GATA3 gene silencing in activated T cells displays a promising option to early-on undermine pathological pathways in the disease formation of allergic asthma. The central transcription factor of T helper 2 (Th2) cell cytokines IL-4, IL-5, and IL-13 plays a major role in immune and inflammatory cascades underlying asthmatic processes in the airways. Pulmonary delivery of small interfering RNAs (siRNA) to induce GATA3 knockdown within disease related T cells of asthmatic lungs via RNA interference (RNAi) presents an auspicious base to realize this strategy, however, still faces some major hurdles. Main obstacles for successful siRNA delivery in general comprise stability and targeting issues, while in addition the transfection of T cells presents a particularly challenging task itself. In previous studies, we have developed and advanced an eligible siRNA delivery system composed of polyethylenimine (PEI) as polycationic carrier, transferrin (Tf) as targeting ligand and melittin (Mel) as endosomolytic agent. Resulting Tf-Mel-PEI polyplexes exhibited ideal characteristics for targeted siRNA delivery to activated T cells and achieved efficient and sequence-specific gene knockdown in vitro. In this work, the therapeutic potential of this carrier system was evaluated in an optimized cellular model displaying the activated status of asthmatic T cells. Moreover, a suitable siRNA sequence combination was found for effective gene silencing of GATA3. To confirm the translatability of our findings, Tf-Mel-PEI polyplexes were additionally tested ex vivo in activated human precision-cut lung slices (PCLS). Here, the formulation showed a safe profile as well as successful delivery to the lung epithelium with 88% GATA3 silencing in lung explants. These findings support the feasibility of Tf-Mel-PEI as siRNA delivery system for targeted gene knockdown in activated T cells as a potential novel therapy for allergic asthma.

To remove a bent femoral nail is always a challenging task for a surgeon. In the last 20 years several case reports and descriptions of techniques are to be found in literature, the goal being to weaken the nail at the apex and then straightening it in situ in most cases. Those reports all have one parameter in common: the necessity to remove the nail relates to a refracture of the femur.We report the case of a patient who presented with the explicit wish to have a bent femoral nail removed and a femoral angulation corrected because of pain in the ipsilateral hip. A femoral fracture had been treated by a Küntscher nail in 1982. A refracture with severe bending of the nail had occurred a short time later, which was treated conservatively without a second surgical procedure.We can show that a planned removal of a bent and well-osseointegrated Küntscher nail can be achieved without complications and that thereby a multiplane angulation in the femur can be corrected, paving the way for a future arthroplasty in this specific case.Einen verbogenen Femurnagel zu entfernen, ist eine anspruchsvolle Aufgabe. Es finden sich mehrere Kasuistiken in der Literatur, meist mit dem Ziel, den Nagel zu schwächen und dann in situ zu begradigen.Im Folgenden wird über einen Patienten berichtet, der aufgrund einer multiplanaren Femurdeformität mit einliegendem Marknagel Hüftschmerzen entwickelt hatte. Eine Femurfraktur war 1982 mittels Marknagel versorgt worden. Eine Refraktur, bei der sich der Nagel verbog, wurde konservativ behandelt.Wir können zeigen, dass die Entfernung eines so verbogenen Marknagels komplikationslos durchgeführt werden kann.

Background: Studies have shown that multidisciplinary team meetings (MDTM) improve diagnostic work-up and treatment-decisions. This study aims to evaluate the influence of implementing a regional-video-Upper-GI-MDTM (uMDTM) for oesophageal cancer (OC) on the number of patients discussed, treatment-decisions, perspectives of involved clinicians and overall survival (OS) in the Eindhoven Upper-GI Network consisting of 1 resection hospital and 5 referring hospitals. Methods: Between 2012 and 2018, patients diagnosed with OC within this region, were selected from the Netherlands Cancer Registry(n = 1119). From 2014, an uMDTM was gradually implemented and a mixed-method quantitative and qualitative design was used to analyse changes. Quantitative outcomes were described before and after implementation of the uMDTM. Clinicians were interviewed to assess their perspectives regarding the uMDTM. Results: After participation in the uMDTM more patients were discussed in an MDTM (80%–89%,p < 0.0001) and involvement of a resection centre during the uMDTM increased (43%–82%,p < 0.0001). The proportion of patients diagnosed with potentially curable OC (cT1-4a-x, any cN, cM0) remained stable (59%–61%, p = 0.452). Endoscopic or surgical resections were performed more often (28%–34%,p = 0.034) and the use of best supportive care decreased (21%–15%,p = 0.018). In the qualitative part an improved knowledge, collaboration and discussion was perceived due to implementation of the uMDTM. Three-year OS for all OC patients increased after the implementation of the uMDTM (24%–30%,p = 0.025). Conclusions: Implementation of a regional Upper-GI MDTM was associated with an increase in patients discussed with a resection centre, more curative resections and a better OS. It remains to be elucidated which factors in the clinical pathway explain this observed improved survival.

Berichtet wird ein Absturzunfall aus 12 m Hohe in einer Onshore-Windenergieanlage. Bei Schadelhirn-, Thorax- und Wirbelsaulentrauma sowie multiplen Extremitatenfrakturen uberlebte der Patient aufgrund der adaquaten Erstversorgung, des zeitnahen Eintreffens professioneller medizinischer Versorgung und des unverzuglichen Transports in eine geeignete Klinik mit anschliesender Therapie. Die Kinematik der Fraktursequenz und Weichteilverletzungen absorbierte zudem die Aufprallenergie uberlebensrelevant. Bei einem analogen Unfall in einer Offshore-Windenergieanlage ware aufgrund verlangerter Rettungszeiten ein schlechteres Patienten-Outcome als Onshore wahrscheinlich.

Im Rahmen der Energiewende werden in der deutschen Nord- und Ostsee Offshore-Windparks (OWP) geplant, errichtet und in Betrieb genommen. Die damit beschaftigten Unternehmen werden durch das maritime Umfeld auch im Hinblick auf Arbeitsschutz- und Notfallkonzepte vor neue Herausforderungen gestellt. Mehr als 1000 Menschen werden nach bisherigen Schatzungen kunftig permanent in den Offshore-Windparks tatig sein und mit den spezifischen Arbeits- und Rahmenbedingungen umgehen mussen, insbesondere mit der zum Teil grosen Entfernung von der Kuste. Ein professionelles Rettungsteam kann aufgrund der Entfernung und je nach Wetterbedingungen haufig erst nach einer langeren Zeit eintreffen. Diese Zeitspanne gilt es durch Erste-Hilfe-Masnahmen zu uberbrucken, fur deren Sicherstellung der Unternehmer verantwortlich ist. Insbesondere vor dem Hintergrund des verlangerten therapiefreien Intervalls bekommen die Masnahmen der Ersten Hilfe durch Laien als erstes Glied der Rettungskette eine herausragende Bedeutung. Die Ergebnisse dieser Arbeit beschreiben entsprechend das Konzept des „Ersthelfer-Offshore“ und die Notwendigkeit erweiterter Erste-Hilfe-Masnahmen wie z. B. die Anlage eines Tourniquet bei kritischer Extremitatenblutung mit Unterstutzung durch Telekonsultation.

Fragestellung: Die Auswirkungen einer ketogenen Diat auf das Wachstum solider Tumoren werden aktuell kontrovers diskutiert. Aus diesem Grund wurde in vitro untersucht, bei welchen Sauerstoffkonzentrationen Tumorzellen Ketonkorper (β-Hydroxybutyrat) und die beiden Metabolite Glukose und Laktat, die auch bei einer ketogenen Diat vorkommen, verstoffwechseln. Zudem wurde exemplarisch das Tumorwachstum unter einer standardisierten ketogenen Diat im Tiermodell uberpruft. Material und Methodik: Konfluente Kulturen von 6 humanen gastrointestinalen Tumorzelllinien wurden in vitro bei 21 % und 1 % Sauerstoff untersucht. Glukose, Laktat und β-Hydroxybutyrat wurden mit Standardenzymassays bestimmt. Mit einer standardisierten ketogenen Diat wurde in Kleinnagern eine physiologische Ketose ausgelost und durch subkutane Injektion von Tumorzellen das Wachstum solider Tumoren induziert. Ergebnisse: Kulturen der 6 Tumorzelllinien verstoffwechseln Ketonkorper und Laktat bei 21 %, nicht aber bei 1 % Sauerstoff; dagegen wird Glukose bei beiden Sauerstoffkonzentrationen metabolisiert. Stehen Tumorzellen bei 21 % Sauerstoff die beiden Energietrager Glukose und Laktat gleichzeitig zur Verfugung, wird zuerst Glukose und anschliesend Laktat verstoffwechselt. Unter einer ketogenen Diat entwickeln Mause eine physiologische Ketose mit signifikant erhohten Ketonkorperspiegeln. Die Tumoren dieser Mause weisen verringerte intratumorale Laktatspiegel und ein verzogertes Wachstum auf. Schlussfolgerung: Die In-vitro-Daten dieser experimentellen Arbeit unterstutzen das Konzept, dass in vivo der Stoffwechsel solider Tumoren durch die lokale Sauerstoffversorgung wesentlich beeinflusst wird. Sauerstoffkonzentrationen von 1 %, die regelhaft in soliden Tumoren vorzufinden sind (Tumorhypoxie), reichen in vitro nicht aus, damit gastrointestinale Tumorzellen Ketonkorper und Laktat verstoffwechseln; im Gegensatz zur Glukose. Um Tumorzellen in Hypoxie zum Absterben zu bringen, sollte bei ihnen daher der Glukosestoffwechsel selektiv gehemmt und zusatzlich das Glukoseangebot durch diatetische Masnahmen eingeschrankt werden.

Hepatitis B virus (HBV) is an infectious, potentially lethal human pathogen. However, there are no effective therapies for chronic HBV infections. Antiviral development is hampered by the lack of high-resolution structures for essential HBV protein-protein interactions. The interaction between preS1, an HBV surface-protein domain, and its human binding partner, γ2-adaptin, subverts the membrane-trafficking apparatus to mediate virion export. This interaction is a putative drug target. We report here atomic-resolution descriptions of the binding thermodynamics and structural biology of the interaction between preS1 and the EAR domain of γ2-adaptin. NMR, protein engineering, X-ray crystallography and MS showed that preS1 contains multiple γ2-EAR-binding motifs that mimic the membrane-trafficking motifs (and binding modes) of host proteins. These motifs localize together to a relatively rigid, functionally important region of preS1, an intrinsically disordered protein. The preS1-γ2-EAR interaction was relatively weak and efficiently outcompeted by a synthetic peptide. Our data provide the structural road map for developing peptidomimetic antivirals targeting the γ2-EAR-preS1 interaction.

Ereignisse mit einer Freisetzung von gefahrlichen Stoffen oder Erregern, zusammengefasst zur Gruppe der CBRN-Lagen, sind jederzeit, uberall und in jedem Schweregrad moglich. Entsprechend mussen sich alle Krankenhauser auf die Aufnahme von nicht oder nicht ausreichend dekontaminierten Patienten beim Massenanfall von CBRN-Patienten vorbereiten. Die Moglichkeiten der Krankenhauser zur Bereitstellung von Material uber den finanzierten Versorgungsauftrag hinaus und die beschrankten Zeitressourcen der Mitarbeiter fuhren zu den Fragen, ob vorhandene Einrichtungen eines Krankenhauses genutzt werden konnen oder durch spezifisches Material erganzt werden mussen, um eine Dekontamination beim Massenanfall von CBRN-Patienten durchzufuhren. Es wurde in der Rettungswagenanfahrt des Krankenhauses eine neu konzipierte Dekontaminationsstelle ausschlieslich mit klinikublichem Material durch 9 nicht spezifisch geschulte Mitarbeiter verschiedener, auch nichtmedizinischer Abteilungen aufgebaut und betrieben. 26 Freiwillige wurden mit einem nicht direkt sichtbaren (Fluoreszin) sowie einem sichtbaren Kontaminationssimulationsstoff (Mehl) standardisiert beaufschlagt. Die Dekontamination der gehfahigen und liegenden Patienten erfolgte mit Wasser, einer klinikublichen Waschsubstanz und Haushaltsschwammen. Alle Patienten wurden bei Ankunft registriert. Die Registriernummer wurde uber ein Patientenidentifikationsarmband, eine laminierte Umhangekarte und Faserschreiberbeschriftung am Handrucken angebracht. Nach der Dekontamination wurde jeder Proband unter Normal- und Schwarzlicht auf Restkontaminationen untersucht. Befragungen der Probanden und Mitarbeiter ermoglichten die Beurteilung der Ablaufe und Belastungen an der Einsatzstelle. Der Aufbau der Dekontaminationsstelle war unproblematisch und binnen 18 min abgeschlossen. Fur die Dekontamination der 26 Probanden wurden 90 min benotigt. Lediglich bei 16 Probanden (61,5%) konnte nach Durchlaufen der Dekontaminationsstrecke eine vollstandige Entfernung der sichtbaren Kontamination attestiert werden. Bei dem unter normalem Licht nicht sichtbaren Stoff gelang es nur bei sieben Probanden (26,9%), diesen im erwunschten Umfang zu entfernen. Die Durchlaufkapazitat der Einheit war geringer als gemas Standardformel errechnet. Als Engpass erwies sich die Dekon-Ankunftstelle, nicht die Anzahl der Duscheinheiten. Die korperliche Belastung der Mitarbeiter in den Geblaseatemschutzanzugen war unkritisch, allerdings wurde die eingeschrankte Kommunikationsfahigkeit aus dem Schutzanzug heraus als belastend eingestuft. Nur die Umhangekarten erlaubten eine sachgerechte Dekontamination und eine sichere Identifikation wahrend und nach der Dekontamination. Die Praxiserprobung zeigt, dass eine Dekontaminationsstelle mit klinikublichem Material durch 9 nicht geschulte Mitarbeiter zeitgerecht aufzubauen und zu betreiben ist. Die Durchfuhrung der Dekontamination erfordert spezifische Kenntnisse und praktische Erfahrung, sodass regelmasige Schulungen und Ubungen zwingend sind. Zur Beurteilung der Durchlaufkapazitat muss der einrichtungsindividuelle Engpassfaktor ermittelt und als Grundlage der Berechnung genutzt werden. Eine sichere Registrierung ist nur mit Umhangekarten gegeben.

Offshore windfarms are constructed in the German North and Baltic Seas. The off-coast remoteness of the windfarms, particular environmental conditions, limitations in offshore structure access, working in heights and depths, and the vast extent of the offshore windfarms cause significant challenges for offshore rescue. Emergency response systems comparable to onshore procedures are not fully established yet. Further, rescue from offshore windfarms is not part of the duty of the German Maritime Search and Rescue Organization or SAR-Services due to statute and mandate reasons. Scientific recommendations or guidelines for rescue from offshore windfarms are not available yet. The present article reflects the current state of medical care and rescue from German offshore windfarms and related questions. The extended therapy-free interval until arrival of the rescue helicopter requires advanced first-aid measures as well as improved first-aider qualification. Rescue helicopters need to be equipped with a winch system in order to dispose rescue personnel on the wind turbines, and to hoist-up patients. For redundancy reasons and for conducting rendezvous procedures, adequate sea-bound rescue units need to be provided. In the light of experiences from the offshore oil and gas industry and first offshore wind analyses, the availability of professional medical personnel in offshore windfarms seems advisible. Operational air medical rescue services and specific offshore emergency reaction teams have established a powerful rescue chain. Besides the present development of medical standards, more studies are necessary in order to place the rescue chain on a long-term, evidence-based groundwork. A central medical offshore registry may help to make a significant contribution at this point.

Polycomb group proteins are repressive chromatin modifiers with essential roles in metazoan development, cellular differentiation and cell fate maintenance. How Polycomb proteins access active chromatin to confer transcriptional silencing during lineage transitions remains unclear. Here we show that the Polycomb repressive complex 2 (PRC2) component PHF19 binds trimethylated histone H3 Lys36 (H3K36me3), a mark of active chromatin, via its Tudor domain. PHF19 associates with the H3K36me3 demethylase NO66, and it is required to recruit the PRC2 complex and NO66 to stem cell genes during differentiation, leading to PRC2-mediated trimethylation of histone H3 Lys27 (H3K27), loss of H3K36me3 and transcriptional silencing. We propose a model whereby PHF19 functions during mouse embryonic stem cell differentiation to transiently bind the H3K36me3 mark via its Tudor domain, forming essential contact points that allow recruitment of PRC2 and H3K36me3 demethylase activity to active gene loci during their transition to a Polycomb-repressed state.

Despite the benefit of safety vests to the reduction of torso injuries in children and adolescents is unclear, its’ use is recommended. The aim of the present study is to determine the effectiveness of safety vests actually used in pediatric equestrian activities. In this case-control-study, we analyzed the accidents of 92 riders aged 18 or younger who fell off a horse onto his/her torso during a period of 18 months. Data were gathered from the clinical records. Additionally, a questionnaire was administered on the day of trauma by the patients and/or their parents. The cases comprised 31 patients who sustained torso injuries. The controls were 61 riders with injuries of other body parts than to the torso. Safety vest use was not associated with a lower risk of torso injuries (OR=1.18, 95% CI (0.50, 2.81), p=0.707). Post hoc power analysis revealed that within such a setting an odds ratio of 0.266 could be found with a power of 80%. This study is not able to show an association between wearing a torso protector and protection from torso injuries, probably due to confounding. We did not detect a high effect of safety vest usage in our study population. Whether the development of a new generation of safety vests might be more effective remains unclear. An effective vest should be adapted to the requirements of children and adolescents and should protect the thorax and abdomen, but also the cervical and the lumbar spine.

Die demographische Entwicklung fuhrt dazu, dass bis zum Jahr 2030 eine Zunahme der distalen Radiusfrakturen um bis zu 30% zu verzeichnen sein wird. Es gibt diverse Therapienoptionen, aber was ist neu? Konservative Therapie: Die konservative Therapie ist den stabilen Frakturen vorbehalten. K-Draht-Osteosynthese: Bei alteren Patienten wurden aufgrund der geringen biomechanischen Stabilitat teilweise schlechte funktionelle und radiologische Ergebnisse erzielt. Fixateur externe: Der Fixateur wird bei offenen und komplexen Frakturen eingesetzt und zeigt bessere Ausheilungsergebnisse als die K-Draht-Osteosynthese. Winkelstabile Plattenosteosynthese: Mit den winkelstabilen Platten konnen komplexe Frakturen mit sehr guten Ergebnis stabilisiert werden und sie sind derzeit Mittel der Wahl. Intramedullare Osteosynthese: Nagel zeigen in ersten Studien einen schnelleren Funktionsaufbau als bei der winkelstabilen Platte. Die Versorgung von C3-Frakturen erscheint problematisch. Neu ist die erheblich haufigere Anwendung winkelstabiler, palmarer Plattenosteosynthesen auch bei osteoporotischen distalen Radiusfrakturen, die in Einzelstudien gute Ergebnisse zeigen.

Single-molecule experimental techniques have recently shown to be of significant interest for use in numerous applications in both the research laboratory and industrial settings. Although many single-molecule techniques exist, the nanopore platform is perhaps one of the more popular techniques due to its ability to act as a molecular sensor of biological macromolecules. For example, nanopores offer a unique, new method for probing various properties of proteins and can contribute to elucidating key biophysical information in conjunction with existing techniques. In the present study, various forms of bovine serum albumin (BSA) are detected including thermally refolded BSA, urea-denatured BSA, and multiple forms of BSA detected at elevated electric field strengths (with and without urea). We also provide excluded volume measurements for each of these states that normally are difficult to obtain due to unknown and unstable protein conformations.

Protein domains usually fold without or with only transiently populated intermediates, possibly to avoid misfolding, which could result in amyloidogenic disease. Whether observed intermediates are productive and obligatory species on the folding reaction pathway or dispensable by-products is a matter of debate. Here, we solved the crystal structure of a small protein domain, SAP97 PDZ2 I342W C378A, and determined its folding pathway. The presence of a folding intermediate was demonstrated both by single and double-mixing kinetic experiments using urea-induced (un)folding as well as ligand-induced folding. This protein domain was found to fold via a triangular scheme, where the folding intermediate could be either on- or off-pathway, depending on the experimental conditions. Furthermore, we found that the intermediate was present at equilibrium, which is rarely seen in folding reactions of small protein domains. The folding mechanism observed here illustrates the roughness and plasticity of the protein folding energy landscape, where several routes may be employed to reach the native state. The results also reconcile the folding mechanisms of topological variants within the PDZ domain family.

Pseudarthrosen sind Frakturheilungsstorungen nach konservativer oder operativer Frakturbehandlung, die langer als 6 Monate bestehen. Entscheidend fur die zielgerichtete Therapie ist die Einteilung in aseptische und septische bzw. atrophe, oligotrophe oder hypertrophe Pseudarthrosen. Die operativen Prinzipien sind Vitalisierung atropher Knochenfragmente, stabilisierende Reosteosynthese bei Instabilitat und die Uberfuhrung einer septischen in eine beruhigte oder aseptische Pseudarthrose.

Aufgrund der im hinteren Mediastinum geschutzten pravertebralen Lage des Ductus thoracicus gilt die Inzidenz des traumatischen Chylothorax als Begleitverletzung beim stumpfen Thoraxtrauma als gering. Die enge topographische Beziehung des Ductus zur BWS erklart jedoch andererseits die Koinzidenz von traumatischer Querschnittlahmung und traumatischem Chylothorax, wie sie von uns bei Unfallen mit groser Gewalteinwirkung auf die Wirbelsaule beobachtet wurde. Bei primar im Vordergrund stehender Diagnostik und Therapie der Querschnittsymptomatik oder anderer vital bedrohlicher Verletzungen wird der Chylothorax selbst haufig erst verzogert erkannt. Wir berichten deshalb anhand von 2 Fallen uber die diagnostischen Fallstricke und die aktuellen konservativen und operativen Behandlungsoptionen beim traumatischen Chylothorax als Begleitverletzung der thorakalen Querschnittlahmung.

Hintergrund. Anhand der Erstversorgung von Notfallpatienten durch ehrenamtliche First Responder wurde gepruft, unter welchen Umstanden ihr Einsatz sinnvoll ist. Material und Methoden. Ausgewertet wurden Protokolle von 200 First-Responder-Einsatzen in 2 Kleinstadten in Bezug auf den durchschnittlichen Zeitvorteil, die erbrachten medizinischen Masnahmen und deren Abhangigkeit von Zeitvorteil und M-NACA-Wert sowie die organisatorischen Hilfeleistungen. Ergebnisse. Bei einem durchschnittlichen Zeitvorteil von 7 min vor dem Rettungsdienst wurden einfache diagnostische und therapeutische Masnahmen in 10–100% aller Einsatze unabhangig von den Zeitvorteilen durchgefuhrt. Ein signifikanter Unterschied fand sich fur die Abhangigkeit der Masnahmen vom M-NACA mit Praferenz bei M-NACA IV und V. Organisatorische Unterstutzung wurde in 3–64% aller Einsatze erbracht. Schlussfolgerung. Bereits bei nur geringen Zeitvorteilen ist die Alarmierung eines Ersthelfersystems zur Verkurzung des therapiefreien Intervalls sinnvoll. Bei leichteren Verletzungen/Erkrankungen kann der Einsatz von First Respondern nicht in jedem Fall gefordert werden.

Die Ruptur des vorderen Kreuzbands zahlt zu den haufigsten Bandverletzungen des Kniegelenks. Ziel der Ersatzplastik ist eine moglichst genaue Wiederherstellung der Stabilitat bei physiologischem Roll-Gleit-Mechanismus. Die Indikation zur Kreuzbandersatzplastik wird zunehmend weiter gefasst. Als Standardtransplantate haben sich die 3 autologen Sehnen der Patella-, des Pes anserinus und der Quadrizepssehne durchgesetzt. Die Auswahl des jeweiligen Transplantats richtet sich nach den Anforderungen der Patienten. Trotz technischer Weiterentwicklungen (bei der Platzierung und Fixation der Transplantate) besteht ein Risiko von technischen Problemen sowie intraoperativen Komplikationen von bis zu 20%. Neben den Schwierigkeiten bezuglich der Transplantathebung sind die Fixation mit Scheibenwischereffekt sowie die Bohrkanalfehllage Hauptursachen der Revisionsoperationen.

Distale Humerusfrakturen stellen sowohl bei Kindern als auch bei Erwachsenen Problemfrakturen dar. Wahrend sie bei Ersteren sehr haufig sind und v. a. suprakondylar extraartikular auftreten, handelt es sich bei Erwachsenen um seltene, uberwiegend intraartikulare Frakturen. Gute Ergebnisse sind sowohl bei Kindern als auch bei Erwachsenen nur unter Beachtung der komplexen Anatomie, der Knochenqualitat, der Begleitverletzungen, der Biomechanik der Verletzung und der daraus abzuleitenden Operationstechniken sowie einer gut strukturierten Nachbehandlung zu erzielen. Besondere Beachtung muss den haufigen Begleitverletzungen und den oft auftretenden intra- und postoperativen Komplikationen gewidmet werden. Es wird ein Uberblick uber die Epidemiologie, die aktuellen Diagnose- und Therapiestandards, die moglichen Komplikationen sowie die zu erwartenden Ergebnisse gegeben.

Ellengelenkfrakturen sind relativ selten und machen etwa 7% aller Frakturen aus. Bei Beschrankung auf den proximalen Unterarm betreffen etwa 38% aller Ellengelenkfrakturen das Olekranon und 20–30% das Radiuskopfchen. Bei den ellengelenknahen Unterarmfrakturen hat sich die Einteilung nach AO nicht durchgesetzt. Hier finden v. a. Klassifikationen Anwendungen, die auch eine therapeutische Konsequenz nach sich ziehen. Bei der Einteilung der Olekranonfraktur verwenden wir die Klassifikation nach Schatzker, die den Frakturverlauf und die Anzahl der Knochenfragmente berucksichtigt und somit die Art der Osteosynthese vorgibt. Bei der Radiuskopfchenfraktur hat sich die Klassifikation nach Mason durchgesetzt. Hier werden je nach Typ das konservative Vorgehen, das operative Verfahren oder eine Resektion empfohlen. Die Frakturen des Processus coronoideus werden nach Regan und Morrey eingeteilt. Dabei wird uber die Grose des Fragments indirekt auf die Instabilitat des Gelenks geschlossen und dementsprechend das operative Vorgehen empfohlen. Neben der Monteggia-Verletzung wird als Sonderformen noch auf die Essex-Lopresti-Verletzung und die „terrible triade of the elbow“ eingegangen.

In Zeiten des reichhaltigen Angebots an Nahrung wird aus diesem ursprunglichen Vorteil ein Problem – die Ubergewichtigkeit. Neben der erhohten Morbiditat und Mortalitat ist das gesteigerte Risiko einer Osteoarthrose in der Literatur gut belegt. Unfallfolgen konnen eine sog. Praarthrose darstellen und fuhren in Abhangigkeit des Verletzungsmusters haufig zu einem Gelenkverschleis. Hier stellt die Adipositas einen deutlichen Progressionsfaktor dar.

Uberlegungen und erste Versuche zu winkelstabilen Verbindungen bei Osteosyntheseimplantaten wurden bereits Ende des 19. Jahrhunderts in Hamburg angestellt, eine fruhe Patentanmeldung ist in den 30er Jahren des 20. Jahrhunderts in Paris erfolgt. Seit damals wurde die Osteosynthesetechnik durch Grundlagenforschung der Knochenheilung, Optimierung der Implantate und Schulung der Operationsmethoden zu einem hohen Niveau gefuhrt. Inzwischen ist es moglich, eine verblockte Osteosynthese mit dunnen Platten durchzufuhren, somit ist auch ein Einsatz in Regionen mit schlechter Weichteildeckung moglich. Dies wurde durch die Entwicklung eines Systems erreicht, bei dem die Verblockung durch Umformung erzielt wird: Der Schraubenkopf aus harterem Titanmaterial formt sich in die Lochwand aus weicherem Reintitan ein. Im vorliegenden Beitrag wird auf die Entwicklung dieses Systems eingegangen, dessen Vorteile gegenuber den bisher verwendeten Plattenosteosynthesen werden vorgestellt und die Vor- und Nachteile uni- und multidirektionaler Winkelstabilitat werden aufgezeigt.

INTRODUCTION In a prospective, controlled study, donor site morbidity after bone graft harvesting from the anterior and posterior iliac crest was documented. METHODS In 113 patients, monocortical to tricortical bone grafts were taken from the anterior (n = 73) or dorsal (n = 40) iliac crest. Bone graft size (0.4 - 43 cm 3, median 9.7 cm 3), Operation time (12 - 65 minutes, median 28 minutes), and postoperative donor site were documented. RESULTS Donor site morbidity was higher after harvesting from the ventral than from the dorsal iliac crest: total morbidity 48 vs. 32.5 %, large haematomas 9.6 vs. 7.5 %, moderate haematomas 34.3 vs. 15 %, wound dehiscence 2.7 vs. 0 %. One revision operation was necessary because of a large haematoma at the ventral crest. After harvesting from the ventral iliac crest, there was one fracture ofthe iliac wing and one avulsion fracture of the iliac crest. There were no infections, no injuries of arteries or of the lateral femoral cutaneous nerve and no hemiation. After harvesting from the dorsal iliac crest, there were no major complications. CONCLUSION Bone graft harvesting from the posterior iliac crest should be preferred over harvesting from the anterior iliac crest beeause of the substantially reduced donor site morbidity. Harvesting from the ventral iliac crest should have a clear indication, synthetic bone substitutes should be taken into consideration.

Medizinische Notfalle in Offshore-Windparks stellen alle Beteiligten vor besondere Herausforderungen. Neben arbeitsstattenspezifischen Aspekten wie engen Raumlichkeiten und grosen Hohen, sowie der Weitlaufigkeit der Windparks muss insbesondere die zum Teil erhebliche Entfernung zum Festland berucksichtigt werden, welche letztlich die Eintreffzeit professioneller Hilfe von Land wie auch die Transportzeit in die nachste geeignete Klinik bestimmt. Auch haben die Wetter- und Meeresbedingungen direkten Einfluss auf die Zuganglichkeit zu den Windenergieanlagen und Plattformen sowie auf die Einsatzfahigkeit luft- und wassergebundener Rettungsmittel. Bei Eintreten eines Notfalls kommt erschwerend hinzu, dass nur begrenzte Ressourcen in Bezug auf Personal wie auch Material fur eine effiziente Notfallversorgung und Rettung zur Verfugung stehen. Durch entsprechende Masnahmen sind im Rahmen des Moglichen und der Verhaltnismasigkeit die signifikanten Nachteile dieser Rahmenbedingungen zu minimieren.

Increasing incidences of osseous metastatic malignancies and higher life expectancy in patients are resulting in a raise of occipitocervical metastases. Those patients with infaust prognosis have a significantly reduced quality of life. In Germany, between 800 and 1680 new cases per year are expected. Treatment algorithms include the evaluation of the general condition, the operability of visceral metastases, the tumor localization, the sensitivity to chemo-/radiotherapy, the fracture risk and the extent of neurological deficits and myelopathies.A systematic review on clinical studies or case series in posterior occipitocervical fusions due to metastases to the craniocervical transition yielded nine publications with 48 patients without neurological deficit. The mean survival time in the given follow-up was 6.44 months (n = 26; SD: 5,28; 95 % CI: 4.3-8.57). When measured, the clinical outcome was improved towards the VAS, the DENIS Pain Scale and the quality of life through the activities of daily living (ADL). We searched our clinical database for occipitocervical stabilizations in patients with craniocervical metastases. The prospectively collected data included the preoperative Tokuhashi score, SIN score, neurological status, length of hospitalization, perioperative course/loss of blood/complication rate, as well as the Karnofsky- index and pain measured by VAS preoperatively and in follow-up.Six patients were treated in this consecutive case series. The median age was 72 years (min./max.: 65/82), the average BMI 31.75 (min./max.: 19.3/38.1). The mean preoperative Karnofsky-index was 35 % (min./max.: 23.99/46.01; 95 % CI: 8.39) the mean preoperative Tokuhashi-score 7 (min./max.: 4/10), the mean preoperative VAS7 (min./max.: 4.8/9.2; 95 % CI: 1.68). There were no perioperative complications. In the follow-up, one patient showed a loosening of the screws in the osteolytic massae laterales and one patient suffered from a construct failure after fall.Metastases of the craniocervical transition are rare. The initial treatment of instability includes the application of a rigid Miami-J-collar or a Halo fixator. The decision for an operative procedure must accurately assess the individual patient characteristics to provide him a balanced concept between operational risk and clinical benefit. The assessment should be based on the life expectation and the expected quality of life in dependency of the respective therapeutic concept and its risks. The sole posterior stabilization of craniocervical instability through occipitocervical fusion leads to a reduction of pain, has a low perioperative risk, and may prevent a hospitalization. It is justified for selected patients to receive this treatment to help alleviate pain and to improve their quality of life. From our experience, rare cases of pain without instability should undergo conservative treatment in the first line. Due to the low availability of data on the manifestation and the clinical course of craniocervical metastases, there is a need for the collection of both the descriptive patient data include the radiographic findings as well as the clinical outcome and socio-economic factors using appropriate scoring systems.

Demographic change not only affects patients with ophthalmological diseases but also ophthalmologists.The aim of this article is to evaluate the consequences of an aging society on health care provision. How can these challenges be overcome?Evaluation of publications from the German Federal Statistical Office about the current and projected population, analysis of own studies about health care provision, utilization, and delivery as well as the presentation and discussion of regulatory and organizational conditions.There is a continuous increase in chronic and age-related diseases. At the same time the prevalence of multimorbidity and the number of patients dependent on long-term care is rising, leading to an increase in the demand for ophthalmological care. Regarding health care providers we observed a cutback in qualified ophthalmological personnel, especially for remote areas thus causing difficulties in providing adequate eye care to the population.To deliver health care to a growing number of patients with a decreasing number of medical professionals is the major challenge of demographic change. This will have an enormous impact on ophthalmological health care in terms of maintaining high quality health services covering a nationwide area.

Epidemiological studies continue to receive growing attention in health services research.How can ophthalmological health services research benefit from the Study of Health in Pomerania (SHIP)?The SHIP is a population-based, epidemiological project conducted in the northeast of Germany. In 1997 a random sample of 6,265 eligible subjects was enrolled for the first SHIP cohort. Between 1997 and 2001 baseline examinations were performed, followed by 5-year follow-up from 2002 to 2006, from 2008 to 2012 and from 2014. A second independent sample of 8,016 eligible subjects was enrolled in 2008. Baseline examinations of this new cohort (SHIP-Trend) were conducted between 2008 and 2012 and the first follow-up is scheduled for 2015. Included in SHIP are a large variety of different assessments including eye examinations. All participants are subjected to non-mydriatic funduscopy and static vessel analysis (SVA).A total number of 4,420 subjects participated in the interdisciplinary baseline examinations of SHIP-Trend. Standardized fundus photography was performed with a non-mydriatic camera in 3,934 subjects.Estimations of the prevalence of major eye diseases may provide valid indicators for healthcare provision within healthcare research. New concepts of prevention with a strengthened integration of ophthalmological diagnostics can be identified from the widespread spectrum of different healthcare examinations. Regional differences in the efficiency of healthcare provision can be evaluated with comparative studies between different German populations.

Die Ursachen fur das Versagen einer vorderen Kreuzbandersatzplastik mit Lig. patellae in „Bone-Tendon-Bone-Technik“ sind zumeist Fehlpositionierung der femoralen oder tibialen Insertion oder primar zu geringe Transplantatspannung. Eine Korrektur durch Mobilisation und Nachspannen oder Versetzen des knochernen Ansatzes ist technisch moglich. Problematisch ist aber die Beurteilung der Qualitat des Transplantats, die letztlich fur die Indikationstellung entscheidend ist. Bei 13 Patienten wurden von 1989–1998 Insertionskorrekturen vorgenommen, in 4 Fallen mit gutem, in 5 Fallen mit akzeptablem Ergebnis. Der Vorteil dieses Verfahrens besteht in einer geringeren Morbiditat durch Vermeidung einer neuerlichen Transplantatentnahme. Die Option auf eine Revisionsplastik bleibt im Falle des Versagens erhalten.

Zusammenfassung Die Behandlung der vorderen Kreuzbandruptur war in den vergangenen 30 Jahren erheblichen Wandlungen unterworfen. Nach einer Periode der direkten Naht und extraartikularer Stabilisierungsverfahren folgte wegen der unbefriedigenden Ergebnisse dieser Methoden eine Phase der konservativen Therapie. Die hohe Rate instabilitatsbedingter Meniskusschaden und Arthrosen lies das Pendel aber wieder in Richtung operativer Verfahren umschlagen. Die Langzeitresultate nach alleiniger direkter Naht sind allerdings nicht besser geworden als fruher, auch wenn die Morbiditat heute aufgrund neuer Zugangswege deutlich geringer ist. Dagegen hat sich die Kreuzbandplastik mit autologer Patellarsehne oder Semitendinosussehne mit guten Ergebnissen als Therapiestandard auch fur die Versorgung frischer Bandverletzungen etabliert. Modifikationen des Operationszeitpunkts und der Rehabilitation haben die Komplikationsrate gesenkt und die Resultate noch verbessert. Erkenntnisse der Neuroanatomie und Neurophysiologie des Kreuzbands lassen heute eine veranderte Nahttechnik mit Augmentation durch autologe oder auch resorbierbare Materialien sinnvoll erscheinen. Die Ergebnisse sprechen dafur, das die Aufrechterhaltung der propriozeptiven Funktion nicht nur theoretisch eine dauerhafte Stabilisierung ermoglicht.

Zusammenfassung Im Gegensatz zur Arthroskopie des Knie- oder Schultergelenks ist die Arthroskopie des Ellbogengelenks mit erheblichen technischen Schwierigkeiten und entsprechend hohen Komplikationsrisiken verbunden. Beim Trauma ist die Arthroskopie zudem aufgrund der veranderten anatomischen Verhaltnisse noch weit komplizierter. Die Operationszeit ist wegen der rasch auftretenden periartikularen Schwellung infolge der meist bestehenden Gelenkkapsellasionen deutlich limitiert. Die Indikation zur Arthroskopie sollte daher nur im Einzelfall unter sorgfaltiger Abwagung der Risiken und der zu erwartenden Vorteile gestellt werden. Eine generelle Empfehlung fur dieses Verfahren bei frischen Verletzungen kann nicht ausgesprochen werden. Sinnvoll ist der Einsatz dagegen bei einigen posttraumatischen Veranderungen, z. B. freien Gelenkkorpern nach osteochondralen Frakturen oder Arthrolysen bei intraartikularen Vernarbungen, wahrend bei „unklaren Gelenkbeschwerden“ oder posttraumatischer Arthrose kaum Verbesserungen zu erwarten sind. Wegen des relativ hohen Risikos neurologischer Komplikationen ist eine intensive praoperative Aufklarung der Patienten erforderlich. Eine diagnostische Arthroskopie beim Trauma ist bei der Aussagekraft der modernen nichtinvasiven Untersuchungsverfahren kaum noch angezeigt.

HBc, the capsid-forming "core protein" of human hepatitis B virus (HBV), is a multidomain, α-helical homodimer that aggressively forms human HBV capsids. Structural plasticity has been proposed to be important to the myriad functions HBc mediates during viral replication. Here, we report detailed thermodynamic analyses of the folding of the dimeric HBc protomer under conditions that prevented capsid formation. Central to our success was the use of ion mobility spectrometry-mass spectrometry and microscale thermophoresis, which allowed folding mechanisms to be characterized using just micrograms of protein. HBc folds in a three-state transition with a stable, dimeric, α-helical intermediate. Extensive protein engineering showed thermodynamic linkage between different structural domains. Unusual effects associated with mutating some residues suggest structural strain, arising from frustrated contacts, is present in the native dimer. We found evidence of structural gatekeepers that, when mutated, alleviated native strain and prevented (or significantly attenuated) capsid formation by tuning the population of alternative native conformations. This strain is likely an evolved feature that helps HBc access the different structures associated with its diverse essential functions. The subtle balance between native and strained contacts may provide the means to tune conformational properties of HBc by molecular interactions or mutations, thereby conferring allosteric regulation of structure and function. The ability to trap HBc conformers thermodynamically by mutation, and thereby ablate HBV capsid formation, provides proof of principle for designing antivirals that elicit similar effects.

Anteroposterior X-ray views of both acromioclavicular (AC) joints with 10-kg weights held in each hand are the generally accepted procedure for diagnosis of Tossy I-III grades of AC joint separation. An analogous diagnosis can be made by standardized ultrasound examination. Ten individuals with Tossy-I, 11 with Tossy-II and 8 with Tossy-III instability were examined both radiographically and by B-mode ultrasound. The degree of AC joint separation was uniformly determined on the basis of a calculated index (AC Index = AC joint width of uninjured side/AC joint width of injured side). The mean AC Index for Tossy-I instability determined by ultrasound was 1.0; mean indices of 0.49 and 0.5 were determined for Tossy-II injury by ultrasound and X-ray, respectively, and of 0.21 and 0.2, respectively, for Tossy-III instability. Statistical analysis showed significant differences between the mean AC indices of all three groups (P < 0.0001). We conclude that the reliability of ultrasound examination of AC joint instability is equal to that of radiographic measurement. Standard X-rays of the shoulder remain mandatory only to exclude fracture. The indication for operative stabilization of the AC joint can be established on the basis of the grade of AC joint instability measured by the side-effect-free and cost-effective method of ultrasound examination (AC Index < 0.3 equivalent to Tossy-III instability).

Horse riding is associated with a high rate of injuries. The possibilities of prevention are limited because of deficient knowledge about the causes and mechanisms of equine-related accidents. In the present study 198 equine-related accidents were analyzed and based on these data risk groups were identified and guidelines to improve safety in horse riding were formulated.In this 2-center study the accidents of 169 riders over a period of 12 months were analyzed. Data on equine-related patterns of injury and the resulting treatment were collated from the clinical records. Additionally, a questionnaire was completed on the day of trauma by the patients, which covered age, gender, the cause and mechanism of injury and the use of safety equipment at the time of the accident.There were 169 (85.5%) female and 29 (14.5%) male riders with a median age of 27.2 years (range 5-74 years). Of the riders 55 (27.8%) were aged 18 years or younger, 134 (67.7%) riders were treated as outpatients while 64 (32.3%) required hospitalization, 51 (25.8%) required surgical treatment, 66 (33.3%) used a helmet and 14 (7.1%) a body protector at the time of accident.Equestrians wear helmets increasingly more when riding but the willingness to wear body protectors is disappointing. Protective headgear has been proven to reduce the risk of injuries but based on these data a positive effect of body protectors could not be shown. In this study safety vest users suffered from injuries of the upper body more often than those who did not wear a body protector. Children and adolescents often overestimate their skills; therefore, teaching and supervision of inexperienced young riders along with the use of protective equipment can prevent major injuries.
