The masticator space is a deep facial space which is centered on the mandibular ramus and outlined by the superficial layer of the deep cervical fascia. A masticator space abscess commonly represents an advanced stage of an odontogenic infection indicated by facial pain, swelling and trismus. Masticator space abscess may have a life threatening prognosis if they are not diagnosed and treated properly. They are very rarely seen and published infections in the literature, which make their diagnosis more important. Due to difficulty of physical examination of the region, the value of computed tomography or magnetic resonance investigation on diagnosis of masticator space abscess is important. This report describes two cases of masticator abscess who came to the clinic with trismus and facial pain complaints and both were surgically drained and postoperatively administered with antibiotics, Mastikatör boşluk merkezinde mandibular ramusun bulunduğu derin servikal fasyanın yüzeyel tabakası ile çevrelenmiş derin yüz boşluğudur. Mastikatör boşluklaki apseler genellikle diş kaynaklı infeksiyonların bir komplikasyonudur ve yüz ağrısı, şişlik ve trismus gibi şikayetlere neden olurlar. Mastikatör boşluk apseleri doğru tanı konulmadığı ve uygun tedavi uygulanmadığı durumlarda hayatı tehdit edebilen bir prognoza sahiptirler. Literatürde oldukça az sayıda bildirilmiş olması tanısının konulmasını daha da zor hale getirmiştir. İlgili bölgenin fizik muayenesinin yapılmasının zor olmasından dolayı tanıda bilgisayarlı tomografi ve manyetik rezonans ile inceleme çok yardımcı olmaktadır. Bu çalışmada hastanemiz acil servisine trismus ve yüz ağrısı yakınması ile başvuran ve yapılan incelemelerle mastikatör boşluk apsesi tanısı konulan ardından cerrahi olarak drene edilen iki hastanın sunumu yapılmıştır

Epulis fissuratum, which is also named ‘inflammatory fibrous hyperplasia’, is a clinically nonspecific lesion resuling from chronic trauma of oral mucosa and it is classified under the hyperplastic lesions of oral mucosa. It occurs mostly in 5th-7 th decades due to denture prosthesis. They appear as a series of hyperplasic tissue folds in the maxillary/mandibulary vestibule and on the lingual aspect of the alveolar ridge area. Generally, the tissue is firm, claret and the size of the lesion depends on the degree of trauma and the time of development. The risk of malignant degeneration is not known exactly. Therefore the treatment is removal of the offending stimulus and surgical excision of the lesion. In this report, a 76 year-old woman with lesion on her superior alveolar arch was presented. The lesion of her was excised and the histopathologic examination was revealed as epulis fissuratum. Epulis fissuratum was discussed revising the literature by reason of this case, ‘Enflamatuar fibröz hiperplazi’ olarak da isimlendirilen ‘epulis fissuratum’ oral mukozanın hiperplastik lezyonları başlığı altında sınıflanan, oral mukozanın kronik irritasyonu sonucu oluşan ve klinik olarak spesifik özellikleri olmayan lezyonlardır. Sıklıkla 5-7. dekatlarda, protez kullanımı sonucu oluşmaktadırlar. Bu lezyonlar maksiller/mandibüler vestibül ya da alveoler köprünün lingual yüzünde hiperplastik doku katlantı dizileri olarak görülürler. Genellikle sert kıvamlı ve koyu kırmızı görünümdedirler, boyutları gelişim süresine ve travma derecesine bağlıdır. Malign dejenerasyon riskinin gerçek oranları bilinmemektedir. Bu nedenle tedavisi irritasyonun ortadan kaldırılarak lezyonun eksize edilmesidir. Bu yazıda üst alveolar arkta kitle ile başvuran 76 yaşındaki bayan hasta sunuldu. Hastanın kitlesi eksize edildi, histopatolojik inceleme sonucu epulis fissuratum olarak rapor edildi. Bu olgu nedeniyle ‘epulis fissuratum’ literatür gözden geçirilerek tartışıldı

ÖZET Plunging veya servikal ranula sublingual bezden kaynaklanan ve mylohyoid kası içinden uzanım gösteren nadir görülen bir mukus ekstravazasyon kisti dir. Mukus doku planlarını ayırarak aşağı doğru iner ve sıklıkla submental veya submandibular alanda şişlik olarak kendini gösterir. Tedavide ile seçe nek plunging ranulanın cerrahi eksizyonudur. Basit eksizyon, marsüpiyali zasyon ve intraoral veya servikal yaklaşımla ranula ve sublingual gland ek sizyonu gibi çeşitli cerrahi varyasyonlar bulunmaktadır. Bu tedavi seçenek lerine rağmen hastaların bir kısmında rekürrens görülmekte ve daha büyük lezyonlar ortaya çıkabilmektedir. Skleroterapi cerrahi önerilmeden önce primer tedavide kullanılabilecek po tansiyel küratif bir tedavi prosedürüdür. Tedavisinde servikal yaklaşım ile sublingual gland ve plunging ranulanın total eksizyonu uygulanan bir olgu sunuldu.

ÖZET Erzincan'da sporun kitlelere yaygınlaştırılması, halkın spora yatkınlığı ve öncelikli spor branşlarım tesbit etmek bu çalışmanın en önemli gayesidir. Bu maksatla önce ilin Tarihi, Coğrafi durumu ve sosyo ekonomik yapısı ile mevcut spor tesisleri ve işletmelerinin durumu, yeterliliği ele alınmış spor faaliyetleri incelenmiş, sporun kitlelere yaygınlaşürılabilmesi hakkındaki teori ele alınmıştır. Araştırmanın uygulama kısmında yapılan anketle Erzincan halkının spora ve spor branşlarına karşı alakası, eğitim, cinsiyet, yaş ve gelir durumları gibi sosyolojik parametrelere dayanılarak ortaya konulmaya çalışılmıştır. Erzincan'da en büyük ilgi futbol dalına gösterilmektedir. Bu da ildeki futbol sahalarının yeterli düzeyde olmasından ve tüm dünyada olduğu gibi ülkemizdede bu branşa karşı büyük bir ilgi duyulmasından ileri gelmektdir. Bu veriler ışığında, sportif gelişimi etkileyen sebepler değerlendirilerek geleceğe yönelik tekliflerin ortaya konması amacı güdülmüştür. For this purpose, at first, it was dealt with the sufficient, the position of the present sport foundations and the managements with the historical, geographical position and the sacio economik structure of the city, it was dealt with the theory about being spreaded for the groups of the sport. It was tried to realise as depend on the sociological parameters as education, sex, age, the income positions and the relation against the sort an the sport branches of the public of Erzincan. It is intirested in football in Erzincan very uch. This is also caused by the sufficient of the football fields in the city and the great relation against this branch in our country as in all the world. Amaç: KBB hekimleri arasında nazal septal cerrahi sonras nda nazal tampon kullan m ara t r ld .Gereç ve Yöntemler: Burun ameliyatlar nda nazal tampon kullan m n ara t rmak için 11 sorudan olu an bir anket formu haz rland . Anket, 4 Kas m 2006 tarihinde Adana da yap lan Ba kent KBB Günleri ve Çukurova KBB Derne i Toplant s nda kat l mc hekimlere da t ld . Toplant lar sonunda anket formlar toplanarak de erlendirildi. Yetmiş iki hekimin anketi yan tlad görüldü. Bulgular: Burun ameliyatlar ndan sonra nazal tampon kullanan hekim say s 61 %84.7 , sütürasyon yapan hekim say s 11 %15.3 olarak bulundu. En s k tampon kullan m nedeni septal hematom ve ödemi önlemek %63.9 olarak bildirildi. Nazal tamponun kal süresini hekimlerin ço unlu u %60.5 iki gün olarak bildirdi. Hekimlerin hepsi nazal tampon kullan lan hastalara antibiyotik tedavisi verirken, %.98.3 ü a r kesici de verdi ini belirtti. Hekimlerin %62.2 si ayr ca antihistaminik verdi ini bildirdi. Sonuç: Nazal tampon kullan m nda cerrah n deneyimleri ve tercihleri ön plana çıkmaktadır, Objectives: We investigated the use of nasal tampon­ ade after nasal septal surgery among ENT surgeons.Materials and Methods: An 11-item questionnaire was prepared to investigate the use of nasal tampon­ ade in nasal operations and administered to partici- pating physicians at a meeting held in Adana, Turkey, on November 4, 2006, titled “Başkent ENT Days and Çukurova ENT Association Meeting”. During the meet­ ing 72 surgeons 13 females, 59 males; mean age 39 years; range 26 to 64 years responded to the ques- tionnaire.Results: Findings shovved that 61 surgeons 84.7% used nasal tamponade after nasal operations, while 11 surgeons 15.3% preferred suturing. The most common reason for tampon use was to prevent sep­ tal hematoma and edema 63.9% . Most of the sur­ geons 60.5% used nasal tamponade for two days.While ali the participants used antibiotic treatment along with nasal tamponade, 98.3% also prescribed an analgesic, and 62.2% prescribed an antihista- minic. At the time of removing the nasal tampon, 13.2% n=8 of the surgeons used a drug to prevent pain. The most common problem encountered dur­ ing removal was pain 54% .Conclusion: The use of nasal tamponade depends on the experience and preference of surgeons.

Amaç: Bu çalışmada Konya yöresinde yaşayan perennial alerjik rinitli PAR hastaların epidemiyolojik özellikleri, alerjen dağılımı ve semptom yoğunluğu incelendi.Hastalar ve Yöntemler: Klinik semptomlar ve cilt prik testi CPT ile PAR tanısı konan 165 erişkin hasta çalışmaya alındı. Hastalarda total IgE düzeyleri ve CPT’de alerjen dağılımı belirlendi ve semptom şiddeti sorgulandı.Bulgular: En sık saptanan alerjenler ev tozu akarları %84.9 idi. Bunu polenler %42.8 , mantarlar %38.6 , hayvan tüyleri %36.7 izlemekteydi. Total IgE değerleri olguların %30.3’ünde normal sınırların üzerinde bulundu. Olguların %45.2’sinde hastalık başlangıç yaşı 25 yaş veya altı idi. Olguların %46.5’i yüksekokul mezunuydu ve %66.2’si Konya şehir merkezinde, %33.8’i kırsal bölgede yaşamakta idi. Astım ve aspirin alerjisi öyküsü sırasıyla %2.9 ve %2.2 oranlarında saptandı. Aile öyküsünde alerji oranı %22.3 idi. En sık yakınma burunda kaşıntı idi. Ancak, yakınma şiddeti açısından en yüksek skor burunda dolgunluk semptomunda elde edildi.Sonuç: Bölgemizdeki PAR’li hastalarda en sık saptanan alerjen ev tozu akarları, en sık semptom burunda kaşıntı, ciddiyeti en yüksek semptom burunda dolgunluk idi, Objectives: We investigated epidemiological characteristics, distribution of allergens, and symptom severity of patients with perennial allergic rhinitis PAR , living in Konya region. Patients and Methods: The study included 165 adult patients 89 females, 76 males; mean age 34.5±10.7 years; range 18 to 68 years diagnosed as having PAR based on clinical symptoms and skin prick test SPT results. Total IgE levels, distribution of allergens in SPT, and symptom severity were determined. Results: The most frequent allergens were house dust mites 84.9% , followed by pollen mixture 42.8% , fungi 38.6% , and animal dander 36.7% . Serum total IgE levels were higher than normal in 30.3%. Age at onset of symptoms was ≤25 years in 45.2% of the patients. Patients with a university degree accounted for 46.5%. Urban and rural residents accounted for 66.2% and 33.8%, respectively. Coexisting asthma and aspirin allergy were detected in 2.9% and 2.2%, respectively. A family history of allergy was present in 22.3%. The most common nasal symptom was itching, but blocked nose had the highest severity score. Conclusion: In our region, house dust mites were the most frequent allergens, itchy nose was the most common symptom, and blocked nose had the highest symptom severity score in patients with PAR.

Amaç: Sinonazal tümörlerde endoskopik cerrahinin etkinliği değerlendirildi.Hastalar ve Yöntemler: Ocak 1998-Ağustos 2004 tarihleriarasında sinonazal tümör tanısıyla endoskopik tümör rezeksiyonu yapılan 20 hastanın kayıtları retrospektif olarak incelendi. Sadece transnazal endoskopik cerrahi yapılan veya kombine endoskopik ve eksternal cerrahi yöntemler uygulananhastalar çalışmaya alındı. Hastaların yaşları, cinsiyetleri, tümörün patolojik tanısı, yerleşimi, ek cerrahi girişim gereksinimi, uygulanan cerrahi tedavi modalitesi, komplikasyonları, rekürrens gelişimi ve takip süreleri belirlenerek elde edilen sonuçlar değerlendirildi.Bulgular: Olguların beşi osteom, biri fibröz displazi, biri ossifying fibrom, yedisi inverted papillom, biri onkositik papillom,üçü anjiofibrom, biri schwannom, biri estesionöroblastomdu.Hastaların takip süresi 6 ay-6 yıl arasındaydı ort. 26 ay .Posterior etmoid kaynaklı fibröz displazili olguda optik sinireyakın komşuluk nedeniyle endoskopik subtotal rezeksiyonyapıldı. Nazal pasaj ve maksiller sinüsü tutan inverted papillomlu bir hastaya kombine endoskopik ve Caldwell-Luc yaklaşımla tümör rezeksiyonu yapıldı. İnverted papillom tanısıyla endoskopik olarak ameliyat edilen bir olguda cerrahi sonrası nüks gelişti. Frontal sinüse yerleşmiş schwannom olgusunda kombine endoskopik ve eksternal frontoetmoidektomiyaklaşımıyla tümör rezeke edildi. İnverted papillomlu bir hastada rekürrens izlendi. Diğer tüm olgularda komplikasyonsuzolarak total endoskopik tümör eksizyonu yapıldı, bu hastalarda takipte nüks görülmedi.Sonuç: Çalışmamızdaki düşük rekürrens oranları, seçilmişolgularda, özellikle de deneyimli ellerde, endoskopik yaklaşımla tümör rezeksiyonunun tek başına ya da diğer yaklaşımlarla kombine edilerek, düşük morbiditeyle ve güvenle uygulanabilecek bir yöntem olabileceğini göstermektedir, Objectives: To evaluate the efficacy of endoscopic surgery for sinonasal tumors.Patients and Methods: Clinical records of 20 patients’ 10 males, 10 females; mean age 34; range 12 to 63 years who underwent endoscopic tumor resection with diagnosis of sinonasal tumor between January 1998 and August 2004 were retrospectively reviewed. Patients who were operated by transnasal endoscopic surgery or by combined endo­ scopic or external surgical methods were included in the study. Patients were reviewed for age, sex, pathologic diag­ nosis and tumor location, surgical techniques, need for additional surgery, complications, recurrence and follow-up periods.Results: There were five osteoms, one fibrous dysplasia, one ossifying fibroma, seven inverted papillomas, one oncocitic papilloma, three angiofibromas, one schwanno- ma and one esthesioneuroblastoma. The patients’ follow up period ranged between 6 months and 6 years mean 26 months . The fibrous dysplasia involving the posterior ethmoid sinuses was subtotally resected due to its close proximity to the optic nerve. One of the inverted papilloma of maxillary sinüs was resected by a combined endoscop­ ic and Caldwell-Luc approach. A combined endoscopic intranasal and external frontoethmoidectomy approach was performed for the removal of frontal sinüs schwannoma. Recurrence was observed in one of the inverted papilloma cases. Total removal of the tumors was achieved in ali other cases without any complication or recurrences.Conclusion: Low recurrence rates in our series showed that the endoscopic resection of nasal-paranasal sinüs tumors in selected cases may be an appropriate method as solely or combined with other surgical techniques for experienced surgeons.

Amaç: Mantarlar, sistemik ya da lokal alerjik etki ile, kronik rinosinüzit KRS patogenezinde rol oynayabilirler. Bu çalışmada KRS’li olgularda mantar alerjisi yaygınlığı ve mantar alerjisinin hastalığın klinik özellikleri üzerine etkisi araştırıldı.Hastalar ve Yöntemler: Polipli ya da polipsiz KRS tanısı konan, 18 yaş ve üzeri 127 hasta çalışmaya alındı. Cilt prik testi sonuçları ile saptanan mantar alerjisinin kan eozinofil ve total immünoglobülin E düzeyleri, polip varlığı, paranazal sinüs bilgisayarlı tomografi skorları üzerine etkileri araştırıldı.Bulgular: Seksen beş hastada %66.9 alerji saptandı. Polipli ve polipsiz KRS olgularında alerji oranları farklı değildi p>0.05 . En sık alerji saptanan antijen grubu ev tozu akarları %62.2 idi. Alerjik KRS olgularında mantar alerjisi oranı %38.8 bulundu. Tek başına mantar alerjisi iki olguda %1.6 saptandı. En sık saptanan alerji Aspergillus’tu, bunu Alternaria ve Penicillium takip ediyordu. Mantar alerjisi ile kan eozinofil ve total immünoglobülin E düzeyleri, polip varlığı ya da paranazal sinüs bilgisayarlı tomografisi skorları arasında ilişki saptanmadı p>0.05 .Sonuç: Bulgularımız KRS’li olgularda mantar alerjisi oranının yüksek olduğunu gösterdi. Mantar alerjisinin KRS klinik özellikleri ile kesin ilişkisini saptamak için kültür ve doku çalışmaları yapmak gerekmektedir, Objectives: Fungi, by systemic or local allergic effect, may play a role in the pathogenesis of chronic rhinosinusitis CRS . We investigated the incidence of fungal allergy in patients with CRS and its effect on the clinical characteristics of the disease. Patients and Methods: The study included 127 patients, aged 18 years or over, with CRS 42 females, 85 males; mean age 43±12 years; range 19 to 78 years . Fungal allergy was determined by skin prick test and its effect was analyzed on blood eosinophil and total immunoglobulin E levels, the presence of polyps, and paranasal sinus computed tomography scores. Results: Eighty-five patients 66.9% were found to have allergy. The incidence of allergy did not differ between patients with and without polyps p>0.05 . House dust mites 62.2% were the most frequent allergens. The incidence of fungal allergy was 38.8% in allergic patients. Isolated fungal allergy was detected in two patients 1.6% . The most frequent fungal allergens were Aspergillus, followed by Alternaria, and Penicillium. No association was found between fungal allergy and blood eosinophil and total immunoglobulin E levels, presence of polyps, or paranasal sinus computed tomography scores p>0.05 . Conclusion: The incidence of fungal allergy in patients with CRS was found to be high in this study. Tissue culture studies are required to determine the definitive relationship between fungal allergy and clinical features of CRS.

Plunging or cervical ranula is a rare phenomenon that represents mucous extravasation pseudocyst arising from the sublingual gland herniating through the mylohyoid muscle. The mucous dissects the tissue planes inferiorly and usually manifests as a swelling in the submental or submandibular regions. Surgical management of plunging ranula is the first-choice therapy. There are variable surgical treatments for plunging ranula, including simple excision, marsupialization, and e xcision of the ranula and sublingual gland via an intraoral or cervical approach. Despite these treatments, many patients have experienced recurrence, and sometimes larger lesions have occurred. Sclerotherapy is a potentially curative procedure that may be used as a primary treatment choice for plunging ranula before considering surgery. A case of plunging ranula is reported. Amaç: Nazal polipozis NP patogenezinde çevresel faktörlerin etkileri ile ilgili çalışmalar sınırlıdır. Bu çalışmada NP’li olgularda, sigara içmenin hastalık üzerine etkileri araştırıldı.Hastalar ve Yöntemler: Nazal polipozis tanısı ile takip edilen 125 hasta sigara kullanımı, polip evresi, paranazal sinüs bilgisayarlı tomografi BT skoru, total IgE düzeyi, kan eozinofil sayısı ve semptom skorları açısından değerlendirildi.Bulgular: Olgularda sigara içme oranı %16.8 21 hasta bulundu. Bu oran çalışmanın yapıldığı bölgedeki sigara içme oranına %34.2 göre düşük idi p0.05 . Alerji, astım ya da aspirin alerjisi sıklığı sigara içen ve içmeyen gruplarda farklı değildi p>0.05 . Semptomların ciddiyeti açısından, sigara içen grupta burun tıkanıklığı semptomu skorunun anlamlı derecede daha düşük p=0.034 çıkması dışında, iki grup arasında belirgin farklılık görülmedi.Sonuç: Bu çalışmada NP’li olgular arasında sigara içme oranı ve sigara içen NP’li hastalarda burun tıkanıklığı yakınması skoru daha düşük bulundu. Nazal polipozisli hastalarda yapılacak doku çalışmaları sigara ve NP ilişkisine yönelik yararlı bilgiler sağlayabilir, Objectives: Research on the role of environmental factors in the pathogenesis of nasal polyposis NP is limited. The aim of this study was to evaluate the effect of smoking on NP. Patients and Methods: A total of 125 patients 80 men, 45 women; mean age 44±12 years; range 19 to 78 years with NP were evaluated with regard to the presence or absence of smoking, polyp stage, paranasal sinus computed tomography CT score, total IgE level, blood eosinophil count, and symptom scores. Results: The rate of smoking in NP patients was 16.8% 21 patients , being significantly low p0.05 . The frequencies of allergy, asthma, and aspirin allergy were not different between smokers and nonsmokers p>0.05 . Symptom scores did not differ significantly between smokers and nonsmokers, except for nasal obstruction score, which was significantly low in nonsmokers p=0.034 . Conclusion: Our results showed that the rate of smoking in patients with NP and the severity of nasal obstruction in smokers compared to nonsmokers were significantly low. Tissue studies in patients with NP may provide useful data on the relationship between smoking and NP.
