1. Cost of care pathways before and after appropriate and inappropriate transfers to the emergency department among nursing home residents: results from the FINE study

Abstract

Abstract Background Transfers of nursing home (NH) residents to the emergency department (ED) is frequent. Our main objective was to assess the cost of care pathways 6 months before and after the transfer to the emergency department among NH residents, according to the type of transfer (i.e. appropriate or inappropriate). Methods This was a part of an observational, multicenter, case-control study: the Factors associated with INappropriate transfer to the Emergency department among nursing home residents (FINE) study. Sixteen public hospitals of the former Midi-Pyrénées region participated in recruitment, in 2016. During the inclusion period, all NH residents arriving at the ED were included. A pluri-disciplinary team categorized each transfer to the ED into 2 groups: appropriate or inappropriate. Direct medical and nonmedical costs were assessed from the French Health Insurance (FHI) perspective. Healthcare resources were retrospectively gathered from the FHI database and valued using the tariffs reimbursed by the FHI. Costs were recorded over a 6-month period before and after transfer to the ED. Other variables were used for analysis: sex, age, Charlson score, season, death and presence inside the NH of a coordinating physician or a geriatric nursing assistant. Results Among the 1037 patients initially included in the FINE study, 616 who were listed in the FHI database were included in this economic study. Among them, 132 (21.4%) had an inappropriate transfer to the ED. In the 6 months before ED transfer, total direct costs on average amounted to 8,145€ vs. 6,493€ in the inappropriate and appropriate transfer groups, respectively. In the 6 months after ED transfer, they amounted on average to 9,050€ vs. 12,094€. Conclusions Total costs on average are higher after transfer to the ED, but there is no significant increase in healthcare expenditure with inappropriate ED transfer. Support for NH staff and better pathways of care could be necessary to reduce healthcare expenditures in NH residents. Trial registration clinicaltrials.gov, NCT02677272.

Abstract

Abstract There is a growing need for collaborative and interdisciplinary research in addressing global ecological challenges, and early career researchers (ECRs) often play a vital role in such ventures. But despite the desire for such approaches, forming new and interdisciplinary collaborations is risky, and disproportionately so for ECRs, whose perspectives on this topic are rarely heard. Here, we present common perceptions among ECRs regarding opportunities for intra‐ and interdisciplinary collaboration, and barriers preventing such collaboration from taking place. We also discuss possible solutions, and the ecological outcomes of fostering more collaboration. The perceptions discussed have been distilled from a two‐day workshop in New Zealand, aiming to investigate the potential for collaboration between 34 ECRs in distinct ecological disciplines across ten research institutes. Commonality in methodology or research aims was vital for potential collaborations to be considered worthwhile, but differences in spatial or temporal scales were a key disconnect that hindered numerous potential crossovers. Individual connectivity and institutional structures were commonly perceived as barriers to acting collaboratively in general. Specifically, barriers included having a small peer network, lack of access to funding, and concerns over the risk/reward ratio of forming new collaborations. Overcoming barriers will require active, practical support from institutions, funding bodies and mentors, and participants commonly called for specific funding support and the creation of ECR‐focused spaces to better foster collaborative behavior. Fostering interdisciplinary ECR collaborations in ecology was perceived to be useful in creating larger and more useful datasets and tools, and more scalable and transferable models and outcomes. Adopting practices that facilitate more ECR‐led interdisciplinary collaboration will help generate a more integrative understanding of ecological systems globally.

Abstract

ABSTRACT Paramyxoviruses, specifically, the childhood pathogen human parainfluenza virus type 3, are internalized into host cells following fusion between the viral and target cell membranes. The receptor binding protein, hemagglutinin (HA)-neuraminidase (HN), and the fusion protein (F) facilitate viral fusion and entry into the cell through a coordinated process involving HN activation by receptor binding, which triggers conformational changes in the F protein to activate it to reach its fusion-competent state. Interfering with this process through premature activation of the F protein has been shown to be an effective antiviral strategy in vitro. Conformational changes in the F protein leading to adoption of the postfusion form of the protein—prior to receptor engagement of HN at the host cell membrane—render the virus noninfectious. We previously identified a small compound (CSC11) that implements this antiviral strategy through an interaction with HN, causing HN to activate F in an untimely process. To assess the functionality of such compounds, it is necessary to verify that the postfusion state of F has been achieved. As demonstrated by Melero and colleagues, soluble forms of the recombinant postfusion pneumovirus F proteins and of their six helix bundle (6HB) motifs can be used to generate postfusion-specific antibodies. We produced novel anti-HPIV3 F conformation-specific antibodies that can be used to assess the functionality of compounds designed to induce F activation. In this study, using systematic chemical modifications of CSC11, we synthesized a more potent derivative of this compound, CM9. Much like CSC11, CM9 causes premature triggering of the F protein through an interaction with HN prior to receptor engagement, thereby preventing fusion and subsequent infection. In addition to validating the potency of CM9 using plaque reduction, fusion inhibition, and binding avidity assays, we confirmed the transition to a postfusion conformation of F in the presence of CM9 using our novel anti-HPIV3 conformation-specific antibodies. We present both CM9 and these newly characterized postfusion antibodies as novel tools to explore and develop antiviral approaches. In turn, these advances in both our molecular toolset and our understanding of HN-F interaction will support development of more-effective antivirals. Combining the findings described here with our recently described physiologically relevant ex vivo system, we have the potential to inform the development of therapeutics to block viral infection. Abstract

The zinc oxide thin films, highly transparent, doped aluminium were prepared on glass substrates by the reactive chemical spray method. The incorporation nature of Al atoms in the ZnO lattice was determined by X-ray diffraction and optical analyses. Indeed, for low doping ⩽2%, the results of X-ray spectra analysis show a simultaneous reduction of lattice parameters (a and c), this variation, which follows VEGARD’s law, tends to indicate a substitution of Zn by Al. By against for doping >2% the increase in the lattice parameters thus the grain sizes, in accordance with the VEGARD’s law can be explained by occupation of the interstitial sites by Al atoms. Beyond 4%, the material tends to get disorderly and the crystallites orientation is random. 5. Cell Loss during Pseudoislet Formation Hampers Profound Improvements in Islet Lentiviral Transduction Efficacy for Transplantation Purposes

Abstract

Islet transplantation is a promising treatment in type 1 diabetes, but the need for chronic immunosuppression is a major hurdle to broad applicability. Ex vivo introduction of agents by lentiviral vectors—improving β-cell resistance against immune attack—is an attractive path to pursue. The aim of this study was to investigate whether dissociation of islets to single cells prior to viral infection and reaggregation before transplantation would improve viral transduction efficacy without cytotoxicity. This procedure improved transduction efficacy with a LV-pWPT-CMV-EGFP construct from 11.2 ± 4.1% at MOI 50 in whole islets to 80.0 ± 2.8% at MOI 5. Viability (as measured by Hoechst/PI) and functionality (as measured by glucose challenge) remained high. After transplantation, the transfected pseudoislet aggregates remained EGFP positive for more than 90 days and the expression of EGFP colocalized primarily with the insulin-positive β-cells. No increased vulnerability to immune attack was observed in vitro or in vivo. These data demonstrate that dispersion of islets prior to lentiviral transfection and reaggregation prior to transplantation is a highly efficient way to introduce genes of interest into islets for transplantation purposes in vitro and in vivo, but the amount of β-cells needed for normalization of glycemia was more than eightfold higher when using dispersed cell aggregates versus unmanipulated islets. The high price to pay to reach stable and strong transgene expression in islet cells is certainly an important cell loss.

Abstract

Antimalarial drug resistance has historically arisen through convergent de novo mutations in Plasmodium falciparum parasite populations in Southeast Asia and South America. For the past decade in Southeast Asia, artemisinins, the core component of first-line antimalarial therapies, have experienced delayed parasite clearance associated with several pfk13 mutations, primarily C580Y. We report that mutant pfk13 has emerged independently in Guyana, with genome analysis indicating an evolutionary origin distinct from Southeast Asia. Pfk13 C580Y parasites were observed in 1.6% (14/854) of samples collected in Guyana in 2016–2017. Introducing pfk13 C580Y or R539T mutations by gene editing into local parasites conferred high levels of in vitro artemisinin resistance. In vitro growth competition assays revealed a fitness cost associated with these pfk13 variants, potentially explaining why these resistance alleles have not increased in frequency more quickly in South America. These data place local malaria control efforts at risk in the Guiana Shield.

Abstract

Orale semaglutide is een nieuwe ‘glucagon-like’ peptide-1-receptoragonist (GLP1-RA) ontwikkeld specifiek voor peroraal gebruik. Orale semaglutide is gecoformuleerd met de absorptiebevorderaar SNAC (sodium N-(8-[2-hydroxybenzoyl]amino)caprylaat), die kortdurend de pH ter hoogte van het maagepitheel verhoogt en hierdoor de enzymatische degradatie verhindert en de opname versterkt. In het klinische onderzoeksprogramma PIONEER werd de werkzaamheid van orale semaglutide bewezen in vergelijking met een placebo en andere antidiabetische therapieën, dit op het vlak van zowel glykemiecontrole als gewichtsreductie. Er is bovendien een bewezen cardiovasculaire veiligheid (non-inferioriteit ten opzichte van een placebo). Het tolerantieprofiel van orale semaglutide is vergelijkbaar met dit van de subcutane vormen van GLP1-RA. In tegenstelling tot de subcutane GLP1-RA zijn de cardiovasculaire voordelen van orale semaglutide op heden nog niet aangetoond. De SOUL-studie, die momenteel lopende is, onderzoekt of de cardiovasculaire outcome van orale semaglutide vergelijkbaar is met die van de subcutane vorm. Resume C’est bien connu, le style de leadership a un impact significatif sur la satisfaction au travail. Cependant, peu d’etudes portent sur le leadership policier (officier) et la satisfaction au travail. La recherche vise a demontrer la relation du style de leadership (MLQ) des officiers sur la satisfaction au travail (MSQ). Les resultats ont ete compares entre les deux groupes composes de 252 employes : policiers et cols bleus (organisation publique). Les resultats obtenus pour le leadership Transactionnel et Laissez-faire sont plus eleves que la norme MLQ pour les officiers en comparaison au style Transformationnel qui est inferieur. Le style Transformationnel a un impact plus significatif que les deux autres styles sur la satisfaction au travail des policiers.

Abstract

Squamous cell carcinoma (SCC) of the breast is a rare entity of breast cancer, with a very poor prognosis, and whose pathophysiology is still unwell established. Therapeutic management is very heterogeneous due to its incomplete understanding. Nevertheless, it seems that two histological entities can be distinguished: pure SCC close to the cutaneous origin, and metaplastic squamous breast cancer (MSBC). The aim of this study is therefore to assess the difference in survival according to the histological type (SCC or MSBC) and to describe the demographic, clinical, and therapeutic characteristics of the two underlying populations. Our data came from a monocentric retrospective series of 39 patients treated between 1985 and 2018 at the Gustave Roussy Institute (France) for a breast SCC. Of the 39 patients included, 64% had MSBC and 36% had a pure form. The overall and recurrence-free survival at 3 years [CI 95%] was 72.3% [56.9%; 87.0%] and 67.2% [51.2%; 83.2%], respectively. The overall 3-year survival of patients with MSBC was significantly lower than that with pure SCC: HR [CI 95%] 9.5 [1.2; 73.1], p = 0.008. The 3-year recurrence-free survival of patients with MSBC was also poorer: HR [CI 95%] 11.9 [1.6; 90.7], p = 0.002. Patients with MSBC also tended to be younger, have a large lesion size, and be more metastatic. The histological nature of SCC seems to bring fundamental new elements to the therapeutic management as it impacts recurrence and survival. It should therefore be better characterized at diagnosis in order to possibly adapt treatments.

Abstract

International audience; The PRECy project foresees the use of a 16–25 MeV energy proton beam produced by the TR24 cyclotron, named CYRCé, recently installed at the Institut Pluridisciplinaire Hubert Curien (IPHC) in Strasbourg for research in radiation biology. One of the exit ports of the cyclotron will be used for this application along with a combination magnet. The platform will consist of up to 5 experimental stations linked to beamlines located in a dedicated area next to the cyclotron vault. One of the beamlines will receive proton beams of a few cm diameter at intensities up to 100 nA. In order to characterize the beam extracted from the cyclotron, the transverse beam emittance was studied by means of different methods.

Abstract

Background The role of chemotherapy in early luminal breast cancer remains challenged. The NEOPAL trial (NCT 02400567; Cottu et al, ESMO 2017 LBA09) compared sequential chemotherapy (CT) and letrozole-palbociclib (LP) as neoadjuvant treatment in PAM50 defined high-risk luminal breast cancer patients, showing that LP might be as efficient as CT with regard to breast conserving surgery and pathological response. We report here extended exploratory pathological results, focusing on tumor infiltrating lymphocytes (TILs), proliferative response and preoperative endocrine prognostic index (PEPI) scores. Material and Methods Tumor blocks from baseline biopsy and surgical specimens were available for centralized review from the 106 randomized patients (53 in each arm). TILs quantification, KI67 staining and counting, and ER quantification were performed according to standard methods. Residual proliferative cancer burden (RPCB) and PEPI scores were computed according to published algorithms. Wilcoxon rank sum test and Mann Whitney test were used to compare paired and unpaired data. The chi-square and Fisher exact tests were used for categorical variables. Results Overall, median TILs count did not differ between LP and CT patients, both at baseline (p=0.37) and at the end of treatment (p=0.42). Median TILs count climbed from 5% (0-60) to 10% (1-60) in the LP arm (p=0.0026) and from 2% (0-30) to 10% (0-60) in the CT arm (p=0.0023). Median Ki67 dropped sharply in both arms, from 30% (1-80) to 1% (0-30) in the LP arm (p=1.10e-8) and from 30% (2-80) to 5% (0-30) in the CT arm (p=3.10e-9). Decrease in the Ki67 geometric mean was as sharp. Of note, while baseline Ki67 was similar in both arms (p=0.315), decrease in the LP arm was significantly more profound than in the CT arm (p=0.00075). Pathological response according to RPCB were as follows, in the LP and CT arm, respectively: class 0: 9.6%/10.2%; class I: 84.6%/73.5%; class II: 5.8%/16.3%. The relapse free survival PEPI scores were as follow in the LP and CT arm, respectively: class I: 13.5%/16.3%; class II: 59.6%/46.9%; class III: 28.9%/36.8% (p=0.504). Breast cancer specific survival PEPI scores were as follow in the LP and CT arm, respectively: class I: 18.9%/8.2%; class II: 54.7%/40.8%; class III: 26.4%/51%. These results were significantly better in the LP arm (p=0.027). There was no correlation between final TILs quantification and the RPCB or PEPI scores. Conclusions In this prospective multicenter study with centralized pathological review, neoadjuvant letrozole-palbociclib combination generates impressive proliferative and endocrine specific response features. It compared well with chemotherapy. The LP combination also significantly increased lymphocytic infiltration. Its clinical significance and utility remain to be elucidated, but it potentially adds new prognostic and theranostic information. Abstract

Essentials Neutrophil extracellular traps (NETs) are generated during deep vein thrombosis (DVT). The role of interferon γ (IFNγ) and natural killer (NK) cells in NET formation was studied. IFNγ promote venous thrombosis through NET formation. NK cell depletion reduces DVT. SUMMARY: Background Neutrophils contribute to venous thrombosis through the release of neutrophil extracellular traps (NETs), but the mechanism triggering their formation remains unclear. In vitro data show that interferon (IFN)-γ induces the formation of NETs. Objectives To determine whether IFN-γ and the transcription factor T-box expressed on T cells (Tbet) promote venous thrombosis through neutrophil activation. Methods Venous thrombosis was induced by flow restriction in the inferior vena cava in IFN-γ-/- , Tbet-/- or wild-type (WT) mice. After 48 h, thrombus size was measured by the use of high-frequency ultrasound. NET formation was determined by immunofluorescence. Results and Conclusions Thrombus formation was reduced in Tbet-/- and IFN-γ-/- mice, suggesting that Tbet/IFN-γ-expressing cells are required for venous thrombosis. The number of NETs formed during thrombosis was significantly lower in Tbet-/- and IFN-γ-/- mice. NET formation was also decreased in WT mice treated with an IFN-γ-blocking antibody. Injection of recombinant IFN-γ into IFN-γ-/- mice rescued the phenotype. Natural killer (NK) cells were specifically depleted prior to venous thrombosis induction. NK cell depletion results in decreased NET formation and smaller thrombi, suggesting that NK cells are required for thrombus development. In depleted mice, adoptive transfer of WT NK cells induced a similar thrombosis burden as in WT mice. In contrast, adoptive transfer of IFN-γ -/- NK cells resulted in thrombi similar in size to those in depleted mice. In vitro, we showed that WT neutrophils released fewer NETs when they were cocultured with IFN-γ-/- NK cells. This study demonstrates that NK cell-dependent IFN-γ production is crucial for thrombus development by promoting the formation of NETs by neutrophils.

Abstract

International audience; We propose a fast aid-to-diagnosis biopsy assessment method convenient at the point-of-care, on account of both the imaging technique and the algorithm applied. The procedure implies a pipeline of classification and localization of tumors in breast cancer biopsies imaged with a recently developed non-invasive imaging modality: Dynamic Cell Imaging (aka Dynamic Full Field Optical Coherence Tomography). This allows for fast and interpretable extemporaneous cancer detection with high confidence; we obtained a performance of 96% classification accuracy together with a coarse localization of tumors, even so for single isolated invasive cells.

Abstract

Resume Introduction Les moisissures et les allergenes d’acariens presents dans les environnements interieurs sont connus pour leurs effets sur la sante respiratoire. Methodes De 2011 a 2015, le service parisien de sante environnementale a investigue 293 logements sur demande medicale. Ces audits ont conduit a la realisation d’analyses fongiques de l’air (12 % des logements) et de la poussiere de sol et matelas (24 %), ainsi que de dosages d’allergenes d’acariens presents dans les matelas et tapis (18 %). Resultats Les niveaux de concentration de moisissures dans l’air interieur ne different pas significativement de ceux mesures dans l’air exterieur. Une augmentation des ratios air interieur/air exterieur est, cependant, observee en cas d’absence ou dysfonctionnement du systeme de ventilation, mettant en evidence un enrichissement en moisissures dans l’air du logement. Dans la poussiere, les teneurs en spores fongiques sont variables selon les supports. La contamination des matelas est plus importante dans les logements ou une surface moisie de plus d’1 m2 est observee. De meme, les concentrations d’allergenes d’acariens Der p1 sont plus elevees dans la poussiere des matelas des logements presentant un developpement fongique. Conclusions Cette etude permet de decrire les niveaux de contamination des logements de patients parisiens.

Abstract

In this study, first-principles investigations were performed using the full-potential linearized augmented plane-wave method of the structural and optoelectronic properties of thorium germinate (ThGeO4), a high-K dielectric material. Under ambient conditions, the structural properties calculated for ThGeO4 in the zircon phase were in excellent agreement with the available experimental data. Furthermore, using the modified Becke -Johnson correction method, the calculated band gaps and optical constants accurately described this compound. Finally, the thermal properties were predicted over a temperature range of 0–700 K and pressures up to 11 GPa using the quasi-harmonic Debye model, where the variations in the heat capacity, primitive cell volume, and thermal expansion coefficients were determined successfully.

Abstract

SIRIUS (Spectroscopy & Identification of Rare Isotopes Using S3) is a detection system designed for the focal plane of S3 (the Super Separator Spectrometer), which is part of the SPIRAL2 (Systeme de Production d’Ions RAdioactifs en Ligne de 2e generation) facility at GANIL (France). This study presents the characterization of the Side silicon detector of SIRIUS. This new “Strippy-pad” detector design benefits from an ultra-high resistivity and the windowless technique from Micron Semiconductor Ltd. These detectors were tested at the IPHC Strasbourg on a dedicated test bench with custom fast preamplifiers coupled to digital electronics cards. Combining all these elements, we obtained resolutions as good as 13.6 keV for certain pixels of this detector for 8.8 MeV alpha particles , with an average resolution measured at 14.7 keV over the whole detector.

Abstract

Tetraphenylporphyrin aluminum(III) salts, TPPAlX, where X = Me, OEt, OiPr, OCHMeCH2Cl, and Cl, and bis(triphenylphosphine)imminium chloride, PPN+Cl− (1 : 1) react with rac-lactide, rac-LA, in neat propylene oxide, PO, to yield chains of enriched isotactic polylactide, PLA, with end groups of PO–Cl and with time these yield cyclic polymers (PO)n(PLA) where n = 2 or 3 and even higher. There is no reaction between TPPAlOR (R = Et or iPr), PPN+Cl−, and rac-LA in neat THF at 25 °C even though TPPAlOR (R = Et or iPr) and PPN+Cl− in neat PO yields polypropylene oxide with a terminal OR group, H(PO)nOR. Taken together, Al(III) acts as a Lewis acid in the ring-opening of PO, in which PPN+Cl− is present and the incipient ClCH2CHMeO− initiates the ROP of LA to yield anion chains of [(PLA)-OCHMeCH2Cl]−, and then the ring-opening of PO yields cycles, (PO)n(PLA), with the liberation of Cl−. The polymer was isolated by the addition of MeOH/HCl and end group analysis by mass spectrometry.

Abstract

Antimalarial drug resistance has historically arisen through convergent de novo mutations in Plasmodium falciparum parasite populations in Southeast Asia and South America. For the past decade in Southeast Asia, artemisinins, the core component of first-line antimalarial therapies, have experienced delayed parasite clearance associated with several pfk13 mutations, primarily C580Y. We report that mutant pfk13 has emerged independently in Guyana, with genome analysis indicating an evolutionary origin distinct from Southeast Asia. Pfk13 C580Y parasites were observed in 1.6% (14/854) of samples collected in Guyana in 2016–2017. Introducing pfk13 C580Y or R539T mutations by gene editing into local parasites conferred high levels of in vitro artemisinin resistance. In vitro growth competition assays revealed a fitness cost associated with these pfk13 variants, potentially explaining why these resistance alleles have not increased in frequency more quickly in South America. These data place local malaria control efforts at risk in the Guiana Shield., eLife digest All recommended treatments against malaria include a drug called artemisinin or some of its derivatives. However, there are concerns that Plasmodium falciparum, the parasite that causes most cases of malaria, will eventually develop widespread resistance to the drug. A strain of P. falciparum partially resistant to artemisinin was seen in Cambodia in 2008, and it has since spread across Southeast Asia. The resistance appears to be frequently linked to a mutation known as pfk13 C580Y. Southeast Asia and Amazonia are considered to be hotspots for antimalarial drug resistance, and the pfk13 C580Y mutation was detected in the South American country of Guyana in 2010. To examine whether the mutation was still circulating in this part of the world, Mathieu et al. collected and analyzed 854 samples across Guyana between 2016 and 2017. Overall, 1.6% of the samples had the pfk13 C580Y mutation, but this number was as high as 8.8% in one region. Further analyses revealed that the mutation in Guyana had not spread from Southeast Asia, but that it had occurred in Amazonia independently. To better understand the impact of the pfk13 C580Y mutation, Mathieu et al. introduced this genetic change into non-resistant parasites from a country neighbouring Guyana. As expected, the mutation made P. falciparum highly resistant to artemisinin, but it also slowed the growth rate of the parasite. This disadvantage may explain why the mutation has not spread more rapidly through Guyana in recent years. Artemisinin and its derivatives are always associated with other antimalarial drugs to slow the development of resistance; there are concerns that reduced susceptibility to artemisinin leads to the parasites becoming resistant to the partner drugs. Further research is needed to evaluate how the pfk13 C580Y mutation affects the parasite’s response to the typical combination of drugs that are given to patients.

International audience; We have measured the radiation tolerance of chemical vapor deposition (CVD) diamond against protons and neutrons. The relative radiation damage constant of 24 GeV protons, 800 MeV protons, 70 MeV protons, and fast reactor neutrons is presented. The results are used to combine the measured data into a universal damage curve for diamond material.

Abstract

International audience; In sight of the luminosity increase of the High Luminosity-LHC (HL-LHC), most experiments at the CERN Large Hadron Collider (LHC) are planning upgrades for their innermost layers in the next 5–10 years. These upgrades will require more radiation tolerant technologies than exist today. Usage of Chemical Vapor Deposition (CVD) diamond as detector material is one of the potentially interesting technologies for the upgrade. CVD diamond has been used extensively in the beam condition monitors of BaBar, Belle, CDF and all LHC experiments. Measurements of the radiation tolerance of the highest quality polycrystalline CVD material for a range of proton energies, pions and neutrons obtained with this material are presented. In addition, new results on the evolution of various semiconductor parameters as a function of the dose rate are described.

Abstract

The planned upgrade of the LHC to the High-Luminosity-LHC will push the luminosity limits above the original design values. Since the current detectors will not be able to cope with this environment ATLAS and CMS are doing research to find more radiation tolerant technologies for their innermost tracking layers. Chemical Vapour Deposition (CVD) diamond is an excellent candidate for this purpose. Detectors out of this material are already established in the highest irradiation regimes for the beam condition monitors at LHC. The RD42 collaboration is leading an effort to use CVD diamonds also as sensor material for the future tracking detectors. The signal behaviour of highly irradiated diamonds is presented as well as the recent study of the signal dependence on incident particle flux. There is also a recent development towards 3D detectors and especially 3D detectors with a pixel readout based on diamond sensors.

Abstract

Insulin degludec (Tresiba®) is characterized by an original mode of prolonged and continuous insulin release after its subcutaneous injection. Thereby, it has a very long glucose-lowering effect, around 42 hours, and a better reproducibility from both a pharmacokinetic and pharmacodynamic point of view. Its efficacy and safety have been assessed in the phase 3 clinical programme BEGIN as compared with insulin glargine U100, in patients with type 1 diabetes (T1D) and type 2 (T2D). For a similar glucose control (reduction in glycated haemoglobin), less hypoglycaemic episodes were recorded, including severe hypoglycaemia, during the nocturnal period, with insulin degludec than with insulin glargine U100. This clinical benefit has been confirmed in the complementary SWITCH programme in T1D and T2D patients at higher risk of hypoglycaemia, in the double-blind cardiovascular outcome trial DEVOTE in T2D patients at high cardiovascular risk and in real-life conditions in the observational European EU-TREAT study in patients with T1D and T2D. Insulin degludec (Tresiba®) is indicated and reimbursed for the treatment of patients with T1D, combined with a prandial insulin, and T2D, alone or combined with oral antidiabetic agents, a glucagon-like peptide-1 receptor agonist or a short-acting insulin.L’insuline dégludec (Tresiba®) présente un mode original de libération prolongée et continue d’insuline, ce qui lui confère une durée d’action ultra-longue, avoisinant 42 heures, et une plus grande reproductibilité, à la fois sur le plan pharmacocinétique et pharmacodynamique. Son efficacité et sa sécurité d’emploi ont été validées dans le programme d’essais cliniques de phase 3 BEGIN en comparaison à l’insuline glargine U100, aussi bien dans le diabète de type 1 (DT1) que de type 2 (DT2). Pour un même niveau de contrôle glycémique (attesté par la réduction du taux d’hémoglobine glyquée), il y a eu moins d’hypoglycémies, y compris sévères, durant la période nocturne, avec l’insuline dégludec qu’avec l’insuline glargine U100. Ce bénéfice a été confirmé dans le programme complémentaire SWITCH chez des patients DT1 et DT2 à plus haut risque hypoglycémique, dans la grande étude DEVOTE, contrôlée en double aveugle, chez des patients DT2 à risque cardiovasculaire et, en vie réelle, dans l’étude observationnelle européenne EU-TREAT chez des patients DT1 et DT2. L’insuline dégludec (Tresiba®) est indiquée et remboursée dans le traitement des patients DT1, en association avec une insuline prandiale, et DT2, seule ou en combinaison avec des antidiabétiques oraux, un agoniste des récepteurs du glucagon-like-peptide 1 ou une insuline à action rapide.

Abstract

Resume Les complications en chirurgie oncologique thoracique varient selon les comorbidites et le type de chirurgie. Les hemorragies, les infections et l’œdeme pulmonaire ne sont pas specifiques du type de resection mais sont plus frequemment rencontres apres pneumonectomies. Les pneumonectomies ont une morbidite due aux modifications majeures du contenu intrathoracique. L’infarctus pulmonaire et le volvulus bronchique sont des situations urgentes rencontrees apres lobectomie. Le scanner met en evidence des signes congestifs localises et la stenose ou l’occlusion d’une veine ou d’une bronche. La reconnaissance rapide d’evenements critiques, en particulier grâce a une analyse systematique du scanner, est capitale pour une prise en charge adequate au decours d’une complication postoperatoire ou retardee en oncologie thoracique.

Abstract

In the present study, the structural, thermal, and electronic properties of some important orthosilicate dielectrics, such as the ZrSiO4, ZrGeO4, and HfSiO4 compounds, have been investigated theoretically with the use of first-principle calculations. We attribute the application of the modified Becke–Johnson exchange potential, which is basically an improvement over the local density approximation and the Perdew–Burke–Ernzerhof exchange–correlation functional, for a better description of the band gaps of the compounds. This resulted in a good agreement with our estimated values in comparison with the reported experimental data, specifically for the ZrSiO4, and HfSiO4 compounds. Conversely, for the ZrGeO4 compound, the calculated electronic band structure shows a direct band gap at the Γ point with the value of 5.79 eV. Furthermore, our evaluated thermal properties that are calculated by using the quasi-harmonic Debye model indicated that the volume variation with temperature is higher in the ZrGeO4 compound as compared to both the ZrSiO4 and HfSiO4 compounds, which is ascribed to the difference between the electron shells of the Si and Ge atoms. Therefore, these results also indicate that while the entropy (S) and enthalpy (U) parameters increase monotonically, the free energy (G), in contrast, decreases monotonically with increasing temperature, respectively. Moreover, the pressure and temperature dependencies of the Debye temperature Θ, thermal expansion coefficient, and heat capacities C V were also predicted in our study.

Abstract

Proceedings of the 13th Pisa Meeting on Advanced Detectors : Frontier Detectors for Frontier Physics, 24-30 May 2015, La Biodola, Italy.; International audience; In the present study, results towards the development of a 3D diamond sensor are presented. Conductive channels are produced inside the sensor bulk using a femtosecond laser. This electrode geometry allows full charge collection even for low quality diamond sensors. Results from testbeam show that charge is collected by these electrodes. In order to understand the channel growth parameters, with the goal of producing low resistivity channels, the conductive channels produced with a different laser setup are evaluated by Raman spectroscopy.

Abstract

Backgound : The primary analysis of the REMAGUS-02 multicenter randomized phase II trial demonstrated that celecoxib did not improve pCR rates in pts with HER2-negative localized invasive breast cancer (BC), whereas trastuzumab increased pCR rates in HER2-positive ones [Pierga BCRT 2010]. We report here the long-term follow-up results of this trial for disease free survival (DFS) and overall survival (OS). Patients and methods: From May 2004 to October 2007, 340 stage II-III BC patients were randomly assigned to receive 4 cycles (c) of epirubicin–cyclophosphamide q 3 w followed by 4 c of docetaxel q 3 w +/- trastuzumab in HER2 positive pts (120 pts) or +/- celecoxib in HER2 negative pts (n=220). From September 2005, all pts with HER2 positive BC received adjuvant T for a total of 18 c (n=106). Patients with hormone receptors (HR) positive tumor received adjuvant endocrine treatment according to menopausal status Results: With a median follow up of nearly 8 years (94.4 months, 20-127m), 112 relapses and 75 deaths have been observed (median DFS and OS not reached). Eight years DFS and OS were respectively 63 % [57%-71%] and 75 % [70%-81%] in HER2 negative group; and 75% [67%-83%] and 82 % [74%-90%] in HER2 positive group. DFS was significantly higher in HER+ pts than in HER2-(HR: 0.64 [0.42-0.99], p=0.042), whereas OS did not differ significantly (HR: 0.67, [0.41-1.11], p=0.123). In the overall population, progesterone receptor (PgR) positivity was associated with a better DFS (p=0.012) and OS (p In the HER2- subgroup, no effect of neoadjuvant celocoxib was observed on survival, neither in intention to treat (ITT) nor in per protocol analyses. In the multivariate analysis clinical stage (T3/T4 versus T2, HR: 1.92 [1.209 - 3.05], p=0.006), PgR status (positive versus negative HR : 0.52, [0.32-0.84], p=0.007) and pCR (yes vs no, HR : 0.213 [0.066-0.687], p=0.01) were significant predictors of DFS. In the HER2+ subgroup, neoadjuvant versus adjuvant trastuzumab was not significantly associated with DFS, neither in the ITT, nor in the per protocol analysis. Conclusion: Celecoxib was not associated with pCR or survival benefit when added to conventional neoadjuvant CT in Her2-negative BC pts. Lack of PgR expression is a major prognostic factor for survival. Neoadjuvant versus adjuvant trastuzumab increased pCR rates but did not change significantly DFS and OS of HER2 positive BC pts. Abstract

Xultophy® (IDegLira) is a fixed ratio combination of basal insulin degludec and glucagon-like peptide-1 (GLP-1) receptor agonist liraglutide. Insulin degludec is characterized by an original mode of prolonged and continuous insulin diffusion after its subcutaneous injection. Thereby, it has a very long half-life, around 25 hours, and a better reproducibility from both a pharmacokinetic and pharmacodynamic point of view, with less hypoglycaemia, especially at night. Liraglutide is a well-known once-daily GLP-1 receptor agonist that showed a cardiovascular and renal protection in patients with type 2 diabetes at high cardiovascular risk. Both molecules exert complementary antihyperglycaemic effects, which allows a better glucose control, both in the fasting and postprandial states. IDegLira is more effective than another basal insulin regimen in reaching individualized glycated haemoglobin target, with a lower daily dose of insulin. It has a better tolerance profile, with a more favourable effect on body weight and less hypoglycaemia compared with a basal insulin and less gastrointestinal adverse effects when compared with liraglutide alone. Xultophy® is presented as a prefilled pen and is indicated in the management of type 2 diabetes not well controlled with basal insulin. The dose of IDegLira is progressively uptitrated, starting from 16 dose steps up to a maximum of 50 dose steps per day (corresponding to 50 IU insulin degludec and 1.8 mg liraglutide).Le Xultophy® (IDegLira) est une combinaison, en proportion fixe, d’une nouvelle insuline basale, l’insuline dégludec, et d’un agoniste des récepteurs du glucagonlike peptide-1 (GLP-1), le liraglutide. L’insuline dégludec présente un mode original de libération prolongée et continue d’insuline, ce qui lui confère une demi-vie ultra-longue, avoisinant 25 heures, et une plus grande reproductibilité, à la fois sur le plan pharmacocinétique et pharmacodynamique, avec moins d’hypoglycémies, notamment nocturnes. Le liraglutide est un agoniste des récepteurs du GLP-1 à injection quotidienne qui a démontré une protection cardiaque et rénale chez des patients avec un diabète de type 2 (DT2) et un haut risque cardiovasculaire. Les deux molécules exercent des effets antihyperglycémiants complémentaires, permettant un meilleur contrôle de la glycémie, à jeun et en période post-prandiale. La combinaison IDegLira permet d’atteindre plus souvent la cible individualisée d’hémoglobine glyquée, avec une dose d’insuline plus faible. Elle offre également un meilleur profil de tolérance, avec un effet plus favorable sur le poids corporel et moins d’hypoglycémies par comparaison à une insuline basale seule et moins de manifestations indésirables digestives par rapport au liraglutide seul. Le Xultophy® est présenté sous forme de stylo pré-rempli et indiqué dans le traitement du DT2 insuffisamment contrôlé sous insuline basale. La dose de la combinaison IDegLira est titrée progressivement en commençant par 16 doses unitaires par jour jusqu’à un maximum de 50 doses unitaires par jour (correspondant à 50 UI d’insuline dégludec et 1,8 mg de liraglutide).

The GABRIELA [1] set-up is used at the FLNR to perform detailed nuclear structure studies of transfermium nuclei. Abstract

International audience; This work considers a two dimensional numerical device simulation system using a novel digitizing scheme based on finite elements coupled with the Delaunay triangulation method which allowed an optimal mesh involved in nonrectangular devices as a corner diode. A grid was generated automatically according to the specified device geometry standing on the Delaunay triangulation process. The solution of the problem consists in the resolution of three strong nonlinear partial differential equations (PDE) which are, in occurrence, two dimensional Poisson and continuity equations. Modeled voltage, free carriers distribution and I-V characteristics were extracted when the PN junction resolution was based on second-order Bezier curves. The results are presented using a Delaunay triangulation meshing mathematical tool.

44. Mixed-Effects Estimation in Dynamic Models of Plant Growth for the Assessment of Inter-individual Variability

Abstract

Modeling inter-individual variability in plant populations is a key issue to understand crop heterogeneity and its variations in response to the environment. Being able to describe the interactions among plants and explain the variability observed in the population could provide useful information on how to control it and improve global plant growth. We propose here a method to model plant variability within a field, by extending the so-called GreenLab functional-structural plant model from the individual to the population scale via nonlinear mixed-effects modeling. Parameter estimation of the population model is achieved using the stochastic approximation expectation maximization algorithm, implemented in the platform for plant growth modeling and analysis PyGMAlion. The method is first applied on a set of simulated data and then on a real dataset from a population of 34 winter oilseed rape plants at the rosette stage. Results show that our method allows for a good characterization of the variability in the population with only a limited number of parameters, which is a key point for plant models. Results on simulated data show that parameters associated with a low sensitivity index are inaccurately estimated by the algorithm when considered as random effects, but a good stability of the results can be obtained by considering them as fixed effects. These results open new ways for the analysis of inter-plant variability within a population and the study of plant–plant competition.Supplementary materials accompanying this paper appear online. © 2018, International Biometric Society.

Abstract

Tumor-infiltrating lymphocytes (TILs) are emerging as strong prognostic factor for early breast cancer patients, especially in the triple-negative subtype. Here, we aim to validate previous findings on the prognostic role of TIL in the context of two randomized adjuvant trials and to investigate whether lymphocyte infiltrates can predict benefit from adjuvant anthracyclines.A total of 816 patients enrolled and treated at the Gustave Roussy in the context of two multicentric randomized trials comparing adjuvant anthracyclines versus no chemotherapy were included in the present analysis. Primary end point was overall survival (OS). Hematoxilin and eosin slides of primary tumors were retrieved and evaluated for the percentage of intratumoral (It) and stromal (Str) TIL. Each case was also defined as high-TIL or low-TIL breast cancer adopting previously validated cutoffs.TIL were assessable for 781 of 816 cases. High-TIL cases were more likely grade 3 and estrogen receptor (ER)-negative (P0.001). In multivariate analysis, both continuous It-TIL and Str-TIL were strong prognostic factors for OS [hazard ratio (HR) 0.85, 95% confidence interval (CI) 0.77-0.95 P = 0.003; HR 0.89, 95% CI 0.81-0.96, P = 0.005 for It-TIL and Str-TIL, respectively]. The prognostic effect of continuous TIL was limited to triple-negative and HER2-positive patients. Ten-year OS rates were: 89% and 68% for triple-negative high-TIL and low-TIL, respectively (HR 0.44, 95% CI 0.18-1.10, P = 0.07) and 78% and 57% for HER2-positive high-TIL versus low-TIL, respectively (HR 0.46, 95% CI 0.20-1.11, P = 0.08). Either continuous or binary TIL variables did not predict for the efficacy of anthracyclines. Test for interaction P value was not significant in the whole study population and in subgroups (ER+/HER2-, HER2+, ER-/HER2-).We confirmed the prognostic role of TIL in triple-negative early breast cancer and suggested a prognostic impact in HER2+ patients as well. Basing on our data, TIL should not be used as a parameter to select patients for anthracyclines chemotherapy.

47. Évaluation du statut ganglionnaire et prostatique à la phase dynamique précoce de la TEP à la 18F-choline dans le cancer de prostate

Abstract

Resume Objectif Evaluer l’apport de la cinetique precoce de l’imagerie TEP a la 18 F-choline (FCH) pour definir le statut ganglionnaire et prostatique dans le cancer de la prostate. Patients et methodes Les resultats de 39 patients ont ete analyses retrospectivement. Des acquisitions pelviennes dynamiques de 10 minutes ont ete realisees immediatement apres l’injection de 3–4 MBq/kg de FCH sur les cameras TEP GE Discovery LS (GE-LS) ou Siemens Biograph mCT (mCT), suivies d’acquisitions corps entier tardives. La SUVmax des lesions ganglionnaires et prostatiques a ete mesuree a chaque minute de l’acquisition dynamique et pour l’acquisition tardive. Le statut des lesions a ete base sur l’analyse histologique ou le suivi clinique du patient. Resultats Les TEP etaient realisees pour recidive biochimique (53,8 %) ou bilan d’extension initial (46,2 %). Le PSA median etait a 8,46 ng/mL et le Gleason median a 3 + 4. Quatre-vingt-douze lesions (43 ganglionnaires et 49 prostatiques) ont ete analysees, incluant 63 lesions malignes. La SUVmax sur les acquisitions dynamiques precoces etait significativement plus elevee pour les ganglions malins que benins respectivement sur la mCT et la GE-LS ( p p = 0,007) ainsi que pour les lesions prostatiques ( p = 0,001, p = 0,02). Les courbes ROC ont montre, pour les lesions ganglionnaires acquises sur la mCT, une tendance a de meilleurs chiffres de sensibilite et de specificite pour les acquisitions precoces, en comparaison avec les acquisitions tardives. Conclusion Les courbes dynamiques precoces semblent un outil diagnostique performant dans la definition du statut ganglionnaire ou prostatique, dans une pathologie ou les preuves de malignite peuvent etre difficiles a obtenir.

Abstract

11th International Conference on Position Sensitive Detectors (PSD11),3–8 sept. 2017, The Open University, Milton Keynes, UK.; International audience; Beam test results of the radiation tolerance study of chemical vapour deposition (CVD) diamond against different particle species and energies is presented. We also present beam test results on the independence of signal size on incident particle rate in charged particle detectors based on un-irradiated and irradiated poly-crystalline CVD diamond over a range of particle fluxes from 2 kHz/cm2 to 10 MHz/cm2. The pulse height of the sensors was measured with readout electronics with a peaking time of 6 ns. In addition functionality of poly-crystalline CVD diamond 3D devices was demonstrated in beam tests and 3D diamond detectors are shown to be a promising technology for applications in future high luminosity experiments.

Abstract

The Advanced GAmma Tracking Array (AGATA) has been installed at the GANIL facility, Caen-France. This setup exploits the stable and radioactive heavy-ions beams delivered by the cyclotron accelerator complex of GANIL. Additionally, it benefits from a large palette of ancillary detectors and spectrometers to address in-beam γ-ray spectroscopy of exotic nuclei. The set-up has been designed to couple AGATA with a magnetic spectrometer, charged-particle and neutron detectors, scintillators for the detection of high-energy γ rays and other devices such as a plunger to measure nuclear lifetimes. In this paper, the design and the mechanical characteristics of the set-up are described. Based on simulations, expected performances of the AGATA l π array are presented.
