Abstract: Giant cell myocarditis is a rare, often rapidly progressive and potentially fatal, disease due to T-cell lymphocyte-mediated inflammation of the myocardium that typically affects young and middle-aged adults. Frequently, the disease course is marked by acute heart failure, cardiogenic shock, intractable ventricular arrhythmias, and/or heart block. Diagnosis is often difficult due to its varied clinical presentation and overlap with other cardiovascular conditions. Although cardiac biomarkers and multimodality imaging are often used as initial diagnostic tests, endomyocardial biopsy is required for definitive diagnosis. Combination immunosuppressive therapy, along with guideline-directed medical therapy, has led to a paradigm shift in the management of giant cell myocarditis resulting in an improvement in overall and transplant-free survival. Early diagnosis and prompt management can decrease the risk of transplantation or death, which remain common in patients who present with cardiogenic shock.

Abstract: Optical Power Converters (OPCs) based on Broadcom's multi-junction photovoltaic design enable high-efficiency optical-to-electrical conversion for applications at various wavelengths and output powers. This paper reviews how the output powers have been extended from < 1W to tens of Watts, and how the spectral range options cover the 800-830nm range, as well as other key laser diode wavelengths such as 960-990nm and 1500-1600nm.

Abstract: We report the first-in-man implantation of the Mitra-Spacer. The device was implanted transapically. FMR was reduced to moderate. At two months, while in NYHA Class II, LVEF had improved, but FMR increased and 2 mL was added, reducing FMR to mild. Despite anticoagulation, thrombi developed around the device and the valve was replaced at eight months. The Mitra-Spacer successfully bridged this patient to surgery after LVEF had recovered.

Abstract: Large liquid argon detectors offer one of the best avenues for the detection of galactic weakly interacting massive particles (WIMPs) via their scattering on atomic nuclei. The liquid argon target allows exquisite discrimination between nuclear and electron recoil signals via pulse-shape discrimination of the scintillation signals. Atmospheric argon (AAr), however, has a naturally occurring radioactive isotope, $^{39}$Ar, a $\beta$ emitter of cosmogenic origin. For large detectors, the atmospheric $^{39}$Ar activity poses pile-up concerns. The use of argon extracted from underground wells, deprived of $^{39}$Ar, is key to the physics potential of these experiments. The DarkSide-20k dark matter search experiment will operate a dual-phase time projection chamber with 50 tonnes of radio-pure underground argon (UAr), that was shown to be depleted of $^{39}$Ar with respect to AAr by a factor larger than 1400. Assessing the $^{39}$Ar content of the UAr during extraction is crucial for the success of DarkSide-20k, as well as for future experiments of the Global Argon Dark Matter Collaboration (GADMC). This will be carried out by the DArT in ArDM experiment, a small chamber made with extremely radio-pure materials that will be placed at the centre of the ArDM detector, in the Canfranc Underground Laboratory (LSC) in Spain. The ArDM LAr volume acts as an active veto for background radioactivity, mostly $\gamma$-rays from the ArDM detector materials and the surrounding rock. Abstract: International audience; Shashlik calorimeters equipped with a compact readout based on Silicon PhotoMultipliers can be longitudinally segmented by directly coupling the WLS fibers with the photosensors thus embedding the readout in the bulk of the calorimeter. Results on energy resolution and particle identification for such calorimeters are presented. The SiPMs for the readout have also been characterized after being exposed to neutron fluences up to 2×10$^{11}$ n/cm$^2$ (1 MeV eq.). Alternative options for the active material were also investigated; we studied in particular polysiloxane as a substitute for plastic scintillator.

Abstract: International audience; The next generation of neutrino experiments requires measurements of absolute neutrino cross sections at the GeV scale with high precision (∼1%) presently limited by the uncertainties on neutrino flux. Monitoring the lepton production in the decay tunnel of neutrino beams is the most straightforward way to measure the neutrino flux at source. The ENUBET Collaboration develops novel technologies to monitor positrons from K + → νee +π0 decays on an event by event basis. This technique can achieve a precision in the νe flux below 1% and enable a new generation of cross section and short baseline experiments. In this paper, we present the achievements of the first year of the Project on beamline simulation, rate and dose assessment, detector prototyping and evaluation of the physics reach.

Abstract: International audience; The ENUBET Collaboration is developing a technology to reduce by one order of magnitude the uncertainty on fluxes in conventional neutrino beam. The ENUBET beamline exploits the large angle production of positrons from $K^+ \rightarrow e^+ \pi^0 \nu_e$ in the decay tunnel to monitor the associated production of $\nu_e$. This method provides the $\nu_e$ rate at source at the 1% level. In this talk, we will summarize the results during the first year of the project and plans up to completion (2021).

Abstract: Prosthetic valve paravalvular leak (PVL) is a known and relatively common complication of surgical valve replacement, which may lead to significant morbidity. Patients with significant mitral valve replacement (MVR) PVL typically present with symptoms of heart failure and elevated filling pressures or with hemolytic anemia. Percutaneous closure of these leaks has become the preferred therapy. Percutaneous closure of MVR PVL can be technically challenging, given the anatomy of the approach (trans-septal, trans-apical), the level of associated comorbidities and the geographic location of the paravalvular defect. Steerable catheters offer a unique ability to position themselves coaxial to the PVL. The Dexterity catheter (Spirus Medical LLC, Bridgewater, MA, USA) is a semi-rigid steerable catheter used in our lab with the ability to articulate at two separate points on the distal tip and flex greater than 360 degrees. We present cases of surgical valves that developed PVL which underwent successful repair with a Dexterity catheter.

Abstract: International audience; We report on the cryogenic characterization of Red Green Blue - High Density (RGB-HD) SiPMs developed at Fondazione Bruno Kessler (FBK) as part of the DarkSide program of dark matter searches with liquid argon time projection chambers. A cryogenic setup was used to operate the SiPMs at varying temperatures and a custom data acquisition system and analysis software were used to precisely characterize the primary dark noise, the correlated noise, and the gain of the devices. We demonstrate that FBK RGB-HD SiPMs with low quenching resistance (RGB-HD-LRq) can be operated from 40 K to 300 K with gains in the range 10(5) to 10(6) and noise rates at a level of around 1 Hz/mm(2).

Abstract: Background— Reducing digital pulse rates (PR) are known to reduce total energy during invasive cardiovascular procedures, which likely has benefits for patients and staff. Physicians may be reluctant to reduce these parameters because they fear a decline in image quality that could affect procedural outcomes. We sought to assess the effect of default rates of fluoroscopy (Fluoro) and CINE-acquisition (CINE) on total x-ray dose and image quality during invasive cardiovascular procedures. Methods and Results— We retrospectively reviewed procedures done with 2 default PRs: a standard dose cohort (PR, 15 for Fluoro and CINE), and a reduced dose cohort (PR, 10 for Fluoro and CINE). Total x-ray dose, Fluoro time, and contrast use were compared between groups. A blinded angiographic image quality assessment was then performed using an objective 10-point angiographic quality score. There were no significant differences between cohorts for fluoroscopy time or contrast use. The reduced dose cohort has a significant reduction in mean total x-ray dose (PR 15, 1763.1 mGy; PR 10, 1179.1 mGy; P P P =0.67), indicating no decline in image quality with PR reduction. Conclusions— Reducing default PRs during invasive cardiovascular procedures yields large and significant reductions in total x-ray energy with no decline in angiographic image quality.

Abstract: The ENUBET project aims at demonstrating that the systematics in neutrino fluxes from conventional beams can be reduced to 1% by monitoring positrons from Ke 3 decays in an instrumented decay tunnel, thus allowing a precise measurement of the ve (and ve) cross section. This contribution will report the results achieved in the first year of activities. The expected neutrino fluxes at an hypothetical far detector have been estimated. Progress have been made in the design of the hadron beamline and a complete simulation of the positron tagger allowed to determine its performance in terms of signal selection efficiency and purity. A deep R&D program has been pursued to explore different technical solution for the tagger instrumentation. Test beam exposures of detector prototypes have been performed to assess their response to pions, electrons and muons and to validate the Monte Carlo simulation.

16. Right heart catheterization using antecubital venous access: Feasibility, safety and adoption rate in a tertiary center

Abstract: Objectives To determine feasibility, safety, and adoption rates of right heart catheterization (RHC) using antecubital venous access (AVA) as compared to using the traditional approach of proximal venous access (PVA). Background RHC via PVA (i.e., internal jugular, femoral or subclavian) is generally a low risk procedure; however, complications may occur and are usually access site related. RHC via an antecubital approach has regained attention given the increase in transradial left heart catheterizations. Methods Patients undergoing RHC for any indication at a single large academic medical center were identified over a 5-year period (January 2008 to December 2012) from a database. Medical records were retrospectively analyzed for demographic, procedural and outcomes data. Results Two hundred seventy-two RHC procedures were included (106 AVA, 166 PVA). The adoption rate of AVA for RHC increased rapidly since its introduction in our laboratory in 2010 (100% PVA in 2008 and 2009, 85% AVA in 2012). All procedures were successful; however, 6% of procedures required additional, alternate access to the original site. Initial success rates were similar in the two groups (91 vs. 96% for AVA and PVA respectively, P = 0.12). Fluoroscopy time was shorter in the group of patients who underwent the procedure via AVA. The complication rate was 0% in the AVA group compared with 3% in the PVA group (P = 0.16). Conclusion RHC via the AVA is a feasible and safe alternative to PVA. Our experience and rapid adoption support the use AVA as the access site of choice for RHC in uncomplicated patients. © 2013 Wiley Periodicals, Inc.

Abstract: We report a case of a 54-year-old patient with a T3c renal mass with intracardiac extension of the thrombus to the level of the pulmonary valve. The patient was not a candidate for cardiopulmonary bypass due to recent pulmonary embolism. Under transesophageal echocardiogram guidance, the intracardiac thrombus was removed percutaneously via transvenous mechanical thrombectomy. The patient was effectively downstaged to T3b and underwent successful radical nephrectomy and inferior vena cava thrombectomy without the use of cardiopulmonary bypass.

18. First results with SiPM tiles for TOF PET based on FBK RGB-HD technology

Abstract: We present the first results of timing and energy resolution of two newly developed tiles based on FBK RGB-HD SiPMs. The first tile has dimensions of 32x32 mm2 and is composed of 8x8 SiPMs, with a regular pitch of 4 mm and a cell size of 25x25 μm2. Although manufactured with a standard bond wire technology, the tile achieves a fill factor at the tile level of 85%. We produced two versions: one with a single-ended and the other with differential readout. We tested the first prototypes with single-ended readout with a scintillator array, perfectly matching the tile pitch and composed of 8x8 LYSO crystals with dimensions of 4x4x22 mm3. First, we tested the tile using a single-channel setup, based on a fast, discrete amplifier, a digital oscilloscope and a PC, reading one SiPM at a time. At 20 °C, we measured an energy resolution of 10.7% FWHM. For the timing measurements we compared two conditions: when only one SiPM was biased and read, and when all the 64 SiPMs were biased but only one was read. At 20 °C, we measured a timing resolution of 200 ps FWHM in the first case, and 220 ps FWHM in the second case. Then, we tested the whole tile with a dedicated ASIC (PETA3), and measured the energy and timing resolution of two tiles in coincidence. The second tile is composed of 144 SiPMs, mounted on a water cooled, ceramic LTCC substrate. On the top side, it contains 12x12 SiPMs with a regular pitch of 2.5 mm. Also in this case, the SiPM technology is the RGB-HD with a cell size of 25x25 μm2. On the bottom side, four readout ASICs of the latest generation (PETA5) are flip-chip mounted. First results will be presented.

Abstract: Objectives This study investigated the impact of adding novel elements to models predicting in-hospital mortality after percutaneous coronary interventions (PCIs). Background Massachusetts mandated public reporting of hospital-specific PCI mortality in 2003. In 2006, a physician advisory group recommended adding to the prediction models 3 attributes not collected by the National Cardiovascular Data Registry instrument. These “compassionate use” (CU) features included coma on presentation, active hemodynamic support during PCI, and cardiopulmonary resuscitation at PCI initiation. Methods From October 2005 through September 2007, PCI was performed during 29,784 admissions in Massachusetts nonfederal hospitals. Of these, 5,588 involved patients with ST-segment elevation myocardial infarction or cardiogenic shock. Cases with CU criteria identified were adjudicated by trained physician reviewers. Regression models with and without the CU composite variable (presence of any of the 3 features) were compared using areas under the receiver-operator characteristic curves. Results Unadjusted mortality in this high-risk subset was 5.7%. Among these admissions, 96 (1.7%) had at least 1 CU feature, with 69.8% mortality. The adjusted odds ratio for in-hospital death for CU PCIs (vs. no CU criteria) was 27.3 (95% confidence interval: 14.5 to 47.6). Discrimination of the model improved after including CU, with areas under the receiver-operating characteristic curves increasing from 0.87 to 0.90 (p Conclusions A small proportion of patients at extreme risk of post-PCI mortality can be identified using pre-procedural factors not routinely collected, but that heighten predictive accuracy. Such improvements in model performance may result in greater confidence in reporting of risk-adjusted PCI outcomes.

Abstract: OBJECTIVE To examine ankle-brachial index (ABI) abnormalities in patients with type 2 diabetes and coronary artery disease (CAD). RESEARCH DESIGN AND METHODS An ABI was obtained in 2,240 patients in the Bypass Angioplasty Revascularization Investigation 2 Diabetes (BARI 2D) Trial. ABIs were classified as: normal, 0.91–1.3; low, ≤0.9; high, >1.3; or noncompressible artery (NC). Baseline characteristics were examined according to ABI and by multivariate analysis. RESULTS ABI was normal in 66%, low in 19%, and high in 8% of patients, and 6% of patients had NC. Of the low ABI patients, 68% were asymptomatic. Using normal ABI as referent, low ABI was independently associated with smoking, female sex, black race, hypertension, age, C-reactive protein, diabetes duration, and lower BMI. High ABI was associated with male sex, nonblack race, and higher BMI; and NC artery was associated with diabetes duration, higher BMI, and hypertension. CONCLUSIONS ABI abnormalities are common and often asymptomatic in patients with type 2 diabetes and CAD.

Abstract: Objective: To evaluate if there are differences in procedural times, success rates, and safety between left and right radial approach (LRA and RRA, respectively) in primary percutaneous coronary intervention (PCI) for ST-elevation myocardial infarction (STEMI). Background: Given conflicting reports of different procedural success with LRA vs. RRA, it is unclear if the side of radial access impacts in-room procedural times and success rates in primary PCI. At our institution the LRA has been commonly used in certain STEMI patients. Our clinical database was reviewed to see if routine use of the LRA could generate favorable technical success and reperfusion times as compared to the RRA. Methods: We retrospectively analyzed 135 consecutive STEMI patients treated with primary PCI performed via the left and right radial approach at our institution. Results: There were 50 cases in the LRA group and 85 in the RRA group. There was no difference in median procedural times including total procedure time (LRA 53.5 mins vs. RRA 52 mins, P = 0.95), room-to-cannulation (LRA 12 min vs. RRA 13 min, P = 0.40) or room-to-balloon times (LRA 30 min vs. RRA 31 min, P = 0.74). There were no significant differences in procedural success rates (LRA 100% vs. RRA 97.6%, P = 0.27), or procedure-related complications or death between the two groups. Conclusions: Left and right trans-radial approach for primary PCI have similar in room procedural times, success rates, and comparable safety. Trans-radial PCI through either arm is a feasible and safe approach in patients with STEMI. © 2010 Wiley-Liss, Inc.

Abstract: Background and objective: To analyze the temporal evolution of imaging findings and clinical follow up in craniocervical arterial dissection (CAD) patients. Methods: 32 patients (M: F=23: 9, mean age=49) with radiographic evidence of at least one of the suggestive imaging findings of CAD (arterial stenosis, arterial occlusion, luminal ectasia, pseudoaneurysm, and dissection flap) were performed. Median imaging follow up period was 89 days (SD=36.69). Clinical symptoms on presentation and follow up were also analyzed. Results: 21 patients had spontaneous dissection (65.6%). 11 had a history of trauma (34.4%). 14 (43.8%) patients presented with a stroke or TIA. Other presenting symptoms include headache (n=4, 12.5%), neck pain (n=9, 28.1%) and Horner’s syndrome (n=5, 15.6%). Involved arteries included Internal carotid artery (n=21, 65.6%), vertebral artery (n=10, 31.3%), and common carotid artery (n=3, 9.4%). Initial imaging findings included, arterial stenosis (n=23, 72%), arterial occlusion (n=6, 19%), ectasis of the lumen (n=7, 22%), pseudoaneurysm (n=12, 38%), and dissection flap (n=12, 38%). 21 patients were managed with anti-coagulation and 3 with an antiplatelet agent. On follow up imaging, arterial stenosis was improved in 73.6% (14/19), worse in 5.3% and no interval change in 21.1%. Regarding the pseudoaneurysm on presentation, no change in shape and size in 64% (7/11) and 2 patients developed new pseudoaneurysm. One patient developed a TIA (3.7%, n=1/27) during follow up. Conclusion: Post-dissection vascular imaging findings are dynamic. With medical management, more than 60% of the arterial stenosis lesions have improved within 3 months, and the risk of repeated neurological events were very low.

Abstract: Consolidated techniques used for space-borne X-ray and gamma-ray instruments are based on the use of scintillators coupled to Silicon photo-detectors. This technology associated with modern very low noise read-out electronics allows the design of innovative architectures able to reduce drastically the system complexity and power consumption, also with a moderate-to-high number of channels. These detector architectures can be exploited in the design of space instrumentation for gamma-spectroscopy with the benefit of possible smart background rejection strategies. We describe a detector prototype with 3D imaging capabilities to be employed in future gamma-ray and particle space missions in the 0.002-100 MeV energy range. The instrument is based on a stack of scintillating bars read out by Silicon Drift Detectors (SDDs) at both ends. The spatial segmentation and the crystal double-side readout allow a 3D position reconstruction with ∼3 mm accuracy within the full active volume, using a 2D readout along the two external faces of the detector. Furthermore, one of the side of SDDs can be used simultaneously to detect X-rays in the 2-30 keV energy range. The characteristics of this instrument make it suitable in next generation gamma-ray and particle space missions for Earth or outer space observations, and it will be briefly illustrated.

Abstract: Recent studies have shown that silicon particle detectors made on p-type substrates feature an improved radiation hardness compared to more conventional single-side n-type devices. In particular, they show an increased charge collection efficiency at very high irradiation levels. At present few devices have been fabricated on these substrates and there are still many doubts regarding the best type of isolation structure to be employed for the interruption of the inversion electron layer present between the n/sup +/ electrodes. In this paper, a description of the behavior of microstrip detectors featuring three isolation solution (p-spray, p-stop and a combination between the previous two) is presented. The analysis is based on device simulations and covers the breakdown and interstrip capacitance issues.

Abstract: The aim of this study is to assess the feasibility and safety of percutaneous treatment of superior vena cava (SVC) obstruction following transvenous device implantation. SVC obstruction is an uncommon but serious complication that can occur following permanent pacemaker or cardioverter defibrillator implantation utilizing transvenous endocardial leads. The treatment has traditionally been surgical but with the advent of stents, percutaneous approach is becoming popular. We report on the prevalence of SVC obstruction and the safety of its percutaneous catheter-based treatment. This is a retrospective study of SVC obstruction following device implantation in our institution from January 1993 through November 2003. A total of 1,850 permanent pacemaker and 1,200 implantable cardioverter defibrillator initial implants were performed during that period. Three patients developed SVC obstruction following implant (prevalence, 1/1,000 implant). Two patients were males and the mean age at implant was 57 ± 13 years. Laser lead extraction and SVC angioplasty with or without stenting were performed in all patients. In two of them, this was followed by reimplantation of new systems. There were no procedural complications or mortality. The patients remain free of SVC obstruction symptoms 24 ± 19 months after treatment. SVC obstruction prevalence after device implantation is low. Percutaneous treatment of SVC obstruction can be safely performed and appears to be effective in maintaining medium-term patency. © 2005 Wiley-Liss, Inc.

This report describes a case of successful transcatheter treatment of severe paravalvular mitral regurgitation and pulmonary edema in a patient in whom repeat mitral valve replacement was not believed possible.

35. Balloon aortic valvuloplasty revisited: The role of the Inoue balloon and transseptal antegrade approach

Abstract: Percutaneous aortic balloon valvuloplasty (PABV) was developed to provide a less invasive alternative to aortic valve replacement. Despite initially favorable results, PABV has not produced reliable and durable outcomes. The Inoue balloon used for PABV via an antegrade transseptal approach may offer an improvement over the Mansfield balloons via the identical route. Thirteen consecutive patients with severe symptomatic aortic stenosis were referred for percutaneous aortic balloon valvuloplasty. All patients were considered unacceptably high-risk surgical candidates. Seven consecutive patients underwent antegrade transseptal PABV with Mansfield balloons and in the following six the Inoue balloon was used. The study group was characterized by advanced age (mean, 77) and multiple comorbid conditions (mean, 2.5/patient). Before PABV, the two groups did not differ with respect to age, mean NYHA class, LVEF, transaortic gradient, cardiac output, or aortic valve area. All patients had initial hemodynamic improvement. Complications included one stroke and one vascular injury. After valvuloplasty, cardiac output was not significantly changed. However, there was a significant decrease in aortic gradient and an increase in aortic valve area in both groups; the increase in aortic valve area was significantly greater in those treated with the Inoue balloon (P = 0. 039). Total follow-up mortality was high but appeared to be delayed in the Inoue group. The use of the Inoue balloon with an antegrade transseptal approach warrants further investigation as a preferred technique for PABV.

Abstract: Acute infarct angioplasty on aortocoronary saphenous vein grafts (SVGs) poses significant challenges because of their degenerate morphology and presence of significant thrombus. Of 370 acute, primary, or rescue myocardial infarct angioplasties performed over 3 years, 21 (5.7%) were on SVGs in patients who had undergone previous coronary artery bypass grafting a mean of 7.2 years earlier. Mean duration of chest pain to start of intervention was 3.9 ± 3.2 hours; 6 (29%) patients presented with cardiac shock and 4 had failed treatment with thrombolytic drugs. At intervention, 11 (52%) of the culprit SVGs were totally occluded. Flow was reestablished or improved in 18 (86%), but classified as Thrombolysis In Myocardial Infarction trial grade 3 in only 10 patients (48%). Distal embolization and “no reflow” occurred with a frequency of 57% and 71%, respectively. In-hospital mortality was 19%. At 6 months, freedom from death, repeat target vessel revascularization, or recurrent myocardial infarction was 55%. In 349 patients undergoing native vessel intervention, success and Thrombolysis In Myocardial Infarction trial 3 flow rates were seen in 95% and 73% of patients, respectively, and in-hospital mortality was 7.9%. This present study demonstrates that infarct angioplasty on culprit SVGs can be successful but is associated with higher rates of embolic complications and worse acute and long-term clinical outcomes compared with a parallel experience of acute infarct angioplasty on native coronary arteries.

Abstract: In the past two years, we have developed 3D detector technologies at ITC-irst (Trento, Italy). We have proposed a new 3D architecture, having columnar electrodes of one doping type only, allowing for a simplified fabrication process. In this paper, we report on preliminary results from the electrical characterization of devices irradiated with neutrons, showing that low depletion voltage values can be achieved even after very large fluences.

Abstract: Background Access site complications contribute to morbidity and mortality during percutaneous coronary intervention ( PCI ). Transradial arterial access significantly lowers the risk of access site complications compared to transfemoral arteriotomy. We sought to develop a prediction model for access site complications in patients undergoing PCI with femoral arteriotomy, and assess whether transradial access was selectively used in patients at high risk for complications. Methods and Results We analyzed 17 509 patients who underwent PCI without circulatory support from 2008 to 2011 at 5 institutions. Transradial arterial access was used in 17.8% of patients. In those who underwent transfemoral access, 177 (1.2%) patients had access site complications. Using preprocedural clinical and demographic data, a prediction model for femoral arteriotomy complications was generated. The variables retained in the model included: elevated age ( P P P P =0.002), emergent PCI ( P =0.01), prior PCI ( P =0.005), diabetes ( P =0.008), and peripheral artery disease ( P =0.003). The model showed moderate discrimination (optimism‐adjusted c‐statistic=0.72) and was internally validated via bootstrap resampling. Patients with higher predicted risk of complications via transfemoral access were less likely to receive transradial access ( P ST ‐segment myocardial infarction and when adjusting for individual physician operator. Conclusions We generated and validated a model for transfemoral access site complications during PCI . Paradoxically, patients most likely to develop access site complications from transfemoral access, and therefore benefit from transradial access, were the least likely to receive transradial access.

Abstract: Coronary stents are often used because of their potential to improve the acute and long-term results of balloon angioplasty. The Palmaz-Schatz stent has been approved for use by the U.S. Food and Drug Administration largely because of a demonstrated reduction in the incidence of restenosis following its primary implantation. The Gianturco-Roubin design has been approved for use when balloon angioplasty results in threatened or acute vessel closure. In practice, both stent types are being used in settings when the results of balloon angioplasty are either potentially or actually unacceptable. In such circumstances it is imperative that stents be placed accurately and carefully. Occasionally, stent misplacement, embolization, or disruption can occur, and the need arises to recover and/or reposition the wayward prosthesis. This review describes the removal and recovery of fully deployed Gianturco-Roubin stents using an intracoronary snare technique. © 1996 Wiley-Liss, Inc.

Abstract: Background Stroke associated with percutaneous coronary intervention (PCI) is a tragic complication. Despite advances in the practice of PCI, the incidence of stroke complicating PCI has not changed over the decades. The objective of the present study was to evaluate incidence and correlates of stroke occurring in patients with myocardial infarction (MI) undergoing PCI. Methods and Results Stroke was defined as the presence of any new focal neurological deficit lasting ≥24 h that occurred anytime during or after PCI until discharge. In 2,281 consecutive patients with PCIs for non-ST-elevation MI, or ST-elevation MI (STEMI), 20 strokes were identified (0.88%). Strokes were ischemic in 95%. On multivariate analyses, ejection fraction ≤30% (odds ratio =4.3, p=0.003) was the only independent predictor for stroke. In patients who developed stroke within 24 h of PCI, PCI of vein grafts was more frequent, and use of glycoprotein IIb/IIIa inhibitor was less frequent. Those patients tended to present late in the course of MI. Stroke found more than 24 h after PCI was related to diabetes, higher serum creatinine, lower ejection fraction, anterior wall STEMI and emergency use of intra-aortic balloon pumps. Conclusions Low ejection fraction was the only independent predictor for stroke, but risk factors for periprocedural stroke are different from those of stroke occurring more than 24 h after PCI. Upstream use of glycoprotein IIb/IIIa inhibitor might decrease the risk of periprocedural stroke. (Circ J 2007; 71: 1370 - 1375)

Abstract: The prevalence of metabolic syndrome (MS) was determined in patients agedor =45 years who presented with acute myocardial infarction and underwent primary percutaneous coronary intervention. Two hundred twenty-three consecutive patients aged 18 to 45 years who underwent cardiac catheterization for acute myocardial infarction from June 2001 to December 2004 were reviewed. MS was diagnosed by National Cholesterol Education Program Adult Treatment Panel III guidelines (modified by substituting body mass indexor =28.8 kg/m2 for waist circumference). One hundred sixty-one patients met all 5 criteria for MS available for evaluation. Seventy-six of these patients (47%) metor =3 of the 5 criteria for MS. Sixteen patients with MS (21%) and 5 patients without MS (6%) had diabetes mellitus. The prevalence of each criterion was significantly higher (p0.05) in the MS group. Average Framingham risk scores were 7.0 and 4.5 for patients with and without MS, respectively. The prevalence of smoking, male gender, and family history of premature coronary artery disease were the same for the 2 groups. In conclusion, MS was highly prevalent in this population of young patients with acute myocardial infarction.

Abstract: \ud Background In-stent restenosis is considered to be a gradual and progressive condition and there is scant data on myocardial infarction (MI) as a clinical presentation. Methods and Results Of 2,462 consecutive patients who underwent percutaneous coronary intervention between June 2001 and December 2002, clinical in-stent restenosis occurred in 212 (8.6%), who were classified into 3 groups: ST elevation MI (STEMI), non-ST elevation MI (NSTEMI) and non-MI. Of the 212 patients presenting with clinical in-stent restenosis, 22 (10.4%) had MI (creatine kinase (CK) ≥2 × baseline with elevated CKMB). The remaining 190 (89.6%) patients had stable angina or evidence of ischemia by stress test without elevation of cardiac enzymes. Median interval between previous intervention and presentation for clinical in-stent restenosis was shorter for patients with MI than for non-MI patients (STEMI, 90 days; NSTEMI, 79 days; non-MI, 125 days; p=0.07). Abstract: Laceration of the inferior epigastric artery during cardiac catheterization is under-reported in the literature, but it is a serious complication leading to retroperitoneal hemorrhage and even death. We report on two cases of retroperitoneal bleeding from unintentional puncture of the inferior epigastric artery during cardiac catheterization. Femoral angiography is a valuable tool to promptly recognize this complication.

48. Impact of gender on in-hospital outcomes following contemporary percutaneous intervention for peripheral arterial disease

Abstract: Percutaneous peripheral arterial intervention (PPAI) has emerged as an effective and less invasive alternative to surgery for peripheral arterial disease (PAD), however, data on gender differences in outcome of PPAI, especially in the stent era, are limited. The purpose of this study was to assess the gender differences on in-hospital outcomes of patients undergoing PPAI.We analyzed data on 268 consecutive patients (women: 122; men: 146) who underwent PPAI for PAD between October 2001 and January 2004. A total of 405 lesions (women: 184; men: 221) were treated.Background characteristics were similar except for less prevalent prior coronary intervention in women (18.0% versus 28.8%; p = 0.04), and more prevalent current smokers in women (22.1% versus 12.3%; p = 0.03). Upper extremities interventions were performed more often in women (9.2% versus 3.2%; p = 0.01). Procedure success was achieved in 89.3% of women and 89.7% of men (p = NS). In-hospital mortality was similar between women and men (1.6% versus 0.7%; p = NS). However, hemorrhagic complications occurred more frequently in women (7.4% versus 0.7%; p = 0.006). The blood transfusion rate was significantly higher in women (6.6% versus 0.7%; p = 0.013). Female gender was the only independent predictor of hemorrhagic complications (OR = 12.2; 95% CI = 1.39-111.1; p = 0.024).Percutaneous intervention for PAD can be performed in women with similar success rates as in men, albeit with a greater than 10-fold higher risk of hemorrhagic complications.

Abstract: Large area, 7.25 cm ×8.76 cm silicon drift detectors have been developed and are in production for the ALICE experiment at LHC. An active area of the detector of more than 50 cm 2 imposes high demands on the quality of processing and raw material. Automated testing procedures have been developed to test detectors before mounting them on the ladders. Probe stations for ALICE SDD testing were designed and built at INFN, Trieste and Ohio State University (OSU). Testing procedures, detector selection criteria and some details of the OSU probe station design are discussed.
