Background: Combination therapy is required for the treatment of moderate acne vulgaris. However, patient compliance in applying multiple topical formulations is poor. Objective: To assess the efficacy and safety of a fixed-dose combination gel with adapalene 0.1% and clindamycin 1% (adapalene-clindamycin) relative to adapalene 0.1% monotherapy and clindamycin 1% monotherapy in patients with moderate facial acne vulgaris. Methods: This was a randomized, controlled, assessor-blind, phase III study conducted in patients with moderate facial acne vulgaris. Results: A total of 1617 patients were enrolled. At week 12, patients in the adapalene–clindamycin gel treatment group showed a significant reduction in the percentage change from baseline in total lesion count (− 66.85%), compared with adapalene alone (− 50.82%) or clindamycin gel alone (− 57.61%). The difference in the least square means of the adapalene–clindamycin gel group and adapalene group, or clindamycin gel group was − 16.08% (95% CI − 19.95% to − 12.21%) and − 9.38% (95% CI − 13.25% to − 5.51%;), respectively. At week 12, 19.28% of participants who received adapalene–clindamycin gel achieved at least 2-grade improvement in IGA, versus 7.74% with adapalene gel (OR 3.05, 95% CI 1.93, 4.80) and 14.77% with clindamycin gel (OR 1.42, 95% CI 0.97, 2.07). The study also achieved all its secondary endpoints. Adverse event rates were mostly mild to moderate and comparable across the three treatment groups. Conclusion: Adapalene 0.1%–clindamycin 1% combination gel is well tolerated and demonstrated superior efficacy over 0.1% adapalene gel monotherapy and 1% clindamycin gel monotherapy for the treatment of moderate acne vulgaris. Background: Acne vulgaris is a chronic inflammatory dermatosis. Cutibacterium acnes plays a crucial role in the acne pathophysiology. Recent works present evidence of C. acnes growing as a biofilm in cutaneous follicles. This development is currently considered one of the leading causes of C. acnes in vivo persistence and resistance to antimicrobials used to treat acne. Objective: Our objective was to evaluate the effects of various active compounds (clindamycin, erythromycin, doxycycline, and myrtle extract) on eight distinct, well‐characterized strains of C. acnes following their growth in biofilm mode. Methods/Results: Cutibacterium acnes isolates from phylotypes IA1 and IA2 produce more biofilm than other phylotypes. No antibiotic effect was observed either during the curative test or preventive test. Myrtle extract at 0.01% (w/v) showed significant efficacy on the biofilm for C. acnes strains (curative assays). Furthermore, it appear that myrtle extract and doxycycline together reduce the overall biomass of the biofilm. A significant dose‐dependent effect was observed during the preventive test, greater than the one observed under curative conditions, with an important loss of activity of the myrtle extract observed from 0.001% (w/v) concentration onwards. Transmission electron microscopy showed that bacteria treated with myrtle extract grew biofilms much less frequently than untreated bacteria. Additionally, when the quantity of myrtle extract grew, the overall number of bacteria dropped, indicating an additional antibacterial action. Conclusion: These findings support the hypothesis that the different C. acnes phylotypes have various aptitudes in forming biofilms. Inflammatory periodontal diseases associated with the accumulation of dental biofilm, such as gingivitis and periodontitis, are very common and pose clinical problems for clinicians and patients. Gingivitis is a mild form of gum disease and when treated quickly and properly is completely reversible. Periodontitis is an advanced and irreversible disease of the periodontium with periods of exacerbations, progressions and remission. Periodontitis is a chronic inflammatory condition that damages the tissues supporting the tooth in its socket, i.e., the gums, periodontal ligaments, root cementum and bone. Periodontal inflammation is most commonly triggered by bacteria present in excessive accumulations of dental plaque (biofilm) on tooth surfaces. This disease is driven by disproportionate host inflammatory immune responses induced by imbalance in the composition of oral bacteria and changes in their metabolic activities. This microbial dysbiosis favors the establishment of inflammatory conditions and ultimately results in the destruction of tooth-supporting tissues. Apart microbial shift and host inflammatory response, environmental factors and genetics are also important in etiology In addition to oral tissues destruction, periodontal diseases can also result in significant systemic complications. Conventional methods of periodontal disease treatment (improving oral hygiene, dental biofilm control, mechanical plaque removal, using local or systemic antimicrobial agents) are not fully effective. All this prompts the search for new methods of therapy. Advanced periodontitis with multiple abscesses is often treated with antibiotics, such as amoxicillin, tetracycline, doxycycline, minocycline, clindamycin, or combined therapy of amoxicillin with metronidazole. However, due to the growing problem of antibiotic resistance, treatment does not always achieve the desired therapeutic effect. This review summarizes pathogenesis, current approaches in treatment, limitations of therapy and the current state of research on the possibility of application of bacteriophages and predatory bacteria to combat bacteria responsible for periodontitis. We present the current landscape of potential applications for alternative therapies for periodontitis based on phages and bacteria, and highlight the gaps in existing knowledge that need to be addressed before clinical trials utilizing these therapeutic strategies can be seriously considered. The wound healing process has long been a focal point within the medical community due to its significant implications. Combining bioactive compounds with chemical drugs concurrently not only mitigates the risk of drug resistance but also enhances the healing process. Herein, the study emphasizes enhancing wound healing by co-delivering clindamycin (CM) and propolis extract (Ps) through poly(ε-caprolactone) (PCL) nanofibers (NFs). Scanning electron microscopy (SEM) analysis confirmed the uniform and continuous structure of nanofibers with diameters of 155.7 ± 52.8 nm, 163.3 ± 51.4 nm, 167.8 ± 73.3 nm, and 152.2 ± 39 nm for PCL, PCL/CM, PCL/Ps, and PCL/CM-Ps, respectively. Successful loading of CM and Ps was confirmed by FTIR analysis. Dual-loaded nanofibers exhibited improved mechanical properties compared to neat PCL. The nanofibers demonstrated favorable porosity, swelling behavior, and water vapor transmission rate. Degradation studies indicated gentle degradation rates with minimal morphological changes. Clindamycin exhibited controlled release from PCL/CM nanofibers, contrasting with propolis extract from PCL/Ps nanofibers. The nanofibers were found to be biocompatible, supporting cell growth and proliferation. Additionally, they offered cytoprotective effects against oxidative stress, with CM-Ps-loaded nanofibers showing the highest cell viability. Hemolysis tests confirmed the blood compatibility of all nanofibers. Clindamycin is a lincosamide antibiotic for the treatment of staphylococcal, streptococcal, and anaerobic bacterial infections. We conducted a single‐center, single‐dose, 2‐preparation, 2‐period, 2‐sequence, randomized, open‐label, 2 × 2 crossover study to evaluate the pharmacokinetics (PKs) and safety of the test and reference clindamycin hydrochloride capsules in healthy Chinese subjects under both fasted and fed conditions. Forty‐eight subjects were enrolled in the study totally, with 24 subjects in each group. All subjects were asked to take a test or reference preparation, under either fasted or fed condition, and their blood samples were collected and assayed by a validated high‐performance liquid chromatography‐tandem mass spectrometry method. PK parameters including maximum plasma concentration, area under the plasma concentration‐time curve from time 0 to the last concentration, and area under the plasma concentration‐time curve from time 0 to infinity were estimated with noncompartmental model and analyzed. Results showed that the PK profiles of the 2 preparations were consistent and met the bioequivalence criteria. Food was identified as a factor that had an impact on clindamycin absorption. The safety of clindamycin hydrochloride capsules was satisfactory. Background/Objectives: Owing to the growing resistance of methicillin-resistant Staphylococcus aureus (MRSA) to conventional antibiotics, the development of innovative therapeutic strategies for the treatment of MRSA-infected cutaneous wounds poses a significant challenge. Methods: Here, by using polyhydroxyalkanoates (PHA), emerging biodegradable and biocompatible polymers naturally produced by various microorganisms, we developed clindamycin-loaded PHA nanoparticles (Cly-PHA NPs) as a novel approach for the treatment of MRSA-infected cutaneous wounds. Results: Cly-PHA NPs were characterized in terms of mean particle size (216.2 ± 38.9 nm), polydispersity index (0.093 ± 0.03), zeta potential (11.3 ± 0.5 mV), and drug loading (6.76 ± 0.19%). Owing to the sustained release of clindamycin over 2 days provided by the PHA, Cly-PHA NPs exhibited potent antibacterial effects against MRSA. Furthermore, Cly-PHA NPs significantly facilitated wound healing in a mouse model of MRSA-infected full-thickness wounds by effectively eradicating MRSA from the wound bed. Background: Marmosets (Callithrix sp.), including black‐tuffed marmosets (C. penicillata), are neotropical primates that can be highly adapted to urban environments, especially parks and forested areas near cities. Staphylococcus spp. are part of the microbiota of many different hosts and lead to opportunistic severe infection. Isolates from wild animals can be resistant to antimicrobial drugs. However, there are a few studies that evaluated Staphylococcus spp. in neotropical primates. The goal of this study was to evaluate Staphylococcus spp. isolated from free‐ranging black‐tuffed marmosets. Methods: Marmosets were captured in six urban parks. After sedation, skin and rectal swabs and feces were sampled. Staphylococcus spp. isolates were identified by MALDI‐ToF and their antimicrobial susceptibility was determined. Results: Over 30% of captured individuals were positive for Staphylococcus spp., and S. aureus was the most isolated species followed by Mammaliicoccus (Staphylococcus) sciuri. With the exception of the marmoset subjected to necropsy, none of the other had lesions, which supports that notion that Staphylococcus spp. are members of the microbiota, but also opportunistic pathogens. Most isolates were susceptible to all antimicrobials tested; however, one isolate of S. epidermidis was resistant to multiple antimicrobials (penicillin, cefoxitin, ciprofloxacin, clindamycin, and erythromycin). We considered S. aureus as the main staphylococci to colonize black‐tuffed marmosets. Conclusions: Black‐tuffed marmosets can be colonized by several Staphylococcus species, most frequently by S. aureus, and the majority of isolates were sensible to the antimicrobials tested. Chronic recurrent multifocal osteomyelitis (CRMO) or chronic nonbacterial osteitis is a sterile autoinflammatory disease of bone in children that can mimic infectious osteomyelitis and osteosarcoma. Early diagnosis, treatment, and long-term follow-up of CRMO are essential. We describe a 10-year-old boy who presented with 15 days of left ankle bone more than joint pain, swelling, and limp. Plain radiographs and magnetic resonance imaging scans were nondiagnostic of osteomyelitis and tibial irrigation and biopsy were negative for infection and malignancy. Four years later, he again presented with similar pain in his right ankle. Repeat bone biopsy noted reactive bone changes and bone culture was sterile. Whole-body magnetic resonance imaging revealed multiple enhancing lesions in the long bones of bilateral lower extremities, spine, and sternum. He was diagnosed with CRMO, and treatment with celecoxib and subsequently pamidronate, infliximab, and methotrexate were initiated. After 6 months of treatment, the patient's gait and pain improved, and 2 years later, his CRMO was in clinical and radiologic remission. Of note, he developed palmoplantar pustular psoriasis, commonly seen in CRMO, that was not determined to be from tumor necrosis factor inhibition. [ABSTRACT FROM AUTHOR]

Acute generalized exanthematous pustulosis (AGEP) is a serious and rare adverse reaction to clindamycin. This study investigated the clinical features of clindamycin-induced AGEP and provided reference for the prevention and treatment of AGEP. Case reports, case series and clinical studies of clindamycin-induced AGEP were collected by retrieving English and Chinese database from inception until May 31, 2024. Of the 35 patients included, 25 (71.4%) were female, and the median age was 57 years (1.6–88 years). The duration of AGEP onset is 2 days (range 0.04,13) after initial administration. The main clinical morphology of AGEP is a non-follicular pustular on an erythematous base, which may be accompanied by fever (54.3%) and pruritus (40.0%). These lesions mainly involved extremities and trunk. The median elevated neutrophil count was 13.3 × 109/L (range 10.3, 31.4). Histologic features of AGEP are characterized by intracorneal, subcorneal, and/or intraepidermal pustules with papillary dermal edema containing neutrophilic, lymphocytic, andeosinophilic infiltrates. Patients gradually recovered after the withdrawal of clindamycin and supportive therapy with a median time of 9 days (range 2, 30). Clinicians should be aware of AGEP as a rare adverse effect of clindamycin. When clindamycin is prescribed, it should be stopped in time when AGEP occurs, and active systemic treatment should be given. AGEP is a self-limiting disease with a good prognosis. This work examined the occurrence characteristics and ecological risks of 31 antibiotics across five classes and seven ARGs in the surface waters of Gaoyou Lake. A total of 27 antibiotics, spanning four classes, were detected in the surface waters of Gaoyou Lake, with an overall concentration ranging from 57.5 to 114 ng/L and an average of 78.8 ng/L. Sulfonamide antibiotics exhibited the highest average concentration at 32.7 ng/L. Spatial analysis revealed that antibiotic concentration levels in the western region of the lake were higher than those in other areas. Similarly, ARGs were most abundant in this area, with sulfonamide ARGs demonstrating a notably higher mean abundance than other ARGs. Correlation analysis revealed strong positive associations between sul1 and several antibiotics, including sulfadimidine, sulfamethoxazole, ciprofloxacin, lincomycin, clindamycin, erythromycin, and intl1 (P < 0.05), with intra-group correlations among sulfonamide ARGs exceeding those between different ARG groups. Ecological risk assessment indicated that erythromycin and sulfamethoxazole presented medium risks, whereas roxithromycin, azithromycin, and lincomycin were associated with low risks to aquatic organisms. The ecological risk proportions across monitoring sites were primarily low (10.6%) and moderate (16.7%), with no high-risk areas identified and 72.7% presenting no risk. Background: Congenital toxoplasmosis occurs when a pregnant woman becomes infected with Toxoplasma gondii (T. gondii) for the first time. Treatment typically involves antimicrobial medications, with spiramycin commonly used to prevent transmission. However, spiramycin's effectiveness is limited due to poor placental penetration. Clindamycin, another antibiotic, can cross the placenta but reaches the fetus at only half the maternal concentration. Encapsulating the drug in chitosan-coated niosomes (Cs-Nio) could enhance its effectiveness by targeting specific organs and ensuring sustained release. To address the challenges of using clindamycin, a niosome-coated chitosan system was investigated for treating congenital toxoplasmosis caused by the VEG strain of T. gondii in an animal model. Methods: Pregnant mice were infected with VEG strain of T. gondii on the 12th day of pregnancy, followed by treatment with various drugs across six groups. The treatments included chitosan-coated niosomes loaded clindamycin (Cs-Nio-Cli) and other controls. Parasitological evaluations (microscopic examination and real-time PCR), along with histopathological and immunological assessments were conducted to assess treatment efficacy. Finally, statistical analysis was conducted using GraphPad Prism 8.0 and SPSS 26, comparing test and control groups with T test and Mann–Whitney test. A p ≤ 0.05 was considered statistically significant. Results: The study found that treatment with Cs-Nio-Cli significantly reduced the number of T. gondii cysts in the brain and eyes (97.59% and 92.68%, respectively) compared to the negative control group. It also mitigated inflammatory changes, prevented cell death, and reduced vascular cuffs in the brain. In addition, Cs-Nio-Cli treatment decreased bleeding, placental thrombosis, and inflammatory cell infiltration in the placenta while improving eye tissue health by reducing retinal folds and bleeds. Immunologically, nanoclindamycin treatment resulted in lower TNF-α cytokine levels and higher IL-10 levels, indicating an enhanced anti-inflammatory response. Conclusions: Although Cs-Nio-Cli demonstrates promise in reducing the transmission of congenital toxoplasmosis and mitigating the effects of congenital toxoplasmosis, additional research is necessary to determine the optimal treatment regimens for the complete eradication of the parasite in the fetus. Background The use of adjunctive antibiotics directed against exotoxin production in Staphylococcus aureus bacteremia (SAB) is widespread, and it is recommended in many guidelines, but this is based on limited evidence. Existing guidelines are based on the theoretical premise of toxin suppression, as many strains of S. aureus produce toxins such as leukocidins (eg, Panton-Valentine leukocidin, toxic shock syndrome toxin 1, exfoliative toxins, and various enterotoxins). Many clinicians therefore believe that limiting exotoxin production release by S. aureus could reduce its virulence and improve clinical outcomes. Clindamycin, a protein synthesis inhibitor antibiotic, is commonly used for this purpose. We report the domain-specific protocol, embedded in a large adaptive, platform trial, seeking to definitively answer this question. Methods and Analysis The Staphylococcus aureus Network Adaptive Platform (SNAP) trial is a pragmatic, randomized, multicenter adaptive platform trial that aims to compare different SAB therapies, simultaneously, for 90-day mortality rates. The adjunctive treatment domain aims to test the effectiveness of adjunctive antibiotics, initially comparing clindamycin to no adjunctive antibiotic, but future adaptations may include other agents. Individuals will be randomized to receive either 5 days of adjunctive clindamycin (or lincomycin) or no adjunctive antibiotic therapy alongside standard-of-care antibiotics. Most participants with SAB (within 72 hours of index blood culture and with no contraindications) will be eligible to participate in this domain. Prespecified analyses are defined in the statistical appendix to the core protocol, and domain-specific secondary analyses will be adjusted for resistance to clindamycin, disease phenotype (complicated or uncomplicated SAB) and Panton-Valentine leukocidin–positive isolate. Objective: The purpose of this study is a new update on the resistance profile, Macrolide–Lincosamide–Streptogramin B resistance mechanisms and biofilm formation in the Staphylococcus aureus isolated from health care workers (HCWs) nasal carriage at a children's teaching hospital in Babol (Northern Iran). Results: A total of 143 non-repetitive nasal swab samples were collected from volunteers, where 53.8% (n; 77/143) were HCWs, 33.6% (n; 48/143) medical students, and 12.6% (n; 18/143) resident students. The prevalence of nasal carriers of S. aureus was 22.4% (n; 32/143), among them, 40.6% (n; 13/32) were identified as methicillin-resistant Staphylococcus aureus (MRSA(carriers. Antimicrobial susceptibility testing showed that erythromycin (68.8%, n; 22/32) and ciprofloxacin (15.6%, n; 5/32) had the highest and lowest resistance rate, respectively. The frequency of resistance genes in the strains was as follows; ermC (n; 17/32, 53.1%), ermA (n; 11/32, 34.4%), ermB (n; 6/32, 18.7%), ereA (n; 3/32, 9.4%). Moreover, 50.0% (n; 16/32), 28.1% (n; 9/32) and 21.8% (n; 7/32) of isolates were strongly, weakly and moderately biofilm producer, respectively. Background and purpose: Group B Streptococcus (GBS), or Streptococcus agalactiae, is a major cause of invasive diseases such as meningitis and septicemia in newborns, as well as various diseases in pregnant women and non-pregnant adults with underlying conditions. This bacterium colonizes the vagina and rectum in 10% to 30% of pregnant women. GBS colonization in pregnant women increases the risk of preterm delivery and transmission of the bacteria to the baby during childbirth. Penicillin is the first-line antibiotic for treating GBS infections, while macrolides (e.g., erythromycin), lincosamides (e.g., clindamycin), and streptogramins have been recommended as alternative treatments. However, the prevalence of antibiotic resistance is rising and could lead to significant clinical challenges. Therefore, this study aimed to investigate the prevalence of resistance to erythromycin and clindamycin, as well as the associated resistance genes, in GBS strains isolated from pregnant women in Iran. Materials and methods: This meta-analysis examines studies focused on resistance to erythromycin and clindamycin in GBS strains isolated from pregnant women. A comprehensive search was conducted across databases, including PubMed, Scopus, Web of Science, SID (Scientific Information Database), and Google Scholar, using keywords such as Streptococcus agalactiae, Group B Streptococcus, antibiotic resistance, erythromycin, clindamycin, MLSB (macrolide, lincosamide, and streptogramin B) resistance, and related terms. Inclusion criteria included sufficient sample size, control of confounding factors, inclusion of the relevant antibiotics, and availability of full-text articles. Data were analyzed using Comprehensive Meta-Analysis software (Biostat V2.2). Heterogeneity between studies was assessed using the I2 index, and a random-effects model was applied due to heterogeneity in the results. Results: The initial database search identified 241 studies. After removing 41 duplicates, 201 studies were considered for meta-analysis. After reviewing the titles and abstracts, 158 studies were excluded for not meeting the research criteria, leaving 16 studies for final analysis. This meta-analysis investigated the prevalence of resistance to erythromycin and clindamycin, resistance phenotypes, and the genes encoding antibiotic resistance in GBS isolates from pregnant women. All studies included were cross-sectional. The pooled resistance rates for erythromycin and clindamycin were 27.9% (CI95%: 16-43.9) and 28.4% (CI95%: 17-43.5), respectively. Continuous and inducible resistance phenotypes were observed in 17.5% (CI95%: 6.2- 40.4) and 10.9% (CI95%: 4.7-23.2) of the isolates. The M and L phenotypes were found in 8.1% (CI95%: 3.9-16) and 4.8% (CI95%: 2.7-8.4) of the isolates, respectively. The prevalence of resistance genes, including ermTR, ermB, and mefA, was 34% (CI95%: 27.4-41.4), 24.5% (CI95%: 3-77.1), and 6.7% (CI95%: 3.9-11.4), respectively. Conclusion: The study shows an increasing trend of erythromycin and clindamycin resistance in GBS isolates from pregnant women in Iran. Methicillin-resistant Staphylococcus aureus (MRSA) is a highly infectious pathogen that poses a serious threat to human life and health. This study aimed to provide a scientific basis for the rational clinical use of antimicrobial drugs for treating MRSA infections and inform the development of preventive and control measures by analyzing the clinical distribution and resistance characteristics of MRSA in a hospital in Hebei China. To accomplish this, bacterial identification and drug sensitivity experiments were performed with 1858 Staphylococcus aureus (S. aureus) strains collected from a hospital from January 2018 to December 2022 using a phoenixTM-100 bacterial identification drug sensitivity analyzer. The experimental data were analyzed using WHONET 5.6 software, and the MRSA strains detected were analyzed for their clinical distribution and drug resistance. Of the 1858 S. aureus strains isolated, 429 were MRSA. Sputum samples had the highest MRSA detection rates (52.45%). Critical care medicine had the highest rate of MRSA (12.59%), followed by dermatology (9.79%). MRSA resistance to tetracycline increased by 13.9% over 5 years; resistance to quinupristin/dalfopristin also increased but remained low (1.9%). Resistance decreased to gentamicin, rifampicin, ciprofloxacin, and cotrimoxazole, though most significantly to erythromycin and clindamycin, exceeding 77% and 83%, respectively. No strains were resistant to vancomycin, teicoplanin, or linezolid, and drug resistance was most prevalent in patients ≥ 60 years old. Background: Staphylococcus aureus stands as a prominent cause for both nosocominal and community acquired infections across the globe. Clindamycin, a member of MLSB family, has emerged as the preferred choice for treating Staphylococcus aureus infections. However, one critical consideration in administering Clindamycin is the potential development of inducible Clindamycin resistance, posing a significant risk of clinical treatment failure. Aim: The study was aimed to determine the occurence of inducible Clindamycin resistance among Staphylococcus aureus isolates in our geographic region. Method: The hospital based cross sectional study was conducted over a period of one year from January 2023 to December 2023. A total of 672 Staphylococcus aureus isolates were identified by conventional methods and subjected to antimicrobial susceptibility test by Kirby Bauer disk diffusion method and Erythromycin resistant isolates were tested for D test. Result: Out of 672 Staphylococcus aureus isolates, 517 were MRSA and 155 were MSSA. Erythromycin resistance was seen in 395 (58.77%) isolates. D test revealed inducible clindamycin resistance in 45 (6.69%) isolates, constitutive clindamycin resistance in 91 (13.54%) isolates and MS phenotype in 259 (38.54%) isolates. Inducible clindamycin resistance was more in MRSA (7.73%) isolates compared to MSSA (3.22%) isolates. Conclusion: D test is a simple and cost-effective test that should be performed as routine laboratory test and will help in guiding the clinicians regarding judicious use of Clindamycin. Objective: This study aimed to evaluate antibiotic susceptibility and antimicrobial resistance trends among clinically significant anaerobes in Kuwait hospitals from 2013 to 2022, comparing these findings with data from 2002 to 2012. Methods: The study prospectively collected 2,317 anaerobic isolates from various body sites across four Kuwaiti hospitals between January 2013 and December 2022. The minimum inhibitory concentrations for 11 antianaerobic antibiotics were determined using E-test methodology. The study analyzed trends and resistance rates across two periods: 2013–2017 and 2018–2022, using statistical analysis for resistance comparison. Results: Of the 2,317 isolates, most were from wounds (42.2%), fluids (28.0%), and tissues (20.5%). Bacteroides fragilis was the most common pathogen (34.0%), followed by Prevotella bivia (13.4%). Over 90% of isolates were susceptible to imipenem, meropenem, tigecycline, and metronidazole, whereas lower susceptibility was observed for penicillin, amoxicillin–clavulanic acid, and clindamycin. Notable differences in resistance profiles since 2002 were observed, especially in amoxicillin–clavulanic acid, piperacillin, piperacillin–tazobactam, and clindamycin. Conclusion: Owing to detected resistance to all antibiotics, susceptibility testing for anaerobic isolates is recommended in severe infections to ensure effective antimicrobial therapy. A 2-year-5-month-old, male, neutered, crossbreed dog presented with third eyelid protrusion, right eye exophthalmos and facial hyperaesthesia. Neurological examination was suggestive of a lesion affecting the right optic nerve. Magnetic resonance imaging allowed a presumptive diagnosis of sphenoid bone osteomyelitis, meningitis and right-sided optic neuritis. The dog was discharged with clindamycin (13 mg/kg/8 h), amoxicillin-clavulanate (25 mg/kg/8 h), gabapentin (10 mg/kg/8 h), paracetamol (15 mg/kg/8 h) and prednisolone (0.5 mg/kg/24 h). Paracetamol, gabapentin and prednisolone were gradually stopped. A 3-month follow-up revealed right-eye blindness. Magnetic resonance imaging showed improvement in changes compatible with osteomyelitis and meningitis resolution, prompting clindamycin cessation. At the 6-month follow-up, magnetic resonance imaging indicated only right optic nerve atrophy. Amoxicillin-clavulanate was stopped. There had been no relapses at 12-month followup. Serial magnetic resonance imaging studies were pivotal in adjusting antibiotic therapy, and instilled confidence for treatment cessation upon lesion resolution. Abstract Background Combination therapy is required for the treatment of moderate acne vulgaris. However, patient compliance in applying multiple topical formulations is poor. Objective To assess the efficacy and safety of a fixed-dose combination gel with adapalene 0.1% and clindamycin 1% (adapalene-clindamycin) relative to adapalene 0.1% monotherapy and clindamycin 1% monotherapy in patients with moderate facial acne vulgaris. Methods This was a randomized, controlled, assessor-blind, phase III study conducted in patients with moderate facial acne vulgaris. Results A total of 1617 patients were enrolled. At week 12, patients in the adapalene–clindamycin gel treatment group showed a significant reduction in the percentage change from baseline in total lesion count (− 66.85%), compared with adapalene alone (− 50.82%) or clindamycin gel alone (− 57.61%). The difference in the least square means of the adapalene–clindamycin gel group and adapalene group, or clindamycin gel group was − 16.08% (95% CI − 19.95% to − 12.21%) and − 9.38% (95% CI − 13.25% to − 5.51%;), respectively. At week 12, 19.28% of participants who received adapalene–clindamycin gel achieved at least 2-grade improvement in IGA, versus 7.74% with adapalene gel (OR 3.05, 95% CI 1.93, 4.80) and 14.77% with clindamycin gel (OR 1.42, 95% CI 0.97, 2.07). The study also achieved all its secondary endpoints. Adverse event rates were mostly mild to moderate and comparable across the three treatment groups. Conclusion Adapalene 0.1%–clindamycin 1% combination gel is well tolerated and demonstrated superior efficacy over 0.1% adapalene gel monotherapy and 1% clindamycin gel monotherapy for the treatment of moderate acne vulgaris. Background and purpose: Group B Streptococcus (GBS), or Streptococcus agalactiae, is a major cause of invasive diseases such as meningitis and septicemia in newborns, as well as various diseases in pregnant women and non-pregnant adults with underlying conditions. This bacterium colonizes the vagina and rectum in 10% to 30% of pregnant women. GBS colonization in pregnant women increases the risk of preterm delivery and transmission of the bacteria to the baby during childbirth. Penicillin is the first-line antibiotic for treating GBS infections, while macrolides (e.g., erythromycin), lincosamides (e.g., clindamycin), and streptogramins have been recommended as alternative treatments. However, the prevalence of antibiotic resistance is rising and could lead to significant clinical challenges. Therefore, this study aimed to investigate the prevalence of resistance to erythromycin and clindamycin, as well as the associated resistance genes, in GBS strains isolated from pregnant women in Iran. Materials and methods: This meta-analysis examines studies focused on resistance to erythromycin and clindamycin in GBS strains isolated from pregnant women. A comprehensive search was conducted across databases, including PubMed, Scopus, Web of Science, SID (Scientific Information Database), and Google Scholar, using keywords such as Streptococcus agalactiae, Group B Streptococcus, antibiotic resistance, erythromycin, clindamycin, MLSB (macrolide, lincosamide, and streptogramin B) resistance, and related terms. Inclusion criteria included sufficient sample size, control of confounding factors, inclusion of the relevant antibiotics, and availability of full-text articles. Data were analyzed using Comprehensive Meta-Analysis software (Biostat V2.2). Heterogeneity between studies was assessed using the I2 index, and a random-effects model was applied due to heterogeneity in the results. Results: The initial database search identified 241 studies. After removing 41 duplicates, 201 studies were considered for meta-analysis. After reviewing the titles and abstracts, 158 studies were excluded for not meeting the research criteria, leaving 16 studies for final analysis. This meta-analysis investigated the prevalence of resistance to erythromycin and clindamycin, resistance phenotypes, and the genes encoding antibiotic resistance in GBS isolates from pregnant women. All studies included were cross-sectional. The pooled resistance rates for erythromycin and clindamycin were 27.9% (CI95%: 16-43.9) and 28.4% (CI95%: 17-43.5), respectively. Continuous and inducible resistance phenotypes were observed in 17.5% (CI95%: 6.2-40.4) and 10.9% (CI95%: 4.7-23.2) of the isolates. The M and L phenotypes were found in 8.1% (CI95%: 3.9-16) and 4.8% (CI95%: 2.7-8.4) of the isolates, respectively. The prevalence of resistance genes, including ermTR, ermB, and mefA, was 34% (CI95%: 27.4-41.4), 24.5% (CI95%: 3-77.1), and 6.7% (CI95%: 3.9-11.4), respectively. Conclusion: The study shows an increasing trend of erythromycin and clindamycin resistance in GBS isolates from pregnant women in Iran. Therefore, antibiotic susceptibility testing should be performed before prescribing these antibiotics to prevent the development and spread of resistant strains.

Acne and rosacea are frequently encountered in the clinical practice of a dermatologist. These diseases can develop simultaneously in the same patient. Most often, the erythematous form of rosacea is combined with acne. Because of the similarity of clinical manifestations with acne, the diagnosis of rosacea can be easily missed. Common pathogenetic factors in the 2 diseases are thought to be genetics, alterations in the microbiome, immune disorders, and skin barrier dysfunction. There are few works addressing the relationship between acne and rosacea. A recent study identified a number of common differentially expressed genes, including interleukin-1B and matrix metalloproteinase 9, and showed that gamma delta T cells may play an important role in the development of both diseases. Despite the common pathogenesis and the similarity of some clinical manifestations (papules and pustules), acne and rosacea must be distinguished. In rosacea, there is persistent facial erythema induced by vasoregulatory neutropeptides, whereas in acne, the inflammatory process is due to excess sebum and changes in the skin microbiome. Isotretinoin is used for systemic therapy of both diseases. The drug affects the level of some cytokines, including inhibiting the expression of matrix metalloproteinase 9. In addition, it reduces sebum production in acne, and due to its ability to alter the skin microenvironment, it can reduce Propionibacterium acnes in acne and Demodex folliculorum in rosacea. Daily and cumulative doses for rosacea and acne will differ, as will topical therapy for these conditions. For acne, the combination of clindamycin and benzoyl peroxide is well established for the treatment of acne, allowing rapid meaningful clinical effect. In rosacea, metronidazole in cream form, which has better tolerability, is recommended as a topical agent. In patients suffering from both diseases simultaneously, a combination of clindamycin and benzoyl peroxide should be used to relieve acute inflammation, after which it is possible to switch to metronidazole.

Abstract Background Congenital toxoplasmosis occurs when a pregnant woman becomes infected with Toxoplasma gondii (T. gondii) for the first time. Treatment typically involves antimicrobial medications, with spiramycin commonly used to prevent transmission. However, spiramycin's effectiveness is limited due to poor placental penetration. Clindamycin, another antibiotic, can cross the placenta but reaches the fetus at only half the maternal concentration. Encapsulating the drug in chitosan-coated niosomes (Cs-Nio) could enhance its effectiveness by targeting specific organs and ensuring sustained release. To address the challenges of using clindamycin, a niosome-coated chitosan system was investigated for treating congenital toxoplasmosis caused by the VEG strain of T. gondii in an animal model. Methods Pregnant mice were infected with VEG strain of T. gondii on the 12th day of pregnancy, followed by treatment with various drugs across six groups. The treatments included chitosan-coated niosomes loaded clindamycin (Cs-Nio-Cli) and other controls. Parasitological evaluations (microscopic examination and real-time PCR), along with histopathological and immunological assessments were conducted to assess treatment efficacy. Finally, statistical analysis was conducted using GraphPad Prism 8.0 and SPSS 26, comparing test and control groups with T test and Mann–Whitney test. A p ≤ 0.05 was considered statistically significant. Results The study found that treatment with Cs-Nio-Cli significantly reduced the number of T. gondii cysts in the brain and eyes (97.59% and 92.68%, respectively) compared to the negative control group. It also mitigated inflammatory changes, prevented cell death, and reduced vascular cuffs in the brain. In addition, Cs-Nio-Cli treatment decreased bleeding, placental thrombosis, and inflammatory cell infiltration in the placenta while improving eye tissue health by reducing retinal folds and bleeds. Immunologically, nanoclindamycin treatment resulted in lower TNF-α cytokine levels and higher IL-10 levels, indicating an enhanced anti-inflammatory response. Conclusions Although Cs-Nio-Cli demonstrates promise in reducing the transmission of congenital toxoplasmosis and mitigating the effects of congenital toxoplasmosis, additional research is necessary to determine the optimal treatment regimens for the complete eradication of the parasite in the fetus.
