- Abstract
Background: Chest compression is a lifesaving intervention in out-of-hospital cardiac arrest (OHCA), but the optimal metrics to assess its quality have yet to be identified. The objective of this study was to investigate whether a new parameter, that is, the variability of the chest compression-generated transthoracic impedance (TTI), namely ImpCC, which measures the consistency of the chest compression maneuver, relates to resuscitation outcome. Methods: This multicenter observational, retrospective study included OHCAs with shockable rhythm. ImpCC variability was evaluated with the power spectral density analysis of the TTI. Multivariate regression model was used to examine the impact of ImpCC variability on defibrillation success. Secondary outcome measures were return of spontaneous circulation and survival. Results: Among 835 treated OHCAs, 680 met inclusion criteria and 565 matched long-term outcomes. ImpCC was significantly higher in patients with unsuccessful defibrillation compared to those with successful defibrillation (p =.0002). Lower ImpCC variability was associated with successful defibrillation with an odds ratio (OR) of 0.993 (95% confidence interval [95% CI], 0.989–0.998, p =.003), while the standard chest compression fraction (CCF) was not associated (OR 1.008 [95 % CI, 0.992–1.026, p =.33]). Neither ImpCC nor CCF was associated with long-term outcomes. Conclusions: In this population, consistency of chest compression maneuver, measured by variability in TTI, was an independent predictor of defibrillation outcome. ImpCC may be a useful novel metrics for improving quality of care in OHCA.

Background: The role of neuromuscular blocking agents (NMBAs) in coronavirus disease 2019 (COVID-19) acute respiratory distress syndrome (ARDS) is not fully elucidated. Therefore, we aimed to investigate in COVID-19 patients with moderate-to-severe ARDS the impact of early use of NMBAs on 90-day mortality, through propensity score (PS) matching analysis. Methods: We analyzed a convenience sample of patients with COVID-19 and moderate-to-severe ARDS, admitted to 244 intensive care units within the COVID-19 Critical Care Consortium, from February 1, 2020, through October 31, 2021. Patients undergoing at least 2 days and up to 3 consecutive days of NMBAs (NMBA treatment), within 48 h from commencement of IMV were compared with subjects who did not receive NMBAs or only upon commencement of IMV (control). The primary objective in the PS-matched cohort was comparison between groups in 90-day in-hospital mortality, assessed through Cox proportional hazard modeling. Secondary objectives were comparisons in the numbers of ventilator-free days (VFD) between day 1 and day 28 and between day 1 and 90 through competing risk regression. Results: Data from 1953 patients were included. After propensity score matching, 210 cases from each group were well matched. In the PS-matched cohort, mean (± SD) age was 60.3 ± 13.2 years and 296 (70.5%) were male and the most common comorbidities were hypertension (56.9%), obesity (41.1%), and diabetes (30.0%). The unadjusted hazard ratio (HR) for death at 90 days in the NMBA treatment vs control group was 1.12 (95% CI 0.79, 1.59, p = 0.534). After adjustment for smoking habit and critical therapeutic covariates, the HR was 1.07 (95% CI 0.72, 1.61, p = 0.729). At 28 days, VFD were 16 (IQR 0–25) and 25 (IQR 7–26) in the NMBA treatment and control groups, respectively (sub-hazard ratio 0.82, 95% CI 0.67, 1.00, p = 0.055). This article examines Marina Warner’s radio play Birgitta’s Cell (BBC Radio 4), later published as Brigit’s Cell (2010) focusing on the themes of memory and desire. By drawing on Jan Assman’s theories on collective memory, the article shows how the two time levels Warner builds in the radio drama succeed in creating a fresh connection between women in the past and in the present as well as between individual and collective experience. We shall examine the relationship between Roberts's "new" saints and food and the desecratory, macabre representations that the author offers of the sacred meal.

WHAT WE ALREADY KNOW ABOUT THIS TOPIC: After pulmonary artery occlusion (mimicking a pulmonary embolism), perfusion is redistributed to the rest of the lung tissue, but the distribution of ventilation is uncertain. WHAT THIS ARTICLE TELLS US THAT IS NEW: Data from anesthetized pigs (uninjured lungs) indicate that the perfusion is redistributed as suspected. Similarly, ventilation is redistributed from nonperfused to perfused lung tissue. This limits the increase in dead space and is accompanied by less density in the occluded lung. BACKGROUND: Acute unilateral pulmonary arterial occlusion causes ventilation-perfusion mismatch of the affected lung area. A diversion of ventilation from nonperfused to perfused lung areas, limiting the increase in dead space, has been described. The hypothesis was that the occlusion of a distal branch of the pulmonary artery would cause local redistribution of ventilation and changes in regional lung densitometry as assessed with quantitative computed tomography. METHODS: In eight healthy, anesthetized pigs (18.5 ± 3.8 kg) ventilated with constant ventilatory settings, respiratory mechanics, arterial blood gases, and quantitative computed tomography scans were recorded at baseline and 30 min after the inflation of the balloon of a pulmonary artery catheter. Regional (left vs. right lung and perfused vs. nonperfused area) quantitative computed tomography was performed. RESULTS: The balloon always occluded a branch of the left pulmonary artery perfusing approximately 30% of lung tissue. Physiologic dead space increased (0.37 ± 0.17 vs. 0.43 ± 0.17, P = 0.005), causing an increase in PaCO2 (39.8 [35.2 to 43.0] vs. 41.8 [37.5 to 47.1] mmHg, P = 0.008) and reduction in pH (7.46 [7.42 to 7.50] vs. 7.42 [7.38 to 7.47], P = 0.008). Anorectal manometry provides an objective assessment of anal sphincter pressure and rectal sensitivity and anorectal reflexes in response to distension. However, its clinical utility is hampered by a lack of standardized protocols and normative data from healthy subjects. Previous studies have used water-perfused systems in normal subjects, but some adopted a rapid pull-through technique; others did not evaluate rectal sensations and others did not carefully exclude patients with functional bowel disorders. Objective To evaluate anorectal function in healthy adults without functional bowel disorders, using a water-perfused system with the stationary technique in order to obtain normative values for anorectal manometry. Method Fifty-two healthy volunteers with no Rome II diagnostic criteria for functional bowel disorders, including only nulliparous women, underwent anorectal manometry with a water-perfused system, according to a standardized protocol. Results Maximum squeeze pressure of the anal sphincter as well as the area under the pressure-time curve during squeeze was significantly lower in women than men (P < 0.01), while sphincter length, resting pressure, volume thresholds for reflex inhibitory recto-anal and rectal sensations were similar. Conclusions This study describes a protocol for stationary anorectal manometry using a water-perfused system, and a method for analysing the various parameters obtained during the procedure, as recently suggested in the international literature. It supplies normative data obtained in a population of healthy subjects including nulliparous women, with no functional bowel disorders.

Quantitative analysis of the nonperfused lung area revealed a significant reduction in lung density (−436 [−490 to −401] vs. −478 [−543 to −474] Hounsfield units, P = 0.016), due to a reduction in lung tissue (90 ± 23 vs. 81 ± 22 g, P < 0.001) and an increase in air volume (70 ± 22 vs. 82 ± 26 ml, P = 0.022). Conclusions Regional pulmonary vascular occlusion is associated with a diversion of ventilation from nonperfused to perfused lung areas. This compensatory mechanism effectively limits ventilation perfusion mismatch. Quantitative computed tomography documented acute changes in lung densitometry after pulmonary vascular occlusion. In particular, the nonperfused lung area showed an increase in air volume and reduction in tissue mass, resulting in a decreased lung density.

Un percorso di consapevolezza legato ad una pratica auto-osservativa e autoriflessiva all’interno del proprio contesto di lavoro è di grande importanza per la formazione personale e per un apprendimento organizzativo; implica infatti, la comprensione dei processi cognitivi individuali, organizzativi o istituzionali in cui si è immersi e costituisce uno spazio indispensabile perchè avvengano degli “apprendimenti di secondo livello”. Ciò comporta lavorare sul piano dei significati, non solo con quelli espliciti propri e altrui, ma anche con le premesse che danno origine a prassi comuni e condivise . Fondamentale in questo senso è la dimensione di gruppo perché consente di adottare una logica plurale invece che singolare e contribuisce a creare una cultura condivisa , indispensabile per progredire nel processo di cambiamento. In un servizio educativo tutto questo può avvenire grazie ad un processo di “valutazione di contesto”: si tratta di una pratica emancipativa e comunitaria nello stesso tempo, che ha delle ricadute evidenti nel miglioramento delle pratiche educative in un processo temporale continuo che è legato alla dimensione “meta”, cioè al fatto che si impara in modo permanente a mettere in discussione le proprie premesse e le proprie griglie osservative riflettendo su di sé e valutando le proprie azioni educative, anche in termini di gruppo di lavoro. A process of awareness linked to a self-observing and self-reflective practice within one's work context is very important for a personal training and organizational learning; in fact, it implies the understanding of the individual, organizational or institutional cognitive processes in which one is immersed and constitutes an indispensable space for “second level learning” to take place. This involves working on the level of meanings, not only with one's own and others' explicit ones, but also with the premises that give rise to common and shared practices. In this sense, the group dimension is fundamental because it allows the adoption of a plural rather than singular logic and helps to create a shared culture, indispensable for the changement. In an educational service this can happen thanks to a process of "context evaluation": it is an emancipatory and community practice at the same time, which has evident implications for the improvement of educational practices in a continuous temporal process that is linked to the “meta” dimension, because one learns permanently to question one's premises and observational frames by reflecting on oneself and evaluating one's educational actions, also in terms of “working group”.

This essay examines the way Marina Warner's radio play They Make a Desert and Call it Peace (2014) and its fictional reworking of contemporary news such as the Bloody Sunday report and the investigation of the Gaza Flotilla Raid of 30 May 2010. After a presentation of the development of radio plays in England, the essay shows what interest they have generated among feminist writers and how Warner uses the voice of women in her play, which also serves as a rewriting of the biblical story of Salome, to question the transmission of the memory of traumatic events such as the Bloody Sunday. The play overturns the point of view from that of the master narrative, emphasized by media and institutional voices, to that of unheard witnesses. The essay analyses the way Warner works on voices and multivocality to offer a comparison between the different points of view, that of a father, a former soldier who has taken part in violent missions, and his daughter’s: the author shows how she questions the repression systems of governments and the narration of mainstream history taking the defense of minority groups in her society.

Esse livro é uma coletânea de contos cujas personagens e ambientação refletem a parte mais marginalizada da sociedade no Brasil atual. Para analisar essa coletânea, é preciso olhar para as temáticas recorrentes nos trabalhos da autora. Conceição Evaristo utiliza o termo escrevivência para unir em uma única palavra todas as temáticas que recorrem em suas obras. Partindo dessa análise evidenciam-se, como assuntos mais frequentes, a comparação entre a vida e a morte, a condição de vida das pessoas marginalizadas, a vida das mulheres afro-brasileiras que tentam sobreviver em uma sociedade profundamente racista, a importância e a representação dos corpos delas tanto no universo cultural brasileiro quanto na literatura e o intento de elas buscar a representação da própria identidade nesse tipo de cultura. I cibi, i luoghi e le occasioni del loro consumo sono dunque mezzi, spazi e tempi elettivi della mediazione tra immanenza e trascendenza, tra al di qua e aldilà, e, al contempo, momenti elettivi di ricreazione e affermazione di un corpo sociale; e in verità non può esservi dialogo tra immanenza e trascendenza e le entità che le popolano senza che il cibo e il suo consumo ritualizzato non ne siano parte costitutiva. Su tali condivise consapevolezze antropologiche, che hanno trovato all’interno di ogni cultura una specifica articolazione legata: a specifiche condizioni ambientali che certo hanno condizionato i regimi alimentari; a peculiari sfide identitarie che hanno portato a scelte alimentari non funzionali ai contesti di vita (ma non per questo, alimentandosi l’uomo anche di simboli, meno necessarie alla sopravvivenza); alle particolari rappresentazioni del divino e dei suoi ambiti di intervento, come dimostrano i contributi a questo convegno, si è fondato per millenni il rapporto tra uomini e dèi e tra uomini ed uomini e, sia pur in certa misura e in contesti sempre più limitati, ancora oggi continua a fondarsi. Busca compreender, portanto, tais aspectos antagônicos que configuram a ambiguidade da personagem e sua relação com a percepção masculina da mulher naquele período. Procura comprovar a hipótese de que a donzela laida, a despeito da polarização característica dos estudos sobre o feminino na Demanda, em que a mulher é idealizada à luz do amor cortês ou condenada em geral por parte do cristianismo, com a exceção de Santa Maria, inscreve-se em um entrelugar marcado pelas culturas celta e cristã, dentro de um universo de evidente teor pedagógico, moralista e misógino. This article shows how Michèle Roberts portrays the characters in The Book of Mrs Noah as travellers in search for their identities and their dried out creative vein. The imaginary journey on board the Ark can be read as a process of self-empowerment and of acquisition of agency for Mrs. Noah and the other women, Roberts in fact denounces the role of the patriarchal system in silencing women’s voices in society and in literary representations.

In 1985, BBC Radio 2 broadcast the radio play The True Tale of Margery Kempe by contemporary writer Eva Figes (born 1932). The text is a remediation of The Book of Margery Kempe which recovers and gives new life to the tale of Margery Kempe re-instituting the memory of a long-lost manuscript and at the same time giving it back the original characteristics of orality. The narrative strategies adopted by Eva Figes in this adaptation for the radio of the medieval text are influenced by questions linked to the issue of the authorial voice. The transformation of the medium from written text to oral text for the radio gives Figes the opportunity, through directions to the actors, the use of monologues and dialogue, sounds, and music, to underline the role of Margery as narrator and author of the work, granting her tale a revitalization which further modernizes her figure by connecting the gender issues in the original texts to those inscribed in the present in a cultural relationship of reciprocal exchange. Her radio play leaves us suspended creatively between the medieval readers and the contemporary audience, technologically modernizing Margery’s tale and restoring to the book the prominence it deserves in the history of literature and at the same time showing the modernity of Margery’s position within her society. In this article, I shall be analysing the radio adaptation focusing on the oral medium – an appropriate form of remediation since The Book of Margery Kempe had been dictated to scribes – and on the transformations provided by the use of the radio. Form informs The True Tale of Margery Kempe, the radio play gives back to The Book of Margery Kempe its oral nature by making use of reconstructed dialogues and interior monologues, which bring to the mind the composition of collaborative life writing. I shall be also taking into account the author’s attempts to give back to Margery’s tale those everyday-life elements which are simply hinted at in the original text, through the use of modernization strategies, especially as regards the body, sexuality, and desire.

Whether it is the chaos created by the extreme and comical disconnection of the paradoxes of the ‘ausencia’ of Macedonio or the magical fragmentation of the stories in Cortázar’s Historias de cronopios y de famas, conceptual humor triggers a powerful gnoseological revolution in the reader with undisputable power. From the ‘logos’ of the absence of Macedonio in Cortázar, one passes to the possibility of forgetting the ‘other world’, hidden behind absence – a change in focus that presumes a more profound structural shift which further destabilizes the reader. The route of deconstruction and simulation begun by Macedonio Fernandez in his texts seems to reach its conclusion in the works of Cortázar: from the antimimetic writing and the creation of a self-referencing system, one passes to the possibility of seeing the ‘other reality’ which is hidden beyond our vision of the world, the mystery which makes age-old certainties crumble, the origin of the eternal human search for the truth.

This paper focuses on Robertsʼs counter-narrative “The Cookery Lesson” and the theme of fairy-tale cannibalism. While questioning the representation of the cannibalizing and the cannibalized bodies through the analysis of the sensuous language describing the desire-violence nexus, we shall take into account the ways in which the narrative form is affected in the passage from traditional fairy tale to contemporary short story, not only as concerns the representation of characters but also as regards the use of narrative voices and narrative strategies recalling the oral origins of fairy tales. At the same time, such formal choices critically re-contextualize source stories in a modern space-time framework, thus dismantling their supposed universal value. Incidentally, at the time of their circulation, the absence of a clear space-time description (which was mainly limited to rural life) helped learned readers reject issues such as cannibalism considered as savage, primitive practices causing moral and physical loathing in western civilised people.

The history of The Book of Margery Kempe, written at the beginning of the 15th century by a burgess of Bishop's Lynn (Norfolk), is quite revealing of the way Medieval Studies have entered the 21th century.
