Zusammenfassung Carlsberg hat im Zeitraum von März 2016 bis November 2019 die Planung, Vergabe und Ausführung für seine neue Holsten-Brauerei in Hamburg realisiert. Das Projekt erfolgte im Rahmen einer Standortverlagerung von Hamburg-Altona in ein Industriegebiet in Hamburg-Hausbruch, die infolge von Überkapazitäten und Ineffizienzen am alten Standort entschieden wurde. Die Produktionskapazität der neuen Brauerei mit einer Brutto-Grundfläche von 25 000 m² beträgt 100 000 000 Liter pro Jahr, die sich auf 1 000 000 Flaschen und 3 000 Fässer pro Tag aufteilt [1]. Das sehr komplexe Projekt war in sechs Workstreams organisiert und durch sein großes internationales Projektteam, die hohe Installationsdichte der Prozessanlagen sowie die komplexen Schnittstellen zwischen dem Gebäude inklusive seiner technischen Ausrüstung und den Prozessanlagen charakterisiert. Dieser Beitrag beschreibt die Vorgehensweise im Projekt in den Phasen der Planung, Vergabe und Ausführung.

15O(α,γ)19Ne is regarded as one of the most important thermonuclear reactions in type I X-ray bursts. For studying the properties of the key resonance in this reaction using β decay, the existing Proton Detector component of the Gaseous Detector with Germanium Tagging (GADGET) assembly is being upgraded to operate as a time projection chamber (TPC) at FRIB. This upgrade includes the associated hardware as well as software and this paper mainly focusses on the software upgrade. The full detector set up is simulated using the ATTPCROOTv 2 data analysis framework for 20Mg and 241Am.

Sensitivity studies have shown that the 15O(α, γ)19Ne reaction is the most important reaction rate uncertainty affecting the shape of light curves from Type I X-ray bursts. This reaction is dominated by the 4.03 MeV resonance in 19Ne. Previous measurements by our group have shown that this state is populated in the decay sequence of 20Mg. A single 20Mg(βp α)15O event through the key 15O(α, γ)19Ne resonance yields a characteristic signature: the emission of a proton and alpha particle. To achieve the granularity necessary for the identification of this signature, we have upgraded the Proton Detector of the Gaseous Detector with Germanium Tagging (GADGET) into a time projection chamber to form the GADGET II detection system. GADGET II has been fully constructed, and is entering the testing phase.

Decellularized scaffolds represent a promising alternative for mitral valve (MV) replacement. This work developed and characterized a protocol for the decellularization of whole MVs. Porcine MVs were decellularized with 0.5% (w/v) SDS and 0.5% (w/v) SD and sterilized with 0.1% (v/v) PAA. Decellularized samples were seeded with human foreskin fibroblasts and human adipose-derived stem cells to investigate cellular repopulation and infiltration, and with human colony-forming endothelial cells to investigate collagen IV formation. Histology revealed an acellular scaffold with a generally conserved histoarchitecture, but collagen IV loss. Following decellularization, no significant changes were observed in the hydroxyproline content, but there was a significant reduction in the glycosaminoglycan content. SEM/TEM analysis confirmed cellular removal and loss of some extracellular matrix components. Collagen and elastin were generally preserved. The endothelial cells produced newly formed collagen IV on the non-cytotoxic scaffold. The protocol produced acellular scaffolds with generally preserved histoarchitecture, biochemistry, and biomechanics.

Classical novae are environments which can produce heavier elements up to mass A ∼ 40. These nuclides at the endpoint of nova nucleosynthesis consist of elements such as Ar, K, and Ca. There is an order of magnitude discrepancy with the predicted and theoretical abundances of these endpoint nuclides produced in a classical nova. The uncertainty in the theoretical 38K(p,γ)39Ca reaction rate has been shown to affect the abundances by an order of magnitude or more. The only direct measurement of this reaction rate was performed with the DRAGON facility at TRIUMF; however additional spectroscopic data could aid the interpretation of this data as well as motivate further study of this reaction rate. In this study, we present the preliminary results of a spectroscopic study of 39Ca using the 40Ca(d,t)39Ca reaction carried out at the Maier-Leibnitz Laboratory in Garching, Germany.

In deutschen Notaufnahmen werden aktuell etwa 22 Mio. Patienten pro Jahr behandelt, ohne dass eine strukturierte Qualitatssicherung oder ein Qualitatsmanagement anhand nationaler Qualitatsindikatoren existiert. Ziel der vorliegenden Arbeit ist die Beschreibung der Ergebnisse einer erstmaligen standardisierten Bewertung von international etablierten Qualitatsindikatoren, welche potenziell in deutschen Notaufnahmen Anwendung finden konnten. Ausgehend von einer Literaturrecherche wurde eine Auswahl von 35 potenziellen Qualitatsindikatoren zu verschiedenen Prozessen einer Notaufnahme zunachst standardisiert mittels QUALIFY bewertet und in einem zweiten Durchgang interprofessionell diskutiert. Hierzu wurde eine 14-kopfige Arbeitsgruppe gebildet. Die Auswertung und Ergebnisdarstellung erfolgte fur beide Bewertungsmethoden mittels deskriptiver Statistik und Ergebniszusammenfassung. Der Vergleich beider Methoden wurde im Sinne einer Paralleltestubereinstimmung mittels Cronbach’s α ermittelt. Die Ergebnisse wurden zusatzlich in einem Indikatorregister angegeben. Bei 20 von 35 Qualitatsindikatoren wurden uberwiegend Starken in den zu bewertenden QUALIFY-Kriterien gesehen. Bei 11 Qualitatsindikatoren sah die Arbeitsgruppe Schwachen im Rahmen der Anwendung in deutschen Notaufnahmen. Die Ergebnisse der anonymen QUALIFY-Methode und diejenigen der offenen Diskussion waren zu 89 % kongruent (Cronbach’s α = 0,865). Durch die offene Diskussion wurden 46 % (16/35) der betrachteten Qualitatsindikatoren zusatzlich prazisiert und 20 % (7/35) wesentlich modifiziert. QUALIFY erlaubt eine schnelle und strukturierte Bewertung potenzieller Qualitatsindikatoren fur Notaufnahmen aus publizierten Versorgungsleitlinien bzw. einer Literaturrecherche und vermeidet einen Bias durch Beeinflussung der Experten untereinander. Die offene Diskussion ist zwar zeitaufwandiger, liefert als Ergebnis jedoch klar definierte und ausformulierte Kennzahlen fur ein Indikatorregister. Dabei zeigen beide Methoden einen hohen Grad an Ubereinstimmung. Vor der Implementierung der bewerteten Qualitatsindikatoren in standardisierte Qualitatssicherungsprogramme steht ihre wissenschaftliche Evaluation anhand nationaler Daten aus Notaufnahmen aus.

Die Aufgaben der klinischen Notfallmedizin in den Notaufnahmen haben sich in Deutschland, aber auch international, in den letzten Jahrzehnten von einer uberwiegend unfallchirurgisch-ambulanten Versorgung hin zu einer Versorgung auch kritisch kranker konservativer Patienten gewandelt. Parallel zu den gestiegenen medizinischen Anforderungen an die klinische Notfallmedizin steigt ebenso die Notwendigkeit, das medizinische Personal so zu qualifizieren, dass die geforderte Behandlungsqualitat gewahrleistet werden kann. Recherche und Auswertung aktueller Literatur. Bisher umfassten gangige Fortbildungskonzepte fur die klinische Notfallmedizin ha ufig Themen aus den Bereichen Reanimation und Traumaversorgung, Schulungen in der (Notfall-)Sonografie aber auch andere Aspekte. So ist z. B. ATLS® weltweit verbreitet und aktuell in der 9. Version sowie auch in deutscher Ubersetzung verfugbar. Ein neueres Format ist der European Trauma Course (ETC®). Verschiedene Kursformate nutzen in unterschiedlichem Umfang die Moglichkeiten der Simulation, um bestimmte Fahigkeiten auch ohne echten Patientenkontakt zu erlernen und zu trainieren. Da die Sonographie ein wesentliches diagnostisches Hilfsmittel in den Notaufnahmen ist, hat die Deutsche Gesellschaft fur Ultraschall in der Medizin (DEGUM) verschiedene Kurse auch fur die Sonografie im Notfall entwickelt. Weiterbildungskonzepte in der klinischen Notfallmedizin sind europaweit seit langerer Zeit bekannt (z. B. Curriculum der European Society for Emergency Medicine, EuSEM) und stehen in Deutschland auf Bundesebene vor der E infuhrung (Zusatzweiterbildung Klinische Akut- und Notfallmedizin). Notaufnahmen sollten zentral und interdisziplinar organisiert sein und uber d ort dauerhaft tatigem entsprechend zusatzlich qualifiziertem Personal verfugen. Die Zusatzweiterbildung Klinische Akut- und Notfallmedizin schafft hier zukunftig eine fundierte Voraussetzung.

Hintergrund: Bis zu 11 % der Patienten einer (zentralen) Notaufnahme (ZNA) stellen sich mit akutem nicht traumatischem Abdominalschmerz vor. Anhand dieses Prasentationssymptoms soll untersucht werden, wie in der ZNA eingesetzte Weiterbildungsassistenten (2. und 3. Ausbildungsjahr) aus unterschiedlichen Fachrichtungen (Chirurgie, Innere Medizin, Anasthesie, weitere Fachrichtungen) diese Patientengruppe behandeln (z. B. Prozesszeiten). Material und Methoden: Durchfuhrung einer prospektiven, monozentrischen Beobachtungsstudie in einer ZNA gemas STROBE-Empfehlung unter Einhaltung der Empfehlungen der Ethikkommission der Universitat Ulm (Antrag Nr. 335/12) und der Empfehlung der Deklaration von Helsinki sowie unter Prufung des Datenschutzbeauftragten des Krankenhauses. Wahrend eines 12-monatigen Zeitraums (12/2012–12/2013) wurde eine Stichprobe von Patienten mit nicht traumatischem Abdominalschmerz durch einen von der Behandlung unabhangigen Beobachter erhoben. Neben demografischen Daten wurde ein spezielles Augenmerk auf Prozesse und Bewertung der Versorgung (inkl. Anamnese und korperlicher Untersuchung) gelegt. Zusatzlich wurden Subgruppen (Fachrichtung des Notaufnahmearztes, Dringlichkeit gemas Manchester Triage Score [MTS] etc.) untersucht. Ergebnisse: 143 Patienten erfullten die Einschlusskriterien. Die Fachrichtung des Notaufnahmearztes hatte keinen Einfluss auf die untersuchten Prozesse wie Anamnese, Erstuntersuchung, angeforderte Konsile oder durchgefuhrte Diagnostik. „Dringlich“ eingeschatzte Patienten wurden signifikant fruher behandelt (Zeit bis 1. Arztkontakt 26 ± 24 vs. 46 ± 34 min; p Schlussfolgerung: Bei hohem diagnostischem Aufwand war die Prozesstreue bei der Behandlung von Patienten mit akutem nicht traumatischem Abdominalschmerz sehr hoch. Die Fachrichtung des Weiterbildungsassistenten (Chirurgie, Innere Medizin, Anasthesie, weitere Facher) in der ZNA hatte keinen Einfluss auf die untersuchten Prozesse.

At the 121st German Physicians Conference 2018 in Erfurt a resolution to accept the full amendment of the (draft) further training regulations (MWBO) was adopted and the State Medical Councils were requested to include them in their respective areas of responsibility. Therefore, the nationwide implementation of the supraspeciality (ZWB) for clinical acute and emergency medicine has been officially finalized. After consultation with the German Medical Council (BÄK) concerning the format, both the new MWBO 2018 and now the content of the ZWB are available as of 15 November 2018.The Physicians Conference resolution and anchoring of the new ZWB clinical acute and emergency medicine in the MWBO were preceded by a process lasting approximately 10 years. The concept of the ZWB clinical acute and emergency medicine, which was scrutinized by the Standing Committee on "medical further training" and the board of the BÄK and presented for approval, was essentially developed by representatives of the German Interdisciplinary Association for Intensive Care and Emergency Medicine (DIVI) and the German Society for Interdisciplinary Emergency and Acute Medicine (DGINA) in consultation with the BÄK. A consensus was finally reached in September 2017 in cooperation with those German specialist societies with a high proportion of emergency patients.In addition to the title, definition, and minimum requirements for applying the ZWB, the content was processed according to the European curriculum for emergency medicine. The structural prerequisites have now been approved, the Standing Committee has configured the content, and the complete MWBO 2018 has been successfully presented, such that the ZWB clinical acute and emergency medicine is expected to be implemented in the individual Federal States within the next 1-2 years.This article describes the history and development of ZWB clinical acute and emergency medicine in Germany and outlines future perspectives.

Supplemental Digital Content is available in the text., Objectives The patient’s presenting complaint guides diagnosis and treatment in the emergency department, but there is no classification system available in German. The Canadian Emergency Department Information System (CEDIS) Presenting Complaint List (PCL) is available only in English and French. As translation risks the altering of meaning, the International Society for Pharmacoeconomics and Outcomes Research (ISPOR) has set guidelines to ensure translational accuracy. The aim of this paper is to describe our experiences of using the ISPOR guidelines to translate the CEDIS PCL into German. Materials and methods The CEDIS PCL (version 3.0) was forward-translated and back-translated in accordance with the ISPOR guidelines using bilingual clinicians/translators and an occupationally mixed evaluation group that completed a self-developed questionnaire. Results The CEDIS PCL was forward-translated (four emergency physicians) and back-translated (three mixed translators). Back-translation uncovered eight PCL items requiring amendment. In total, 156 comments were received from 32 evaluators, six of which resulted in amendments. Conclusion The ISPOR guidelines facilitated adaptation of a PCL into German, but the process required time, language skills and clinical knowledge. The current methodology may be applicable to translating the CEDIS PCL into other languages, with the aim of developing a harmonized, multilingual PCL.

Infection with the emerging pathogen Clostridioides (Clostridium) difficile might lead to colonization of the gastrointestinal tract of humans and mammals eventually resulting in antibiotic-associated diarrhea, which can be mild to possibly life-threatening. Recurrences after antibiotic treatment have been described in 15-30% of the cases and are either caused by the original (relapse) or by new strains (reinfection). In this study, we describe a patient with ongoing recurrent C. difficile infections over 13 months. During this time, ten C. difficile strains of six different ribotypes could be isolated that were further characterized by phenotypic and genomic analyses including motility and sporulation assays, growth fitness and antibiotic susceptibility as well as whole-genome sequencing. PCR ribotyping of the isolates confirmed that the recurrences were a mixture of relapses and reinfections. One recurrence was due to a mixed infection with three different strains of two different ribotypes. Furthermore, genomes were sequenced and multi-locus sequence typing (MLST) was carried out, which identified the strains as members of sequence types (STs) 10, 11, 14 and 76. Comparison of the genomes of isolates of the same ST originating from recurrent CDI (relapses) indicated little within-patient microevolution and some concurrent within-patient diversity of closely related strains. Isolates of ribotype 126 that are binary toxin positive differed from other ribotypes in various phenotypic aspects including motility, sporulation behavior and cell morphology. Ribotype 126 is genetically related to ribotype 078 that has been associated with increased virulence. Isolates of the ribotype 126 exhibited elongated cells and a chaining phenotype, which was confirmed by membrane staining and scanning electron microscopy. Furthermore, this strain exhibits a sinking behavior in liquid medium in stationary growth phase. Taken together, our observation has proven multiple CDI recurrences that were based on a mixture of relapses and reinfections.

IntroductionLong-term exposure of women with Multiple sclerosis (MS, wwMS) to immunomodulatory or immunosuppressive treatments may increase the risk of cervical dysplasia. However, little is known about cervical dysplasia risk and Human Papillomavirus (HPV)-vaccine coverage in wwMS.MethodsAdult wwMS were recruited from two tertiary MS clinics. To explore the association between MS treatments (DMTs) and abnormal cervical screening tests (CSTs), we linked individual data from MSBase, the Victorian Cervical Screening Registry, and National HPV vaccination program registry (NHPVPR).ResultsTo date, we have recruited 208 wwMS of whom 102 had complete data (vaccination status, cervical screening tests, MSBase data) and no previous history of abnormal CST at MS onset for this interim analysis. The average age was 33.8 (18 to 59 yrs) and most (n=58, 88%) were unvaccinated. 19 wwMS (19%) had an abnormal CST after MS onset (incidence rate 20.6 cases/1000 person-years, 95% confidence interval 12.4–32.1) over average 9.0 years of follow-up. 57 wwMS were treated with lower-efficacy therapies (56%), 73 with a high-efficacy therapy (72%), and 44 were exposed to both. Eight abnormal CSTs were detected before starting high-efficacy therapy (rate 12.6, 95% CI (5.4–24.8)) and 11 were detected after starting high-efficacy therapy (rate 38.6, 95% CI (19.3–69.0), p=0.022.ConclusionWe provide preliminary data that high efficacy DMTs may increase the risk of abnormal CSTs over time. A larger cohort and inclusion of additional cervical dysplasia risk factors are required to fully elucidate risk in wwMS.

Most of the commonly available patient data management systems (PDMS) have proved their usability beyond pilot installations. The most important reason against the introduction of a PDMS is high financial costs. Beside acquisition costs (hardware, software licenses, service agreements), associated in-house efforts (e.g., labor costs) have also to be taken into consideration. In addition to the acquisition of technical competence, in-house efforts also lead to a structured reflection of the clinic's innate processes and structure. PDMS are able to make use of clinical documentation to derive or visualize information relevant to payroll. Interfaces for automated exchange of said information are convenient but also require high implementation and maintenance effort. However, compilation of payroll-relevant data still provides a considerable reduction of work. PDMS provide the information necessary for rational resource management and allocation. In addition, through the use of standards, PDMS enable improved therapy and care pathway adherence. To what extent a contribution to a positive cost-benefit calculation can be achieved needs to be considered individually.

The thermonuclear ^{30}P(p,γ)^{31}S reaction rate is critical for modeling the final elemental and isotopic abundances of ONe nova nucleosynthesis, which affect the calibration of proposed nova thermometers and the identification of presolar nova grains, respectively. Unfortunately, the rate of this reaction is essentially unconstrained experimentally, because the strengths of key ^{31}S proton capture resonance states are not known, largely due to uncertainties in their spins and parities. Using the β decay of ^{31}Cl, we have observed the β-delayed γ decay of a ^{31}S state at E_{x}=6390.2(7) keV, with a ^{30}P(p,γ)^{31}S resonance energy of E_{r}=259.3(8) keV, in the middle of the ^{30}P(p,γ)^{31}S Gamow window for peak nova temperatures. This state exhibits isospin mixing with the nearby isobaric analog state at E_{x}=6279.0(6) keV, giving it an unambiguous spin and parity of 3/2^{+} and making it an important l=0 resonance for proton capture on ^{30}P.

Gleichzeitig ist der Anspruch an die praund innerklinische Notfallmedizin heute deutlich hoher als noch vor 20 Jahren, was erheblichsteigendeAnforderungensowohl an die Ausbildung, aber auch die kontinuierliche Fortund Weiterbildung von Rettungsfachpersonal, Arzten und Pflegekraften stellt. Dass hierbei aktuell Defizite bestehen, wird auch in den Leitlinien des European Resuscitation Council 2010 und 2015 deutlich, in denen es heist: „Einige Studien zeigen, dass Arzte und PflegepersonalmangelhaftesWissen und geringe Fertigkeiten in der Akutbehandlung aufweisen, z. B. in der Sauerstofftherapie, der Flussigkeitsund Elektrolytbalance, der Schmerzbekampfung, des Einverstandnisses von Patienten, der Pulsoxymetrie und der Dosierung von Medikamenten. Ausund Fortbildung desmedizinischenPersonals sind einwesentlicher Teil der Einfuhrung eines Systems zur Vorbeugung von Kreislaufstillstanden [1].“ In den Beitragen dieses Leitthemenhefts werden Konzepte vorgestellt, um die Qualifikation der verschiedenen, anderNotfallversorgungbeteiligten Berufsgruppen zu verbessern. Wir haben dies in einem Heft in der Uberzeugung zusammengefasst, dass sowohl gute Kenntnisse als auch ein gutes Zusammenspiel aller beteiligten Akteure letztlich fur den Behandlungserfolg entscheidend sind. Zentrale Notaufnahmen und eine intensiveDiskussionumdie richtigenQualifikationen fur das eingesetzte arztliche und pflegerische Personal beschaftigen uns seit einigen Jahren und haben zu unterschiedlichen Empfehlungen gefuhrt [2, 3].
