Abstract

This contribution aims to deepen the formulas for the application of 3D modelling tools in their application to the protection of heritage in the Spanish legislative framework: Although they emerge as instruments for their eminently industrial application, new architecture or infographics, the Rhinoceros software, together with the Vray rendering engine, constitute a tandem that has proved to be very useful for determining the degrees of impact (heights, perspectives, buildability, occupation and other urban parameters) in real estate environments declared to be of "Cultural Interest" (from now on referred to as "BIC" - or Asset of Cultural Interest), according to the regional legislation of Andalusia. In addition to the didactic character of the results, which allows their dissemination as teaching material applied in the Degree in Fundamentals of Architecture of the University of Seville, the three case studies presented facilitate the reading of the environment affected by the specific regulations, as well as the technical determination of the visual pollution that the surrounding buildings cause in the building itself. The three case studies include buildings declared "BIC", which are in very different situations of conservation and use, all in the city of Seville, in the south of Spain: while the City Council, which presides over the most important square in the city, is the paradigmatic model of a dynamic and inhabited square, the Atarazanas (former shipyard factory) are in a good state of conservation, but without defined use. The last case, the extinct convent of San Agustin, in addition to not being used, is in a state of ascending deterioration, since a large part of its construction has disappeared thanks to real estate expansion and the absence of legislative protection of the heritage.

5. The role of pore fluids in supershear earthquake ruptures

Abstract

Abstract The intensity and damage potential of earthquakes are linked to the speed at which rupture propagates along sliding crustal faults. Most earthquakes are sub-Rayleigh, with ruptures that are slower than the surface Rayleigh waves. In supershear earthquakes, ruptures are faster than the shear waves, leading to sharp pressure concentrations and larger intensities compared with the more common sub-Rayleigh ones. Despite significant theoretical and experimental advances over the past two decades, the geological and geomechanical controls on rupture speed transitions remain poorly understood. Here we propose that pore fluids play an important role in explaining earthquake rupture speed: the pore pressure may increase sharply at the compressional front during rupture propagation, promoting shear failure ahead of the rupture front and accelerating its propagation into the supershear range. We characterize the transition from sub-Rayleigh to supershear rupture in fluid-saturated rock, and show that the proposed poroelastic weakening mechanism may be a controlling factor for intersonic earthquake ruptures.

Abstract

It is challenging for the institution to provide students with ideas about courses or programs to pursue. This study aims to propose a tool that employs multiple regression to forecast incoming college students’ courses at Notre Dame of Midsayap College. The proponents developed a prediction model based on the identified predictors and Cumulative Semestral Grade Point Average of all College of Information Technology and Engineering students from the first semester of S.Y. 2013-2014 to S.Y. 2015-2016, using the ex post facto method. The necessary variables were Entrance Exam results, High School Grade Point Average, and Cumulative Semestral Grade Point Average. Also, Pearson’s R correlation was used to determine the relationship between EE and HSGPA to CSGPA. Conclusively, this study supported the notion that EE and HSGPA considerably impact CSGPA. Additionally, the developed predictive model was considered appropriate for course recommendation.

Abstract

La técnica AVO con apilados parciales se aplicó en un campo gasífero para detectar presencia de gas al tope de la Formación Ciénaga de Oro (FCO) usando apilados parciales del programa sísmico terrestre Güepajé-3D y apoyado con registros del pozo Ayombe-1. Mediante la técnica de sustitución de fluidos se simuló el reemplazamiento de gas por agua para conocer la sensibilidad de la respuesta sísmica al cambio del fluido en un segmento del pozo Ayombe-1. Se observó una anomalía clase I al tope de la FCO, aun cuando las respuestas de sustituir gas por agua fueron muy similares al aplicar la técnica AVO con apilados parciales se observaron anomalías AVO clase I al tope de la FCO en los pozos con gas Ayombe-1 y Güepajé-1 y ninguna en el pozo seco Güepajé-3. A pesar de tener gas, en el pozo Güepajé-2 los resultados permiten diferenciar entre una incipiente anomalía clase I y ninguna. Al usarse en el volumen sísmico del Proyecto Güepajé-3D la técnica suministra un mapa de indicador directo de gas que se correlaciona con estructura de la superficie al tope de la FCO. Pueden factores líticos como compactación, cementación y presencia de carbonatos supra yaciendo la FCO, así como la variabilidad en el espesor variable de las capas parcial y totalmente saturadas al tope de la FCO, generar anomalías en zonas con baja probabilidad de tener gas. doi: https://doi.org/10.1016/S0016-7169(13)71475-2

Resumen Las redes sociales surgen como medios de comunicación entre las personas; en su expansión han llegado a abarcar no solamente la interacción social, sino que hemos llegado a utilizarlas e integrarlas en el ámbito médico y científico. Se componen de muchas plataformas que permiten compartir información en tiempo real entre médicos, entre pacientes y entre médicos-pacientes. En consecuencia, el uso de las redes sociales en medicina ha aumentado notablemente, cerca del 90% de los médicos que tienen una cuenta en las redes sociales y hasta el 80% de los pacientes que recurren a Internet para obtener información médica1,2. Dentro de la cirugía general y digestiva, las redes sociales son una herramienta valiosa para el cirujano tanto a nivel académico, como en la práctica clínica. Los cirujanos usamos las redes sociales para difundir intervenciones quirúrgicas, cursos, congresos y artículos académicos, lo que da lugar a enriquecedoras discusiones y debates. Independientemente del entorno de nuestra práctica clínica, las redes sociales facilitan la creación de grupos de trabajo, y lo que es más importante, funcionan como un canal directo para que los médicos compartan información con sus compañeros, colegas y cirujanos en formación a nivel mundial. Actualmente existen diferentes tipos de redes sociales que son accesibles a todos los profesionales sanitarios. Nuestra intención es identificar cuál puede ser más útil para los profesionales de la salud y más concretamente para los cirujanos generales.

Introducción y objetivo: Las tiroidopatias autoimmunes juveniles (TAIJ) son las enfermedades órgano específicas más comunes y con frecuencia hacen parte de la enfermedad autoinmune múltiple (EAIM). El objetivo del estudio es determinar el tipo y frecuencia de TAIJ en pacientes con poliautoinmunidad. Métodos: En un registro de pacientes juveniles que durante el seguimiento en 13 centros de reumatología pediátrica, han asociado enfermedades autoinmunes (EAI), se identificaron pacientes con TAIJ. Revisión de historias clinicas permitió determinar variables demográficas, clínicas, serológicas y relación temporal entre EAI. Resultados: 110/286 (39%) desarrollaron TAIJ durante el seguimiento promedio de 33,8 meses. Edad promedio al inicio de la primera EAI: 10,3 años (1-16). 33% presentaban historia familiar de EAI. Tiroiditis de Hashimoto (TH) fue 18 veces mas frecuente que enfermedad de Graves, su distribución por género fue F17:M1 y F6:M0 respectivamente. TAIJ se asoció a 19 diferentes EAI, siendo las más frecuentes: LES, AIJ y SAF. Se identificaron 26 casos de EAIM, 4 de ellos con endocrinopatía autoinmune múltiple. TH precedió al desarrollo de otras EAI en 21%, coincidió con el debut de otra EAI 33% y se documentó durante el seguimiento de otra(s) EAI en 46%. Conclusiones: La poliautoinmunidad es un fenómeno frecuente en TAIJ. Estas pueden preceder al desarrollo de otras EAI, asociarse al debut o durante el curso de EAI órgano específícas o multisistémicas. Además, con frecuencia hacen parte de EAIM.

20. Social inequality in obesity in an Eastern Mediterranean population: evidence from a national health survey in Cyprus

Abstract

We aimed to explore socioeconomic factors associated with obesity among adults and to investigate social inequality in obesity prevalence in Cyprus.Cross-sectional study.We conducted a survey among 3,021 Greek-Cypriots aged 25-64 years, collecting self-reported demographics, health behaviors, socioeconomic characteristics and anthropometric measurements. We performed univariable and multivariable (adjusting for demographics and health behaviors) sex-specific Poisson's regression with robust variance, reporting adjusted prevalence ratios (PRs) and 95% confidence intervals.The prevalence of obesity was 22% among males and 17% among females. According to univariable analyses, higher obesity prevalence was associated with increased age, decreased physical activity and decreased alcohol consumption in both genders. In addition, obesity was associated with refugee status and former smoking in males and with a higher healthy diet score in females. There was a clear linear decrease in obesity prevalence each step up the socioeconomic hierarchy in both genders. In the fully adjusted model, a clear inverse gradient in obesity prevalence by educational attainment was observed in females (p=0.002), while, in males, lower obesity prevalence remained significantly associated with the highest level of family-net income and educational attainment (aPR:0.48; 95% CI:0.27-0.84 and aPR:0.46; 95% CI:0.25-0.84, respectively). Occupational social class was not associated with obesity.This study highlights striking social inequalities in obesity in an Eastern Mediterranean population, which only recently moved from rural living to high levels of development. We recommend that public health interventions should address education - and income-related barriers, as a means of tackling health inequalities.

BackgroundThe COVID-19 pandemic and the widespread adoption of virus control measures have inevitably disrupted efforts to address lifestyle risk factors for non-communicable diseases (NCD). This study aimed to explore the effects of COVID-19 lockdown on all lifestyle medicine pillars, namely diet, physical activity, sleep, stress, social support and use of risky substances.MethodsThis was a cross-sectional study on a convenient sample of adults who resided in Cyprus during the Spring 2020 lockdown. Participants completed an anonymous online questionnaire comprised of six validated tools regarding the following lifestyle behaviours before and during lockdown: adherence to the Mediterranean diet, physical activity, stress and social support levels, sleep pattern and use of risky substances such as smoking and alcohol. Paired before and during lockdown comparisons for each lifestyle pillar were undertaken using Wilcoxon Signed-Rank test and Bowker symmetry Test where response was numerical (non-parametric data) and categorical respectively. Furthermore, stratified analyses for sociodemographic characteristics were performed.ResultsOut of 745 participants, 74% were female and median age was 39 years. Overall participants reported significantly higher perceived stress score (22 v 25,p p p p p trend = 0.03 and ConclusionVarious lifestyle factors were adversely affected by the COVID-19 lockdown in Cyprus. Evidence from this study supports development of holistic lifestyle interventions during and following the pandemic to reduce short and long-term NCD risks by building on lifestyle behaviour strengths and addressing longstanding and emerging gaps and needs.

Abstract

During the first quarter of 2020 the world is experiencing a pandemic of Severe Acute Respiratory Syndrome‑Coronavirus‑2 (SARS‑CoV‑2), a novel beta coronavirus that is responsible for the 2019 novel coronavirus disease (COVID‐19). The COVID‐19 pandemic revealed that healthcare systems around the world were not prepared to deal with either the direct effects of the pandemic or with the indirect effects that are imposed on the health of patients with chronic disorders such as cancer patients. Some challenges and dilemmas currently faced during the pandemic include the management of cancer patients during the treatment and follow‐up phases, the assessment of the safety of treatments currently used for the management of SARS‑CoV‑2 for use in cancer patients, the development of psychoeducation and emotional support for cancer patients and the safe conduct of clinical trials involving participation of cancer patients. Evidence from the literature supports the need for the urgent development of a holistic contingency plan which will include clear guidelines for the protection and comprehensive care of cancer patients. The implementation of such a plan is expected to have many beneficial effects by mainly minimizing the increased morbidity and mortality of cancer patients that could result as an adverse consequence of the COVID‐19 or future pandemics., The COVID‐19 pandemic has revealed that health care systems around the world were not prepared to deal with either the direct effects of the pandemic or with the indirect effects that are imposed on the health of cancer patients. The evidence from the literature supports that the development of a contingency plan is as urgent as ever to minimise the increased morbidity and mortality of cancer patients that could result as a consequence of the current COVID‐19 pandemic or any other future pandemics.

Abstract

There are significant levels of concern about both the safety assessment and financial evaluation of the whole hydropower system, especially at early project stages. In addition, there is a variety of reliable and accurate methods for analysis, design, and optimization of steel pressure liners in hydropower plants. Several countries have developed specific regulations and codes for the design, installation, and safety evaluation of under-pressure piping, as well as estimates of the potential risks associated with failure. This paper reviews the current methodologies and codes available for design and safety assessment of either unstiffened or stiffened pressure steel liners in hydropower plants. After examining the main guidelines and practical conclusions drawn from recent research in the field of liners subjected to either internal or external pressure, this article then discusses the relevant regulations and codes. The scope of this work is to summarize the advantages, disadvantages, and main characteristics of the existing design criteria, analysis methods, and other issues related to steel liners. Afterwards, this paper compares the main reliable formulations and modelling techniques, even the most recent or sophisticated ones. Lastly, we draw some conclusions regarding their accuracy and scope of applicability.

BACKGROUND Juvenile Idiopathic Arthritis (JIA) is the most common inflammatory joint disease in children. OBJECTIVE To Investigate association between gastrointestinal, rheumatologic clinical variables and the presence of autoantibodies in patients with JIA in treatment. Abstract

Background: African American (AA) women and patients with Triple Negative Breast Cancer (TNBC) have poor outcomes. It is unclear whether these worse outcomes in AA women are due to a higher frequency of TNBC in AA women. The purpose of our study was to determine difference in outcomes between AA and Caucasian (CA) female with TNBC. Methods: A retrospective study of women diagnosed with ER, PR and HER2 (1+) negative breast cancer between January 1/2001 and December 31/2013 at Vidant Medical Center (East Carolina University) in Greenville, North Carolina was undertaken. Patient demographics and tumor characteristics were analyzed to determine impact on overall survival. Results: In our institution, 4,434 women were diagnosed with breast cancer in this study period, of which 378 had TNBC. Our TNBC population was predominantly AA (53%). AA patients were younger (54.1 years vs 58.7 years, p=0.005), less often postmenopausal (62.6% vs 74.5%, p=0.013), more likely to have Medicaid (24.3% vs 7.9%, p Conclusions: Poorer outcomes in AA women cannot be attributed to the higher frequency of TNBC. Abstract

Background In Directly Observed Treatment (DOT) monitoring strategy for the treatment of tuberculosis (TB), the patient has to travel daily to the health institution to take the TB medication. Although has been usefulness, this strategy increases the catastrophic costs of the disease, rising the probability of rejection, abandonment, and failure to treatment. Therefore, a monitoring strategy was implemented through video calls phone known as Synchronous Video Observed Therapy (S-VOT), to document the experience and its results in a series of patients from a low-middle income country. Methods A prospective case series study was conducted involving 23 TB patients managed with standard treatment, who were supervised through daily video call phone, during 2019, Cali-Colombia. Adherence to VOT strategy and treatment were evaluated, as well as patient characteristics, adverse drug effects, perception and costs. Categorical variable data were summarized in absolute and relative frequency tables. Quantitative variables were described with median and Interquartile range. Proportional differences were calculated with Z test, risk opportunities and difference of medians with Mann Whitney’s U test were performed. We take p values < 0.05 as significant. Results Adherence to S-VOT monitoring strategy was 99.8% in first phase of treatment and 98.2% in second phase. All patients completed treatment, achieving 100% cure criteria in patients with pulmonary TB. Most adverse drug effects were reported on first month (31.6%, p=0.003). Skin changes were the most frequent complaint at the beginning of management (16.4%) and epigastralgia at the end (20%). The likelyhood of adverse effects was significantly reduced when patients started maintenance phase on third month (OR: 0.29, p = 0.0003). The experience of the strategy was generally positive for patients. Time savings was major advantage. Travel costs in S-VOT were lower than DOT for patients, as well as the daily time investment in TB treatment. Conclusion S-VOT strategy was well tolerated and accepted by all patients, allowing an excellent level of adherence, with reduction in travel costs and investment of time for treatment. Abstract

This paper presents an alternative backhaul link using relay buoy to provide low-cost, sustainable, and broadband connectivity for community cellular networks (CCN) residing in coastal areas. The proposed solution attempts to address the high operational cost of CCN due to monthly subscription cost of VSAT which is used to connect the network to the global internet. Due to geographic conditions of these coastal communities, terrestrial solutions such as land-based wireless repeaters and fiber cables may not be suitable for small-scale networks such as CCN. Relay buoy on the other hand which has line-of-sight (LOS) paths to these coastal communities and possibly to an internet service provider (ISP), can be placed in the coastal waters and serves as backhaul link. It uses low-cost, off-the-shelf, and high bandwidth 2.4GHz Wi-Fi radios and highly directive antennas. However, establishing a communication system in maritime environment has its own challenges which must be considered in the design of the relay buoy system. Furthermore, radio planning using radio propagation simulation tool and high-resolution digital elevation maps were used to determine the optimum location of the relay buoy. Given the derived location, 3-ray path loss analysis was applied to evaluate the impact of variation in evaporation duct height, and antenna height. The feasibility of such system was successfully demonstrated and evaluated through various wireless tests conducted. The proposed solution could enable not only the backhaul of CCN but also other applications such as weather sensor gateway, emergency warning system, and wireless access for different fishing activities.

Abstract

In the Philippines, several projects had been implemented which gives people living in remote areas access to the internet. Some of these projects face geographic problems like remote coastal areas separated by mountains, which leads to the use of expensive Very Small Aperture Terminal. One possible solution is using an ocean long-range wireless communication link buoy as a relay station. However, this faces challenges such as high fading due to the reflection of sea surface and the effects of ocean activities. To address this, a mechanical antenna stabilizer for an omnidirectional antenna is used to compensates the angular movement of the buoy.

Abstract

In this paper, the dependence of the crystallographic direction in the critical concentration of IV-type atoms for the zincblende–diamond (order–disorder) transition in (III–V)–IV semiconductor alloys were found for the (001), (110), and (111) growth directions by using Monte Carlo simulations. Intervals for the concentration values were determined by simulation, generating a band that is consistent with experimental data. Selecting the average value of this band produces good agreement with previous experimental results. It was also found that the relationship between order parameter and concentration does not have unique values and that there exists a band of events. The existence of this band can explain the scatter in the experimental data reported in x-ray, Raman, and optical anisotropy. It was found that the site percolation threshold is independent of the crystallographic direction of growth and converges to a single concentration value x ≈ 0.437.

Abstract

This paper presents the development and national deployment of hydro-meteorological monitoring devices, termed as HydroMet Stations, for flood warning and monitoring applications. We describe various challenges encountered during the course of implementation particularly those related to transportation, local logistics, location, and security. We also discussed lessons learned such as the early involvement of stakeholders, the modification of the design to further protect the sensors from observed issues that came up during implementation, as well as those related to local community engagements. Information, Education and Communication (IEC) Campaign was also discussed as a mode of information dissemination to communities for the currently deployed system consisting of 1,834 HydroMet stations. Likewise, various instances and use cases were enumerated stemming from end-users that have utilized the data for use in public service, research, and academic work. This includes government agencies, research institutes, academia, and other government-funded projects. Finally, a set of recommendations and next steps relevant to operation and further system improvements were discussed.

41. AVO analysis with partial stacking to detect gas anomalies in the GÜEPAJÉ-3D project

Abstract

Resumen La tecnica AVO con apilados parciales se aplico en un campo gasifero para detectar presencia de gas al tope de la Formacion Cienaga de Oro (FCO) usando apilados parciales del programa sismico terrestre Guepaje-3D y apoyado con registros del pozo Ayombe-1. Mediante la tecnica de sustitucion de fluidos se simulo el reemplazamiento de gas por agua para conocer la sensibilidad de la respuesta sismica al cambio del fluido en un segmento del pozo Ayombe-1. Se observo una anomalia clase I al tope de la FCO, aun cuando las respuestas de sustituir gas por agua fueron muy similares al aplicar la tecnica AVO con apilados parciales se observaron anomalias AVO clase I al tope de la FCO en los pozos con gas Ayombe-1 y Guepaje-1 y ninguna en el pozo seco Guepaje-3. A pesar de tener gas, en el pozo Guepaje-2 los resultados permiten diferenciar entre una incipiente anomalia clase I y ninguna. Al usarse en el volumen sismico del Proyecto Guepaje-3D la tecnica suministra un mapa de indicador directo de gas que se correlaciona con estructura de la superficie al tope de la FCO. Abstract

As is well known, the metallurgical microstructure of carbon steel is formed by ferrite and pearlite after the annealing heat treatment. When the cooling rate increases, the diffusive process is interrupted causing a change in the metallurgical microstructure which will affect steel properties. The aim of this work was to study thermal, structural, and microstructural properties of annealed carbon steel samples with four different carbon contents. Crystalline structure and crystalline quality were studied by the X-ray diffraction technique, where the full width at half maximum analysis showed that as the carbon content increased, the crystalline quality decreased. The metallurgical microstructure morphology was studied by scanning electron microscopy. The thermal diffusivity and the heat capacity were determined by the photoacoustic technique and by the thermal relaxation method, respectively. The thermal diffusivity and the thermal conductivity decreased as the carbon content increased. The amplitude signal of photothermal radiometry increased as the carbon content increased, while the phase signal of photothermal radiometry did not show significant differences among studied carbon steel types. The photoacoustic technique represents an important alternative in the steel characterization field.

Resumen: Introducción: Los defectos oculares congénitos (DOC) pueden originar importante discapacidad. Objetivo: El objetivo de este estudio fue conocer la prevalencia total de los DOC en Asturias, su tendencia y realizar una descripción de su forma de presentación. Metodología: Análisis de los datos del Registro de Defectos Congénitos de Asturias (RDCA) de los años 1990–2004. La población estudiada fueron los 103.452 nacidos de madres residentes en Asturias en este periodo. Se calcularon las tasas de prevalencia total. Resultados: De los 3.035 casos con defectos congénitos registrados durante los 15 años estudiados, 70 tenían un DOC. La prevalencia total media fue de 6,8 por 10.000 nacidos, con una tendencia estable. Los más frecuentes fueron: las cataratas congénitas (2,0 por 10.000 nacidos vivos), la anoftalmia/microftalmia (1,4 por 10.000 nacidos vivos) y los colobomas (1,3 por 10.000 nacidos vivos). El 40 % de los DOC se presentaron de forma aislada, 37% pertenecían a un síndrome y 23% se asociaban a otras anomalías congénitas no sindrómicas. Conclusiones: La prevalencia total de los DOC durante este periodo en Asturias fue estable siendo las cataratas congénitas el DOC más frecuente. Más de la mitad de los DOC, en especial la anoftalmia/microftalmia se asociaron a otras malformaciones congénitas. Objective: The purpose of this investigation was to assess the prevalence of COAs, their trends and to describe the associated malformations and syndromes in a geographically defined population. Method: Data from the Asturias Registry of Congenital Defects were used. The period studied was from 1990 to 2004 and the study population the 103,452 births of mothers living in the region. Total prevalence was calculated. Results: A total of 3035 cases with congenital defects were recorded, of these 70 had COAs. The total prevalence was 6.8 per 10000 births, with a stable trend during this period. The most common COAs were: congenital cataract (2.0 per 10000 births), anophthalmos/microphthalmos (1.4 per 10000 births) and coloboma (1.3 per 10000 births). 40% of COAs occurred as isolated defects, 37% were syndromes and 23% were associated with other congenital defects. Conclusions: The prevalence of COAs in Asturias over this period had a stable trend and the congenital cataract was the commonest COAs. COAs, particularly the anophthalmos/microphthalmos were associated with other congenital anomalies.

44. UVEÍTIS CRÓNICA EN LA ARTRITIS IDIOPÁTICA JUVENIL. IMPACTO PRONÓSTICO

Abstract

Introducción y objetivo: La uveitis en la artritis idiopática juvenil (AIJ) es causa de importante morbilidad. La cronicidad de inflamación intraocular determina severidad de complicaciones y secuelas. El objetivo del estudio es evaluar la extensión, tratamiento y complicaciones de la uveítis crónica en pacientes con AIJ. Métodos: Estudio descriptivo, retrospectivo. Se incluyeron pacientes con seguimiento mínimo de 24 meses por oftalmología y reumatología. Se analizaron variables dermográficas, clínicas, complicaciones y tratamiento. La uveitis se evaluó según clasificación SUN (standardization of uveítis nomenclature). Resultados: El 37% de 51 pacientes con uveítis-AIJ desarrollaron curso crónico. Tiempo de seguimiento promedio 102 meses. Distribución género F2.2:M1. Edad promedio 7.1 años al primer episodio de uveítis. Uveítis precedió a artritis 32%. 74% asociada a AIJ oligoarticular y con igual frecuencia asociada a ARE, psoriasis y AIJ poliarticular seronegativa. Uveítis anterior crónica 47%, 37% panuveítis, anterior e intermedia 11% e intermedia 5%. Complicaciones: sinequias 63%, queratopatia en banda 16%, cataratas 63%, 42% glaucoma, 16% edema macular cistoide, pthisis bulbi 16% y ceguera 21%. El 100% recibió tratamiento corticoide tópico y midriático, ciclo(s) de corticoide sistémico 95% y 10% corticoide intraocular. 100% MTX, 26% AZA y ciclosporina 5%. 58% recibieron biológicos y 18% requirieron mas de un biológico. 63% recibieron manejo quirúrgico: faquectomia 75%, vitrectomia 33%, iridectomía 17% u otros procedimientos quirúrgicos o fotocoagulación. Conclusiones: La uveítis crónica en pacientes con AIJ es una severa complicación. Demanda un estricto seguimiento oftalmologíco con medidas clinimétricas que evalúen respuesta al tratamiento. El manejo farmacológico oportuno, dinámico e individualizado busca obtener un efectivo y sostenido control de la inflamación intra ocular y reducir el número y severidad de complicaciones y secuelas.

Abstract

Introduccion: El síndrome antifosfolipídico (SAF) condiciona hipercoagulabilidad y puede provocar manifestaciones no trombóticas. Se asocia o no a otra enfermedad autoinmune (EAI). En el presente estudio se exploraron diferencias entre géneros en cohortes de pacientes juveniles con SAF. Métodos: Estudio retrospectivo descriptivo. Se revisaron historias clínicas de pacientes con SAF seguidos en 13 centros de reumatología pediátrica en Colombia y en Hospital San Joan (Barcelona, España). Se determinaron variables demográficas y frecuencia de manifestaciones clínicas y resultados serológicos. Resultados: Se incluyeron 131 pacientes. Distribución por género F4.5: M1. Edad media F12,2/M 13,1 años. Seguimiento medio: 32.2 meses. De los pacientes masculinos, 46% asoció EAI (LES 20/24). Un paciente asoció SAF+mutación MTHR. 71% desarrollaron trombosis (venosa 53% y arterial 47%). Trombosis recurrente 21%. Manifestaciones no trombóticas: trombocitopenia 38%, anemia hemolítica 21%. Con 8% de frecuencia: síndrome de Evans, convulsiones y migraña. Se detectaron: anticoagulante lúpico 83 %, anticardiolipina IgG 67% e IgM 58 % en diferentes combinaciones. En pacientes femeninas 66% asoció otra EAI (LES 69/107). 36% desarrollaron trombosis (venosa 61% y arterial 39%). Trombosis recurrente 9%. Manifestaciones no trombóticas: Trombocitopenia 43%, anemia hemolítica 33%, Sd. de Evans 14%, migraña 16%, 7% corea y convulsiones 7%. Los principales hallazgos serológicos fueron: anticoagulante lúpico (68%), anticardiolipina IgG (75 %) e IgM (80 %), en diferentes combinaciones. Abstract

Objective: Assessment of HIV prevalence and associated risk behaviours among female commercial sex workers (FCSW) across major cities in South America. Methods: Seroepidemiological, cross sectional studies of 13 600 FCSW were conducted in nine countries of South America during the years 1999-2002. Participants were recruited in brothels, massage parlours, hotels, and streets where anonymous questionnaires and blood samples were collected. HIV infection was determined by enzyme linked immunosorbent assay (ELISA) screening and western blot confirmatory tests. Results: The overall HIV seroprevalence was 1.2% (range 0.0%-4.5%). The highest HIV seroprevalences were reported in Argentina (4.5%) and Paraguay (2.6%); no HIV infected FCSW were detected in Venezuela and Chile. Consistent predictors of HIV seropositivity were: (1) a previous history of sexually transmitted infections (STI, AORs = 3.8-8.3), and (2) 10 years or more in commercial sex work (AORs = 2.2-24.8). In addition, multiple (≥3) sexual contacts (AOR = 5.0), sex with foreigners (AOR = 6.9), use of illegal drugs (AOR = 3.2), and marijuana use (AOR = 8.2) were associated with HIV seropositivity in Southern Cone countries. Conclusions: Consistently low HIV seroprevalences were detected among FCSW in South America, particularly in the Andean region. Predictors of HIV infection across the continent were STI and length of commercial sex work; however, use of illegal drugs, especially marijuana, and sexual contacts with foreigners were also found to be associated risk factors in the Southern Cone region. Interventions for the control of HIV and other STI need to be region and country specific; drug use appears to have an ever increasing role in the spread of HIV among heterosexually active populations. Abstract

Objectives To assess at a population-based level the frequency with which severe structural congenital malformations are detected prenatally in Europe and the gestational age at detection, and to describe regional variation in these indicators. Methods In the period 1995–1999, data were obtained from 17 European population-based registries of congenital malformations (EUROCAT). Included were all live births, fetal deaths and terminations of pregnancy diagnosed with one or more of the following malformations: anencephalus, encephalocele, spina bifida, hydrocephalus, transposition of great arteries, hypoplastic left heart, limb reduction defect, bilateral renal agenesis, diaphragmatic hernia, omphalocele and gastroschisis. Results The 17 registries reported 4366 cases diagnosed with the 11 severe structural malformations and of these 2300 were live births (53%), 181 were fetal deaths (4%) and 1863 were terminations of pregnancy (43%); in 22 cases pregnancy outcome was unknown. The overall prenatal detection rate was 64% (range, 25–88% across regions). The proportion of terminations of pregnancy varied between regions from 15% to 59% of all cases. Gestational age at discovery for prenatally diagnosed cases was less than 24 weeks for 68% (range, 36–88%) of cases. There was a significant relationship between high prenatal detection rate and early diagnosis (P < 0.0001). For individual malformations, the prenatal detection rate was highest for anencephalus (469/498, 94%) and lowest for transposition of the great arteries (89/324, 27%). Termination of pregnancy was performed in more than half of the prenatally diagnosed cases, except for those with transposition of the great arteries, diaphragmatic hernia and gastroschisis, in which 30–40% of the pregnancies with a prenatal diagnosis were terminated. Conclusion European countries currently vary widely in the provision and uptake of prenatal screening and its quality, as well as the ‘culture’ in terms of decision to continue the pregnancy. This inevitably contributes to variation between countries in perinatal and infant mortality and in childhood prevalence and cost to health services of congenital anomalies. Copyright © 2004 ISUOG. Abstract

Juvenile idiopathic arthritis is the most common cause of chronic arthritis in children. JIA is a heterogeneous disease that has been classified by the International League of Associations for Rheumatology (ILAR) on seven different subtypes according to the most relevant clinical and serological features. Previous studies have demonstrated an association between the presence of antinuclear antibodies (ANA) and features like: early-onset oligoarticular disease, female predominance, asymmetric arthritis and higher frequency of uveitis. There are limited data on the characteristics clinical and analytical into different subtypes of JIA in Colombian children and their relationship with type of onset and clinical course.

Porphyromonas gingivalis (Pg) is considered to be one of the most important oral cavity infectious agents in adults patients. Pg is uncommon or found in low numbers in healthy children and don't know information in Juvenile Idiophatic Arthritis (J)IA patients.
