Resume Introduction Le stress occasionne par la gestion des appels de detresse recus au SAMU peut representer, pour les assistants de regulation medicale (ARM), un risque psychologique. Objectif Etudier les repercussions psychologiques sur les ARM. Methode Etude prospective, multicentrique, nationale, d’un mois aupres d’ARM de 13 SAMU par questionnaire electronique de cinq sections : caracteristiques de la population, typologie des appels, echelle PCL-5, echelle ProQOL, ressentis et retentissements du travail sur leur vie. Recherche des criteres associes a une symptomatologie evocatrice de trouble de stress post-traumatique (TSPT) complet defini par le DSM-5 et PCL-5 ≥ 34. Resultats Tous les ARM ont ete confrontes a la mort dont 129 (46 %) dans les sept derniers mois. Trente (11 %) presentaient une symptomatologie evocatrice d’un TSPT complet et 42 (15 %) celle d’un TSPT incomplet. Le groupe TSPT complet avait significativement plus d’ARM ayant au plus le baccalaureat (n = 22, 74 % vs n = 145, 57 % ; p = 0,04). Aucune autre difference n’etait significative. Le groupe TSPT complet developpait significativement (p Conclusion Chez 1/3 des ARM interroges, une symptomatologie evocatrice de TSPT complet ou incomplet etait retrouvee avec des processus psychotraumatiques specifiques. La formation semble etre une protection efficace. Elle doit etre renforcee, tout comme la prevention.

Direct proton-knockout reactions of ^{55}Sc at ∼220 MeV/nucleon were studied at the RIKEN Radioactive Isotope Beam Factory. Populated states of ^{54}Ca were investigated through γ-ray and invariant-mass spectroscopy. Level energies were calculated from the nuclear shell model employing a phenomenological internucleon interaction. Theoretical cross sections to states were calculated from distorted-wave impulse approximation estimates multiplied by the shell model spectroscopic factors, which describe the wave function overlap of the ^{55}Sc ground state with states in ^{54}Ca. Despite the calculations showing a significant amplitude of excited neutron configurations in the ground-state of ^{55}Sc, valence proton removals populated predominantly the ground state of ^{54}Ca. This counterintuitive result is attributed to pairing effects leading to a dominance of the ground-state spectroscopic factor. Owing to the ubiquity of the pairing interaction, this argument should be generally applicable to direct knockout reactions from odd-even to even-even nuclei.

8 pags., 4 figs., 3 tabs., The nuclear structure of 51Ar, an uncharted territory so far, was studied by the (p,2p) reaction using γ-ray spectroscopy for the bound states and the invariant mass method for the unbound states. Two peaks were detected in the γ-ray spectrum and six peaks were observed in the 50Ar+n relative energy spectrum. Comparing the results to our shell-model calculations, two bound and six unbound states were established. Three of the unbound states could only be placed tentatively due to the low number of counts in the relative energy spectrum of events associated with the decay through the first excited state of 50Ar. The low cross sections populating the two bound states of 51Ar could be interpreted as a clear signature for the presence of significant subshell closures at neutron numbers 32 and 34 in argon isotopes. 7 pags., 4 figs., 1 tab., Excited states in the N=40 isotone Ti were populated via the V(p,2p)Ti reaction at ∼200 MeV/nucleon at the Radioactive Isotope Beam Factory and studied using γ-ray spectroscopy. The energies of the 2 →0 and 4 →2 transitions, observed here for the first time, indicate a deformed Ti ground state. These energies are increased compared to the neighboring Cr and Fe isotones, suggesting a small decrease of quadrupole collectivity. The present measurement is well reproduced by large-scale shell-model calculations based on effective interactions, while ab initio and beyond mean-field calculations do not yet reproduce our findings. The shell-model calculations for Ti show a dominant configuration with four neutrons excited across the N=40 gap. In France, the emergency call center is called SAMU (service d'aide médicale d'urgence). The Medical Dispatcher Assistant (MDA) is the first responder and is exposed to first calls of distress and has a high risk of stress disorder.Psychological impact of emergency calls on MDA.National multicenter prospective study from January to August 2018 by electronic surveys, including all MDA of 13 SAMU, subdivided in 5 sections: population characteristics, PCL-5 scale (DSM-5) assessing post-traumatic stress disorder (PTSD), ProQOL assessing professional quality of life, call categories and an MDA's emotional perception, and work impacts on an MDA's quality of life. Univariate descriptive statistical analysis of the group with PCL-5≥34 (=complete PTSD group) and with PCL-534 (=group without complete PTSD).Of 400 MDA asked to be interviewed, 283 (71 %) replied of whom 72 % (205) were women and 28 % (79) men. Age groups: 9 % (25) for 18-25 yrs, 39 % (110) for 26-35 yrs, 31 % (89) 36-45 yrs, 15 % (43) 46-55 yrs and 6 % (16) for more than 56 yrs. All MDA reported having been exposed to death experience. For 46 % (129) the most recent traumatic event occurred within the last 7 months. 78 % (219) have reported intense fear, feeling helpless, or even sensed horror when answering the calls. 97 % (273) could talk about it with colleagues but only 64 % (180) with family. 72 % (203) felt lack of recognition at work. 78 % (220) had no knowledge about psycho-traumatic disorder. While 11 % (30) suffered symptoms suggestive of a complete PTSD, 15 % (42) an incomplete PTSD, 3 % (8) suffer burnout and 4 % (11) compassion fatigue, none reported secondary traumatic stress. The only significant difference (P0.05) between the two groups characteristics was on the education level. 74 % (22) of the MDA with a complete PTSD had a High School diploma or less. MDA with symptoms suggestive of complete PTSD developed significantly (P0.001) more stress reduction strategies (alcohol, drugs, medication) (13 % vs 2 %), had more food disorders (80.5 % vs 38 %), more sleeping problems (75.5 % vs 21 %), more anxiety (67 % vs 17 %), and more sick leaves (13 % vs 4 %) than the group without complete PTSD.Part of the surveyed MDAs showed symptoms suggestive of PTSD. The study highlights that MDAs is a vulnerable population, and PTSD prevention techniques should be systematically implemented for them. The study also highlights that a higher education level prevents the psycho traumatic process with its accompanying disorders.

Exclusive cross sections and momentum distributions have been measured for quasifree one-neutron knockout reactions from a ^{54}Ca beam striking on a liquid hydrogen target at ∼200 MeV/u. A significantly larger cross section to the p_{3/2} state compared to the f_{5/2} state observed in the excitation of ^{53}Ca provides direct evidence for the nature of the N=34 shell closure. This finding corroborates the arising of a new shell closure in neutron-rich calcium isotopes. The distorted-wave impulse approximation reaction formalism with shell model calculations using the effective GXPF1Bs interaction and ab initio calculations concur our experimental findings. Obtained transverse and parallel momentum distributions demonstrate the sensitivity of quasifree one-neutron knockout in inverse kinematics on a thick liquid hydrogen target with the reaction vertex reconstructed to final state spin-parity assignments.

Triple-negative breast cancer (TNBC) commonly develops resistance to chemotherapy, yet markers predictive of chemoresistance in this disease are lacking. Here, we define WNT10B-dependent biomarkers for β-CATENIN/HMGA2/EZH2 signaling predictive of reduced relapse-free survival. Concordant expression of HMGA2 and EZH2 proteins is observed in MMTV-Wnt10bLacZ transgenic mice during metastasis, and Hmga2 haploinsufficiency decreased EZH2 protein expression, repressing lung metastasis. A novel autoregulatory loop interdependent on HMGA2 and EZH2 expression is essential for β-CATENIN/TCF-4/LEF-1 transcription. Mechanistically, both HMGA2 and EZH2 displaced Groucho/TLE1 from TCF-4 and served as gatekeepers for K49 acetylation on β-CATENIN, which is essential for transcription. In addition, we discovered that HMGA2-EZH2 interacts with the PRC2 complex. Absence of HMGA2 or EZH2 expression or chemical inhibition of Wnt signaling in a chemoresistant patient-derived xenograft (PDX) model of TNBC abolished visceral metastasis, repressing AXIN2, MYC, EZH2, and HMGA2 expression in vivo. Combinatorial therapy of a WNT inhibitor with doxorubicin synergistically activated apoptosis in vitro, resensitized PDX-derived cells to doxorubicin, and repressed lung metastasis in vivo. We propose that targeting the WNT10B biomarker network will provide improved outcomes for TNBC. Significance: These findings reveal targeting the WNT signaling pathway as a potential therapeutic strategy in triple-negative breast cancer.

The first γ-ray spectroscopy of ^{52}Ar, with the neutron number N=34, was measured using the ^{53}K(p,2p) one-proton removal reaction at ∼210 MeV/u at the RIBF facility. The 2_{1}^{+} excitation energy is found at 1656(18) keV, the highest among the Ar isotopes with N>20. This result is the first experimental signature of the persistence of the N=34 subshell closure beyond ^{54}Ca, i.e., below the magic proton number Z=20. Shell-model calculations with phenomenological and chiral-effective-field-theory interactions both reproduce the measured 2_{1}^{+} systematics of neutron-rich Ar isotopes, and support a N=34 subshell closure in ^{52}Ar.

After nucleon knockout reactions on the secondary liquid hydrogen MINOS target the known γ rays of the neutron-rich 55Sc isotope were observed (shown in this proceedings) and γ rays from 57,59Sc isotopes have been identified for the first time. Summary Background Intensive antiplatelet therapy with three agents might be more effective than guideline treatment for preventing recurrent events in patients with acute cerebral ischaemia. We aimed to compare the safety and efficacy of intensive antiplatelet therapy (combined aspirin, clopidogrel, and dipyridamole) with that of guideline-based antiplatelet therapy. Methods We did an international, prospective, randomised, open-label, blinded-endpoint trial in adult participants with ischaemic stroke or transient ischaemic attack (TIA) within 48 h of onset. Participants were assigned in a 1:1 ratio using computer randomisation to receive loading doses and then 30 days of intensive antiplatelet therapy (combined aspirin 75 mg, clopidogrel 75 mg, and dipyridamole 200 mg twice daily) or guideline-based therapy (comprising either clopidogrel alone or combined aspirin and dipyridamole). Randomisation was stratified by country and index event, and minimised with prognostic baseline factors, medication use, time to randomisation, stroke-related factors, and thrombolysis. The ordinal primary outcome was the combined incidence and severity of any recurrent stroke (ischaemic or haemorrhagic; assessed using the modified Rankin Scale) or TIA within 90 days, as assessed by central telephone follow-up with masking to treatment assignment, and analysed by intention to treat. This trial is registered with the ISRCTN registry, number ISRCTN47823388. Findings 3096 participants (1556 in the intensive antiplatelet therapy group, 1540 in the guideline antiplatelet therapy group) were recruited from 106 hospitals in four countries between April 7, 2009, and March 18, 2016. The trial was stopped early on the recommendation of the data monitoring committee. The incidence and severity of recurrent stroke or TIA did not differ between intensive and guideline therapy (93 [6%] participants vs 105 [7%]; adjusted common odds ratio [cOR] 0·90, 95% CI 0·67–1·20, p=0·47). By contrast, intensive antiplatelet therapy was associated with more, and more severe, bleeding (adjusted cOR 2·54, 95% CI 2·05–3·16, p Interpretation Among patients with recent cerebral ischaemia, intensive antiplatelet therapy did not reduce the incidence and severity of recurrent stroke or TIA, but did significantly increase the risk of major bleeding. During January and February 2007 a European consumer test of eight new apple varieties and three standard ones was carried out in different European countries. The most common apple varieties were compared to new ones, focusing on consumer acceptance and the potential for increasing apple consumption. We sought to test the hypothesis that the introduction of new improved varieties would help to increase fruit consumption. Data from a sensory test involving 4290 consumers in seven different countries were analysed for the effects of variety and test country, age, gender and apple consumption habit of the consumer on eating quality acceptance. The results obtained showed a strong interaction between country and variety, although a group of four varieties (‘Gold Pink’, ‘CIVni’, ‘Cripps Pink’ and ‘Fuji’) formed part of a group of the varieties that were most accepted in all of the countries except Germany. Significant interactions between variety and age and variety and gender were observed, but their magnitudes were not considered to be of any practical significance. No statistically significant interaction was found between variety and apple consumption habit on eating quality acceptance.

We investigated the development of a low (T-type) and two high voltage-activated (N- and L-type) calcium channel currents in large diameter dorsal root ganglion neurones acutely isolated from embryonic mice using the whole-cell patch-clamp technique. The low and high voltage-activated barium currents (LVA and HVA) were identified by their distinct threshold of activation and their sensitivity to pharmacological agents, dihydropyridines and omega-conotoxin-GVIA, at embryonic day 13 (E13), E15 and E17-18, respectively, before, during and after synaptogenesis. The amplitude and density of LVA currents, measured during a -40 mV pulse from a holding potential of -100 mV, increased significantly between E13 and E15, and remained constant between E15 and E17-18. The density of global HVA current, elicited by 0 mV pulse, increased between E13 and E15/E17-18. The density of the N-type current studied by the application of omega-conotoxin-GVIA (1 microM) increased significantly between E13 and E15/E17-18. The use of the dihydropyridine nitrendipine (1 microM) revealed that the density of L-type current remained constant at each stage of development. Nevertheless, application of dihydropyridine Bay K 8644 (3 microM) demonstrated a significant slowing of the deactivation tail current between embryonic days 13 and 15, which may reflect a qualitative maturation of this class of calcium channel current. The temporal relationship between the changes in calcium channel pattern and the period of target innervation suggests possible roles of T-, N- and L-type currents during developmental key events such as natural neurone death and onset of synapse formation.

A consumer test carried out in 7 different European countries compared 3 standard apple varieties to 8 new ones. A total of 4290 consumers took part in the test. Data from this test was used to develop a preference map for apple. The preference map was constructed with 3 main dimensions (1 – sweetness, fruitiness, flowery attributes, 2 – acidity, firmness, 3 – juiciness and crispness). Consumers were segmented in 6 clusters according to their preferences. The 6 clusters were grouped into two main mega clusters A (68% of consumers) and B (32% of consumers). Megacluster A (Clusters 1, 2, 5 and 6) was characterized by preferring sweet apples. Clusters 2 and 5 (41% of consumers) liked sweet apples independently of their acidity and firmness and moderate positive values on dimension of juiciness and crispness. Cluster 1 (21% of consumers) had an optimal point in positive values of the sweetness dimension, moderate negative value for acidity and firmness and moderate positive value for juiciness and crispness. Cluster 6 (6% of consumers) besides preferring sweet varieties disliked acid-firm varieties. As to regard to megacluster B (Clusters 3 and 4) (32% of consumers), they preferred varieties that were acidic-firm and juiciy and crisp with values in the mid range of the sweetness dimension. In spite of the difficulties in translating preference dimensions into standard practical values for fruit quality and the fact of being a punctual measurement of consumer behaviour, this preference map could be of practical use of different actors on the fruit value chain like marketers and breeders.

We characterized toxin-insensitive calcium currents expressed by acutely dissociated embryonic dorsal root ganglion neurons. In the presence of 3 microM omega-conotoxin-GVIA, 3 microM nitrendipine and either 500 nM omega-agatoxin-IVA or 500 nM omega-conotoxin-MVIIC to inhibit N-, L- and P/Q-type currents, respectively, all neurons expressed two residual currents: a T-type and another which we referred to as toxin-resistant current. The toxin-resistant current (i) consisted of an inactivating and a sustained components, (ii) had a threshold of activation and a steady-state inactivation comprised between that of the T-type current and that of the other high-voltage-activated currents, (iii) had the same permeability for barium and calcium used as charge carriers, (iv) was highly sensitive to both cadmium and nickel; and (v) was insensitive to 500 microM amiloride which abolished the T-type at this concentration. The properties of the toxin-resistant current are very similar to those of the currents expressed in oocytes following injection of alpha(1E) subunits which we demonstrated to be present in these neurons. Therefore a component of the toxin-resistant current calcium channels in sensory neurons may be closely related to those calcium channels formed by alpha(1E) subunits.

Analysis of neuronal development has emphasized the importance of voltage-activated Ca2+ currents during the initial period of differentiation. We investigated non-N, non-L Ba2+ currents through Ca2+ channels in freshly dissociated large diameter embryonic mouse dorsal root ganglion neurons using the whole-cell patch-clamp technique. Two types of omega-agatoxin IVA-sensitive currents were clearly distinguished at embryonic day 13: a sustained P-type current blocked selectively at 30 nM (IC50 = 3nM) and an inactivating Q-type current blocked in the range 50-500 nM (IC50 = 120nM). The P-type Ca2+ current disappeared at day 15 whereas the Q-type Ca2+ current increased two- to three-fold during the same embryonic period. In contrast, the contribution of the non-L, non-N, omega-agatoxin IVA-resistant current (R-type) was constant during this developmental span. In conclusion, our results clearly show that P- and Q-type Ca2+ currents are differentially expressed during ontogenesis in large diameter dorsal root ganglion neurons. The developmental change, which occurs during the period of target innervation, could be related to specific key events such as natural neuron death and onset of synapse formation.

The aim of this study was to gain information concerning apple and peach consumption frequency within different European countries in relation to age and gender. The survey was a part of a complex experiment with the aim of evaluating consumers’ preferences towards new varieties, and the data is based on the self-reported declarations of respondents, male and female, between 15 and 70 years old. 4271 consumers from 7 European countries were invited to supply information about their apple consumption habits, whereas 499 respondents from 5 countries answered questions relating to frequency of peach and nectarine consumption. In both, the apple and the peach surveys, data analysis of declared intake showed significant differences between nationalities. The highest apple consumption was in Poland, where over 55% declared a consumption of more than 5 apples per week. In comparison, Italian consumers most often indicated eating 3–5 apples per week (39.3%). The lowest apple consumption was in the Netherlands and Spain. In the case of peaches, the highest consumption was indicated in France where 48% of respondents declared a peach consumption of 3–5 per week with 40% eating more than 5 fruits per week. The lowest peach intake was declared in Germany. Irrespective of country women were shown to eat more apples that men. Furthermore, the group of older people (61–70 years) consume apples more often than the adult group (36–60), while within the youngest group of consumers (16–35) eating apples was not at all popular. As with apples females declared a higher peach consumption, and again significantly lower fruit consumption by the youngest group (16–35) was indicated. Although the availability of fruit at the market remains a prime factor in determining apple and peach consumption, our survey confirmed the trends of declining this popular fruit intake by the younger generation, as well as the persistent tendency of lower frequency of fruit consumption among men than women.

International audience; NH3 and NO have been added to low pressure, premixed laminar CH4-O2 flames of different equivalence ratios. The profiles of concentrations of major radicals and stable species have been obtained, using a molecular beam sampling technique coupled with a mass spectrometer. Laser-induced fluorescence has been used to measure CH radicals. Absorption spectroscopy has been applied to OH radical and temperature measurements. The behaviours of NO and HCN are mainly discussed, and the whole experimental result compared to complete flame structure computations. Using a recent reaction mechanism described by Miller, we notice that pollutants NO and HCN are generally underestimated by computations. Another mechanism, proposed by Bian et al., for nitrogen oxidation has been tested. Sensitivity and reaction path analysis made it possible to identify the reactions which would require modifications, according to a review of recently published kinetic values, in order to get a better fit between the experimental profiles and the computed ones.

Background At this time, no predictive factor for response to biotherapy in RA patients is available in the daily practice. Objectives The objective is to determine predictive factors for the first biological maintenance in rheumatoid arthritis (RA) treatment. Methods From our database including 663 RA patients, 289 RA patients were received their first biotherapy between 2000 and 2009. Among these 289 RA patients, we selected 2 subsets of patients. In the first group, RA patients received the same biological therapy for at least 3 years (n=56). In the second group, RA patients required at least 3 different biological therapies over the 3 years following the first biological (n=36). Demographic parameters (age, sex, smoking, body mass index), RA activity (DAS28), biological parameters, DMARDs and glucocorticoid therapy were recorded. Chi-squared statistical analysis and odds ratios (OR) were performed. Results Due to the follow-up of three years, TNFa blockers were mainly used as first biotherapy (Etanercept, adalimumab, or infliximab) in 93%. Biotherapies were equally distributed in both groups. Demographic parameters and disease ac tivity were similar in the two groups. In the same way, biological parameters of RA severity were not different in both groups. However, combination of DMARDs before starting the first biotherapy was associated with requirement to switch frequently biotherapy compared to the first goup (OR = 4.2; 95%CI: 1.193 to 14.79; p Conclusions Our study shows that the presence of methotrexate at the starting time of a biotherapy is a factor for maintenance of the first biological therapy for at least three years.
