Diverse learners attending online K-12 schools deserve high quality and accessible educational programs. This research studies the preparation, knowledge, and practices of online middle school general and special education teachers in relation to supporting the needs of diverse learners, namely students with Specific Learning Disabilities (SLD). A study of the literature indicated online teachers need training and development related to meeting the needs of students with disabilities and researchers call for more studies analyzing the preparedness and support of our teachers to meet the needs of diverse learners. This study responds to this need for action through a quantitative approach to examining the perceptions of middle school teachers across nine online public schools seeking to understand their preparedness, knowledge, and pedagogical practices related to teaching students with SLD in online schools. Teachers expressed a lack of preparation when reflecting on their pre-service training programs, in-service new teacher training and ongoing professional learning. In online schooling, it is necessary for teachers to have technological knowledge and skills to effectively teach all students. The findings from this study support an intervention that aims to improve online middle school teachers' knowledge and skills related to teaching students with SLD in online school environments.

Abstract Background Neurological infection is an important cause of critical illness, yet little is known on the epidemiology of neurological infections requiring critical care. Methods We analysed data on all adults with proven or probable neurological infection admitted to UK (NHS) critical care units between 2001 and 2020 reported to the Intensive Care National Audit and Research Centre. Diagnoses, physiological variables, organ support and clinical outcomes were analysed over the whole period, and for consecutive 5-year intervals within it. Predictors of in-hospital mortality were identified using a backward stepwise regression model. Results We identified 20,178 critical care admissions for neurological infection. Encephalitis was the most frequent presentation to critical care, comprising 6725 (33.3%) of 20,178 cases. Meningitis– bacterial, viral or unspecified cases - accounted for 10,056 (49.8%) of cases. In-hospital mortality was high, at 3945/19,765 (20.0%) overall. Over the four consecutive 5-year periods, there were trends towards higher Glasgow Coma Scale scores on admission, longer critical care admissions (from median 4 [IQR 2–8] to 5 days [IQR 2–10]), and reduced in-hospital mortality (from 24.9 to 18.1%). We identified 12 independent predictors of in-hospital death which when used together showed good discrimination between patients who die and those who survive (AUC = 0.79). Conclusions Admissions with neurological infection to UK critical care services are increasing and the mortality, although improving, remains high. To further improve outcomes from severe neurological infection, novel approaches to the evaluation of risk stratification, monitoring and management strategies are required.

Summary: Eggshell quality is one of the most significant factors affecting the egg industry as it economically influences the quantity of saleable eggs. Eggshell quality can be improved through optimization of genotype, housing system, and mineral nutrition. This study aimed to compare genotypes and evaluate the morphological, physical, and chemical properties of eggs (weight, breaking strength, Haugh units, and dry matter) by hens of two different strains fed a 1940 and 2016 representative diet. Egg production was measured daily and there were four periods with 10-wk intervals in which eggs were analyzed starting at 30 wk and ending at 60 wk of age measuring quality parameters. This study was set as a 2 × 2 factorial. The factors consisted of 2 leghorn genetic strains that were a 2016 commercial layer (W36) and a 1940 random-bred leghorn line, then 2 diets based on 2016 and 1940 dietary standards. The treatment groups: 1) 2016 hen on 1940 diet, 2) 2016 hen on 2016 diet, 3) 1940 hen on 1940 diet, and 4) 1940 hen on 2016 diet had 8 replicates with 10 hens per treatment housing 2 hens per cage. Body weights were higher in the 2016 hens when compared to the 1940 hens, however, the feed conversion ratio fluctuated in hens with the 2016 hens on the 1940 diet consuming the most feed throughout the majority of the trial. The 1940s hens came into production later than the 2016 hens; however, the rate of production was consistent with one another. Oviduct and ovary weights were heavier in the 2016 hens when compared to the 1940 hens. Egg quality, both interior and exterior was greater in the 2016 hens when compared to the 1940 hens. Parameters measured demonstrated significant differences (P ≤ 0.05) among treatment groups suggesting that genetics and nutrition both played a role in production rate and egg quality.

In 1994, Susskind and Uglum argued that it is possible to derive the Bekenstein-Hawking entropy $A/4G_N$ from string theory. In this article we explain the conceptual underpinnings of this argument, while elucidating its relationship to induced gravity and ER=EPR. Following an off-shell calculation by Tseytlin, we explicitly derive the classical closed string effective action from sphere diagrams at leading order in $\alpha^{\prime}$. We then show how to use this to obtain black hole entropy from the RG flow of the NLSM on conical manifolds. (We also briefly discuss the more problematic "open string picture" of Susskind and Uglum, in which strings end on the horizon.) We then compare these off-shell results with the rival "orbifold replica trick" using the on-shell $\mathbb{C}/Z_{N}$ background, which does not account for the leading order Bekenstein-Hawking entropy-unless perhaps tachyons are allowed to condense on the orbifold. Possible connections to the ER=EPR conjecture are explored. Finally, we discuss prospects for various extensions, including prospects for deriving holographic entanglement entropy in the bulk of AdS.

We explain why Tseytlin's off-shell formulation of string theory is well-defined. Although quantizing strings on an off-shell background requires an arbitrary choice of Weyl frame, this choice is not physically significant since it can be absorbed into a field redefinition of the target space fields. The off-shell formalism is particularly subtle at tree-level, due to the treatment of the noncompact conformal Killing group SL(2,$\mathbb{C}$) of the sphere. We prove that Tseytlin's sphere prescriptions recover the standard tree-level Lorentzian S-matrix, and show how to extract the stringy $i\varepsilon$ prescription from the UV cutoff on the worldsheet. We also demonstrate that the correct tree-level equations of motion are obtained to all orders in perturbation theory in $g_s$ and $\alpha^{\prime}$, and illuminate the close connection between the string action and the c-theorem.

BackgroundSARS-CoV-2 variant Omicron rapidly evolved over 2022, causing three waves of infection due to sub-variants BA.1, BA.2 and BA.4/5. We sought to characterise symptoms and viral loads over the course of COVID-19 infection with these sub-variants in otherwise-healthy, vaccinated, non-hospitalised adults, and compared data to infections with the preceding Delta variant of concern (VOC).MethodsIn a prospective, observational cohort study, healthy vaccinated UK adults who reported a positive polymerase chain reaction (PCR) or lateral flow test, self-swabbed on alternate weekdays until day 10. We compared participant-reported symptoms and viral load trajectories between infections caused by VOCs Delta and Omicron (sub-variants BA.1, BA.2 or BA.4/5), and tested for relationships between vaccine dose, symptoms and PCR cycle threshold (Ct) as a proxy for viral load using Chi-squared (χ2) and Wilcoxon tests.Results563 infection episodes were reported among 491 participants. Across infection episodes, there was little variation in symptom burden (4 [IQR 3-5] symptoms) and duration (8 [IQR 6-11] days). Whilst symptom profiles differed among infections caused by Delta compared to Omicron sub-variants, symptom profiles were similar between Omicron sub-variants. Anosmia was reported more frequently in Delta infections after 2 doses compared with Omicron sub-variant infections after 3 doses, for example: 42% (25/60) of participants with Delta infection compared to 9% (6/67) with Omicron BA.4/5 (χ2 P < 0.001; OR 7.3 [95% CI 2.7-19.4]). Fever was less common with Delta (20/60 participants; 33%) than Omicron BA.4/5 (39/67; 58%; χ2 P = 0.008; OR 0.4 [CI 0.2-0.7]). Amongst infections with an Omicron sub-variants, symptoms of coryza, fatigue, cough and myalgia predominated. Viral load trajectories and peaks did not differ between Delta, and Omicron, irrespective of symptom severity (including asymptomatic participants), VOC or vaccination status. PCR Ct values were negatively associated with time since vaccination in participants infected with BA.1 (β = -0.05 (CI -0.10-0.01); P = 0.031); however, this trend was not observed in BA.2 or BA.4/5 infections.ConclusionOur study emphasises both the changing symptom profile of COVID-19 infections in the Omicron era, and ongoing transmission risk of Omicron sub-variants in vaccinated adults.Trial registrationNCT04750356.

The emergence of successive Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) variants of concern (VOCs) during 2020 to 2022, each exhibiting increased epidemic growth relative to earlier circulating variants, has created a need to understand the drivers of such growth. However, both pathogen biology and changing host characteristics-such as varying levels of immunity-can combine to influence replication and transmission of SARS-CoV-2 within and between hosts. Disentangling the role of variant and host in individual-level viral shedding of VOCs is essential to inform Coronavirus Disease 2019 (COVID-19) planning and response and interpret past epidemic trends. Using data from a prospective observational cohort study of healthy adult volunteers undergoing weekly occupational health PCR screening, we developed a Bayesian hierarchical model to reconstruct individual-level viral kinetics and estimate how different factors shaped viral dynamics, measured by PCR cycle threshold (Ct) values over time. Jointly accounting for both interindividual variation in Ct values and complex host characteristics-such as vaccination status, exposure history, and age-we found that age and number of prior exposures had a strong influence on peak viral replication. Older individuals and those who had at least 5 prior antigen exposures to vaccination and/or infection typically had much lower levels of shedding. Moreover, we found evidence of a correlation between the speed of early shedding and duration of incubation period when comparing different VOCs and age groups. Our findings illustrate the value of linking information on participant characteristics, symptom profile and infecting variant with prospective PCR sampling, and the importance of accounting for increasingly complex population exposure landscapes when analysing the viral kinetics of VOCs. Abstract We investigate a new approach to holography in asymptotically AdS spacetimes, in which time rather than space is the emergent dimension. By making a sufficiently large T 2-deformation of a Euclidean CFT, we define a holographic theory that lives on Cauchy slices of the Lorentzian bulk. (More generally, for an arbitrary Hamiltonian constraint equation that closes, we show how to obtain it by an irrelevant deformation from a CFT with suitable anomalies.) The partition function of this theory defines a natural map between the bulk canonical quantum gravity theory Hilbert space, and the Hilbert space of the usual (undeformed) boundary CFT. We argue for the equivalence of the ADM and CFT Hamiltonians. We also explain how bulk unitarity emerges naturally, even though the boundary theory is not reflection-positive. This allows us to reformulate the holographic principle in the language of Wheeler-DeWitt canonical quantum gravity. Along the way, we outline a procedure for obtaining a bulk Hilbert space from the gravitational path integral with Dirichlet boundary conditions. Following previous conjectures, we postulate that this finite-cutoff gravitational path integral agrees with the T 2-deformed theory living on an arbitrary boundary manifold — at least near the semiclassical regime. However, the T 2-deformed theory may be easier to UV complete, in which case it would be natural to take it as the definition of nonperturbative quantum gravity.

This study identified and evaluated differences between microbiome compositions of the ileum and ceca of 1940 and 2016 white leghorn genetic strains fed representative contemporary diets from those times. Ileal and cecal samples were collected at 69 weeks of age. Alpha and beta diversity metrics were generated, and the Analysis of Composition of Microbiomes (ANCOM) was utilized to determine significantly different taxa. Ileum and ceca alpha diversity were significantly different (p = 0.001; Q = 0.001); however, no differences between genetic lineage were observed (p > 0.05; Q > 0.05). Beta diversity between the ileum and ceca and the genetic lines was significantly different (p = 0.001; Q = 0.001). The ANCOM of the ileum showed significant differences between Proteobacteria and Actinobacteriota phyla (p ˂ 0.05) and significant differences between Pseudomonas, Rhizobiaceae, Leuconostoc, and Aeriscardovia genera (p ˂ 0.05). For ceca ANCOM, Proteobacteria, Firmicutes, Actinobacteriota, and Euryarchaeota phyla were significantly different (p ˂ 0.05), with Firmicutes having the highest relative abundance across all groups, and there were significant differences in genera Pseudomonas, Leuconostoc, Alloprevotella, and Aeri scardovia, with Alloprevotella having the highest relative abundance. The results suggest that genetic makeup in conjunction with the nutritional composition influences the cecal and ileal microbiota of corresponding hens.

Vaccination against COVID-19 has shown activation of different immune cell types. Here the authors characterise the immune response to the SARS-CoV-2 mRNA vaccine using longitudinal CyTOF single cell approaches to characterise antigen specific B and T-cell responses promoted by this vaccine.

Abstract Background Lepidic adenocarcinoma represents a histologic pattern of non-small cell lung cancer that characteristically arises in the lung periphery with tracking alongside pre-existing alveolar walls. Noninvasive and invasive variants of lepidic adenocarcinoma are dependent on parenchymal destruction, vascular, or pleural invasion. The lepidic-predominant lung malignancies are collectively recognized as slow growing with rare metastasis and excellent prognosis. The World Health Organization classification of lung malignancies depends on molecular and histopathological findings. CT findings most commonly include ground-glass characteristics, commonly mistaken for inflammatory or infectious etiology. These tumors are generally surgically resectable and associated with better survival given infrequent nodal and extrathoracic involvement. Rarely these tumors present with diffuse pneumonic-type involvement associated with worse outcomes despite lack of nodal and distant metastases. Case presentation A 63-year-old Caucasian athletic immunocompetent female presented with 2 months of progressive shortness of breath, fatigue, loss of appetite and 15 pound weight loss. History was only notable for well controlled essential hypertension and hypothyroidism. Contrast computed tomography angiogram and positron emission tomography revealed diffuse hypermetabolic interstitial and airspace abnormalities of the lungs without lymphadenopathy (or distant involvement) in addition to right hydropneumothorax and left pleural effusion. Baseline laboratory testing was unremarkable, and extensive bacterial and fungal testing returned negative. Bronchoscopy and video-assisted thoracoscopic surgery was subsequently performed with pleural fluid cytology, lung and pleural biopsies returning positive for lepidic adenocarcinoma with 2% programmed death ligand 1 expression and genomic testing positive for PTEN gene deletion. Prior to treatment, the patient perished on day 15 of admission. Conclusion We present a rare case of lepidic predominant adenocarcinoma with extensive bilateral aerogenous spread in the context of no lymphovascular invasion in a healthy, low risk patient. This case presentation may add to the body of knowledge regarding the different behavior patterns of lepidic predominant adenocarcinomas.

IntroductionDespite routine implementation of cleaning and disinfection practices in clinical healthcare settings, high-touch environmental surfaces and contaminated equipment often serve as reservoirs for the transmission of pathogens associated with healthcare-associated infections (HAIs).MethodsThe current study involved the analysis of high-touch surface swabs using a metatranscriptomic sequencing workflow (CSI-Dx™) to assess the efficacy of cleanSURFACES® technology in decreasing microbial burden by limiting re-contamination. This is a non-human single center study conducted in the Emergency Department (ED) and on an inpatient Oncology Ward of Walter Reed National Military Medical Center that have followed hygienic practices during the COVID-19 pandemic environment.ResultsAlthough there was no difference in observed microbial richness (two-tailed Wilcoxon test with Holm correction, P > 0.05), beta diversity findings identified shifts in microbial community structure between surfaces from baseline and post-intervention timepoints (Day 1, Day 7, Day 14, and Day 28). Biomarker and regression analyses identified significant reductions in annotated transcripts for various clinically relevant microorganisms' post-intervention, coagulase-negative staphylococci and Malassezia restricta, at ED and Oncology ward, respectively. Additionally, post-intervention samples predominantly consisted of Proteobacteria and to a lesser extent skin commensals and endogenous environmental microorganisms in both departments.DiscussionFindings support the value of cleanSURFACES®, when coupled with routine disinfection practices, to effectively impact on the composition of active microbial communities found on high-touch surfaces in two different patient care areas of the hospital (one outpatient and one inpatient) with unique demands and patient-centered practices.

Introduction Long COVID (LC), the persistent symptoms ≥12 weeks following acute COVID-19, presents major threats to individual and public health across countries, affecting over 1.5 million people in the UK alone. Evidence-based interventions are urgently required and an integrated care pathway approach in pragmatic trials, which include investigations, treatments and rehabilitation for LC, could provide scalable and generalisable solutions at pace. Methods and analysis This is a pragmatic, multi-centre, cluster-randomised clinical trial of two components of an integrated care pathway (Coverscan™, a multi-organ MRI, and Living with COVID Recovery™, a digitally enabled rehabilitation platform) with a nested, Phase III, open label, platform randomised drug trial in individuals with LC. Cluster randomisation is at level of primary care networks so that integrated care pathway interventions are delivered as “standard of care” in that area. The drug trial randomisation is at individual level and initial arms are rivaroxaban, colchicine, famotidine/loratadine, compared with no drugs, with potential to add in further drug arms. The trial is being carried out in 6–10 LC clinics in the UK and is evaluating the effectiveness of a pathway of care for adults with LC in reducing fatigue and other physical, psychological and functional outcomes at 3 months. The trial also includes an economic evaluation which will be described separately. Ethics and dissemination The protocol was reviewed by South Central—Berkshire Research Ethics Committee (reference: 21/SC/0416). All participating sites obtained local approvals prior to recruitment. Coverscan™ has UK certification (UKCA 752965). All participants will provide written consent to take part in the trial. The first participant was recruited in July 2022 and interim/final results will be disseminated in 2023, in a plan co-developed with public and patient representatives. The results will be presented at national and international conferences, published in peer reviewed medical journals, and shared via media (mainstream and social) and patient support organisations. Background: The sinus tarsi (ST) approach for calcaneus fractures has gained popularity in recent years with an increased interest in shifting to less invasive approaches for calcaneal fracture fixation allowing for adequate fixation if complications do not arise. Although the ST approach has gained acceptance as standard for calcaneus fracture fixation, the literature surrounding early complications rates based on age differences for this specific approach and pathology is lacking. The objective of this study was to determine if rates of complications based on age varied for patients undergoing open reduction and internal fixation (ORIF) of closed calcaneus fractures using the ST approach. Methods: A retrospective review of patients undergoing ORIF for closed calcaneus fractures from 2012 to 2020 was performed. Inclusion criteria were based on an age greater than 18 years, surgical management of a closed calcaneus fracture using a ST approach, requirement of a preoperative computed tomographic scan, and a minimum of 180 days’ follow-up. Patients were divided into 2 groups: those aged 50 years. Results: A total of 196 fractures were included with 114 fractures in the 50-year age group. Mean age was 34.2 and 59.7 years in the younger and older groups, respectively. The older group had similar rates of wound dehiscence (1.2% vs 4.4%, P = .204), superficial surgical site infection (1.2% vs 2.6%, P = .490), deep infection (9.8% vs 7.9%, P = .648), and nonunion (4.9% vs 3.5%, P = .633) compared with the younger group. Rates of 30-day readmission, unplanned reoperation, and symptomatic hardware were not significantly different. Postoperative Bohler and Gissane angles were not significantly different between both groups. Conclusion: Older patients with intraarticular calcaneus fractures treated via the ST approach maintain complication rates similar to those in younger individuals. Objective: Methicillin-resistant Staphylococcus aureus (MRSA) continues to be a pathogen worldwide. Empiric anti-MRSA therapy is often prescribed in hospital inpatients with potential infection. Recent studies have suggested, particularly for respiratory infections, that MRSA colonization as determined by nasal swab has a high negative predictive value (NPV) for MRSA infections during the index hospitalization. We examined the predictive value of a prior intensive care unit (ICU) MRSA nasal swab on the results from a subsequent ICU admission in the same patient and the results of the latter admission MRSA nasal swab. Methods: A retrospective chart review of patients 18 years or older admitted to a large tertiary care hospital in the Midwest of the United States in 2016 who had a MRSA nasal swab performed and had an ICU admission stay of over 24 h was conducted. This group of patients was matched to a patient list of subjects who were admitted as an inpatient to the same ICU at least once during the following year. Data were collected on demographic and clinical information, as well as the results of MRSA swabs and the presence of a MRSA infection during both hospitalizations. Predictive values were calculated using 2 × 2 tables including sensitivity and specificity of a first MRSA swab result with a MRSA infection during the subsequent ICU stay. Findings: Seventy-seven patients were matched who had MRSA swabs performed on two separate ICU admissions. The negative predictive value of the first MRSA swab result on a MRSA infection during the second ICU stay was 96%. Conclusion: In this pilot study, a previous negative MRSA nasal swab may predict a lack of a MRSA infection in a subsequent infection during a 1-year period.

Abstract We present a general procedure for constructing tensor networks that accurately reproduce holographic states in conformal field theories (CFTs). Given a state in a large-N CFT with a static, semiclassical gravitational dual, we build a tensor network by an iterative series of approximations that eliminate redundant degrees of freedom and minimize the bond dimensions of the resulting network. We argue that the bond dimensions of the tensor network will match the areas of the corresponding bulk surfaces. For “tree” tensor networks (i.e., those that are constructed by discretizing spacetime with non­ intersecting Ryu-Takayanagi surfaces), our arguments can be made rigorous using a version of one-shot entanglement distillation in the CFT. Using the known quantum error correcting properties of AdS/CFT, we show that bulk legs can be added to the tensor networks to create holographic quantum error correcting codes. These codes behave similarly to previous holographic tensor network toy models, but describe actual bulk excitations in continuum AdS/CFT. By assuming some natural generalizations of the “holographic entanglement of purification” conjecture, we are able to construct tensor networks for more general bulk discretizations, leading to finer-grained networks that partition the information content of a Ryu-Takayanagi surface into tensor-factorized subregions. While the granularity of such a tensor network must be set larger than the string/Planck scales, we expect that it can be chosen to lie well below the AdS scale. However, we also prove a no-go theorem which shows that the bulk-to-boundary maps cannot all be isometries in a tensor network with intersecting Ryu-Takayanagi surfaces.

Summary: The emergence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) lineages that are more transmissible and resistant to currently approved antibody therapies poses a considerable challenge to the clinical treatment of coronavirus disease (COVID-19). Therefore, the need for ongoing discovery efforts to identify broadly reactive monoclonal antibodies to SARS-CoV-2 is of utmost importance. Here, we report a panel of SARS-CoV-2 antibodies isolated using the linking B cell receptor to antigen specificity through sequencing (LIBRA-seq) technology from an individual who recovered from COVID-19. Of these antibodies, 54042-4 shows potent neutralization against authentic SARS-CoV-2 viruses, including variants of concern (VOCs). A cryoelectron microscopy (cryo-EM) structure of 54042-4 in complex with the SARS-CoV-2 spike reveals an epitope composed of residues that are highly conserved in currently circulating SARS-CoV-2 lineages. Further, 54042-4 possesses uncommon genetic and structural characteristics that distinguish it from other potently neutralizing SARS-CoV-2 antibodies. Together, these findings provide motivation for the development of 54042-4 as a lead candidate to counteract current and future SARS-CoV-2 VOCs.

Soundscapes offer rich descriptions of composite acoustic environments. Characterizing marine soundscapes simply through sound levels results in incomplete descriptions, limits the understanding of unique features, and impedes meaningful comparisons. Sources that contribute to sound level metrics shift in time and space with changes in biological patterns, physical forces, and human activity. The presence of a constant or chronic source is often interwoven with episodic sounds. Further, the presence and intensity of sources can influence other sources, calling for a more integrated approach to characterizing soundscapes. Here, we illustrate this complexity using data from a national-scale effort, the Sanctuary Soundscape Monitoring Project (SanctSound), an initiative designed to support collection of biological, environmental, and human use data to compliment the interpretation of sound level measurements. Using nine examples from this diverse dataset we demonstrate the benefit of integrating source identification and site features to interpret sound levels across a diversity of shallow water marine soundscapes (

Passive acoustic data collection has grown exponentially over the past decade resulting in petabytes of data that document our ocean soundscapes. This effort has resulted in two big data challenges: (1) the curation, management, and global dissemination of passive acoustic datasets and (2) efficiently extracting critical information and comparing it to other datasets in the context of ecosystem-based research and management. To address the former, the NOAA National Centers for Environmental Information recently established an archive for passive acoustic data. This fast-growing archive currently contains over 100 TB of passive acoustic audio files mainly collected from stationary recorders throughout waters in the United States. These datasets are documented with standards-based metadata and are freely available to the public. To begin to address the latter, through standardized processing and centralized stewardship and access, we provide a previously unattainable comparison of first order sound level-patterns from archived data collected across three distinctly separate long-term passive acoustic monitoring (PAM) efforts conducted at regional and national scales: NOAA/National Park Service Ocean Noise Reference Station Network, the Atlantic Deepwater Ecosystem Observatory Network, and the Sanctuary Soundscape Monitoring Project. Nine sites were selected from these projects covering the Alaskan Arctic, Northeast and Central Pacific, Gulf of Mexico, Caribbean Sea, and Mid and Northwest Atlantic. Abstract We expand our recent work on the outer entropy, a holographic coarse-grained entropy defined by maximizing the boundary entropy while fixing the classical bulk data outside some surface. When the surface is marginally trapped and satisfies certain “minimar” conditions, we prove that the outer entropy is exactly equal to a quarter the area (while for other classes of surfaces, the area gives an upper or lower bound). We explicitly construct the entropy-maximizing interior of a minimar surface, and show that it satisfies the appropriate junction conditions. This provides a statistical explanation for the area-increase law for spacelike holographic screens foliated by minimar surfaces. Our construction also provides an interpretation of the area for a class of non-minimal extremal surfaces. On the boundary side, we define an increasing simple entropy by maximizing the entropy subject to a set of “simple experiments” performed after some time. We show (to all orders in perturbation theory around equilibrium) that the simple entropy is the boundary dual to our bulk construction.

Abstract The AdS/CFT understanding of CFT entanglement is based on HRT surfaces in the dual bulk spacetime. While such surfaces need not exist in sufficiently general spacetimes, the maximin construction demonstrates that they can be found in any smooth asymptotically locally AdS spacetime without horizons or with only Kasner-like singularities. In this work, we introduce restricted maximin surfaces anchored to a particular boundary Cauchy slice C ∂ . We show that the result agrees with the original unrestricted maximin prescription when the restricted maximin surface lies in a smooth region of spacetime. We then use this construction to extend the existence theorem for HRT surfaces to generic charged or spinning AdS black holes whose mass-inflation singularities are not Kasner-like. We also discuss related issues in time-independent charged wormholes.

Passive acoustic monitoring is a well-established tool for researching the occurrence, movements, and ecology of a wide variety of marine mammal species. Advances in hardware and data collection have exponentially increased the volumes of passive acoustic data collected, such that discoveries are now limited by the time required to analyze rather than collect the data. In order to address this limitation, we trained a deep convolutional neural network (CNN) to identify humpback whale song in over 187,000 h of acoustic data collected at 13 different monitoring sites in the North Pacific over a 14-year period. The model successfully detected 75 s audio segments containing humpback song with an average precision of 0.97 and average area under the receiver operating characteristic curve (AUC-ROC) of 0.992. The model output was used to analyze spatial and temporal patterns of humpback song, corroborating known seasonal patterns in the Hawaiian and Mariana Islands, including occurrence at remote monitoring sites beyond well-studied aggregations, as well as novel discovery of humpback whale song at Kingman Reef, at 5∘ North latitude. This study demonstrates the ability of a CNN trained on a small dataset to generalize well to a highly variable signal type across a diverse range of recording and noise conditions. We demonstrate the utility of active learning approaches for creating high-quality models in specialized domains where annotations are rare. These results validate the feasibility of applying deep learning models to identify highly variable signals across broad spatial and temporal scales, enabling new discoveries through combining large datasets with cutting edge tools.

Background: Mucorales is a zygomycete fungi known to cause opportunistic infections in immunosuppressed hosts. Spores may be inhaled, causing rhinocerebral or pulmonary infections, or gastrointestinal infections if swallowed. Less often, cutaneous mucormycosis develops after inoculation via broken skin. Presentation: A 72-year old male on ibrutinib and prednisone for chronic lymphocytic leukemia (CLL) presented with localized, right forearm cutaneous mucormycosis at the site of a dog-scratch sustained three weeks prior. The patient failed to respond to cephalexin as an outpatient, prompting biopsy showing ribbon-like pseudo septate hyphae and possible vascular invasion suggestive of Mucorales. Treatment course included liposomal amphotericin B 5 mg/kg IV every 24 h for ten days followed by a 90-day course of posaconazole 300 mg daily after general surgery consultation was sought. Conclusion: We outline the second reported case of localized cutaneous mucormycosis arising in the setting of ibrutinib use. Because the combination of immunosuppressed states, ibrutinib and skin trauma may serve as a nidus for mucormycosis, practitioners should be vigilant of thorough skin evaluations in these patients and appropriate anti-fungal treatment. Although amphotericin B has been well studied as first line therapy, oral posaconazole has been shown as an efficacious second-line treatment.

Vessel-based sonar systems that focus on the water column provide valuable information on the distribution of underwater marine organisms, but such data are expensive to collect and limited in their spatiotemporal coverage. Satellite data, however, are widely available across large regions and provide information on surface ocean conditions. If satellite data can be linked to subsurface sonar measurements, it may be possible to predict marine life over broader spatial regions with higher frequency using satellite observations. Here, we use random forest models to evaluate the potential for predicting a sonar-derived proxy for subsurface biomass as a function of satellite imagery in the California Current Ecosystem. We find that satellite data may be useful for prediction under some circumstances, but across a range of sonar frequencies and depths, overall model performance was low. Performance in spatial interpolation tasks exceeded performance in spatial and temporal extrapolation, suggesting that this approach is not yet reliable for forecasting or spatial extrapolation. We conclude with some potential limitations and extensions of this work.

BackgroundMortality from bacterial meningitis, predominately caused by Streptococcus pneumoniae, exceeds 50% in sub-Saharan African countries with high HIV prevalence. Underlying causes of high mortality are poorly understood. We examined the host and pathogen proteome in the CSF of adults with proven pneumococcal meningitis (PM), testing if there was an association between differentially expressed proteins and outcome.Materials/MethodsCSF proteomes were analyzed by quantitative Mass-Spectrometry. Spectra were identified using the Swissprot human and TIGR4 pneumococcal protein libraries. Proteins were quantitated and analyzed against mortality. Unique proteins in PM were identified against published normal CSF proteome. Random-Forest models were used to test for protein signatures discriminating outcome. Proteins of interest were tested for their effects on growth and neutrophil opsonophagocytic killing of S. pneumoniae.ResultsCSF proteomes were available for 57 Adults with PM (median age 32 years, 60% male, 70% HIV-1 co-infected, mortality 63%). Three hundred sixty individual human and 23 pneumococcal proteins were identified. Of the human protein hits, 30% were not expressed in normal CSF, and these were strongly associated with inflammation and primarily related to neutrophil activity. No human protein signature predicted outcome. Abstract After turning on an interaction that couples the two boundaries of an eternal BTZ black hole, we find a quantum matter stress tensor with negative average null energy, whose gravitational backreaction renders the Einstein-Rosen bridge traversable. Such a traversable wormhole has an interesting interpretation in the context of ER=EPR, which we suggest might be related to quantum teleportation. However, it cannot be used to violate causality. We also discuss the implications for the energy and holographic entropy in the dual CFT description.
