Abstract

The mission of WEST (tungsten-W Environment in Steady-state Tokamak) is to explore long pulse operation in a full tungsten (W) environment for preparing next-step fusion devices (ITER and DEMO) with a focus on testing the ITER actively cooled W divertor in tokamak conditions. Following the successful completion of phase 1 (2016-2021), phase 2 started in December 2022 with the lower divertor made entirely of actively cooled ITER-grade tungsten mono-blocks. A boronization prior the first plasma attempt allowed for a smooth startup with the new divertor. Despite the reduced operating window due to tungsten, rapid progress has been made in long pulse operation, resulting in discharges with a pulse length of 100 s and an injected energy of around 300 MJ per discharge. Plasma startup studies were carried out with equatorial boron nitride limiters to compare them with tungsten limiters, while Ion Cyclotron Resonance Heating assisted startup was attempted. High fluence operation in attached regime, which was the main thrust of the first campaigns, already showed the progressive build up of deposits and appearance of dust, impacting the plasma operation as the plasma fluence increased. In total, the cumulated injected energy during the first campaigns reached 43 GJ and the cumulated plasma time exceeded 5 h. Demonstration of controlled X-Point Radiator regime is also reported, opening a promising route for investigating plasma exhaust and plasma-wall interaction issues in more detached regime. This paper summarises the lessons learned from the manufacturing and the first operation of the ITER-grade divertor, describing the progress achieved in optimising operation in a full W environment with a focus on long pulse operation and plasma wall interaction.

Abstract

Extended-spectrum β-lactamase-producing Enterobacterales (E-ESBL) are commensal multidrug-resistant (MDR) bacteria of the digestive tract whose prevalence has risen sharply worldwide and in Europe over the past two decades.To assess digestive carriage at hospital readmission of a large cohort of 2509 patients with E-ESBL carriage over a five-year survey; 833 (33%) patients were readmitted at least once.A retrospective, single-centre survey conducted at a tertiary care hospital in France.Among patients with several hospital readmissions (range: 2-13), the proportion of patients still E-ESBL-colonized at hospital readmission, detected by systematic screening for E-ESBL colonization, was80% within an 18-month period after prior hospitalization with the first E-ESBL isolation.There is a need to reconsider the continuation of systematic screening for E-ESBL colonization because of a high rate of patients still colonized at hospital readmission over a long period of time.

Abstract

Summary We report the investigation to control an Enterobacter cloacae complex outbreak in a neonatal intensive care unit from November 2020 to February 2021. Pulsed-field gel electrophoresis showed that five of eight cases were infected with a clonal strain. Breast pumps, shared among mothers in the unit, could have contributed to the spread of the clonal spread.

Abstract

Resume Introduction Nous disposons, a ce jour, de peu de donnees de prescription des antibiotiques et antifongiques en reanimation pediatrique. Objectif Decrire et comprendre l’evolution de l’utilisation des antibiotiques et antifongiques au sein du service de reanimation pediatrique sur une periode de cinq ans. Materiel et methode Nous avons utilise la methodologie du programme national ATB-RAISIN, permettant de recueillir des donnees sur la consommation d’anti-infectieux et d’obtenir des indicateurs de suivi utilisables au niveau local. Resultats Au total, nous notons une diminution de la consommation d’antibiotiques de 9,28 % entre 2013 et 2017 et une diminution de la consommation d’antifongiques de 19,84 % entre 2013 et 2017. Conclusion Cette etude descriptive a mis en evidence une rationalisation de la prescription des antibiotiques et antifongiques au sein du service de reanimation pediatrique. Une analyse multidisciplinaire a permis de suggerer les causes de cette evolution.

Abstract

Background The overuse of antimicrobials and empirical prescriptions are associated with the higher prevalence of antibiotics’ resistance, leading to the longer duration of illness and increased healthcare costs. To preserve their efficacy and prevent the risks of resistance emergence, surveillance of antibiotic consumption is essential. There are limited data published about antibiotics and antifungal consumption in terms of defined daily doses (DDD) in paediatrics. Purpose To describe and analyse antibiotic and antifungal drug consumption, DDD/1000 bed-days in a paediatric intensive care unit (ICU) over a 5 year period. Material and methods A retrospective and descriptive study was performed in a university paediatric hospital of 400 beds with 32 ICU beds. According to the French ‘ATB-Raisin’ national network methodology, systemic antibiotics and antifungal dispensation from 2013 to 2018 to the ICU were measured and analysed by a multidisciplinary approach. DDD/1000 bed-days and/or ratios were calculated for each antibiotic and antifungal, and overall. Results A 0.9-fold decrease (−9%) in the overall number of antibiotics DDD/1000 bed-days from 2792 in 2013 to 2533 in 2018 was measured. The most important decreases were observed for three classes of antibiotics: penicillin M (ratio=0.05), imipenem (ratio=0.17) and imidazole (ratio=0.28). The most important antibiotics’ consumption increases were observed for classes: first- and second-generation cephalosporins (ratio=2.26), levofloxacin (ratio=2.09) and amoxicillin-clavulanique (ratio=1.64). A 0.8-fold (−19%) decrease in the overall number of antifungals DDD/1000 bed-days from 314 in 2013 to 252 in 2018 was measured. The main decreases were observed for amphotericin B (ratio=0.3), voriconazole (ratio=0.50) and caspofungin (ratio=0.59). The most important increases have been shown for: flucytosine (ratio=10.25), micafungin (ratio=2.73) and fluconazole (ratio=1.22). Fluctuation in consumption is linked to several factors: drug shortages, evolution in recommendations and patient profiles. French drug market supplies break of oxacillin/penicillin M increases first- and second-generation cephalosporin prescriptions. A local guideline for transplant patients recently replaces fluconazole by mycafungin in antifungal prophylaxis. Conclusion Both the overall numbers of antibiotics and antifungals DDD/1000 beds-days decrease over the 5 year study period. A multidisplinary analysis comprehends the consumption evolution in our paediatric ICU. It should be monitored on a continuous basis by pharmacists in healthcare settings. Abstract

International audience; Tore Supra routinely addresses the physics and technology of very long-duration plasma discharges, thus bringing precious information on critical issues of long pulse operation of ITER. A new ITER relevant lower hybrid current drive (LHCD) launcher has allowed coupling to the plasma a power level of 2.7 MW for 78 s, corresponding to a power density close to the design value foreseen for an ITER LHCD system. In accordance with the expectations, long distance (10 cm) power coupling has been obtained. Successive stationary states of the plasma current pro le have been controlled in real-time featuring (i) control of sawteeth with varying plasma parameters, (ii) obtaining and sustaining a `hot core' plasma regime, (iii) recovery from a voluntarily triggered deleterious magnetohydrodynamic regime. The scrape-off layer (SOL) parameters and power deposition have been documented during L-mode ramp-up phase, a crucial point for ITER before the X-point formation. Disruption mitigation studies have been conducted with massive gas injection, evidencing the difference between He and Ar and the possible role of the q = 2 surface in limiting the gas penetration. ICRF assisted wall conditioning in the presence of magnetic eld has been investigated, culminating in the demonstration that this conditioning scheme allows one to recover normal operation after disruptions. The effect of the magnetic eld ripple on the intrinsic plasma rotation has been studied, showing the competition between turbulent transport processes and ripple toroidal friction. During dedicated dimensionless experiments, the effect of varying the collisionality on turbulence wavenumber spectra has been documented, giving new insight into the turbulence mechanism. Turbulence measurements have also allowed quantitatively comparing experimental results with predictions by 5D gyrokinetic codes: numerical results simultaneously match the magnitude of effective heat diffusivity, rms values of density uctuations and wavenumber spectra. A clear correlation between electron temperature gradient and impurity transport in the very core of the plasma has been observed, strongly suggesting the existence of a threshold above which transport is dominated by turbulent electron modes. Dynamics of edge turbulent uctuations has been studied by correlating data from fast imaging cameras and Langmuir probes, yielding a coherent picture of transport processes involved in the SOL.

Abstract

The main results of the Tore Supra experimental programme in the years 2007–2008 are reported. They document significant progress achieved in the domain of steady-state tokamak research, as well as in more general issues relevant for ITER and for fusion physics research. Three areas are covered: ITER relevant technology developments and tests in a real machine environment, tokamak operational issues for high power and long pulses, and fusion plasma physics. Results presented in this paper include test and validation of a new, load-resilient concept of ion cycotron resonance heating antenna and of an inspection robot operated under ultra-high vacuum and high temperature conditions; an extensive experimental campaign (5 h of plasma) aiming at deuterium inventory and carbon migration studies; real-time control of sawteeth by electron cyclotron current drive in the presence of fast ion tails; ECRH-assisted plasma start-up studies; dimensionless scalings of transport and turbulence; transport experiments using active perturbation methods; resistive and fast-particle driven MHD studies. The potential role of Tore Supra in the worldwide fusion programme before the start of ITER operation is also discussed.

Abstract

We report on the multiple wavelength laser monitoring system designed for the CMS lead tungstate crystal calorimeter read-out with avalanche photodiodes (Barrel calorimeters) and vacuum phototriodes (End Cap calorimeters). Results are presented for the test beam performance of the system designed to achieve 0.5% relative inter-calibration of the optical transmittance for lead tungstate scintillation emission over nearly 80 000 channels. The system operates in continuous measurement cycles to follow each crystal?s evolution under irradiation and recovery periods foreseen during operation at the LHC.

Abstract

This paper describes in detail the recent progresses which have been made in Tore Supra for developing a new technique of calibration of the soft x-ray (SXR) detectors in the range 1-30 keV. The diode response as a function of the flux of photons resulted accurately linear over almost three orders of magnitude. Apart from a limited number of deficient detectors, promptly replaced with new ones, the spread of the diode responses (84 detectors) is about 20% total of the average value. It allowed the derivation of the calibration factor for each detector of the tomography system. The effect of the environmental temperature, in the range 15 degrees-40 degrees, has also been studied, revealing that up to 35 degrees the linearity of diode response and these calibration factors remain constant. It demonstrates the capability of discriminating slight and localized changes in the two dimensional spatial distribution of the SXR intensity.

Abstract

In order to improve the resolution in the inking of the dots, vertical anisotropy media configuration is applied to magnetography. The magnetization transition zone length between the dot and the nonmagnetized drum is hoped to be shorter than in the longitudinal configuration. Some preliminary inking tests were successfully carried out on plane samples. A specially designed sputtering group was built for the deposition of CoCr layers. The substrates are full‐size drums (diameter=10 cm, length=40 cm) and the Co80Cr20 target is a cylinder (diameter=20 cm, length=50 cm) around the drum. In this paper, the first results obtained with this group are described. The dispersion of the properties of the CoCr layer around the drum is less than 10%. The whole incident power range (up to 15 kW) was scanned. The deposition rate continuously increases with increasing incident power (Pw) up to 9 kW. Abstract

Équipe 107 : Physique des plasmas chauds; International audience; In parallel to the direct contribution to the procurement phase of ITER and Broader Approach, CEA has initiated research & development programmes, accompanied by experiments together with a significant modelling effort, aimed at ensuring robust operation, plasma performance, as well as mitigating the risks of the procurement phase. This overview reports the latest progress in both fusion science and technology including many areas, namely the mitigation of superconducting magnet quenches, disruption-generated runaway electrons, edge-localized modes (ELMs), the development of imaging surveillance, and heating and current drive systems for steady-state operation. The WEST (W Environment for Steady-state Tokamaks) project, turning Tore Supra into an actively cooled W-divertor platform open to the ITER partners and industries, is presented.

Abstract

Background: Extended-spectrum β-lactamase-producing Enterobacterales (E-ESBL) are commensal multidrug-resistant (MDR) bacteria of the digestive tract whose prevalence has risen sharply worldwide and in Europe over the past two decades., Aim: To assess digestive carriage at hospital readmission of a large cohort of 2509 patients with E-ESBL carriage over a five-year survey; 833 (33%) patients were readmitted at least once., Methods: A retrospective, single-centre survey conducted at a tertiary care hospital in France., Findings: Among patients with several hospital readmissions (range: 2-13), the proportion of patients still E-ESBL-colonized at hospital readmission, detected by systematic screening for E-ESBL colonization, was >80% within an 18-month period after prior hospitalization with the first E-ESBL isolation., Conclusion: There is a need to reconsider the continuation of systematic screening for E-ESBL colonization because of a high rate of patients still colonized at hospital readmission over a long period of time., (Copyright © 2022 The Healthcare Infection Society. Abstract

We conducted a retrospective study from 2005 to 2019 to describe the epidemiology and mortality of enterobacterial producing extended-spectrum β-lactamase (E-ESBL) infections in our university hospital over a 17-year period of time. Clinical and microbiological data were extracted from different software used for continuous surveillance. Stool samples from systematic screening for E-ESBL colonization were excluded from the study. The incidence rate of infected patient was calculated by E-ESBL species and by year. A comparison of mortality rate in patients with bloodstream infections versus other types of infections was conducted using a Kaplan-Meier method survival curves. A log rank test (with a risk of 5%) was carried out. A total of 3324 patients with E-ESBL infection were included with an increased incidence density per 1000 days of hospitalization from 0.03 in 2005 to 0.47 in 2019. Escherichia coli was the most frequently isolated pathogen (64%). Global mortality rate was significantly higher with E. coli than with Klebsiella spp. and Enterobacter spp. (p < 0.001). Mortality was higher in patients with E-ESBL bloodstream infection than in patients with other type of E-ESBL infection (p < 0.001). Our study showed a significant increase of the E-ESBL incidence density over a 17-year period survey with a higher mortality in patients with E-ESBL bacteremia. This highlights the need to continue efforts to control the spread of these multi-resistant bacteria in our institution., (© 2022. Abstract

We report the investigation to control an Enterobacter cloacae complex outbreak in a neonatal intensive care unit from November 2020 to February 2021. Pulsed-field gel electrophoresis showed that five of eight cases were infected with a clonal strain. Abstract

Systemic use of voriconazole (VCZ) might be restricted by adverse events, such as hepatotoxicity and neurotoxicity, or drug-drug interactions. Topical VCZ application to skin may help to treat local infection more effectively and limit unwanted whole-body exposure. Topical VCZ cream was stable for 90 days when refrigerated. 22. An original calibration technique for soft x-ray detectors and its use in the Tore Supra tomographic system.

Abstract

This paper describes in detail the recent progresses which have been made in Tore Supra for developing a new technique of calibration of the soft x-ray (SXR) detectors in the range 1-30 keV. The diode response as a function of the flux of photons resulted accurately linear over almost three orders of magnitude. Apart from a limited number of deficient detectors, promptly replaced with new ones, the spread of the diode responses (84 detectors) is about 20% total of the average value. It allowed the derivation of the calibration factor for each detector of the tomography system. The effect of the environmental temperature, in the range 15 degrees-40 degrees, has also been studied, revealing that up to 35 degrees the linearity of diode response and these calibration factors remain constant. It demonstrates the capability of discriminating slight and localized changes in the two dimensional spatial distribution of the SXR intensity.
