1. Validation of the ERO2.0 code using W7-X and JET experiments and predictions for ITER operation

Abstract

The paper provides an overview of recent modelling of global material erosion and deposition in the fusion devices Wendelstein 7-X (W7-X), JET and ITER using the Monte-Carlo code ERO2.0. For validating the modelling tool in a three-dimensional environment, W7-X simulations are performed to describe carbon erosion from the graphite test divertor units, which were equipped in operational phase OP 1.2 and analysed post-mortem. Synthetic spectroscopy of carbon line emission is compared with experimental results from the divertor spectrometer measurement system, showing a good agreement in the e-folding lengths in the radial intensity profiles of carbon. In the case of metallic wall materials, earlier modelling of the Be/W environment in JET and ITER is revisited and extended with an updated set of sputtering and reflection data, as well as including the mixing model for describing the Be/W dynamics in the divertor. Motivated by recent H/D/T isotope experiments in JET, limited and diverted configuration pulses are modelled, showing the expected trend of both Be and W erosion increasing with isotope mass. For the JET diverted configuration pulses, it is shown that Be migrates predominantly to the upper part of the inner divertor where it initially leads to strong W erosion. With longer exposure time, the growth of a Be deposited layer leads to a reduction of W erosion in that region. A similar trend is observed in simulations of the ITER baseline Q = 10 scenario, however with a more symmetric Be migration pattern leading to deposition also on the outer divertor.

Abstract

An integral approach to plasma-wall interaction (PWI) modelling for DEMO is presented, which is part of the EUROfusion Theory and Advanced Simulation Coordination activities that were established to advance the understanding and predictive capabilities for the modelling of existing and future fusion devices using a modern advanced computing approach. In view of the DEMO design, the aim of PWI modelling activities is to assess safety-relevant information regarding the erosion of plasma-facing components (PFCs), including its impact on plasma contamination, dust production, fuel inventory, and material response to transient events. This is achieved using a set of powerful and validated computer codes that deal with particular PWI aspects and interact with each other by means of relevant data exchange. Steady state erosion of tungsten PFC and subsequent transport and re-deposition of eroded material are simulated with the ERO2.0 code using a DEMO plasma background produced by dedicated SOLPS-ITER simulations. Dust transport simulations in steady state plasma also rely on the respective SOLPS-ITER solutions and are performed with the MIGRAINe code. In order to improve simulations of tungsten erosion in the divertor of DEMO, relevant high density sheath models are being developed based on particle-in-cell (PIC) simulations with the state-of-the-art BIT code family. PIC codes of the SPICE code family, in turn, provide relevant information on multi-emissive sheath physics, such as semi-empirical scaling laws for field-assisted thermionic emission. These scaling laws are essential for simulations of material melting under transient heat loads that are performed with the recently developed MEMENTO code, the successor of MEMOS-U. Fuel retention simulations assess tritium retention in tungsten and structural materials, as well as fuel permeation to the coolant, accounting for neutron damage. Simulations for divertor monoblocks of different sizes are performed using the FESTIM code, while for the first wall the TESSIM code is applied. Respective code-code dependencies and interactions, as well as modelling results achieved to date are discussed in this contribution.

3. Arecibo measurements of D-region electron densities during sunset and sunrise: implications for atmospheric composition

Abstract

Earth's lower ionosphere is the region where terrestrial weather and space weather come together. Here, between 60 and 100 km altitude, solar radiation governs the diurnal cycle of the ionized species. This altitude range is also the place where nanometre-sized dust particles, recondensed from ablated meteoric material, exist and interact with free electrons and ions of the ionosphere. This study reports electron density measurements from the Arecibo incoherent-scatter radar being performed during sunset and sunrise conditions. An asymmetry of the electron density is observed, with higher electron density during sunset than during sunrise. This asymmetry extends from solar zenith angles (SZAs) of 80 to 100∘. This D-region asymmetry can be observed between 95 and 75 km altitude. The electron density observations are compared to the one-dimensional Sodankylä Ion and Neutral Chemistry (SIC) model and a variant of the Whole Atmosphere Community Climate Model incorporating a subset SIC's ion chemistry (WACCM-D). Both models also show a D-region sunrise–sunset asymmetry. However, WACCM-D compares slightly better to the observations than SIC, especially during sunset, when the electron density gradually fades away. An investigation of the electron density continuity equation reveals a higher electron–ion recombination rate than the fading ionization rate during sunset. The recombination reactions are not fast enough to closely match the fading ionization rate during sunset, resulting in excess electron density. At lower altitudes electron attachment to neutrals and their detachment from negative ions play a significant role in the asymmetry as well. A comparison of a specific SIC version incorporating meteoric smoke particles (MSPs) to the observations revealed no sudden changes in electron density as predicted by the model. However, the expected electron density jump (drop) during sunrise (sunset) occurs at 100∘ SZA when the radar signal is close to the noise floor, making a clear falsification of MSPs' influence on the D region impossible.

4. Modelling the influence of meteoric smoke particles on artificial heating in the D-region

Abstract

We investigate if the presence of meteoric smoke particles (MSPs) influences the electron temperature during artificial heating in the D-region. By transferring the energy of powerful high-frequency radio waves into thermal energy of electrons, artificial heating increases the electron temperature. Artificial heating depends on the height variation of electron density. The presence of MSPs can influence the electron density through charging of MSPs by electrons, which can reduce the number of free electrons and even result in height regions with strongly reduced electron density, so-called electron bite-outs. We simulate the influence of the artificial heating by calculating the intensity of the upward-propagating radio wave. The electron temperature at each height is derived from the balance of radio wave absorption and cooling through elastic and inelastic collisions with neutral species. The influence of MSPs is investigated by including results from a one-dimensional height-dependent ionospheric model that includes electrons, positively and negatively charged ions, neutral MSPs, singly positively and singly negatively charged MSPs, and photochemistry such as photoionization and photodetachment. We apply typical ionospheric conditions and find that MSPs can influence both the magnitude and the height profile of the heated electron temperature above 80 km; however, this depends on ionospheric conditions. During night, the presence of MSPs leads to more efficient heating and thus a higher electron temperature above altitudes of 80 km. We found differences of up to 1000 K in electron temperature for calculations with and without MSPs. When MSPs are present, the heated electron temperature decreases more slowly. The presence of MSPs does not much affect the heating below 80 km for night conditions. For day conditions, the difference between the heated electron temperature with MSPs and without MSPs is less than 25 K. We also investigate model runs using MSP number density profiles for autumn, summer and winter. The night-time electron temperature is expected to be 280 K hotter in autumn than during winter conditions, while the sunlit D-region is 8 K cooler for autumn MSP conditions than for the summer case, depending on altitude. Finally, an investigation of the electron attachment efficiency to MSPs shows a significant impact on the amount of chargeable dust and consequently on the electron temperature.

5. Dust sputtering within the inner heliosphere: a modelling study

Abstract

The aim of this study is to investigate through modelling how sputtering by impacting solar wind ions influences the lifetime of dust particles in the inner heliosphere near the Sun. We consider three typical dust materials, silicate, Fe0.4Mg0.6O, and carbon, and describe their sputtering yields based on atomic yields given by the Stopping and Range of Ions in Matter (SRIM) package. The influence of the solar wind is characterized by plasma density, solar wind speed, and solar wind composition, and we assume for these parameter values that are typical for fast solar wind, slow solar wind, and coronal mass ejection (CME) conditions to calculate the sputtering lifetimes of dust. To compare the sputtering lifetimes to typical sublimation lifetimes, we use temperature estimates based on Mie calculations and material vapour pressure derived with the MAGMA chemical equilibrium code. We also compare the sputtering lifetimes to the Poynting–Robertson lifetime and to the collision lifetime. We present a set of sputtering rates and lifetimes that can be used for estimating dust destruction in the fast and slow solar wind and during CME conditions. Our results can be applied to solid particles of a few nanometres and larger. The sputtering lifetimes increase linearly with the size of particles. We show that sputtering rates increase during CME conditions, primarily because of the high number densities of heavy ions in the CME plasma. The shortest sputtering lifetimes we find are for silicate, followed by Fe0.4Mg0.6O and carbon. In a comparison between sputtering and sublimation lifetimes we concentrate on the nanodust population. The comparison shows that sublimation is the faster destruction process within 0.1 AU for Fe0.4Mg0.6O, within 0.05 AU for carbon dust, and within 0.07 AU for silicate dust. The destruction by sputtering can play a role in the vicinity of the Sun. We discuss our findings in the context of recent F-corona intensity measurements onboard Parker Solar Probe.

6. Erosion estimates for the divertor and main wall components from STEP

Abstract

The tungsten erosion within Spherical Tokamak for Energy Production (STEP) assuming tungsten main wall and tungsten divertor has been estimated with ERO at the inner and outer divertor, at the inner and outer midplane and at the outboard baffle entrance. Plasma parameters are based on SOLPS simulations applying argon puffing for edge cooling. The plasma parameter range covers peak electron temperatures T _e between 3 and 25 eV in the divertor. At the inner midplane T _e ∼ 13 eV, at the outer midplane ∼7 eV and at the outboard baffle entrance between 1 eV and 4 eV. The modelled peak gross erosion is highest in the divertor with up to 1E19 W m ^−2 s ^−1 within the inner and 7E19 W m ^−2 s ^−1 in the outer one for the plasma parameter range studied. At the main wall the gross erosion is about 2E18 W m ^−2 s ^−1 at the inner midplane and 1.3E17 W m ^−2 s ^−1 at the outer one. However, tungsten deposition within the divertor is much larger with amounts between 88% and 98% and only between 10% and 60% at the midplane. At all locations studied, tungsten erosion due to deuterium ions is negligibly small compared to the erosion by argon ions. Erosion due to deuterium atoms has been studied for the outer midplane and is there at least four times smaller than the erosion due to argon ions. The simulations have been performed considering singly ionised Ar. However, according to the SOLPS runs the mean charge of Ar impinging the surfaces is about two at the locations of largest erosion, which leads to an increase of the gross erosion by a factor between 1.5 and 5 with the largest increase occurring at the outer divertor target.

7. Suppression of errors in simulated ultrarelativistic bunch propagation using the X-dispersionless Maxwell solver

Abstract

Because of the inherent value of high-energy particle beams for the study of quantum electrodynamics effects, it is of great importance to accurately model the physics in numerical simulations. Numerical effects may alter the dynamics of a simulation and may change the physics of energy losses on account of the radiation reaction (RR) force. In this work, the numerical Cherenkov effect in combination with the RR force are analyzed in the vacuum propagation of an ultrarelativistic electron bunch. It is revisited that the use of the standard Yee solver in the present setup triggers the numerical Cherenkov instability. With the instability at hand, the simulation results of the Yee solver are compared to data obtained by the X-dispersionless Maxwell solver, also known as the rhombi-in-plane (RIP) solver. There, the numerical instability is suppressed by several orders of magnitude. Afterward, the impact of radiation reaction on the dynamics is studied for both cases. It is shown that the combination of the Yee solver and the RR force enhances the error significantly and the electron bunch loses about 90% of its energy as a result. These huge energy losses can be observed only if both the Lorentz force and the RR force are enabled in the code. In contrast, the RIP Maxwell solver is not plagued by these issues and accurately calculates the dynamics of the electron bunch.

8. Ion acceleration and D-D fusion neutron generation in relativistically transparent deuterated nanowire arrays

Abstract

Laser irradiation of deuterated polyethylene nanowire arrays at intensities of 5×10^{19}Wcm^{–2} was recently reported to accelerate deuterons to multi-MeV energies, resulting in microscale fusion. Here we show that irradiation of deuterated nanowire targets at intensities of ∼3×10^{21}Wcm^{–2} with high contrast λ=400nm laser pulses of 45 fs duration leads to different plasma dynamics in which the tip of the nanowires rapidly explodes to form an overdense plasma, but the onset of relativistic transparency allows the ultrashort laser pulse to penetrate deep into the nanowire array, enhancing particle acceleration. Experiments and particle-in-cell simulations show that ions are accelerated in the laser beam backward direction to energies up to 13 MeV by a target normal sheath acceleration (TNSA) field that develops in the front of the target, forming collimated beams characterized by full wave at half maximum divergence as low as 7.5°. The simulations also show that deeper within the nanowire array ions are accelerated radially to MeV energies by an internal TNSA field normal to the nanowire surfaces. These radially accelerated deuterons collide with other D atoms, as do those directed toward the CD_{2} substrate, leading to D-D fusion reactions. Irradiation with 8 J laser pulses is measured to generate up to 1.2×10^{7} D-D fusion neutrons per shot. The simulations suggest the use of a thinner substrate would also accelerate ions originating from a small area in the back of the target in the laser forward direction.

9. Secondary electron emission from meteoric smoke particles inside the polar ionosphere

Abstract

The charging by secondary electron emission (SEE) from particles is known as a significant charging process in astrophysical plasmas. This work aims at evaluating the significance of SEE for charging of meteoric smoke particles (MSPs) in the Earth's polar atmosphere. Here, the atmosphere is subject to a bombardment of energetic electrons from the magnetosphere (and partly the sun). We employ the SEE formalism to MSPs in the upper mesosphere using electron precipitation fluxes for three different precipitation strengths. In addition, we address the possible effect of tertiary electron emission (TEE) from MSPs induced by atmospheric secondary electrons for one precipitation case. The SEE and TEE rates from MSPs of different sizes are compared to plasma attachment and photodetachment and photoionization rates of MSPs. The needed concentration of electrons and ions have been modeled with the Sodankylä Ion and Neutral Chemistry (SIC) model with included electron precipitation spectra as an additional ionization source. We find that secondary electron emission from MSPs is not a relevant charging mechanism for MSPs. The electron attachment to MSPs and photodetachment of negatively charged MSPs are the most important processes also during energetic electron precipitation.

Abstract

Current discussions about a practice turn in human geography raise questions about whether discourse analytical approaches are focusing too strongly on symbolic representations. However, practice-theoretical conceptions privilege the analysis of everyday practices, discussing issues of embodiment and materiality. Following the considerations of Ludwig Wittgenstein, we assume certain connections between these two fields of study and explore the links between discourse and practice theory by employing the Wittgensteinian concepts of language-games and rule-following. Focussing on the discourse theory of Laclau and Mouffe and the practice theory of Schatzki, we show to what extend both theories are informed by Wittgenstein's work and can be seen as different types of language-games analyses. We highlight the similarities and discuss the differences in the basic interests of these theories. Finally, we introduce an integrative „Wittgensteinian approach“ that is illustrated by two empirical examples of geographical inquiry – practices of a new rurality and rap-music in stigmatized banlieus.

Abstract

In high-energy collisions, the spatio-temporal size of the particle production region can be measured using the Bose–Einstein correlations of identical bosons at low relative momentum. The source radii are typically extracted using two-pion correlations, and characterize the system at the last stage of interaction, called kinetic freeze-out. In low-multiplicity collisions, unlike in high-multiplicity collisions, two-pion correlations are substantially altered by background correlations, e.g. mini-jets. Such correlations can be suppressed using three-pion cumulant correlations. We present the first measurements of the size of the system at freeze-out extracted from three-pion cumulant correlations in pp, p–Pb and Pb–Pb collisions at the LHC with ALICE. At similar multiplicity, the invariant radii extracted in p–Pb collisions are found to be 5–15% larger than those in pp, while those in Pb–Pb are 35–55% larger than those in p–Pb. Our measurements disfavor models which incorporate substantially stronger collective expansion in p–Pb as compared to pp collisions at similar multiplicity.

Abstract

Transverse momentum spectra of π±, K± and p(p¯) up to pT=20 GeV/c at mid-rapidity in pp, peripheral (60–80%) and central (0–5%) Pb–Pb collisions at sNN=2.76 TeV have been measured using the ALICE detector at the Large Hadron Collider. The proton-to-pion and the kaon-to-pion ratios both show a distinct peak at pT≈3 GeV/c in central Pb–Pb collisions. Below the peak, pT10 GeV/c particle ratios in pp and Pb–Pb collisions are in agreement and the nuclear modification factors for π±, K± and p(p¯) indicate that, within the systematic and statistical uncertainties, the suppression is the same. This suggests that the chemical composition of leading particles from jets in the medium is similar to that of vacuum jets.

Abstract

This work investigates the influence of meteoric smoke particles (MSP) on the charge balance in the D-region ionosphere. Both experimental in situ measurements and a one-dimensional ionospheric model reveal a clear impact of MSP on the ionospheric composition of the D-region. The study reviews rocket-borne in situ measurements of electron and positive ion density, which show a distinct deficit of electrons in comparison to positive ions between 80 and 95 km. This deficit can be explained by the ambient negatively charged MSP measured simultaneously with a Faraday cup. The influence of MSP on the D-region charge balance is addressed with a simplified ionospheric model with only six components, i.e. electrons, positive and negative ions and neutral and charged MSP (both signs). The scheme includes reactions of plasma captured by MSP and MSP photo reactions as well as the standard ionospheric processes, e.g. ion-ion recombination. The model shows that the capture of plasma constituents by MSP is an important process leading to scavenging of electrons. Since Faraday cup measurements are biased towards heavy MSP because of aerodynamical filtering, we have applied an estimate of this filter on the modelled MSP densities. By doing that, we find good qualitative agreement between the experimental data and our model results. In addition, the model study reveals an increase of positive ions in the presence of MSP. That is primarily caused by the reduced dissociative recombination with electrons which have been removed from the gas phase by the MSP.

Abstract

The ECOMA (Existence of Charge state Of meteoric smoke particles in the Middle Atmosphere) sounding rocket campaign was conducted during the Geminid meteor shower in December 2010 in order to explore whether there is a change of the properties of meteoric smoke particles due to the stream. In parallel to the rocket flights, three radars monitored the Geminid activity located at the launch site in Northern Norway and in Northern Germany to gain information about the meteor flux into the atmosphere. The results presented here are based on specular meteor radar observations measuring the radiant position, the velocity and the meteor flux into the atmosphere during the Geminids. Further, the MAARSY (Middle Atmosphere Alomar Radar System) radar was operated to conduct meteor head echo experiments. The interferometric capabilities of MAARSY permit measuring the meteor trajectories within the radar beam and to determine the source radiant and geocentric meteor velocity, as well as to compute the meteor orbit.

Radiation oncology (RO) has seen declines in Medicare reimbursement (MCR). However, there are no recent studies analyzing the contributions of specific billing codes to overall RO reimbursement. We compared total MCR for specific Healthcare Common Procedure Coding System (HCPCS) codes in 2019 with MCR for those codes in 2010 and 2015, corrected for inflation, to see how the same basket of RO services in 2019 would have been reimbursed in 2010 and 2015 (adjusted MCR).The Centers for MedicareMedicaid Services Physician/Supplier Procedure Summary database was used to obtain MCR data for RO HCPCS codes in 2010, 2015, and 2019. For each code, the total allowed charge was divided by the number of submitted claims to calculate the average MCR per claim in 2010, 2015, and 2019. The 2019 billing frequency for each code was then multiplied by the inflation-adjusted average MCR for those codes in 2010 and 2015 to determine what the MCR would have been in 2010 and 2015 using 2019 dollars and utilization rates. Results were compared with actual 2019 MCR to calculate the projected difference.Total inflation-adjusted RO MCR was $2281 million (M), $1991 M, and $1848 M in 2010, 2015, and 2019 respectively. This represents a cut of $433 M (19%) and $143 M (7%) from 2010 and 2015, respectively, to 2019. After utilization adjustment, total reimbursement was $2534 M, $2034 M, and $1848 M for 2010, 2015, and 2019, respectively, representing a cut of $686 M (27%) and $186 M (9%) from 2010 and 2015, respectively, to 2019. Intensity modulated radiation therapy (IMRT) treatment delivery and planning accounted for $917 M (36%), $670 M (33%), and $573 M (31%) of the adjusted MCR in 2010, 2015, and 2019, respectively.Medicare reimbursement decreased substantially from 2010 to 2019. A decline in IMRT treatment reimbursement was the primary driver of MCR decline. When considering further cuts, policymakers should consider these trends and their consequences for health care quality and access.

Abstract

There is evidence of practice consolidation in US health care in recent years. To our knowledge, a detailed quantitative study of recent changes in radiation oncology practice size has not been performed. We aim to evaluate radiation oncology practice size changes between 2012 and 2020 in the United States.Using the Medicare Physician Compare Database, we identified practices employing radiation oncologists using their taxpayer identification number and individual radiation oncologists using their national provider identifier. We grouped individual radiation oncologists into categories by practice size (which includes the number of physicians of all specialties) and compared the number of radiation oncologists in each category between 2012 and 2020. Further analyses by US geographic census region, single-specialty practice, academic practice, and high- and low-population density areas were performed.Between 2012 and 2020, the total number of practicing radiation oncologists increased by 9%, and the number of practices employing radiation oncologists decreased by 11.5%. The number of radiation oncologists in practices of size 1 to 2, 3 to 9, 10 to 24, and 25 to 49 decreased by 3.7%, 4.7%, 4.9%, and 2%, respectively, and the number of radiation oncologists in practices of size 50 to 99, 100 to 499, and 500+ increased by 1.4%, 2.1%, and 11.8%, respectively (all 500+ practices are multispecialty groups). The increase in practice size was significant in all regions, for single-specialty and multispecialty practices, academic and nonacademic practices, and for practices in high-, middle-, and low-population density areas (P.05 for all comparisons). The proportion of single-specialty practices has decreased significantly (P.001), and the proportion of academic practices increased significantly (P = .004). Additionally, the proportion of practices and physicians in high- and low-population density regions remained stable during this period (P.05).Our analysis suggests that practice size consolidation has occurred within the US radiation oncology workforce from 2012 to 2020. The impact of this consolidation on quality, cost, and patient access deserves further attention.

Abstract

Although natural killer (NK) cells are well known for their ability to kill tumors, few studies have addressed the interactions between resting (nonactivated) NK cells and freshly isolated human tumors. Here, we show that human leukocyte antigen class Ilow tumor cells isolated directly from patients with advanced ovarian carcinoma trigger degranulation by resting allogeneic NK cells. This was paralleled by induction of granzyme B and caspase-6 activities in the tumor cells and significant tumor cell lysis. Ovarian carcinoma cells displayed ubiquitous expression of the DNAX accessory molecule-1 (DNAM-1) ligand PVR and sparse/heterogeneous expression of the NKG2D ligands MICA/MICB and ULBP1, ULBP2, and ULBP3. In line with the NK receptor ligand expression profiles, antibody-mediated blockade of activating receptor pathways revealed a dominant role for DNAM-1 and a complementary contribution of NKG2D signaling in tumor cell recognition. These results show that resting NK cells are capable of directly recognizing freshly isolated human tumor cells and identify ovarian carcinoma as a potential target for adoptive NK cell–based immunotherapy. We queried the National Cancer Database for high-risk PCa in patients aged 80 yr or older diagnosed during 2004–2016. Intervention: Eligible patients underwent no treatment following biopsy (ie, observation), androgen deprivation therapy (ADT) alone, radiation therapy (RT) alone, RT + ADT, or surgery. Outcome measurements and statistical analysis: Kaplan-Meier, log rank, and multivariate Cox proportional hazard regression was performed to compare overall survival (OS). Results and limitations: A total of 19 920 men were eligible for analysis, and the most common treatment approach was RT + ADT (7401 patients; 37.2%). Observation and ADT alone declined over time (59.3% in 2004 vs 47.5% in 2016). There was no observed difference in OS between observation and ADT alone (adjusted hazard ratio [HR] 1.04, 95% confidence interval [CI], 0.99–1.09; p = 0.105). Definitive local treatment was associated with improved OS compared with ADT alone (RT alone, HR 0.54, 95% CI, 0.50–0.59, p 5 yr; minimal comorbidity) should be offered definitive, life-prolonging therapy.

Abstract

Circulating tumor DNA (ctDNA) sensitivity remains subpar for molecular residual disease (MRD) detection in bladder cancer patients. To remedy this problem, we focused on the biofluid most proximal to the disease, urine, and analyzed urine tumor DNA in 74 localized bladder cancer patients. We integrated ultra-low-pass whole genome sequencing (ULP-WGS) with urine cancer personalized profiling by deep sequencing (uCAPP-Seq) to achieve sensitive MRD detection and predict overall survival. Variant allele frequency, inferred tumor mutational burden, and copy number-derived tumor fraction levels in urine cell-free DNA (cfDNA) significantly predicted pathologic complete response status, far better than plasma ctDNA was able to. A random forest model incorporating these urine cfDNA-derived factors with leave-one-out cross-validation was 87% sensitive for predicting residual disease in reference to gold-standard surgical pathology. Both progression-free survival (HR = 3.00, p = 0.01) and overall survival (HR = 4.81, p = 0.009) were dramatically worse by Kaplan–Meier analysis for patients predicted by the model to have MRD, which was corroborated by Cox regression analysis. Additional survival analyses performed on muscle-invasive, neoadjuvant chemotherapy, and held-out validation subgroups corroborated these findings. In summary, we profiled urine samples from 74 patients with localized bladder cancer and used urine cfDNA multi-omics to detect MRD sensitively and predict survival accurately.

The NCCN panel meets annually to discuss updates to the guidelines based on comments from panel members and the Institutional Review, as well as submissions from within NCCN and external organizations. These NCCN Guidelines Insights focus on the introduction of a new surgical recommendation terminology (peripheral and deep en face margin assessment), as well as recent updates on topical prophylaxis, immunotherapy for regional and metastatic disease, and radiation therapy.

Abstract

There are limited data regarding the optimal management of patients with pelvic node-positive, but non-metastatic, bladder cancer. Increasing data demonstrate that this is a distinct clinical entity with outcomes bridging between bladder-confined muscle-invasive bladder cancer and metastatic advanced bladder cancer. Guidelines and staging systems have formalized the need to incorporate the unique considerations of management of pelvic node-positive bladder cancer. However, there remains an absence of a definite standard of care. Treatment options include systemic therapy alone, neoadjuvant chemotherapy followed by radical cystectomy, or bladder-preserving trimodality therapy. Furthermore, ongoing studies aim to determine the benefit of incorporating immunotherapy into these treatment paradigms. In this review article, we will discuss the key considerations for management of patients with pelvic node-positive bladder cancer.

Abstract

Collisions of ultrarelativistic electron beams are considered using full-scale 3D particle-in-cell simulation. In this process, the particles can be affected by the superintense fields of the counterpropagating beam, and the interaction can pass to the regime of nonperturbative quantum electrodynamics. In this experimentally unexplored regime, the emission of photons and the production of electron – positron pairs are extremely probable processes. It is shown that due to the transverse displacement of the beams and an increase in the number of particles located in the region of the field maximum, it is possible to increase the yield of both photons and electron – positron pairs.

Summary Background Two radiotherapy fractionation schedules are used to treat locally advanced bladder cancer: 64 Gy in 32 fractions over 6·5 weeks and a hypofractionated schedule of 55 Gy in 20 fractions over 4 weeks. Long-term outcomes of these schedules in several cohort studies and case series suggest that response, survival, and toxicity are similar, but no direct comparison has been published. The present study aimed to assess the non-inferiority of 55 Gy in 20 fractions to 64 Gy in 32 fractions in terms of invasive locoregional control and late toxicity in patients with locally advanced bladder cancer. Methods We did a meta-analysis of individual patient data from patients (age ≥18 years) with locally advanced bladder cancer (T1G3 [high-grade non-muscle invasive] or T2–T4, N0M0) enrolled in two multicentre, randomised, controlled, phase 3 trials done in the UK: BC2001 (NCT00024349; assessing addition of chemotherapy to radiotherapy) and BCON (NCT00033436; assessing hypoxia-modifying therapy combined with radiotherapy). In each trial, the fractionation schedule was chosen according to local standard practice. Co-primary endpoints were invasive locoregional control (non-inferiority margin hazard ratio [HR]=1·25); and late bladder or rectum toxicity, assessed with the Late Effects Normal Tissue Task Force-Subjective, Objective, Management, Analytic tool (non-inferiority margin for absolute risk difference [RD]=10%). If non-inferiority was met for invasive locoregional control, superiority could be considered if the 95% CI for the treatment effect excluded the null effect (HR=1). One-stage individual patient data meta-analysis models for the time-to-event and binary outcomes were used, accounting for trial differences, within-centre correlation, randomised treatment received, baseline variable imbalances, and potential confounding from relevant prognostic factors. Findings 782 patients with known fractionation schedules (456 from the BC2001 trial and 326 from the BCON trial; 376 (48%) received 64 Gy in 32 fractions and 406 (52%) received 55 Gy in 20 fractions) were included in our meta-analysis. Median follow-up was 120 months (IQR 99–159). Patients who received 55 Gy in 20 fractions had a lower risk of invasive locoregional recurrence than those who received 64 Gy in 32 fractions (adjusted HR 0·71 [95% CI 0·52–0·96]). Both schedules had similar toxicity profiles (adjusted RD −3·37% [95% CI −11·85 to 5·10]). Interpretation A hypofractionated schedule of 55 Gy in 20 fractions is non-inferior to 64 Gy in 32 fractions with regard to both invasive locoregional control and toxicity, and is superior with regard to invasive locoregional control. 55 Gy in 20 fractions should be adopted as a standard of care for bladder preservation in patients with locally advanced bladder cancer. Abstract

Rationale: Alpha particle emitting radiopharmaceuticals are generating considerable interest for the treatment of disseminated metastatic disease. Molecular imaging of the distribution of these agents is critical to safely and effectively maximize the clinical potential of this emerging drug class. The present studies aim to investigate the feasibility and limitations of quantitative SPECT for 223Ra, 225Ac and 227Th. Methods: Three state-of-the-art SPECT/CT systems were investigated: the GE Discovery NM/CT 670, the GE Optima NM/CT 640, and the Siemens Symbia T6. A series of phantoms, including the NEMA IEC Body phantom, were used to compare and calibrate each camera. Additionally, anthropomorphic physical tumor and vertebrae phantoms were developed and imaged to evaluate the quantitative imaging protocol. Results: This work describes and validates a methodology to calibrate each clinical system. The efficiency of each gamma camera was analyzed and compared. Using the calibration factors obtained with the NEMA phantom, we were able to quantify the activity in 3D-printed tissue phantoms with an error of 2.1%, 3.5% and 11.8% for 223Ra, 225Ac, and 227Th, respectively. Conclusion: The present study validates that quantitative SPECT/CT imaging of 223Ra, 225Ac, and 227Th is achievable but that careful considerations for camera configuration are required. These results will aid in future implementation of SPECT-based patient studies and will help to identify the limiting factors for accurate image-based quantification with alpha particle emitting radionuclides.

Abstract

Emerging evidence suggests an interaction between renal inflammation and afferent renal nerves in some preclinical models of hypertension. In this study we tested the hypothesis that the inflammatory cytokine IL-1β interacts with afferent renal nerves in the pathogenesis of the DOCA-salt mouse model of hypertension. Specifically, we compared the effect of total renal denervation (TRDN) and afferent RDN (ARDN) on the development of DOCA-salt hypertension to that observed in animals receiving the IL-1 receptor antagonist anakinra. In the first study , 10-week-old male C57BL6/J mice were implanted with radiotelemeters for measurement of mean arterial pressure (MAP). DOCA was administered via subcutaneous pellet containing 50 mg of DOCA, controls received a drug free pellet. Uni-nephrectomy, pellet insertion, salt treatment, TRDN, ARDN, or sham denervation were all performed on the same day. TRDN and ARDN was performed through peri-axonal application of phenol and capsaicin respectively. In the second study , the IL-1 receptor antagonist anakinra (75mg/kg) or vehicle control (0.9% saline) was delivered via intraperitoneal injection daily 10 days post-induction of hypertension. Renal cytokine protein was quantified by Multiplex ELISA. All values reported are mean + SEM, all groups n=5. In the first study , MAP increased in sham treated DOCA-salt mice +43 ± 1 mmHg from baseline by the end of the 3 rd week of DOCA-salt. This response was attenuated by 40% in TRDN (+26 ± 4 mmHg) and by 44% in ARDN DOCA-salt mice (+24 ± 3 mmHg). IL-1β was increased in DOCA-salt kidneys (2.1 ± 0.4 pg/mg) compared to control kidneys (0.36 ± 0.1 pg/mg). In the second study , anakinra similarly attenuated the increase in MAP by 45% (+21 ± 2 mmHg) compared to vehicle controls (+38 ± 1 mmHg). Neither ARDN nor IL-1 receptor antagonism had any effect on renal inflammatory cytokines IL-1β, IL-6, or TNFα. The comparable attenuation in the MAP response to DOCA-salt ARDN and anakinra treated groups as well as the unchanged inflammatory phenotype is consistent with a common mechanism of action. We hypothesize that intrarenal IL-1β activates sympathoexcitatory renal afferent nerves to increase MAP in DOCA-salt mice. Future studies are needed to directly test this hypothesis.

Abstract

Digital autoradiography (DAR) is a powerful tool to quantitatively determine the distribution of a radiopharmaceutical within a tissue section and is widely used in drug discovery and development. However, the low image resolution and significant background noise can result in poor correlation, even errors, between radiotracer distribution, anatomical structure, and molecular expression profiles. Differing from conventional optical systems, the point spread function (PSF) in DAR is determined by properties of radioisotope decay, phosphor and digitizer. Calibration of an experimental PSF a priori is difficult, prone to error, and impractical. We have developed a content-adaptive restoration algorithm to address these problems. Methods: We model the DAR imaging process using a mixed Poisson-Gaussian model, and blindly restore the image by a Penalized Maximum-Likelihood Expectation-Maximization algorithm (PG- PEM). PG-PEM implements a patch-based estimation algorithm with "Density-Based Spatial Clus- tering of Applications with Noise" to estimate noise parameters, and utilizes L2 and Hessian Frobenius (HF) norms as regularization functions to improve performance. Abstract

Background Sentinel lymph node biopsy (SLNB) has not been studied for invasive melanomas treated with Mohs micrographic surgery using frozen-section MART-1 immunohistochemical stains (MMS-IHC). The primary objective of this study was to assess the accuracy and compliance with National Comprehensive Cancer Network (NCCN) guidelines for SLNB in a cohort of patients who had invasive melanoma treated with MMS-IHC. Methods This retrospective cohort study included all patients who had primary, invasive, cutaneous melanomas treated with MMS-IHC at a single academic center between March 2006 and April 2018. The primary outcomes were the rates of documenting discussion and performing SLNB in patients who were eligible based on NCCN guidelines. Secondary outcomes were the rate of identifying the sentinel lymph node and the percentage of positive lymph nodes. Results In total, 667 primary, invasive, cutaneous melanomas (American Joint Committee on Cancer T1a-T4b) were treated with MMS-IHC. The median patient age was 69 years (range, 25-101 years). Ninety-two percent of tumors were located on specialty sites (head and/or neck, hands and/or feet, pretibial leg). Discussion of SLNB was documented for 162 of 176 (92%) SLNB-eligible patients, including 127 of 127 (100%) who had melanomas with a Breslow depth >1 mm. SLNB was performed in 109 of 176 (62%) SLNB-eligible patients, including 102 of 158 melanomas (65%) that met NCCN criteria to discuss and offer SLNB and 7 of 18 melanomas (39%) that met criteria to discuss and consider SLNB. The sentinel lymph node was successfully identified in 98 of 109 patients (90%) and was positive in 6 of those 98 patients (6%). Conclusions Combining SLNB and MMS-IHC allows full pathologic staging and confirmation of clear microscopic margins before reconstruction of specialty site invasive melanomas. SLNB can be performed accurately and in compliance with consensus guidelines in patients with melanoma using MMS-IHC.

Abstract

Technological advancement has facilitated patient-specific radiotherapy in bladder cancer. This has been made possible by developments in image-guided radiotherapy (IGRT). Particularly transformative has been the integration of volumetric imaging into the workflow. The ability to visualise the bladder target using cone beam computed tomography and magnetic resonance imaging initially assisted with determining the magnitude of inter- and intra-fraction target change. It has led to greater confidence in ascertaining true anatomy at each fraction. The increased certainty of dose delivered to the bladder has permitted the safe reduction of planning target volume margins. IGRT has therefore improved target coverage with a reduction in integral dose to the surrounding tissue. Use of IGRT to feed back into plan and dose delivery optimisation according to the anatomy of the day has enabled adaptive radiotherapy bladder solutions. Here we undertake a review of the stepwise developments underpinning IGRT and adaptive radiotherapy strategies for external beam bladder cancer radiotherapy. We present the evidence in accordance with the framework for systematic clinical evaluation of technical innovations in radiation oncology (R-IDEAL).

Abstract

Liver and skin tumours in brown bullhead (Ameiurus nebulosus) and white sucker (Catostomus commersoni) have been associated with contamination of the aquatic environment. In this study, we present a Bayesian hierarchical logistic-Bernoulli framework that considers the role of fish covariates (e.g., age, total weight, gonad weight, liver weight, fork length) on tumour prevalence to identify the presence of “hot-spots” around the Canadian waters of the Great Lakes, where high fish tumour frequencies are registered. We developed methods to discern the degree of impairment that are either based on the comparison of tumour frequencies in contaminated (or impacted) sites against those predicted in their corresponding reference areas, or the assessment of the prevailing conditions in impaired sites independently, without the need to establish baseline conditions for comparison purposes. Our modelling study predicts low frequencies of neoplastic tumours in all the impacted locations. In contrast, the same comparisons with the preneoplastic lesions provided evidence of distinct differences between impacted and reference sites in Jackfish Bay, St. Mary’s River, Niagara River, Hamilton Harbour, and Bay of Quinte. We also found weak to moderately strong relationships between tumour occurrence and fish physical characteristics that varied considerably in terms of their strength and nature (sign) among the different locations. Our study concludes that the prevalence of neoplastic tumours appears to have reached acceptable levels around the Great Lakes, but the distinctly higher levels of preneoplasms in several impacted locations underscore the need to improve our understanding of the lesions that may lead to carcinogenesis.

Abstract

Highlights • The optimal sequence of HDR-BT boost and EBRT for prostate cancer is unclear. • We compared early toxicity based on the timing of HDR-BT boost. • The timing of HDR-BT was not based on any specific patient or clinical factors. • We found no difference in early GI/GU toxicity between the two groups. • Longer follow-up is needed to evaluate late toxicity and long-term disease control., Background We present the first report comparing early toxicity outcomes with high-dose rate brachytherapy (HDR-BT) boost upfront versus intensity modulated RT (IMRT) upfront combined with androgen deprivation therapy (ADT) as definitive management for intermediate risk or higher prostate cancer. Methods and Materials We reviewed all non-metastatic prostate cancer patients who received HDR-BT boost from 2014 to 2019. HDR-BT boost was offered to patients with intermediate-risk disease or higher. ADT use and IMRT target volume was based on NCCN risk group. IMRT dose was typically 45 Gy in 25 fractions to the prostate and seminal vesicles ± pelvic lymph nodes. HDR-BT dose was 15 Gy in 1 fraction, delivered approximately 3 weeks before or after IMRT. The sequence was based on physician preference. Biochemical recurrence was defined per ASTRO definition. Gastrointestinal (GI) and Genitourinary (GU) toxicity was graded per CTCAE v5.0. Abstract

Background: Circulating cell-free tumor DNA (ctDNA) analysis for minimal residual disease (MRD) detection is transforming cancer care. However, the sensitivity of these approaches remains subpar and there is significant risk of false negative results. We analyzed the most proximal biofluid (urine) in localized muscle-invasive bladder cancer (MIBC) patients, and performed two orthogonal ctDNA analytical techniques, one focusing on driver mutation detection, and another focusing on genome-wide copy number alterations. The primary objective of this combinatorial approach was to sensitively detect ctDNA MRD, predict pathologic complete response (pCR), and improve patient risk-stratification. Methods: Urine samples from 74 localized bladder cancer patients were collected preoperatively on the day of curative-intent radical cystectomy (RC) to assess urine tumor DNA (utDNA). We performed ultra-low pass whole genome sequencing (ULP-WGS) of urine cfDNA from all 74 patients as well as 15 healthy adults. Tumor fraction (TFx) level based on genome-wide copy number alterations was estimated using ichorCNA. Variant allele frequency (VAF) based on single-nucleotide variants (SNVs) was estimated by uCAPP-Seq. We also noninvasively inferred tumor mutation burden (iTMB). pCR was determined by surgical pathology. A random forest (RF) model with leave-one-out cross-validation (LOOCV) was utilized to predict disease status. Kaplan-Meier (KM) and Cox proportional hazards model survival analyses were performed to assess overall survival (OS) and progression-free survival (PFS). Results: Our study cohort consisted of 74 patients, of which 58 (78%) harbored localized MIBC, and 16 (22%) harbored treatment-refractory high-risk localized NMIBC. Among MIBC patients, 64% (37/58) received neo-adjuvant chemotherapy. VAF, iTMB and TFx levels significantly predicted pCR status. Our RF model incorporating these three utDNA parameters achieved a sensitivity of 87%, a negative predictive value of 77%, and a positive predictive value of 65% for predicting residual disease. AUC for the model was 0.80 (p Conclusions: Integration of ULP-WGS with uCAPP-Seq enabled robust detection of residual urine tumor DNA with high sensitivity and predicted survival in localized bladder cancer patients.
