Rogue waves are intense and unexpected wavepackets ubiquitous in complex systems. In optics, they are promising as robust and noise-resistant beams for probing and manipulating the underlying material. Localizing large optical power is crucial especially in biomedical systems, where, however, extremely intense beams have not yet been observed. We here discover that tumor-cell spheroids manifest optical rogue waves when illuminated by randomly modulated laser beams. The intensity of light transmitted through bio-printed three-dimensional tumor models follows a signature Weibull statistical distribution, where extreme events correspond to spatially-localized optical modes propagating within the cell network. Experiments varying the input beam power and size indicate the rogue waves have a nonlinear origin. We show these optical filaments form high-transmission channels with enhanced transmission. They deliver large optical power through the tumor spheroid, which can be exploited to achieve a local temperature increase controlled by the input wave shape. Our findings shed new light on optical propagation in biological aggregates and demonstrate how extreme event formation allows light concentration in deep tissues, paving the way to using rogue waves in biomedical applications such as light-activated therapies.

BackgroundAlthough Black/African American older adults bear significant inequities in prevalence, incidence, and outcomes of Alzheimer's disease and related dementias, they are profoundly under-included in Alzheimer's Disease research. Community-Engaged Research (e.g., equitable community/science partnerships) is an evidence-based approach for improving engagement of underrepresented populations into Alzheimer's Disease research, but has lacked scalability to the national level. As internet use among older adults from underrepresented populations continues to grow, internet-based research shows promise as a feasible, valid approach to engagement and longitudinal assessment. The Community Engaged Digital Alzheimer's Research (CEDAR) study utilizes a community-engaged research approach to increase the engagement and research participation of Black/African American adults in the Brain Health Registry (BHR) and Alzheimer Disease clinical research.ObjectivesTo describe the methods and evaluate the feasibility of the CEDAR culturally-informed digital platform within BHR.DesignAll Black/African American participants in BHR were invited to enroll in CEDAR and to consider serving on a newly convened Community-Scientific Partnership Board to guide the study. The community board guided the development a culturally-informed cadre of engagement materials and strategies to increase research participation. Engagement strategies included incentives for study task completion, culturally-informed communications (e.g., landing page, emails and social media), resources about brain health, and video and written testimonials by CEDAR participants.SettingBHR, an Internet-based registry and cohort.ParticipantsBHR participants self-identifying as Black/African American were invited to enroll. All participants who signed an online informed consent document were enrolled.MeasurementsWe report the number of participants invited, enrolled, completed tasks, and volunteered to join the community board. We compared the demographics, cognitive profile, and baseline BHR task completion rates between CEDAR participants and all those invited to join the study.ResultsOf 3738 invited, 349 (9.34%) enrolled in CEDAR. 134 (37% of CEDAR participants) volunteered to join the community board, of which 19 were selected for the community board. Compared to those invited, the CEDAR cohort had a higher percentage of female participants (84.5%) and a lower percentage of participants who identify as belonging to more than one ethnocultural group (21.8%). Compared to those did not enroll in CEDAR, those enrolled in CEDAR had a higher percentage of participants completing all BHR tasks (22%) and a higher percentage of participants completing at least one cognitive test (76%). Those enrolled in CEDAR also had a higher percentage of participants having an enrolled study partner (18%).ConclusionsA culturally-informed Community-Engaged Research approach, including a remotely-convened community board, to engagement of Black/African American participants in an online research registry is feasible. This approach can be adapted for use in various clinical studies and other settings. Future studies will evaluate the effectiveness of the engagement strategies.

BackgroundFailure of Alzheimer's disease and related diseases (ADRD) research studies to include and engage Black participants is a major issue, which limits the impact and generalizability of research findings. Little is known about participation of Black adults in online ADRD-related research registries.ObjectivesAs part of the Community Engaged Digital Alzheimer's Research (CEDAR) Study, this study aims to increase our understanding of facilitators and barriers of Black adults to participating in ADRD-related online registries, as well as to understand their preferences for communication channels.Design, setting, participants, measurementsWe invited all Black participants enrolled in the Brain Health Registry (BHR) to complete a cross-sectional online survey. The survey consisted of rating scales and open-text questions asking about their attitudes towards brain health research, reasons for joining and continuing to participate in BHR, difficulties with participating, and preferences for modes of contact and website usage.ResultsOf all invited Black BHR participants (N=3,636), 198 (5.5%) completed the survey. The mean age was 58.4 (SD=11.3), mean years of education were 16.3 (SD=2.4), and 85.5% identified as female. Reported facilitators for joining and continuing to participate in BHR were personal interest (e.g., learning more about own brain health) and altruism (e.g., helping research). Among additional registry features which could encourage return, receiving feedback or scores about BHR tasks was rated the highest. Of those who found BHR participation difficult (21%), the most frequent reason was time burden. The most preferred way of receiving study information was via email. Participants reported that the websites that they used the most were YouTube and Facebook.DiscussionThe results of our study can inform the development of culturally-responsive registry features and engagement efforts to improve inclusion and participation of Black adults in online ADRD research. Providing participants with feedback about their registry performance and reducing the number of registry tasks are among the recommended strategies.

The process of identifying suitable genome-wide association (GWA) studies and formatting the data to calculate multiple polygenic risk scores on a single genome can be laborious. Here, we present a centralized polygenic risk score calculator currently containing over 250,000 genetic variant associations from the NHGRI-EBI GWAS Catalog for users to easily calculate sample-specific polygenic risk scores with comparable results to other available tools. Polygenic risk scores are calculated either online through the Polygenic Risk Score Knowledge Base (PRSKB; https://prs.byu.edu ) or via a command-line interface. We report study-specific polygenic risk scores across the UK Biobank, 1000 Genomes, and the Alzheimer's Disease Neuroimaging Initiative (ADNI), contextualize computed scores, and identify potentially confounding genetic risk factors in ADNI. We introduce a streamlined analysis tool and web interface to calculate and contextualize polygenic risk scores across various studies, which we anticipate will facilitate a wider adaptation of polygenic risk scores in future disease research.

We study the Aharonov-Bohm caging effect in a one-dimensional lattice of theta-shaped units defining a chain of interconnected plaquettes, each one threaded by two synthetic flux lines. In the proposed system, light trapping results from the destructive interference of waves propagating along three arms, this implies that the caging effect is tunable and it can be controlled by changing the tunnel couplings $J$. These features reflect on the diffraction pattern allowing to establish a clear connection between the lattice topology and the resulting AB interference.

10. A New Nuclear Model for Neutrinoless $\beta\beta$-Decay and Double Charge-Exchange Resonances

We have recently developed a nuclear model, specially designed to describe double charge exchange (DCE) processes,which is a natural extension of the $pn$-QRPA model for $\beta$-decay to $\beta\beta$-decay. Ergo, this model, in addition to including the pairing correlations and having the same number of free parameters, brings into play the $ppnn$-quasiparticle excitations and the Pauli Principle, which are essential ingredients to correctly account for the nuclear matrix elements (NMEs) for $\beta\beta$-decays and nuclear reactions to all $0^+$ and $2^+$ final states, including resonances, and not just to the ground state as in $pn$-QRPA. In addition, it allows us to evaluate the $Q_{\beta\beta}$-values, and the excitation energies in final nucleus, and to work with a single-particle space large enough for the DCE sum rules to be satisfied. So far it has been applied to the $2\nu\beta\beta$-decay, and here we extend it to the $0\nu\beta\beta$-decay. The model that comes closest in terms of its advantages is the shell model. We describe here the DCE observables in: i) $^{76}$Se, which have been recently measured in the GERDA and MAJORANA Experiments, and ii) $^{124}$Te, for which has been reported recently the first direct observation of $2\nu$ double electron capture with the XENON1T Dark Matter detector. A good agreement with the data is obtained for both the ground state and the excited states. We have found that the reaction and $0\nu\beta\beta$-decay NMEs are of the same order of magnitude even in the region of giant resonances., Comment: 15 pages,4 figures

Combinatorial optimization problems are crucial for widespread applications but remain difficult to solve on a large scale with conventional hardware. Novel optical platforms, known as coherent or photonic Ising machines, are attracting considerable attention as accelerators on optimization tasks formulable as Ising models. Annealing is a well-known technique based on adiabatic evolution for finding optimal solutions in classical and quantum systems made by atoms, electrons, or photons. Although various Ising machines employ annealing in some form, adiabatic computing on optical settings has been only partially investigated. Here, we realize the adiabatic evolution of frustrated Ising models with 100 spins programmed by spatial light modulation. We use holographic and optical control to change the spin couplings adiabatically, and exploit experimental noise to explore the energy landscape. Annealing enhances the convergence to the Ising ground state and allows to find the problem solution with probability close to unity. Our results demonstrate a photonic scheme for combinatorial optimization in analogy with adiabatic quantum algorithms and enforced by optical vector-matrix multiplications and scalable photonic technology., Comment: 9 pages, 4 figures

13. Large-scale photonic Ising machine by spatial light modulation

Quantum and classical physics can be used for mathematical computations that are hard to tackle by conventional electronics. Very recently, optical Ising machines have been demonstrated for computing the minima of spin Hamiltonians, paving the way to new ultra-fast hardware for machine learning. However, the proposed systems are either tricky to scale or involve a limited number of spins. We design and experimentally demonstrate a large-scale optical Ising machine based on a simple setup with a spatial light modulator. By encoding the spin variables in a binary phase modulation of the field, we show that light propagation can be tailored to minimize an Ising Hamiltonian with spin couplings set by input amplitude modulation and a feedback scheme. We realize configurations with thousands of spins that settle in the ground state in a low-temperature ferromagnetic-like phase with all-to-all and tunable pairwise interactions. The new era of artificial intelligence demands large-scale ultrafast hardware for machine learning. Optical artificial neural networks process classical and quantum information at the speed of light, and are compatible with silicon technology, but lack scalability and need expensive manufacturing of many computational layers. New paradigms, as reservoir computing and the extreme learning machine, suggest that disordered and biological materials may realize artificial neural networks with thousands of computational nodes trained only at the input and at the readout. Here we employ biological complex systems, i.e., living three-dimensional tumour brain models, and demonstrate a random neural network (RNN) trained to detect tumour morphodynamics via image transmission. The RNN, with the tumour spheroid as a three-dimensional deep computational reservoir, performs programmed optical functions and detects cancer morphodynamics from laser-induced hyperthermia inaccessible by optical imaging. Moreover, the RNN quantifies the effect of chemotherapy inhibiting tumour growth. We realize a non-invasive smart probe for cytotoxicity assay, which is at least one order of magnitude more sensitive with respect to conventional imaging. Our random and hybrid photonic/living system is a novel artificial machine for computing and for the real-time investigation of tumour dynamics., Comment: 14 pages, 4 figures

15. Nonlinear transmission matrices of random optical media

Random media with tailored optical properties are attracting burgeoning interest for applications in imaging, biophysics, energy, nanomedicine, spectroscopy, cryptography and telecommunications. A key paradigm for devices based on this class of materials is the transmission matrix, the tensorial link between the input and the output signals, that describes in full their optical behavior. The transmission matrix has specific statistical properties, as the existence of lossless channels, that can be used to transmit information, and are determined by the disorder distribution. In nonlinear materials, these channels may be modulated and the transmission matrix tuned accordingly. Here we report the direct measurement of the nonlinear transmission matrix of complex materials, exploiting the strong optothermal nonlinearity of scattering Silica Aerogel (SA). We show that the dephasing effects due to nonlinearity are both controllable and reversible, opening the road to applications based on the nonlinear response of random media.

16. Plasmon-enhanced spin-orbit interaction of light in graphene

We develop a novel theoretical framework describing polariton-enhanced spin-orbit interaction of light on the surface of two-dimensional media. Starting from the integral formulation of electromagnetic scattering, we exploit the reduced dimensionality of the system to introduce a quantum-like formalism particularly suitable to fully take advantage of rotational invariance. Our description is closely related to that of a fictitious spin one quantum particle living in the atomically thin medium, whose orbital, spin and total angular momenta play a key role in the scattering process. Conservation of total angular momentum upon scattering enables to physically unveil the interaction between radiation and the two-dimensional material along with the detailed exchange processes among orbital and spin components. In addition, we specialize our model to doped extended graphene, finding such spin-orbit interaction to be dramatically enhanced by the excitation of surface plasmon polaritons propagating radially along the graphene sheet. We provide several examples of the enormous possibilities offered by plasmon-enhanced spin-orbit interaction of light including vortex generation, mixing, and engineering of tunable deep subwavelength arrays of optical traps in the near field. Our results hold great potential for the development of nano-scaled quantum active elements and logic gates for the manipulation of hyper-entangled photon states as well as for the design of artificial media imprinted by engineered photonic lattices tweezing cold atoms into the desired patterns.

- Abstract
Background: In Italy, surgeons continue to drain the abdominal cavity in more than 50 per cent of patients after colorectal resection. The aim of this study was to evaluate the impact of abdominal drain placement on early adverse events in patients undergoing elective colorectal surgery. Methods: A database was retrospectively analysed through a 1:1 propensity score-matching model including 21 covariates. The primary endpoint was the postoperative duration of stay, and the secondary endpoints were surgical site infections, infectious morbidity rate defined as surgical site infections plus pulmonary infections plus urinary infections, anastomotic leakage, overall morbidity rate, major morbidity rate, reoperation and mortality rates. The results of multiple logistic regression analyses were presented as odds ratios (OR) and 95 per cent c.i. Results: A total of 6157 patients were analysed to produce two well-balanced groups of 1802 patients: group (A), no abdominal drain(s) and group (B), abdominal drain(s). Group A versus group B showed a significantly lower risk of postoperative duration of stay >6 days (OR 0.60; 95 per cent c.i. 0.51–0.70; P < 0.001). A mean postoperative duration of stay difference of 0.86 days was detected between groups. No difference was recorded between the two groups for all the other endpoints. Conclusion: This study confirms that placement of abdominal drain(s) after elective colorectal surgery is associated with a non-clinically significant longer (0.86 days) postoperative duration of stay but has no impact on any other secondary outcomes, confirming that abdominal drains should not be used routinely in colorectal surgery.

- Abstract
Retrospective evaluation of the effects of mechanical bowel preparation (MBP) on data derived from two prospective open-label observational multicenter studies in Italy regarding elective colorectal surgery. MBP for elective colorectal surgery remains a controversial issue with contrasting recommendations in current guidelines. The Italian ColoRectal Anastomotic Leakage (iCral) study group, therefore, decided to estimate the effects of no MBP (treatment variable) versus MBP for elective colorectal surgery. A total of 8359 patients who underwent colorectal resection with anastomosis were enrolled in two consecutive prospective studies in 78 surgical centers in Italy from January 2019 to September 2021. A retrospective PSMA was performed on 5455 (65.3%) cases after the application of explicit exclusion criteria to eliminate confounders. The primary endpoints were anastomotic leakage (AL) and surgical site infections (SSI) rates; the secondary endpoints included SSI subgroups, overall and major morbidity, reoperation, and mortality rates. Overall length of postoperative hospital stay (LOS) was also considered. Two well-balanced groups of 1125 patients each were generated: group A (No MBP, true population of interest), and group B (MBP, control population), performing a PSMA considering 21 covariates. Group A vs. group B resulted significantly associated with a lower risk of AL [42 (3.5%) vs. 73 (6.0%) events; OR 0.57; 95% CI 0.38–0.84; p = 0.005]. No difference was recorded between the two groups for SSI [73 (6.0%) vs. 85 (7.0%) events; OR 0.88; 95% CI 0.63–1.22; p = 0.441]. Regarding the secondary endpoints, no MBP resulted significantly associated with a lower risk of reoperation and LOS > 6 days. This study confirms that no MBP before elective colorectal surgery is significantly associated with a lower risk of AL, reoperation rate, and LOS < 6 days when compared with MBP.

- Abstract
Background: There are uncertainties whether the impairment of lung diffusing capacity in COVID-19 is due to an alteration in the diffusive conductance of the alveolar membrane (Dm), or an alteration of the alveolar capillary volume (Vc), or a combination of both. The combined measurement DLNO and DLCO diffusion, owing to NO higher affinity and faster reaction rate with haemoglobin compared to CO, enables the simultaneous and rapid determination of both Vc and Dm. The aim of the present study was to better identify the precise cause of post-COVID-19 diffusion impairment. Methods: Using the combined NO and CO gas transfer techniques (DLNO and DLCO), it is possible to better understand whether gas exchange abnormalities are due to membrane or alveolar capillary volume components. The present study was aimed at evaluating pulmonary gas exchange one year after severe COVID-19. Results: The cohort included 33 survivors to severe COVID-19 (median age 67 years, 70% male) with no preexisting lung disease, who underwent clinical, lung function and imaging assessments at 12 months due to persistence of respiratory symptoms or radiological impairment. The gas exchange abnormalities were mainly determined by the compromise of the vascular component as demonstrated by vascular pattern of gas exchange impairment (i.e., DLNO/DLCO≥110%, 76% of the sample), and by a reduction of the Vc (73%), while the Dm was reduced only in 9% of the entire sample. We did not find a correlation between the gas exchange impairment and the extent of the chest CT alterations (DLCO p = 0.059 and DLNO p = 0.054), which on average were found to be mild (11% of the parenchyma). Conclusion: In COVID-19 survivors who are still symptomatic or have minimal CT findings at one year, gas exchange abnormalities are determined by impairment of the vascular component, rather than the diffusive component of the alveolar membrane.

We present a study of the nonmesonic weak decay (NMWD) of charmed hypernuclei using a relativistic formalism. We work within the framework of the independent particle shell model and employ a ($\pi$,$K$) one-meson-exchange model for the decay dynamics. We implement a fully relativistic treatment of nuclear recoil. Numerical results are obtained for the one-neutron-induced transition NMWD rates of the $_{\Lambda^{+}_{c}}^{12}$N. The effect of nuclear recoil is sizable and goes in the direction to decrease the nuclear decay rate. We found that the NMWD decay rate of $_{\Lambda^{+}_{c}}^{12}$N is of the same order of magnitude as the partial decay rate for the corresponding mesonic decay $\Lambda^+_c \rightarrow \Lambda + \pi^+$, suggesting the feasibility of experimental detection of such heavy-flavor nuclear processes., Comment: 24 pages, 3 figures

Pulsed Laser Deposition allows to obtain W and W-Ta alloy coatings with different nanostructures, monitored by X-ray diffraction. The correlation between such structures and the elastic properties is investigated for amorphous-like, ultra-nano- and nano-crystalline coatings obtained by tuning the gas pressure during deposition, annealing temperature and Ta concentration. The full elastic characterization is achieved by surface Brillouin spectroscopy, interpreted by isotropic and anisotropic film models. Amorphous like coatings are obtained with He pressures of tens of Pa. In comparison with bulk W, they have lower stiffness, by about 60\%, closely correlated to the mass density (lower by about 40\%). In the nanocrystalline regime the stiffness is more correlated to the average grain size, approaching the bulk values for increasing crystallite size. Vacuum annealing of amorphous like coatings leads to the nucleation of ultra-nano crystalline seeds, embedded in an amorphous matrix with intermediate values for mass density and stiffness. Here, the stiffness results from an interplay between the crystal size and the density. Alloying with Ta leads to properties which are consistent with the lever rule in the nanocrystalline regime, and deviate from it when the higher Ta concentration, interfering with crystal growth, induces an ultra-nano crystalline structure., Comment: The research leading to these results has also received funding from the European Research Council Consolidator Grant ENSURE (ERC-2014-CoG No. 647554)

Optical parametric oscillators are widely-used pulsed and continuous-wave tunable sources for innumerable applications, as in quantum technologies, imaging and biophysics. A key drawback is material dispersion imposing the phase-matching condition that generally entails a complex setup design, thus hindering tunability and miniaturization. Here we show that the burden of phase-matching is surprisingly absent in parametric micro-resonators adopting monolayer transition-metal dichalcogenides as quadratic nonlinear materials. By the exact solution of nonlinear Maxwell equations and first-principle calculation of the semiconductor nonlinear response, we devise a novel kind of phase-matching-free miniaturized parametric oscillator operating at conventional pump intensities. We find that different two-dimensional semiconductors yield degenerate and non-degenerate emission at various spectral regions thanks to doubly-resonant mode excitation, which can be tuned through the incidence angle of the external pump laser. In addition we show that high-frequency electrical modulation can be achieved by doping through electrical gating that efficiently shifts the parametric oscillation threshold. Our results pave the way for new ultra-fast tunable micron-sized sources of entangled photons, a key device underpinning any quantum protocol. Highly-miniaturized optical parametric oscillators may also be employed in lab-on-chip technologies for biophysics, environmental pollution detection and security., Comment: 22 pages, 7 figures

Light manifests extreme localized waves with long-tail statistics that seem analogous to the still little understood rogue waves in oceans, and optical setups promise to become laboratory test-beds for their investigation. However, to date there is no evidence that optical extreme events share the dynamics of their oceanic counterparts, and this greatly limits our ability to study rogue wave predictability using light. Using the Grassberger-Procaccia embedding method, we here demonstrate that optical spatial rogue wave data in photorefractive crystals has the same predictability and dynamic features of ocean rogue waves. For scales up to the autocorrelation length, a chaotic and predictable behavior emerges, whereas complexity in the dynamics causes long-range predictability to be limited by the finite size of data sets. The appearance of same dynamics validates the conjecture that rogue waves share universal features across different physical systems, these including their predictability.

Having in mind its future extension for theoretical investigations related to charmed nuclei, we develop a relativistic formalism for the nonmesonic weak decay of single-$\Lambda$ hypernuclei in the framework of the independent-particle shell model and with the dynamics represented by the $(\pi,K)$ one-meson-exchange model. Numerical results for the one-nucleon-induced transition rates of ${}^{12}_{\Lambda}\textrm{C}$ are presented and compared with those obtained in the analogous nonrelativistic calculation. There is satisfactory agreement between the two approaches, and the most noteworthy difference is that the ratio $\Gamma_{n}/\Gamma_{p}$ is appreciably higher and closer to the experimental value in the relativistic calculation. Large discrepancies between ours and previous relativistic calculations are found, for which we do not encounter any fully satisfactory explanation. The most recent experimental data is well reproduced by our results. In summary, we have achieved our purpose to develop a reliable model for the relativistic calculation of the nonmesonic weak decay of $\Lambda$-hypernuclei, which can now be extended to evaluate similar processes in charmed nuclei.

We report on a novel kind of transition in random lasers induced by the geometrical confinement of the emitting material. Different dye doped paper devices with controlled geometry are fabricated by soft-lithography and show two distinguished behaviors in the stimulated emission: in the absence of boundary constraints the energy threshold decreases for larger laser volumes showing the typical trend of diffusive {\it non-resonant} random lasers, while when the same material in lithographed into channels, the walls act as cavity and the {\it resonant} behavior typical of standard lasers is observed. Spin-glass theory is one of the leading paradigms of complex physics and describes condensed matter, neural networks and biological systems, ultracold atoms, random photonics, and many other research fields. According to this theory, identical systems under identical conditions may reach different states and provide different values for observable quantities. This effect is known as Replica Symmetry Breaking and is revealed by the shape of the probability distribution function of an order parameter named the Parisi overlap. However, a direct experimental evidence in any field of research is still missing. Here we investigate pulse-to-pulse fluctuations in random lasers, we introduce and measure the analogue of the Parisi overlap in independent experimental realizations of the same disordered sample, and we find that the distribution function yields evidence of a transition to a glassy light phase compatible with a replica symmetry breaking., Comment: 10 pages, 5 figures

The random laser emission from the functionalized thienyl-S,S-dioxide quinquethiophene (T5OCx) in confined patterns with different shapes is demonstrated. Functional patterning of the light emitter organic material in well defined features is obtained by spontaneous molecular self-assembly guided by surface tension driven (STD) lithography. Such controlled supramolecular nano-aggregates act as scattering centers allowing the fabrication of one-component organic lasers with no external resonator and with desired shape and efficiency. Atomic force microscopy shows that different geometric pattern with different supramolecular organization obtained by the lithographic process tailors the coherent emission properties by controlling the distribution and the size of the random scatterers.

We experimentally investigate the interplay between spatial shock waves and the degree of disorder during nonlinear optical propagation in a thermal defocusing medium. We characterize the way the shock point is affected by the amount of disorder and scales with wave amplitude. Evidence for the existence of a phase diagram in terms of nonlinearity and amount of randomness is reported. We study medium modifications of the nucleon-nucleon (NN) cross sections and their influence on the nucleon knockout reactions. Using the eikonal approximation, we compare the results obtained with free NN cross sections with those obtained with a purely geometrical treatment of Pauli-blocking and with NN obtained with more elaborated Dirac-Bruecker methods. The medium effects are parametrized in terms of the baryon density. We focus on symmetric nuclear matter, although the geometrical Pauli-blocking allows for the treatment of asymmetric nuclear matter. It is shown that medium effects can change the nucleon knockout cross sections and momentum distributions up to 10% in the energy range E=50-300 MeV/nucleon. - Abstract
Background: Since most anastomoses after left-sided colorectal resections are performed with a circular stapler, any technological change in stapling devices may influence the incidence of anastomotic adverse events. The aim of the present study was to analyze the effect of a three-row circular stapler on anastomotic leakage and related morbidity after left-sided colorectal resections. Materials and methods: A circular stapled anastomosis was performed in 4255 (50.9%) out of 8359 patients enrolled in two prospective multicenter studies in Italy, and, after exclusion criteria to reduce heterogeneity, 2799 (65.8%) cases were retrospectively analyzed through a 1:1 propensity score-matching model including 20 covariates relative to patient characteristics, to surgery and to perioperative management. Two well-balanced groups of 425 patients each were obtained: group (A) – true population of interest, anastomosis performed with a three-row circular stapler; group (B) – control population, anastomosis performed with a two-row circular stapler. The target of inferences was the average treatment effect in the treated (ATT). The primary endpoints were overall and major anastomotic leakage and overall anastomotic bleeding; the secondary endpoints were overall and major morbidity and mortality rates. The results of multiple logistic regression analyses for the outcomes, including the 20 covariates selected for matching, were presented as odds ratios (OR) and 95% confidence intervals (95% CI). Results: Group A versus group B showed a significantly lower risk of overall anastomotic leakage (2.1 vs. 6.1%; OR 0.33; 95% CI 0.15–0.73; P = 0.006), major anastomotic leakage (2.1 vs. 5.2%; OR 0.39; 95% CI 0.17–0.87; P = 0.022), and major morbidity (3.5 vs. 6.6% events; OR 0.47; 95% CI 0.24–0.91; P = 0.026). - Abstract
Phase transitions in a non-perturbative regime can be studied by ab initio Lattice Field Theory methods. The status and future research directions for LFT investigations of Quantum Chromo-Dynamics under extreme conditions are reviewed, including properties of hadrons and of the hypothesized QCD axion as inferred from QCD topology in different phases. We discuss phase transitions in strong interactions in an extended parameter space, and the possibility of model building for Dark Matter and Electro-Weak Symmetry Breaking. Methodological challenges are addressed as well, including new developments in Artificial Intelligence geared towards the identification of different phases and transitions.

- Abstract
We report the analysis of 1 year of data from the first cohort of 15 patients enrolled in an open-label, first-in-human, dose-escalation phase I study (ClinicalTrials.gov: NCT03282760, EudraCT2015-004855-37) to determine the feasibility, safety, and tolerability of the transplantation of allogeneic human neural stem/progenitor cells (hNSCs) for the treatment of secondary progressive multiple sclerosis. Participants were treated with hNSCs delivered via intracerebroventricular injection in combination with an immunosuppressive regimen. No treatment-related deaths nor serious adverse events (AEs) were observed. All participants displayed stability of clinical and laboratory outcomes, as well as lesion load and brain activity (MRI), compared with the study entry. Longitudinal metabolomics and lipidomics of biological fluids identified time- and dose-dependent responses with increased levels of acyl-carnitines and fatty acids in the cerebrospinal fluid (CSF). The absence of AEs and the stability of functional and structural outcomes are reassuring and represent a milestone for the safe translation of stem cells into regenerative medicines.

Background: In patients with pneumonia or acute respiratory distress syndrome who survived hospitalization, one-year mortality can affect up to one third of discharged patients. Therefore, significant long-term mortality after COVID-19 respiratory failure could be expected. The primary outcome of the present study was one-year all-cause mortality in hospitalized COVID-19 patients. Methods: Observational study of COVID-19 patients hospitalized at Papa Giovanni XXIII Hospital (Bergamo, Italy), during the first pandemic wave. Results: A total of 1326 COVID-19 patients were hospitalized. Overall one-year mortality was 33.6% (N 446/1326), with the majority of deaths occurring during hospitalization (N=412, 92.4%). Thirty-four patients amongst the 914 discharged (3.7%) subsequentely died within one year. A third of these patients died for advanced cancer, while death without a cause other than COVID-19 was uncommon (8.8% of the overall post-discharge mortality). In-hospital late mortality (i.e. after 28 days of admission) interested a population with a lower age, and fewer comorbidities, more frequentely admitted in ICU. Independent predictors of post-discharge mortality were age over 65 years (HR 3.19; 95% CI 1.28-7.96, p-value=0.013), presence of chronic obstructive pulmonary disease (COPD) (HR 2.52; 95% CI 1.09-5.83, p-value=0.031) or proxy of cardiovascular disease (HR 4.93; 95% CI 1.45-16.75, p-value=0.010), and presence of active cancer (HR 3.64; 95% CI 1.50-8.84, p-value=0.004), but not pneumonia severity. Conclusions: One-year post-discharge mortality depends on underlying patients’ comorbidities rather than COVID-19 pneumonia severity per se. Awareness among physicians of predictors of post-discharge mortality might be helpful in structuring a follow-up program for discharged patients.

We investigate mode-locking processes in lasers displaying a variable degree of structural randomness, from standard optical cavities to multiple-scattering media. By employing methods mutuated from spin-glass theory, we analyze the mean-field Hamiltonian and derive a phase-diagram in terms of the pumping rate and the degree of disorder. Three phases are found: i) paramagnetic, corresponding to a noisy continuous wave emission, ii) ferromagnetic, that describes the standard passive mode-locking, and iii) the spin-glass in which the phases of the electromagnetic field are frozen in a exponentially large number of configurations. The way the mode-locking threshold is affected by the amount of disorder is quantified. The results are also relevant for other physical systems displaying a random Hamiltonian, like Bose-Einstein condensates and nonlinear optical beams., Comment: 4 pages, 2 figures

The nonlinear propagation of 3D wave-packets in a 1D Bragg-induced band-gap system, shows that tranverse effects (free space diffraction) affect the interplay of periodicity and nonlinearity, leading to the spontaneous formation of fast and slow conical localized waves.
