We conducted a preregistered multilaboratory project ( k = 36; N = 3,531) to assess the size and robustness of ego-depletion effects using a novel replication method, termed the paradigmatic replication approach. Each laboratory implemented one of two procedures that was intended to manipulate self-control and tested performance on a subsequent measure of self-control. Confirmatory tests found a nonsignificant result ( d = 0.06). Confirmatory Bayesian meta-analyses using an informed-prior hypothesis (δ = 0.30, SD = 0.15) found that the data were 4 times more likely under the null than the alternative hypothesis. Hence, preregistered analyses did not find evidence for a depletion effect. Exploratory analyses on the full sample (i.e., ignoring exclusion criteria) found a statistically significant effect ( d = 0.08); Bayesian analyses showed that the data were about equally likely under the null and informed-prior hypotheses. Exploratory moderator tests suggested that the depletion effect was larger for participants who reported more fatigue but was not moderated by trait self-control, willpower beliefs, or action orientation.

We have studied the temperature and magnetic field dependence of the total magnetic moment of large-area permalloy artificial square spin ice arrays. The temperature dependence and hysteresis behavior are consistent with the coherent magnetization reversal expected in the Stoner-Wohlfarth model, with clear deviations due to inter-island interactions at small lattice spacing. Through micromagnetic simulations, we explore this behavior and demonstrate that the deviations result from increasingly complex magnetization reversal at small lattice spacing, induced by inter-island interactions, and depending critically on details of the island shapes. These results establish new means to tune the physical properties of artificial spin ice structures and other interacting nanomagnet systems, such as patterned magnetic media., 17 pages, 5 figures

Summary Background Data on patients with COVID-19 who have cancer are lacking. Here we characterise the outcomes of a cohort of patients with cancer and COVID-19 and identify potential prognostic factors for mortality and severe illness. Methods In this cohort study, we collected de-identified data on patients with active or previous malignancy, aged 18 years and older, with confirmed severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection from the USA, Canada, and Spain from the COVID-19 and Cancer Consortium (CCC19) database for whom baseline data were added between March 17 and April 16, 2020. We collected data on baseline clinical conditions, medications, cancer diagnosis and treatment, and COVID-19 disease course. The primary endpoint was all-cause mortality within 30 days of diagnosis of COVID-19. We assessed the association between the outcome and potential prognostic variables using logistic regression analyses, partially adjusted for age, sex, smoking status, and obesity. This study is registered with ClinicalTrials.gov, NCT04354701, and is ongoing. Findings Of 1035 records entered into the CCC19 database during the study period, 928 patients met inclusion criteria for our analysis. Median age was 66 years (IQR 57–76), 279 (30%) were aged 75 years or older, and 468 (50%) patients were male. The most prevalent malignancies were breast (191 [21%]) and prostate (152 [16%]). 366 (39%) patients were on active anticancer treatment, and 396 (43%) had active (measurable) cancer. At analysis (May 7, 2020), 121 (13%) patients had died. In logistic regression analysis, independent factors associated with increased 30-day mortality, after partial adjustment, were: increased age (per 10 years; partially adjusted odds ratio 1·84, 95% CI 1·53–2·21), male sex (1·63, 1·07–2·48), smoking status (former smoker vs never smoked: 1·60, 1·03–2·47), number of comorbidities (two vs none: 4·50, 1·33–15·28), Eastern Cooperative Oncology Group performance status of 2 or higher (status of 2 vs 0 or 1: 3·89, 2·11–7·18), active cancer (progressing vs remission: 5·20, 2·77–9·77), and receipt of azithromycin plus hydroxychloroquine (vs treatment with neither: 2·93, 1·79–4·79; confounding by indication cannot be excluded). Compared with residence in the US-Northeast, residence in Canada (0·24, 0·07–0·84) or the US-Midwest (0·50, 0·28–0·90) were associated with decreased 30-day all-cause mortality. Race and ethnicity, obesity status, cancer type, type of anticancer therapy, and recent surgery were not associated with mortality. Interpretation Among patients with cancer and COVID-19, 30-day all-cause mortality was high and associated with general risk factors and risk factors unique to patients with cancer. Longer follow-up is needed to better understand the effect of COVID-19 on outcomes in patients with cancer, including the ability to continue specific cancer treatments. The Elliott-Yafet theory of spin relaxation in nonmagnetic metals predicts proportionality between spin and momentum relaxation times for scattering centers such as phonons. Here, we test this theory in Al nanowires over a very large thickness range (8.5-300 nm), finding that the Elliott-Yafet proportionality "constant" for phonon scattering in fact exhibits a large, unanticipated finite-size effect. Supported by analytical and numerical modeling, we explain this via strong phonon-induced spin relaxation at surfaces and interfaces, driven in particular by enhanced spin-orbit coupling.

Applied magnetic fields are an important tuning parameter for artificial spin ice (ASI) systems, as they can drive phase transitions between different magnetic ground states, or tune through regimes with high populations of emergent magnetic excitations (e.g., monopole-like quasiparticles). Here, using simulations supported by experiments, we investigate the thermodynamic properties and magnetic phases of square and quadrupolar ASI as a function of applied in-plane magnetic fields. Monte Carlo simulations are used to generate field-dependent maps of the magnetization, the magnetic specific heat, the thermodynamic magnetization fluctuations, and the magnetic order parameters, all under equilibrium conditions. These maps reveal the diversity of magnetic orderings and the phase transitions that occur in different regions of the phase diagrams of these ASIs, and are experimentally supported by magneto-optical measurements of the equilibrium "magnetization noise" in thermally-active ASIs., 12 pages, 7 figures, in press at Physical Review B

The 2016 U.S. presidential election was marked by hostile political discourse, often on social media, where users were exposed to divergent, and potentially distressing, political discourse. This research explores the effects of this election on the well-being of emerging adults who receive the majority of their news via social media. Using data from the Emerging Adulthood Measured at Multiple Institutions 2 Study, we expected greater social media use to be associated with greater perceived stress, and lower well-being, among emerging adults who are more politically extreme, and expected these relationships would be moderated by social support and social media use. Our preregistered analysis did not support our hypotheses. Although there were some effects of extremity on stress and well-being, overall the direction of the effects were inconsistent and neither social media use nor social support was found to moderate the effects of extremity on stress and well-being.

The Clair Field is a giant oilfield containing in the region of 6–7 Bbbl of stock tank oil initially in place, located approximately 75 km west of the Shetland Islands. As such, it represents the single biggest hydrocarbon accumulation on the UK Continental Shelf. Clair was discovered in 1977, but first production did not occur from Phase 1 until 2005, after a lengthy appraisal period. The major appraisal milestone occurred in 1991 after well 206/8-8 proved up fractured clastic red beds of the Devonian Lower Clair Group. This was followed up with an extended well test on 206/8-10Z, which demonstrated the longer-term performance of the reservoir. Further appraisal on Clair Ridge led to the sanction of the Clair Ridge, which came on stream in November 2018. Following the Greater Clair appraisal programme in 2013–15, development options are currently being worked for Clair South, which will develop the Lower Clair Group reservoirs together with overlying shallow-marine reservoirs of the Cretaceous and Jurassic.

Open science initiatives, which are often collaborative efforts focused on making research more transparent, have experienced increasing popularity in the past decade. Open science principles of openness and transparency provide opportunities to advance diversity, justice, and sustainability by promoting diverse, just, and sustainable outcomes among both undergraduate and senior researchers. We review models that demonstrate the importance of greater diversity, justice, and sustainability in psychological science before describing how open science initiatives promote these values. Open science initiatives also promote diversity, justice, and sustainability through increased levels of inclusion and access, equitable distribution of opportunities and dissemination of knowledge, and increased sustainability stemming from increased generalizability. In order to provide an application of the concepts discussed, we offer a set of diversity, justice, and sustainability lens questions for individuals to use while assessing research projects and other organizational systems and consider concrete classroom applications for these initiatives.

Psychiatric assistance dogs for military veterans with posttraumatic stress disorder (PTSD) currently make up over 19% of assistance dog partnerships globally. We conducted a systematic review of the literature relating to these partnerships, with specific aims to (1) summarize their characteristics, (2) evaluate the quality of existing evidence, and (3) summarize outcomes. A total of 432 records were independently screened (Cohen’s kappa = 0.90). Of these, 41 articles (29 peer-reviewed publications and 12 unpublished dissertations) met inclusion criteria. Data extraction was conducted to address the research aims, including a meta-analysis (quantitative outcomes) and meta-synthesis (qualitative outcomes). All peer-reviewed publications on the topic of psychiatric assistance dogs for veterans with PTSD were published within the last five years. The majority of included articles were quantitative (53%), 41% were qualitative, and 6% employed mixed methods. Mean methodological rigor scores were 80% for peer reviewed articles and 71% for dissertations, where higher scores represent more rigorous methodology. Quantitative articles reported significant improvements in the domains of PTSD severity, mental health, and social health. Impacts on physical health and global quality of life appear inconclusive. In a now-classic study by Srull and Wyer (1979), people who were exposed to phrases with hostile content subsequently judged a man as being more hostile. And this “hostile priming effect” has had a significant influence on the field of social cognition over the subsequent decades. However, a recent multi-lab collaborative study (McCarthy et al., 2018) that closely followed the methods described by Srull and Wyer (1979) found a hostile priming effect that was nearly zero, which casts doubt on whether these methods reliably produce an effect. To address some limitations with McCarthy et al. (2018), the current multi-site collaborative study included data collected from 29 labs. Each lab conducted a close replication (total N = 2,123) and a conceptual replication (total N = 2,579) of Srull and Wyer's methods. The hostile priming effect for both the close replication (d = 0.09, 95% CI [-0.04, 0.22], z = 1.34, p =.16) and the conceptual replication (d = 0.05, 95% CI [-0.04, 0.15], z = 1.15, p =.58) were not significantly different from zero and, if the true effects are non-zero, were smaller than what most labs could feasibly and routinely detect. Despite our best efforts to produce favorable conditions for the effect to emerge, we did not detect a hostile priming effect. We suggest that researchers should not invest more resources into trying to detect a hostile priming effect using methods like those described in Srull and Wyer (1979)., NA

When the COVID-19 pandemic began, formal frameworks to collect data about affected patients were lacking. The COVID-19 and Cancer Consortium (CCC19) was formed to collect granular data on patients with cancer and COVID-19 at scale and as rapidly as possible. CCC19 has grown from five initial institutions to 125 institutions with >400 collaborators. More than 5,000 cases with complete baseline data have been accrued. Future directions include increased electronic health record integration for direct data ingestion, expansion to additional domestic and international sites, more intentional patient involvement, and granular analyses of still-unanswered questions related to cancer subtypes and treatments.

With over 4 million ha irrigated cropland, the Lower Mississippi River Valley is a highly productive agricultural region where irrigation practices are similar and the Mississippi River Valley alluvial aquifer (MRVA) serves as a primary source of on-demand irrigation. The power conversion coefficient method was used to measure season-long irrigation applied by producers to cotton, maize, rice, and soybean grown in the Mississippi Delta over a 12-year period that spanned a range of weather and cropping conditions. Averaged across all years and irrigation methods, irrigation rates were 9200, 3100, 2800, and 1800 m3 ha−1 for rice, maize, soybean, and cotton, respectively. Pivot and flood methods resulted in the lowest application rates when compared to furrow irrigation of row crops. In terms of rice, zero-grade fields received the least irrigation while multiple-inlet rice irrigation reduced irrigation by 19% relative to cascade flooding. No differences were measured between contour- and straight-levee rice fields. Irrigation rates for cotton and rice remained unchanged over time. Maize-soybean rates increased by approximately 200 m3 ha−1 y−1 and correlated (r > 0.67) with cumulative moisture deficit at Stoneville. Study results provide irrigation rates necessary to calculate detailed water budgets for the MRVA. These data may also serve as benchmarks for irrigation performance evaluations.

Background: Germline variants in the melanocortin 1 receptor gene (MC1R) might increase the risk of childhood and adolescent melanoma, but a clear conclusion is challenging because of the low number of studies and cases. We assessed the association of MC1R variants with childhood and adolescent melanoma in a large study comparing the prevalence of MC1R variants in child or adolescent patients with melanoma to that in adult patients with melanoma and in healthy adult controls. Methods: In this retrospective pooled analysis, we used the M-SKIP Project, the Italian Melanoma Intergroup, and other European groups (with participants from Australia, Canada, France, Greece, Italy, the Netherlands, Serbia, Spain, Sweden, Turkey, and the USA) to assemble an international multicentre cohort. We gathered phenotypic and genetic data from children or adolescents diagnosed with sporadic single-primary cutaneous melanoma at age 20 years or younger, adult patients with sporadic single-primary cutaneous melanoma diagnosed at age 35 years or older, and healthy adult individuals as controls. We calculated odds ratios (ORs) for childhood and adolescent melanoma associated with MC1R variants by multivariable logistic regression. Subgroup analysis was done for children aged 18 or younger and 14 years or younger. Findings: We analysed data from 233 young patients, 932 adult patients, and 932 healthy adult controls. Children and adolescents had higher odds of carrying MC1R r variants than did adult patients (OR 1·54, 95% CI 1·02–2·33), including when analysis was restricted to patients aged 18 years or younger (1·80, 1·06–3·07). All investigated variants, except Arg160Trp, tended, to varying degrees, to have higher frequencies in young patients than in adult patients, with significantly higher frequencies found for Val60Leu (OR 1·60, 95% CI 1·05–2·44; p=0·04) and Asp294His (2·15, 1·05–4·40; p=0·04). Compared with those of healthy controls, young patients with melanoma had significantly higher frequencies of any MC1R variants. Interpretation: Our pooled analysis of MC1R genetic data of young patients with melanoma showed that MC1R r variants were more prevalent in childhood and adolescent melanoma than in adult melanoma, especially in patients aged 18 years or younger. Our findings support the role of MC1R in childhood and adolescent melanoma susceptibility, with a potential clinical relevance for developing early melanoma detection and preventive strategies. This replication meta-analysis explored the robustness of a highly cited study showing that those with low self-esteem perceived benefits for self-disclosure through Facebook compared to face-to-face interactions (i.e., Forest & Wood, 2012, Study 1). Seven pre-registered direct replication attempts of this study were conducted by research teams as part of the Collaborative Replication and Education Project (CREP), and results were meta-analyzed to better understand the strength and consistency of the effects reported in the original study. Half of the original results were clearly supported: Self-esteem negatively predicted perceived safety of self-disclosure on Facebook as compared to face-to-face interactions (meta-analytic effect size = -.28, original effect size = -.31), and self-esteem did not relate to perceived opportunities for self-expression (across the seven replications, all 95% confidence intervals (CIs) for effect sizes included zero). However, two other findings received less support: Self-esteem only weakly and inconsistently predicted perceived advantages of self-disclosure on Facebook (meta analytic effect size = -.16, original effect size = -.30), and contrary to the original study, there was no evidence for self-esteem predicting perceived opportunities for connection with others on Facebook (six of the seven replication effect size CIs contained zero). The results provided further evidence regarding the original study’s generalizability and robustness. The implications of the research and its relevance to social compensation theory is presented, and considerations for future multi-site replications are proposed.

We tested psychometric properties of the Markers of Adulthood (MoA) importance scale and a revised Inventory of the Dimensions of Emerging Adulthood (IDEA-8) in a large, diverse multisite and multinational sample the Emerging Adulthood Measured at Multiple Institutions 2 project. We used multilevel confirmatory factor analyses and multilevel alphas to examine external validity and internal consistency of the scales. We also performed correlational and exploratory multilevel analyses to determine the extent to which emerging adulthood dimensions overlap across scales. The IDEA-8 subscales demonstrated acceptable psychometric properties. Our research suggests that recent approaches to combine the MoA markers provide four modestly reliable factors, but perceptions of adulthood varied considerably as a function of sample. We recommend that the structure of these marker items be examined for any given sample, since their relative importance seems to vary, not just across time but also sample location.

Non-Hodgkin's lymphoma (NHL) represents 4% of newly diagnosed cancer in 2013 with a 59-82% 5-year survival depending on the tumor location. Primary presentation of lymphoma consists of lymphadenopathy or swelling of the lymph nodes and non-specific systemic symptoms such as fevers, night sweats, and weight loss. Less commonly, NHL arises from non-lymphoid tissue. We report a unique case of NHL arising from the ureteral wall which was visualized via non-contrast CT and direct vision through ureteroscopy.

The randomized first-line trials, including the CRYSTAL trial, the OPUS trial, and the PRIME trial, have demonstrated the significant efficacy of cetuximab or panitumumab in patients with v-Ki-ras2 Kirsten rat sarcoma viral oncogene homolog (KRAS) wild-type tumors. The addition of an antiepidermal growth factor receptor (anti-EGFR)-directed monoclonal antibody to chemotherapy for these patients significantly improved progression-free survival, response rates, and R0 resection rates to a greater extent than overall survival compared with patients who received chemotherapy alone. However, 2 recent randomized phase 3 trials, the MRC COIN trial and the Nordic VII trial, reported an unexpected lack of benefit from the addition of cetuximab to chemotherapy in the first-line setting. In addition, recent retrospective analyses performed on a pooled data set from major clinical trials added more complexity, reporting an unexpected association of KRAS G13D mutation with a better clinical outcome compared with patients who had other KRAS mutations in the first-line and salvage settings, whereas the other independent analysis failed to demonstrate a benefit from panitumumab in patients with the same KRAS G13D mutation. The anti-EGFR monoclonal antibody-associated skin toxicity and the controversial strategies of management also are discussed. In this review, the authors analyze the previous randomized clinical trials and more critically re-evaluate recent trials and subgroup analyses to derive 3 factors that need to be taken into consideration regarding the addition of EGFR-directed monoclonal antibodies to chemotherapy: the preclinical data on mechanisms of action between chemotherapy and anti-EGFR antibodies along with mechanisms of resistance to anti-EGFR antibodies, the role of cross-over events in overall survival data, and the significant dose reductions of chemotherapeutic agents when combined with anti-EGFR agents. The National Cancer Institute (NCI) Community Cancer Centers Program (NCCCP) formed an Early-Phase Working Group to facilitate site participation in early-phase (EP) trials. The Working Group conducted a baseline assessment (BA) to describe the sites' EP trial infrastructure and its association with accrual.EP accrual and infrastructure data for the sites were obtained for July 2010-June 2011 and 2010, respectively. Sites with EP accrual rates at or above the median were considered high-accruing sites. Analyses were performed to identify site characteristics associated with higher accrual onto EP trials.Twenty-seven of the 30 NCCCP sites participated. The median number of EP trials open per site over the course of July 2010-June 2011 was 19. Median EP accrual per site was 14 patients in 1 year. Approximately half of the EP trials were Cooperative Group; most were phase II. Except for having a higher number of EP trials open (P = .04), high-accruing sites (n = 14) did not differ significantly from low-accruing sites (n = 13) in terms of any single site characteristic. High-accruing sites did have shorter institutional review board (IRB) turnaround time by 20 days, and were almost three times as likely to be a lead Community Clinical Oncology Program site (small sample size may have prevented statistical significance). Most sites had at least basic EP trial infrastructure.Community cancer centers are capable of conducting EP trials. Infrastructure and collaborations are critical components of success. This assessment provides useful information for implementing EP trials in the community.

Chemotherapy-induced neurotoxicity is a significant source of morbidity for cancer patients. This study aimed to assess the relationship between preexisting diabetes and clinically significant (National Cancer Institute Common Toxicity Criteria grades 2 and 3) OXIPN; between diabetes, and the cumulative dose at onset of OXIPN; and between other preexisting medical conditions and the development of OXIPN.We reviewed medical records of all patients with stage II-IV colon cancer treated in the Albert Einstein Cancer Center, Philadelphia, with oxaliplatin from 2005 to 2009. Exclusion criteria included preexisting neuropathy, previous neurotoxic chemotherapy exposure, and incomplete medical records. The NCI Common Toxicity Criteria was used to grade sensory neuropathy. Univariate analysis was used to estimate odds ratios and confidence limits for prevalence of OXIPN in patients with and without diabetes. The mean level and cumulative doses were compared using the t test.Sixty-two patients met the study criteria; 23 oxaliplatin-treated patients were excluded. The crude incidence of any OXIPN was 65%. There was no relationship found between development of OXIPN and the presence of diabetes, smoking, hypertension, or statin use. However, the mean cumulative dose of oxaliplatin was significantly lower for patients with diabetes who developed neuropathy, compared with those without diabetes (388 vs. 610 mg/m(2); P = .021).Although the presence of diabetes did not appear to affect the severity of OXIPN, patients with diabetes developed OXIPN at a lower cumulative dose of oxaliplatin (P.05). The results may have implications for treatment of patients with diabetes and colon cancer.

Although anemia is a well-described complication after Roux-en-Y gastric bypass (RYGB) in association with iron deficiency, no studies have been published regarding changes in the white blood cell count.Mixed longitudinal models were used to follow the changes in white blood cell count, platelet count, and hematocrit over time after RYGB.A total of 590 patients, who had undergone RYGB from 2006 to 2010, inclusively, had laboratory studies available. The mean follow-up was 398 days (range 30-1484). The incidence of leukopenia (white blood cell count ≤4000 cm(3)) increased significantly from 2.0% (12 of 590) before surgery to 14.6% (86 of 590) afterward (P.0005). A lower white blood cell count was independently predicted by greater weight loss, longer time after surgery, a lower hematocrit, and a lower platelet count (P.0005). No patient developed neutropenia. The incidence of pre-existing anemia was ∼17% for both men and women. After surgery, the incidence of anemia substantially increased only in premenopausal women (from 16% to 33%). Anemia occurred independently of the degree of weight loss. The platelet counts decreased by a clinically insubstantial, although statistically significant, amount (281,000-250,000; P.0005).RYGB is associated with a generalized decrease in the white blood cell and platelet counts. These decreases do not seem clinically important, unlike the substantial decrease in red blood cell mass in premenopausal women. A generalized suppression of hematopoiesis might occur after RYGB.

Psychiatry has its place in the ranks of social disciplines and the goal is scientific humanism. Failure leaves two archaic alternatives: either to fall apart and become a decadent civilization, or to concentrate on some focal point of fanaticism, radical or religious, rising on fabricated emotions with artificial wings like Icarus until they melt and then come tumbling down, leaving science like Daedalus to mourn.At the present time there is a flow of stimulating ideas between anthropology and psychiatry. Each of these two disciplines is growing more and more interested in the material of the other. Anthropology offers psychiatry a wide range of data valuable for comparison and control, much as comparative anatomy offers material to human anatomy. Colorectal cancer is the fourth most common malignancy in the United States. Its single most important prognostic factor is lymph node involvement. Multiple guidelines recommend sampling a minimum of 12 nodes, to ensure accurate staging and treatment. However, this standard of care is not always achieved. The objective of this study was to identify potential modifiable factors that may explain this inadequacy between the optimal approach and routine practice.The medical charts of all patients treated for colorectal cancer stages I-III at the Albert Einstein Medical Center from 1999-2007 were reviewed. Associations between multiple surgical and pathologic variables and the presence ofor= 12 lymph nodes in the final pathology report were examined.In total, 337 patients were included in this study. The mean number of nodes reported was 12.7 (standard deviation, +/- 7.6), and 173 patients (51%) hador= 12 lymph nodes. Regarding patients' demographic characteristics, 78% were more than 60 years of age; 47.8% were male; and 27% were white, 67% were African American, and 6% were of other ethnic groups. Using a univariate analysis, several variables were statistically associated with the/= 12 lymph nodes reported: colon length (Pearson r = 0.384; P.001); thickness of the mesocolon (Pearson r = 0.294; P.001); size of tumor (Pearson r = 0.154; P = .005); site of tumor (right vs. left, P.001); type of surgery (right or subtotal colectomy vs. others, P.001), experience of pathologist (P = .02); pathologist's assistant (P = .006); and experience of surgeon (P.001). Using a multivariate logistic regression analysis, adjusting for age, sex, and race, colon length (P = .001), type of surgery (odds ratio [OR], 3.37; 95% confidence interval [CI], 2.0-5.6; P.001), surgeon's experience (OR, 2.33; 95% CI, 1.4-3.9; P = .001), pathologist's experience (OR, 1.9; 95% CI, 1.1-3.2; P = .01), and role of the pathologist's assistant (OR, 2.5; 95% CI, 1.4-4.3; P = .001) remained as significant predictors.This study showed that multiple factors influence the number of lymph nodes sampled. The roles of the surgeon, the pathologist, and especially the pathologist's assistant comprise significant variables that could be modified with appropriate education.

Aims and objectives This study investigates how long-term colon cancer survivors evaluate their health, functional status and quality of life, and whether there are differences based on age, gender or ethnicity. Methods Thirty long-term survivors of at least stage I colon cancer were interviewed in person between December 2004 and May 2005. The interview protocol included the Medical Outcomes Study 36-Item Short Form, Quality of Life ‐ Cancer Survivor, and study-specific questions that asked about physical and non-physical problems they attributed to colon cancer. Results Substantial percentages of survivors attributed their problems with lack of energy (83%), sexual functioning (67%), bowel problems (63%), poor body image (47%) and emotional problems (40%) to having had colon cancer. Of those problems attributed to colon cancer, sexual functioning and pain were given the highest severity rankings by survivors. The majority of long-term colon cancer survivors reported distress regarding future diagnostic tests, a second cancer, and spread of cancer. Women reported greater problems completing daily activities as a result of physical problems ( P = 0.003) and more pain ( P = 0.07) than men. African Americans appear to report marginally better overall quality of life ( P = 0.07) and psychological well-being than whites ( P = 0.07). Conclusion The majority of long-term colon cancer survivors with resected colon cancer and disease-free for 5 years reported problems with low energy, sexual functioning and bowel problems.

Although it may appear, when examining the deciduous dentition, that similar malocclusions to those seen in the permanent dentition can be identified, it does not always follow that they will persist. Incipient crowding of the teeth is usually masked by spacing which should normally be present. Small discrepancies of occlusion can arise or be masked as a result of deviation of the mandible away from centric occlusion. Unilateral crossbites, anterior open bites and excessive overjets which are associated with a sucking habit, will usually prove transient and improve spontaneously. It is also possible that crossbites and buccal occlusion of individual premolars may develop as these teeth erupt; these are often caused by displacement due to crowding. If these factors are allowed for then a fairly accurate prediction of arch relationship can be made. Correlations of tooth size, arch size and spacing of teeth were not strong enough to be used individually for predicting the degree of crowding to be expected in the permanent dentition. However, observation of all these factors, especially if radiographs are available may be used to give a fairly accurate prognosis.

Life-sustaining treatments such as cardiopulmonary resuscitation, mechanical ventilation, vasopressors, and admission to critical care units, if used when recovery chance was remote, may unnecessarily cause discomfort and increase cost of care. Outcomes of these treatments in chronic, refractory congestive heart failure (CHF) and metastatic cancer patients were poor. Although both conditions were the leading causes of death, previous studies indicated that hospice utilization and do-not-resuscitate orders were less common in CHF patients. To date, the use of life-sustaining treatments in these patients and the influence of do-not-resuscitate orders remains unknown.We conducted a retrospective medical record review of the patients who died in our hospital in 1999 and had discharge diagnoses of CHF or cancer. Medical records were screened for seriously ill patients according to the modified SUPPORT criteria, which included patients with CHF functional class IV or ejection fraction of 20% or less at baseline and with metastatic cancer not receiving any curative treatments. Analyses were performed using SPSS, version 9.0.There were 58 and 82 patients in CHF and cancer groups, respectively. CHF patients were older (78.8 vs. 67.3 yrs, p.001) and stayed in the hospital longer (11.9 vs. 7.9 days, p = .014). The majority of patients in both groups received do-not-resuscitate orders before death (84% and 72%, respectively). CHF patients received do-not-resuscitate orders later than did cancer patients (6.7 vs. 2.8 days, p = .006). However, there was no significant difference in prevalence of do-not-resuscitate orders. All studied life-sustaining treatments were more common in CHF patients than in cancer patients. A subgroup analysis between CHF patients with do-not-resuscitate orders and those without do-not-resuscitate orders revealed cardiopulmonary resuscitation to be the only treatment less common in those with do-not-resuscitate orders.Patients who died of chronic, refractory CHF received more life-sustaining treatments than did patients who died of metastatic cancer.

Integrative complexity reflects the degree to which the source of a communication perceives several dimensions and points of view relevant to the topic (differentiation) and the degree to which such characteristics are seen as related to each other (integration). During international crises, bilateral decreases in the integrative complexity of communications frequently precede the outbreak of war; a unilateral decrease reliably precedes surprise strategic attacks. In the current study, complexity was scored in the messages of selected leaders from before to approximately a month after the terrorist attacks of 11 September 2001. Even this limited database replicated some of the complexity patterns found previously, as well as showing some novel characteristics. The in vivo efficacy of the echinocandin antifungal caspofungin acetate (caspofungin; MK-0991) was evaluated in models of disseminated aspergillosis and candidiasis in mice with cyclophosphamide (CY)-induced immunosuppression. Caspofungin is a 1,3-β- d -glucan synthesis inhibitor efficacious against a number of clinically relevant fungi including Aspergillus and Candida species. Models of CY-induced transient or chronic leukopenia were used with once daily administration of therapy initiated 24 h after microbial challenge. Caspofungin was effective in treating disseminated aspergillosis in mice that were transiently leukopenic (significant prolongation of survival at doses of ≥0.125 mg/kg of body weight and a 50% protective dose [PD 50 ] of 0.245 mg/kg/day at 28 days after challenge) or chronically leukopenic (50 to 100% survival at doses of ≥0.5 mg/kg and PD 50 s ranging from 0.173 to 0.400 mg/kg/day). Caspofungin was effective in the treatment and sterilization of Candida infections in mice with transient leukopenia with a 99% effective dose based on reduction in log 10 CFU of Candida albicans /gram of kidneys of 0.119 mg/kg and 80 to 100% of the caspofungin-treated mice having sterile kidneys at caspofungin doses from 0.25 to 2.0 mg/kg. In Candida -infected mice with chronic leukopenia, caspofungin was effective at all dose levels tested (0.25 to 1.0 mg/kg), with the log 10 CFU of C. albicans /gram of kidneys of caspofungin-treated mice being significantly lower (>99% reduction) than that of sham-treated mice from day 4 to day 28 after challenge. Also, 70 to 100% of the caspofungin-treated, chronic leukopenic mice had sterile kidneys at caspofungin doses of 0.5 to 1.0 mg/kg from day 8 to 28 after challenge. Sterilization of Candida infections by caspofungin in the absence of host leukocytes provides compelling in vivo evidence for fungicidal activity against C. albicans . Further human clinical trials with caspofungin against serious fungal infections are in progress.

We have measured the temperature dependence of the electrical resistivity of a sample of the dilute magnetic persistent photoconductor Cd1−xMnxTe:In (x=0.17) as a function of persistent photocarrier concentration. We observe a resistivity with the form ρ=ρ 3 exp (E 3 / k B T) over the temperature range investigated (20–4 K), where E3 and ρ3 vary with the carrier density in a manner that suggests a nearest neighbor hopping transport mechanism. Magnetotransport measurements have also been made and we observe an anisotropic negative magnetoresistance (MR) at higher temperatures, which crosses over to a MR that is positive, and essentially isotropic, at low temperatures. We interpret this behavior as a cross over from a mechanism that is sensitive to the length of the nearest neighbor hop (an “orbital” effect) to a situation where the interaction between the localized electron and the magnetic ions of the host becomes important.
