Abstract

Two hundred and eight pregnancy sera were tested for the presence of antibodies specific for lymphocyte sub-populations by using the isolated B and T lymphocytes from the women's mating partners. This was done by the microlymphocytotoxicity and the indirect immunofluorescence techniques. Five sera (2.5%) reacted exclusively with B lymphocytes and sixty-three sera (30.2%) reacted with both B and T lymphocytes; none of the sera was specific for T cells. Several sera, reacting with both B and T lymphocytes, were absorbed with platelets and this procedure revealed nine additional antiseraa specific for B lymphocyte antigens. Specificity studies on a panel of forty-eight HLA-ABCD typed individuals indicated that most antisera possibly defined new B-cell antigens. Family studies established that the antigens defined by these antiser were coded for by genes in the Major Histocompatibility Complex.

Abstract

During the VIIth Histocompatibility Workshop, 44 sera were selected that defined B-cell alloantigens showing correlation with the HLA--D antigenic determinants. The segregation of these antigens, now called DRw (HLA--D-related), was studied in a family that contained one HLA--A/B recombinant and one HLA--B/D recombinant sibling. It could be established that the DRw-1 antigenic determinants segregated with the B-D region in the A/B recombinant sibling and that the DRw-2 antigen segregated with the D region of the B/D recombinant child. In the second family, DRw typing segregated with the D region of the B/D recombinant child. In the second family, DRw typing showed that one of the children was identical with an HLA--ABC non-identical sibling. This suggested that a crossing-over had occurred. This was confirmed by mutual non-stimulation of these HLA--ABC non-identical children in the mixed lymphocyte culture. This crossing-over involved the DRw-3 alloantigen. These data confirm the assumption that serologically defined B-cell alloantigens are coded for by genes located outside the HLA--A/B region on the B-D side of chromosome. Moreover, these data suggest that, within families, serological B-cell genotype identity can predict a mutual unresponsiveness in the lymphocyte culture. However, findings in these two families indicate that discrepancies nevertheless exist between B-cell serology and mixed lymphocyte reactions and that mutual stimulation can occur despite apparent identity of serological B-cell phenotype and that B-cell phenotype identity does not necessarily predict mutual unresponsiveness in the mixed lymphocyte culture.

Abstract

A new HLA-A antigen, called LN, is described in this report. It was defined by a panel of antisera against the antigens that make up the HLA-A10 cross-reacting group of antigens. Population and family studies confirm that the antigen is a product of an allele at the HLA-A locus. An unusual CYNAP phenomenon is described, i.e. CYNAP was obtained with one or more of the antigens making up the HLA-A10 cross-reacting group.

Abstract

Since the first successful haematopoietic stem cell transplantation (HSCT) using cord blood from a sibling in 1988 and the establishment of the first public umbilical cord blood bank in New York Blood Center in 1992, umbilical cord blood banks have been instituted in many countries. Funding was received from regular blood banks, health councils, charity funds or commercial investments. The international medical society is indebted to the New York Blood Center for publishing their procedures and EuroCord for fighting a patent on cord blood processing. Although it is unknown how many cord blood samples are currently banked and have been transplanted worldwide, the figures of the international registration of World Marrow Donor Association (WHDA) show an increase of cord blood use, in addition to other sources of unrelated HSCT (Fig. 1). Cord blood for HSCT is, in addition to national/regional use, exchanged worldwide. Donor counselling, human leucocyte antigen (HLA)-typing, tests for transmittable diseases and product quality control requirements to comply with (inter)national [AABB, Paul-Ehrlich-Institut, NetCord/ Foundation for the Accreditation of Cellular Therapy (FACT)] standards are expensive, making storage of cord blood for unrelated allogeneic haematopoietic transplantation currently a loss-making activity, not particularly attractive for private enterprise. In the last years, hopes have been fueled that other stem cells in cord blood may be used for future repair of metabolic or degenerative diseases. Abstract

Summary. The purpose of this study was to determine whether quantitative or qualitative factors are of major importance in the destruction of red cells sensitized with incomplete warm autoantibodies of subclass IgG1. To that end, the relative amount of IgG1 antibody present on the red cells of patients with autoantibodies of this subclass only, was measured by means of continuous flow cytofluorometry. This method appeared to give an idea of the amount of antibody on red cells and was reproducible. The intensity of the fluorescence of patient's red cells, measured after incubation with a FITC-labelled anti-IgG1, was compared with the presence or absence of signs of increased haemolysis in vivo and the cytotoxic activity of normal monocytes towards these red cells in vitro. It appeared that it was predominantly the amount of IgGl autoantibody that determined whether or not these antibodies induced haemolysis in vivo or cytotoxicity of monocytec in vitro. This was also true with methyldopa-induced IgG1 autoantibodies.

Since the provision of blood and blood products depends entirely on the willingness of donors to spend their blood for this purpose, we felt that an International Forum of Vox Sanguinis should be devoted to donor recruitment and in particular to ‘donor loyalty’ and to ‘barriers to blood donation’. To obtain information on these subjects, the following questions were sent to experts in the field. Donor loyalty: Question 1 . How important do you consider donor loyalty and for which reasons? Question 2 . Do you have a method to evaluate donor loyalty? Question 3 . Which factors do you believe to have a positive, and which factors a negative effect on donor loyalty? Question 4 . Have you been able to influence donor loyalty and, if so, how? Barriers to blood donation: Question 5 . Have you been able to identify the key barriers that prevent eligible subjects to become blood donors? Question 6 . Which percentage of first-time donors do not return to donate for a second time and have you learned their reason(s) for not returning? Question 7 . How have you been able to persuade first time (and repeat) donors to continue to donate if they were not inclined to do so. Unfortunately, we have been able to obtain only five contributions to this forum, which, however, seem of sufficient interest to warrant its publication.

Abstract

Fetal alloimmune thrombocytopenia is caused by maternal sensitization to paternally-derived antigens on fetal platelets, most commonly HPA-1a.1 It occurs in approximately 1 in 1000 live births and is the commonest cause of severe fetal and neonatal thrombocytopenia, and of intracranial hemorrhage in neonates born at term.2 Since there is currently no routine screening, first-time cases of fetal alloimmune thrombocytopenia are generally identified following the birth of a markedly thrombocytopenic neonate. Antenatal management is thus only possible in subsequent pregnancies. Intracranial hemorrhage is the most devastating complication of fetal alloimmune thrombocytopenia and often occurs antenatally. Assessment of projected clinical severity is thus based on the development of intracranial hemorrhage in a previous sibling. If there is such a history of intracranial hemorrhage, the chance of this complication occurring again in the next pregnancy is extremely high in an untreated, antigen-positive sibling.3 Administration of intravenous immunoglobulin (IVIG) to the mother, initially given in conjunction with dexamethasone, was first used to prevent recurrence of antenatal intracranial hemorrhage in 1988.4 This approach of providing IVIG-based medical therapy administered to the mother to increase the fetal platelet count has since been extensively investigated in hundreds of maternal-fetal pairs.5 The efficacy of IVIG-based therapy has been supported by numerous studies6–16 (Table 1A) but not by others17–19 (Table 1B). The studies presented in Tables 1A and 1B surprisingly report virtually identical percentages of cases of intracranial hemorrhage: 2.7% versus 2.9%, respectively. However, overall mean birth platelet counts differed markedly between the two groups. While platelet counts are considered to be surrogate markers of intracranial hemorrhage, fortunately, the likelihood of fetal and neonatal intracranial hemorrhage, in the absence of this complication having occurred in a previous sibling, is relatively low.

Abstract

Neither canimmunization against platelet-specific al-loantigens be prevented by removing leuco-cytes from platelet concentrates, but platelet-specific antibodies seem to occur mainly inpatients who develop HLA class I antibodies.Not all patients with HLA antibodies are re-fractory. Often the antibodies only react witha small percentage of panel lymphocytes andthen most random donors are compatible.Furthermore, although there are individualdifferences, some HLA class I antigens areweakly expressed on platelets. Nevertheless,refractory alloimmunized patients in need ofplatelet transfusions will continue to occur.Opinions on the optimal management of suchpatients differ. Moreover, some simple and in-teresting new crossmatch techniques have re-cently been developed. Abstract

In cases of Rh D alloimmunization, strong results in the antibody-dependent cell-mediated cytotoxicity (ADCC) assay (> 80% lysis as compared to that of the standard anti-D serum) are indicative of severe hemolytic disease to occur in the newborn (HDN). However, discrepant cases were found in which the maternal anti-D gave strong ADCC results and the newborns had no or only mild hemolysis. In the majority of these cases the mother had produced monocyte-reactive IgG alloantibodies, mostly with HLA-DR specificity. Such antibodies may be capable of blocking FcR-mediated functions of the fetal MPS, and it has been postulated that they inhibit destruction of anti-D-sensitized red cells. We here describe 2 families in which such discrepancies were noticed. In 1 family, in spite of ADCC results of > 80%, the Rh D-positive second child was born without signs of hemolysis. However, the Rh D-positive third child suffered from very severe hemolytic disease. The mother had produced monocyte-reactive HLA-DR antibodies in the second pregnancy which, however, did not react with the cells of the third child. In the other family, the severely Rh D-alloimmunized mother had lost her fourth child because of intrauterine death due to HDN. The Rh D-positive fifth child was born with only mild HDN and only in this pregnancy had the mother produced such antibodies. These 2 case histories give further evidence that maternal monocyte-reactive alloantibodies, in both these cases directed against HLA-DR antigens, can protect the Rh D-positive child against hemolytic disease in case of severe Rh D alloimmunization.
