Background COVID-19 has quickly become a global pandemic with a substantial number of deaths and is a considerable burden for healthcare systems worldwide. Although most cases are paucisymptomatic and limited to the viral infection-related symptoms, some patients evolve to a second phase, with an impaired inflammatory response (cytokine storm) that may lead to acute respiratory distress syndrome and death. This is thought to be caused by increased bradykinin synthesis. Methods ICAT-COVID is a multicenter, randomized, open-label, proof-of-concept phase II clinical trial assessing the clinical efficacy and safety of adding icatibant to the standard of care in patients hospitalized with COVID-19 without invasive mechanical ventilation. Patients hospitalized with a confirmed COVID-19 pneumonia diagnosis (RT-PCR or antigen test ≤ 10 days prior to randomization, and radiographic evidence of pulmonary infiltrates), rated “4” or “5” on the WHO’s clinical status scale, are eligible. Patients will be randomized on a 1:1 ratio to either standard of care-plus-icatibant (experimental group) or to standard of care alone (control group). The experimental group will receive 30 mg of icatibant subcutaneously 3 times a day for 3 days (for a total of 9 doses). The expected sample size is 120 patients (60 per group) from 2 sites in Spain. Primary outcomes are the efficacy and safety of Icatibant. The main efficacy outcome is the number of patients reaching grades “2” or “1” on the WHO scale within 10 days of starting treatment. Secondary outcomes include “long-term efficacy”: number of patients discharged who do not present COVID-19-related relapse or comorbidity up until 28 days after discharge, and mortality. Discussion Icatibant, a bradykinin type 2 receptor antagonist with proven effectiveness and safety against hereditary angioedema attacks, may be beneficial for COVID-19 patients by inhibiting bradykinin’s action on endothelial cells and by inhibiting the SARS-CoV-2 M protease. Our working hypothesis is that treatment with standard of care-plus-icatibant is effective and safe to treat patients infected with SARS-CoV-2 admitted to hospital for pneumonia without invasive mechanical ventilation. En este estudio descriptivo correlacional se tuvo como objetivo el determinar la frecuencia y severidad de caries según edad en niños entre 12-71 meses con riesgo social del Gran Mendoza, y el impacto de las lesiones de esmalte sobre estas variables. MÉTODO: Muestra: 404 niños de 12-71 meses de edad asistentes a SEOS del Gran Mendoza. Indicadores: grupos de edad; ceod con 2 categorías: para "c" según incluya lesión esmalte y dentina (c2-6) o dentina (c4-6) según criterios de ICDAS II; y severidad de caries. Se aplicó estadística paramétrica y no paramétrica para analizar variablesindependientes, dispersión y comparación de grupos; p≤0,05. RESULT ADOS: para ceod2-6: 49.8% sin experiencia de caries, c 2,13±3.12; ei 0,14±0.77; o 0,3 ±0,27; ceod 2,30±3,38.Para ceod4-6: 64% sin experiencia de caries, c 1,21±2,35; ei 0,13±0.72; o 0,3 ±0,27; ceod 1,37±2,59. Se encontró asociación estadísticamente significativa para diferencias entre: ceod4- 6 y ceod2-6 y componentes discriminados;para categorías de severidad y los dos grupos de ceod; para severidad y grupos de edad con aumento con la edad del componente c y del ceod total en ceod2-6y ceod4-6. El porcentaje de lesiones de caries de esmalte fue mayor en los primeros años de vida, aumentando luego el de lesiones dentinarias. La carga de caries de esmalte para toda la población estudiada fue del 30,9%. CONCLUSIONES: en los niños del Gran Mendoza evaluados el valor de ceod se incrementa hacia el 2do y 3er año de vida, cuadruplicándose hacia el 5to. Aumentan significativamente cuando se incluyen lesiones de esmalte, con alta carga de las mismas en los niños más pequeños. In this descriptive correlational study, the objective was to determine the frequency and severity of caries according to age in children between 12-71 months with social risk of Greater Mendoza, and the impact of enamel lesions on these variables. METHOD: Sample: 404 children aged 12- 71 months attending SEOS in Greater Mendoza, Indicators: age groups; dmft with 2 categories: for "d" according to include enamel and dentin (d2-6) or dentin (d4-6) lesion according to ICDAS II criteria; and caries severity. Parametric and non-parametric statistics were applied to analyze independent variables, dispersion and comparison of groups; P≤0.05. RESULTS: for dmft2-6: 49.8% without caries experience, d 2.13 ± 3.12; m 0.14 ± 0.77; f 0.3 ± 0.27; dmft2-6 2.30 ± 3.38. For dmft4-6: 64% with no caries experience, d 1.21 ± 2.35; m 0.13 ± 0.72; f 0.3 ± 0.27; dmft4-61.37 ± 2.59. A statistically significant association was found for differences between: dmft4-6 and dmft2-6 and discriminated components; for severity categories and the two groups of dmft; for severity and age groups with an increase with age of the d component and total dmft in dmft4-6 and dmft2-6. The percentage of enamel caries lesions was higher in the first years of life, then the dentine lesions increased. The enamel caries load for the entire study population was 30.9%. CONCLUSIONS: in the studied children of Greater Mendoza the value of dmft increases in the 2nd and 3rd year of life, quadrupling foward the 5th. Los bracket de autoligado son presentados al ámbito de la ortodoncia, como un sistema de alta eficiencia, al servicio de la baja fricción. 13 casos fueron tratados durante el ciclo de cursos de ortodoncia, dictados por el Dr. Edgardo Grynberg, en la Facultad de Odontología de la Universidad de Cuyo, Mendoza, con aparatologia de autoligado en combinación con arcos superelasticos, en los cuales se aprecio una importante disminución en el tiempo de tratamiento y menor tiempo de sillón. Las favorables expresiones de los arcos fueron más notorias de mes a mes. Al no existir la necesidad de cambiar periódicamente las ligaduras, las consultas se pueden espaciar más en el tiempo. Cuando el profesional posee un plan de tratamiento acertado, establecido de acuerdo a un diagnóstico correcto, puede ver a sus pacientes cada 8 semanas de promedio. Es importante dar el tiempo adecuado a cada arco. Por lo que consideramos, a los bracket de autoligado una evolución importante de la ortodoncia y un camino interesante sobre el cual seguir avanzando. Self-ligating brackets are presented in the field of orthodontics as a high-efficiency system, at service of low friction. 13 cases were treated during the cycle of orthodontic courses given by Dr. Edgardo Grynberg, in the Faculty of Dentistry at the University of Cuyo, Mendoza, with self-ligating appliances in combination with super elastic arches, in which a significant decrease in treatment and chair time was appreciated. The favorable expressions of the arches were stronger from month to month. In the absence of the need to regularly change ligations, consultation can be spaced over time. When the practitioner holds a successful treatment plan established under the correct diagnosis, he can see their patients every 8 weeks on average. It is important to give adequate time to each arch. For that we believe that self-ligating brackets are a significant evolution of orthodontics and an interesting path on which to move forward. Los objetivos de este trabajo fueron determinar el estado dental de la población bajo estudio, detectar subgrupos de riesgo específico para enfermedades bucales y proponer estrategias de intervención apropiadas para la promoción de la salud bucal para ellos. Método: sobre la base de datos del subprograma de salud bucal "El hospital y los chicos" se efectuó un estudio descriptivo del estado dental sobre 277 niños entre 2 meses y 13 años de edad asistentes al CDlF Nº11, Guaymallén, Mendoza. Se registraron: ceod, CPOD, ceos CPOS, índice de Necesidad de tratamiento de caries, índice de Paca de Silness y Loe, ICDAS II, y datos demográficos. Además, como parte del programa, se llevó a cabo la enseñanza de Técnicas de Higiene Bucal adecuada a la edad, dirigidas a madres y niños, y una topicación de flúor profesional siguiendo protocolos específicos según edad del niño. Resultados: la población libre de caries fue del 31,4%, mientras que el 68,6% tuvo experiencia de caries pasada o presente. La media de la sumatoria de ceod+ cpod fue 4,91 = 5, con una carga de enfermedad muy alta denotada por un componente c+C de 4.25=4.55. Presentó un valor para ceos+CPOS de 9,22+11 96 y de c+CS de 6.8918 61. En cuanto a la severidad de las lesiones de caries expresadas con las categorías de ICDAS II del 2 al 6, se observó que las medias mayores corres- , ponden al nivel 6 (x2 Friedman= 20,999, p= 0,000). La media de necesidad de tratamiento de caries fue de 5,74, pero los niveles 8 y más abarcan al 33.2% de la población de estudio Se observó un aumento progresivo de estos indicadores en la primera infancia comenzando en la primera franja etaria con una media de ceod+CPOD de 0.8811,92, de 3, 93t4.65a los tres años, de 6,38+6,36 a los 5 años, y de 8,40+5,47 a los 7 años, siendo esta población la que presentó el mayor valor del indicador (x2 Kruskall Wallis= 104,637, p=O,OO). El índice CPOD fue de 1,35~1,34 a los 7 años y de 4.65+3 99 a los 9 ( x2 Kruskall Wallis =17,609 y p=0,001) Conclusiones~ este grupo de niños de alto riesgo social presentó elevados índices de caries y de necesidad de tratamiento, que requerirán de un sistema de salud que pueda contenerlos. Al observar las medias de ceod+CPOD según las categorías de edad se pone de manifiesto una tendencia a agravarse el estado dentario en la primera infancia, llegando a valores muy por encima de la media general para los 5, 6 ,7 y 8 años El CEOD también tiende a aumentar con la edad, poniendo en evidencia la susceptibilidad de caries de los molares permanentes erupcionados Las tendencias de los indicadores permitieron reconocer dos subgrupos de riesgo para desarrollar programas preventivos' el de niños de O a 3 años, y el de 6 a 12, es decir niños escolares. Se sugieren dos programas prioritarios Materno- infantil y Protección de 1er molar permanente. The goals of this study were the following: to determine the dental situation of the population under study, to identify specific risk subgroups of oral diseases and to propose appropriate intervention strategies to promote oral health for them. Method: on the data base of "The hospital and the kids" oral health subprogram, a descriptive study about the dental health of 277 children between 2 months and 13 years old attending the CDIyF No. 11 in Guaymallén, Mendoza, was performed. We recorded the following: dmft, DMFT , DMFS ceos , Need of Caries Treatment Index, Bacterian Plaque Index of Silness and Löe , ICDAS II , and demographic data. In addition, as part of the program, the teaching of oral hygiene techniques was performed according to the age, aimed at mothers and children. Also, a professional flour application was carried out following the specific protocols according to the child’s age. Results: caries free population was 31,4 %, while 68.6 % had caries experiences in the past or in the present time. The average result of the dmft + DMFT sum was 4,91 +/- 5, with a high disease load denoted by a d + D component of 4,25 +/- 4,55. It showed a value for dmfs + DMFS of 9,22 +-/ 11,96 and for d + DS of 6,89 +/- 8,61. Regarding the severity of carious lesions expressed with ICDAS II from categories from 2 to 6, it mas observed that the higher average correspond to level 6 (x2 Friedman= 20,999; p= 0,000). The average needing caries treatment was of 5,74, but levels 8 comprehend the 33,2% of the population under study. There was a progressive increase of these indicators in early childhood starting with the youngest children with an average of dmft + DMFT of 0.88 +/- 1,92 and of 93 +/- 4,65 at 3 years old, 6,38 +/- 6,36 at 5 years old, and 8,40 +/- 5,47 at 7 years old. The latter had the highest value of the indicator (x2 Kruskall Wallis= 104,637, p=0, 00). The DMFT index was 1,35 +/- 1,34 at 7 years old and 4,65 +/- 3,99 at 9 years old (x2 Kruskall Wallis = 17,609 and p=0,00 1). Conclusions: This group of children with high social risks has higher rates of caries and more need for caries treatments; they require a health system that may include them. The observation of the dnzft + DMFT average according to age categories reveals a tendency to worsen the dental status in early childhood, reaching values well above the overall average for children of 5, 6, 7 and 8 years old. The DMFT also tends to increase with age, revealing caries susceptibility in erupted permanent molars. The indicators tendencies allowed to recognize e two risk subgroups in order to develop prevention programs: the 0-3 year old children and the 6- 12 year old children, i.e. school children. Póster presentado en la International Pasteurellaceae Conference, celebrada en Prato (Italia) del 13 al 16 de mayo de 2014, Nontypable Haemophilus influenzae (NTHi) has emerged as an important opportunistic pathogen causing infection in adults suffering obstructive lung diseases. Existing evidence associates chronic infection by NTHi to the progression of the chronic respiratory disease, but specific features of NTHi associated with persistence have not been comprehensively addressed. To provide clues about adaptive strategies adopted by NTHi during persistent infection, we compared sequential persistent isolates with newly acquired isolates in sputa from six patients with chronic obstructive lung disease. Pulse field gel electrophoresis (PFGE) identified three patients with consecutive persistent strains and three with new strains. Phenotypic characterisation included infection of respiratory epithelial cells, bacterial self-aggregation, biofilm formation and resistance to antimicrobial peptides (AMP). Persistent isolates differed from new strains in showing low epithelial adhesion and inability to form biofilms when grown under continuous-flow culture conditions in microfermenters. Self-aggregation clustered the strains by patient, not by persistence. Increasing resistance to AMPs was observed for each series of persistent isolates; this was not associated with lipooligosaccharide decoration with phosphorylcholine or with lipid A acylation. Variation was further analyzed for the series of three persistent isolates recovered from patient 1. These isolates displayed comparable growth rate, natural transformation frequency and murine pulmonary infection. Genome sequencing of these three isolates revealed sequential acquisition of single-nucleotide variants in the AMP permease sapC, the heme acquisition systems hgpB, hgpC, hup and hxuC, the 3-deoxy-D-manno-octulosonic acid kinase kdkA, the long-chain fatty acid transporter ompP1, and the phosphoribosylamine glycine ligase purD. Collectively, we frame a range of pathogenic traits and a repertoire of genetic variants in the context of persistent infection by NTHi.

Como sabemos, la caries es una enfermedad infecciosa, transmisible de los dientes, que se caracteriza por la desintegración de sus tejidos calcificados. La lesión cariosa es definida como DINÁMICA. Es por esto que debemos establecer si el fenómenos DES/RES se encuentra en equilibrio o la desmineralización ha alcanzado un papel preponderante, dando como resultado pérdida de la porción mineral y la subsecuente disgregación de la parte orgánica. Así es como el diagnóstico finaliza, poniéndose en auge la función terapéutica. Durante el desarrollo de estos casos clínicos las lesiones de mancha blanca fueron tratadas con sustancias a base de fosfatos y calceinatos. As we all know, decay is an infections and transmissible teeth desease that is characterised by the disintegration of their calcified tissues. The carious lesion is defined as DYNAMIC. For this reason, we must establish whether the DES/RES phenomena is in equilibrium or whether the demineralisation has reached a significant role, having as a result a lost of the mineral portion lost and the subsequent disintegration of the organic part. Giving importance to the therapeutical function, this is how a diagnose comes to an end. Póster presentado en la International Pasteurellaceae Conference, celebrada en Elsinore (Dinamarca) del 24 al 27 de agosto de 2011., [Aim] Non-typable Haemophilus influenzae (NTHi) causes acute respiratory infections, is associated with the progression of chronic respiratory diseases, and responsible for a high proportion of exacerbations. We analysed population dynamics, diversity of bacterial surface-related gene patterns and associated sequences, and virulence-related phenotypes in (i) NTHi strains from respiratory and non-respiratory samples isolated from unrelated patients, and (ii) NTHi serial isolates from COPD exacerbations., [Methods] (i) 111 NTHi isolates from unrelated patients were used to analyse nine genes encoding surface molecules. PCR- and sequencing-based genetic profiles were associated to bacterial resistance to serum mediated killing and epithelial adhesion. (ii) 54 serial NTHi isolates from 20 COPD patients isolated in exacerbation were used to establish genotypic dynamics, bacterial surface molecules conservation, and association to bacterial resistance to antimicrobial peptides and epithelial adhesion., [Results] (i) 94.6% of unrelated NTHi isolates presented one prevalent molecular pattern. Such a genetic profile was associated with a higher bacterial resistance to serum mediated killing and enhanced adherence to human respiratory epithelial cells. (ii) Serial NTHi strains from COPD exacerbations could have identical genotypes, different genotypes, or alternation of isogenic and nonisogenic isolates. Identical strains conserved molecular pattern and ompP5 and oapA gene sequences, otherwise variable among different isolates, but displayed variation in the number of simple sequence repeats in lic1A and lic2B., [Conclusions] The prevalence of particular surface related molecular features can be advantageous for NTHi. The selection of bacterial clones endowed with those features may be modulated by the host.
