1. T helper 2 inflammatory markers are associated with recurrence in chronic rhinosinusitis with nasal polyps after endoscopic sinus surgery

Abstract

Background Despite maximum medical treatment and endoscopic sinus surgery (ESS), chronic rhinosinusitis with nasal polyps (CRSwNP) can require revision surgery. With a growing literature on the diversity of cytokine inflammation patterns in CRSwNP, an endotype-driven approach could lead to the identification of cytokine profiles that predict recurrence. Methods A monocentric longitudinal study was carried out until June 2019 following CRSwNP patients who underwent surgery for the first time between December 2010 and January 2012. The biomarker profiles were established on blood and nasal secretions at the time of the first surgery (Interleukin (IL)-5, IgE, IgA, eosinophilic cationic protein (ECP) and eosinophilic- derived neurotoxin (EDN)). Profiles were compared between the patients still controlled by medical treatment and the patients requiring revision surgery during the course of the follow-up period. Results Among the 48 patients initially enrolled in our study, 8 required revision surgery (16,7%). Clinical features (asthma, allergy, aspirin intolerance, active smoking) and levels of blood markers measured at the time of the first surgery were comparable between the 2 groups of patients. Levels of IL-5, IgE and ECP in nasal secretions were significantly increased in the group of patients needing revision surgery. Conclusions Based on simple approach of nasal secretions sampling, we showed that a predominant T helper 2 proteins expression profile can be associated with recurrent CRSwNP after ESS. Initial immunoprofiling in CRSwNP disease may contribute to better predict the therapeutic response to optimal medical and surgical treatment, and help define the role of innovative targeted treatment, beside corticosteroids and ESS.

Abstract

This work investigated the ability of a 6-day heat acclimation protocol to impart heat acclimation-mediated cross-tolerance (HACT) in C2C12 myotubes, as indicated by changes in inflammatory and apoptotic responses to subsequent lipopolysaccharide (LPS) challenge. Myotubes were incubated at 40 °C for 2 h/day over 6 days (HA) or maintained for 6 days at 37 °C (C). Following 24 h recovery, myotubes from each group received either no stimulation or 500 ng/ml LPS for 2 h (HA + LPS and C + LPS, respectively). Cell lysates were collected and analyzed for protein markers of the heat shock response, inflammation, and apoptosis. Abstract

This work investigated the effect of a 6-day heat acclimation (HA) protocol on myotube metabolic responses at baseline and in response to a subsequent lipopolysaccharide (LPS) challenge. C2C12 myotubes were incubated for 2 h/day at 40 °C for 6 days (HA) or maintained at 37 °C (C). Following 24-h recovery, myotubes were challenged with 500 ng/ml LPS for 2 h, then collected for analysis of protein markers of mitochondrial biogenesis and macronutrient storage. Functional significance of these changes was confirmed with mitochondrial respiration and glycolytic measurements on a Seahorse XF-96 analyzer. HA stimulated mitochondrial biogenesis and increased indicators of mitochondrial content [SIRT1 (+ 62%); PGC-1α (+ 57%); NRF-1 (+ 40%); TFAM (+ 141%); CS (+ 25%); CytC (+ 38%); all p 0.05) but increased LDH-B (+ 30%; p = 0.02) and CPT-1 (+ 55%; p 0.05). Abstract

BACKGROUND: Scientific guidelines consider atrial fibrillation (AF) complicating degenerative mitral regurgitation (DMR) a debated indication for surgery. OBJECTIVES: This study analyzed the prognostic/therapeutic implications of AF at DMR diagnosis and long-term. METHODS: Patients were enrolled in the MIDA (Mitral Regurgitation International Database) registry, which reported the consecutive, multicenter, international experience with DMR due to flail leaflets echocardiographically diagnosed. RESULTS: Among 2,425 patients (age 67 ± 13 years; 71% male, 67% asymptomatic, ejection fraction 64 ± 10%), 1,646 presented at diagnosis with sinus rhythm (SR), 317 with paroxysmal AD, and 462 with persistent AF. Underlying clinical/instrumental characteristics progressively worsened from SR to paroxysmal to persistent AF. During follow-up, paroxysmal and persistent AF were associated with excess mortality (10-year survival in SR and in paroxysmal and persistent AF was 74 ± 1%, 59 ± 3%, and 46 ± 2%, respectively; p < 0.0001), that persisted 20 years post-diagnosis and independently of all baseline characteristics (p values

Abstract

This work investigated the effect of 3 days of 500 mg/day dietary curcumin supplementation on gastrointestinal barrier damage and systems-physiology responses to exertional heat stress in non-heat-acclimated humans. Eight participants ran (65% V̇o2max) for 60 min in a Darwin chamber (37°C/25% relative humidity) two times (Curcumin/Placebo). Intestinal fatty acid-binding protein (I-FABP) and associated proinflammatory [monocyte chemoattractant protein-1, tumor necrosis factor-α (TNF-α), interleukin-6] and anti-inflammatory [interleukin-1 receptor antagonist (IL-1RA), interleukin-10 (IL-10)] cytokines were assayed from plasma collected before (Pre), after (Post) and 1 (1-Post) and 4 (4-Post) h after exercise. Core temperature and HR were measured throughout exercise; the physiological strain index (PSI) was calculated from these variables. Condition differences were determined with 2-way (condition × time) repeated-measures ANOVAs. The interaction of condition × time was significant ( P = 0.05) for I-FABP and IL-1RA. Post hoc analysis indicated I-FABP increased more from Pre to Post (87%) and 1-Post (33%) in Placebo than in Curcumin (58 and 18%, respectively). IL-1RA increased more from Pre to 1-Post in Placebo (153%) than in Curcumin (77%). TNF-α increased ( P = 0.01) from Pre to Post (19%) and 1-Post (24%) in Placebo but not in Curcumin ( P > 0.05). IL-10 increased ( P < 0.01) from Pre to Post (61%) and 1-Post (42%) in Placebo not in Curcumin ( P > 0.05). The PSI, which indicates exertional heatstroke risk, was also lower ( P < 0.01) in Curcumin than Placebo from 40 to 60 min of exercise. These data suggest 3 days curcumin supplementation may improve gastrointestinal function, associated cytokines, and systems-level physiology responses during exertional heat stress. This could help reduce exertional heatstroke risk in non-heat-acclimated individuals.NEW & NOTEWORTHY Exercise-heat stress increases gastrointestinal barrier damage and risk of exertional heatstroke. Over the past decade at least eight different dietary supplements have been tested for potential improvements in gastrointestinal barrier function and systems-level physiology responses during exercise-heat stress. None have been shown to protect against both insults simultaneously. In this report 3 days of 500 mg/day dietary curcumin supplementation are shown to improve gastrointestinal barrier function, associated cytokine responses, and systems-level physiology parameters. Further research is warranted.

Abstract

Tendon injuries are common in hand wounds and their functional consequences are not negligible. In emergency, tendon repairs techniques are well codified to restore function. These tendon repairs can be made in one session, in two sessions, or even use tendon transfers. Tendon transfers are not usual in the emergency but more common in the treatment of sequelae or failure of primary repair. We report one case of transfer to restore thumb flexion in emergency.

8. Performances motrices respectives de l’arthroplastie totale de hanche et du resurfaçage : une évaluation comparative avec des sujets non opérés

Abstract

Resume Introduction Plusieurs etudes ont demontre des avantages en termes de resultat clinique en faveur du resurfacage total de hanche (RTH) comparativement aux protheses totales de hanche (PTH) conventionnelles y compris chez des patients jeunes. Mais aucune n’a compare la stabilometrie dans ces deux configurations. Hypothese L’arthroplastie de resurfacage permet un meilleur controle postural que les PTH conventionnelles. Objectifs Tester cette hypothese au moyen d’une etude stabilometrique afin de comparer l’equilibre et le comportement postural de patients porteurs d’un RTH, d’une PTH « standard » et des sujets sains. Patients et methode Nous avons analyse les resultats de trois cohortes de 20 patients sains, porteurs d’un RTH unilateral ou d’une PTH unilaterale. Les 40 patients porteurs de protheses etaient comparables en termes de sexe, d’âge, de poids (indice de masse corporelle), de date de chirurgie et de resultats cliniques. Les 20 patients du groupe « temoin » etaient plus jeunes car ils servaient de reference. L’analyse de l’equilibre etait effectuee au moyen d’une plateforme de force couramment utilisee en stabilometrie. L’analyse de l’equilibre bipodal et unipodal etait ainsi standardisee. Le logiciel interpretait l’equilibre de chaque patient par la mesure des variations des centres de pression (CP) plantaire des sujets durant l’analyse. La variation des CP dessinait une aire d’equilibre pour chaque patient en appui bipodal et monopodal (statokinesigramme en mm 2 ). Resultats L’analyse de l’equilibre bipodal objectivait des resultats comparables entre le groupe temoin et RTH. Le statokinesigramme etait plus important en appui bipodal dans le groupe PTH ( p p p Discussion et conclusion Ces resultats d’etude posturale de controle moteur confirment un meilleur comportement des protheses de resurfacage par rapport aux PTH conventionnelles et peut concourir a expliquer les excellents resultats radio-cliniques observes apres RTH. Niveau de preuve Niveau III etude comparative cas-temoins.

Abstract

The goals of this study were to test the effects of exogenous hormones and hibernation on breeding behavior and gamete release by boreal toads (Bufo boreas boreas). Each year, a subset of 77 toads was hibernated and then paired with hibernated or nonhibernated mates and treated with luteinizing hormone releasing hormone analogue (LHRHa), human chorionic gonadotropin (hCG), or left untreated. Amplexus and egg and sperm production were recorded. At 1 yr of age, only 19% of pairs exhibited amplexus, and no sperm or eggs were produced. At 2 and 3 yr of age, most male toads treated with LHRHa exhibited amplexus (56.9% and 100%, respectively). Among 2-yr-old males, amplexus was more prevalent (P0.05) in those that were hibernated than in those that were nonhibernated (54.0% and 33.3%, respectively), but most males in each group (93.3% and 75%, respectively) produced sperm in response to LHRHa treatment. Only one 2-yr-old and two 3-yr-old females produced eggs. At 4 yr of age, eight females produced eggs, but two died from egg retention. More nonhibernated than hibernated females developed eggs (7 of 10 vs. 1 of 10, P0.05). Mean (+/-SD) weight of female toads producing eggs (58.9 +/- 11.9g) was greater (P0.05) than that of nonproducing females (43.6 +/- 7.0g). Similarly, four of seven nonhibernated females (58.8+/-8.3g) produced eggs at 5 yr of age. All eggs were produced by females treated once with LHRHa. Number of eggs per female varied (141 to 3307), and development to tadpoles was low (0 to 36.5%), although tadpoles did become toadlets. In conclusion, male and female boreal toads matured at 2 and 4 yr of age, respectively, and heavier females were more likely to produce eggs. To enhance breeding success, males should be hibernated and treated with LHRHa. In contrast, female productivity was enhanced by improving their body condition instead of subjecting them to hibernation prior to LHRHa treatment.

Abstract

Tendon injuries are common in hand wounds and their functional consequences are not negligible. In emergency, tendon repairs techniques are well codified to restore function. These tendon repairs can be made in one session, in two sessions, or even use tendon transfers. Tendon transfers are not usual in the emergency but more common in the treatment of sequelae or failure of primary repair. We report one case of transfer to restore thumb flexion in emergency.

Abstract

But de la presentation Le role central du polynucleaire eosinophile (PNEo) est reconnu dans la physiopathologie de la polypose nasosinusienne (PNS). Leur presence au sein du stroma et de l’epithelium nasal est une caracteristique de la PNS dans la population europeenne. L’impact d’autres modifications histologiques comme les lesions epitheliales et le remodelage cicatriciel a aussi ete explore sans qu’une correlation precise ait pu etre etablie avec la presentation clinique de la PNS (asthme, allergie, recidive apres chirurgie). Notre objectif est d’etablir un lien entre les caracteristiques histopathologiques du polype et ces elements cliniques. Materiel et methodes Une lecture en microscopie optique par 2 observateurs etait realisee sur 23 coupes de polypes preleves au cours d’une chirurgie endoscopique des sinus pour PNS resistante au traitement medical. Etait exclu toute autre rhinosinusite chronique diffuse type oedemato-purulente et tout patient ayant beneficie d’une corticotherapie per os dans les 4 semaines precedant l’intervention. Les criteres histologiques etaient analyses de maniere standardisee selon une grille preetablie. Resultats Vingt-trois patients etaient inclus de decembre 2013 a avril 2014, soit 19 hommes et 4 femmes. La moyenne d’âge etait de 46 ans (29-76 ans). Huit patients presentaient une PNS seule, 2 etaient allergiques, 6 etaient asthmatiques, et 7 presentaient un asthme et une allergie associes. La repartition des populations cellulaires au sein des polypes etait homogene avec une infiltration majoritaire en eosinophiles et en lymphocytes. Il n’etait pas mis en evidence de differences concernant la repartition cellulaire, la structure de l’epithelium et la structure du chorion entre les patients allergiques et asthmatiques. L’analyse architecturale permettait de regrouper les polypes en 3 grands types histologiques: œdemateux avec infiltrat en PNEo majoritaire (11/23), intermediaires (4/23) et fibreux (3/23). Le contenu en glandes sero-muqueuses etait significativement plus important dans le groupe « polype intermediaire » (p = 0,049). Il n’etait pas mis en evidence de lien entre ces structures architecturales et les criteres cliniques. Conclusion L’analyse histologique dans la PNS fait sous-tendre l’existence de « plusieurs » polyposes, avec une place predominante pour les polyposes oedemateuses avec infiltrat en eosinophiles. L’implication clinique et pronostique de ces donnees morphologiques.

Abstract

The 13C-mixed-triglyceride CO2-exhalation test (MTE) has been proposed for the noninvasive assessment of intraluminal duodenal pancreatic lipase activity. Up to now, stable isotope analysis of carbon dioxide of the MTE has been carried out with isotope ratio mass-spectrometry. The aim of the present study was to evaluate the MTE in patients with morphological signs of chronic pancreatitis (stages I-III) and exocrine pancreatic insufficiency by using an isotope-selective nondispersive infrared spectrometer (NDIRS).20 healthy volunteers (9 females, 11 males, age range 19-61 years) and 16 patients (7 females, 9 males, age range 33-76 years) were examined. After an overnight fast each patient received a solid-liquid test meal containing 250 mg 1,3 distearyl, 2[13C] octanoyl glycerol. Breath samples were obtained at baseline and at 30 min intervals over a period of 6 h after the test meal. The 13C/12C isotope ratio in each breath sample was determined by NDIRS as delta (%) and delta over baseline (%). Results were expressed as cumulative percentage dose of 13C recovered (cPDR) at 3, 4, 5, 6 h and maximal PDR (PDRpeak) (median; 5./95. percentile).Significant lower values concerning cPDR 3, 4, 5, 6 hours and PDRpeak [%] were found between healthy subjects and patients with chronic pancreatitis (p0.05): cPDR 6 h: 8.1 (0.4-20.5)% vs 29.1 (10.3-59.3)%; PDRpeak: 4.7 (0.4-10.2)% vs 9.2 (5.4-14.3)%.In general, the MTE discriminates between healthy controls and patients with chronic pancreatitis and exocrine pancreatic insufficiency. However, the MTE using NDIRS cannot be recommended as a method of clinical routine because of marked data overlap between pathologic and normal values.

Cytochrome-P450-dependent liver function can be measured with the 13C-methacetin breath test (MBT). This has heretofore been done with the use of a mass-spectrometer. This study was undertaken to evaluate the MBT (NDIRS) done with the isotope-selective nondispersive infrared spectrometer.20 healthy volunteers (ten women, ten men, aged 22-76 years) and 16 patients (ten women, six men, aged 48-71 years) with histologically confirmed liver cirrhosis (Child-Pugh stage A [n = 7], B [n = 5] or C [n = 4]) were given 13C-methacetin in 100 ml of tea after a 12-hour fasting period. Breath tests were performed before the test drink and 5, 10, 15, 20, 30, 40, 50, 60, 80, 100, 120, 150 and 180 min thereafter. The ratio of 13C to 12C was determined, as delta (/1000), and from it the maximal percentage rate (PDRmax) calculated, as well as the cumulative rate (cPDRmax) after 30, 60, 120 and 180 min (Median and 5th and 95th percentiles).For patients with liver cirrhosis there were significantly lower values for cPDRmax and cPDR after 30, 60, 120 and 180 min than in the healthy subjects (P0.002): PDR [%]/h: 3.9 (0.7-15.9) vs. 36.5 (23.1-50.0); cDPR 30 min [%]: 1.1 (-0.2-6.0) vs. 12.4 (7.6-17.1); cDPR 3 h [%]: 9.8 (-2.3-27.5) vs. 36.0 (29.9-45.1). There were significant differences among the patients, depending on their Child-Pugh staging.The MBT with the cost-effective NDIRS can reliably and noninvasively distinguish between healthy subjects and patients with liver cirrhosis. The test is therefore suitable for the quantitative analysis of liver functions.

27. [Increased occurrence of esophageal hypermotility disorders in patients with arterial hypertension]

Abstract

It has been noted in previous manometric examinations of the oesophagus in patients with chest pain that abnormal motility was often associated with arterial hypertension. A systematic study of this relationship was therefore undertaken.In 40 patients with chest pain (18 women and 22 men, mean age 54.7 [24-70] years) and in 20 healthy volunteers (12 men, 8 women, mean age 50.8 [22-63] years) standardized oesophageal manometry and arterial blood pressure monitoring were performed over 24 hours. Coronary heart disease and gastrointestinal lesions had been excluded by angiography and endoscopy, respectively.20 patients (group H) had hypertension (median 24-hour blood pressure135/85 mmHg), while 20 patients (group N) and the normal controls (group K) were normotensive. Oesophageal manometry data differed significantly between the three groups regarding distal pressure amplitude (in hPa [hectopascals]; group H: 62 hPa*,**, group N 44 hPa* and group K 36 hPa**; [*P0.0005]) and the proportion of simultaneous contractions (group H 23%, group N 22%**, group K 10%***; ***P0.001). The hypertensive patients had significantly more frequent motility abnormalities than normal controls (13/20 vs 4/20, P0.001); while normotensive patients had more frequent episodes of abnormal propulsion in the oesophagus (proportion of propulsive contractions in group H: 53%, in N: 44%, in K: 59%; P0.01).Oesophageal motility differed significantly in patients with chest pain from that in healthy controls. Patients with chest pain and hypertension more frequently had oesophageal hypermotility. This suggests a generalized abnormality of smooth muscle.

Abstract

In a prospective study 27 patients (13 women, 14 men; mean age 62 [45-83] years) with Helicobacter (H.) pylori associated disease received over 7 days pantoprazole (40 mg twice daily), clarithromycin (500 mg twice daily) and metronidazole (500 mg twice daily). Six patients had gastric ulcer, 4 duodenal ulcer, 4 erosive gastritis, 6 erosive duodenitis and 7 had H. pylori-positive functional dyspepsia. Pre-treatment oesophago-gastro-duodenoscopy was combined in 4 patients with antral and in 4 others with body-of-stomach biopsies to demonstrate H, pylori (urease test, specific culture and histology). The H. pylori status was checked with the 13C-urea breath test 4 weeks after the end of treatment. In addition, 9 patients with peptic ulcer were examined endoscopically at least 2 weeks after onset of the treatment to check for any healing of the ulcers, 25 of the patients completed the study according to the protocol. The H. pylori eradication rate was 100% (25 of 25 patients), while the "intention to treat" analysis gave a rate of 92.6% (25 of the 27 patients). The peptic ulcers were found to be healed in all 9 patients who had been endoscoped. One woman developed a reversible stomatitis, but the drug treatment did not have to be stopped. -These findings indicate that short-term triple treatment in the described manner is efficacious in curing H. pylori infection and any peptic ulcer. It is thus a highly promising treatment of H. pylori-associated diseases.

Von Hippel-Lindau disease is a heritable tumour syndrome caused by the loss of the function of a tumour suppressor gene on the short arm of human chromosome 3. The interval RAF1-D3S18 (3p25–3p26) has been identfied by genetic linkage studies to harbour the von Hippel-Lindau gene. We have constructed a long range restriction map of this region and have succeeded in demonstrating the physical linkage of loci D3S726 (DNA probe LIB31-38), D3S18 (c-LIB-1, L162E5), D3S601 (LIB1963) and D3S587 (LIB 12–48). Since multipoint analysis has located D3S601 proximal to D3S726, the physical map should be oriented with D3S726 towards the telomere. Abstract

The global amphibian extinction crisis has stimulated interest in captive breeding programs for this diverse taxa. The goals of this study were to test the effects of exogenous hormone injections (LHRH and hCG) and hibernation on breeding behavior and gamete release by the boreal toad, a temperate species. The 5-year study was initiated with 77 toadlets. Each year, a subset of female and/or male toads was hibernated for 5 months at 4°C. The remaining toads were maintained in a lab receiving 12 h of light daily and ample food and water. Within 48 h of removal from hibernation, toads were paired with hibernated or non-hibernated mates. All toads were weighed and pairs were randomly assigned to treatments. Toads were injected IP with 6 μg LHRH, 250 IU hCG, or saline control and observed for amplexus and egg production. Some treatment groups received a second hormone injection if eggs were not produced within 48 h. Eggs were counted and transferred to aquariums for tadpole and toadlet development. During years 1 to 3, sperm production was assessed in male toads by sampling tank water or urine samples after injections. Data were analyzed using unpaired t-tests and chi-square analyses. In year 1, only 19% of pairs overall exhibited amplexus, none of the males produced sperm and none of the females produced eggs. In year 2, only male toads injected with LHRH exhibited amplexus, and it was more prevalent (P < 0.05) in those that were hibernated than non-hibernated (54.0% and 33.3%, respectively). Percentage of pairs in amplexus in years 2 and 3 (56.9% and 100%) was greater (P < 0.05) than in year 1. Only one and two females produced eggs in years 2 and 3, respectively. In contrast, most hibernated (87.5%) and non-hibernated (75%) males produced sperm. In year 4, eight females produced eggs, but two of these died egg-bound. Interestingly, more (P < 0.05) non-hibernated (7/10) than hibernated (1/10) females produced eggs despite no differences (P ≥ 0.05) in proportion of pairs amplexing or time spent in amplexus between groups. Mean (±SD) weight of female toads producing eggs (58.9 ± 11.9 g) was greater (P < 0.05) than that for non-producing females (43.6 ± 7.0 g). Similarly, 4/7 non-hibernated females produced eggs in year 5, and their weights averaged (58.8 ± 8.3 g). All eggs were produced by females injected a single time with LHRH. Number of eggs per female varied (141–3307), and percentage of eggs developing to tadpoles was low (0–36.5%), but tadpoles were capable of developing into adult toads. In conclusion, two-year-old male boreal toads are mature, exhibiting amplexus and releasing sperm in response to LHRH injections. Females appear to mature at four years of age, and eggs are more likely to be produced by heavier females. Therefore, to enhance breeding success, males should be hibernated and injected with LHRH. In contrast, better productivity in females may be achieved by improving their body condition instead of subjecting them to winter hibernation prior to LHRH injection. Abstract

Since Kloeze [13] demonstrated that prostaglandin E1 (PGE1) inhibits platelet aggregation, its role as a regulator of platelet activity has been extensively studied. Additionally, this substance is a potent vasodilator and can increase regional peripheral blood flow by reduction of the total peripheral resistance. Concerning these activities, PGE1 was introduced into the clinics for the treatment of severe peripheral arterial occlusive disease in 1973 by Carlsson and Eriksson [6] and its clinical efficacy after intraarterial and intravenous infusion was demonstrated in numerous clinical trials (for review see [25]).

Abstract

Daily administration of the trapidil derivative AR 12463 (20 mg/kg body weight i.p.) to cholesterol-fed rabbits diminished statistically significantly the increase in serum total cholesterol. After 8 weeks of treatment all measured lipoprotein fractions were significantly lower in animals treated with AR 12463 (group 3) compared with the values of the untreated control (group 1) or vehicle-treated group (group 2). The reduction of serum levels was associated with statistically significantly reduced levels of cholesterol esters in kidney, liver and aorta. The levels of free cholesterol in the liver of group 3 animals were statistically significantly lower compared with the levels in the two other groups, whereas in kidney and aortas the levels of free cholesterol remained unchanged under the influence of AR 12463. The area of aorta covered with fatty streaks was significantly smaller in group 3 versus group 1. The results of this study indicate that treatment of rabbits with AR 12463 while feeding a cholesterol-rich diet prevents hypercholesterolemia as well as the cholesterol incorporation into tissues. The mode of action of AR 12463 on serum and tissue lipids as well as on the development of atherosclerosis is discussed.

The actions of the in vivo metabolite of PGE1, 13,14-dihydro-PGE1 (PGE0), on platelet and neutrophil (PMN) function and vessel tone were studied in vitro. PGE0 inhibited aggregation, ATP release and thromboxane generation by human platelets (IC50 10-100 nmoles/l). The compound also inhibited superoxide anion generation and lysosomal enzyme release from human PMN. PGE0 was equipotent to PGE1 in both systems, while 15-keto-PGE1 was ineffective in platelets but produced some inhibition in PMN. These inhibitory effects of PGE0 and PGE1 were paralleled by concentration-dependent increases in cyclic AMP in platelets and PMN. Both compounds were also potent relaxants of several arterial preparations in the rabbit at comparable concentrations. In general, the vasorelaxing properties of PGE0 were somewhat lower and the contractile effects somewhat stronger in comparison to PGE1. These data demonstrate a significant and concentration-dependent inhibition of receptor-mediated activation of human platelets and PMN by PGE0. The molar potency of the compound is comparable to that of PGE1. Generation of PGE0 from infused PGE1 may contribute to the clinical efficacy of PGE1 in treatment of severe peripheral arterial occlusive disease.

Abstract

This study investigates the effects of 13,14-Dihydro-PGE1 (PGE0) in comparison to PGE1 on human platelets, human polymorphonuclear granulocytes (PMN) and a number of vessel preparations of different species. The potency of PGE0 in inhibition of platelet and neutrophil activation is similar to PGE1. The vascular action differs. At a comparable molar potency, PGE0 showed stronger contractile and less relaxing effects than PGE1. The biological activities of PGE0 may contribute to the in vivo effects of PGE1 in treatment of peripheral arterial occlusive disease, in particular at high-dose i.v. administration.

Abstract

Modulations of spongiosa density in the femoral head of rabbits by atherosclerosis. In a long-term study 31 rabbits (+5 control animals) were fed about 20 weeks on a diet rich in cholesterol. An important observation in this study was, that spongiosa density in the femoral head decreases after cholesterol rich diet in contrast to the control group. This effect could be modulated by the Trapidil AR 12463 content (50 mg/die) of the diet, but not by the high density lipoprotein application. Serum cholesterol levels and induced atherosclerosis were determined. Correlations between spongiosa density and atherosclerosis were discussed.

Abstract

Summary: The distribution of B and T lymphocytes and macrophages in the middle ear fluid (MEF) was studied in groups of children with otitis media associated with serous, seromucinous, or mucoid effusions in the middle ear. The subpopulations of T and B lymphocytes and macrophages were identified in the MEF by employing the techniques of spontaneous erythrocyte rosetting (E), membrane immunofluorescence, erythrocyte-amboceptor-complement (EAC) and erythrocyte-amboceptor (EA) rosette formation, respectively. A significant number of B and T lymphocytes or macrophages were detected in all specimens; however, certain differences were observed in their relative distribution in different types of effusions. Serous MEF was characterized by a cellular response associated predominantly with macrophages, although T lymphocytes were also seen frequently. No B lymphocytes were seen in such exudates. The seromucinous MEF contained B and T lymphocytes and macrophages in approximately similar proportions. Significantly, however, the mucoid effusion consisted mainly of B lymphocytes. Only macrophages and no T cells were observed in these effusions. The cytologic findings reported here may be helpful in a more definitive classification of middle ear effusions. These data may suggest a possible role of the different components of cellular immunity in the development of middle ear effusions. Speculation: E rosette-forming cells predominate in the serous and seromucinous effusions of otitis media. EAC rosette-forming cells appeared as a predominant cell type in mucoid effusions. The different types of effusions in otitis media may represent the various stages of the same inflammatory process. The differences in the types of immunocompetent cells observed may reflect the nature of the underlying immune response in serous, seromucinous, or mucoid forms of otitis media.

A cyst involving the gland of the 3rd eyelid was removed from an 8-year-old dog. Clinically the circumscribed mass resembled a pigmented neoplasm. Histologically the cyst was multiloculated, with homogeneous eosinophilic periodic acid-Schiff-positive content. Melanin pigment was present in epithelial cells and cystic lumens. Protective functions of the 3rd eyelid were retained following excisional biopsy.

ConclusionInjection of mice with 30 mg of GSH, especially intravenously, was complicated by the unphysiologic, extracellular distribution of the exogenous glutathione. The GSH given to mice to replace nonprotein sulfhydryl lost as a result of endotoxin shock provided only slight protection. Survival was significantly improved by intraperitoneal injection of 30 mg of GSH 4 hr before the endotoxin. Thus, there appears to be little therapeutic potential for GSH in endotoxin shock.

Abstract

International audience; Aims In degenerative mitral regurgitation (DMR), lack of mortality scores predicting death favours misperception of individual patients' risk and inappropriate decision-making. Methods and results The Mitral Regurgitation International Database (MIDA) registries include 3666 patients (age 66 +/- 14 years; 70% males; follow-up 7.8 +/- 5.0 years) with pure, isolated, DMR consecutively diagnosed by echocardiography at tertiary (European/North/South-American) centres. The MIDA Score was derived from the MIDA-Flail-Registry (2472 patients with DMR and flail leaflet-Derivation Cohort) by weighting all guideline-provided prognostic markers, and externally validated in the MIDA-BNP-Registry (1194 patients with DMR and flail leaflet/prolapse-Validation Cohort). The MIDA Score ranged from 0 to 12 depending on accumulating risk factors. In predicting total mortality post-diagnosis, the MIDA Score showed excellent concordance both in Derivation Cohort (c = 0.78) and Validation Cohort (c = 0.81). In the whole MIDA population (n = 3666 patients), 1-year mortality with Scores 0, 7-8, and 11-12 was 0.4, 17, and 48% under medical management and 1, 7, and 14% after surgery, respectively (P < 0.001). Five-year survival with Scores 0, 7-8, and 11-12 was 98 +/- 1, 57 +/- 4, and 21 +/- 10% under medical management and 99 +/- 1, 82 +/- 2, and 57 +/- 9% after surgery (P < 0.001).
