Abstract: The improvement of recycling rates of metal waste, namely those of end-of life vehicles, is nowadays becoming imperative. Aluminum and its alloys are the main metal components in non-ferrous fraction of auto-shredders separated out after sampling. Silicon, Mg, Cu, Zn, Mn, Ti, Fe and Cr are alloying elements, which allowed the identification and differentiation of Al alloys. Flame Atomic Absorption Spectrometry was used for quantification after HCl-HNO3 digestion, with the exception of Si, where HCl–HNO3–HF–H3BO3 digestion was used. Performance characteristics of measurement procedures, namely analytical dynamic ranges, limits of detection and quantification, precision and trueness were evaluated and measurement uncertainty estimated by applying an approach based on precision and trueness validation studies and quality control data. Target values, for repeatability, intermediate precision, trueness (recovery) and measurement uncertainty, were specified to differentiate Al alloys based on their own fit-for-purpose. Metrological traceability of the measurements results of the alloying elements was established by using certified values of British Chemical Standard (BCS) of Al alloys, BCS nº 181/1, BCS Nº 268. BCS Nº 300 and Standard Reference Material of Al-base alloy, SRM 87. The quantification of Si, Mg, Cu, Zn, Mn, Ti, Fe and Cr in aluminum alloys from the non-ferrous fraction of automatic crushers was determined successfully through the validated procedures. info:eu-repo/semantics/publishedVersion

2. Target measurement uncertainty as a tool for validation of uncertainties estimated by different approaches: determination of total hardness in drinking and natural waters

Abstract: In this work, a procedure for total hardness determination in drinking and natural waters, expressed by the equivalent mass concentration of CaCO3, c s , using an in-house complexometric titration with ethylenediamine- tetraacetic acid based on standardized procedure was detailed. Compliance of the procedures requirements with legislation is discussed. Validation and quality control actions were implemented, and a limit of detection of 0.6 mg L -1 was determined fulfilling the target limit of detection of 15 mg L -1 imposed by specific legislation. Measurement precision was estimated under repeatability conditions, where relative standard deviations close to 1 % were estimated, and in within-laboratory reproducibility, where relative standard uncertainties of 4 % and 0.5 % were obtained for cs lower and higher than 100 mg L -1 , respectively. The results were considered unbiased when- ever recoveries were within the range 0.90-1.10, fulfilling the target trueness value stated as 10 % of parametric value. The performance of the measurement procedure was also assessed by proficiency testing and z scores less than 2 were obtained. Measurement uncertainty was estimated by a modeling approach, using the law of propagation of uncertainty for variables not independent, and by empirical ones, such as single-laboratory validation and quality control data and proficiency testing. Measurement uncer- tainty computed from each model was assessed by n score performance indicator showing a good estimated uncer- tainty (|n| \ 2). Finally, a way is proposed to assess the suitability of the combined uncertainty by the different models, comparing them to a target expanded measurement uncertainty within the analytical range, calculated from maximum decreed precision and trueness criteria since no target uncertainties were stated in the legislation.

Abstract: Biochemical oxygen demand after 5 days (BOD5) is a bioassay frequently used to determine the polluting strength of wastewaters. It quantifies the mass concentration of dissolved O2 consumed during an incubation period of 5 days at 20 °C, in the dark, by biological oxidation of organic and/or inorganic matter in wastewater. In this work, the BOD5 measurand is reported as γ(O2). Reporting analytical results with estimation of uncertainty is an ISO/IEC 17025:2005 requirement and allows results to be compared and their fitness for purpose to be demonstrated. The γ(O2) uncertainty was estimated using two models: the individual components’ model (model 1) and the quality-control-criteria model in which the trueness component is calculated using the equation for recovery from CRMs (model 2.A) and the standard deviation of all recoveries (model 2.B). Model 1 provides a realistic strategy for estimation of γ(O2) uncertainty and for assessment of performance. Models 2.A and 2.B are expeditious and simple procedures for the 100–800 mg L−1 range. 10. Evaluation of acid matrix effects in the determination of major elements in biomass by atomic absorption spectrometry from an environmentally friendly point of view

Abstract: The estimation of major element content in solid biofuels is required for prediction and prevention of eventual ash-related problems during combustion. These analyses have to be achieved with minimum impact on the environment. The quantitation of Al, Ca, Mg, Na, K, Fe, Si and Mn in biofuels was carried out according to EN 15290 using acid decomposition of solid samples followed by atomic absorption spectrometry (AAS). A microwave-assisted acid digestion with a HNO3/H2O2/HF mixture was used, followed by HF complexation using H3BO3. Due to the presence of tetrafluoroboric acid complex in the digestion solution, matrix effects were noticed during elemental quantification by AAS. Standard addition calibration methods did not compensate for this matrix effect. Matrix effects that constrain an analytical response may be overcome by applying the procedure used for samples to the calibration standards using the same reagents. However, this entails using large amounts of toxic reagents. In this work, the fluoric–boric acid matrix matching was assessed statistically using one-way ANOVA tests. For the seven groups of nitric acid and reagent blank (HNO3/H2O2/HF/H3BO3) mixtures used, ranging from 0 to 1 volume ratios, no significant differences were observed for Si, Al, Fe and Mn. The calculated F values were lower than the critical value, F 6,14 = 2.85 (p = 0.05). However, for Ca, Mg, Na and K, significant differences were observed. Tenfold dilution was used for samples where the mass fraction exceeded the analytical dynamic range of the AAS instrument. The calibration solutions were prepared using the reagent blanks in the same proportion thus decreasing the amount of acids used. The procedure was validated using SRM 1573a—tomato leaves—purchased from the National Institute of Standards and Technology. Target recoveries of (1 ± 0.1) were achieved.

11. Procedure validation and laboratory performance monitoring for the measurement of moisture, ash and volatile matter mass fractions in solid biofuels

Abstract: Participation in interlaboratory comparisons is a requirement of the accreditation bodies for granting laboratory accreditation by EN ISO/IEC 17025:2005 as external quality control. Proficiency testing (PT) is used to demonstrate the individual performance of a laboratory for a specific test or measurement. Monitoring PT performance over time shows the continuing performance and allows the identification of potential problems related to random and systematic errors. In this study, it is shown that the procedures applied to measure mass fractions of moisture, ash and volatile matter in solid fuels fulfill the conditions stated in the European standards EN 14774-3, EN 14775 and EN 15148:2009 regarding target uncertainty, repeatability and reproducibility. Repeatability was assessed from sample duplicate analysis and combined standard uncertainty. Limits of detection and quantification were also estimated although no target values are stated. The obtained values fulfill the requirements for specifications and classes of solid biofuels. Laboratory performance over the time from 2011 to 2013 regarding such measurements was evaluated with 18 materials provided by WEPAL within the BIMEP program. Apart from monitoring the individual z-scores, their sequence was checked with summarizing parameters including the ‘rescaled sum z-scores’, RSZ, the ‘sum squared z-scores’, SSZ, and J-scores. For none of the analytes, the parameters indicated a trend over time and therefore it was not necessary to trigger any investigation or correcting procedure.

Abstract: Oral anticoagulant therapy remains one of the most frequent options for treatment and prevention in patients with arterial and venous thromboembolism. Clinical guidelines have been updated in recent years by various associations such as the American Heart Association (AHA), the American College of Cardiology (ACC) and the European Society of Cardiology (ESC), as well as organizations in several other countries. The authors present a review of therapy with vitamin K antagonists, focusing on their mechanism of action and metabolism, as well as on the fundamentals of such therapy. Clinical recommendations for the most frequent indications are described. One of the most important issues is the use of these drugs for atrial fibrillation therapy, which is a common indication. Prosthetic valvular disease is a compelling indication for anticoagulation, for which there is a broad consensus. Ischemic heart disease is another indication described for oral anticoagulation. Several practical issues in cardiac patients are discussed. These include the appropriate initial dose, schemes for reversal of anticoagulation, and management of surgical patients. Finally, risk factors for deep venous thrombosis and pulmonary embolism are detailed in this review, presenting current clinical recommendations for oral anticoagulation of these patients.

Abstract: Introduction: Pulmonary hypertension (PH) covers a group of conditions characterized by an increase in pulmonary vascular resistance leading to right ventricular failure. Risk stratification is crucial for adequate prognostic and therapeutic assessment. However, the accuracy of conventional parameters is limited, especially biomarkers., Objectives: To determine the prognostic value of new biomarkers and their combination in a multi-biomarker approach to predict outcome in patients with PH., Methods: In this prospective cohort study, PH patients underwent clinical, echocardiographic and laboratory assessment, including quantification of serum N-terminal pro-brain natriuretic peptide (NT-proBNP) and of the following new biomarkers: mid-regional pro-adrenomedullin (MR-proADM), copeptin, endothelin-1, mid-regional pro-atrial natriuretic peptide (MR-proANP) and soluble ST2 (sST2), the interleukin-33 receptor. The accuracy of the different parameters for predicting all-cause mortality and death or hospitalization of cardiac causes was determined. The prognostic value of a multi-biomarker score based on the tertile distribution of serum NT-proBNP, MR-proANP, renin and sST2 was compared to conventional markers., Results: Forty-three patients (72.1% female, age 59±15 years) were included, most of whom (65.1%) had group 1 PH. During a median follow-up of 34 months, 26% of the patients died and 35% were hospitalized for cardiac causes. Atrial and ventricular dimensions and right ventricular fractional area change were prognostic predictors. Log NT-proBNP (HR: 31.14; 95% CI: 3.12-310.7; p=0.003) and renin (HR: 1.02; 95% CI: 1.005-1.038; p=0.009) were independent predictors of mortality. MR-proANP (HR: 1.008; 95% CI 1.004-1.011; p<0.001) and sST2 (HR: 1.005; 95% CI 1.001-1.009; p=0.04) were predictors of death or hospitalization. The prognostic value of the multi-biomarker score was higher than any of the conventional parameters, and enabled identification of risk groups (the high-risk group had three-year mortality of 77.8%)., Conclusion: A multi-biomarker approach was superior for risk stratification to any single marker. Abstract: Background: MicroRNAs (miRNAs) are key players in cardiovascular development and disease. However, not only miRNAs of a cardiac origin have a critical role in heart function. Recent studies have demonstrated that miR-122-5p, a hepatic miRNA, increases in the bloodstream during ischemic cardiogenic shock and it is upregulated in the infarcted myocardium. The aim of the present study was to determine the potential of circulating miR-122-5p as a biomarker for early prognostic stratification of ST-segment elevation acute myocardial infarction (STEMI) patients., Methods and results: One hundred and forty-two consecutive STEMI patients treated with primary angioplasty were included in the study. Serum levels of miR-1-3p, -122-5p, -133a-3p, -133b, -208b-3p and -499a-5p were measured at the time of cardiac catheterization by quantitative polymerase chain reaction and related to in-hospital and long-term outcome. During a follow up of 20.8 months, 9 patients died, 6 had recurrence of myocardial infarction, and 26 patients suffered an adverse cardiovascular event. Event-free survival was significantly worse in patients with a higher miR-122-5p/133b ratio (3rd tertile distribution, above 1.42 Log(10)), having almost a 9-fold higher risk of death or myocardial infarction and a 4-fold higher risk of adverse cardiovascular events., Conclusions: This study showed that the miR-122-5p/133b ratio is a new prognostic biomarker for the early identification of STEMI patients at a higher risk of developing major adverse events after undergoing primary percutaneous coronary intervention. Abstract: Background: Pulmonary arterial hypertension (PAH) is a rare, deadly condition. Although risk stratification is extremely important for assessment of prognosis and to guide therapy, there is lack of evidence concerning the role of novel biomarkers. In a pivotal study, we sought to comparatively investigate the predictive power of several new biomarkers in PAH., Methods: Patients with prevalent PAH were enrolled in the study protocol, which included clinical, functional and echocardiographic assessment. Blood samples were collected at baseline for determination of NT-proBNP, CT-proET-1, MR-proANP, MR-proADM, copeptin and troponin I. Patients were clinically followed-up up to 12 months for first occurrence of hospital admission due to PAH-related clinical worsening, heart/lung transplantation or all-cause mortality., Results: Among the 28 included patients the pre-specified end-point occurred in 8 (29% event rate). There were higher baseline levels of CT-proET-1, copeptin, MR-proANP, NT-proBNP and troponin I in patients who reached the composite end-point. They also had larger right atria. In multivariate Cox regression, CT-proET-1 was the only biomarker associated with increased hazard of reaching the primary composite end-point (hazard ratio per tertile increase = 10.1; 95% CI 2.0 to 50.6)., Conclusions: CT-proET-1 provided prognostic information independent of other biomarkers. Importantly, we have provided the first evidence that CT-proET-1 may be superior to commonly used biomarkers., (Copyright © 2013 International Society for Heart and Lung Transplantation. 18. Rheumatoid arthritis and anemia: the impact of different anti-inflammatory therapies on hemoglobin levels. An observational study.

Abstract: Unlabelled: Rheumatoid Arthritis (RA) has been associated with anemia. The treatment of anemia in RA includes blood transfusions, erythropoietic agents and iron supplements. Our observations suggest that the treatment of the inflammatory disease is associated with an improvement of hemoglobin (Hgb) levels in RA patients., Methods: Record review, analyzing anemic RA patients and hemoglobin level changes after intensification of RA therapy. Three initial regimes were identified: corticosteroid (Prednisone), Disease Modifying Anti-rheumatic Drugs (DMARD's) plus Prednisone and for patients with no initial treatment with the initiation of Prednisone and or TNF a inhibitors (Etanercept)., Results: 8 female and 2 male patients with average age 59.7 + 14 years were selected. The results for the group treated with Prednisone initially N=4; showed a 0.9 g/dL increase in Hgb and Hct increase of 3% after addition of Etanercept. Those with DMARDs and corticosteroid regime had an Hgb and Hct increase of 1.2 g/dL and 3.5% after addition of Etanercept. The group with no initial treatment was further divided in two groups. The first was treated with Etanercept and the second was treated with Prednisone, resulting in average Hgb increases of 1.9 g/dl Hgb and 1.8 g/dL, and HCT increase 4% and 3.9% respectively. The global average change in Hgb was 1.3g/dL and HCT increase 3.2%., Conclusion: The intensity of treatment of RA including Etanercept and Prednisone improved cases of mild anemia in rheumatoid arthritis.

19. Nephropathy induced by contrast medium in an hispanic population after coronary angiography.

Abstract: Unlabelled: Contrast medium-induced nephropathy (CIN) is regarded as one of the most important complications after intravascular administration of radiographic contrast media and the third most common cause of hospital-acquired acute kidney injury. Contrast-induced nephropathy may develop in up to10% of patient with normal renal function but may be as high 25% in patient with risk factors., Methods: A total of 192 medical records were obtained, in which 55 patient were eligible by our inclusion and exclusion criteria., Results: A total four cases of CIN (7%) was identified and all were considered high-risk patients. In the patients that developed CIN only 50% received preventive IV hydration therapy in accordance to guidelines of the Contrast-Induced Nephropathy Consensus Panel. In 51 cases CIN was absent (93%). In the absent CIN group 22 were considered high-risk patients (43%) and 29 patients were low risk (57%). In the high-risk group with absent CIN, 77% received preventive IV hydration., Conclusion: Study didn't show diabetes as a direct contributory factor for CIN development, but glycemia was not properly stratified. Screening and preventive strategies were not diligently complied in our institution, so there is a great need to incorporate a protocol in our institution in accordance to published guidelines.

Abstract: Oral anticoagulant therapy remains one of the most frequent options for treatment and prevention in patients with arterial and venous thromboembolism. Clinical guidelines have been updated in recent years by various associations such as the American Heart Association (AHA), the American College of Cardiology (ACC) and the European Society of Cardiology (ESC), as well as organizations in several other countries. The authors present a review of therapy with vitamin K antagonists, focusing on their mechanism of action and metabolism, as well as on the fundamentals of such therapy. Clinical recommendations for the most frequent indications are described. One of the most important issues is the use of these drugs for atrial fibrillation therapy, which is a common indication. Prosthetic valvular disease is a compelling indication for anticoagulation, for which there is a broad consensus. Ischemic heart disease is another indication described for oral anticoagulation. Several practical issues in cardiac patients are discussed. These include the appropriate initial dose, schemes for reversal of anticoagulation, and management of surgical patients. Finally, risk factors for deep venous thrombosis and pulmonary embolism are detailed in this review, presenting current clinical recommendations for oral anticoagulation of these patients.
