Abstract Background: Brain concussion (BC) is seen as a public health priority due to its high incidence and morbidity rate, among thousands of people around the world. There are needs for fast identification, accurate diagnosis and correct management in order to reduce the short and long-term problems relating to BC. Proper knowledge of BC in the population and among clinicians is a critical factor in achieving this. Objectives: To evaluate the level of self-reported BC knowledge and gaps/misconceptions, and to identify variables correlated with this level. Methods: A cross-sectional descriptive survey was performed. A Brain Concussion Knowledge Questionnaire (BCKQ) that had been created to capture data was widely distributed. Total scores, domain partial scores and percentages of correct and incorrect answers were calculated to ascertain the level of knowledge relating to BC. Results: The sample was formed by 1,247 Brazilian adults (age: 41.7±11.8 years). Partial scores of the BCKQ revealed the existence of poor knowledge and misconceptions in all domains of the questionnaire, especially regarding questions about recovery from and management of BC. Moderate correlations between BCKQ scores and professions (p=0.312; P=0.00) or previous brain concussion knowledge (p=0.489; P=0.00) were observed. In a multiple linear regression model, age, profession and sports practice were predictors of BC knowledge. Conclusion: This first study to analyze the level of BC knowledge in a sample of Brazilian adults suggests that poor knowledge and misconceptions are present. Thus, meaningful and useful information was provided by this study for developing health education programs about BC for the population in order to improve fast diagnosis and correct BC management.

The study aimed to evaluate the reactivity, as well as the efficacy of stunning in cattle before and after staff training at the slaughterhouse. For reactivity evaluation, vocalization, slips, falls, use of shockers and presence of lying animal were observed for handling in the corridor, bathing station and at the end of raceway in front of the stunning box. For stunning evaluation, shots applied to each animal to complete stunning and the distance between the shot site and the correct position for stunning were measured. Through mapping the main deficiencies and failures in each handling region, a training was elaborated focused on adoption of correct handling practices. After the informational lecture, data was collected again using the same procedures and in the same handling regions. After training, the quantification of all handling variables was considerably lower in the three regions of the pre-slaughter course, and there was a significant reduction in the bathing station and at the end of raceway in front of the stunning box (P

Purpose of the study. Concussion does not cause any lesions available for visualization using computed tomography and magnetic resonance imaging. However, it can cause changes at the microstructural level, which can be detected by the diffusion-tensor imaging. The purpose of this study is to identify the effect of acute concussion on diffusion parameters in the corpus callosum, corticospinal tract, and thalamus in children.Patients and methods. Fractional anisotropy and the apparent diffusion coefficient were determined in 11 patients with a diagnosis of concussion (41 ± 19 hours from the moment of injury) and in 11 healthy subjects. Philips Achieva dStream 3T magnetic resonance imager was used. Diffusion tensor imaging data were processed in the Philips Intellispace Portal program in the Fibertrack section.Results. Fractional diffusion anisotropy significantly increases and the apparent diffusion coefficient decreases in the thalamus of patients with concussion. In corpus callosum there is a growth trend in fractional anisotropy.Conclusion. The detected changes indicate the initial stage of cell edema in the thalamus caused by concussion. Diffusion-tensor imaging is the only magnetic resonance imaging method which may be sensitive to this pathology.

We investigated differences in the ascending reticular activating system (ARAS) injuries between patients with mild traumatic brain injury (mTBI) and cerebral concussion by using diffusion tensor tractography (DTT).

The management of concussion in pediatric patients has always been guided by treatment guidelines that have been drawn from consensus statements rather than clinical research projects. Grool and colleagues conducted a clinical research project on an early return to physical activity and its effect on post-concussion symptoms. The study enrolled 3063 pediatric patients, age 5.0 to 17.99 years of age who presented to one of nine Pediatric Emergency Research Centers in Canada. 2413 patients completed the primary outcome for exposure. A total of 1677 patients (69.5%) instituted some level of early physical activity, including light aerobic exercise (n = 795 or 32.9%), sport-specific exercise (n = 214 or 8.9%), non-contact drills (n = 143 or 5.9%), full-contact practice (n = 106 or 4.4%), or full competition (n = 419 or 17.4%), while 736 (30.5%) instituted no physical activity. Patients were evaluated by a web-based survey or a telephone-based survey at days 7 and 28 after their initial visit, and their symptoms were evaluated by using the Post Concussive Symptom Inventory (PCSI). Early return to physical activity was associated with a lower risk of Persistent Post-Concussive Syndrome (PPCS) than in patients reporting no physical activity at 28 days (24.6% vs. 43.5%, Absolute risk difference, (ARD), 18.9% (95% CI 14.7-23.0%). Among the sub-group of patients who were symptomatic at day 28 (n = 803), PPCS was more present in the patients that reported no physical activity, (n = 584: PPCS 52.9%), than those with light aerobic activity (n = 494 [46.4%; ARD, 6.5%; 95% CI 5.7-12.5%], moderate activity [n = 176 (38.6%; ARD, 14.3%; 95% CI 5.9%-22.2%], or full contact activity [n = 133 (36.1%; ARD, 16.8%; 95% CI 7.5%- 25.5%]. Therefore, in patients aged 5 to 18 years with acute concussion, a return to physical activity within 7 days of acute injury was associated with less symptoms at 28 days than in patients who had a period of prolonged rest rather than an early return to physical activity. Objective: To test the hypothesis that mild cognitive impairment (MCI) rates are higher among retired professional contact sport athletes than in noncontact athlete controls and compare history of contact sports with other MCI risk factors. Setting: University Concussion Management Clinic. Participants: Twenty-one retired National Football League and National Hockey League players and 21 aged-matched noncontact athlete controls. Designs: Case-control. Main Measures: Comprehensive criteria were used to assess MCI based on the following: Wisconsin Card Sorting Test, Delis-Kaplan Executive Function System; Trail Making Parts A and B; Wechsler Adult Intelligence Scale--Third Edition subtests; Neuropsychological Assessment Battery Memory Module List Learning, Story Learning, and Naming subtests; and Controlled Oral Word Association Test. The Wide Range Achievement Test was used as a proxy measure for IQ. Atherosclerotic cardiovascular disease risk factors were self-reported and blood cholesterol was measured. Depression was measured by the Beck Depression Inventory-II (BDI). Results: Eight contact sport athletes (38%) and 3 noncontact athletes (14%) met MCI criteria (P = .083). Contact sport athletes' scores were significantly worse on Letter Fluency and List B Immediate Recall. Contact athletes were more obese, had more vascular risk factors, and had higher scores on the BDI than controls. Conclusion: Athletes with a history of playing professional contact sports had more vascular risk factors and higher depression scores. MCI rates were somewhat higher, though not significant. Objective: To prospectively examine gait characteristics of participants acutely after concussion with and without receded near point of convergence (NPC), compared with healthy controls. Design: Cross-sectional study. Setting: Sports-medicine clinic. Participants: Patients examined after concussion (n = 33; mean ± SD = 7.2 ± 3.1 days) and a group of uninjured athletes (n = 31) completed a Postconcussion Symptom Scale, underwent NPC testing, and single/dual-task gait assessments. Independent Variables: Near point of convergence was defined as the patient-reported diplopia distance when a fixation target moved toward the nose. Receded NPC was defined as a distance >5 cm from the tip of the nose. Main Outcome Measures: Spatiotemporal gait characteristics in single-task and dual-task conditions were evaluated with analysis of variance; correlations were calculated between NPC and gait measures. Results: Eighteen of 33 (55%) patients with concussion presented with receded NPC. Those with receded NPC exhibited slower gait speed (single-task = 1.06 ± 0.14 m/s vs 1.19 ± 0.15 m/s; dual-task = 0.80 ± 0.13 m/s vs 0.94 ± 0.13 m/s; P = 0.003) and shorter stride lengths (single-task = 1.11 ± 0.10 m vs 1.24 ± 0.11 m; dual-task = 0.97 ± 0.11 m vs 1.09 ± 0.11 m; P = 0.001) than healthy controls. Near point of convergence was moderately correlated with dual-task average walking speed for the normal NPC group (ρ = -0.56; P = 0.05). Postconcussion Symptom Scale scores did not significantly differ between groups (27 ± 18 vs 28 ± 16). Conclusions: After concussion, adolescents with receded NPC exhibited significant gait-related deficits compared with healthy controls, whereas those with normal NPC did not. Concussion, a peculiar type of mild traumatic brain injury (mTBI), is an injury frequently encountered in various contact and noncontact sports, such as boxing, martial arts, American football, rugby, soccer, ice hockey, horse riding, and alpine skiing. Concussion occurs anytime external forces of specific intensities provoke acceleration−deceleration of the brain, and it is characterized by the rapid onset of short-lived impairment of neurologic functions, spontaneously resolving within weeks, persisting for longer times only in a small percentage of cases. A wide range of molecular alterations, including mitochondrial dysfunction, energy deficit, and gene and protein expression changes, is triggered by concussion and lasts longer than clinical symptoms. In recent years, concussion has become a primary issue of discussion among sports medicine professionals, athletes, media, and sports sponsors in relation to athletes’ return to play, after a concussion. Continued improvement in prevention and management of concussed athletes requires extensive research from different disciplines. Research work needs to focus on both prevention and management. Researchers and clinicians’ efforts should be dedicated to a better understanding of the molecular changes occurring in the post-concussed brain and to clearly define healing after concussion for a safe return of athletes to play. It is essential for sports medicine professionals to stay informed about the advances in understanding concussions and how to rehabilitate each single player who sustained a concussion.

This study investigated the characteristics of the small-world brain network architecture of patients with mild traumatic brain injury (MTBI), and a correlation between brain functional connectivity network properties in the resting-state fMRI and Standardized Assessment of Concussion (SAC) parameters. The neurological conditions of 22 MTBI patients and 17 normal control individuals were evaluated according to the SAC. Resting-state fMRI was performed in all subjects 3 and 7 days after injury respectively. After preprocessing the fMRI data, cortex functional regions were marked using AAL90 and Dosenbach160 templates. The small-world network parameters and areas under the integral curves were computed in the range of sparsity from 0.01 to 0.5. Independent-sample t- tests were used to compare these parameters between the MTBI and control group. Significantly different parameters were investigated for correlations with SAC scores; those that correlated were chosen for further curve fitting. The clustering coefficient, the communication efficiency across in local networks, and the strength of connectivity were all higher in MTBI patients relative to control individuals. Parameters in 160 brain regions of the MTBI group significantly correlated with total SAC score and score for attention; the network parameters may be a quadratic function of attention scores of SAC and a cubic function of SAC scores. MTBI patients were characterized by elevated communication efficiency across global brain regions, and in local networks, and strength of mean connectivity. Objective: Postural stability is often affected by sport-related injuries and subsequently evaluated during postinjury examinations. Intrinsic factors, however, may also affect postural control. We sought to compare the postural control of female and male athletes as measured simultaneously by (1) the modified balance error scoring system (mBESS) and (2) a video-force plate system. Design: Cross-sectional study. Setting: Sports injury prevention center. Participants: Pediatric, adolescent, and young adult athletes who performed mBESS during an injury prevention evaluation. Independent Variables: We compared the postural control of female and male athletes. We also accounted for independent variables associated with postural stability, including age, body mass index, and history of ankle injury, concussion, and migraine headache. Main Outcome Measures: Total errors committed during the mBESS and measurements derived from integrated kinematic and kinetic data obtained by a video-force plate system. Differences between males and females were tested using analysis of covariance. Results: Participants (n = 409) ranged in age from 10 to 29 years (mean = 14.6 ± 2.8); 60% were female. No significant differences on mBESS were detected between females and males; however, female athletes demonstrated significantly better postural stability on the video-force plate analysis during double-leg (P = 0.03, d = 0.28), single-leg (P < 0.001, d = 0.62), and tandem stances (P < 0.001, d = 0.53) when compared with males. Conclusions: Uninjured female athletes demonstrate better postural stability on video-force plate analysis than their uninjured male counterparts. Behçet disease (BD) is rare in childhood. We report a 9-year-old boy with neuro-Behçet disease who presented diplopia and weakness on the left side after a cerebral concussion. Brain magnetic resonance imaging (MRI) revealed hyperintensity of the right mesodiencephalic junction on T2-weighted and fluid attenuated inversion recovery images. Prednisolone administration resulted in complete remission and normalization of abnormal MRI finding. Brain MRI is a useful diagnostic tool when the neurological sign is the first symptom of subclinical BD.

Objectives: The Balance Error Scoring System (BESS) has been documented as a useful way to evaluate postural control following sport-related concussions sustained by youth athletes. However, limitations have been reported with its use due to the reliance on visual observation as the primary measurement outcome. The primary purpose of this study was to examine the correlation between the modified BESS (mBESS) as rated by a clinician and a simultaneous analysis performed by an integrated video-force plate system. The secondary purpose was to assess if a history of prior concussion affected postural control. Methods: A group of healthy youth athletes (n = 398; mean age 13.7 ± 2.4 years) completed the mBESS while simultaneously undergoing an integrated video-force plate evaluation to measure postural stability. Spearman rank-order correlations were used to determine the strength of correlation between the 2 rating systems. In addition, performance on the mBESS between those with and without a history of concussion was compared using univariate ANCOVAs. Results: A moderately high correlation was found during single-leg stance (ρ = -0.64,p< .001), while a weak correlation was found during tandem stance (ρ = -0.30,p< .001). No postural control differences were found between groups with and without a concussion history. Conclusion: The video-force plate rating system correlates well with the clinician rating during the single-leg stance of the mBESS, but not during double-leg or tandem stances. Objective: The effect of physical activity resumption on functional recovery following concussion is poorly understood. We sought to examine the relationship between physical activity resumption timing and gait stability 2 months following concussion. Setting: Motion analysis laboratory. Participants: Twenty-nine individuals who sustained a concussion and were subsequently allowed to return to preinjury levels of physical activity within 2 months postinjury. Design: Prospective cohort study. Main Measures: The return-to-activity day (RTA-day) was identified as the first day postinjury that each participant was medically cleared to resume regular physical activity. Whole-body center-of-mass (COM) motion during single-task and dual-task walking was assessed 2 months postinjury. Correlation coefficients were calculated between the RTA-day and single/dual-task gait stability as well as symptom severity measurements 2 months postinjury. Results: Dual-task medial-lateral COM displacement (r = -0.52, P = .004) and peak medial-lateral COM velocity (r = -0.37, P = .048) were significantly correlated with RTA-day. Dual-task peak forward velocity and single-task gait stability measures were not significantly correlated with RTA-day. Conclusions: The time of RTA-day clearance, within a 2 months postinjury period, is significantly correlated with dual-task medial-lateral gait stability measured at the end of that period, suggesting that frontal plane gait stability recovery is sensitive to the timing of RTA-day. Objective: Various concussion characteristics and personal factors are associated with cognitive recovery in athletes. We developed an index based on concussion frequency, severity, and timeframe, as well as cognitive reserve (CR), and we assessed its predictive power regarding cognitive ability in retired professional football players.Method: Data from 40 retired professional American football players were used in the current study. On average, participants had been retired from football for 20 years. Current neuropsychological performances, indicators of CR, concussion history, and play data were used to create an index for predicting cognitive outcome.Results: The sample displayed a range of concussions, concussion severities, seasons played, CR, and cognitive ability. Many of the participants demonstrated cognitive deficits. The index strongly predicted global cognitive ability (R2= .31). The index also predicted the number of areas of neuropsychological deficit, which varied as a function of the deficit classification system used (Heaton:R2= .15; Wechsler:R2= .28).Conclusions: The current study demonstrated that a unique combination of CR, sports concussion, and game-related data can predict cognitive outcomes in participants who had been retired from professional American football for an average of 20 years. Objective: To determine the compliance of schools and school nurses in the United States with national recommendations for cognitive rest in students who sustain a concussion. Design: Cross-sectional questionnaire based. Participants: Members of the National Association of School Nurses working at the high school level. Intervention: A questionnaire, developed by the authors and based on recommendations for cognitive rest, was electronically distributed 3 times during the 2012 to 2013 academic year. Main Outcome Measures: Self-reported responses were collected regarding demographics and compliance of schools and school nurses with recommendations for the management of the postconcussion student, including the presence of specific guidelines for individualized care and the responsibility of the nurse for the prevention, detection, and management of concussions. Results: Analysis was performed on 1033 completed questionnaires (36% usable response rate). Fifty-three percent of schools have guidelines to assist students when returning to school after a concussion. These guidelines include extension of assignment deadlines (87%), rest periods during the school day (84%), postponement or staggering of tests (75%), reduced workload (73%), and accommodation for light or noise sensitivity (64%). Sixty-six percent of nurses in our sample have had special training in the recognition and management of concussions. Nurses reported involvement in the following roles: identifying suspected concussions (80%), providing emotional support for recovering students dealing with concussion-related depression (59%), and guiding the student's postconcussion graduated academic and activity re-entry process (58%). Conclusions: We detected a wide variability in compliance of schools and school nurses with national recommendations for cognitive rest. Clinical Relevance: Ensuring that schools have policies established for a student's return to learning, having specific guidelines to provide an individualized approach to return to learning based on postconcussion signs/symptoms, training school nurses in the recognition and management of concussions, and involving school nurses in the re-entry process are identified areas for improvement. Background: Age has been described as a factor that affects recovery after concussion. The recommended management protocol is to treat adolescents in a more cautious manner than adults. However, few studies have prospectively and longitudinally assessed the way these age groups perform on motor tasks after concussion. Purpose: To examine dual-task gait balance control deficits after concussion in a group of adolescents and young adults in reference to matched control subjects within 72 hours of injury and throughout 2 months after injury. Study Design: Cohort study; Level of evidence, 2. Methods: Adolescents and young adults who sustained a concussion and individually matched controls completed a whole-body motion gait analysis while simultaneously performing a cognitive task. Subjects with concussion reported to the laboratory within 72 hours after injury and at the following time points: 1 week, 2 weeks, 1 month, and 2 months after injury. Control subjects completed the same protocol at similar time points. Gait balance control measurements included whole-body center-of-mass (COM) medial-lateral displacement/velocity and anterior velocity. Results: A total of 38 subjects with concussion, 19 young adults (mean ± SD age, 20.3 ± 2.4 years) and 19 adolescents (mean ± SD age, 15.1 ± 1.1 years), and 38 individually matched control subjects were tested. Within 72 hours of injury, adolescents displayed significantly greater COM medial-lateral displacement (P = .001) and peak velocity (P = .001) relative to their control group, and the young adult concussion group displayed significantly less peak COM anterior velocity than their control group (P = .01). Across the 2 months of testing, adolescents with concussion displayed significantly greater total COM medial-lateral displacement than did adolescent controls (P = .001), while young adults with concussion did not significantly differ from their matched controls (P = .07). Conclusion: An examination of gait balance control during dual-task walking revealed that after concussion, in reference to matched controls, adolescents demonstrated greater gait balance control deficits than did young adults initially and throughout the 2-month postinjury period, supporting the recommendation of conservative management for adolescents after concussion. He offers insights into the significance for clinicians to be familiar on the characterization and natural history of PTHA. The present study sought to evaluate the Wechsler Test of Adult Reading (WTAR) Full-Scale IQ (FSIQ) estimate as an index of premorbid ability in a sample 574 of healthy college athletes participating in a sports concussion management program. We compared baseline neuropsychological test performance with the WTAR FSIQ estimate obtained at baseline. Results revealed that the discrepancy between actual neuropsychological test scores and the WTAR FSIQ estimate was greatest for those with estimated FSIQs greater than 107. The clinical implication of this finding was evaluated in the 51 participants who went on to sustain a concussion. For individuals with higher IQ estimates, the WTAR estimate obtained post-concussion suggested greater post-concussion decline than that indicated by comparison with actual baseline neuropsychological performance. Abstract: Fields studying movement generation, including robotics, psychology, cognitive science, and neuroscience utilize concepts and tools related to the pervasiveness of variability in biological systems. The concept of variability and the measures for nonlinear dynamics used to evaluate this concept open new vistas for research in movement dysfunction of many types. This review describes innovations in the exploration of variability and their potential importance in understanding human movement. Far from being a source of error, evidence supports the presence of an optimal state of variability for healthy and functional movement. This variability has a particular organization and is characterized by a chaotic structure. Deviations from this state can lead to biological systems that are either overly rigid and robotic or noisy and unstable. Both situations result in systems that are less adaptable to perturbations, such as those associated with unhealthy pathological states or absence of skillfulness. [Copyright &y& Elsevier]

Some individuals suffering from mild traumatic brain injuries, especially repetitive mild concussions, are thought to develop a slowly progressive encephalopathy characterized by a number of the neuropathological elements shared with various neurodegenerative diseases. A central pathological mechanism explaining the development of progressive neurodegeneration in this subset of individuals has not been elucidated. Yet, a large number of studies indicate that a process called immunoexcitotoxicity may be playing a central role in many neurodegenerative diseases including chronic traumatic encephalopathy (CTE). The term immunoexcitotoxicity was first coined by the lead author to explain the evolving pathological and neurodevelopmental changes in autism and the Gulf War Syndrome, but it can be applied to a number of neurodegenerative disorders. The interaction between immune receptors within the central nervous system (CNS) and excitatory glutamate receptors trigger a series of events, such as extensive reactive oxygen species/reactive nitrogen species generation, accumulation of lipid peroxidation products, and prostaglandin activation, which then leads to dendritic retraction, synaptic injury, damage to microtubules, and mitochondrial suppression. In this paper, we discuss the mechanism of immunoexcitotoxicity and its link to each of the pathophysiological and neurochemical events previously described with CTE, with special emphasis on the observed accumulation of hyperphosphorylated tau. Transcranial color duplex scanning of the cerebral vessels was used to study arterial and venous reactivity in 38 patients in the acute period of cerebral concussion (CC) and in 32 healthy volunteers. Arterial flow was assessed in the middle cerebral artery (MCA) and venous drainage in the basal vein of Rosenthal (BV). Cerebrovascular reactivity was assessed using hypercapnic and orthostatic tests. These studies showed that CC was not accompanied by marked changes in cerebral hemodynamics in the state of rest. During the acute phase of CC, about 20% of patients showed increases in peak blood flow velocity in the MCA, typical of cerebral hyperperfusion. Increases in the peak blood flow velocity in the BV were seen in 25% of patients with CC, compensating for impaired drainage via the superficial cerebral system. In normal subjects, cerebral venous reactivity to hypercapnia was greater than arterial reactivity, while reactivity to orthostasis corresponded to the magnitudes of arterial changes. The absence of quantitative differences in the responses of arterial and venous blood flow to hypercapnia and the predominance of venous reactivity to orthostasis in patients with CC indicates that these patients had impairments in the regulation of venous tone. Abstract: Postural control assessments are commonly administered to athletes as part of a pre-season screening. Establishing a baseline level of function permits the clinician to compare post-injury results to normal functioning during the return to play decision-making process. In the athletic setting, follow-up tests may be completed on the sideline immediately following injury. We sought to examine the effect of commonly administered external ankle joint support on postural control using the balance error scoring system (BESS) and the NeuroCom sensory organization test (SOT). Nineteen volunteers free from balance issues completed three sessions with varied ankle support: bilateral prophylactic ankle taping, laced bracing device, or barefoot. Each session included an initial balance assessment on the BESS and SOT, a 20min treadmill walk, and post-walk balance test. Fewer errors, indicating improved balance, were committed on the BESS during the barefoot condition than the braced ankle condition (p =0.044) at the pre-walk assessment. During the post-walk assessment, fewer errors were committed during the barefoot condition compared to the braced ankle condition (p =0.034) and the taped ankle condition (p =0.037). All ankle support conditions showed similar improvements in balance between the pre and post-walk assessments on the BESS (p <0.001) and SOT composite balance score (p =0.009). These findings indicate that ankle support devices may influence postural control on the BESS, but not on the NeuroCom SOT. Clinicians using the BESS as a balance assessment device at multiple time points should be consistent in the application of ankle support devices. [Copyright &y& Elsevier]

Abstract: The management of concussion and mild traumatic brain injury is an area of clinical uncertainty for many sports medicine professionals. While recent studies suggest sex differences in neurocognitive function may exist, our understanding of the effects of menstrual cycle phase and oral contraceptive pill (OCP) use in healthy females is limited. The purpose of this study was to investigate whether there were changes in neurocognitive function, postural stability and self-reported symptoms between the early and late stages of the menstrual cycle, and also to identify whether performance across menstrual cycle would differ between females using OCP and eumenorrheic females not using OCP. Healthy college-aged females completed a standard concussion test battery including computerised assessment of neurocognitive function, postural stability and symptom status. Participants completed the counterbalanced testing design during the early and late phases of their menstrual cycle. No significant main effects or interactions for any neurocognitive function or postural stability outcome measure were observed. Eumenorrheic females endorsed a higher number of symptoms and reported an increased symptom severity score compared to females using OCP. Menstrual cycle phase had no effect on the total severity or the number of symptoms endorsed. These results suggest preseason neurocognitive and postural stability baseline tests are stable across the menstrual cycle, regardless of OCP use. Therefore, decreased performance on these measures following a suspected concussion is likely not attributable to menstrual cycle phase or use of OCP. [Copyright &y& Elsevier]

Summary: Introduction: mild head trauma (MHT) is defined as a transient neurological deficit after trauma with a history of impairment or loss of consciousness lasting less than 15min and/or posttraumatic amnesia, and a Glasgow Coma Scale between 13 and 15 on hospital admission. We evaluated 50 MHT patients 18 months after the trauma, addressing signs and symptoms of post-concussion syndrome, quality of life and the presence of anxiety and depression. We correlate those findings with the S100B protein levels and cranial CT scan performed at hospital admission after the trauma. Method: patients were asked to fill out questionnaires to assess quality of life (SF36), anxiety and depression (HADS), and signs and symptoms of post-concussion syndrome. For the control group, we asked the patient''s household members, who had no history of head trauma of any type, to answer the same questionnaires for comparison. Results: total quality of life index for patients with MHT was 58.16 (±5), lower than the 73.47 (±4) presented by the control group. Twenty patients (55.2%) and four (11.1%) controls were depressed. Seventeen patients (47.2%) presented anxiety, whereas only eight (22.2%) controls were considered anxious. Victims of MHT complained more frequently of loss of balance, dry mouth, pain in the arms, loss of memory and dizziness than their respective controls (p <0.05). We found no correlation between the presence of these signs and symptoms, quality of life, presence of anxiety and depression with S100B protein levels or with presence of injury in the cranial CT performed at hospital admission. Conclusion: MHT is associated with a higher incidence of post-concussion syndrome symptoms, lower quality of life and anxiety than their respective controls even 18 months after the trauma. The article reports on a study which assess the differential recovery of behavioral status and brain function after mild traumatic brain injury (MTBI) in a rat through functional magnetic resonance imaging. Results revealed that despite normal appearing diffusion weighted imaging and T2-weighted imaging finding following MTBI, persistent hypoperfusion developed. It is concluded that the techniques may provide great potentional for understanding MTBI.

Context: The return-to-play decision after sport-related cerebral concussion depends in part on knowing when an athlete has fully recovered postural control after injury. Objective: To describe the postconcussion recovery of postural control using approximate entropy (ApEn), a regularity statistic from nonlinear dynamics. Design: Retrospective case series analysis. Setting: Sports medicine research laboratory. Patients or Other Participants: Collegiate athletes from whom center-of-pressure and symptom data were collected at preseason, less than 48 hours after injury, and 48 to 96 hours after injury. Main Outcome Measure(s): Approximate entropy values reflecting the amount of randomness contained in center-of-pressure oscillations were calculated for anterior-posterior (AP) and medial-lateral (ML) time series. Equilibrium scores reflecting the amplitude of center-of-pressure AP oscillations were used to indicate postural stability. The number and severity of symptoms were described. Results: Compared with the healthy preseason state, ApEn values for the AP and ML time series generally declined immediately after injury in both steady and unsteady injured athletes. At 48 to 96 hours after injury, ApEn values for the ML time series remained significantly depressed (mean difference compared with preseason = -0.268, standard error = 0.072), even among athletes whose initial postural instability had resolved. We found few significant relationships between changes in ApEn values and changes in symptoms before and after injury. Conclusions: The effects of cerebral concussion on postural control appear to persist for longer than 3 to 4 days, even among athletes with no signs of unsteadiness. Our results may reflect changes in neurophysiologic or mechanical constraints on postural control. Purpose: To validate a correction factor for the extracranial release of the astroglial protein, S-100B, based on concomitant creatine kinase (CK) levels. Methods: The CK- S-100B relationship in non-head injured marathon runners was used to derive a correction factor for the extracranial release of S-100B. This factor was then applied to a separate cohort of 96 mild traumatic brain injury (TBI) patients in whom both CK and S-100B levels were measured. Corrected S-100B was compared to uncorrected S-100B for the prediction of initial head CT, three-month headache and three-month post concussive syndrome (PCS). Results: Corrected S-100B resulted in a statistically significant improvement in the prediction of 3-month headache (area under curve [AUC] 0.46 vs 0.52, p=0.02), but not PCS or initial head CT. Using a cutoff that maximizes sensitivity (&ges;90%), corrected S-100B improved the prediction of initial head CT scan (negative predictive value from 75% [95% CI, 2.6%, 67.0%] to 96% [95% CI: 83.5%, 99.8%]). Conclusions: Although S-100B is overall poorly predictive of outcome, a correction factor using CK is a valid means of accounting for extracranial release. By increasing the proportion of mild TBI patients correctly categorized as low risk for abnormal head CT, CK-corrected S100-B can further reduce the number of unnecessary brain CT scans performed after this injury. Minor Traumatic Brain Injury (mTBI) is caused by the inertial effect of a mechanical impact to the head with sudden rotational acceleration forces. MTBI produces, in the less severe cases, only transient disturbances of ionic homeostasis with temporary disturbances of brain function. Depending on the severity of the trauma, animal and human studies have demonstrated focal intraaxonal alterations in neurofilamentous/cytoskeletal network and impairment of axoplasmic transport, which may lead to progressive axonal swelling, detachment or even cell death over a period of hours or days, the so-called process of delayed axotomy. Disturbances of ionic homeostasis, acute metabolic changes and cerebral blood flow alterations compromise the ability of neurons to function and render brain cells vulnerable. These processes may predispose brain cells to a vulnerable state for an undetermined period; therefore, it is recommended that any confused player with or without amnesia should be taken off the ice and not be permitted to play again for at least 72 h. The activity of mitochondrial enzymes (succinic dehydrogenase and cytochrome oxidase) and enzymes associated with blood-brain barrier function (butyrylcholinesterase and alkaline phosphatase) in the CNS of rats was studied from 5 minutes to 62 hours after cerebral concussion. There was a transient increase in succinic dehydrogenase activity during the first hour after concussion in the neurons of the structures close to the impact. The alkaline phosphatase activity, strongly positive in the walls of normal blood vessels, decreased within five minutes after concussion; it virtually disappeared in 15 minutes but returned to normal level after 62 hours.
