Abstract

The artichoke (Cynara cardunculus subsp. scolymus) is an intriguing source of indigestible sugar polymers such as inulin-type fructans. Artichoke represents an important component of a traditional Mediterranean diet and its edible parts are a good source of many high added value compounds such as inulin, a polymer showing relevant prebiotic properties. Compared to the cultivated varieties, the wild cardoon (C. cardunculus var. sylvestris) growing naturally under harsh conditions and well-adapted to many marginal areas, could have a good potential for use in sustainable production in stressed lands. Here, we evaluated by enzymatic assay, the amount of inulin-type fructans both in artichoke and wild cardoon in the two different organs, heads and rhizomes. The expression pattern of the genes encoding the two key enzymes sucrose:sucrose 1-fructosyltransferase and fructan 1-fructosyltransferase, involved in fructan biosynthesis, have been also evaluated. Our results showed that the amount of inulin-type fructans was higher in the wild cardoon than in the artichoke heads, together with a higher expression of the two key genes involved in the fructan biosynthetic pathway. A conspicuous content of inulin-type fructans was found also in the rhizome, supporting the significant role of these compounds in the storage and in protection from cold and/or winter stresses.

Abstract

Abstract KM3NeT is a research infrastructure located in the Mediterranean Sea, that will consist of two deep-sea Cherenkov neutrino detectors. With one detector (ARCA), the KM3NeT Collaboration aims at identifying and studying TeV–PeV astrophysical neutrino sources. With the other detector (ORCA), the neutrino mass ordering will be determined by studying GeV-scale atmospheric neutrino oscillations. The first KM3NeT detection units were deployed at the Italian and French sites between 2015 and 2017. In this paper, a description of the detector is presented, together with a summary of the procedures used to calibrate the detector in-situ. Finally, the measurement of the atmospheric muon flux between 2232–3386 m seawater depth is obtained.

Abstract

Abstract Studying atmospheric neutrino oscillations in the few-GeV range with a multi-megaton detector promises to determine the neutrino mass hierarchy. This is the main science goal pursued by the future KM3NeT/ORCA water Cherenkov detector in the Mediterranean Sea. In this paper, the processes that limit the obtainable resolution in both energy and direction in charged-current neutrino events in the ORCA detector are investigated. These processes include the composition of the hadronic fragmentation products, the subsequent particle propagation and the photon-sampling fraction of the detector. GEANT simulations of neutrino interactions in seawater produced by GENIE are used to study the effects in the 1-20 GeV range. It is found that fluctuations in the hadronic cascade in conjunction with the variation of the inelasticity y are most detrimental to the resolutions. The effect of limited photon sampling in the detector is of significantly less importance. These results will therefore also be applicable to similar detectors/media, such as those in ice.

Abstract

Multileaf collimators (MLCs) are an essential component in modern radiotherapy that shape the X-ray treatment beam. Currently, MLC leaf position accuracy is verified to ±1 mm every month. However, leaf position accuracy can drift between verification dates and treatment verification only occurs pretreatment. To prevent serious errors, it would be highly beneficial to use a real-time verification system. We are developing a system based on monolithic active pixel sensors (MAPSs). MAPS are radiation hard under photon and electron irradiation, have high readout rates, low attenuation and are suitable for high-resolution applications making them an ideal upstream radiation detector. Here, we report results using the Lassena MAPS, which measures $12\times14$ cm2 and is three-side buttable, allowing full treatment fields to be monitored. The Lassena detector was placed in the treatment field of an Elekta Synergy LINAC which has an MLC leaf width of 0.5 cm. An MLC leaf was extended to ten different positions within the field. Sobel-based methods were used to reconstruct the leaf edge position. Correspondence between reconstructed and set leaf position was excellent and resolutions ranged between 60.6± 8 and $109\pm 12~\mu \text{m}$ for a central leaf with leaf extensions ranging from 1 to 35 mm using ~0.3 s of treatment beam time while the sensor was placed at an SSD of 85 cm.

7. Development of muon scattering tomography for detection of reinforcement in concrete

Abstract

Inspection of ageing, reinforced concrete structures is a world-wide challenge. Existing evaluation techniques in civil and structural engineering have limited penetration depth and do not allow to precisely ascertain the configuration of reinforcement within large concrete objects. The big challenge for critical infrastructure (bridges, dams, dry docks, nuclear bioshields etc.) is understanding the internal condition of the concrete and steel, not just the location of the reinforcement. Muon scattering tomography is a non-destructive and non-invasive technique which shows great promise for high-depth 3D concrete imaging. Here a method is presented to locate reinforcement meshes placed in a large-scale concrete object. A reinforcement mesh was simulated as two layers of 2 m long bars, forming a mesh. Two layers of the mesh were placed at several distances from each other inside a large concrete block. Previously, we have shown that using our autocorrelation technique for single meshes inside the concrete and using only one week worth of data taking, bars with a diameter of 7 mm and larger, could easily be detected for a 10 cm mesh spacing. The signal for 6 mm diameter bar exceeds the background and becomes very clear after two weeks of data taking. Here we show that we can detect the vertical positions of two mesh layers inside the concrete. This is a very important result for non-destructive evaluation of civil structures.

SRAM-based Field Programmable Gate Arrays (FPGAs) represent an attractive solution for mission-critical computationally intensive applications due to their high integration, flexibility, and computational capabilities. However, the static memory cells of SRAM-based configuration memory present a high sensitivity to radiation effects. Due to the increasing interest in using these devices in radiation environments such as aerospace and high energy physics, the evaluation of their reliability through radiation tests is a key role in their validation. Radiation Testing consists in exposing the physical device to radioactive source or radiation beams and represents an accurate solution for evaluating the device sensitivity. However, this implies high costs both in terms of experimental setup and money due to the low availability of the required facilities. Among the possible solutions, neutron generator testing would represent an efficient solution even if challenging from the instrumentation needed for the experimental setup. In this work, we present a test instrumentation and methodology specific for cost-effective short-time neutron generator testing to efficiently evaluate FPGA radiation-induced soft error sensitivity.

Abstract

Monolithic active pixel sensor (MAPS) devices are an effective tool for upstream verification of intensity-modulated radiotherapy (IMRT) treatments. It is crucial to measure with high precision the positions of the multi-leaf collimators (MLCs) used to shape the beam in real time, in order to enhance the quality and safety of treatments. This article describes r-UNet, a deep learning-based solution for leaf position reconstruction. The model is used to analyze the high-resolution images produced by a Lassena MAPS device in order to automatically determine the leaf positions. Image segmentation and leaf position estimation are performed simultaneously in a multitask setting. r-UNet obtained an average Dice coefficient of 0.96 ± 0.03 for the reconstructed image masks in the held-out test set; whilst the mean squared error (MSE) resulting from the estimation of the MLC positions is 0.003 mm, with a resolution ranging between 45 and 53 $\mu \text{m}$ for leaf extensions between 1 and 35 mm. On unseen leaf positions, r-UNet yielded a single-leaf resolution between 54 and 88 $\mu \text{m}$ depending on the leaf extension, and an average MSE of 0.07 mm. These results were obtained using single frames of data collected at 34 frames/s.

Abstract

Static RAM modules are widely adopted in high performance systems. Single Event Effects (SEEs) resilient memories are required in many embedded systems applied in automotive and aerospace applications to increase their overall resiliency against SEEs. The current SEE resilient SRAM modules are obtained by applying radiation-hardened by design solutions which leads to elevated area overhead and difficulty to tune the resiliency capability with respect to the particle's radiation profile. To overcome these limitations, we propose a methodology for the analysis and mitigation of embedded SRAMs generated by the OpenRAM memory compiler. A technology-oriented radiation analysis tool is presented to support the interaction of the charged radiation particles with the SRAM layout and depict the sensitive transistors of the SRAM memory. A selective duplication of the sensitive transistors has been applied to the 6T-SRAM cell designed at the layout level. The designed cell is included in the OpenRAM compiler and used to generate a mitigated 8Kb SRAM-bank. We evaluated the SEEs sensitivity by comparative simulation-based radiation analysis observing a reduction more than 6 times with respect to the original 6T-SRAM cell for the SEE sensitivity at high energy heavy ions particles, with negligible degradation of operations margins and power consumption and area overhead of less than $\sim$ 4%.

Abstract

The current trend in X-ray radiotherapy is to treat cancers that are in difficult locations in the body using beams with a complex intensity profile. Intensity-modulated radiotherapy (IMRT) is a treatment which improves the dose distribution to the tumor whilst reducing the dose to healthy tissue. Such treatments administer a larger dose per treatment fraction and hence require more complex methods to verify the accuracy of the treatment delivery. Measuring beam intensity fluctuations is difficult as the beam is heavily distorted after leaving the patient and transmission detectors will attenuate the beam and change the energy spectrum of the beam. Monolithic active pixel sensors (MAPSs) are ideal solid-state detectors to measure the 2-D beam profile of a radiotherapy beam upstream of the patient. MAPS sensors can be made very thin ( ${\sim }30~\mu \text{m}$ ) with still very good signal-to-noise performance. This means that the beam would pass through the sensor virtually undisturbed ( $2{~\mu }\text{m}$ to $100~{\mu }\text{m}$ are commercially available. Large area devices ( ${\sim }15 \times 15$ cm2) have been produced. MAPS can be made radiation hard enough to be fully functional after a large number of fractions. All this makes MAPS a very realistic transmission detector candidate for beam monitoring upstream of the patient. A remaining challenge for thin, upstream sensors is that the detectors are sensitive to the signal of both therapeutic photons and electron contamination. Here, a method is presented to distinguish between the signal due to electrons and photons and thus provide real-time dosimetric information in very thin sensors that does not require Monte Carlo simulation of each linear accelerator treatment head.

Abstract

Multileaf collimators (MLC) are an integral component in modern radiotherapy as they shape the X-ray treatment beam and therefore need to be closely controlled. Currently, MLC leaves are calibrated to ± 1 mm every month, however leaves can drift beyond this during calibration dates and treatment verification only occurs pre-treatment. MAPS are radiation hard in photon and electron irradiation, have high readout speeds and low attenuation which makes them an ideal upstream radiation detector. Here, we report results using the Lassena MAPS, which is 12×14 cm2, three side buttable and is capable of clinical deployment. We have previously shown that the Achilles MAPS can reconstruct 1 cm leaf edge positions with a 52 ± 4 µm precision in 0.1 s of treatment beam. The Achilles however, is sized 6×6 cm2, is not buttable and hence, cannot be used for real time clinical monitoring. Additionally, Achillies has a pixel pitch of 14.5 µm whilst the Lassena has a 50 µm pixel pitch. The Lassena detector was placed in the treatment field of an Elekta Synergy LINAC and an MLC leaf of width 0.5 cm, positioned stepwise. Sobel-based methods were used to calculate leaf edge positions, resolutions and plot as a function of set leaf position. Correspondence between reconstructed and set leaf position was excellent and resolutions ranged between 52.9 ± 8.5 µm and 142 ± 31.6 µm for 0.3 s of treatment beam, which is significantly lower than the ± 0.5 mm uncertainty in LINAC set position.

FinFET technology has recently gained attention in the semiconductor industry due to many benefits it offers, such as lower power consumption, faster performance, and lower static leakage current. However, the behavior of this technology for mission-critical applications where radiation effects are the main concern is not very well known. This work is dedicated to the detailed characterization of radiation-induced transient errors in 7 nm FinFET technology, calculating the sensitivity of basic logic gates implemented using ASAP7 PDK library and predicting the distribution of heavy ions induced Single Event Transient (SET) pulses.

Abstract

Monolithic Active Pixel Sensor (MAPS) devices are an effective tool for upstream verification of Intensity Modulated Radio Therapy treatments. It is crucial to measure with high precision the positions of the Multi Leaf Collimators (MLC) used to shape the beam, in order to enhance the quality and safety of treatments. This work describes a two-step procedure applied to the problems of leaf detection and position reconstruction. This procedure represents the building block towards the definition of a real-time verification device capable of detecting and reconstructing the position of the MLC used in the everyday radiotherapy treatments. A Fully Convolutional Neural Network model is used to analyse the high-resolution images produced by Lassena MAPS devices in order to automatically detect the leaves. A Random Forest regression model is then applied to estimate the positions of the detected leaves. The dataset used in the experiments contains 1200 single-leaf images, referring to four leaf positions in the range 1÷25 mm, split in training and test sets with the 80% and 20% of the total samples, respectively. Performance have been evaluated using the Dice loss coefficient for leaf detection, and mean squared error (MSE) as for leaf position estimation. The proposed approach obtained an average dice loss coefficient of 0.85±0.03 on the images in the test set; the MLC positions are estimated with a MSE of 0.04, and a resolution of 68±2µm on the same images.

Abstract

KM3NeT will be a network of deep-sea neutrino telescopes in the Mediterranean Sea. The KM3NeT/ARCA detector, to be installed at the Capo Passero site (Italy), is optimised for the detection of high-energy neutrinos of cosmic origin. Thanks to its geographical location on the Northern hemisphere, KM3NeT/ARCA can observe upgoing neutrinos from most of the Galactic Plane, including the Galactic Centre. Given its effective area and excellent pointing resolution, KM3NeT/ARCA will measure or significantly constrain the neutrino flux from potential astrophysical neutrino sources. At the same time, it will test flux predictions based on gamma-ray measurements and the assumption that the gamma-ray flux is of hadronic origin. Assuming this scenario, discovery potentials and sensitivities for a selected list of Galactic sources and to generic point sources with an $E^{-2}$ spectrum are presented. These spectra are assumed to be time independent. The results indicate that an observation with $3\sigma$ significance is possible in about six years of operation for the most intense sources, such as Supernovae Remnants RX\,J1713.7-3946 and Vela Jr. If no signal will be found during this time, the fraction of the gamma-ray flux coming from hadronic processes can be constrained to be below 50\% for these two objects., Comment: 32 pages, 9 figures, submitted to Astroparticle Physics

Abstract

Commercial SRAM-based FPGAs are widely used in aerospace applications thanks to their limited costs and high performances. Dynamic Partial Reconfigurability (DPR) allows to rewrite sections of the configuration memory of these devices while they are normally working. This is a powerful feature which provides advantages in terms of area, power consumption and flexibility. Moreover, DPR plays the main role in the recovery from faults that may occur in the configuration memory of the device when it operates in a radiation environment. Indeed, ionized particles which strike electronic devices, may cause various types of events. The Single Event Transient (SET) is a well-known phenomenon, which affects sensitive nodes causing transitory voltage spikes that can be sampled by a memory element, causing a fault. In this paper, an evaluation methodology for the errors caused by SETs during the reconfiguration of the configuration memory in SRAM-based FPGAs is presented. The evaluation methodology includes the fault model evaluation and the development of a fault injection and error evaluation workflow. The illustrated methodology has been applied on a physical device implementing a benchmark design and the obtained results are reported.

Abstract

Background and aims: Recently, the albuminocentric view of diabetic kidney disease (DKD) in type 2 diabetes (T2DM) has been changing. Therefore, the relationship between diabetic retinopathy (DR) and chronic kidney disease (CKD) has to be addressed according to this new clinical presentation of DKD. The aim of this study was to evaluate, in a real-world setting, the correlation DR–DKD in T2DM. Methods and results: A total of 2068 type 2 diabetic patients enrolled in a multicenter cross-sectional study were investigated. Albuminuric subjects were largely prevalent among subjects with DR (p = 0.019). In the whole study population, no difference in albumin excretion rate (AER) was observed between presence/absence of DR; instead, AER was significantly higher among patients with glomerular filtration rate (eGFR) < 60 mL/min/1.73 m2 (CKD) (p = 0.009), above all in those with CKD and AER ≥0.03 g/24 h (p = 0.005). Multivariate analysis confirmed that eGFR (O.R. 0.976; 95% C.I.: 0.960–1.028; p < 0.001) and AER (O.R. 1.249; 95% C.I. 1.001–1.619; p = 0.004) were independently associated with DR and HDL–cholesterol (O.R.: 1.042; 95% C.I.: 1.011–1.120; p = 0.014). Abstract

Due to the increasing demand for high performance in embedded systems, devices such as SRAM-based programmable devices are becoming an appealing solution to reach high performance with limited costs. However, SRAM-based programmable devices are subjected to various sources of radiation-induced faults that affect their reliability, such as ionizing radiation and particles, even at sea-level. In this paper, we evaluate the reliability of the interconnection module, implemented on the programmable hardware, against radiation-induced faults in the configuration layer. To do so, we performed a fault injection campaign in order to emulate the radiation-induced effects impacting the configuration memory of AP-SoC Zynq 7000, specifically targeting the configuration memory section programming the interconnection module implemented on the programmable logic. This interconnection module is a crucial element for a wide range of applications and mitigation techniques such as hardware-accelerated designs, Dynamic Partial Reconfiguration, or Triple Modular Redundancy; especially if they are adopted to achieve high performance, high bandwidth and high reliability. The fault injection results have been analyzed and classified accordingly with the effect observed on the processor-system side in terms of availability and fault model affecting data computed by cores implemented on the programmable logic side.

Abstract

Intensity modulated radiotherapy is a widely used technique for accurately targeting cancerous tumours in difficult locations. As treatments are becoming more complex, new methods need to be developed to monitor them. Monolithic active pixel sensors are a viable candidate for providing upstream beam monitoring during treatment. A MAPS based system can be made thin enough to have less than 1% attenuation. We have already demonstrated leaf position resolutions below 130µm at the iso-centre for 5mm wide leaves sampled 34 times per second. We have shown that the signal due to therapeutic photons can be determined and thus the dose in patient. Furthermore, the sensor works well inside an MR-linac, allowing leaf position verification even in that challenging environment.

Abstract

[EN] The Hamamatsu R12199-02 3-inch photomultiplier tube is the photodetector chosen for the first phase of the KM3NeT neutrino telescope. About 7000 photomultipliers have been characterised for dark count rate, timing spread and spurious pulses. Abstract

Recent studies suggest a potential relationship between rosacea and Helicobacter pylori (H. pylori) infection or small intestinal bacterial overgrowth (SIBO), but there is no firm evidence of an association between rosacea and H. pylori infection or SIBO. We performed a prospective study to assess the prevalence of H. pylori infection and/or SIBO in patients with rosacea and evaluated the effect of H. pylori or SIBO eradication on rosacea.We enrolled 90 patients with rosacea from January 2012 to January 2013 and a control group consisting of 90 patients referred to us because of mapping of nevi during the same period. We used the (13)C Urea Breath Test and H. pylori stool antigen (HpSA) test to assess H. pylori infection and the glucose breath test to assess SIBO. Patients infected by H. pylori were treated with clarithromycin-containing sequential therapy. Patients positive for SIBO were treated with rifaximin.We found that 44/90 (48.9%) patients with rosacea and 24/90 (26.7%) control subjects were infected with H. pylori (p = 0.003). Moreover, 9/90 (10%) patients with rosacea and 7/90 (7.8%) subjects in the control group had SIBO (p = 0.6). Within 10 weeks from the end of antibiotic therapy, the skin lesions of rosacea disappeared or decreased markedly in 35/36 (97.2%) patients after eradication of H. pylori and in 3/8 (37.5%) patients who did not eradicate the infection (p 0.0001). Rosacea skin lesions decreased markedly in 6/7 (85.7%) after eradication of SIBO whereas of the two patients who did not eradicate SIBO, one (50%) showed an improvement in rosacea (p = 0.284).Prevalence of H. pylori infection was significantly higher in patients with rosacea than control group, whereas SIBO prevalence was comparable between the two groups. Eradication of H. pylori infection led to a significant improvement of skin symptoms in rosacea patients.

Abstract

[EN] The KM3NeT research infrastructure, currently under construction in the Mediterranean Sea, will host neutrino telescopes for the identification of neutrino sources in the Universe and for studies of the neutrino mass hierarchy. These telescopes will house hundreds of thousands of photomultiplier tubes that will have to be operated in a stable and reliable fashion. In this context, the stability of the dark counts has been investigated for photomultiplier tubes with negative high voltage on the photocathode and held in insulating support structures made of 3D printed nylon material. Small gaps between the rigid support structure and the photomultiplier tubes in the presence of electric fields can lead to discharges that produce dark count rates that are highly variable. A solution was found by applying the same insulating varnish as used for the high voltage bases directly to the outside of the photomultiplier tubes. Abstract

The authors would like to thank D Allard, A Esmaili, G Fogli, O Mena, K Murase, S Palomares Ruiz, E Parizot, S T Petcov, S Razzaque, D Semikoz, A Yu Smirnov, F L Villante, F Vissani and R Zukanovich Funchal for their support and valuable discussions and suggestions in preparing this Letter of Intent., The main objectives of the KM3NeT Collaboration are (i) the discovery and subsequent observation of high-energy neutrino sources in the Universe and (ii) the determination of the mass hierarchy of neutrinos. These objectives are strongly motivated by two recent important discoveries, namely: (1) the highenergy astrophysical neutrino signal reported by IceCube and (2) the sizable contribution of electron neutrinos to the third neutrino mass eigenstate as reported by Daya Bay, Reno and others. To meet these objectives, the KM3NeT Collaboration plans to build a new Research Infrastructure consisting of a network of deep-sea neutrino telescopes in the Mediterranean Sea. A phased and distributed implementation is pursued which maximises the access to regional funds, the availability of human resources and the synergistic opportunities for the Earth and sea sciences community. Three suitable deep-sea sites are selected, namely off-shore Toulon (France), Capo Passero (Sicily, Italy) and Pylos (Peloponnese, Greece). The infrastructure will consist of three so-called building blocks. A building block comprises 115 strings, each string comprises 18 optical modules and each optical module comprises 31 photo-multiplier tubes. Each building block thus constitutes a threedimensional array of photo sensors that can be used to detect the Cherenkov light produced by relativistic particles emerging from neutrino interactions. Two building blocks will be sparsely configured to fully explore the IceCube signal with similar instrumented volume, different methodology, improved resolution and complementary field of view, including the galactic plane. Abstract

Purpose: This study aimed to assess the role of CEUS in the evaluation of pancreatic masses. Material and methods: 78 patients underwent CEUS from 2008 to 2012. Lesion was located in pancreatic head in 36 cases, in pancreatic body and tail respectively in 23 and 19 cases. Vascular pattern of pancreatic lesions was evaluated during arterial phase of contrast distribution. CEUS-based diagnosis was compared with histological evaluation of biopsy specimens obtained by US-guided percutaneous biopsy (66 cases), endosonography (EUS)-guided biopsy (4 cases) and surgery (8 cases). Results: Final histological diagnosis was the following: ductal adenocarcinoma in 60 cases, neuroendocrine tumor in 9 cases, pancreatic non-Hodgkin lymphoma in 2 cases, metastasis from kidney and breast cancers in 2 cases, serous and mucinous cystic tumors in 4 cases and pancreatic pseudocyst in one case. Hypovascular pattern during arterial phase, characteristic of pancreatic carcinoma, was observed in 58/60 cases of ductal adenocarcinoma, with 2 false negative results. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and diagnostic accuracy (DA) of hypovascular pattern for diagnosis of adenocarcinoma were respectively 96%, 100%, 100%, 90%, 97%. Hypervascular pattern during arterial phase was characteristically observed in all cases of neuroendocrine tumors, with 6 false positive results in 2 cases of ductal adenocarcinoma, 2 cases of metastasis and 2 cases of non-Hodgkin lymphoma. Sensitivity, specificity, PPV, NPV and DA of hypervascular pattern for neuroendocrine masses were respectively 100%, 92%, 60%, 100% and 92%. Cystic lesions were correctly identified by CEUS but no different pattern of vascularization between serous and mucinous lesions was observed. Also pseudocysts were correctly identified by CEUS. We observed no complications following CEUS, US and EUS-guided biopsies. Conclusion: CEUS allows detection and characterization of pancreatic masses, better than US. It is an effective procedure in pancreatic imaging and avoids more invasive procedures.

Becker's nevus (BN) is a unilateral hyperpigmented, often hairy, cutaneous hamartoma, with geographic borders. In most cases, the lesion appears in late childhood or adolescence, and both congenital and familial cases have been described.

35. Simulation of cell cycle effects on DNA strand break induction due to α-particles.

Abstract

Purpose: Understanding cell cycle variations in radiosensitivity is important for α-particle therapies. Differences are due to both repair response mechanisms and the quantity of initial radiation-induced DNA strand breaks. Genome compaction within the nucleus has been shown to impact the yield of strand breaks. Compaction changes during the cell cycle are therefore likely to contribute to radiosensitivity differences. Simulation allows the strand break yield to be calculated independently of repair mechanisms which would be challenging experimentally., Methods: Using Geant4 the impact of genome compaction changes on strand break induction due to α-particles was simulated. Genome compaction is considered to be described by three metrics: global base pair density, chromatin fibre packing fraction and chromosome condensation. Nuclei in the G1, S, G2 and M phases from two cancer cell lines and one normal cell line are simulated. Repair mechanisms are not considered to study only the impact of genome compaction changes., Results: The three compaction metrics have differing effects on the strand break yield. For all cell lines the strand break yield is greatest in G2 cells and least in G1 cells. More strand breaks are induced in the two cancer cell lines than in the normal cell line., Conclusions: Compaction of the genome affects the initial yield of strand breaks. Some radiosensitivity differences between cell lines can be attributed to genome compaction changes between the phases of the cell cycle. 36. Targeted alpha therapies using 211 At: A Geant4 simulation of dose and DNA damage.

Abstract

Introduction: Targeted alpha therapies show great potential for cancer treatment due to their high linear energy transfer (LET) and low range. 211 At is currently employed in clinical trials. Targeted alpha therapies (TAT) are effective as an adjuvant treatment for cancer or to treat micrometastases and diffuse cancers. A deeper understanding of the induced initial damage is crucial to enhance treatment planning., Methods: This study shows Geant4(-DNA)-based simulations to calculate absorbed dose profiles and DNA damaging potential in intravenously administered TAT with 211 At. It assumes radionuclide decay on the blood vessel wall, and calculates the DNA damage in the surrounding tissue., Results: The calculated dosimetric quantities show that the effect of such treatment is mainly due to the emitted alpha particles, and is localised in a region of up to 80μm from the blood vessel. The RBE of the treatment is in the range 2.5-4, and is calculated as a function of the number of double-strand breaks., Conclusions: Targeted therapies with 211 At are effective within the range of the emitted alpha particles. Abstract

Purpose: Diffusing alpha-emitters radiation therapy (DaRT) is a brachytherapy technique using α-particles to treat solid tumours. The high linear energy transfer (LET) and short range of α-particles make them good candidates for the targeted treatment of cancer. Treatment planning of DaRT requires a good understanding of the dose from α-particles and the other particles released in the 224 Ra decay chain., Methods: The Geant4 Monte Carlo toolkit has been used to simulate a DaRT seed to better understand the dose contribution from all particles and simulate the DNA damage due to this treatment., Results: Close to the seed α-particles deliver the majority of dose, however at radial distances greater than 4 mm, the contribution of β-particles is greater. The RBE has been estimated as a function of number of double strand breaks (DSBs) and complex DSBs. A maximum seed spacing of 5.5 mm and 6.5 mm was found to deliver at least 20 Gy RBE weighted dose between the seeds for RBE DSB and RBE cDSB respectively., Conclusions: The DNA damage changes with radial distance from the seed and has been found to become less complex with distance, which is potentially easier for the cell to repair. Abstract

Objective. Higher energy and intensity radiotherapy beams are being used, in part, due to the increased spatial accuracy of treatments. However, higher intensity beams can result in a larger total dose error, motivating the increasing need for real-time dose monitoring. We are developing a thin, real-time upstream monolithic active pixel sensor based system for beam monitoring with excellent precision on measuring the beam shape. Here we present a method to additionally provide dosimetry by adding thin conversion material in strips to the surface of the detector, a grating structure. Approach. By modulating the thickness of the conversion material to minimally disturb the contamination electron signal while enhancing the photon signal, the difference in these signals can be used to extract a photon-only signal, and hence dose. The simulation software Gate, based on Geant4, is utilised to study whether well functioning gratings can be better made from aluminium or copper and to optimise the thickness of a copper grating. Main results. It is possible to enhance the photon signal by a factor 6.7 (7.7) compared to the bare sensor for a 5.8 (6.7) MV beam, without modulation of the signal due to beam electrons. Abstract

Diffusing alpha-emitters radiation Therapy (DaRT) is an interstitial brachytherapy technique using 224 Ra seeds. For accurate treatment planning a good understanding of the early DNA damage due to α-particles is required. Geant4-DNA was used to calculate the initial DNA damage and radiobiological effectiveness due to α-particles with linear energy transfer (LET) values in the range 57.5-225.9 keV/μm from the 224 Ra decay chain. The impact of DNA base pair density on DNA damage has been modelled, as this parameter varies between human cell lines. Results show that the quantity and complexity of DNA damage changes with LET as expected. Indirect damage, due to water radical reactions with the DNA, decreases and becomes less significant at higher LET values as shown in previous studies. As expected, the yield of complex double strand breaks (DSBs), which are harder for a cell to repair, increases approximately linearly with LET. The level of complexity of DSBs and radiobiological effectiveness have been found to increase with LET as expected. The quantity of DNA damage has been shown to increase for increased DNA density in the expected base pair density range of human cells. The change in damage yield as a function of base pair density is largest for higher LET α-particles, an increase of over 50% for individual strand breaks between 62.7 and 127.4 keV/μm. Abstract

Intensity-modulated radiotherapy is a widely used technique for accurately targeting cancerous tumours in difficult locations using dynamically shaped beams. This is ideally accompanied by real-time independent verification. Monolithic active pixel sensors are a viable candidate for providing upstream beam monitoring during treatment. We have already demonstrated that a Monolithic Active Pixel Sensor (MAPS)-based system can fulfill all clinical requirements except for the minimum required size. Here, we report the performance of a large-scale demonstrator system consisting of a matrix of 2 × 2 sensors, which is large enough to cover almost all radiotherapy treatment fields when affixed to the shadow tray of the LINAC head. When building a matrix structure, a small dead area is inevitable. Here, we report that with a newly developed position algorithm, leaf positions can be reconstructed over the entire range with a position resolution of below ∼200 μm in the centre of the sensor, which worsens to just below 300 μm in the middle of the gap between two sensors. A leaf position resolution below 300 μm results in a dose error below 2%, which is good enough for clinical deployment.

Abstract

Purpose: Between 15 and 30% of all nosocomial bacteremias and sepsis are associated with the use of intravascular devices. Catheter-related bloodstream infections (CRBI) are infections in which the organism identified in the blood is also present on the tip of the catheter itself or in a blood sample taken through it. The aim of the study was to evaluate the role of ultrasound in the diagnosis of infections related to the use of central catheters., Methods: Between January 2018 and June 2019, we carried out a prospective study on 36 patients with a central catheter, such as a central venous catheter (CVC), a central catheter with peripheral insertion (PICC), or a fully implanted central venous catheter (PORT-a-cath) and who had signs and symptoms of infection. These patients were submitted to an ultrasound of the catheter upon arrival in the ward in case of suspected infection, or at the time of the onset of signs and symptoms of infection (if these arose during hospitalization). Patients with a central catheter but without signs and symptoms of infection were not included in the study. The end point of the study was to evaluate sensitivity (SENS), specificity (SPEC), positive and negative predictive value (PPV-NPV) and overall diagnostic accuracy (ODA) of ultrasound in the diagnosis of CRBI through Receiver Operating Characteristic (ROC) curve analysis., Results: US showed a SENS of 94%, a SPEC of 84%, a PPV of 84%, an NPV of 94% and an ODA of 88.8% for the diagnosis of CRBI., Conclusions: Preliminary data from our study show that US of intravascular devices has a high SENS and SPEC in the diagnosis of CRBI, and can, therefore, be used as a valid tool to decide whether to remove the device early or leave it in place., (© 2021. Abstract

The aim of this study was to explore the role of lung ultrasound (LUS) in the diagnosis of SARS-CoV-2 infection and to verify its utility in the prediction of lung disease's severity and outcome. Fifty-three consecutive patients presenting to the Emergency Department of Santa Maria delle Grazie Hospital with high suspicion of SARS-CoV-2 infection underwent diagnostic test for SARS-CoV-2 on samples obtained from nasopharyngeal swab as well as complete proper diagnostic work-up that included clinical evaluation, laboratory tests, blood gas analyses, chest CT and LUS. A semiquantitative analysis of B-lines distribution was performed to calculate the LUS score. Patients were divided into two groups according to the results of both SARS-CoV-2 diagnostic test and other exams (Group A = pneumonia due to SARS-CoV2 infection vs Group B = no SARS-CoV2 infection and another definite diagnosis). LUS showed an excellent accuracy in predicting the diagnosis of SARS-CoV-2 infection (area under the ROC curve of 0.92 with a sensibility of 73% and a specificity of 89% a the cut-off of 12.5). LUS score was more impaired in SARS-CoV-2 patients (18.1 ± 6.0 vs 7.6 ± 5.9, p < 0.00001) and it is significantly negatively correlated with PF ratio values (r = - 0.719, p < 0.0001). An intrahospital mortality rate of 46% was found; patients with adverse outcome had significant higher value of LUS, PF, LDH, and APACHE II score. None of these parameters was predictive of mortality. LUS is a useful tool for the early detection of SARS-CoV-2 infection and for the evaluation of the disease severity, but does not predict mortality. Further studies with repeated evaluations of LUS score are needed to further explore the role of LUS in the assessment of severity in SARS-CoV-2 disease and in the monitoring of the response to treatments.

Abstract

Background and Aims: Diabetic retinopathy (DR) is the most common microvascular complication of diabetes. Diabetic macroangiopathies, particularly cardiovascular (CV) diseases, seem closely related to diabetes microvascular complications. Aspirin represents the most prescribed compound in CV prevention. Aspirin impact on DR is still object of debate. As it is already recommended among diabetics at high CV risk, aim of this study was to assess a potential relationship between DR and aspirin therapy, in a type 2 diabetes cohort of patients screened through telemedicine., Methods and Results: NO Blind is a cross-sectional, multicenter, observational study, which involved nine Italian outpatient clinics. Primary endpoint was the assessment of the relationship between aspirin treatment and DR. 2068 patients were enrolled in the study, subsequently split in two subpopulations according to either the presence or absence of DR. Overall, 995 subjects were under aspirin therapy. After adjusting for most common potential confounders, age and gender, aspirin reveals significantly associated with DR (OR: 1.72, 95%CI: 1.58-2.89, p = 0.002) and proliferative DR (PDR) (OR: 1.89, 95%CI: 1.24-2.84, p = 0.003). Association comes lost further adjusting for MACEs (OR: 1.28, 95%CI: 0.85-1.42, p = 0.157) (Model 4) and eGFR (OR: 0.93; 95%CI: 0.71-1.22; p = 0.591) (Model 5)., Conclusion: In this multicenter cross-sectional study including a large sample of outpatients with T2DM, we showed that aspirin was not associated with DR after adjustment for several cardio-metabolic confounders. Abstract

Regenerative medicine is a multidisciplinary field that combines engineering and life science principles to promote regeneration, potentially restoring the physiological condition in diseased tissues. Specifically, the developments of complex grafts enhance the intrinsic regenerative capacity of the host by altering its environment. Autologous micrografts obtained through Rigenera® micrografting technology are able to promote derma and bone regeneration. Androgenetic alopecia (AGA) leads to a progressive thinning of scalp hair affecting 60-70% of the adult population worldwide. Pharmacological treatment offers moderate results and hair transplantation represents the only permanent treatment option. The aim of this study was to demonstrate the role of dermis micrografting in the treatment of AGA by clinical and histological evaluations after 4, 6, and 12 months. Hair growth and density were improved at all indicated times. Those outcomes were also confirmed by the TrichoScan® analysis, reporting an increase of total hair count and density with an increase and reduction of anagen and telogen phases, respectively. Scalp dermoscopic analysis showed an improvement of hair density and histological analysis indicated a clear amelioration of the scalp, development of hair follicles, and a beginning of cuticle formation. Abstract

Background and Aims: Recently, the albuminocentric view of diabetic kidney disease (DKD) in type 2 diabetes (T2DM) has been changing. Therefore, the relationship between diabetic retinopathy (DR) and chronic kidney disease (CKD) has to be addressed according to this new clinical presentation of DKD. The aim of this study was to evaluate, in a real-world setting, the correlation DR-DKD in T2DM., Methods and Results: A total of 2068 type 2 diabetic patients enrolled in a multicenter cross-sectional study were investigated. Albuminuric subjects were largely prevalent among subjects with DR (p = 0.019). In the whole study population, no difference in albumin excretion rate (AER) was observed between presence/absence of DR; instead, AER was significantly higher among patients with glomerular filtration rate (eGFR) < 60 mL/min/1.73 m 2 (CKD) (p = 0.009), above all in those with CKD and AER ≥0.03 g/24 h (p = 0.005). Multivariate analysis confirmed that eGFR (O.R. 0.976; 95% C.I.: 0.960-1.028; p < 0.001) and AER (O.R. 1.249; 95% C.I. 1.001-1.619; p = 0.004) were independently associated with DR and HDL-cholesterol (O.R.: 1.042; 95% C.I.: 1.011-1.120; p = 0.014). Additionally, among patients with eGFR <60 mL/min/1.73 m 2 and albuminuria, both eGFR and AER significantly varied between those with/without DR (p = 0.012 and p = 0.005, respectively), and this finding was observed among only albuminuric patients. Analogous results were obtained considering DR classification. AER was significantly higher among subjects with either proliferative DR (PDR) or severe nonproliferative DR (NPDR), with regard to mild NPDR (0.498 and 0.938 g/die vs. 0.101 g/die; p < 0.001, respectively). Similar results were obtained in the specular subgroups., Conclusion: In T2DM with DKD, the AER seems to be related to the presence of DR. Abstract

Muography consists in observing the differential absorption of muons - elementary particles produced through cosmic-ray interactions in the Earth atmosphere - going through the volcano and can attain a spatial resolution of tens of meters. We present here the first experiment of nuclear emulsion muography at the Stromboli volcano. Muons have been recorded during a period of five months by a detector of 0.96 m 2 area. The emulsion films were prepared at the Gran Sasso underground laboratory and were analyzed at Napoli, Salerno and Tokyo scanning laboratories. Our results highlight a significant low-density zone at the summit of the volcano with density contrast of 30-40% with respect to bedrock. The structural setting of this part of the volcanic edifice controls the eruptive dynamics and the stability of the "Sciara del Fuoco" slope, which is affected by recurrent tsunamigenic landslides. Periodical imaging of the summit of the Stromboli volcano such as that provided by muography can become a useful method for studying the evolution of the internal structure of the volcanic edifice.

Abstract

Platelet rich plasma (PRP) and Micrografts containing human follicle mesenchymal stem cells (HF-MSCs) were tried as a potential treatment for androgenetic alopecia (AGA). However, little to no work has yet to be seen wherein the bio-molecular pathway of HF-MSCs or PRP treatments were analyzed. The aims of this work are to report the clinical effectiveness of HF-MSCs and platelet-rich plasma evaluating and reviewing the most updated information related to the bio-molecular pathway. Twenty-one patients were treated with HF-MSCs injections and 57 patients were treated with A-PRP. The Wnt pathway and Platelet derived-growth factors effects were analyzed. 23 weeks after the last treatment with mean hair thickness increments (29 ± 5.0%) over baseline values for the targeted area. 12 weeks after the last injection with A-PRP mean hair count and hair density (31 ± 2%) increases significantly over baseline values. The increment of Wnt signaling in Dermal Papilla Cells evidently is one of the principal factors that enhances hair growth. Signaling from mesenchymal stem cells and platelet derived growth factors positively influences hair growth through cellular proliferation to prolong the anagen phase (FGF-7), inducing cell growth (ERK activation), stimulating hair follicle development (β-catenin), and suppressing apoptotic cues (Bcl-2 release and Akt activation)., Competing Interests: The authors declare no conflict of interest.

Aims: To assess the correlation between diabetic retinopathy (DR) and potential risk factors, as well as the relationship between DR and the other complications of diabetes, in a real-life population of type 2 diabetes patients recruited in several centres in Italy., Methods: The NO BLIND is a cross-sectional, multicentre, observational study, which involved nine public outpatient clinics in Italy. The patients were assessed for eligibility from November 2016 till November 2017. Those enrolled underwent standard fundus oculi exam. Clinical and laboratory data were also collected., Results: 2068 T2DM underwent fundus oculi exam. 435 received diagnosis of diabetic retinopathy (21%). Diabetic retinopathy was independently associated with HDL cholesterol (O.R.: 1.042; 95% C.I.: 1.012-1.109; p = 0.004), Albumin Excretion Rate (AER) (O.R.: 1.001; 95% C.I.: 1.000-1.002; p = 0.034) and GFR (O.R.: 1.159; 95% C.I.: 1.039-1.294; p = 0.008). HDL cholesterol values were hence split in two classes according to a potential cut-off (40 mg/dL), as defined by the ROC curve. Following analysis confirmed the association between DR and high HDL values (p = 0.032). Aims: Diabetic retinopathy (DR) represents the main cause of blindness among adults in the industrialized Countries. Use of telemedicine could offer an easy, smart specialist fundus oculi examination, as well as putting in a screening programme many patients who otherwise would be excluded., Materials and Methods: The NO BLIND is a transversal, multicentre, observational study. Its pilot phase involved nine public outpatient clinics for 6 months. As endpoint of the study, we assessed the prevalence of DR by retinography in a subset of the Italian population. Patients' fundus oculi photos were performed by trained diabetologists through a digital smart ophthalmoscope., Results: According to our endpoint, in the final study population (n = 1461), obtained excluding patients for whom retinography was not able to provide any diagnosis, DR prevalence was equal to 15.5%. According to the receiver operating characteristic (ROC) curve performed, we can observe how retinography appears a highly accurate method to detect DR (AUROC 0.971, 95% confidence interval, 0.954-0.989), with a specificity of the 100% and a sensitivity of the 94.3%., Conclusions: Our findings, in an Italian setting, confirm main data in the literature about DR prevalence.
