1. European Registry on the management of Helicobacter pylori infection: features of diagnosis and treatment in Kazan

Abstract: Background. As part of an observational multicenter prospective study European Registry on the management of Helicobacter pylori infection, conducted on the initiative of the European H. pylori and Microbiota Study Group, the compliance of clinical practice in the management of patients with Helicobacter pylori infection in Kazan with clinical guidelines was assessed. Materials and methods. The data of 437 patients included into the register by clinical sites in Kazan in 20132019 were analyzed. The methods used for the initial diagnosis of H. pylori infection and eradication control were evaluated. The frequency of various eradication therapy regimens prescription was analyzed in 379 cases. Data regarding the effectiveness of eradication therapy was analyzed in 173 patients. Results. The rapid urease test (44.2% of cases) and cytology/histology (60% of cases) were most often used for the initial diagnosis of H. pylori infection; however non-invasive methods such as 13C-urea breath (9.2%), serology (6.2%), H. pylori stool antigen test (2.3%) were less common. In 21.7% of patients two methods of H. pylori detection were used for primary diagnosis. The control test to evaluate the effectiveness of eradication therapy at the recommended timepoint was performed in 46.2% of patients. 13C-urea breath test (31.7%), stool PCR/stool antigen test (28.7%), rapid urease test (22.3%), cytology/histology (26.2% of cases) prevailed in the assessment of eradication rate. Standard triple therapy, including proton pump inhibitor, clarithromycin and amoxicillin was most commonly prescribed as first-line therapy (64.6% of cases). The duration of eradication therapy was 14 days in the majority of cases with pantoprazole as the most common proton pump inhibitor in standard triple therapy regimens (84.8%). The efficacy of 14-day standard triple therapy (mITT) was 87.0%. Conclusion. The results indicate a high frequency of non-invasive methods use for assessing the effectiveness of eradication therapy; however, the overall rate of eradication efficacy assessment is low, limiting the possibility of analyzing the eradication results. The effectiveness of the most common 14-day standard triple first-line therapy in Kazan doesnt reach the recommended 90% eradication level. This could be explained by high rate of pantoprazole use, which is not an optimal proton pump inhibitor in eradication therapy regimens.

2. The role of rifaximin in irritable bowel syndrome derived from a network meta-analysis of randomized control trials

Abstract: Objective: Recent randomized control trials (RCTs), have demonstrated the beneficial therapeutic effects of rifaximin for the treatment of the irritable bowel syndrome (IBS). However, in these studies different drug doses have been used and still the optimal therapeutic dose is missing. We aimed to determine rifaximin therapeutic benefit and optimal dose for IBS as evidenced by the results of a network meta-analysis (NWM) of published randomized controlled trials (RCTs). Materials and Methods: PubMed/MEDLINE, EMBASE, and Cochrane Library databases were searched for RCTs investigating the therapeutic effects of rifaximin on IBS through December 2019. Data from each selected RCT were evaluated individually based on an intention-to-treat analysis. A Bayesian NWM was performed to investigate the efficacy rank order of rifaximin therapeutic interventions in IBS. Results: Four eligible studies, including 5 sets of data, were included in this NWM. They included 1,803 IBS patients, randomized to placebo (908 patients), and rifaximin (895 patients). In patients who received rifaximin, four regimens were examined, i.e., (A) = 400 mg tds for 10 days, (B) = 400 mg bid for 10 days, (C) = 550 mg bid for 2 weeks and (D) = 550 mg tds for 2 weeks. The results showed that in IBS rifaximin 400 mg tid for 10 days showed the highest efficacy [SUCRA (surface under cumulative ranking) value 89.5%], in comparison to other rifaximin regimens used and placebo. Conclusions: This NWM showed that the therapeutic efficacy of rifaximin 400 mg tid in IBS patients was greater than that of placebo and the other rifaximin doses studied.

Abstract: The emergence of the microbiome as a significant factor influencing health and disease across a number of organ systems has posed a new challenge for those wishing to identify the optimum treatments for Helicobacter pylori infection. This review aims to address some of these questions. There is a specific concern in the clinical setting about a spectrum of sequelae of this ranging from mild gastrointestinal upset to life-threatening conditions, such as pseudomembranous colitis, as well as overweight and obesity. The literature on the changes induced in the gut microbiota related to H. pylori eradication can be best considered as those which study immediate effects, short-term effects, and long-term effects. Immediate effects usually include decreased bacterial diversity, however, the use of probiotics may reduce the magnitude of change. Studies looking at short term change have consistently shown bacterial diversity to be altered three months after eradication therapy. Most of the long-term studies have shown regression to baseline microbiome diversity after one year, however, this may not be the case for quadruple therapies. Other challenges around the development of antibiotic resistance and the effect of proton pump inhibitors are also discussed.

Abstract: Background: Helicobacter pylori eradication requires a combination of antibiotics. The short and long-term effect of different H. pylori eradication regimens on the gut microbiota has received little attention. Aims: To evaluate and compare the impact of the two most frequently recommended first-line H. pylori eradication regimens — bismuth and non-bismuth quadruple regimens — on the gut microbiota. Patients and Methods: Two 14-day quadruple treatments were evaluated and compared: bismuth-based quadruple therapy (proton pump inhibitor, bismuth, tetracycline, and metronidazole) and non-bismuth quadruple “concomitant” therapy (proton pump inhibitor, amoxicillin, clarithromycin and metronidazole). Stool samples were collected before, and at two and six months after finishing treatment. Gut microbiota was analyzed from the stool samples by sequencing of the 16S rRNA gene and sample-specific barcode sequences. Results: 51 patients were initially included and 38 completed the six months follow-up. A total of 136 stool samples were analyzed. Eradication rates were similar between treatments (bismuth 90%, non-bismuth 89%). Tolerance was also similar. Both treatment regimens induced a significant decrease in alpha diversity two months after treatment, which was partially recovered at six months. Beta diversity was also similarly affected by both treatments and partially recovered at six months follow-up. Longitudinal pairwise Aichison distances were calculated: large individual variation was observed during treatment and at follow-up for both regimens but the average distance was similar, i.e., 1.70 and 1.74, respectively, at two months after treatment, and 1.19 and 1.14 at six months. Statistically significant changes of specific taxa were also observed and these were different for each treatment. Conclusions: H. pylori eradication using quadruple therapies significantly reduces microbiota diversity, although it is partially recovered six months after the end of treatment. No difference was found between bismuth and non-bismuth quadruple therapies; therefore, other aspects should guide prescription.

5. Current trends in the treatment of H. pylori infection in Ukraine: results of the European Registry on Helicobacter pylori Management (Hp EuReg)

Abstract: Current trends in the treatment of H. pylori infection in Ukraine: results of the European Registry on Helicobacter pylori Management (Hp EuReg) The European Registry on Helicobacter pylori Management (Hp EuReg) was established to evaluate the efficacy and safety of the different eradication H. pylori treatments, as well as to audit H. pylori infection consensus and clinical guidelines implementation. The current study presents data from a sample of patients from Ukraine who were registered in the Hp EuReg until February 2022. The aim was to assess the frequency, effectiveness and safety of treatment against H. pylori infection in Ukraine. Materials and methods. The Hp EuReg, an international multicenter prospective non‑interventional registry, was launched in 2013 and is promoted by the European Helicobacter and Microbiota Study Group (www.helicobacter.org). The Hp EuReg protocol was approved by the Ethics Committee of La Princesa University Hospital (Madrid, Spain) and registered with Clinical Trials.gov under the code NCT02328131. The effectiveness analysis included mainly a modified intention‑to‑treat (mITT) analysis, aiming to mimic the clinical practice outcomes and including all records with a confirmatory test after the eradication treatment, regardless of compliance. Given that some treatment regimens were used only in a small number of patients, only regimens with a relevant number of patients were selected for analyses. Results. Overall 841 patients (54 % women, 46 % men), whose average age (SD) was 48.35 (± 15.52) were included in the register. The main method for diagnosing on‑going H. pylori infection was histology, and the use of urea breath tests and stool antigen tests was low — (approximately 6 % of cases). Assessment of bacterial antibiotic resistance was performed in 13 % of patients, with less than 1 % reporting a positive result. More than half of the patients received quadruple therapy and 40 % triple therapy. Over 90 % of patients were treatment‑ naïve. Almost all patients received low‑dose proton pump inhibitors (PPIs), and a smaller proportion received standard (7.2 %) or high‑dose PPIs (1.2 %). Low‑doses PPI prescriptions negatively affected the effectiveness of therapy: in treatment‑naïve patients’ overall effectiveness was below 85 %, while the use of standard‑ and high‑doses of PPIs achieved 100 % eradication rates in all patients. Despite the fact that the duration of therapy in the vast majority of patients was at least prescribed for 10 days, there were cases (12 %) of administration of 7‑day therapy with the reported suboptimal (< 90 %) effectiveness. In those prescribed with a first‑line treatment of 7 — 10 days, the eradication rate was 82 %, while therapy for 14 days provided 100 % treatment eradication success. In the overall mITT analysis, the use of therapy in naïve patients resulted in the eradication of H. pylori in more than 90 % of patients. It is noteworthy that effectiveness with PPI+ amoxicillin + levofloxacin was below 84 %, suboptimal according to the most up‑to‑date consensus. The administered therapy was well tolerated, which allowed to complete the treatment of more than 90 % of patients, and the incidence of adverse events was less than 4 % in naïve patients, while in patients with repeated courses of therapy it was more than 3 times higher. Conclusions. In Ukraine, the frequency of use of validated non‑invasive methods (urea breath tests and stool antigen test) to detect H. pylori infection is very low. However, Ukrainian doctors often use methods to assess the antibiotic resistance of Helicobacter pylori, which certainly helps to increase the rate of therapy‑guided eradication. The vast majority of eradication therapies administered to patients in the Ukrainian population corresponds to the current guidelines, but very often PPIs were administered in low doses and there were still some cases of regimens lasting 7 days, even in those patients receiving a second‑line therapy, which reduced the treatment effectiveness. Even though overall effectiveness of eradication therapy was over 90 %., there is a need to closely monitor treatment management. Prescribed first‑line therapy was well tolerated by patients — more than 98 % of patients completed treatment.

Abstract: Environmental and lifestyle factors play an important role in the natural history of Crohn's disease and ulcerative colitis. A group of international experts from the International Organization for the Study of Inflammatory Bowel Diseases voted on a series of consensus statements to inform the management of inflammatory bowel disease (IBD). The recommendations include avoiding traditional cigarette smoking in patients with Crohn's disease or ulcerative colitis, screening for symptoms of depression, anxiety, and psychosocial stressors at diagnosis and during flares (with referral to mental health professionals when appropriate), and encouraging regular physical activity as tolerated. Patients using dietary approaches for treatment of their IBD should be encouraged to adopt diets that are best supported by evidence and involve monitoring for the objective resolution of inflammation. We recommend formal assessment for obesity and nutritional deficiencies, and patients should be encouraged to maintain a normal body-mass index. A shared decision-making approach to contraception should include the consideration of IBD-related factors, and risk factors for venous thromboembolism. Long-term or frequent use of high-dose non-steroidal anti-inflammatory drugs should be avoided. For primary prevention of disease in the offspring of patients with IBD, we recommend avoiding passive exposure to tobacco, using antibiotics judiciously, and considering breastfeeding when able.

Abstract: Introduction: The impact of consensus, prescription choices and efficacy trends on clinical practice over time has not been studied in depth.Aims & Methods: International multicenter prospective non-intervention-al registry aimed to evaluate the decisions and outcomes of H. pyloriman-agement by European gastroenterologists. All infected adult patients were systematically registered at AEG-REDCap e-CRF from 2013 to December 2021. Variables included: previous eradication attempts, prescribed treatment, adverse events, and outcomes. Modified intention- to-treat (mITT) and time trend analyses were performed. Data were subject to quality review.Results: So far 49, 739 patients from 29 European countries have been in-cluded, and 35, 203 (71%) were first-line empirical prescriptions. Overall, the most common prescribed treatments in the 2013-21 period were triple therapies ; however, a shift in antibiotic regimens was identi-fied. Triple therapies decreased from over 50% of prescription in 2013/15 to less than 25% in 2020/21 ; likewise, non-bismuth concomitant therapy use decreased from 21% in 2013/14 to 13% in 2020/21, while three-in-one single- capsuleincreased from 0- 1% in 2014/2015 to 17% in 2020/21. An increase in the average duration of treatments from 9.8 days in 2013 to 13 days in 2021 was identified, as well as the daily high- dose of PPIs increasing from 16% in 2013 to 50% in 2021. Detailed description of most common treatments is shown in Table 1. Re-garding the effectiveness of each specific treatment, no trend was identi-fied (data now shown) ; however, there was a 10% overall improvement in first-line mITT overall effectiveness from 2013 to 2021. Conclusion: European gastroenterological practice is constantly adapting to the newest published evidence and recommendations (reducing the use of triple therapies and increasing both the duration of treatment and the dose of PPIs), with a subsequent progressive improvement in overall effectiveness.

Abstract: Introduction: Management of Helicobacter pylori infection (H. pylori) re-quires co- treatment with proton pump inhibitors (PPIs) and the use of an- tibiotics to achieve successful eradicationAims & Methods: To evaluate the role of PPIs in the eradication of H. py-lori, based on the “European Registry onHelicobacter pylori Management” (Hp- EuReg).The Hp-EuReg is a multicentre, prospective, non-interventional, registry assessing decisions and outcomes of H. pylori management by European gastroenterologists in routine clinical practice. Countries recruiting more than 1, 000 patients were included. Effectiveness was assessed by modi- fied intention-to-treat (mITT). Data were quality reviewed.Results: Overall, 22, 078 patients from 5 countries were analysed. Optimal (»90%) first-line mITT effectiveness was achieved with the following treatments: 14-day clarithromycin- amoxicillin- bismuth quadruple therapy, and 14- day metronidazole-tetracycline-bismuth quadruple therapy, both inde-pendently of the PPI dose prescribed ; in all 10- or 14-day therapies with high-dose PPIs (except with standard 10-day triple therapy) ; in 10-day se-quential and 10- day quadruple clarithromycin-amoxicillin- bismuth, and 10-day quadruple metronidazole- tetracycline- bismuth with standard-dose PPIs. None of the 7-day therapies achieved optimal cure rates re-gardless of the PPI dose used. The PPI dose was significantly associated with the effectiveness of: 10- or 14- day standard triple and quadruple con-comitant therapies, and 10-day quadruple sequential and metronidazole-tetracycline-bismuth therapies. Table 1 details the mITT effectiveness by duration and PPI dose used.Conclusion: In first-lineH. pylori treatment, ≥90% effectiveness is obtained with standard-dose PPIs in 10-day sequential and 10- or 14-day bismuth quadruple therapies and with high-dose PPIs in all 10- or 14-day therapies except with standard 10-day triple therapy.

Abstract: Introduction: Antibiotic resistance is the main factor influencing the ef-ficacy of treatments to eradicate the infection by Helicobacter pylori (H. pylori). The treatments including clarithromycin are the most frequently used in first-line ; however, the bacterial resistance to clarithromycin in treatment-naïve patients in Europe is, overall, >20%.Aims & Methods: To assess the effectiveness of first-line H. pylori treat- ments in Europe according to the resistance to clarithromycin.Non-interventional, prospective, multicenter, international European re-gistry on the management of H. pylori (Hp-EuReg). All culture-diagnosed infected adult patients were registered at AEG-REDCap e-CRF from 2013 to 2021. Per-protocol (PP) analysis was performed based on the presence or absence of bacterial resistance to clarithromycin in treatment-naïve pati-ents. The data was subject to quality control.Results: In total, 2, 479 culture-naïve patients were included. Of these, 627 (25%) reported resistance to clarithromycin. The eradication rate with the standard triple therapy (PPI+clarithromycin+amoxicillin) in patients susceptible to clarithromycin was 92% (424/460). The eradication rate in these clarithromycin-susceptible patients was higher both with the con-comitant regimen (PPI+clarithromycin+amoxicillin+metronidazole) and with the sequential treatment with tinidazole (95%, 882/929), and also with the bismuth quadruple regimen (PPI+bismuth+tetracycline+metronidazole) as single capsule (100%, 67/67) (p

Abstract: Helicobacter pylori is a recognized cause of a variety of gastroduodenal pathology. The high prevalence of both H pylori infection and related diseases within the community warrants its consideration as a public health care issue. The availability of reliable and safe noninvasive diagnostic techniques coupled with the development of effective and tolerable treatments has enabled primary health care personnel to manage this infection actively. The role of the primary care physician in the future management of H pylori infection is thus of central importance. The wealth of evidence produced by over 15 years of research into H pylori has expanded the list of disease associations and treatment benefits as well as elucidated the pathophysiological mechanisms involved. As a result, there has been a growing need to harmonize this information with clinical practice and to provide direction for the appropriate management by both specialists and general practitioners. Several national guidelines have been produced. The areas relating to H pylori infection that they considered and their recommendations vary. In 1994, the National Institutes of Health produced globally accepted recommendations for the management of H pylori-related peptic ulceration. The broader role of H pylori as a gastroduodenal pathogen and a public health care issue was not addressed. Recently, European and Canadian consensus guidelines have been published that identified overall management issues, including the role of primary and specialist care, and considered the appropriateness of employing eradication therapy for the spectrum of conditions in which H pylori has a direct or indirect association based on the available information. These guidelines, while in agreement regarding many issues, differ considerably in their recommendations for primary health care and regarding central issues such as the management of dyspepsia and gastric cancer. Some variations may reflect differing health care structures as well as the prevalence of both infection and associated diseases. However, the interpretation of evidence produced by recent research contributes to their conflicting statements.

Abstract: BACKGROUND: Gastroparesis is a condition characterized by epigastric symptoms and delayed gastric emptying (GE) rate in the absence of any mechanical obstruction. The condition is challenging in clinical practice by the lack of guidance concerning diagnosis and management of gastroparesis. METHODS: A Delphi consensus was undertaken by 40 experts from 19 European countries who conducted a literature summary and voting process on 89 statements. Quality of evidence was evaluated using grading of recommendations assessment, development, and evaluation criteria. Consensus (defined as ≥80% agreement) was reached for 25 statements. RESULTS: The European consensus defined gastroparesis as the presence of symptoms associated with delayed GE in the absence of mechanical obstruction. Nausea and vomiting were identified as cardinal symptoms, with often coexisting postprandial distress syndrome symptoms of dyspepsia. The true epidemiology of gastroparesis is not known in detail, but diabetes, gastric surgery, certain neurological and connective tissue diseases, and the use of certain drugs recognized as risk factors. While the panel agreed that severely impaired gastric motor function is present in these patients, there was no consensus on underlying pathophysiology. The panel agreed that an upper endoscopy and a GE test are required for diagnosis. Only dietary therapy, dopamine-2 antagonists and 5-HT4 receptor agonists were considered appropriate therapies, in addition to nutritional support in case of severe weight loss. No consensus was reached on the use of proton pump inhibitors, other classes of antiemetics or prokinetics, neuromodulators, complimentary, psychological, or more invasive therapies. Finally, there was consensus that gastroparesis adversely impacts on quality of life and healthcare costs and that the long-term prognosis of gastroparesis depends on the cause. Abstract: Background: The impact of consensus, prescription choices and efficacy trends on clinical practice over time has not been studied in depth. Methods: International multicenter prospective non-interventional registry aimed to evaluate the decisions and outcomes of H. pylori management by European gastroenterologists. All infected adult patients were registered at AEG-REDCap e-CRF up to February 2021. Modified intention-to-treat (mITT) and time trend analyses were performed. Results: So far 29, 634 first-line empirical prescriptions from 31 European countries have been included. Overall, the most common prescribed treatments in the 2013-20 were triple therapies ; however, a shift in antibiotic regimens was identified. Triple therapies decreased from over 50% of prescription in 2013/15 to less than 20% in 2018/20. Non- bismuth concomitant therapy use decreased from 21% in 2013/14 to 13% in 2019/20, while Pylera® increased from 0-1% in 2014/2015 to 19% in 2019/20. An increase in the average duration of treatments from 11 to 13 days in 2013-2020, and of the daily dose of PPI, was identified (No trend was identified (data now shown) ; however, there was an 8% overall improvement in first-line mITT overall effectiveness from 2013 to 2020 (Table 1). Conclusions: European gastroenterological practice is constantly adapting to the newest published evidence and recommendations (reducing the use of triple therapies and increasing the duration of treatment and the dose of PPIs), with a subsequent progressive improvement in overall effectiveness.

Abstract: Introduction: The best approach for Helicobacter pylori management re-mains unclear. An audit process is essential to ensure clinical practice is aligned with best standards of care.Aims & Methods: International multicentre prospective non-intervention-al registry starting in 2013 aimed to evaluate the decisions and outcomes in H. pylori management by European gastroenterologists. Patients were registered in an e-CRF by AEG-REDCap up to February 2021. Variables in-cluded: demographics, previous eradication attempts, prescribed treat-ment, adverse events, and outcomes. Modified intention-to-treat (mITT) and per-protocol (PP) analyses were performed and data were subject to quality review to ensure information reliability.Results: In total 41, 562 patients from 31 European countries were evaluat-ed and 29, 634 (70%) first-line empirical H. pylori treatments were included for analysis. Triple therapy with amoxicillin and clarithromycin was most commonly prescribed (39%), followed by non-bismuth concomitant treat-ment (18%) and bismuth quadruple (three-in-one single capsule) (12%), achieving 84%, 90% and 94% mITT eradication rate, respectively. Over 90% effectiveness was obtained only with 10 and 14-day bismuth quadru- ple or with 14-day concomitant treatment (Table). Longer treatment dura-tion, higher acid inhibition and compliance were associated with higher eradication rates. Conclusion: Management of H. pylori infection by European gastroenter-ologists is heterogeneous. Only quadruple therapies lasting at least ten days are able to achieve over 90% eradication rates.

Abstract: Background: After a first eradication attempt, approximately 10-20% of patients will fail to achieve H. pylori eradication. Aims: To evaluate the effectiveness of second-line empirical treatments. Methods: A systematic prospective registry of the clinical practice of European gastroenterologists (31 countries) on H. pylori management was established. All infected adult patients were systematically registered at AEG-REDCap e-CRF until February 2021. Modified intention-to-treat (mITT) and per- protocol (PP) analyses were performed. Results: Overall, 5, 228 patients received a second-line empirical therapy. Overall effectiveness was 84% (both by mITT and PP). Over 97% of patients were compliant. AEs were reported in 28% of the cases. Most frequent second-line prescriptions and effectiveness per antibiotic combination is shown in table 1. After failure of first-line clarithromycin-containing treatment, optimal eradication (>90%) was obtained with moxifloxacin-containing triple therapy, or quadruple therapy with levofloxacin and bismuth. In patients receiving triple regimens containing levofloxacin or moxifloxacin and levofloxacin-bismuth quadruple regimens, cure rates were optimized with 14-day regimens using high doses of proton pump inhibitors (PPIs). However, Pylera® or quadruple therapy with levofloxacin and bismuth achieved consistent eradication rates regardless of the PPI dose, duration of therapy, or previous firstline treatment regimen. Conclusion: Empirical second-line regimens including either 14-day triple therapies with levofloxacin or moxifloxacin, or 14-day levofloxacin-bismuth quadruple or 10-day bismuth quadruple therapies provided optimal effectiveness. However, many other second-line treatments evaluated reported low eradication rates.

Over 90% effectiveness was obtained only with 10 and 14-day bismuth quadruple or 14-day concomitant treatment. Longer treatment duration, higher acid inhibition and compliance were associated with higher eradication rates. Conclusions: Management of H. pylori infection by European gastroenterologists is heterogeneous. Only quadruple therapies lasting at least 10 days are able to achieve over 90% eradication rates.

Abstract: European Registry on the management of Helicobacter pylori infection («Hp-EuReg») - a multicenter prospective observational study initiated by the European Helicobacter and Microbiota Study Group, conducted in 27 European countries in order to evaluate the real clinical practice of diagnosis and treatment of H. pylori and its comparison with international recommendations.The analysis of 2360 patients entered in the register by the Russian centres of «Hp-EuReg» in 2013-2017, who were underwent 1st line eradication therapy.The most common methods of primary diagnosis of H. pylori are histological (37.7%), rapid urease test (29.2%) and serology (29.7%). The duration of eradication therapy in 9.4% of cases was 7 days, in 65.3% - 10 days, and in 25.3% - 14 days. To control the effec- tiveness of treatment, H. pylori antigen in feces (31.3%), urea breath test (23.4%) and histological method (23.3%) were used. In 3.6% cases was used serology by mistake. In 17.3% of patients control was not carried out. The effectiveness of triple therapy with a PPI, amoxicillin, clar- ithromycin (per protocol) was 67.6%, with 7-day course, 81.1% at 10-day and 86.7% at 14-day course. Eradication rate of triple therapy with addition of bismuth (per protocol) reached 90,6% in the group receiving 10-day scheme and 93.6% in the group receiving the 14-day treatment.Significant deviations of clinical practice from expert recommendations, most pronounced at the stage of monitoring the effectiveness of therapy, were noted. The suboptimal efficacy of triple therapy is shown.

Abstract: SummaryBackground Empiric triple therapy for Helicobacter pylori should be abandoned when clarithromycin resistance rate is >15–20%. Optimisation of triple therapy (high-dose acid suppression and 14-day duration) can increase eradication rates by 10%. Aim To compare the efficacy and safety of optimised triple (OPT-TRI) and nonbismuth quadruple concomitant (OPT-CON) therapies. Methods Prospective multicentre study in 16 Spanish centres using triple therapy in clinical practice. In a 3-month two-phase fashion, the first 402 patients received an OPT-TRI therapy [esomeprazole (40 mg b.d.), amoxicillin (1 g b.d) and clarithromycin (500 mg b.d) for 14 days] and the last 375 patients an OPT-CON treatment [OPT-TRI therapy plus metronidazole (500 mg b.d)]. Results Seven-hundred seventy-seven consecutive patients were included (402 OPT-TRI, 375 OPT-CON). The OPT-CON therapy achieved significantly higher eradication rates in the per-protocol [82.3% (95% CI = 78–86%) vs. 93.8% (91–96%), P 90%) compared to OPT-TRI therapy. Addition of metronidazole to OPT-TRI therapy increased eradication rates by 10%, resulting in more mild adverse effects, but without impairing compliance with therapy.

Abstract: Background: Guidelines on the management of Helicobacter pylori , which cover indications for management and treatment strategies, were produced in 2000. Aims: To update the guidelines at the European Helicobacter Study Group (EHSG) Third Maastricht Consensus Conference, with emphasis on the potential of H pylori eradication for the prevention of gastric cancer. Results: Eradication of H pylori infection is recommended in ( a ) patients with gastroduodenal diseases such as peptic ulcer disease and low grade gastric, mucosa associated lymphoid tissue (MALT) lymphoma; ( b ) patients with atrophic gastritis; ( c ) first degree relatives of patients with gastric cancer; ( d ) patients with unexplained iron deficiency anaemia; and ( e ) patients with chronic idiopathic thrombocytopenic purpura. Recurrent abdominal pain in children is not an indication for a “test and treat” strategy if other causes are excluded. Eradication of H pylori infection ( a ) does not cause gastro-oesophageal reflux disease (GORD) or exacerbate GORD, and ( b ) may prevent peptic ulcer in patients who are naive users of non-steroidal anti-inflammatory drugs (NSAIDs). H pylori eradication is less effective than proton pump inhibitor (PPI) treatment in preventing ulcer recurrence in long term NSAID users. In primary care a test and treat strategy using a non-invasive test is recommended in adult patients with persistent dyspepsia under the age of 45. The urea breath test, stool antigen tests, and serological kits with a high accuracy are non-invasive tests which should be used for the diagnosis of H pylori infection. Triple therapy using a PPI with clarithromycin and amoxicillin or metronidazole given twice daily remains the recommended first choice treatment. Bismuth-containing quadruple therapy, if available, is also a first choice treatment option. Rescue treatment should be based on antimicrobial susceptibility. Conclusion: The global burden of gastric cancer is considerable but varies geographically. Eradication of H pylori infection has the potential to reduce the risk of gastric cancer development.

Abstract: Background.Eradication of Helicobacter pylori leads to faster ulcer healing and a significant decrease in ulcer recurrence. Clarithromycin is the most effective monotherapy for eradicating H. pylori from the gastric mucosa, and omeprazole frequently is used for the treatment of duodenal ulcer disease, prompting the interest to investigate rigorously the combination of clarithromycin and omeprazole for eradicating H. pylori. Materials and Methods.The aim of this double-blind, randomized, multicenter (n=30), multinational (n=10) study was to compare clarithromycin and omeprazole with omeprazole monotherapy for the eradication of H. pylori from the gastric mucosa, endoscopic healing, and reduction of symptoms and ulcer recurrence in patients with active duodenal ulcer. Patients with active duodenal ulcer associated with H. pylori infection were randomized to receive omeprazole, 40 mg every morning for 14 days, with either clarithromycin, 500 mg, or placebo three times daily, which was followed by omeprazole, 20 mg every morning for 14 days. Patients underwent endoscopy before enrolling in the study, immediately after finishing treatment, and at 4- to 6-week and 6-month follow-up evaluations or at the recurrence of symptoms. Results.Two hundred and eight patients with active duodenal ulcer associated with confirmed H. pylori infection were randomized to treatment with either clarithromycin and omeprazole (n=102) or omeprazole and placebo (n=106). Four to six weeks after treatment was completed, H. pylori was eradicated in 74% (95% confidence interval, 63.0%–82.4%) of patients receiving clarithromycin and omeprazole, compared with 1% (0.0%–6.2%) of patients receiving omeprazole monotherapy (p < .001). Clarithromycin resistance developed in eight patients treated with clarithromycin and omeprazole and in none given omeprazole and placebo. Abstract: Ulcerative Colitis (UC) is a chronic inflammatory condition that frequently affects men and women of childbearing age. Concerns are often raised by both patients and clinicians regarding the effects of the disease and its treatments on reproduction and lactation. Many studies have examined fertility in UC patients in comparison with the general population. Most of these have not demonstrated a significant difference between the two groups. The observed decrease in fertility in some reports was largely attributable to patient choice, and the use of certain drugs such as sulfasalazine in men. Decreased fertility was also reported in women who have undergone surgery for their disease. Pregnancy is considered high risk in patients with UC. Numerous studies have described the obstetric and neonatal outcomes of these patients, and reported similar pregnancy outcomes in comparison to the general population. However some population-based studies have found an increased incidence of low birth weight and preterm delivery in UC patients. This observed difference seems to be mainly related to disease activity and severity during pregnancy and the appropriate treatment required. On the other hand pregnancy does not appear to increase the risk of disease relapse or severity. The course of UC during pregnancy is partly determined by its activity at conception. Therefore UC patients should be advised to attempt to conceive during periods of disease remission. Patients are often concerned about the safety of UC pharmacotherapy during pregnancy and lactation. Some patients choose to discontinue their maintenance therapy at least until the birth of their baby while others rely on their treating clinician to make the appropriate and safe decision regarding their treatment. Two issues have to be considered in this setting; first is the relative safety of the drug and secondly the benefit risk ratio of its use in pregnancy and lactation. A large amount of safety data is available on drugs that have been in use for decades, however modern medicine has witnessed the introduction of new therapeutic agents that have revolutionized the treatment of inflammatory diseases but there is limited data available regarding their safety during pregnancy and breast feeding. The topic of UC and pregnancy is quite complex involving a range of issues including the effects of both the disease and its treatment options on fertility, pregnancy and lactation, in addition to the effects of pregnancy and lactation on the course of the disease. The aim of this chapter is to review the available data and discuss this topic in more detail.

Abstract: There is no international agreement on scoring systems used to measure disease activity in ulcerative colitis, nor is there a validated definition for disease remission.To review the principles and components for defining remission in ulcerative colitis and propose a definition that will help improve patient outcomes.A review of current standards of remission from the perspective of clinical trials, guidelines, clinical practice and patients was conducted by the authors. Selected literature focused on the components of a definition of remission, the utility of a definition and treatment strategies, based on current definitions.Different definitions of remission affect the assessment of outcome and make it difficult to compare trials. In the clinic, endoscopy is rarely used to confirm remission, because mucosal healing has only recently begun to be related to the duration of subsequent remission in a way that will affect clinical practice. Histopathology may be the ultimate arbiter of mucosal healing. There is no agreement on the definition of remission in current guidelines. Patient-defined remission may predict endoscopic remission, but has yet to be shown to predict duration of remission.A standard based on clinical symptoms and endoscopy is proposed. Histopathology is a third dimension of remission that may have prognostic value. The definition of remission should help predict long-term outcome. The expectations of patients and their physicians need to be raised, as the goal of treatment of active ulcerative colitis should be to induce remission.

Abstract: Details of a personal computer-based data acquisition system, which allows monitoring of large-diameter plasma source ion current profiles and Langmuir probe data, using an interchangeable rotatable, Faraday cup array and Langmuir probe array respectively are described. The arrays span the plasma diameter and can be rotated under computer control. Software-selectable monitor modes allow investigation of the angular and temporal dependence of ion current profiles, basic plasma parameters and their associated non-uniformities. Isolated aluminium plates can be fixed to the respective arrays to investigate the intrusive influence of a substrate holder and substrate bias on plasma parameters. Abstract: Ornithine decarboxylase (ODC) is a modifier of adenomatous polyposis coli-dependent tumourigenesis. The G316A polymorphism in intron 1 of the ODC gene lies between two myc-binding domains and alters the expression of the gene to affect polyamine metabolism. This variant may be associated with recurrence of colorectal adenoma. We examined whether this variant also modified the susceptibility to sporadic colorectal cancer (CRC).The G316A variant was analysed in a large (n = 754) CRC case-controlled series of hospital patient volunteers (n = 627) in the Czech Republic, and the relationship with cancer risk was estimated by conditional logistic regression.After adjusting for age and sex, G316A was associated with no decrease in CRC risk for either heterozygotes [odds ratio 0.98, 95% confidence interval (CI) 0.77-1.23] or rare allele homozygotes (odds ratio 0.92, 95% CI 0.61-1.37).The G316A functional variant in the ODC gene is unlikely to make much impact on reducing CRC risk regardless of the reduction in risk found for the recurrence of colorectal adenoma.

Abstract: Forecasting clinical and economic outcomes in ulcerative colitis (UC) and Crohn's disease (CD) patients is complex, but necessary.To determine: the frequency of treatment-classified clinical states; the probability of transition between states; and the economic outcomes.Newly diagnosed UC and CD patients, allocated into seven clinical states by medical and surgical treatments recorded in serial 3-month cycles, underwent Markov analysis.Over 10 years, 630 UC and 318 CD patients had 22,823 and 11,871 cycles. The most frequent clinical outcomes were medical/surgical remission (medication-free) and mild disease (on 5-aminosalicylates, antibiotics, topical corticosteroids), comprising 28% and 62% of UC cycles and 24% and 51% of CD cycles respectively. The probability of drug-response in patients receiving systemic corticosteroids/immunomodulators was 0.74 in UC, 0.66 in CD. Both diseases had similar likelihood of persistent drug-dependency or drug-refractoriness. Surgery was more probable in CD, 0.20, than UC, 0.08. In terms of economic outcomes, surgery was costlier in UC per cycle, but the outlay over 10 years was greater in CD. Drug-refractory UC and CD cases engendered high costs in the cohort.Most patients on 5-aminosalicylates, corticosteroids and immunomodulators had favourable clinical and economic outcomes over 10 years. Drug-refractory and surgical patients exhibited greater long-term expenses.

Abstract: Sigmoidal and rectal biopsies were taken from 10 patients at colonoscopy. Colonic crypt cell proliferation was measured using a monoclonal antibody to bromodeoxyuridine by an immunohistochemical technique. Labelling index and crypt cell proliferation profiles were analysed and compared between the two sites. No statistically significant difference was found in proliferation parameters. Assessment of colonic proliferation is a useful indicator for colorectal cancer risk and for determining the effects of ongoing drug treatment. Rectal biopsies are easier to take than sigmoidal biopsies. The results of this study suggest that there is no difference in cell proliferation at the two sites. Rectal biopsies are more practical for clinical studies.

Abstract: Abnormalities of the thyroid gland are said to be common in patients with dermatitis herpetiformis (DH), treated with dapsone (Thyroid abnormalities in dermatitis herpetiformis. Cunningham andZone, Annals Int.Med. 1985: 102, 194-196). Thepossibility that the goitrogenic effects of dapsone may play a role in this has not been evaluated. We therefore studied thyroid abnormalities in 40 patients with DH, 25 of whom were taking dapsone and 15 of whom were on a gluten free diet alone. All patients were examined clinically for evidence of thyroid dysfunction and blood was drawn for total thyroxine, thyrotrophin levels and thyroid microsomal antibdy titres. Six (24%) of dapsone treated patients had thyroid abnormalities, compared to 1 (7%) of the 15 patients treated with diet alone. Two patients became thyrotoxic when on treatment with dapsone. One patient had myxoedema prior to diagnosis of DH and one developed myxoedema while on dapsone. Two patients with asymptomatic goitres at the time of screening were taking dapsone and one patient on dapsone had thyroid antibodies. Thus it seems likely that the increased incidence of thyroid abnormalities in DH may be related to the effects of the sulphonamide on thyroid function.
