, deep cervical burns often develop retractions, ranging between simple vertical retractions to major global ones. They generate functional, aesthetic and psychological sequelae. Surgical treatment, adapted to both the patient and the type of retraction, has several goals including restoring the neck-chin angle, restoring a correct aesthetic aspect, and allowing social rehabilitation. In our unit, we managed 47 patients (mean age 22) with neck contractures over 8 years, from 2012- 2020. Scald is more frequent in children while flame burns prevail in adults. Most (58%) patients healed spontaneously. Moderate and severe contractures (Achauer's classification) accounted for 30.6 and 38.8% respectively. In most (94%) patients, the contracture leads to a distortion of the neck-chin angle and limits the extension of the neck. Surgery techniques were skin grafts (67.3%), local plasties (24.5%) and flaps (8.2%). During the follow-up, results were considered as good in 83%, acceptable in 8.5%, while 8.5% needed another surgery. Plasties (Z, IC, VY...) and local flaps (with or without skin expansions) are indicated in minor contractures. Authors still debate on the best technique (graft or flap) for severe and major ones. Post-operative rehabilitation is a cornerstone for good results. [ABSTRACT FROM AUTHOR]

L'expansion cutanée est un outil clé dans l'arsenal thérapeutique proposé pour la réparation des pertes de substances de l'extrémité céphalique. Cette technique aussi intéressante soit-elle est non démunie de risques. Le but de ce travail est l'identification des différents maillons de la procédure d'expansion cutanée, de déterminer les facteurs à risque de complications inhérents à chaque phase de l'expansion afin d'optimiser les résultats fonctionnels et esthétiques. Nous avons réalisé une étude rétrospective sur l'expansion de l'extrémité céphalique sur une période de 10 ans s'étalant de janvier 2009 à décembre 2018 avec au moins un an de suivi pour chaque patient. Notre étude a compté la pose de 98 prothèses chez 63 patients, nous avons tenu compte des différentes variables qui interviennent dans le processus d'expansion afin de déterminer celles qui augmentent le risque de complications. Dans notre série nous avons compté 66,3 % de complications et 22,4 % d'échec. Nous avons trouvé qu'un jeune âge a exposé à un plus grand risque de souffrance cutanée, que le tabac à celui de la survenue d'hématome. L'expansion cervicale est la plus pourvoyeuse de complications suivie du scalp qui augmente le risque d'exposition surtout au niveau pariétal. La face a été retrouvée comme un site favorable à l'expansion. Nous avons trouvé qu'il avait moins de risque pour une expansion multiple dans le même temps opératoire plutôt que les expansions itératives et le choix de prothèses volumineuses. L'expansion au niveau de l'extrémité céphalique nécessite une planification rigoureuse en tenant compte de ses différents intervenants. Tissue expansion is a key tool in the therapeutic arsenal that is proposed for repairing soft tissue losses of the head and neck. This technique, however interesting it may be, is not without its risks. The aim of this work is to identify the different steps of this procedure, determine the complications risk factors inherent to each step of the expansion and propose recommendations to improve functional and aesthetic results. We carried out a retrospective study on tissue expansion of the head and neck over a period of 10 years extending from January 2009 to December 2018, with at least one year follow-up for each patient. Our study counted 63 patients with the placement of 98 prostheses. We considered the various variables involved in the expansion process in order to determine those that increase the risk of complications. In our series we counted 66.3 % of complications and 22.4 % of failures. We found that a young age increased the risk of skin suffering and that smoking increased the risk of hematoma occurrence. Neck expansion was found to be the riskiest site of expansion followed by the scalp that increased the risk of exposure (especially the parietal section of the scalp). The face is considered as an interesting site for expansion. We found that the use of multiple expanders is an attractive alternative to iterative expansions and to the choice of large expanders. [ES] Aunque la actividad extraescolar aparece en las leyes educativas a partir de 1985, a finales de la década de los setenta un grupo de alumnos de COU publica una antología literaria, confeccionada fuera del horario escolar durante su último año académico, en las dependencias del IES Besaya (Torrelavega, Cantabria) con los medios que el instituto les ofrecía. Esa promoción de estudiantes plasmó sus sentimientos y sus ideas sobre los profundos cambios sociales, políticos, culturales y existenciales que vivieron. [EN] Altough education laws begin to consider extracurricular activity since 1985, in the seventies a group of senior students published an anthology of literature, created and edited during many afternoons of their last year, in a room of Besaya High School, and with de machines and materials that the high school offered them. Those students printed their feelings and ideas about the deep social, political, cultural and vital changes they lived.

Significance Statement Congenital obstructive uropathy (COU) is a prevalent human developmental defect with highly heterogeneous clinical presentations and outcomes. Genetics may refine diagnosis, prognosis, and treatment, but the genomic architecture of COU is largely unknown. Comprehensive genomic screening study of 733 cases with three distinct COU subphenotypes revealed disease etiology in 10.0% of them. We detected no significant differences in the overall diagnostic yield among COU subphenotypes, with characteristic variable expressivity of several mutant genes. Our findings therefore may legitimize a genetic first diagnostic approach for COU, especially when burdening clinical and imaging characterization is not complete or available. Background Congenital obstructive uropathy (COU) is a common cause of developmental defects of the urinary tract, with heterogeneous clinical presentation and outcome. Genetic analysis has the potential to elucidate the underlying diagnosis and help risk stratification. Methods We performed a comprehensive genomic screen of 733 independent COU cases, which consisted of individuals with ureteropelvic junction obstruction (n=321), ureterovesical junction obstruction/congenital megaureter (n=178), and COU not otherwise specified (COU-NOS ; n=234). Results We identified pathogenic single nucleotide variants (SNVs) in 53 (7.2%) cases and genomic disorders (GDs) in 23 (3.1%) cases. We detected no significant differences in the overall diagnostic yield between COU sub-phenotypes, and pathogenic SNVs in several genes were associated to any of the three categories. Hence, although COU may appear phenotypically heterogeneous, COU phenotypes are likely to share common molecular bases. On the other hand, mutations in TNXB were more often identified in COU-NOS cases, demonstrating the diagnostic challenge in discriminating COU from hydronephrosis secondary to vesicoureteral reflux, particularly when diagnostic imaging is incomplete. Pathogenic SNVs in only six genes were found in more than one individual, supporting high genetic heterogeneity. Finally, convergence between data on SNVs and GDs suggest MYH11 as a dosage-sensitive gene possibly correlating with severity of COU. Conclusions We established a genomic diagnosis in 10.0% of COU individuals. The findings underscore the urgent need to identify novel genetic susceptibility factors to COU to better define the natural history of the remaining 90% of cases without a molecular diagnosis.

6. Effect of whole body vibration on cervical (neck) proprioception in young, healthy individuals serving as their own control: a pilot study.

Objective: The objective of this pilot study is to determine the effects of whole body vibration on head repositioning accuracy. Methods: Twenty-one participants had a bicycle helmet with an attached laser pointer placed on their heads while standing on a vibration platform. After aligning the laser beam to their determined neutral position on wall-mounted chart paper, they were instructed to close their eyes, flex their neck maximally then return to their perceived neutral position. The point where the laser beam stopped as close to the neutral position as possible was marked on the chart and the sequence was repeated for extension, left and right rotation and left and right lateral flexion. The vibration platform was then activated and the process was repeated for the same six neck movements. Results: T-tests showed significant differences (p < 0.01) for head repositioning errors between normal and vibration data for all neck movements (in mm), except flexion, with vibration trials exhibiting greater re-positioning errors. Data tabulated from the four Cartesian quadrants demonstrated a preponderance of overshoot re-positioning errors in which the laser stopped in individual Cartesian quadrants for each movement and each subject. Conclusions: Whole body vibration contributes to greater head repositioning errors in young, healthy, asymptomatic individuals. Larger scale trials should establish a normal data base for head re-positioning with vibration. Future studies might investigate the relationship between whole body vibration on neck proprioception as an indicator of therapeutic efficacy in neck disorders. [ABSTRACT FROM AUTHOR]

7. Kyste thymique: étiologie rare d'un kyste cervical de l'adulte

Les kystes thymiques sont des tumeurs congénitales rares, présentant 1% des masses cervicales kystiques de l'enfant. Ces kystes sont extrêmement rares chez l'adulte. Nous rapportons le cas d'une femme âgée de 50 ans, sans antécédents particuliers, qui a consulté pour une tuméfaction basi-cervicale antérieure évoluant progressivement depuis 2 ans avec une dyspnée d'effort. A l'examen, elle présentait une tuméfaction basi-cervicale antérieure faisant 4 * 4cm de diamètre, molle, non mobile à la déglutition. L'échographie cervicale a montré une formation kystique ovalaire sous cutanée du creux sus sternal plongeant au niveau du médiastin de 4 * 2.5 * 3cm de dimensions. Le complément par une TDM cervico-thoracique a montré une masse hypodense cervico-médiastinale médiane venant au contact des troncs supra aortiques, refoulant la thyroïde en haut et se développant en bas vers la loge thymique, mesurant 46 * 64 * 58mm. L'exploration chirurgicale par cervicotomie a trouvé une masse kystique tendue bien encapsulée de la loge thyroïdienne étendue au médiastin antérieur, à contenu jaune citrin. La masse était extirpée complètement par voie cervicale. L'étude histologique de la pièce opératoire était en faveur d'un kyste thymique. Le recul est de 1 an avec une bonne évolution sans récidive. Les kystes thymiques, bien que rares, doivent être évoqués devant toute masse kystique cervicale ou cervico-médiastinale, même de l'adulte. La TDM, l'IRM et la cytoponction préopératoires sont utiles, mais la confirmation diagnostique nécessite l'identification du tissu thymique contenant les corpuscules de Hassall à l'examen anatomopathologique. Le traitement est chirurgical avec une exérèse complète de la paroi kystique afin d'éviter les récidives.

8. Patient-centered management strategy for concurrent musculoskeletal complaints in elbows, shoulders, and neck after an isolated hand/forearm complaint: A prospective interventional study

The aim was to evaluate patient-reported outcomes before and after a patient-centered management strategy targeting concurrent proximal musculoskeletal complaints (MSCs) in patients with an isolated hand/forearm complaint. A prospective interventional study included 66 patients. Intervention targeting concurrent MSCs was implemented as a patient-centered add-on to standard treatment for primary hand/forearm complaints. The patient-centered management strategy included patient education, individualized exercises, and manual therapy. Patient reported outcome measures and pain questionnaires regarding the location, frequency and intensity of pain in hands, elbows, shoulders and neck were collected at baseline, after the last session of the patient-centered management strategy, and at 3 months’ follow-up. There were significant improvements in all patient-reported outcomes between baseline and follow-up. DASH scores improved significantly, by 17 to 29 points on the 3 subscales. There was a significant improvement of 6 points in PCS, 2 points in HADS, and 0.051 points in EQ-5D index. Median pain intensity on VAS decreased from 6 (4–8) to 5 (2.5–7) in hands, 3 (0–6) to 0 (0–3) in elbows, 5 (2–7) to 2.5 (0–5) in shoulders, and 3 (0–6) to 2 (0–3) in the neck, between baseline and discharge. 9. Analyse clinique avant chirurgie de rajeunissement cervico-facial.

Résumé L’examen clinique est la première étape de toute démarche thérapeutique médicale ou chirurgicale. Sa qualité est essentielle afin d’adapter le traitement à la demande du patient. Dans le cadre du rajeunissement facial, l’interrogatoire et la compréhension des motivations, photos anciennes à l’appui, participent à l’évaluation. La connaissance de l’anatomie et du processus de vieillissement facial facilite l’analyse. Il s’agit non seulement de déterminer le degré de ptôse tissulaire, mais aussi d’évaluer les variations volumétriques. Enfin, les tensions musculaires et la qualité cutanée participent à la genèse de rides distinctes. Ces modifications complexes aboutissent à un aspect fatigué ou triste, voire âgé. Les demandes des patients, souvent limitées à une seule région, ne doivent pas dispenser d’une analyse globale. Le rôle du praticien est aussi de faire prendre conscience au patient, avec tact, de l’incidence des autres régions sur l’impression générale. L’objectif sera d’obtenir un résultat harmonieux, naturel qui signe l’excellence du savoir-faire médicochirurgical. L’examen clinique doit, en outre, permettre de repérer les difficultés techniques. Enfin, un bilan photographique, voire vidéo, fixe l’analyse et permet de disposer d’éléments objectifs pour affiner le traitement, aider à sa compréhension et évaluer le résultat. Le temps passé à examiner et expliquer permet de statuer avec le patient sur les choix thérapeutiques et d’en préciser leurs limites. Il permettra ainsi d’accroître la satisfaction du patient qui doit s’approprier le résultat. Cet effort d’analyse est dès lors un acte opératoire, tout aussi important que la technique elle-même. Apporter un résultat « sur mesure », indispensable lorsqu’il s’agit de l’image de la personne, constitue enfin la garantie du plaisir à toujours vouloir améliorer notre pratique. Summary The first step to determine a medical or surgical intervention is the clinical exam. The clinical exam is the paramount step to provide a treatment plan that can be modified and individualized accounting the patient preferences. During the consultation for facial rejuvenation, attention should be paid to understand the patient's motivation and expectations. A thorough understanding of the anatomy and the natural history of ageing will facilitate the analysis of the face. Not only the degree of ptosis but also the degree of volume loss will need to be assessed, as well as the influence of the facial muscles, the skin quality, and the different causes of rythids. The comprehensive age-related anatomical changes are often perceived and described as tiredness or sadness. Patients very commonly only point out a single anatomical region. During the consultation, the surgeon should provide the patient with the information of the comprehensive interplay of the different anatomical regions and their individual ageing process. Obtaining a harmonious, natural appearing outcome is the result of excellent surgical skills and applied knowledge. The clinical exam should also find out traps and technical difficulties. Although standardized photographs allow a static evaluation of one's result, video may deliver additional information about the postoperative result, and may contribute to the understanding of the technique used. Spending the additional time by performing a thorough facial analysis and preoperative planning is well-invested time. Having a good understanding of the possible surgical improvements and limitations will be beneficial for both, the patient and the surgeon. Managing the expectations of the patient and careful preoperative planning will increase patient's satisfaction. At the same time, the surgeon will able to critically assess his/her own result and taking pleasure improving their own technique. [ABSTRACT FROM AUTHOR]

Les lymphangiomes kystiques cervico-thoraciques sont des tumeurs bénignes rares, ils proviendraient d'une séquestration du sac lymphatique embryonnaire qui se remplirait progressivement de liquide lymphatique. Le diagnostic est évoqué par la clinique (tuméfaction latéro-cervicale) et l'imagerie (échographie et tomodensitométrie), puis confirmé par l'histologie après la chirurgie qui constitue la base du traitement. Nous rapportons un cas de lymphangiome kystique cervico-thoracique avec une revue de la littérature.

Néanmoins certaines de ces tuméfactions restent d'origine non précisée, portant donc l'indication d'une cervicotomie exploratrice. Il s'agit d'une étude rétrospective d'une série de 300 cas de tuméfactions cervicales isolées colligées au service d'ORL de l'hôpital militaire Avicenne de Marrakech entre 2001 et 2014. Tous nos patients ont bénéficié d'une cervicotomie exploratrice avec étude anatomo-pathologique. L'âge des patients varie entre 1 et 76 ans avec un âge moyen de 32,57 ans et une légère prédominance masculine de 52%. La symptomatologie qui a motivé une consultation chez 81% des patients était la tuméfaction latérocervicale. La localisation la plus fréquente était sous mandibulaire (33,34%). Les tuméfactions d'installation progressive ont été retrouvées chez 93,34% des patients. Les principales étiologies retrouvées dans notre étude après examen anatomopathologique étaient de deux types: soit d'origine ganglionnaire dominées par la tuberculose ganglionnaire cervicale (53,66%), le lymphome malin non hodgkinien (6 ,66%), les adénites réactionnelles non spécifiques (4,66%), la maladie de hodgkin (4,33%) et les métastases ganglionnaires cervicales (3,33 %); soit d'origine non ganglionnaire dont le lipome cervicale (17,66%), les kystes branchiaux (6%), les kystes du tractus thyréoglosse (1,66%) et le lymphangiome kystique (1,66%). A la lumière des résultats obtenus et des données de la littérature, nous allons discuter l'intérêt et l'utilité de la cervicotomie exploratrice dans le diagnostic étiologique des tuméfactions cervicales lorsque les examens cliniques et paracliniques ne sont pas concluants, et ainsi d'analyser les aspects, épidémiologiques, cliniques et paracliniques des différentes étiologies retrouvées. La cervicotomie exploratrice reste, avec l'étude anatomo-pathologique, un outil nécessaire pour le diagnostic de certitude de certaines tuméfactions cervicales malgré son caractère invasif.

Les séquelles de brûlures cervicales représentent une entité fréquente des séquelles de brûlure, elles affectent la fonction, l'esthétique et l'état psychologique des patients et peuvent être de traitement difficile. Il s'agit d'une étude rétrospective étalée sur 5 ans de Mars 2009 au Octobre2014, réalisée au centre national des brûlés et de chirurgie plastique au CHU Ibn Rochd Casablanca.Nous avons analysé les caractéristiques épidémiologiqueset cliniques ainsi que les indications et les résultats thérapeutiques chez 300 patients présentant des rétractions cervicales post-brûlure, suivis dans notre formation. Les jeunes femmes étaient le plus souvent touchées (56 %).la brûlure thermique par flamme de butane dans le cadre d'accident domestique était l'étiologie la plus fréquente (91%).75% des patients ont été pris en charge dans un délai de 18 mois après avoir présenté une incapacité fonctionnelle.Les brides cervicales modérées et sévères sont les plus fréquentes et représentent respectivement 60% et 16% des cas. Le traitement chirurgical a fait appel aux greffes cutanées dans 67%des cas, aux plasties locales dans 24%des cas et aux lambeaux dans 24% des cas, les résultats sont jugés bons dans 75%des cas et moyens dans 18% des cas, tandis que les cas restants (7%) ont nécessité une reprise chirurgicale. Le traitement des brides cervicales doit être associé à un programme de rééducation adapté, afin d'assurer la pérennité des résultats fonctionnels et esthétiques.

Summary Objective Compare the activation of the cervical and shoulder muscles in the execution of adapted swimming with and without a snorkel. Methods and subjects This study uses cross-sectional, analytical inference analysis. We studied the musculature of the right shoulder and cervical spine of 16 healthy participants. We examined seven muscles with surface electromyography including the cervical erector spinae, trapezius, infraspinatus, pectoralis, anterior deltoid, middle deltoid and latissimus dorsi. Each participant was measured in five complete cycles of adapted swimming. Results There was significantly more activity in the infraspinatus muscle (increased by 18%, P = .020) and significantly less activity in the cervical erector spinae muscle (decreased by 27%, P = .050) when using a snorkel compared to not using a snorkel. Conclusion Using a snorkel during adapted swimming produces changes in the level of muscle activation in the shoulder and neck. An increase in infraspinatus and a decrease in cervical erector spinae activity due to reduced head movement and an absence of significant cervical rotation when using a snorkel may lead to increased resistance and work for the shoulder. Résumé Introduction La ranula est un pseudo-kyste de la glande sublinguale (GSL) de cause souvent inconnue. Rarement, elle peut s’étendre au niveau cervical lorsqu’elle franchit le muscle mylo-hyoïdien. Le diagnostic des ranulas cervicales n’est pas toujours simple à établir, d’autres affections kystiques cervicales pouvant donner le même aspect clinique. Certaines de ces ranulas récidivent après exérèse. L’objectif de cet article était de faire une mise au point sur les ranulas, en centrant notre étude sur les examens paracliniques les plus utiles et sur les traitements les plus appropriés. Matériel et méthodes Une revue de la littérature a été effectuée dans la base de données PubMed ® entre 2010 et 2015 en utilisant les mots clés suivants : « plunging ranula », « recurrent plunging ranula ». Résultats Treize articles ont été sélectionnés. Parmi les 37 cas de ranula à expression cervicale trouvés, 3 ont présenté une récidive. Les examens paracliniques réalisés étaient l’échographie, le scanner et l’IRM. Le traitement était l’exérèse de la GSL parfois associée à une marsupialisation. Discussion L’examen paraclinique de référence est le scanner oro-cervical. Le traitement le plus efficace d’une ranula cervicale est l’exérèse complète de la GSL, sans geste cervical associé. Summary Introduction Ranula is a pseudocyst of the sublingual gland, often of unknown etiology. In few cases, it can extend to the neck when passing through the mylohyoid muscle. Diagnosis is not always easy as other cervical cystic lesions may have the same clinical aspect. Some ranulas recur after removal. The aim of our study was to conduct a review about plunging ranulas, with a focus on the most useful paraclinical exams and the most effective treatment. Material and methods A review of the literature has been conducted in the PubMed database between 2015 and 2010 using following keywords: plunging ranula, recurrent plunging ranula. Results Thirteen articles reporting 37 cases of plunging ranulas have been selected. Among these cases, 3 recurred. Paraclinical exams consisted in US, CT scan and MRI. Main reported treatment was sublingual gland removal, sometime associated with marsupialization. Discussion The most useful paraclinical exam is CT scan. STUDY DESIGN Continuous measurements and computation of absolute metrics of cervical subarachnoid space (CSS) and spinal cord (SC) geometries proposed are based on in vivo magnetic resonance imaging and 3D reconstruction. OBJECTIVE The aim of the study is to offer a new methodology to continuously characterize and to quantify the detailed morphology of the CSS and the cervical SC in 3D for healthy subjects in both neutral supine and flexion. SUMMARY OF BACKGROUND DATA To the best of our knowledge, no study provides a morphological quantification by absolute indices based on the 3D reconstruction of SC and CSS thanks to in vivo magnetic resonance imaging. Moreover, no study provides a continuous description of the geometries. METHODS Absolute indices of SC (cross-sectional area, compression ratio, position in the canal, length) and of CSS (cross-sectional area, occupational ratio, lengths) were computed by measures from 3D semi-automatic reconstructions of high resolution in vivo magnetic resonance images (3D T2-SPACE sequence) on healthy subjects (N = 11) for two postures: supine neutral and flexion neck positions. Abstract: Objective: The purpose of the present experiment was to determine the reliability of the cervicocephalic relocation test (CRT). Methodology: Thirteen young healthy adults were recruited to undergo a CRT test-retest. The test-retest reliability was determined by repeatedly measuring CR to a neutral position after active movement, with trials approximately 1 hour apart. Each CRT involved 20 CRs to a natural head position, 10 repositioning after right head rotation, and 10 repositioning after left head rotation (with the order of rotation sides randomized). The performance score for each subject for each CRT was the mean of the errors made during the 20 relocations (in degrees). Study Results: The mean rate of error of the first and second trials was 3.2±1.1° and 2.9±0.9°, respectively. The intra class correlation coefficient was 0.81, and the standard error of measurement 0.90°. Conclusion: CRT testing supports evidence of reliable cervicocephalic performance stability in healthy young adults. CRT shows promise as a relevant tool of assessment in head and neck proprioception. [Copyright &y& Elsevier]

Despite the recent progresses in occupant safety, protection of children is not still optimal. To offer a better understanding of child injury mechanisms, the present study proposes a human-like finite element model of a 3-year-old child neck. The subject underwent scanning. The images were first segmented semi-automatically in order to extract the soft tissues and the bones. In the second step, we separated the different bones slice by slice on the geometry previously reconstructed. The anatomic structures were identified and each vertebra was reconstructed independently with special attention to the articular process. In the third step, we generated an original meshing on the previous geometry to obtain a finite element model of the child’s neck. The modelled anatomical structures are the head, the seven cervical vertebrae (C1–C7), the first thoracic vertebra (T1), the intervertebral discs and the principal ligaments which were modelled using non-linear shock-absorbing spring elements. The stiffness values used were taken from the literature, and scaled down using scale factors from Irwin. This model incorporates 7,340 shell elements to model the 8 vertebrae, the head and 1,068 solid 8-node elements to model the intervertebral discs. Contact between the articular surfaces is represented by interfaces permitting frictionless movement. Since this study does not aim to reproduce bone fractures, we have modelled the cervical vertebrae as rigid bodies. Given that validation data was not available, the model validation was conduced against crash test dummy component sled tests. The accelerometric responses of the head model were similar with those recorded experimentally with a Q3 dummy neck in backward, frontal and lateral impact direction. [ABSTRACT FROM AUTHOR]

Introduction Isometric manipulation is a current practice in osteopathy and treatment benefits have been reported in the literature. Such benefits could be assessed using experimental non-invasive cervical mobility measurements. The main objective was to quantitatively measure the effects of isometric manipulation on principal and compensatory cervical motions. Methods 101 healthy volunteers were included in this study. 51 healthy volunteers selected randomly underwent the experimental protocol before and after isometric treatment and were compared to 50 healthy volunteers who underwent a placebo treatment. Osteopathic diagnosis was performed on each healthy volunteer before and after the treatment. The experimental protocol included measurements by a motion capture system focusing on principal range of motion and compensatory motions. Results In both the isometric and the placebo sample, respectively including 51 (age: 29.2 ± 8.1, BMI: 22.2 ± 3.5) and 50 healthy volunteers (age: 27.4 ± 6.8, BMI: 22.9 ± 2.8), a pre-treatment diagnosis revealed a light cervical dysfunction in all subjects, mainly in levels C3 and C4. Altered ranges of motion thresholds (C3/C4 alterations) were identified: 113.2° for flexion, 130.0° for rotation and 90.2° for lateral flexion. After manipulations, the volunteers who underwent the isometric treatment presented a slight increase in amplitude for lateral flexion (p \textless 0.04), which was not found in the volunteers who underwent the placebo treatment. Compensatory motions showed differences pre and post isometric treatment without reaching significant values. Conclusion Principal ranges of motion were found significantly higher after osteopathic treatment when compared to the placebo treatment. Osteopathic palpatory diagnosis showed significant correlation with range of motions before treatment.

The number of concussions has indeed been reported to increase in the last decades (Finch et al. 2013). This is mostly due to the anatomy of professional rugby players (muscular development, higher body mass index- BMI).While the rugby union rules have been changing to promote safer practice in scrum, the players get highly injured during tackles (Fuller et al. 2010). Multiple influences on injury risks, such as the impact conditions or the player's morphology, have not been fully studied. In tackling, both the tackler (T) and the ball carrier (BC), their morphology and initial position need to be studied to fully describe the tackle impacts. Experimentally, on field data can be acquired with sensors and epidemiological studies have been carried to correlate the type of injury with the impact conditions (Kuster et al. 2012)

44th Congress of the Société de Biomécanique, Poitiers, France, 28-/10/2019 - 30/10/2019; Task related estimation of the musculo-tendon and intervertebral forces in the neck can help gaining some insights on the role of the central nervous system and in assessing cervical spine disorders. Musculoskeletal models have been developed to solve the muscular redundancy and estimate these forces, generally based on a static equilibrium computed at the level of the C4/ C5 joint, e.g., (Moroney et al. 1988; Van den Abbeele et al. 2018). However, solving the equilibrium of the entire cervical spine system is essential to be able to evaluate its muscular response (including from its numerous multi-level muscles) as well as the loads transmitted to the vertebrae and inter-vertebral discs. This study investigates the distribution of the intervertebral compression and shear forces in the cervical spine during two isometric maximum voluntary contractions (MVC) tasks, toward neck flexion and extension. The forces at the C4/C5 joint and the musculo-tendon forces of 8 muscle groups are compared with the results of two other neck models reported in the literature.

We compared the minute-by-minute muscle activity and oxygenation responses to a repetitive arm motion-induced fatiguing task between the sexes in order to address the literature gap on these time-dependent fatigue responses. Twenty-six (13 females) healthy adults performed a repetitive pointing task (RPT) with the arm moving forward/backward at shoulder height until reaching 8/10 (Borg CR10) for neck/shoulder perceived exertion (RPE). Neck/shoulder RPE, oxygenation and electromyography were recorded every minute and compared between first and second half of the task and between the sexes. Greater changes in oxygen supply and activation amplitude occurred during the second half of the task. Despite similar time to fatigue-terminal ( p > 0.05), females showed greater anterior deltoid activation amplitude at all time points than males, and only the males showed increases in anterior and posterior deltoid activation amplitudes. In females, middle (ρ = -0.34, p = 0.04) and posterior (ρ = -0.44, p = 0.01) deltoid amplitudes were negatively correlated with perceived exertion during the first half of the task. Results suggest that reduced modulation of anterior deltoid activation amplitude in females may reflect a sub-optimal fatigue-mitigation mechanism compared with males and may help explain their greater susceptibility to neck/shoulder musculoskeletal disorders. Novelty: Despite similar fatigability and trapezius oxygenation, females showed greater deltoid activation throughout the task. Deltoid activation increased in males but not in females. The results support the important role of the deltoid in sex-specific neck/shoulder injury mechanisms.

Background Literature is still limited regarding reports of non-invasive assessment of the cervical range of motion in normal subjects. Investigations into compensatory motions, defined as the contribution of an additional direction to the required motion, are also limited. The objectives of this work were to develop and assess a reliable method for measuring the cervical range of motion in order to investigate motion and compensatory strategies. Methods and data collection Ninety-seven no neck-related pain subjects (no severe cervical pathology, 57 women, age: 28.3 ± 7.5y. old, BMI: 22.5 ± 3.2 kg/m2) underwent a non-invasive cervical range of motion assessment protocol. In-vivo head’s motion relative to the thorax was assessed through the measurement of the main angular amplitudes in the 3 directions (flexion/extension, axial rotations and lateral inclinations) and associated compensatory motions using an opto-electronic acquisition system. Results The principal motion reproducibility resulted in intra-class correlation coefficients ranging from 0.81 to 0.86. The following maximum ranges of motion were found: 127.4 ± 15.1° of flexion/extension, 89.3 ± 12° of lateral inclinations and 146.4 ± 13° of axial rotations after 6 outlier exclusions. 30. Visual control of steering in locust flight: the effects of head movement on responses to roll stimuli.

The contribution of head movement to the control of roll responses in flying locusts ( Locusta migratoria) has been examined (i) on a flight balance, recording the angles through which the locust turns when following an artificial horizon; (ii) by recording activity in a pair of flight muscles in restrained conditions; and (iii) by observations on free flying locusts. Responses were compared when the head was free to turn about the thorax, as normal, and when the head was waxed to the thorax, blocking any relative motion between the two ('head-fixed'). These experiments suggest that the major signal generating corrective roll manoeuvres is the visual error between the angle of the head and the horizon, rather than a signal that includes a measure of the head-thorax angle. These data suggest that the reduction in visual inputs caused by compensatory motion of the head during roll manoeuvres is not functionally replaced by inputs from cervical proprioceptors. Les tuméfactions cervicales représentent un motif fréquent de consultation, et les adénopathies en constituent l'étiologie la plus fréquente. L'examen clinique et les bilans paracliniques permettent, dans la majorité des cas de retrouver une étiologie. Néanmoins certaines de ces tuméfactions restent d'origine non précisée, portant donc l'indication d'une cervicotomie exploratrice. Il s'agit d'une étude rétrospective d'une série de 300 cas de tuméfactions cervicales isolées colligées au service d'ORL de l'hôpital militaire Avicenne de Marrakech entre 2001 et 2014. Tous nos patients ont bénéficié d'une cervicotomie exploratrice avec étude anatomo-pathologique. L'âge des patients varie entre 1 et 76 ans avec un âge moyen de 32,57 ans et une légère prédominance masculine de 52%. La symptomatologie qui a motivé une consultation chez 81% des patients était la tuméfaction latérocervicale. La localisation la plus fréquente était sous mandibulaire (33,34%). Les tuméfactions d'installation progressive ont été retrouvées chez 93,34% des patients. Les principales étiologies retrouvées dans notre étude après examen anatomopathologique étaient de deux types: soit d'origine ganglionnaire dominées par la tuberculose ganglionnaire cervicale (53,66%), le lymphome malin non hodgkinien (6 ,66%), les adénites réactionnelles non spécifiques (4,66%), la maladie de hodgkin (4,33%) et les métastases ganglionnaires cervicales (3,33 %); soit d'origine non ganglionnaire dont le lipome cervicale (17,66%), les kystes branchiaux (6%), les kystes du tractus thyréoglosse (1,66%) et le lymphangiome kystique (1,66%). A la lumière des résultats obtenus et des données de la littérature, nous allons discuter l'intérêt et l'utilité de la cervicotomie exploratrice dans le diagnostic étiologique des tuméfactions cervicales lorsque les examens cliniques et paracliniques ne sont pas concluants, et ainsi d'analyser les aspects, épidémiologiques, cliniques et paracliniques des différentes étiologies retrouvées. La cervicotomie exploratrice reste, avec l'étude anatomo-pathologique, un outil nécessaire pour le diagnostic de certitude de certaines tuméfactions cervicales malgré son caractère invasif.

Le diagnostic est évoqué par la clinique (tuméfaction latéro-cervicale) et l'imagerie (échographie et tomodensitométrie), puis confirmé par l'histologie après la chirurgie qui constitue la base du traitement. Nous rapportons un cas de lymphangiome kystique cervico-thoracique avec une revue de la littérature.

Achilles tendon vibration generates proprioceptive information that is incongruent with the actual body position; it alters the perception of body orientation leading to a vibration-induced postural response. When a person is standing freely, vibration of the Achilles tendon shifts the internal representation of the verticality backward thus the vibration-induced postural response realigned the whole body orientation with the shifted subjective vertical. Because utricular otoliths information participates in the creation of the internal representation of the verticality, changing the natural orientation of the head-neck system during Achilles tendon vibration could alter the internal representation of the earth vertical to a greater extent. Consequently, it was hypothesized that compared to neutral head-neck orientation, alteration in the head-neck orientation should impair balance control immediately after Achilles tendon vibration onset or offset (i.e., sensory transition) as accurate perception of the earth vertical is required. Results revealed that balance control impairment was observed only immediately following Achilles tendon vibration offset; both groups with the head-neck either extended or flexed showed larger body sway (i.e., larger root mean square scalar distance between the center of pressure and center of gravity) compared to the group with the neutral head-neck orientation. The fact that balance control was uninfluenced by head-neck orientation immediately following vibration onset suggests the error signal needs to accumulate to a certain threshold before the internal representation of the earth vertical becomes incorrect.

We have designed a microfluidic microreactor chip for electrochemical conversion of analytes, containing a palladium reference electrode and platinum working and counter electrodes. The counter electrode is placed in a separate side-channel on chip to prevent unwanted side-products appearing in the measured spectrograms. Using this chip, cyclic voltammograms are measured in volumes of 9.6 nL. Furthermore the conversion efficiency of ferricyanide is characterized using UV/vis-spectroscopy. We have obtained an on-line conversion of 97% using a flow rate of 1 microL/min. We have used the microreactor chip to study the electrochemical metabolism pathway of amodiaquine using electrochemistry (EC)-liquid chromatography (LC)-mass spectrometry (MS). We have compared our results with measurements obtained with commercially available electrochemical flow-through cells. Using our chip it was possible to obtain similar results. Therefore, we have fabricated an electrochemical cell on-chip which is used successfully in EC-UV/vis and EC-LC-MS experiments.

Cette étude compare les sollicitations musculaires et posturales, la performance et la satisfaction des utilisateurs lors de l’utilisation d’une souris traditionnelle et de deux souris verticales, l’une présentant une inclinaison de 35 degrés et l’autre de 65 degrés par rapport à l’horizontale, à 3 emplacements différents sur le bureau. Sept tâches, habituellement effectuées lors de recherche d'informations sur écran ou en conception assistée par ordinateur, ont été réalisées. La souris présentant une inclinaison de 35 degrés apparait comme le meilleur compromis en termes de sollicitations musculaires et posturales, de performance et de satisfaction des utilisateurs. Placer la souris devant le clavier, plutôt qu’à côté du clavier, réduit les sollicitations musculaires. La performance et la satisfaction des utilisateurs sont meilleures lorsque la souris est placée librement, sans contrainte, sur le plan de travail. Cependant, cette position libre de la souris sur le plan de travail nécessite l’absence de clavier ce qui se rencontre peu dans le quotidien. La tâche de reprise de la souris après frappe sur le clavier est plus sollicitante que les tâches de pointage, cliquage ou déplacement de cibles.

This work focuses on the microstructure evolution of a TiAl alloy during sintering by a process called Spark Plasma Sintering (SPS). The initial powders, elaborated by atomization, consist primarily of a metastable phase. The transformations of the return to equilibrium of the latter during heating are finely characterized using SEM, TEM and EBSD. These phase transformations are then used as a thermal indicator during the SPS densification to estimate the thermal and mechanical gradients. The densification mechanisms responsible for the neck formation and the origins of the microstructure heterogeneities of fully densified samples are discussed. A comparison between the densification mechanisms and the final microstructures of a milled powder and a no milled powder is showed. Finally, the effect of the application of a dynamic stress during the compaction using an original process is presented; Ce travail porte sur l'évolution microstructurale d'un alliage TiAl lors du frittage par un procédé appelé Spark Plasma Sintering (SPS). Les poudres initiales, élaborées par atomisation, sont constituées principalement d'une phase métastable. Les transformations qui accompagnent le retour à l'équilibre de cette dernière durant un chauffage sont finement caractérisées par MEB, MET et EBSD. Ces transformations seront ensuite utilisées comme marqueur thermique lors de la densification SPS afin de mieux estimer les amplitudes des gradients thermiques et mécaniques du procédé de frittage. Les mécanismes de densification responsables de la formation des cous sont discutés, ainsi que les origines des hétérogénéités microstructurales des échantillons complètement densifiés. Un comparatif des mécanismes de densification et des microstructures finales entre une poudre broyée et une poudre non broyée est dressé. Enfin, l'influence de l'application d'une contrainte dynamique pendant la compaction au moyen d'un dispositif original est présentée

Les séquelles de brûlures cervicales représentent une entité fréquente des séquelles de brûlure, elles affectent la fonction, l'esthétique et l’état psychologique des patients et peuvent être de traitement difficile. Il s'agit d'une étude rétrospective étalée sur 5 ans de Mars 2009 au Octobre2014, réalisée au centre national des brûlés et de chirurgie plastique au CHU Ibn Rochd Casablanca. Nous avons analysé les caractéristiques épidémiologiqueset cliniques ainsi que les indications et les résultats thérapeutiques chez 300 patients présentant des rétractions cervicales post-brûlure, suivis dans notre formation. Les jeunes femmes étaient le plus souvent touchées (56%). la brûlure thermique par flamme de butane dans le cadre d'accident domestique était l’étiologie la plus fréquente (91%).75% des patients ont été pris en charge dans un délai de 18 mois après avoir présenté une incapacité fonctionnelle. Les brides cervicales modérées et sévères sont les plus fréquentes et représentent respectivement 60% et 16% des cas. Le traitement chirurgical a fait appel aux greffes cutanées dans 67%des cas, aux plasties locales dans 24%des cas et aux lambeaux dans 24% des cas, les résultats sont jugés bons dans 75%des cas et moyens dans 18% des cas, tandis que les cas restants (7%) ont nécessité une reprise chirurgicale. Le traitement des brides cervicales doit être associé à un programme de rééducation adapté, afin d'assurer la pérennité des résultats fonctionnels et esthétiques.

Introduction : Faute de tests diagnostiques précis, une étude histologique est souvent nécessaire pour diagnostiquer les tuméfactions latérales solides cervicales (TLSC) chez l’enfant. Nous étudierons les modalités diagnostiques pour les TLSC afin de créer une approche diagnostique standardisée intégrant de nouveaux outils diagnostics à ceux actuellement offerts. Méthodologie : Après révision des étiologies et des modalités diagnostiques, une revue de la littérature a été effectuée. Une étude rétrospective entre 2002 à 2012 est présentée suivie d’une étude de faisabilité de la cytoponction. Puis, un arbre décisionnel est créé basé sur nos résultats et sur l’avis d’un groupe d’experts de différentes disciplines médicales. Résultats : Le diagnostic différentiel des TLSC est varié, la littérature scientifique est désuète et la comparaison reste difficile. Pour nos 42 enfants avec un âge médian de sept ans, les tuméfactions inflammatoires représentent 59% (26/44 biopsies) des TLSC, surtout des lymphadénites à mycobactérie atypique (13/26) qui ont un dépistage ardu et multimodal. La biopsie fut peu contributive à la prise en charge dans 39% (17/44) des cas. La cytoponction sous échoguidance est une technique diagnostique faisable et moins invasive que la biopsie. L’arbre décisionnel offre aux cliniciens une approche diagnostique standardisée des TLSC appuyée sur des faits scientifiques que nous souhaitons valider par une étude prospective. Conclusion : Les TLSC chez l’enfant représentent un défi diagnostic et notre arbre décisionnel répond au manque de standardisation dans l’approche diagnostique. Une étude prospective sur notre arbre décisionnel est en voie d’acceptation au CHU Sainte-Justine., Introduction: Histological studies are often required to establish the etiology of solid lateral cervical masses (SLCM) in children since diagnostic tools are lacking accuracy. We evaluated the diagnostic tools available in order to develop a systematic and standardized diagnostic approach. Methods: After etiologies and diagnostic tools were described, a review of the scientific literature on the topic was performed. A retrospective study between 2002 and 2012 of SLCM was conducted. A case report showing the feasibility of Fine Needle Aspiration (FNA) is presented. Finally, a decision algorithm integrating our results and the scientific opinion of an expert group is proposed. Results: Differential diagnosis for SLCM is wide. The scientific literature regarding their diagnosis is weak, outdated and comparison is difficult. The 42 children studied had a median age of seven year old. They had inflammatory masses in 59% (26/44 biopsies) with mostly Atypical Mycobacteria (13/26), which has been shown to be difficult to confirm and often requires multiple diagnostic tests. The benefit of biopsy is questionable in 39% (17/44) of biopsies. Ultrasound-guided FNA appears to be a feasible technique and less invasive than biopsy. The algorithm proposes a standardized approach based on scientific facts that we want to evaluate through a prospective study. Conclusion: SLCM in children represent a diagnostic challenge and the algorithm intends to guide the clinician’s diagnostic approach. A prospective study on the algorithm created is at its final step for acceptance through the CHU Sainte-Justine authorities.

The objective of this thesis, as part of the DEMU2NECK European project, was to contribute to develop a 3D, deformable model of the neck, with contractile muscles and including the possibility of patient-specific geometric personalisation. The aim of this model is to contribute to a better understanding of the links that exist between pathologies, cervical spine posture and muscular activation in order to help clinicians and medical devices manufacturers in their decision making process. To achieve this goal, the thesis work was divided into four main tasks: after a i) bibliographic synthesis, ii) a passive generic finite element model was developed and validated. The third step consisted in iii) the development of a finite element muscle model and its integration to the generic passive model. The contractile function of the muscles was implemented during isometric and dynamic simulations of simple functional tasks of the neck. The transverse forces that were transmitted from the muscles to the cervical spine are in good agreement with the hypothesis of a contribution of these forces to the cervical spine stability. The last task covered iv) the personalisation process of the generic model. The responses of subject-specific models based on volunteers were compared with the ones obtained from the generic model. They showed significant differences and, therefore, the scientific relevance of the personalisation approach., L'objectif du travail de thèse, dans le cadre du projet Européen DEMU2NECK, a été de contribuer à développer un modèle volumique déformable et personnalisable du cou, incluant la prise en compte de la capacité d'activation musculaire.
